技术领域
本发明涉及包括可替换装载单元的外科吻合装置。更具体地,涉及一种装运构件,所述装运构件在装运构件与装载单元脱离之前阻止装载单元的击发,并促进装载单元与外科吻合装置的脱离。
背景技术
环形吻合装置通常被用于经环形吻合术连接管状组织段。典型地,环形吻合装置包括用于给组织施加吻合钉的环形阵列的装载单元。该装载单元典型地在击发后是可替换的,从而允许环形吻合装置的重复使用。在运输和将装载单元连接到环形吻合装置期间,装载单元内的推动器和刀片组件可能变得不对准,从而阻碍装载单元到环形吻合装置的正确安装。另外,在击发后,装载单元不包括任何防止空装载单元的后续击发的机构和/或促使装载单元从环形吻合装置脱离的机构。为了保证装载单元到外科吻合装置的正确安装,为了防止空装载单元的击发,以及为了促进装载单元与环形吻合装置的分离,期望能提供一种将装载单元保持在可操作状态下且在击发后能重新附接到装载单元的装运构件。
发明内容
提供一种用于环形吻合装置的装载单元。装载单元包括限定了一对第一开口和一对第二开口的壳体、可操作地被接收在壳体内且在壳体内能从第一位置移动到第二位置的推动器构件、被设置在壳体的远端上且包括多个吻合钉的钉仓、以及被可操作地接收在壳体的远端上以防止推动器构件从第一位置移动到第二位置的装运构件。壳体的第一开口可以大于第二开口。装运构件可以以第一定向和第二定向被接收在壳体上。在第一定向,装运构件可以被可拆卸地固定到壳体,在第二定向,装运构件可以被牢固地固定到壳体。
在一个实施例中,装运构件包括基础部、一对从基础部伸出的支腿、以及被设置在每条支腿的自由端上的锁定结构。所述锁定结构可以被可拆卸地固定在所述一对第一开口内,并且能牢固地固定在所述一对第二开口内。装运构件的基础部可以包括被配置以被设置在钉仓附近的内表面。基础部的内表面可以被配置以将多个吻合钉保持在钉仓内。装运构件还可以包括从基础部朝近侧伸出的保持部。保持部可以被配置以接合推动器构件。保持部可以包括至少一个延伸部。所述至少一个延伸部可以包括一对延伸部。每个锁定结构可以包括一对凸缘,每个凸缘可以被配置以向内弯曲,从而便于凸缘进入第二开口之一。每个凸缘可以包括从其径向向外延伸的突起。所述一对支腿的每一条支腿可以包括凸片,从而便于被使用者可操作地接合。

参见图1,外科吻合装置10包括可替换的装载单元(根据本发明的实施例,被大致地画成装载单元100)以及装运构件(根据本发明的实施例,被大致地画成装运盖200)。装载单元100被画出可操作地连接到外科吻合装置10的致动单元12的细长本体16。关于示范性致动单元12的详细描述,请参见共同拥有的美国专利No.8,590,763(“’763”专利),其全部内容以提及的方式被并入本文。
参见图1和2,装运盖200被有选择地接收在装载单元100的远端上,且操作成将吻合钉“S”(图4)保持在装载单元100的钉仓120(图4)内,并在装载单元100附接到致动单元12的细长本体16之前或期间防止推动器构件130(图4)的提前行进。另外,在装运盖200已经被重新附接到装载单元100后,装运盖200操作成防止装载单元100的再次击发。另外,在装载单元100的击发之后将装运盖200重新附接到装载单元100,这有利于装载单元100与致动单元12的细长本体16的分离,这将在下面被详细地描述。

参见图2-5,装载单元100包括壳体110、被固定在壳体110的远端上的钉仓120、以及被可操作地接收在壳体110内的推动器构件130(图3)。装载单元100的壳体110包括外圆筒部112和内圆筒部114。多个肋(未画出)将外内圆筒部112,114相互连接。壳体110的外圆筒部112和内圆筒部114是共轴的,并限定了被配置以滑动地接收推动器构件130的空腔113。壳体110的近端被配置以有选择地连接到致动单元12的细长本体16或一个连接到致动单元12的适配器组件(未画出)。例如,装载单元100的壳体110可以利用插栓式联接器或以任意合适的方式连接到细长本体16或适配器组件。
具体地参见图2和3,壳体110的外圆筒部112限定了与装载单元100的远端纵向隔开且延伸穿过外圆筒部112的大开口115。大开口115径向上彼此相对地被定位在外圆筒部112上(图7),且被配置以接收装运盖200的锁定结构226(图8)。壳体110的外圆筒部112还限定了与装载单元100的远端纵向隔开且延伸穿过外圆筒部112的小开口117。小开口117与大开口115周向地隔开且径向上彼此相对地被定位(图9)并延伸穿过外圆筒部112。小开口117被配置以固定地接收装运盖200的锁定结构226(图8)。
参见图4和5,装载单元100的钉仓120被固定到壳体110的远端且包括多个吻合钉凹部121(图5)。每个吻合钉凹部121被配置以接收并保持多个吻合钉“S”。
装载单元100的推动器构件130包括被配置以可释放地联接到致动单元12的驱动机构18(图1)的近端132。具体地,推动器构件130的近端132包括一对臂134,凸片135被形成在每个臂134的近端134a上。凸片135被配置以可释放地与致动单元12的驱动构件18的带凹口远端18a接合。如下面详细讨论那样,臂134的近端134a被配置以径向向内弯曲,从而有利于推动器构件130连接到驱动构件18以及有利于推动器构件130与驱动构件18分离。推动器构件130的远端136包括被接收在钉仓120的吻合钉凹部121内且处于与吻合钉“S”相接合的位置的多个吻合钉推动器136a。推动器构件130在钉仓120内的行进使吻合钉“S”从钉仓120中射出。圆形刀138被连接到刀架140(图5),所述刀架在靠近推动器构件130的远端136上的吻合钉推动器136a的位置处被形成在推动器构件130内。圆形刀138被配置以随推动器构件130在钉仓120内朝远侧移动,从而切断在已吻合的组织内的组织(未画出)。

装运盖200的基础部210包括基本扁平或平坦的圆形本体。尽管被画出具有圆的形状,但能预见到基础部210可以包括与装载单元100(图1)的横截面形状对应的任意形状。还能预见到基础部210可以是锥形或被配置以便于装载单元100在患者(未画出)的组织(未画出)内的插入与定位。
装运盖200的基础部210的朝近侧表面212被配置以在装运盖200被附接到装载单元100的远端时抵接装载单元100的钉仓120的远侧表面,从而将吻合钉“S”保持在钉仓120内。在运输期间以及装载单元100到致动单元12的细长本体16(图1)的安装期间,钉保持表面212操作成将吻合钉“S”(图5)保持在钉仓120的吻合钉凹部121(图5)内。尽管被画出为圆形以对应于装载单元100的钉仓120的横截面形状,但是能预见到基础部210的钉保持表面212可以修改以对应于具有其他横截面构型的钉仓、或者可以具有与钉仓不同的构型。
装运盖200的附接部220包括从基础部210朝近侧延伸的一对支腿220a,220b。每条支腿220a,220b包括从基础部210垂直伸出的第一段222a,222b以及相对于各第一段222a,222b倾斜延伸的第二段224a,224b。如图2所示,各个支腿220a,220b的第一段222a,222b和第二段224a,224b贴合装载单元100的壳体110的外圆筒部112的形状。
锁定结构226从附接部220的支腿220a,220b的第二段224a,224b中的每一个径向向内延伸。每个锁定结构226包括多个可向内变形的凸缘227。尽管被画出包括一对凸缘227,但能预见到锁定结构226可以包括多于两个的凸缘227。每个凸缘227被配置以朝着所述一对凸缘227中的另一个凸缘227弯曲。如在下面被更详细地描述那样,每个凸缘227的自由端227a被配置以接合装载单元100的推动器构件130的臂134的外表面,从而有利于推动器构件130从致动单元12(图1)的驱动构件18(图5)的释放,如在下面被更详细地描述那样。突起228从每个凸缘227向外伸出。如在下面被更详细地描述那样,凸缘227上的突起228被配置以接合装载单元100的壳体110的外圆筒部112。每条支腿220a,220b包括凸片229或被配置以与使用者接合的其他结构,从而有利于支腿220a,220b弯曲离开装载单元100的壳体110的外圆筒部112,从而允许装运盖200与装载单元100分离。
装运盖200的保持器部230从由基础部210所限定的平面垂直地伸出。如图所示,保持器部230包括两个延伸部232。但是保持器部230可以只包括单个延伸部或多于两个的延伸部。在替代实施例中,保持器部230包括环状延伸部。延伸部232被配置以接收在装载单元100的推动器构件130的远端136内、且在装运盖200被附接到装载单元100时接合推动器构件130的刀架140。当装运盖200被附接到装载单元100时,装运盖200的保持部230的延伸部232与装载单元100的推动器构件130的刀架140的接合阻止了推动器构件130的行进。
参见图7和8,装运盖200被画出被附接到装载单元100。当装运盖200以第一定向或击发前定向被附接到装载单元100时,形成在装运盖200的附接部220的各支腿220a,220b的第二段224a,224b上的锁定结构226的凸缘227被接收在装载单元100的壳体110的外圆筒部112中所形成的大开口115内。凸缘227在大开口115内的收纳将装运盖200固定到壳体110。如图8所示,大开口115被配置以在凸缘227不弯曲的情况下容纳凸缘227。通过这种方式,支腿部220的支腿220a,220b能被弯曲离开壳体110的外圆筒部112,从而使锁定结构226的凸缘227从大开口115内退出。所以,当装运盖在装载单元100的壳体110上处于第一定向时,装运盖200被可释放地固定到装载单元100。在第一定向下,装运盖200的延伸部232接合装载单元100的推动器构件130的刀架140,从而阻止推动器构件130的行进。
在装运盖200以第一定向被附接到装载单元100的状态下,将装载单元100提供给临床医生。当装运盖200以第一定向被附接到装载单元100时,装载单元100可以以常规的方式被固定到致动单元12(图1)的细长本体16(图1)。如之前所述,装载单元100的壳体110可以通过插栓式联接器或以任意其他合适的方式被连接到致动单元12的细长本体16。通过将形成在推动器构件130的臂134的近端上的凸片135接收在驱动构件18(图5)的带凹口远端18a(图5)内,推动器构件130被连接到致动单元12的驱动构件18(图5)。当装载单元100被固定到致动单元12(图1)后,装运盖200可以被移除。
仍参见图7和8,通过使附接部220的支腿220a,220b弯曲离开装载单元100的壳体110的外圆筒部112,如图7中箭头“A”所示,来实现装运盖200从装载单元100上的移除。如之前所述,在壳体110的外圆筒部112上的大开口115的构型允许当支腿220a,220b弯曲离开外圆筒部112时,支腿220a,220b的锁定结构226从大开口115中被取出。形成在附接部220的每个支腿220a,220b上的凸片229有利于临床医生接合(即抓住)支腿220a,220b。在支腿220a,220b被充分地弯曲离开壳体110从而使锁定结构226被彻底地从大开口115内移除后,装运盖200能围绕外圆筒部112沿着朝向远侧的方向滑动,如箭头“B”所示,从而实现装运盖200与装载单元100的分离。当装运盖200与装载单元100分离后,将以常规的方式使用装载单元100。
在使用外科吻合装置10(图1)的吻合手术之后,装运盖200被重新附接到装载单元100,以阻止装载单元100的后续再击发、并且促进装载单元100与致动单元12的细长本体16的分离。具体地,装运盖200的支腿220a,220b被定位在装载单元100的相对的侧部,如图9中的箭头“C”所示,从而使装运盖200的延伸部232被接收在推动器构件130的远端136内,并且装运盖200的锁定结构226与壳体110的外圆筒部112的小开口117对准。当装运盖200的锁定结构226与形成在壳体110的外圆筒部112上的小开口117对准后,装运盖200的附接部220的支腿220a,220b被径向向内推动,如图9中的箭头“D”所示,从而迫使形成在凸缘227上的突起228与壳体110的外圆筒部112接合。径向向内的作用力在支腿220a,220b上的继续施加使得锁定结构226的凸缘227朝着彼此向内弯曲,由此允许凸缘227被接收在小开口117内。因为支腿220a,220b被径向向内地推动、且锁定结构226的凸缘227被接收在壳体110的外圆筒部112的小开口117内,所以凸缘227的自由端227a接合装载单元130的推动器构件130的臂134。凸缘227与推动器构件130的臂134的接合使得臂134径向向内弯曲,如图9中箭头“E”所示,从而有利于装载单元100的推动器构件130与致动单元12(图1)的驱动构件18(图5)的分离。
具体参见图10,在装运盖200的锁定结构226穿过小开口117被完全地收纳在壳体110的外圆筒部112内时,锁定结构226的凸缘227回到初始的未弯曲状态。在该初始状态,从锁定结构226的凸缘227伸出的突起228接合壳体110的外圆筒部112的内表面。突起228与壳体110的外圆筒部112的接合阻止锁定结构226从小开口117中移除,由此将装运盖200固定到装载单元100。
在装运盖200以第二定向被重新附接到装载单元100后,装载单元100能以常规的方式(例如相对于细长本体16转动装载单元100)与致动单元12(图1)的细长本体16(图1)分离。然后,另外的装载单元100能够被附接到致动单元12并按照以上所描述的方式进行使用。

Claims (14)
1.一种用于环形吻合装置的装载单元,包括:
限定了一对第一开口和一对第二开口的壳体,其中第一开口大于第二开口;
被可操作地接收在壳体内且在壳体内能从第一位置移动到第二位置的推动器组件;
被设置在壳体的远端上且包括多个吻合钉的钉仓;以及
被可操作地接收在壳体的远端上以防止推动器组件从第一位置移动到第二位置的装运构件,装运构件能以第一定向和第二定向被接收在壳体上,其中在第一定向,装运构件被可拆卸地固定到壳体,并且在第二定向,装运构件被牢固地固定到壳体;
其中装运构件包括:
基础部;
从基础部伸出的一对支腿;以及
被设置在所述一对支腿的每一个的自由端上的锁定结构,其中锁定结构能被可拆卸地固定在所述一对第一开口内且能被牢固地固定在所述一对第二开口内。
2.根据权利要求1所述的装载单元,其中装运构件的基础部包括被配置以设置在钉仓附近的内表面,基础部的内表面被配置以将多个吻合钉保持在钉仓内。
3.根据权利要求1所述的装载单元,其中装运构件还包括从基础部朝近侧伸出的保持部,保持部被配置以接合推动器组件。
4.根据权利要求3所述的装载单元,其中保持部包括至少一个延伸部。
5.根据权利要求4所述的装载单元,其中所述至少一个延伸部包括一对延伸部。
6.根据权利要求1所述的装载单元,其中每个锁定结构包括一对凸缘。
7.根据权利要求6所述的装载单元,其中所述一对凸缘中的每一个被配置以向内弯曲,从而有利于凸缘进入第二开口之一。
8.根据权利要求7所述的装载单元,其中每个凸缘包括从其径向向外延伸的突起。
9.根据权利要求1所述的装载单元,其中所述一对支腿中的每一条支腿包括凸片,从而便于使用者可操作地接合。
10.一种外科吻合装置,包括:
本体部分;
被可拆卸地固定到本体部分的装载单元;
被可操作地接收在装载单元的远端上的装运构件,装运构件能以第一定向和第二定向被接收在装载单元上,其中在第二定向,装运构件实现装载单元与本体部分的分离。
11.根据权利要求10所述的外科吻合装置,其中装载单元包括推动器组件,本体部分包括驱动组件,推动器组件被可拆卸地固定到驱动组件。
12.根据权利要求11所述的外科吻合装置,其中装运构件与推动器组件的接合使推动器组件与驱动组件脱离接合。
13.根据权利要求12所述的外科吻合装置,其中装运构件包括用于接合推动器组件的至少一个保持部。
14.根据权利要求13所述的外科吻合装置,其中装载单元包括壳体,所述至少一个保持部包括突起,当装运构件处于第二定向时突起接合壳体,从而将装运构件牢固地固定到壳体。
