DESCRIÇÃO
DISPOSITIVO PARA A FIXAÇÃO E ANCORAGEM DE PRÓTESES DE
VÁLVULAS CARDÍACAS O invento refere-se a um dispositivo para a fixação e ancoragem de próteses de válvulas cardíacas, formado essencialmente por elementos interligados em forma de arame. No estado dobrado pode ser introduzido de forma minimamente invasiva através da aorta e, após desdobramento, ser ancorado na parede da aorta, podendo desta maneira a prótese valvular cardíaca implantada e fixada assumir a função da própria válvula cardíaca do corpo.
Até à data não se conseguiu propor, de maneira satisfatória, uma solução com a qual possa ser assegurada, não só uma vedação segura em relação à parede da aorta, como também um assentamento seguro. Neste âmbito é necessário que um dispositivo ou suporte de ancoragem ("stent") deste género possa ser dobrado de forma a ficar com dimensões suficientemente pequenas para, a seguir, poder ser desdobrado no local de implantação. No entanto, nas soluções que se conhecem não se obtém um desdobramento suficiente que desenvolva uma força tensora adequada que possa garantir o assentamento seguro. Também as propostas nas quais se pretende utilizar um metal com memória (memory metal), não satisfazem as exigências, embora estes materiais se dilatem ao atingirem ou ultrapassarem uma temperatura de transição. 1 ΕΡ1994913Β1
As características do termo genérico da reivindicação 1 são também conhecidas a partir da patente US 5,411,552. A solução descrita na US 5,411,552 também não pode cumprir as exigências, uma vez que se pretende utilizar uma estrutura relativamente instável.
Um outro problema resolvido até à data apenas de maneira pouco satisfatória, é a fixação segura de uma prótese de válvula cardíaca artificial ou biológica. Em geral, as próteses são cosidas de maneira complicada ao "stent" para se fixarem. Esta acção leva muito tempo e tem de ser realizada com muito cuidado, para evitar danos.
Uma vez que as próteses de válvulas cardíacas implantadas têm de exercer a sua função durante períodos de tempo prolongados, também a realização construtiva desempenha um papel essencial, uma vez que, caso contrário, podem ocorrer danos e fugas após a implantação, que podem provocar estados de risco de morte do paciente.
Assim, o problema a resolver pelo invento é o de propor um dispositivo para a fixação e ancoragem de próteses de válvulas cardíacas que, para ser implantado de forma tão pouco invasiva quanto possível, através da aorta, possa ser dobrado em dimensões pequenas, sendo depois desdobrado no local de implantação, assegurando-se assim um assentamento e uma vedação seguros em relação à parede da aorta.
Este problema é resolvido com um dispositivo de acordo com a reivindicação 1. 2 ΕΡ1994913Β1
Podem conseguir-se formas de realização convenientes e aperfeiçoamentos do invento com as caracteristicas mencionadas nas reivindicações dependentes.
Os elementos essenciais da solução de acordo com o invento são três pares de arcos iguais, que estão ligados uns aos outros num arranjo com um deslocamento de 120° cada um em relação aos outros. Os dois arcos de um par têm uma curvatura oposta uma à outra e estão ligados por meio de articulações de corpo sólido. As articulações de corpo sólido cumprem simultaneamente a função de charneiras, em torno das quais os arcos de um par podem ser virados, à semelhança de um baloiço. No caso de ser exercida uma força de pressão num dos arcos, por exemplo, pelo peristaltismo da aorta, este arco é virado em conformidade, no mesmo sentido, em torno do eixo giratório existente na articulação de corpo sólido. Simultaneamente, o outro arco correspondente do par é virado no sentido contrário. Em consequência disso, é sempre apertado um dos dois arcos contra a parede da aorta, o que melhora a vedação e o assentamento. É conveniente dimensionar os arcos de um par de forma a serem mantidas tanto quanto possível as mesmas condições de alavanca em relação às articulações de corpo sólido que formam as charneiras, sendo portanto formados braços de alavanca de comprimento igual, ou pelo menos aproximadamente igual. São também vantajosas as distâncias relativamente grandes das articulações de corpo sólido que resultam da 3 ΕΡ1994913Β1 disposição em 120° dos pares de arcos e também as grandes áreas cobertas pelos arcos.
Neste âmbito, os arcos distais não servem apenas para a fixação da prótese de válvula cardíaca, mas assumem também a função de apoio.
As vantagens mencionadas podem ainda ser mais aperfeiçoadas através de um outro arco curvo e disposto adicionalmente na direcção distai.
Neste caso, o segundo arco distai possui, na sua zona distai, uma curvatura aproximadamente igual à do primeiro arco distai. Em parte, estes dois arcos são realizados e moldados de forma a se estenderem de forma paralela, formando-se fendas entre eles. Podem estar ligados um ao outro no mesmo local onde também estão dispostas as articulações de corpo sólido, como ligação ao arco com curvatura na direcção proximal. Por este motivo, as fendas formadas estão abertas na direcção distai, podendo ser introduzidas e seguradas na fenda partes da prótese de válvula cardíaca.
Pelo menos uma parte de um arco distai está retraída em direcção ao lado proximal e continua até um ponto de inflexão onde se encontram arcos vizinhos. Em dois arcos dispostos em posição distai, acontece isto com o respectivo arco distai externo.
Para o reforço e uma possibilidade de fixação adicional da prótese de válvula cardíaca, pode utilizar-se 4 ΕΡ1994913Β1 um arco com curvatura angular, igualmente retraído em direcção ao lado proximal, cujas partes curvas estão em ambos os casos dispostas entre dois arcos vizinhos e acompanham, em parte, a curvatura correspondente. Estes arcos estão fixados com as suas respectivas extremidades distais no arco distai exterior ou no respectivo arco distai exterior. Também neste caso a fixação forma em ambos os casos uma articulação de corpo sólido. No entanto, estas deveriam estar dispostas de forma distanciada em relação às outras articulações de corpo sólido que ligam os arcos de um par.
Uma vez implantado e desdobrado o dispositivo, os bolsos de uma prótese valvular cardíaca podem ser ai introduzidos, segurados e apoiados. A construção em arcos do dispositivo de acordo com o invento apoia uma prótese de válvula cardíaca numa grande área e, por conseguinte, de maneira cuidadosa. Além disso, pode ser fixada de forma substancialmente menos trabalhosa, por exemplo cosendo-a. A solução construtiva torna possível um assento seguro e a vedação necessária na ou em relação à parede da aorta. Um efeito favorável em relação à vedação e à diminuição da carga que actua na prótese de válvula cardíaca tem também a pressão exercida na prótese de válvula cardíaca desde o interior, por meio dos arcos. 0 dispositivo de acordo com o invento pode ser implantado mediante um cateter com balão e ser desdobrado 5 ΕΡ1994913Β1 no local da implantação. É conveniente utilizar para o dispositivo também um metal com memória com uma temperatura de transição adequada, com o qual se consegue obter uma dilatação adicional. Para este efeito pode utilizar-se uma liga contendo níquel e titânio, disponível sob a denominação nitinol.
Além disso, a parte do dispositivo que suporta e segura a prótese de válvula cardíaca, pode ser implantada separadamente de um corpo de apoio, ao qual nos referiremos ainda posteriormente, na descrição de um exemplo de realização, sem que as características vantajosas sejam diminuídas. A implantação desta peça, que consiste no essencial nos três segmentos com a prótese de válvula cardíaca fixada nos mesmos, pode então ser realizada de maneira convencional através de uma intervenção cirúrgica. A seguir, o invento será melhor pormenorizado por meio de um exemplo de realização, no qual:

Neste exemplo utilizam-se dois arcos 4 e 5, dispostos distalmente do lado exterior, podendo eventualmente prescindir-se do arco exterior 5. 0 arco 4 com curvatura arqueada está ligado a um arco 3 curvo no sentido contrário. As ligações dos dois lados formam articulações de corpo sólido 7 as quais desempenham simultaneamente a função de charneiras para os dois arcos 3 e 4 que representam alavancas, como já se explicou na parte geral da descrição. 0 segundo arco 3, com curvatura para fora e disposto distalmente, aumenta a estabilidade e oferece uma possibilidade adicional de apoio e fixação para a prótese de válvula cardíaca. Os dois arcos 4 e 5 dispostos distalmente do lado exterior estão também ligados um ao outro, podendo a ligação ser realizada no mesmo local onde também estão dispostas as articulações de corpo sólido 7.
Entre os dois arcos 4 e 5 existem fendas abertas na direcção distai, nas quais podem ser introduzidas, e nelas fixadas, partes da prótese de válvula cardíaca. 0 arco 5, aqui no lado exterior, está puxado mais para dentro na direcção proximal e a sua extremidade está ligada a uma nervura de apoio 8. Neste exemplo, as nervuras de apoio 8 estendem-se paralelamente ao eixo longitudinal do dispositivo e formam, em conjunto com nervuras transversais em forma de dentes de serra, rombos ou meandros, um corpo de apoio, o qual fica no estado desdobrado encostado à parede da aorta. Para a ancoragem, as nervuras de apoio 8 7 ΕΡ1994913Β1 e/ou as nervuras transversais podem possuir pontas adicionais 9 que se prendem na parede da aorta. A disposição e o comprimento das nervuras de apoio 8 e a distância adequadamente grande em relação à prótese valvular cardíaca fixada na zona dos arcos 3, 4 e 5, tornam possível o posicionamento da prótese de válvula cardíaca, sem fechar ou tapar os vasos coronários.
No exemplo aqui mostrado, existem entre os segmentos individuais utilizados numa disposição de 120°, arcos adicionais 2 retraídos em direcção ao lado proximal, os quais estão ligados aos arcos distais do lado exterior 5. Também neste caso, as ligações são articulações de corpo sólido 6 que, no entanto, deveriam estar dispostas, se possível, distanciadas das articulações de corpo sólido 7. Deste modo podem usar-se duas alavancas por segmento, realizando-se com uma estrutura duplamente simétrica deste género, aproximadamente, o dobro das forças para fixar o dispositivo.
Mais uma vez, no estado implantado desdobrado podem ser introduzidas alternadamente, apoiadas deste modo e aí fixadas, entre as partes 1 dos arcos 5 e os arcos 2, partes da prótese de válvula cardíaca.
No entanto, pode ainda aumentar-se o número de arcos utilizados para melhorar o assentamento e diminuir ainda mais a carga exercida na prótese de válvula cardíaca.

Claims (8)
- ΕΡ1994913Β1 REIVINDICAÇÕES 1. Dispositivo para a fixação e ancoragem de próteses de válvulas cardíacas, formado por elementos em forma de arame, sendo que o dispositivo apresenta um "stent" que consiste em três construções em arco iguais (3, 4, 5) numa disposição de 120°, caracterizado por cada construção em arco (3, 4, 5) apresentar os seguintes elementos: - um primeiro arco (4) com curvatura para o exterior e disposto numa primeira extremidade do "stent", assim como um segundo arco (5) também com curvatura para o exterior e disposto na primeira extremidade do "stent"; e um arco (3) com curvatura em direcção oposta relativamente ao primeiro e ao segundo arco (4, 5) , ligado ao primeiro arco (4) com curvatura para o exterior, apresentando o stent, além disso, nervuras de apoio (8) que estão ligadas às extremidades dos segundos arcos (5) estendidos ainda mais em direcção à segunda extremidade do "stent", e que se estendem paralelamente ao eixo longitudinal em direcção à segunda extremidade do "stent", estando as nervuras de apoio (8) ligadas umas às outras através de nervuras transversais realizadas sob a forma de dentes de serra, losangos ou meandros, para formação de um corpo de apoio na segunda extremidade do "stent".
- 2. Dispositivo de acordo com a reivindicação 1, 1 ΕΡ1994913Β1 em que os primeiros e segundos arcos (4, 5) com curvatura para o exterior de cada construção em arco (3, 4, 5) estão dispostos uns em relação aos outros de maneira a haver entre eles uma fenda na qual possam ser introduzidas e fixadas ai partes de uma prótese de válvula cardíaca a fixar ao "stent".
- 3. Dispositivo de acordo com uma das reivindicações anteriores, em que o "stent" pode ser transformado de um estado dobrado durante a implantação, para um estado desdobrado no local de implante.
- 4. Dispositivo de acordo com a reivindicação 3, em que o "stent" é realizado sob a forma auto-expansível, consistindo num material com memória de forma, com temperatura de transição adequada.
- 5. Dispositivo de acordo com uma das reivindicações anteriores, em que estão realizadas, além disso, pontas adicionais (9) nas nervuras de apoio (8) e/ou nas nervuras transversais, para a ancoragem do "stent" no local de implante.
- 6. Dispositivo de acordo com uma das reivindicações anteriores, em que estão realizados, na segunda extremidade do "stent", ao todo, três ilhós, distribuídos uniformemente pela periferia do "stent".
- 7. Dispositivo de acordo com uma das reivindicações anteriores, em que está prevista, além disso, uma prótese 2 ΕΡ1994913Β1 de válvula cardíaca fixada nas respectivas construções em arco (3, 4, 5) através de costura.
