Oppfinnelsen område
Foreliggende oppfinnelse angår fullstendig humane antistoffer og, nærmere bestemt, fullstendig humane antistoffer mot human 4-IBB (CD137).
BAKGRUNN FOR OPPFINNELSEN
Et omfattende bevismateriale har utvetydig vist at det eksisterer en viss grad av immunrespons mot kreft hos mennesker og dyr. Hos kreftpasienter kan cellulære komponenter av immunsystemet gjenkjenne antigener uttrykt ved tumorceller, slik som differensiering av onkoføtale antigener eller muterte genprodukter (S. Rosenberg, Nature, 411:380-4 (2001); P. van der Bruggen et al., Immunological Rev., 188:51-64 (2002)). Flere kliniske undersøkelser har vist at tumorinfiltrerende lymfocytter har fordelaktig prognostisk betydning (E. Halapi, Med. Oncol., 15(4):203-11 (1998); Y. Naito et al., Cancer Res., 58(16):3491-4 (1998); L. Zhang et al., N.E. J. Med., 348(3):203-13 (2003)). Videre har behandling med immunmodulatorer, slik som cytokiner eller bakterielle produkter, kreftvaksiner eller adoptiv immunterapi ført til tumorregresjon hos flere pasienter (S. Rosenberg, Cancer J. Sei. Am. 6(S):2 (2000); P. Bassi, Surg. Oncol., 11(1-2):77-83 (2002); S. Antonia et al., Current Opinion in Immunol., 16:130-6 (2004)). Til tross for disse responsene mislykkes immunitet mot kreft ofte i å effektivt fjerne tumorceller. Årsakene til denne svikten kan grupperes inn i tre hovedkategorier: (i) svekket tumorgjenkjennelse ved immunceller, enten ved variabel ekspresjon av tumorantigener eller redusert ekspresjon av klasse I major histocompatibility komplekset (MHC); (ii) immunosuppressivt tumor mikromiljø, som et resultat av sekresjon av hemmende cytokiner ved tumorceller (feks. TGF-Ø); og (iii) dårlig tumorimmunogenisitet på grunn av mangel på ekspresjon av kostimulerende molekyler ved tumorceller, hvilket fører til manglende evne for tumorcellene til å effektivt stimulere T-celler. Forbedring av vår forståelse av kravene for gjenkjennelse av tumorantigen og immun effektorfunksjon indikerer at en potensiell strategi for å forbedre en anti-tumor immunrespons er å gi kostimulering gjennom et hjelpemolekyl. Tumorantigen spesifikke T-celler krever kostimulering for å initiere og opprettholde effektorfunksjoner. Følgelig kan behandlinger som målretter kostimulerende molekyler anvendes for å modulere og forsterke immunresponsen til tumorer.
Den rådende modellen for T-celleaktivering postulerer at naive T-celler krever to signaler for full aktivering: (i) et signal gitt gjennom binding av prosesserte antigener presentert for T-cellereseptoren av major histocompatibility kompleks (MHC) klasse I-molekyler; og (ii) et ytterligere signal gitt ved interaksjon av kostimulerende molekyler på overflaten av T-celler og deres ligander på antigenpresenterende celler. Gjenkjennelse av et antigen ved en naiv T-celle er ikke tilstrekkelig i seg selv til å utløse T-celleaktivering. Uten et kostimulerende signal, kan T-celler elimineres ved død eller ved induksjon av anergi. Signalering gjennom det kostimulerende molekylet CD28 synes å være nøkkelen for initieringen av T-celleresponser. Imidlertid er CD137 (4-lBB)-signalering vist å være primordial for opprettholdelse og ekspansjon av immunresponsen til antigener, så vel som, for dannelse av memory T-celler.
CD137 (4-IBB) er et medlem av tumornekrose reseptor (TNF-R)-genfamilien, som omfatter proteiner involvert i regulering av celleproliferasjon, differensiering og programmert celledød. CD 137 er et 30 kDa type I membran glykoprotein uttrykt som en 55 kDa homodimer. Reseptoren ble initielt beskrevet hos mus (B. Kwon et al., P.N.A.S. USA, 86:1963-7 (1989)) og senere identifisert hos mennesker (M. Alderson et al., Eur. J. Immunol., 24: 2219-27 (1994); Z. Zhou et al., Immunol. Lett., 45:67 (1995)) (Se også publiserte PCT-søknader WO95/07984 og W096/29348 og U.S. Patent nr. 6,569,997, (se SEKV ID NR:2.)). De humane og murine formene av CD137 er 60% identiske på aminosyrenivå. Konserverte sekvenser forekommer i det cytoplasmatiske domenet, så vel som i 5 andre regioner av molekylet, hvilket indikerer at disse restene kan være viktige for funksjon av CD137-molekylet (Z. Zhou et al., Immunol. Lett., 45:67 (1995)). Ekspresjon av CD137 er vist å være overveiende i celler av lymfoid linje slik som aktiverte T-celler, aktiverte Natural Killer (NK) celler, NKT-celler, CD4CD25 regulatoriske T-celler og også i aktiverte tymocytter og intraepiteliale lymfocytter. I tillegg har CD 137 også blitt vist å uttrykkes i celler av myeloid opprinnelse slik som dendrittiske celler, monocytter, nøytrofiler og eosinofiler. Selv om ekspresjon av CD137 hovedsakelig er begrenset til immun/inflammatoriske celler, finnes det rapporter som beskriver dets ekspresjon i endotelceller forbundet med et lite antall vev fra inflammatoriske seter og tumorer.
Funksjonelle aktiviteter av CD137 på T-celler er utførligkarakterisert. Signalering gjennom CD137 i nærvær av suboptimale doser av anti-CD3 har blitt vist å indusere T-celleproliferasjon og cytokinsyntese (hovedsakelig IFN-y) og å hemme aktivert celledød. Disse effektene har blitt observert med både murine og humane T-celler (W. Shuford et al., J. Exp. Med., 186(l):47-55 (1997); D. Vinay et al., Semin. Immunol., 10(6):481-9 (1998); D. Laderach et al., Int. Immunol, 14(10): 1155-67 (2002)). Hos både mennesker og mus, øker kostimulering effektorfunksjoner, slik som produksjon av IFN-y og cytotoksisitet, ved å forøke antallet av antigenspesifikke og effektor CD8+ T-celler. I fravær av anti-CD3-signalering, endrer ikke stimulering gjennom CD137 T-celle funksjon, hvilket indikerer at CD 137 er et kostimulerende molekyl.
De fysiologiske hendelsene observert etter CD137-stimulering på T-celler medieres av NF-kB og PI3K/ERKl/2-signaler med atskilte fysiologisk funksjoner. NF-KB-signaler utløser ekspresjon av Bc1-xl, et antiapopotisk molekyl, og resulterer følgelig i øket overlevelse, mens PI3K og ERKl/2-signaler er spesifikt ansvarlige for CD137-mediert cellecyklusprogresjon (H. Lee et al., J. Immunol., 169(9):4882-8 (2002)). Effekten av CD137-aktivering på hemming av aktiveringsindusert celledød ble vist in vitro av Hurtado et al. (J. Hurtado et al., J. Immunol, 158(6):2600-9 (1997)) og i et in vzvo-system hvor anti-CD137 monoklonale antistoffer (mabs) ble vist å frembringe langvarig overlevelse av superantigen-aktiverte CD8+ T-celler ved å forhindre klonal delesjon (C. Takahashi et al, J. Immunol, 162:5037 (1999)). Senere viste to rapporter, under ulike forsøksbetingelser, at CD137-signalet regulerte både klonal ekspansjon og overlevelse av CD8+ T-celler (D. Cooper et al, Eur. J. Immunol, 32(2):521-9 (2002); M. Maus et al, Nat. Biotechnol, 20:143 (2002)). Redusert apoptose observert etter kostimulering samsvarte med økede nivåer av Bc1-xli CD8+ T-celler, mens Bcl-2-ekspresjon forble uendret. Oppregulering av de anti-apoptotiske genene Bc1-xlog bfl-1 via 4-IBB ble vist å være mediert av NF-kB-aktivering, siden PDTC, en NF-KB-spesifikk blokker, hemmet 4-lBB-mediert oppregulering av Bcl-xL(H. Lee et al, J. Immunol, 169(9):4882-8 (2002)). På den annen side synes klonal ekspansjon av aktiverte T-celler å være mediert av øket ekspresjon av cyklinene D2, D3 og E og nedregulering av p27<klpl->proteinet. Denne effekten forekommer både på en IL-2-avhengig og uavhengig måte (H. Lee et al, J. Immunol, 169(9):4882-8
(2002)).
Alt i alt fører CD137-stimulering til forbedret ekspansjon, overlevelse og effektorfunksjoner av nylig "primet" CD8+ T-celler, som virker, delvis, direkte på disse cellene. Både CD4+ og CD 8+ T-celler er vist å respondere til CD137-stimulering, imidlertid synes det som om forbedring av T-celle funksjon er større i CD8+ celler (W. Shuford et al, J. Exp. Med., 186(l):47-55 (1997); I. Gramaglia et al, Eur. J. Immunol, 30(2):392-402 (2000); C. Takahashi et al, J. Immunol, 162:5037 (1999)). Basert på den kritiske rollen av CD137-stimulering i CD8+ T-cellefunksjon og overlevelse, gir manipulering av CD137/CD137L-systemet en rimelig tilnærmingsmåte for behandling av tumorer og virale patogener.
Nylig ble konstitutiv ekspresjon av CD 137 på nyisolerte dendrittiske celler (DC'er) vist hos mus (R. Wilcox et al., J. Immunol, 169(8):4230-6 (2002); T. Futagawa et al, Int. Immunol, 14(3):275-86 (2002)) og mennesker (S. Pauly et al, J. Leukoc. Biol. 72(1):35-42 (2002)). Disse rapportene viste at stimulering av CD137 på dendrittiske celler førte til sekresjon av IL-6 og IL-12 og, viktigere, det forbedret de dendrittiske cellenes evne til å stimulere T-celleresponser til alloantigener. Videre viste Pan et al. at CD137-signalering i dendrittiske celler resulterte i oppregulering av MHC Klasse I og kostimulerende molekyler og produserte en øket evne for dendrittiske celler til å infiltrere tumorer (P. Pan et al, J. Immunol, 172(8):4779-89 (2004)). Derfor synes CD137 kostimulering av dendrittiske celler å være en ny overføringsbane ("pathway") for proliferasjon, modning og migrering av dendrittiske celler.
Aktivert Natural Killer (NK) celler uttrykker CD 137 etter stimulering med cytokiner (I. Melero et al, Cell Immunol, 190(2): 167-72 (1998); R. Wilcox et al, J. Immunol, 169(8):4230-6 (2002)). Mange rapporter viste at NK-celler synes å være kritisk for moduleringen av antitumor immunresponsen indusert av agonistiske CD137-antistoffer ((I. Melero et al, Cell Immunol, 190(2): 167-72 (1998); R. Miller et al, J. Immunol, 169(4): 1792-800 (2002); R. Wilcox et al, J. Immunol, 169(8):4230-6 (2002)). Deplesjon av NK-celler reduserer antitumoraktiviteten til anti-CD137 mabs i betydelig grad. Ligering av CD137 til NK-celler induserer proliferasjon og sekresjon av IFN-y, men påvirker ikke deres cytolytiske aktivitet. Særlig, in vitro, fremviste CD137-stimulerte NK-celler en immunregulerende eller "hjelper"-aktivitet for CD8+ cytolytiske T-celler hvilket resulterer i ekspansjon av aktiverte T-celler. Derfor kan CD137-signalering på NK-celler modulere innate immunitet til tumorer.
En paradoksal effekt har blitt beskrevet for CD137-stimulering i at agonistiske CD137-antistoffer kan indusere suppresjon av de humorale responsene til T-celleantigener i primat og musemodeller (H. Hong et al, J. Immunother., 23(6):613-21 (2000); R. Mittler et al, J. Exp. Med., 190(10):1535-40 (1999)). Særlig ble CD137 agonistiske antistoffer vist å frembringe betydelig forbedring av symptomene forbundet med antistoffavhengige autoimmune sykdommer slik som lupus erythematosus disseminatus og eksperimentell autoimmun encefalomyelitt (J. Foell et al, N.Y. Acad. Sei., 987:230-5 (2003); Y. Sun et al, Nat. Med., 8(12): 1405-13 (2002)). Nylig viste Seo et al. at, i en musemodell av revmatoid artritt, forhindret behandling med et agonistisk anti-CD137 antistoff utviklingen av sykdommen og bemerkelsesverdig, blokkerte sykdomsprogresjon (S. K. Seo et al., Nat. Med. 10; 1099-94 (2004)). Mekanismen ansvarlig for denne effekten er ikke veldefinert, men i modellen av revmatoid artritt ble det vist at behandling med et CD 137 agonistisk antistoff resulterte i ekspansjon av IFN-y-produserende CD11C-CD8+ T-celler. IFN-y stimulerte i sin tur dendrittiske celler til å produsere indolamin-2,3-dioksygenase (IDO), som utøver immunosuppressive aktiviteter. Det har også vært postulert at CD 137-signalering av antigenaktiverte CD4 + T-celler fører til induksjon av IFN-y-sekresjon hvilket aktiverer makrofager. Aktiverte makrofager kan på sin side produsere dødssignaler for B-celler. Kontinuerlig signalering gjennom CD137 på CD4+ T-celler kan deretter indusere aktiveringsindusert celledød (AICD) for disse CD4+-aktiverte T-cellene. Derfor, ved å fjerne antigenaktiverte T-celler og B-celler, observeres en redusert antistoffrespons og, følgelig observeres en dramatisk reduksjon av Th2-medierte inflammatoriske sykdommer (B. Kwon et al, J. Immunol., 168(11):5483-90 (2002)). Disse undersøkelsene angir en funksjon for anvendelse av agonistiske CD137-antistoffer for behandling av inflammatoriske eller autoimmune sykdommer, uten å indusere en generell suppresjon av immunsystemet.
Den naturlige liganden for CD137, CD137-ligand (CD137L), et 34kDa glykoproteinmedlem av TNF-superfamilien, detekteres hovedsakelig på aktiverte antigenpresenterende celler (APC), slik som B-celler, makrofager, dendrittiske celler og også på murine B-cellelymfomer, aktiverte T-celler og humane karsinomlinjer av epitelial opprinnelse (R. Goodwin et al., Eur. J. Immunol., 23(10):2631-41 (1993); Z. Zhou et al., Immunol. Lett., 45:67 (1995); H. Salih et al., J. Immunol., 165(5):2903-10 (2000)). Human CD137L har 36% homologi med dens murine motstykke (M. Alderson et al., Eur. J. Immunol., 24: 2219-27 (1994)).
I tillegg til å overlevere signaler til CD137-uttrykkende celler, initierer binding av CD137 til CD137L et bidireksjonalt signal som resulterer i funksjonelle effekter på CD137L-uttrykkende celler. Langstein et al. viste at binding av CD137-Ig fusjonsprotein til CD137L på aktiverte monocytter induserte produksjon av IL-6, IL-8 og TNF-a, oppregulerte ICAM og inhiberte IL-10, hvilket fører til øket adherens (J. Langstein et al., J. Immunol., 160(5):2488-94 (1998)). I tillegg ble proliferasjon av monocytter vist sammen med en høyere rate av apoptose (J. Langstein et al., J. Leukoc. I et annen viktig aspekt av T-cellefunksjon, har det blitt vist at agonistiske anti-CD137-antistoffer reddet T-celleresponser til proteinantigener hos eldre mus. Det har blitt godt dokumentert at det forekommer en aldersrelatert tilbakegang i immunresponsen til antigener, en prosess kjent som immunosenescens (R. Miller, Science, 273:70-4 (1996); R. Miller, Vaccine, 18:1654-60 (2000); F. Hakim et al., Curr. Opinion Immunol., 16:151-156
(2004)). Dette fenomenet synes å skyldes endringer i likevekt mellom graden av cellulær ekspansjon og cellulær overlevelse eller død. Bansal-Pakala et al. testet hypotesen om at sekundær kostimulering gjennom CD 137 kan anvendes for å forbedre T-celleresponser i tilfeller hvor T-celler ikke mottar tilstrekkelig stimulering, på grunn av enten redusert ekspresjon av CD3 eller CD28 eller redusert kvalitet av signaler. Deres studier viste at eldre mus hadde en manglende in vitro "recall respons" sammenlignet med unge mus (P. Bansal-Pakala et al., J. Immunol, 169(9):5005-9 (2002)). Når eldre mus ble behandlet med anti-CD137 mabs, var imidlertid de proliferative og cytokinresponsene av T-celler identisk med responsene observert hos unge mus. Selv om den spesifikke mekanismen ansvarlig for denne effekten ikke ble klarlagt, ble det spekulert på at forøkning av både ekspresjon av antiapoptotiske molekyler som Bc1-xlog fremming av IL-2-sekresjon in vivo kan spille en rolle med hensyn til å redde ("rescue") defekte T-celleresponser. I en avhandling ved Melero et al, frembrakte agonistisk anti-mus CD 13 7-antistoff helbredelser avP815 mastocytom tumorer og i den lave immunogene tumormodellen Agl04 (I. Melero et al, Nat. Med., 3(6):682-5
(1997)). Anti-tumoreffekten krevde både CD4+ og CD8+ T-celler og NK-celler, siden selektiv in vivo deplesjon av hver subpopulasjon resulterte i reduksjon eller fullstendig tap av anti-tumoreffekten. Det ble også vist at en minimal induksjon av en immunrespons var nødvendig for at anti-CD137-behandling skulle være effektiv. Mange forskere har anvendt anti-CD137 antistoffer for å demonstrere levedyktigheten av denne metoden for kreftterapi (J. Kim et al, Cancer Res., 61(5):2031-7 (2001); O. Til støtte for anti-tumor effektivitetsdataene med agonistiske CD137-antistoffer, har signaler gitt av CD137L vist å fremkalle CTL-aktivitet og anti-tumorresponser (M. DeBenedette et al., J. Immunol., 163(9):4833-41 (1999); B. Guinn et al., J. Immunol., 162(8):5003-10 (1999)). Salih et al. har angitt ekspresjon av CD137L i humane karsinomer og human karsinomcellelinjer (H. Salih et al., J. Immunol, 165(5):2903-10 (2000)) og viste at tumorcellene som uttrykker liganden var i stand til å levere et kostimulerende signal til T-celler hvilket førte til frigjøring av IFN-y og IL-2 og at denne effekten samsvarte med nivåene av CD137L ved tumorer. Hvorvidt ekspresjon av CD137L i humane tumorer kunne gjøre disse tumorene mer mottagelige for agonistiske CD 137-antistoffer er ikke kjent.
CD137L -/- mus har understreket betydningen av CD137/CD137L-systemet i T-celleresponser til både virus og tumorer (M. DeBenedette et al, J. Immunol, 163(9):4833-41 (1999); J. Tan et al, J. Immunol, 164(5):2320-5 (2000); B. Kwon et al, J. Immunol, 168(11):5483-90 (2002)). Undersøkelser ved anvendelse av CD137- og CD137L-defisitte mus har vist betydningen av CD 137 kostimulering i graft-versus-host sykdom og anti-virale cytolytiske T-celleresponser. CD137-defisitte mus hadde en forbedret proliferasjon av T-celler, men en reduksjon i cytokinproduksjon og cytotoksisk T-celle aktivitet (B. Kwon et al, J. Immunol, 168(11):5483-90 (2002); D. Vinay et al, Immunol. Cell Biol, 81(3): 176-84 (2003)). Senere ble det vist at knockout-mus (CD137-/-) hadde en høyere hyppighet av tumormetastaser (4 ganger) sammenlignet med kontrollmus. Disse dataene indikerer at gjenopprettelse av CD137-signalering ved anvendelse av agonistiske anti-CD137-antistoffer er en anvendbar metode for å øke cellulære immunresponser til virale patogener og kreft.
I tillegg til dataene i mus in vzvo-modeller som støtter involvering av CD 137-signalering i antitumor immunresponser, har undersøkelser utført i primær human tumor-prøver bekreftet rollen til CD 137 med hensyn til frembringelse av effektor T-celler. Hos pasienter med Ewing sarkom, viste Zhang et al. at intratumorale effektor T-celler fremviste CD3+/CD8+/CD28-/CD137+ -fenotypen. Uventet ble sameksistens av progressiv tumorvekst og anti-tumor-immunitet (effektor T-celler) observert. Ex vivo stimulering-studier med pasienters celler viste at tumorindusert T-celleproliferasjon og aktivering krevde kostimulering med CD137L. Stimulering av PBL med anti-CD3/CD137L, men ikke anti-CD3/anti-CD28, induserte tumor lytiske effektorer. Disse undersøkelser ga ytterligere bevis for at CD137-mediert kostimulering kunne resultere i ekspansjon av tumorreaktive CTL'er (H. Zhang et al., Cancer Biol. Ther., 2(5):579-86 (2003)). Videre ble ekspresjon av CD137 vist i i tumorinfiltrerende lymfocytter i hepatocellulære karsinomer (HCC) (Y. Wan et al., World J. Gastroenterol, 10(2): 195-9 (2004)). CD137-ekspresjon ble detektert i 19 av 19 HCC ved RT-PCR og i 13/19 ved immunfluorescensfarging. Omvendt ble ikke CD137 detektert i de perifere mononukleære cellene fra de samme pasientene. Analyser utført i levervev fra frisk donor mislyktes i å demonstrere ekspresjon av CD 137. Disse undersøkelser forsøkte ikke å korrelere klinisk sykdom med CD137-ekspresjon. Undersøkelser utført i Ewing sarkom og hepatocellulært karsinom avslørte følgelig tilstedeværelse av TIL som uttrykker CD137, med ledsagende sykdomsprogresjon. I Ewing sarkom ble det vist at CD137+TIL var i stand til drepe tumorceller ex vivo hvilket indikerer at CD137-overføringsbanen var intakt hos disse pasientene og at kanskje suppressive faktorer i mikromiljøet i tumor hemmet deres funksjon. Følgelig kan det postuleres at systemisk administrering av agonistiske CD137-antistoffer kan gi signalet nødvendig for ekspansjon av disse effektor T-cellene.
I tillegg til dens funksjon ved utviklingen av immunitet til kreft, støtter eksperimentelle data anvendelse av CD137 agonistiske antistoffer for behandling av autoimmune og virale sykdommer (B. Kwon et al., Exp. Mol. Med., 35(1):8-16 (2003); H. Salih et al., J. Immunol., 167(7):4059-66 (2001); E. Kwon et al., P.N.A.S. USA, 96:15074-79 (1999); J. Foell et al., N.Y. Acad. Sei., 987:230-5 (2003); Y. Sun et al, Nat. Med., 8(12):1405-13 (2002) S. K. Seo et al, Nat. Med. 10;1099-94 (2004)).
W09816249 beskriver en fremgangsmåte for å forsterke lymfocyttisk dreping av tumorceller i en vert som innbefatter administrering til verten av en effektiv dose av en anti-4-1 BB- (CD137) Ab. For human administreroing er Ab fortrinnsvis og i hovedsak en human Ab (E.F. p 5,1 30? 35). W02004010947 tilveiebringer et humanisert anti-human 4-1 BB antistoff, Le. Hu39E3.G4, som ikke hemmer bindingen av humant 4-1 BB til en human 4-1 BB ligand. Også tilveiebrakt er en metode for behandling av kreft i et individ omfattende administrering av en terapeutisk effektiv mengde av antistoffet til nevnte individ.
LOO D T et al. (The Journal of Biological Chemistry, 7 mars 1997, 272 (10): 6448-6456) rapporterer at binding av 4 1 BBL til 4-1 BB.LN kompleks ikke fortrenger laminin (LN). Dvs. de to ligander bindes til proksimale, men distinkte områder på 4-1 BB. 6/8 anti-4-1 BB monoklonale antistoffer blokkere interaksjonen mellom 4-1 BB og 4-1 BBL, mens 7 blokkerer LN binding. Antistoff 1D8 er vist å blokkere LN men ikke murin 4-1 BB binding og gjenkjenner 4-1 BBL-bindingsmangel trunkert 4-1 BB protein som mangler det N-terminale LN-homologe domenet (sammendraget; s 6452).
Følgelig, basert på funksjonene for 4-IBB i å modulere immunrespons, ville det være ønskelig å produsere anti-human 4-lBB-antistoffer med agonistiske aktiviteter som kunne anvendes for behandling eller forebygging av humane sykdommer slik som kreft, infeksiøse sykdommer og autoimmune sykdommer.
KORT OPPSUMMERING AV OPPFINNELSEN:

Foreliggende oppfinnelse tilveiebringer fullstendig humane antistoffer som binder til human 4-IBB (H4-1BB) og som tillater binding av H4-1BB til en human 4-lBB-ligand (H4-1BBL). Oppfinnelsen angår følgelig antistoffer som binder til H4-1BB og som ikke blokkerer bindingen av H4-1BB til H4-1BBL, hvilket derved tillater binding av både et antistoff ifølge oppfinnelsen og H4-1BBL til H4-1BB. Oppfinnelsen tilveiebringer også antistoffer med agonistiske aktiviteter ved at binding av antistoffene til H4-1BB resulterer i en forsterking og stimulering av H4-lBB-medierte immunresponser. Disse antistoffene kan anvendes som immun-enhancere av en anti-tumor eller anti-viral immunrespons eller som immunmodulatorer av T-cellemedierte autoimmune sykdommer. Antistoffene kan også anvendes som diagnostisk verktøy for deteksjon av H4-1BB i blod eller vev fra pasienter med kreft, autoimmune eller andre sykdommer.

Antistoffene ifølge oppfinnelsen har bred terapeutisk anvendelse som immunmodulatorer av sykdommer slik som kreft, autoimmune sykdommer, inflammatoriske sykdommer og infeksiøse sykdommer.
Oppfinnelsen tilveiebringer videre fullstendig humant monoklonalt antistoff for behandling av kreft hos et individ omfattende å administrere en terapeutisk effektiv mengde av et antistoff ifølge oppfinnelsen til subjektet. I ett aspekt omfatter denne anvendelsen videre administrering av en vaksine. Egnede vaksiner omfatter for eksempel en tumorcelle vaksine, en DNA-vaksine, en GM-CSF-modifisert tumorcelle vaksine eller en antigen-ladet dendrittisk cellevaksine. Kreften kan for eksempel være prostatakreft, melanom eller epitelial kreft.
Fullstendig humant monoklonalt antistoff kan anvendes for å forbedre immunresponsen, omfattende å administrere et antistoff ifølge oppfinnelsen og en SIV gag-vaksine. Oppfinnelsen omfatter også en cellelinje som produserer et antistoff eller antigenbindende del derav ifølge oppfinnelsen, rekombinante ekspresjonsvektorer som omfatter nukleotidene ifølge oppfinnelsen og fremgangsmåter for fremstilling av antistoffene ifølge oppfinnelsen ved dyrking av en antistoffproduserende cellelinje.

Dyr ble behandlet med en SIV gag-vaksine (dag 0, 28, 56; FIG. 16A), SIV gag-vaksine (dag 0, 28, 56) og mab 20H4,9-IgG4 (dag 12, 15 og 19; FIG. 16B) eller SIV gag-vaksine (dag 0, 28, 56) og hu39E3.G4 (dag 12, 15 og 19; FIG 16C). En gruppe dyr ble latt være ubehandlet (FIG. 16D). Omfanget av foreliggende oppfinnelse er definert gjennom kravene.
Oppfinnelsen angår fremstilling og karakterisering av antistoffer og antigenbindende fragmenter derav (omfattende fusjonsproteiner som omfatter et antigenbindende fragment av et antistoff ifølge oppfinnelsen), for anvendelse ved behandling av en sykdom, slik som kreft, infeksiøs sykdom, inflammatorisk sykdom eller autoimmun sykdom. Kreften kan for eksempel være prostatakreft, melanom eller epitelial kreft.
Antistoffene er i stand til å binde til H4-1BB og kan fremvise høy affinitet for H4-1BB og effektivt forsterke T-celleresponser. I ett aspekt induserer antistoffet IFN-y-produksjon i kostimulerende analyser, men påvirker ikke bindingen av H4-1BB til dens korresponderende ligand, H4-1BBL og binder ikke komplement.
Antistoffene ifølge oppfinnelsen kan produseres ved fremgangsmåter velkjente på området. I ett aspekt kan antistoffene fremstilles ved ekspresjon i transfekterte celler, slik som immortaliserte eukaryote celler, slik som myelom eller hybridomceller.
Antistoffene ifølge oppfinnelsen kan anvendes alene eller sammen med andre terapeutiske midler slik som radioterapi (omfattende stråling), hormonbehandling, cytotoksiske midler, vaksiner og andre immunmodulerende midler, slik som cytokiner og biologiske responsmodifikatorer. Disse midlene er spesielt anvendelige for behandling av kreft og immunproliferative lidelser.
I ett aspekt tilveiebringer foreliggende oppfinnelse det monoklonale antistoffet (mab) 20H4,9-IgG4. FIG. 1 og 2 tilveiebringer plasmidkart av pD17-20H4,9.h4a og pD16 gate-20H4,9.LC, henholdsvis, som kan anvendes for å produsere mab 20H4,9-IgG4. Oppfinnelsen omfatter også antistoffer med konservative aminosyresubstitusjoner av aminosyresekvensene av tung og lett kjede vist i SEKV ID NR: 3, 6 og 9 som hovedsakelig ikke har noen effekt på H4-lBB-binding. Konservative substitusjoner omfatter typisk substitusjon av én aminosyre med en annen med lignende egenskaper, f.eks. substitusjoner innen de følgende gruppene: valin, glysin; glysin, alanin; valin, isoleucin, leucin; asparaginsyre, glutaminsyre; asparagin, glutamin; serin, treonin; lysin, arginin; og fenylalanin, tyrosin.
Polynukleotidene som koder for polypeptidene ifølge oppfinnelsen omfatter typisk ytterligere en ekspresjonskontrollsekvens operabelt bundet til de polypeptidkodende sekvensene, omfattende naturlig assosierte eller heterologe promoterregioner. Fortrinnsvis vil ekpresjonskontrollsekvensene være eukaryote promotersystemer i vektorer som er i stand til å transformere eller transfektere eukaryote vertsceller, men kontrollsekvenser for prokaryote verter kan også anvendes. Når vektoren først er innført i en passende vert, opprettholdes verten under betingelser egnet for høynivå-ekspresjon av nukleotidsekvensene og, som ønsket, kan oppsamling og rensning av lett kjede, tung kjede, dimerer av lett/tung kjede eller intakt antistoff, bindende fragmenter eller andre immunglobulinform følge, (se S. Enkeltkjede antistoffer eller minibodies (enkeltkjede antistoffer fusjonert til ett eller flere CH-domener) kan også fremstilles ved sammenføying av nukleinsyresekvenser som koder for VL og VH-regionene beskrevet heri med DNA som koder for en polypeptidlinker.
Prokaryote verter, slik som E. coli og andre mikrober, slik som gjær, kan anvendes for å uttrykke et antistoff ifølge oppfinnelsen. I tillegg til mikroorganismer, kan cellekultur fra mammalsk vev også anvendes for å uttrykke og produsere antistoffene ifølge oppfinnelsen. Eukaryote celler kan være foretrukket, fordi flere egnede vertscellelinjer som er i stand til å utskille intakte immunglobuliner er utviklet omfattende for eksempel CHO (kinesisk hamster ovarie)-cellelinjer, COS (fibroblastcellelinje fra African greene monkey) cellelinjer, HeLa-celler, myelom-cellelinjer og hybridomer. Ekspresjonsvektorer for disse cellene kan omfatte ekspresjonskontrollsekvenser, slik som en promoter eller enhancer og nødvendige prosesseringsinformasjonsseter, slik som ribosombindingsseter, RNA splicingsseter, polyadenyleringsseter og transkripsjonsterminatorsekvenser, alle velkjente på området.
Vektorene inneholdende DNA-segmentene av interesse (feks. tung og/eller lett kjede som koder for sekvenser og ekspresjonskontrollsekvenser) kan overføres inn i vertscellen ved velkjente metoder, som varierer avhengig av type cellulær vert. For eksempel er kalsiumklorid-transfeksjon vanlig anvendt for prokaryote celler, mens kalsiumfosfatbehandling, lipofeksjon eller elektroporering kan anvendes for andre cellulære verter, (se feks. T. Maniatis et al., Molecular Cloning: A Laboratory Manual, Cold Spring Harbor Press (1982)).
Når de først er uttrykt kan antistoffene, deres dimerer, individuelle lette og tunge kjeder eller andre immunglobulinformer, renses i henhold til standardprosedyrer på området, slik som ammoniumsulfat prespitering, affinitetskolonner, kolonnekromatografi og gelelektroforese. Hovedsakelig rene immunglobuliner med minst 90 til 95% homogenitet er ønskelig og 98 til 99% eller mer homogenitet er mer ønskelig.
Antistoffene ifølge oppfinnelsen er anvendelige for å modulere T-celle og antistoffmedierte immunresponser. Typiske sykdomstilstander egnet for behandling omfatter kreft, infeksiøse sykdommer, inflammatoriske sykdommer og autoimmune sykdommer slik som multippel sklerose, revmatoid artritt, lupus erythematosus disseminatus og myaesthenia gravis.
Oppfinnelsen tilveiebringer også farmasøytiske blandinger omfattende minst ett antistoff ifølge oppfinnelsen og en farmasøytisk akseptabel bærer. De farmasøytiske blandingene kan steriliseres ved konvensjonelle velkjente steriliseringsteknikker. De farmasøytiske blandingene kan også inneholde farmasøytisk akseptable hjelpestoffer som nødvendig for tilnærmet fysiologiske betingelser slik som pH-regulerende og buffermidler, midler som øker stabiliteten slik som mannitol eller tween 80 og toksisitetsregulerende midler, for eksempel natriumacetat, natriumklorid, kaliumklorid, kalsiumklorid, natriumlaktat eller humant albumin.
Antistoffene og de farmasøytiske blandingene ifølge oppfinnelsen er spesielt anvendelige for parenteral administrering, omfattende subkutan, intramuskulær og intravenøs administrering. De farmasøytiske blandingene for parenteral administrering kan omfatte en løsning av antistoffet oppløst i en akseptabel bærer, fortrinnsvis en vandig bærer. En rekke vandige bærere kan anvendes, alle velkjente på området, f.eks. vann, bufret vann, saltoppløsning og glysin. Disse løsningene er sterile og generelt fri for partikulært materiale. Det er spesielt fordelaktig å formulere parenterale blandinger i doseringsenhetsform for å lette administrering og ensartethet av dosene.
Den farmasøytiske blandingen kan videre omfatte et ytterligere middel for behandling av en sykdom. I ett aspekt omfatter den farmasøytiske sammensetningen et middel for behandling av kreft, en infeksiøs sykdom, inflammatorisk sykdom eller autoimmun sykdom. Antistoffet ifølge oppfinnelsen kan også koadministreres eller administreres adskilt med et ytterligere middel for behandling av en sykdom.
Antistoffene ifølge oppfinnelsen kan anvendes med andre midler for å forbedre immunresponsen til kankrøse celler hos en pasient. I ett aspekt anvendes antistoffet i kombinasjon med et immunogent middel, slik som kankrøse celler, rensede tumorantigener (omfattende rekombinante proteiner, peptider og karbohydratmolekyler) eller celler transfektert med gener som koder for immunstimulerende cytokiner og celleoverflateantigener. I et annet aspekt anvendes antistoffet i kombinasjon med en vaksine slik som for eksempel en tumorcelle vaksine, en DNA-vaksine, en genmodifisert tumorcelle vaksine, slik som GM-CSF-modifisert tumorcelle vaksine, en peptidvaksine eller en antigen-ladet dendrittisk celle vaksine.
Mange eksperimentelle strategier for vaksinasjon mot tumorer er overveid. I én av disse strategiene, fremstilles en vaksine ved anvendelse av autologe eller allogene tumorceller. Disse cellulære vaksinene er vist å være mest effektive når tumorcellene er transdusert til å uttrykke GM-CSF. GM-CSF har blitt vist å være en potent aktivator av antigenpresentasjon for tumorvaksinasjon (Dranoff et al., P.N. A.S., 90:3539-43 (1993); E. Jafee et al., J. Clin. Oncol., 19:145-56 (2001); R Salgia et al., J. Clin. Oncol., 21:624-30
(2003)).
Studiet av genekspresjon og storskala genekspresjonsmønstre i forskjellige tumorer har ført til definisjonen av såkalt tumorspesifikke antigener (S. Rosenberg, Immunity 10:281-7 (1999)). I mange tilfeller er disse tumorspesifikke antigenene differensieringsantigener uttrykt i tumorene og i cellen fra hvilken tumoren oppsto for eksempel melanocytt antigener gp 100, MAGE-antigener, Trp-2. Mange av disse antigenene kan vises å være målene for tumorspesifikke T-celler funnet i verten. Antistoffene ifølge oppfinnelsen kan anvendes i konjunksjon med en samling av rekombinante proteiner og/eller peptider uttrykt i en tumor for å amplifisere og dirigere immunresponsen til disse antigenene mot en Thl-respons. Disse proteinene betraktes vanligvis av immunsystemet som selvantigener og er derfor tolerante i forhold til dem.
I ett aspekt kombineres antistoffet ifølge oppfinnelsen med et immunmodulatorisk middel omfattende SIV gag-antigenet (som en modell for HIV DNA-vaksine) eller prostata spesifikt antigen (PS A) eller en DNA-vaksine omfattende en nukleotidsekvens som koder for SIV gag-antigenet eller prostata spesifikt antigen (PSA). PSA-vaksiner er beskrevet i, for eksempel M. Pavlenko et al., Br. J. Cancer, 91(4):688-94 (2004); J. Wolchok et al., Semin. Oncol, 30(5):659-66 (2003); J. Kim et al, Clin. Tumorantigenet kan også omfatte, for eksempel proteinet telomerase, som er nødvendig for syntese av telomerer av kromosomer og som uttrykkes ved mer enn 85% av human kreft og i kun et begrenset antall somatiske vev (N. Kim et al, Science, 266, 2011-2013 (1994)). Tumorantigen kan også være "neo-antigener" uttrykt i kreftceller på grunn av somatiske mutasjoner som endrer proteinsekvensen eller danner fusjonsproteiner mellom to ubeslektede sekvenser eller idiotype fra B celle-tumorer. Andre tumorvaksiner kan omfatte proteinene fra virus implisert i human kreft slik et humant papillomavirus (HPV), hepatittvirus (HBV og HCV) og Kaposi's herpes sarkom virus (KHSV). En annen form av tumorspesifikt antigen som kan anvendes med et antistoff ifølge oppfinnelsen er rensede varmesjokkproteiner (HSP) isolert fra tumorvevet i seg selv. Disse varmesjokkproteinene inneholder fragmenter av proteiner fra tumorcellene og disse HSPs er svært effektive ved levering til antigenpresenterende celler for å frembringe tumorimmunitet (R. Suot et al, Science 269: 1585-1588 (1995); Y. Tamura et al, Science 278: 117-120(1997)).
Antistoffene ifølge oppfinnelsen kan også anvendes for å forbedre immunresponsen til vaksiner til virale antigener, slik som HIV eller HCV. Antistoffene ifølge oppfinnelsen kan også anvendes for å forbedre immunresponsen til andre immunmodulerende midler og å indusere en hukommelse immunrespons. Eksempler på disse midlene er cytokiner slik som GM-CSF, IL-2, IL-15, IL-12, F13-ligand, CD40-ligand, adjuvans slik som CpG-oligodeoksynukleotid (bakterielt DNA) eller antistoffer mot OX-40 eller CTLA-4.
De farmasøytiske blandingene ifølge oppfinnelsen kan administreres for profylaktiske og/eller terapeutiske behandlinger. Ved terapeutisk anvendelse administreres den farmasøytiske blandingen til en pasient som allerede lider av en sykdom, i en mengde tilstrekkelig til å kurere eller i det minste delvis stanse eller behandle sykdommen. En mengde tilstrekkelig til å oppnå dette defineres som en "terapeutisk effektiv dose." Mengder effektive for denne anvendelsen vil avhenge av alvorlighetsgraden av sykdomstilstanden og pasienten (omfattende for eksempel den generelle tilstanden til pasientens eget immunsystem) og kan bestemmes av fagfolk på området. Ved profylaktiske anvendelser administreres den farmasøytiske blandingen til en pasient som ikke allerede i sykdomstilstanden, for å forbedre pasientens resistens mot sykdomstilstanden. En slik mengde er definert som en "profylaktisk effektiv dose". Ved denne anvendelsen avhenger de nøyaktige mengdene av pasientens helsetilstand (omfattende for eksempel den generelle tilstanden til pasientens eget immunsystem) og kan bestemmes av fagfolk på området. Eksempel 1: Fremstilling av antistoffer
Materialer og metoder
Fullstendig humane monoklonale antistoffer mot den humane CD 137 (4-1BB)-reseptoren ble fremstilt i HuMAb-Mouse® (Medarex, Inc., Princeton, New Jersey). HuMAb-mus ble immunisert fem ganger intraperitonealt (i.p.) og subkutant (s.c.) med 25 Hg av det ekstracellulære domenet av human CD137 i RIBI-adjuvans (Ribi Immunochemical). Før fusjon, ble mus boostet intravenøst (i.v.) med samme mengde antigen. Miltceller fra immuniserte mus med tilstrekkelig titere av antistoffer mot huCD137 ble fusjonert med myelomceller fra mus etter standardmetoder.
Anti-human CD3 mab (klon:HIT3a), ELISA kit for human og ape IFN-y, cytometric bead array (CBA)-kit og alle konjugerte antistoffer for flowcytometri ble anskaffet fra BD Pharmingen (San Diego, California). Human IgGi A. og Human IgGi k ble anskaffet fra Sigma-Aldrich (St. Louis, Missouri). CEM-celler (ATCC-CRL 2265) ble anskaffet fra ATCC. Dyrkningsmedier (RPMI), og føtalt bovint serum (FBS) ble anskaffet fra Mediatech Inc. (Herndon, Virginia). Sheep Reed Blood Alsevers ble anskaffet fra Colorado Serum Co. (Denver, Colorado).
Screening av hybridom: Deteksjon av binding til huCD137 ved ELISA: For å identifisere hybridomer som skiller ut anti-human CD137-antistoffer, ble ELISA-plater (Nunc MaxiSorp) belagt med human CD137-mus IgGrebfusjonsprotein ved 1 (xg/ml i PBS over natten ved 4°C. Plater ble deretter vasket 3 ganger med PBS med 0,01% Tween-80 (PBS-T) og deretter blokkert med PBS-T samt 1% bovint serumalbumin (BSA), i 20 min ved romtemperatur. Femti mikroliter supernatanter fortynnet 1:3 i PBS-T ble tilsatt til platene og inkubert i 1-2 timer ved omgivelsestemperatur. Deretter ble plater vasket som før og binding av antistoffer ble detektert ved en inkubering med alkalisk fosfatase-konjugert geit F(ab')2anti-humant IgG antistoff (Jackson Laboratories, West Grove, Pennsylvania). Plater ble utviklet med pNPP og avlest ved 405 nm.
Blokkeringsanalyse: Tjueseks hybridomer som sekreterte antistoffer som gjenkjente huCD137 ved ELISA ble evaluert for deres evne til å tillate CD137-CD137L-interaksjoner. Disse analysene ble utført initielt i et ELISA-format. Plater ble belagt med human CD137-muIgG2bved 0,2 ug/ml, 100 ul/brønn. Seriefortynninger av mab 20H4,9-IgGl eller kontrollantistoffer, fortynnet i PBS-T og 1% bovint serumalbumin, ble tilsatt til platen. Fusjonsprotein CD137L-CD8 ble tilsatt til brønnene i en konsentrasjon på 0,2Hg/ml. Binding av antistoffer ble detektert med et biotinylert anti-CD8-antistoff (0,2Hg/ml, Ancell Corporation, Bayport, Minnesota). Etter mange vaskinger ble streptavidin-alkalisk fosfatase (1:2000) og pNPP for deteksjon av bundne antistoffer tilsatt og platene ble avlest ved 405 nm.
For å bekrefte at de selekterte antistoffene ikke endret CD137-CD137L-binding, ble rensede antistoffer ytterligerekarakterisert vedBIAcore-analyser. Alle eksperimenter ble utført i et BIAcore 3000 instrument (BIAcore Inc., Piscataway, New Jersey). Human CD 137 ble immobilisert kovalent ved en høy tetthet på en karboksymetylert dekstranoverflate på en BIAcore sensorchip (BIAcore Inc., Piscataway, New Jersey). Injeksjoner ble utført ved 2 (xg/ml i 10 mM acetatbuffer, pH 5,0. Ikke opptatte aktive estere ble deretter blokkert ved injeksjon av et overskudd av etanolamin. Regenerering av overflaten ble utført med 10 mM glysin, pH 2,0.
Rensede prøver av anti-CD137 antistoffer ble fortynnet til konsentrasjoner mellom 200 og 1000 nM ved anvendelse av HEPES bufret saltoppløsning, pH 7,4, supplert med 0,15 M NaCl og 0,005% surfaktant P20 (HBS-EP). Humane CD137L-CD8 fusjonsproteiner (huCD137L) ble anvendt som kilde for CD137-ligand. Det ble gjennomført eksperimenter hvor huCD137L ble injisert før anti-CD137-antistoffer eller omvendt. Injeksjoner ble utført ved en strømningshastighet på 5 ul/min. Bundet ligand og antistoffer ble fjernet ved regenerering med 10 mM glysinbuffer, pH 2,0.
Rensing av human T-celle: T-celler eller PBMC er ble oppnådd fra friske humane donorer. Blod ble oppsamlet i EDTA, oppslemmet i slemmingsbuffer (RPMI inneholdende 2,5 mM EDTA, 10 ug/ml polymyksin B), støttet med Lymphocyte Separation Medium (LSM, Mediatech Inc., Herndon, Virginia) og sentrifugert ved 1800 rpm i 25 minutter. Cellulære grenseflater ble oppsamlet og sentrifugert ved 1500 rpm i 10 minutter. Deretter ble cellepellet resuspendert i slemming buffer og vaskede Sheep Red Blood Cells (SRBC, 1:10 fortynning) og inkubert på is i 1 time. Celler ble deretter støttet med LSM og sentrifugert ved 2500 rpm i 25 minutter. Grenseflater ble fjernet og SRBC ble lysert med SRBC Lysisbuffer. Isolerte T-celler ble vasket og resuspendert i 10% FBS/RPMI.
Flowcytometriske analyser: Binding av anti-humane CD137 antistoffer til CD137 uttrykt på celler ble bestemt ved flowcytometri. En human T-celleleukemi-cellelinje (CEM) eller cynomolgus-ape perifert blod monocytiske celler (PBMC) ble anvendt i disse undersøkelsene. Disse cellene uttrykker ikke CD137 konstitutivt, men reseptoren kan induseres ved stimulering med forbol myristat (PMA, 10 ng/ml) og ionomycin (1 uM) i 18 timer. Celler ble deretter vasket og inkubert med forskjellige konsentrasjoner av antistoffene i merkingsbuffer (fosfatbuffer saltoppløsning, PBS, samt 1 % FCS og 0,01 % natriumazid). Binding av antistoffene til stimulerte eller ikke-stimulerte celler ble detektert ved et fluorescein (FITC) eller phycoerithrin (PE) konjugert geit anti-humant IgG (Jackson Immunoresearch, West Grove, Pennsylvania). For å bekrefte ekspresjon av CD137 ble det anvendt et fusjonsprotein bestående av det ekstracellulære domenet av CD137-ligand og mus CD8 (Ancell Corporation, Bayport, Minnesota), etterfulgt av inkubering med PE-konjugert anti-mus CD8 (BD Pharmingen, San Diego, California). Prøver ble fiksert i 1% formalin, oppbevart ved 4 °C og avlest ved flowcytometri.
Funksjonell analyse: Primære humane T-celler eller ape PBMC oppnådd fra friske donorer ble stimulert med immobilisert anti-CD3 antistoff for å gi det første signalet for T-celleaktivering, og kostimulert med humane anti-human CD137-antistoffer. Som en uspesifikk kontroll, ble et humanisert anti-karsinom antistoff (BR96) anvendt ved samme antistoffkonsentrasjon. Plater ble belagt med anti-CD3 antistoff (0,5-1 (xg/ml) ved 4 °C over natten. Neste dag ble T-celler eller PBMC platet ut ved konsentrasjoner på 1-1,5 xlO<5>/brønn. Syntese av IFN-y ble målt etter 72 timers dyrking ved 37 °C enten ved cytometric bead array (CBA) eller ved ELISA.
Cytokinanalyser
ELISA: Etter stimulering av T-celler ved forskjellige tidspunkter, ble plater sentrifugert og media ble fjernet. Cytokinnivåer ble detektert ved en ELISA i henhold til produsentens instruksjoner (BD Pharmingen, San Diego, California). I korthet ble test-prøver og standarder tilsatt til anti-cytokin-belagte 96-brønners plater. Etter inkubering i 2 timer ved omgivelsestemperatur, ble plater vasket 3 ganger i PBS-T og deretter inkubert først med et anvendelig detektor antistoff, etterfulgt av tilsetning av substrat. Absorbans ble avlest ved 405 nm og konsentrasjoner ble beregnet basert på standardkurven.
Cytometric Bead Array: En annen metode anvendt for å bestemme cytokinproduksjon in vitro var flowcytometri som anvender Cytometric Bead array (CBA) utviklet av BD Pharmingen. Nivåer av IFN-y, IL-2, IL-5, IL-4, IL-10 og TNF-oc ble målt i kultursupernatanter etter produsenters instruksjoner. Resultater ble analysert ved strømningscytometri med CBA analyse-programvaren.
Resultater
Hybridomer som sekreterer antistoffer som oppviste binding til human CD 137 ble videre ekspandert og subklonet. Utskilte antistoffer ble renset og testet for deres evne til å binde til huCD137 og til å muliggjøre interaksjonen av CD137-CD137L. Fra panelet av anti-humane CD 137 antistoffer evaluert, ble mab 20H4,9-IgGl valgt for videre evaluering basert på dets bindingsprofil og ikke-blokkerende egenskaper. Antistoffet 20H4,9-IgGl er IgGi kappa som bestemt ved ELISA ved anvendelse av alkalisk fosfatase anti-human IgGi, 2, 3, 4 og anti-kappa og lambda-reagenser (Southern Biotech, Birmingham, Alabama). FIG. 8 (FIG. 8A - binding til human CD137 ved ELISA; FIG. 8B - effekt av mab 20H4,9-IgGl på CD137-CD137L-interaksjon) tilveiebringer den innledende karakteriseringen av mab 20H4,9-IgGl. Seriefortynninger av mab 20H4,9-IgGl, 26 G6 (et blokkerende anti-CD137 antistoff) eller tetanustoksoid (TT, negativ kontroll) ble evaluert for deres evne til å endre binding av CD 137 til CD137L. Mab 20H4,9-IgGl i konsentrasjoner opptil 10 ug/ml blokkerte ikke CD 137L-binding, mens mab 26 G6 hemmet binding i konsentrasjoner > 0,37 ug/ml.
Mab 20H4,9-IgGl ble også testet for reaktivitet mot CD 137 uttrykt på humane T-celler (CEM) og i perifert blod monocytiske celler fra cynomolgus-ape (PBMC) stimulert med PMA og ionomycin. Tidligere undersøkelser bestemte at CD 137 oppreguleres på T-celler etter aktivering med PMA og ionomycin. Kontrollmolekyler besto av et irrelevant human IgG antistoff (negativ kontroll) eller CD137L-CD8 fusjonsprotein (positiv kontroll, BD Pharmingen, San Diego, California). Resultater fra disse undersøkelsene indikerte at mab 20H4,9-IgGl bandt til aktiverte humane CEM og PBMC er fra cynomolgus-aper, med minimum binding til ustimulerte celler. Lignende prosentdeler av positive celler ble detektert med enten mab 20H4,9-IgGl eller CD137L. FIG. 9 gir resultatene oppnådd som viser bindingen av mab 20H4,9-IgGl til PMA-ionomycin-stimulerte humane eller cynomolgus-ape-celler. Human CEM (FIG. 9A) eller ape PBMC (FIG. 9B) ble inkubert med 20H4,9-IgGl eller humant CD137L fusjonsprotein. Sekundære antistoffer ble tilsatt og prøver ble avlest ved flowcytometri.
Deretter ble det bestemt hvorvidt mab 20H4,9-IgGl kunne indusere forbedring av IFN-y i kostimulerende analyser i nærvær av anti-CD3-stimulering, den viktigste funksjonelle effekten ønsket for et agonistisk CD137-antistoff. Mab 20H4,9-IgGl ble evaluert for dets kostimulerende aktivitet i funksjonelle studier i humane og ape-lymfocytter. Basert på initielle data, ble en konsentrasjon på 20 ug/ml anti-CD137-antistoff (overskudd av antistoff) anvendt i disse undersøkelsene. Nivåer av anti-CD3 antistoff på mellom 0,2-1 (xg/ml ble testet, hvilket resulterte i 10-20% CD137-positive lymfocytter. Nivåer av IFN-y i supernatanter ble målt etter 72 timers dyrking. Som vist i FIG. 10, forbedret mab 20H4,9-IgGl IFN-y-syntese både i humane og ape kostimulerende analyser til nivåer betydelig høyere enn kontroller. Resultater av undersøkelser utført med T-celler isolert fra 8 friske humane donorer viste at i seks av dem, økte mab 20H4,9-IgGl syntese av IFN-y mellom 2,2 - 4,3 ganger sammenlignet med kontroller. En av de andre to donorene oppviste en 1,6 gangers økning. Nivået av forsterkning var bedre enn det observert med hu39E3.G4, et humanisert anti-CD137-antistoff gitt i publisert PCT-søknad WO04/010947 som viste økning av IFN-y i 5 av 8 donorer og ved nivåer lavere enn mab 20H4,9-IgGl (1,5 - 2-gangers økning) (FIG. 10A). I ape kostimulerende studier, viste også mab 20H4,9-IgGl forbedret funksjonell aktivitet hvilket resulterer i betydelig økning av IFN-y over kontroller (FIG. 10B). Som i de humane studiene, var forøkning av IFN-y gjennomført høyere enn med hu39E3.G4.
Induksjon av TNF-oc-syntese over kontrollnivåer ble også observert i humane kulturer, om enn ved mye lavere nivåer enn IFN-y. TNF-oc nivåer indusert ved anti-CD3 antistoff alene (baseline) var ca. 20-50 ganger lavere enn baselinenivåer for IFN-y. Mab 20H4,9-IgGl induserte en økning på~2 til 4,7-ganger i 3 av 8 donorer. Igjen induserte hu39E3.G4 (testet parallelt)~2-ganger økning i de samme donorene men ved lavere nivåer. Andre cytokiner testet, IL-2, IL-5, IL-10 og IL-4 ble ikke betydelig endret ved noen av behandlingene.
Samlet viste disse undersøkelsene at mab 20H4,9-IgGl oppviste den funksjonelle aktiviteten ønsket hos både mennesker og aper ved å indusere en Thl-type respons. Siden in vivo anti-tumoraktivitet er forbundet med evnen av anti-CD137-antistoffer til å indusere IFN-y-syntese, støttet disse resultatene, betegnende nok, valget av mab 20H4,9-IgGl for isotypes witching.
Eksempel 2: In v/7/*o-karakterisering av mab 20H4,9-IgG4
Basert på dets bindingskinetikk, manglende evne til å blokkere CD137-CD137L-interaksjon og funksjonelle effekter på humane T-celler, ble mab 20H4,9-IgGl valgt for switching til en IgG4-form. IgG4-formen av mab 20H4,9-IgGl er 20H4,9-IgG4 (vist i FIG.
3 og 4).
Den andre fasen av disse studiene omfattet sammenligning av in vzZro-egenskapene til mab 20H4,9-IgG4 og mab 20H4,9-IgGl. I dette avsnittet er bindingskinetiske egenskaper og funksjonelle effekter av begge antistoffene i humane og ape-lymfocytter beskrevet.
Bindingskinetikk
Kinetiske egenskaper av anti-human CD137-antistoffer ble evaluert ved overflate plasmonresonans ved anvendelse av et BIAcore 3000-instrument. Antigenet, humant CD137-murin IgG2a, ble immobilisert kovalent ved lav tetthet på overflaten av en CM5 sensorchip. Mab 20H4,9-IgG4 og mab 20H4,9-IgGl ble injisert i konsentrasjoner på mellom 25 og 200 nM. FIG. 11 viser injeksjoner ved 100 nM for både mab 20H4,9-IgGl og mab 20H4,9-IgG4. Data beregnet ved anvendelse av BIAevaluation programvare (bivalent modell, global kurvetilpasning analyse) resulterte i kinetiske parametere som var lik for begge antistoffene (se Tabell 1). Dissosiasjonskonstanter Kdfor mab 20H4,9-IgGl og mab 20H4,9-IgG4 ble bestemt som 11,2 og 16,6 nM, henholdsvis. Under lignende eksperimentelle betingelser, bandt ikke mab 20H4,9-IgG4 til murin 4-IBB.
Flowcytometriske analyser
Biotinylert mab 20H4,9-IgG4 ved konsentrasjoner i området fra 0,32 ng/ml til 5 ug/ml ble testet for binding til CEM-celler ± PMA-ionomycin. Mab 20H4,9-IgG4 bundet til PMA-ionomycin stimulerte CEM-celler på en konsentrasjonsavhengig måte. Metning ble oppnådd ved 0,2 ug/ml. På den annen side, som vist for dets parentale molekyl mab 20H4,9-IgGl, bandt ikke mab 20H4,9-IgG4 til CEM-celler som ikke var stimulert med PMA-ionomycin (FIG. 12). Konsentrasjonsavhengig binding av mab 20H4-,9-IgG4 ble vist i PMA-ionomycin-stimulerte CEM-celler (FIG. 12). Prøver ble avlest ved flowcytometri.
Cellulære/Funksjonelle analyser
For å bekrefte at fremgangsmåten med switching av isotypen av mab 20H4,9-IgGl ikke endret aktiviteten av antistoffet, ble in v/Yro-undersøkelser utført for å sammenligne aktiviteten av mab 20H4,9-IgG4 med den parentale mab 20H4,9-IgGl i ape PBMC og humane T-celler. De funksjonelle effektene av mab 20H4,9-IgG4 på humane og ape T-celler eller PBMC ble bestemt og sammenlignet med dets parentale molekyl, mab 20H4,9-IgGl. Primære humane T-celler eller ape PBMC oppnådd fra friske donorer ble stimulert med anti-CD3 -antistoff (0,5 ug/ml-1 (xg/ml) +/- anti-human CD137-antistoffer. Syntese av IFN-y ble målt etter 72 timers dyrking ved 37 °C ved cytometric bead array (CBA) for humane prøver eller ved ELISA for ape-prøver. Antistoffer ble testet i kostimulerende analyser i nærvær av suboptimale konsentrasjoner av anti-CD3-antistoff (1 ug/ml) eller Concavalin A (1 ug/ml) (donorer M5170 og 81 kun). Resultater er uttrykt som ganger økning i pg/ml over kontroller. På grunn av den variable bakgrunnsresponsen blant donorer, ble data normalisert relativt til kontrollbehandlinger (=1). FIG. 13A tilveiebringer de humane T-celleresultatene og FIG. 13B gir resultatene fra ape-PBMC. Som vist i FIG. 13A-13B, viste mab 20H4,9-IgG4 kostimulerende egenskaper hvilket gir høyere nivåer av IFN-y i humane og ape-celler sammenlignet med kontroller. Nivået av forbedring av IFN-y-syntese var komparabel med dets parentale molekyl i humane og ape-prøver.
Deretter ble effekten av kryssbinding av antistoff på den funksjonelle effekten av mab 20H4,9-IgG4 evaluert. Det har blitt vist at kryssbinding av antistoffer kan resultere i potensiering av deres evne til signalering. Følgelig ble det utført en undersøkelse for å bestemme den funksjonelle aktiviteten til mange batcher av mab 20H4,9-IgG4 ± et anti-humant IgG-antistoff Som vist i FIG. 14A, ble det observert betydelig forbedring av IFN-y-syntese for alle lots testet i fravær av kryssbindende antistoffer, med et platå ved konsentrasjoner på 400 ng/ml. Økningen av IFNy-syntese ved mab 20H4,9-IgG4 ble ytterligere forbedret ved tilsetning av anti-human IgG kryssbindende antistoff som vist i FIG. 14B. Forskjellige batcher av mab 20H4,9-IgG4 hadde komparable cellulære aktiviteter.
Følgelig førte kryssbinding av mab 20H4,9-IgG4 til en forbedring av antistoffets evne til å indusere IFN-y-syntese. Kryssbinding av antistoff in vivo kan forekomme ved cellulære reseptorer for Fc-delen av immunglobuliner eller ved dimerisering av antistoff. Mab 20H4,9-IgG4 er av IgG4-isotypen, som, sammenlignet med andre IgG-isotyper, har lav affinitet for Fc-reseptorer. Imidlertid kan IgG4 binde til FcyRI (CD64) uttrykt på monocytter og nøytrofiler.
To andre metoder ble anvendt for å ytterligere karakterisere mab 20H4,9-IgG4: (i) effekt på T-celleoverlevelse og (ii) effekt på cyklin D2-ekspresjon. For å bestemme hvorvidt mab 20H4,9-IgG4 kunne frembringe signalering gjennom CD 137 på humane T-celler og gi kostimulerende signaler til T-celler som fører til celleoverlevelse og ekspansjon, ble humane T-celler stimulert med anti-CD3 antistoffer +/- mab 20H4,9-IgG4 i konsentrasjoner kjent å indusere IFN-y-syntese merket med annexin-V og propidiumjodid for å bestemme antall levende celler (Annexin V/Propidiumjodid negativ) og med Cyklin D2 for å bestemme dens effekt på celleprogresjon. FIG. 15 viser de gjennomsnittlige resultatene for 4 forskjellig lots av mab 20H4,9-IgG4 på cyklin D2-ekspresjon og overlevelse av T-celler. Konsentrasjoner av mab 20H4,9-IgG4 på 0,4-10 ug/ml resulterte i en økning i antallet levende celler ved omtrent 1,8-2 ganger og ga en betydelig økning i antallet cyclin D2-uttrykkende T-celler (2,5 - 3 ganger).
Eksempel 3: In vivo-evaluering av 4-lBB-antistoffer i en farmakodynamisk modell i cynomolgus-aper.
Dette eksemplet illustrerer evnen til mab 20H4,9-IgG4 og mab hu39E3.G4 til å forbedre den antigenspesifikke immunresponsen fremkalt ved DNA-vaksiner.
Materialer og Metoder
Eksperimentelle dyregrupper: Hunn og hann cynomolgus-aper (2,5 til 5,0 kg) ble anskaffet fra Charles River BRF (Houston, Texas) for dette studiet og ble oppstallet i par. Hver eksperimentell gruppe besto av 4 hanner og 2 hunner som ble randomisert inn i grupper med hensyn til kroppsvekt. Eksperimentelle grupper var som følger: Gruppe 1 - SIV gag og PSA DNA-vaksine (2 mg hver), dag 0, 28, 56, i.m., samt kontroll med saltoppløsning, i.v., ved dagene 12, 15 og 19;
Gruppe 2 - SIV gag og PSA DNA-vaksine (2 mg hver), dag 0, 28, 56, i.m., samt mab hu39E3.G4, i.v., ved dagene 12, 15 og 19;
Gruppe 3 - SIV gag og PSA DNA-vaksine (2 mg hver), dag 0, 28, 56, i.m., samt mab 20H4,9-IgG4, i.v., ved dagene 12, 15 og 19;
Gruppe 4 - ubehandlet kontrollgruppe.
Immuniseringer og antistoffbehandlinger: PSA og SIV gag DNA-vaksineer ble oppnådd fra David B. Weiner, Department of Pathology and Laboratory of Medicine, University of Pennsylvania, (se Kim et al., Oncogene 20, 4497-4506 (2001); Muthumani et al., Vaccine 21, 629-637 (2003).)
Aper ble immunisert ved den intramuskulære ruten med både PSA og SIV gag DNA-konstruksjoner (2 mg/konstruksjon/immunisering) samtidig, fulgt av to booster med 4 ukers mellomrom (dagene 0, 28 og 56). Tolv dager etter den initielle immuniseringen, ble behandling med mab 20H4,9-IgG4 eller mab hu39E3.G4 initiert. Antistoffer ble administrert i. v, ved 50 mg/kg, ved dagene 12, 15 og 19 etter den første immuniseringen. Denne planen ble valgt fordi den ble vist å undertrykke antistoffresponsen til mab hu39E3.G4.
Klinisk og klinisk patologi
Gjennom hele varigheten av studiet, ble det foretatt fysiske undersøkelser av alle apene ved de ansvarlige veterinærene. Blodprøver for hematologi og serumkjemi-analyser ble oppsamlet før vaksinasjoner og deretter 12, 42, 70, 97, 134 og 168 dager etter immuniseringer.
Immunologiske analyser
For å bestemme effekten på immunresponsene indusert ved disse terapeutiske
regimene, ble en enzyme-linked immunospot-analyse (ELISPOT) anvendt for deteksjon av IFN-y-produksjon ved antigen-spesifikk stimulerte lymfocytter. Blodprøver for ELISPOT-analyser ble oppsamlet før vaksinasjoner og deretter 12, 42, 70, 97, 134 og 168 dager etter immuniseringer. Syntetiske peptider svarende til de fullstendige sekvensene av SIV gag og PSA-antigenet ble anvendt for ex vivo stimulering av PBMC.
Resultater
Antigen-spesifikk IFN-y-sekreterende celler i respons til PSA eller SIV gag-peptider ble kvantifisert ved ELISPOT. FIG. 16 (FIG. 16A-16D) viser resultatene oppnådd fra Gruppene 1-4, henholdsvis. Nivået av respons til PSA var svært lavt i alle grupper, hvilket indikerer at vaksinen alene ikke induserte en målbar og konsistent immunrespons sammenlignet med ikke-vaksinerte dyr. På den annen side førte SIV gag-vaksinasjon alene til et betydelig antall antigenspesifikke IFN-Y-utskillende celler som økte over tid (FIG. 16A). Ubehandlede dyr (ikke vaksinert) viste 100-1,000 spots/10<6>PBMC gjennom hele forløpet av undersøkelsen (FIG. 16D). Disse resultatene etablerte startresponsen til vaksinen; dyr som fremviste < 1,000 spots/10<6>PBMC ble betraktet som ikke-respondere. I gruppen dyr som mottok vaksine, viste 5 av 6 aper en øket respons over tid, med et gjennomsnittlig antall spots etter den tredje immuniseringen (dag 70) på 1,727 spots/10<6>PBMC (SD=242, område=1,403-1,968 spots/10<6>PBMC). En ape ble betraktet som en ikke-responder (620 spots/million PBMC). Siden MHC typebestemmelse ikke ble utført i disse studiene, er det sannsynlig at mangelen på T-celleresponser til vaksinen ved noen aper kan skyldes MHC-mismatch. Bemerkelsesverdig nok fremviste, ved dag 70, 4 av 6 dyr behandlet med SIV gag samt mab 20H4,9-IgG4 et betydelig høyere antall IFN-y spots (FIG. 16C) sammenlignet med kontrolldyr (FIG. 16D) og med makaker som var immunisert med DNA-vaksine alene (FIG. 16A). Gjennomsnittlig antall spots etter den tredje immuniseringen for den mab 20H4,9-IgG4-behandlede gruppen var 3,465 spots/10<6>PBMC (SD=1,236, område=2,070-4,780 spots/10<6>PBMC). To aper i den gruppen responderte ikke til vaksinen (< 800 spots/million PBMC). Etter den tredje immuniseringen (dag 70), førte behandling med mab hu39E3.G4 samt DNA-vaksine til at 6 av 6 dyr ble ansett som respondere med et gjennomsnittlig antall spots/10<6>PBMC på 2,348 (SD=588, område=l,738-3,283) (FIG. 16B). For denne gruppen var området av antall spots mindre sammenlignet med de makakene som ble behandlet med mab 20H4,9-IgG4.
Behandling med både mab 20H4,9-IgG4 og mab hu9E3.G4 var veltolerert og førte ikke til noen betydelige endringer i kliniske tegn, klinisk kjemi eller hematologiske parametere relativt til kontroll-aper.
Disse dataene viser at mab 20H4,9-IgG4-behandling i kombinasjon med en DNA-vaksine fremkalte en in vzvo-forbedring av størrelsen av den spesifikke cellulære responsen til det testede antigenet relativt til kontroller eller til behandling med mab hu39E3.G4, som målt ved antigenspesifikke IFN-y-utskillende celler. Siden kun ett dosenivå av antistoffene og ett doseringsregime ble anvendt i disse preliminære undersøkelsene, er det ikke sannsynlig at maksimale responser ble indusert og det er nødvendig med videre arbeid for å optimalisere betingelser. Det er imidlertid klart at selv med denne ikke-optimaliserte fremgangsmåten ble det oppnådd en forbedring av den cellulære responsen til test- antigener med mab 20H4,9-IgG4, hvilket indikerer at modulering av CD137-funksjon kan være en attraktiv metode for å øke effektiviteten av DNA-vaksiner.
Selv om oppfinnelsen er beskrevet i noen detaljer for å illustrere og eksemplifisere med det formål å oppnå klarhet og forståelse, vil det være klart at visse endringer og modifikasjoner kan utføres innenfor omfanget av de vedlagte kravene.
