Abstract
본 개시는, 경구 즉시 방출의 고체 약제학적 제형 및 이의 제조 방법에 관한 것이다. 본 개시는 또한, 부분적으로는 아포리포단백질 A-I(ApoA-I)의 발현을 조절하기 위한 약제학적 제형의 사용 방법 및 죽상동맥경화증과 같은 콜레스테롤- 또는 지질-관련 장애를 비롯한 심혈관 질환 및 관련 질환 상태를 치료 및 예방하기 위한 상기 제형의 용도의 제공에 관한 것이다.
Description
고밀도 리포단백질 콜레스테롤(HDL-C)의 순환 수준(circulating level)과 임상적으로 유의한 죽상동맥경화증의 발생률과의 사이에 역관계(inverse relationship)가 있는 것으로 역학 자료는 입증하고 있다. HDL-C 혈청 수준의 매 1 mg/dl씩의 증가는 심장혈관 위험성의 2 내지 3% 감소와 관련되고; LDL-C의 1% 감소는 관상 심장 질환(CHD: coronary heart disease)의 위험성을 2% 만큼 감소시킨다[참조: Gordon et al. (1997) Am . J. Med. 62, 707-714]. 심혈관 질환에 대한 HDL-C의 보호 효과를 실험적 증거가 추가로 지지한다. 예를 들면, 낮은 HDL-C를 가진 대상에서, 젬피브로질(gemfibrozil)의 투여로 HDL-C 수준이 6% 증가하고 상응하는 CHD의 위험이 22% 감소한다[참조: Rubins et al. (1999) N. Engl . J. HDL-C는 콜레스테롤이 말초 조직으로부터 모여서 간으로 수송되는 역 콜레스테롤 수송(RCT: reverse cholesterol transport)을 매개함으로써 이의 항-죽상생성 효과(anti-atherogenic effect)를 발휘하는 것처럼 보인다. 또한, HDL-C는 또한 소염 및 항산화 효과를 보여 주고, 섬유소용해를 촉진시킨다. 암 및 심혈관 질환을 포함하는 여러 질환의 예방에 기여하는 것으로 생각되는 화합물 중 한 부류는 폴리페놀이다. 폴리페놀은 대부분 식물 기원의 식품 및 음료에 존재하며 가장 풍부한 식이성 항산화물질이다[참조: Scalbert & Williamson (2000) J. Nutr. 130, 2073S-2085S]. 그러나, 폴리페놀의 보호 성질들은 불량한 생체이용률[참조: Manach et al. (2005) Am . J. Clin . Nutr. 81, 230S-242S], 폴리페놀 평가에 대한 여러 보고된 연구에서 임상적 유의성 부족[참조: Williamson & Manach (2005) Am . J. Clin . Nutr. 81, 243S-255S] 및 더 높은 투여 농도에서의 유해 효과 때문에 완전히 실현되지 못하였다. 예를 들어, 널리 공지된 스틸벤 폴리페놀인 레스베라트롤의 풍부하고 이용가능한 원료는 적포도주이다[참조: Wu et al. (2001) Int . J. Mol . 다양한 식품 또는 음료에서의 다양한 폴리페놀의 항산화 효과와 관련된 사람에 대한 수 개의 임상 연구에서 산화 스트레스(oxidative stress), 지질혈증 및 염증과 같은 주요 임상적 종점과 관련된 분명한 이점을 증명하는데 실패하였다[참조: Williamson & Manach (2005) Am . J. Clin . Nutr. 81, 243S-255S]. 예를 들어, 상이한 폴리페놀 원료를 사용한 최근의 12개의 중재 연구(intervention study) 중에서, 6개의 연구에서는 지질 변수에 대해 어떠한 효과도 나타내지 않았고, 6개의 연구에서는 지질 변수의 개선을 나타내었다[참조: Manach (2005) Curr . Opin . Lipidol. 16, 77-84]. 폴리페놀의 많은 유리한 성질들에도 불구하고 상기와 같은 결론을 내릴 수 없는 자료 때문에 폴리페놀의 잠재적 용도가 제한되어 왔다.
잠재적 치료제로서 자연 발생 폴리페놀의 사용은 또한, 부분적으로는 불량한 생체이용률 때문에 신체 내에서 효과적인 수준을 달성할 수 없어서 지연되어 왔다[참조: Manach et al. (2005) Am . J. Clin . 실시예
본원에 인용에 의해 포함된 미국 특허 출원 제11/670,238호 및 제12/490,877호에 기술된 합성 방법들에 따라 2-(4-(2-하이드록시에톡시)-3,5-디메틸페닐)-5,7-디메톡시퀴나졸린-4(3H)-온(화합물 1)을 제조하였다.
임의의 적합한 장치 또는 절차를 사용하여 본 발명의 제형을 함유한 캡슐을 제조할 수 있다. 전형적으로, 활성 약제학적 성분 및 선택적으로 나트륨 전분 글리콜레이트의 적절한 양을 칭량하고, V-블렌더 또는 빈(bin)-블렌더로 옮겨서, 예를 들어, 약 2분 동안 약 25 rpm에서 블렌딩한다. 콜로이드성 이산화규소 및 미세결정성 셀룰로즈와 같은 충전제/희석제의 목적하는 양의 대략 1/3을 스크리닝하여 상기 V-블렌더에 첨가하고, 상기 성분들을 약 2분 동안 약 25 rpm에서 블렌딩한다. 미세결정성 셀룰로즈와 같은 잔류 충전제/희석제를 상기 V-블렌더에 첨가하고, 상기 성분들을 약 4분 동안 약 25 rpm에서 블렌딩한다.
스테아르산마그네슘과 같은 윤활제를 30 메시 스크린을 통해 스크리닝하고 다른 성분들을 함유한 V-블렌더로 옮긴다. 최종 제형을 약 3분 동안 약 25 rpm에서 블렌딩한다.
캡슐 쉘로부터 제형 블렌드를 이형 및 분산시키기 위해 캡슐을 터트림으로써 확인될 수 있는 바와 같이, 용해 시험을 실시하면서 캡슐의 붕해를 처음 5분 동안 육안으로 모니터링하였다. 0.1 N HCl 및 37℃에서 50 및/또는 75 rpm으로 USP 패들 타입 II 장치에서 용해 시험을 실시하였다. 5, 10, 15, 30, 45, 60 및 90분과 같이 여러 시간 지점에서 제형으로부터 용해 매질 중으로 방출된 API를 샘플링함으로써 제형의 용해 프로파일을 결정하였다. 샘플들의 약물 함량에 대해 HPLC로 분석하고 용해 프로파일을 얻었다. 이러한 실험에서, 용해 프로파일의 상부 임계치에는 75 rpm 패들 속도에서 30분 이하의 기간 내에 방출된 85% 초과의 약물을 나타내는 것들이 포함되었다. 유사하게 수행되는 제형의 용해 성능과 구별하기 위해 더 낮은 패들 속도(50 rpm)를 사용하였다.
예를 들어, 부형제의 수, 블렌드의 밀도, 안정성 및 확장성(scaleability)과 같은 요소들을 고려하여, 많은 제형을 다양한 API 중량%에서 제조하였다. 하기 제형들은 더 높은 약물 충전량 수준 및 더 높은 밀도를 제공하여 생산성을 증가시키고 비활성 성분의 대상에의 노출을 감소시켰다. 또한, 높은 수준의 활주제(예: 이산화규소)와 둘 이상의 붕해제와의 배합으로 붕해 및 용해 프로파일이 개선되었다.

상기 제형 중에서, D4가 가장 적은 불활성 성분을 가졌으며, 따라서 약물 충전량 및 밀도가 가장 높은 수준이었으며, 이에 의해 불활성 성분에 대한 불필요한노출이 감소되었다. 상기 제형들의 용해 프로파일은 표 1에 제시되어 있다.
따라서, 본 개시는 부분적으로 화학식 I의 화합물의 생체이용률을 증가시키면서 화합물의 안정성 및 저장 수명을 유지하는 제형을 개발하고자 하는 현재의 문제에 대한 기술적 해결책을 제공한다. ApoA-I의 발현을 조절하는 그리고 BET 억제제로서의 화학식 I의 화합물의 공지된 능력 때문에, 전술된 즉시 방출 제형은 또한, 예를 들어, 대사 증후군, 염증 질환, 알츠하이머병, 죽상동맥경화증, 당뇨병 및 암을 비롯한 심혈관 질환 및 콜레스테롤- 또는 지질-관련 장애를 치료하고 예방하는 방안을 제공한다.
Claims (27)
- (i) 화학식 I의 화합물 또는 이의 약제학적으로 허용되는 염, 입체이성체, 수화물 또는 토토머:
(ii) 하나 이상의 활주제(glidant); 및
(iii) 하나 이상의 붕해제
를 포함하는, 경구 즉시 방출 제형.

R1이 수소인 경우, R3은 알콕시이고;
R3이 수소인 경우, R1은 알콕시이고;
