Abstract
출혈성 합병증을 일으킬 가능성이 낮은 선택적 치료 제제를 제공하는 활성화 단백질 C를 투여하는 것을 포함하는 패혈증, 전격성 자반증, 수막염구균성 패혈증, 골수 및 다른 장기 이식, 심한 화상, 임신, 대수술, 심한 외상 또는 ARDS와 연관된 후천성 응고 항진 상태 또는 후천성 단백질 C 결핍에 걸린 인간 환자의 치료 방법.

Description
본 발명은 특히 활성화 단백질 C를 사용하여 응고 항진 상태 또는 후천성 단백질 C 결핍을 치료하는 의학에 관한 것이다.
단백질 C는 세린 프로테아제로서, 응혈 연속단계에서 인자 Va와 VIIIa를 불활성화하여 트롬빈 생산물의 생성을 차단시키는 능력을 통해 지혈 조절의 작용을 하는 천연 항응혈제이다. 인간 단백질 C는 생체 내에서 461 개 아미노산의 단일 폴리펩티드로서 간에서 주로 생성된다. 이 전구체 분자는 1) 42 아미노산 시그널 서열의 절단; 2) 155 위치의 라이신 잔기와 156 위치의 아르기닌 잔기를 단쇄 자이모겐으로부터 단백분해적으로 제거하여 2 쇄형 분자 (즉, 세린 프로테아제를 함유하는 262 개 아미노산 잔기의 중쇄에 디술피드 다리를 통해 부착된 155 개 아미노산 잔기의 경쇄) 형성; 3) 경쇄의 처음 42 개 아미노산에 분산된 (clustered) 9 개 글루탐산 잔기의 비타민 K-의존성 카르복실화로, 9 개 감마-카르복시글루탐산 잔기의 생성; 및 4) 4 개의 위치에 탄수화물 부착 (경쇄에 1 개, 중쇄에 3 개)을 포함하는 다중 변역후 변형을 거친다. 중쇄는 Asp 257, His 211 및 Ser 360의 잘 확립된 세린 프로테아제 트리아드를 함유한다. 마지막으로, 순환성 2 쇄 자이모겐이 생체 내에서 칼슘 이온의 존재 하에 인지질 표면에서 트롬빈에 의해 활성화된다. 중쇄의 N-말단에서 도데카펩티드를 제거하여 활성화시키면 효소 활성을 갖는 활성화 단백질 C (aPC)가 생성된다.

패혈증
패혈증은 감염에 대한 전신성 염증반응으로 정의되며, 시토킨 네트워크, 백혈구, 보체 및 응고/피브린용해계를 포함하는 수많은 숙주 방어 기전에 대한 활성화와 연관되고 그에 의해 매개된다 [메스터스 (Mesters) 등, Blood 88:881-886, 199 6]. 다양한 기관들의 미소혈관계에 피브린이 확산 침착하는 산재성 혈관내 응고 (DIC)는 패혈증/패혈성 쇼크의 초기 증상이다. DIC는 많은 장기 부전의 전개에 있어 중요한 매개인자이며, 패혈성 쇼크를 앓고 있는 환자의 나쁜 징후에 원인이 된다 [푸리에 (Fourrier) 등, Chest 101:816-823, 1992].
패혈증의 다양한 동물 모델에서 단백질 C를 사용한 몇 가지 조장된 임상전 연구가 보고되었다. 테일러 (Taylor) 등은 비비 (baboon) 패혈증 모델의 연구 [J. Clin. Invest. 79:918-25, 1987]에서 혈장-유래 인간 활성화 단백질 C를 사용했다. 동물들을 예방 처리하였다 (즉, LD100 E. coli의 두 시간 주입을 시작하며 aPC를 투여함). 5 마리 동물 중 5 마리가 7 일간 생존했고, 이들을 실험 프로토콜에 대한 영구 생존체로 간주하였다. E. coli을 동일하게 주입한 대조군 동물들에서 5 마리 동물 중 5 마리가 24 내지 32 시간 내에 죽었다. 유효량은 7 내지 8 ㎎/㎏이었다.
래트의 지다당류 (LPS; E. coli) 패혈증 모델 [무라카미 (Murakami) 등, Blood 87:642-647, 1996]에서 LPS에 의해 유발된 폐혈관 손상이 투여량 100 ㎍/kg의 혈장-유래 인간 활성화 단백질 C에 의해 억제되었다. 더구나, 토끼의 결찰 및 천자 폐혈증 모델에서, 오카모토 (Okamoto) 등 [Gastroenterology 106:A747, 1994]은 혈장-유래 인간 활성화 단백질 C가 9 시간 동안 12 ㎍/kg/hr의 투여량에서 응고장애와 장기 부전으로부터 동물들을 보호하는데 효과적임을 입증했다. aPC의 종 특이성 때문에, 상기 동물 모델에서 얻어진 결과가 반드시 인간 치료를 예측하는 것은 아니다. 인간 활성화 단백질 C의 유효량 수준은 선택된 동물 모델에 의존하기엔 매우 다양하고 예측불가능하다. 예를 들어, 비비의 8 내지 10 분의 반감기와 토끼의 90 분인 반감기와 비교해 볼 때, 인간에서의 인간 활성화 단백질 C의 혈청 반감기는 30 내지 40 분이다.
최근 상기 질환의 병변생리학적으로 연관된 염증 매개인자를 차단시키는 약제를 사용하여 인간의 패혈증을 치료하기 위해 수많은 시도가 있었다. 그러나 염증 매개인자를 막는 다양한 약제를 사용하는 임상 연구는 성공적이지 못했다 [나탄 손 (Natanson) 등의 연구 , An.Intern.Med 120:771-783, 1994; 기발디 (Gibaldi), Pharmacothera py 13:302-308, 1993]. 염증에 관여하는 많은 매개인자들이 보상성 반응이어서 공중 보건에 영향을 미치므로, 몇몇의 연구자들은 매개인자들의 작용을 차단하는 것이 적절하지 않을 수 있다고 제안했다 [파릴로 (Parrillo), N.Engl. J. Med.328:1471-147 7, 1993].
최근 DIC를 차단하는 것이 패혈증의 임상 실험의 새로운 목표로 제안되었다 [레비 (Levi) 등, JAMA 270:975-979, 1993]. 그러나, 패혈증에서 단순히 응혈 결함만을 차단하는 것이 충분하지 않을 수 있다. 에스몬 (Esmon)[ Arteriosclerosis & Thromb. 12:135 -145, 1992]이 조사한 바와 같이, 활성 위치-차단된 인자 Xa [테일러 (Taylor) 등, Blood 78:364-368. 1991], 히루딘과 히루로그 [마라가노어 (Maraganore) , Pe rspective in Drug Discovery and Design 1:461-478, 1994]를 비롯한 몇몇 항혈전증제는 비비 패혈증 모델에서 효능을 나타내지 않았다. 이들 항혈전증제들은 각각 동물의 소멸성 응고장애를 차단시킬 수는 있었으나 생존률을 향상시킬 수는 없었다. 또한, 일본의 한 연구진들은 혈장-유래 활성화 단백질 C를 사용하여 간 부전증과 연관된 DIC 유사 증상 발현 가능성이 있는 응고장애를 치료하는 법을 제안했다 [특허 출원 JP7097335A].
최근, 박테리아성 패혈증의 전격성 자반증을 앓고 있는 25 명의 환자들을 관리하는 공격적 전통 치료법에 혈장-유래된 인간 단백질 C 자이모겐을 부가물로 사용하여 성공을 거두었다 (22명 생존) [게르손 (Gerson) 등, Pediatrics 91:418-4 22, 1993; 스미스 (Smith) 등, Thromb. Haemost, PS1709, p419, 1997; 린탈라 (Rintala) 등, Lancet 347:1767, 1996; 리바드 (Rivard) 등, J.Pediatr, 126:646-652, 1995].
게르손 (Gerson) 등은 1993 년 문헌에서, 공격적 전통 치료법에 반응하는데 실패한 그람 양성 균혈증 및 전격성 자반증을 앓고 있는 유아 치료에 대한 사례연구를 기재했다. 환자들을 혈장-유래 인간 단백질 C 자이모겐 (280 ㎍/kg 볼러스 + 40 ㎍/kg/hr 주입)으로 치료한 결과, 응고 장애 및 DIC의 통합적으로 교정되고 패혈성 쇼크 관련 전격성 자반증의 전개의 임상적 징후가 저지되었다. 린탈라 (Rintala) 등은 1996 년 문헌에서 전격성 자반증과 함께 나타나는 수막염구균성 패혈증에 걸린 성인 2명의 치료를 보고했다. 환자들을 8 내지 10일 동안 매 6 시간 마다 혈장-유래된 단백질 C 자이모겐 400 ㎍/kg 볼러스로 처치하였다. 한 명은 사망했고, 한 명은 생존했다. 리바드 (Rivard) 등은 1995 년 문헌에서 전격성 자반증과 또한 함께 나타나는 수막염구균성 혈증인 환자 4 명의 치료를 보고했는데, 인간 단백질 C 자이모겐 치료법을 따른 환자 전원이 생존했다. 상기 환자들을 매 6 시간 마다 400 ㎍/kg 볼러스의 양으로 처치하였다. 상기 연구의 샘플 크기는 작지만, 전격성 자반증과 함께 나타나는 수막염구균성 혈증과 연관된 사망률은 50% 초과이다 [포워스 (Powars) 등, Clin. Infectious Diseases 17:254-261, 1993]. 그러나, 상기 연구는 인간 단백질 C 자이모겐을 사용하여 수행했기 때문에, 활성화 단백질 C의 투여량 및 치료 기간 확립에 대해서는 거의 제안하지 않았다.
전격성 자반증 또는 DIC는 수막염구균성 혈증 이외에 리케치아 (Rickett sia; 록키산 홍반열, 진드기열, 발진티푸스 등) [그래이빌 (Graybill)등, Southern Medical Journal, 66(4):410-413, 1973; 로우브서 (Loubser)등, Annals of Tropic al Pa ediatrics 13:277-280, 1993]; 살모넬라 (Salmonella; 장티푸스열, 서교열) [코울 (Koul)등, Acta Haematol, 93:13-19, 1995]; 페렴구균 (Pneumococci)[카펜터 (Carpenter)등, Scand J Infect Dis, 29:479-483, 1997] 예르시니아 페스티스 (Yer sina pestis; 림프절 페스트)[버틀러 (Butler) 등, The Journal of Infectious Disease, 129:578-584, 1974]; 리지오넬라 뉴모필라 (Legionella pneumophilla; 리지오넬라병 (Legionaires Disease)); 열대열 말라리아 (Plasmodium falciparum; 뇌성 말라리아 (cerebral malaria) [레르카리 (Lercari) 등, Journal of Clinical Apheresis, 7:93-96, 1992]; 버크홀데리아 슈도말레이 (Burkholderia pseudomallei; 유비저 (Meliodosis)); 슈도모나스 슈도말레이 (Pseudomonas psudomallei; 유비저 (Meliodosis)) [푸투체아리 (Puthucheary)등, Transactions of the Royal Society of Tropical Medicine and Hygiene, 86:683-685, 1992]; 연쇄상 구균 (Steptococci; 치성의 감염 (Odontogenic infection)) [오타 (Ota), Y.,J.Japanese Assoc. Dis., 68:157-161]; 대상포진 바이러스 (zoster virus)[구옌 (Nguyen)등, Eur J Pediatr, 153:646-649, 1994]; 탄저균 (Bacillus anthracis; 탄저병 (Anthrax)) [프란즈 (Franz)등, Journal of the American Medical Assoc., 278(5):399-411, 1997]; 렙토스피라 인터로간스 (Leptospira interrogans; 렙토스피라병 (leptospirosis)) [힐 (Hill)등, Seminars in Respiratory Infections, 12(1):44-49, 1997]; 포도상구균 (Staphylococci) [레빈 (Levin), M.,Pediatric Nephrology, 8:223-229]; 이집트 헤모필루스 (Haemophilus aegyptius; 브라질 자반성열 (Brazilian purpuric fever)); 나이세리아 (Neisseria; 임균혈 (gonococcemia), 수막염구균혈증 (meningococcemia)); 및 마이코박테륨 튜버쿨로시스 (mycobacterium tuberculosis; 속립결핵 (miliary tuberculosis))에 의해 유발되는 감염을 포함하지만 이에 제한되지 않는 수많은 세균성, 바이러스성, 원충성 감염과 연관이 있다.
패혈증/패혈성 쇼크 또는 다른 감염들에서의 전격성 자반증, DIC 또는 후천성 단백질 C 결핍 상태가 상기 지시된 문헌들에 잘 기재되어 있지만, 활성화 단백질 C로 상기 환자를 치료하는 방법에 대한 자료는 거의 없다. 단백질 C의 생물학적 작용의 종 특이성 때문에, 동물 모델에서 활성화 단백질 C로 치료하여 수득한 임상전 약리학적 자료를 이용하여 인간의 투여량 수준을 확립하는 것이 어렵다.
이식
골수 이식 (BMT), 간 이식, 신장 이식 또는 다른 장기 이식 후에는 다양한 이식 관련 혈전색성 합병증이 있을 수 있다 [하이레 (Haire)등, JAMA 274:1289-1295, (1995); 하퍼 (Harper)등, Lancet 924-927 (1988); 및 소렌센 (Sorensen)등, J.Inter. Med 226:101-105 (1989); 고돈 (Gordon)등, Bone Marrow Transplan. 11:61-65, (1993)]. BMT [바자르바치 (Bazarbachi) 등, Nouv Rev Fr Hematol 35:135-140 (1993); 고돈 (Gordon) 등, Bone Marrow Transplan. 11:61-65, (1993)],신장 이식 [소렌센 (Sorensen) 등, J.Inter. Med 226:101-105 (1989)] 및 간 이식 [하퍼 (Harper) 등, Lancet 924-927 (1988)] 이후에 순환 단백질 C의 수치가 저하된다는 것이 보고된 바 있다. 이러한 단백질 C의 결핍은 응고 항진 상태인 환자를 혈전색전성 합병증에 대한 위험에 노출시킨다.
예를 들어, 간의 간정맥폐색증 (VOD)은 BMT에 대한 이식전 섭생법의 투여량을 제한하는 주요 합병증이다. VOD는 피브린의 혈관내 침착때문에 생긴 작은 간내 세정맥 변성의 결과로 추측된다 [파이오니 (Faioni) 등, Blood 81:3458-3462 (1993)]. 그 밖에, VOD는 BMT 이후 상당한 이환률과 치사율을 초래한다 [콜린스 (Collins)등, Throm. and Haemo. 72:28-33 (1994)]. VOD의 피크 발생과 동시적인 단백질 C의 수치 감소가 보고되었으며, 이는 이 상태의 발생에 대한 기여 인자일 것이다 [하퍼 (Harper) 등, Bone Marrow Transplan. 5:39-42, (1990)].
폐, 중추신경계, 간장 또는 신장을 포함하는 BMT 이후의 장기 부전은 이식 환자에게 높은 퍼센트로 발생하는 합병증이다 [하이레 (Haire) 등, JAMA 274:1289-1295, (1995)]. BMT의 단순 장기 부전은 BMT 환자를 사망에까지 이르게 하는 복합 장기 부전 증상 (MODS)의 유력한 요인이다. 응고계가 대량으로 활성화되고, 다양한 장기들의 미소혈관계에 피브린의 침착이 확산되는 산재성 혈관내 응고(DIC)는 MODS 발달의 중요한 매개인자이다 [푸리에 (Fourrier)등, Chest 101:816-823 (1992)]. 따라서, 골수 또는 다른 장기 이식을 받은 환자들의 단백질 C 결핍은 정맥성 혈전색전성 합병증 및 장기 부전과 같은 질환에 걸리기 쉬운 환자에게 응고 항진 상태를 일으킨다. 활성화 단백질 C를 이용하여 장기 이식과 연관된 응고 항진 상태에 있는 환자들을 치료하는 방법이 조속히 요구된다.
화상
심한 화상 환자는 응고 항진과 연관된 합병증을 가진다고 오랫동안 인식되어 왔다 [큐레리 (Curreri) 등, Ann, Surg. 181:161-163 (1974)]. 화상 환자는 갑작스런 광범위한 출혈의 개시; 피브리노겐, 혈소판 및 인자 VIII 활성의 소멸; 혈관내 용혈 작용; 이차 피브린용해; 및 미소혈전증의 생검 증거 [맥마니스 (Mcmanis) 등, J. of Trauma 13:416-422, (1973)]를 특징으로 하는 DIC가 자주 발병하고, 시험관내에서 정상 이상의 혈괴 활성이 일어난다. 최근 심한 화상 환자는 단백질 C 수치가 급격히 감소되고, 이러한 천연 항응고제의 감소로 DIC의 위험이 증가될 수 있다는 보고가 있었다 [로 (Lo) 등, Burns 20:186-187 (1994)]. 또한, 우에야마 (Ueyama) 등은 화상의 초기 단계의 DIC에 대한 병인을 논의하면서, 화상의 심각도와 비례하여 대량의 트롬빈 생성과 항응고 활성의 감소가 일어날 수 있다고 결론지었다 [우에야마 (Ueyama) 등, Nippon Geka Gakkai Zasshi 92:907-12 (1992)]. DIC는 심한 화상을 입은 환자의 통상적인 합병증의 하나이다.
상술한 바와 같이, 심한 화상 환자에게서도 단백질 C 결핍이 보고되었으나, 단백질 C 대체 요법이 효과적이라거나 화상 환자를 활성화 단백질 C로 치료하는 방법에 관한 자료는 거의 없다.
임신
임신이 응고 항진 상태를 초래할 수 있도록 응고계에 많은 변화를 준다는 것이 잘 알려져 있다. 예를 들어, 임신과 분만후 (post-partum) 기간 동안에, 정맥성 혈전증의 위험은 임신이 아닐 때보다 거의 5 배 정도 더 높다. 게다가, 혈괴 인자가 증가하고, 응고의 천연 억제제가 감소하고, 피브린용해계에 변화가 일어나며, 정맥 울혈이 증가할 뿐만 아니라 태반 분리, 제왕 절개 또는 감염에 의한 출산시 혈관 손상이 증가한다 [바르보어 (Barbour) 등, Obstet Gynecol 86:621-633, 1995].
어떠한 위험 인자도 없는 여성에서는, 상기 응고 항진 상태로 인한 합병증의 위험이 적지만, 혈전색전증에 대한 병력을 가진 여성이라면 임신시 재발할 위험이 증가한다. 또한, 최근 발견된 활성화 단백질 C에 대한 유전적 내성을 포함하여, 잠재적인 응고 항진 상태에 있는 여성도 더 높은 재발 위험이 있다 [다을백 (Dahlback), Blood 85:607-614, 1995].
그러므로, 항트롬빈-III, 단백질 C 또는 단백질 S이 결핍되는 것으로 밝혀진 정맥성 혈전색전증의 병력을 가진 여성은 혈전증 재발에 대한 위험이 상당히 높고, 항응혈 예방 치료가 고려되야 한다고 제안되었다 [콘라드 (Conrad)등, Throm Haemost 63:319-320, 1990].
임신 여성의 자간전증과 자간증 상태는 피브린 형성이 증가되고 피브린용해계 및 혈소판이 활성화되며 혈소판 수가 감소되는 것으로 나타나는 응고장애 증가 상태를 보인다 [Clin Obstet Gynecol 35:338-350, 1992]. 자간전증은 태반의 "혈관 삽입" 이상으로 인한 자궁 태반의 국소빈혈의 결과 발병하는 것으로 생각된다. 자간전증의 결과로는 태반, 신장, 간장 및 뇌의 손상 [Rev Fr Gynecol Obstet 86:158-163, 1991]을 일으키는 수많은 미소혈전증의 방출을 초래하는 DIC 뿐만 아니라 고혈압도 포함된다. 더구나, 자간전증은 혈소판 감소증, 용혈 작용 및 간 기능 저하에 의한 자간전증의 합병증으로 정의되는 HELLP 증후군으로 공지된 심각한 생명을 위협하는 상태를 야기시킬 수 있다 [라스게버 (Rathgeber) 등, Anarth Intensivther Notfallmed 25:206-211, 1990]. 그밖에, 정상의 임신부와 비교할 때 심각한 자간전증을 앓고 있는 임신 중인 여성의 단백질 C 수치가 감소한다는 것이 보고되었다 [데 스테파노 (De Stefano) 등, Thromb Haemost 74;793-794, 1995].
따라서, 임신 중인 여성에게 발생하는 정맥성 혈전색전성 합병증은 특히 혈전색전성 질환의 병력을 갖고 있는 여성 환자에게 있어서 주요 관심사이다. 자간전증이나 DIC와 같은 심각한 합병증의 가능성이 비교적 낮을지라도, 가능한 빨리 진단하여 활성화 응고계를 억제함으로써 DIC 치료를 시작하는 것이 필수적이라고 제안되었다 [라스게버 (Rathgeber) 등, Anasth Intensivther Notfallmed 25:206-211, 1990]. 자간전증이나 DIC의 합병증은 응고 항진 상태가 되고 단백질 C 수준이 저하되는 패혈증에서 발생하는 상황과 유사하다.
대수술/외상
대수술이나 사고 외상으로부터 회복한 환자는 유도 응고 항진 상태의 결과로 혈액 응고 합병증에 자주 걸리게 된다 [와트킨 (Watkin)등, Klin Wochenschr 63:1019-1027, 1985]. 응고 항진 상태는 점차적으로 수술 환자들에게 있어서 정맥성 혈전색전증의 원인으로 인식되어 왔다 [토마스 (Thomas)등, Am J Surg. 158:491-494, 1989; 레클럭 (LeClerc), J.R., Clin Appl Thrombosis/Hemostasis 3(3):153-156, 1997]. 이 응고 항진 상태는 빈번하지는 않지만 DIC 유사 증상을 보이는 합병증을 초래할 수 있으며, 발병하면 파괴적이고 종종 치명적일 수 있다 [콜린스 (Collins) 등, Am J Surg. 124:375-380, 1977].
또한, 심장 동맥 바이패스 이식술 (CABG) [멘지스 (Menges) 등, J Cardiothor Vasc An. 10:482-489, 1996], 척수 수술 [메이어 (Mayer) 등, Clin Orthop. 245:83-89, 1989], 복부 대수술 [브라메이 (Blamey) 등, Thromb Haemost. 54:622-625, 1985], 정형 대수술 또는 하지 관절성형 수술 [레클럭 (LeClerc), 1997], 또는 다른 형태의 수술 [토마스 (Thmas) 등, Am J Surg. 158:491-494, 1989]을 받은 환자들은 때때로 정맥성 혈전색전성 합병증이 발병할 수 있다. 그밖에, 일본의 연구원들은 성인의 경우 매일 1 내지 10 ㎎의 투여량으로, 바람직하게는, 2 내지 6 ㎎을 1 내지 2 회에 나눠서 볼러스로 투약하거나 정맥내 주입하여 척추 손상과 연관된 미소혈관계 혈전증을 혈장 유래된 단백질 C로 치료하는 방법 [일본 특허 JP8325161A]을 제안하였다.
항응고 치료법이 대수술이나 외상 환자들의 정맥성 혈전색전성 질환에 대한 예방 요법으로서 중요하다고 제안되었다 [토마스 (Thomas) 등, 1989; 레클럭 (LeClerc), 1997]. 예를 들어, 폐동맥 색전증으로 사망한 많은 환자들은 사전에 혈전색전성 질환의 임상 증거가 없었으며 진단을 내리고 치료법을 시행하기도 전에 사망했다 [레클럭 (LeClerc), 1997]. 현존하는 예방 방법, 예를 들면, 저분자량 헤파린인 와파린은 잔존하는 근위혈전증이나 빈번한 투여량 조정이 필요한 한계를 갖는다.
ARDS
성인 호흡 곤란 증후군 [ARDS]은 폐부종, 미소혈전, 염증성 세포 침윤 및 후섬유증을 특징으로 한다. 응고 항진 상태를 초래하는 응고의 활성화가 이러한 다중 세포성 및 염증 반응의 핵심이다. 통상적인 ARDS와 연관된 응고 질환으로 혈관 내 응고와 피브린용해의 억제가 있다. 응고계의 활성화와 피브린용해의 억제로 형성된 피브린이 급성 폐손상의 병인에 원인이 될 수 있다. 패혈증, 외상 및 다른 임상 질환들이 ARDS를 일으키는 중요한 위험 인자이다 [하세가와 (Hasegawa)등, Chest 105 (1):268-277, 1994].
ARDS는 응고 활성화 및 섬유소 용해의 억제와 관련이 있다. DIC 중에 나타나는 응고 항진화와 비슷한 폐 정맥성 미세혈전의 존재에 대한 많은 임상 증거가 존재한다. 그러므로, ARDS와 연관된 이러한 응고 항진 상태에 대한 효과적인 치료가 조속히 요구된다.

그러나 본 보고서에도 불구하고 패혈증, 이식, 화상, 임신, 대수술, 외상, 또는 ARDS와 연관된 후천성 응고 항진 상태 또는 후천성 단백질 C 결핍을 앓고 있는 인간에 대한 안전하고 효과적인 치료법을 위한 투여 체계는 여전히 알려져 있지 않다. 상기 연구로부터 인간의 응고 항진 상태 또는 후천성 단백질 C 결핍의 치료에 본 발명의 재조합 활성화 단백질 C를 사용하는 것을 예견할 수는 없다.
본 발명은 심각한 패혈증 환자의 임상 실험에서의 aPC 용도를 개시한다. 이런 환자들에서, r-aPC 치료군은 위약 대조군에 비해 DIC 표지가 낮아지고 사망률이 감소되는 장기 기능 상의 통계학적 개선을 나타내었다. 심각한 패혈증 환자에게 사용된 aPC의 투여량은 48 시간 주입 동안 12, 18, 24 및 30 ㎍/kg/hr이었다. 12 및 18 ㎍/kg/hr의 투여량은 본 연구에서는 효과적이지 않았다. 놀랍게도, 본 연구에서 사용된 24 및 30 ㎍/kg/hr의 투여량이 효과적이었으며, 예상외로 공지된 임상전 약리학 자료에 비해 상당히 낮았다.

aPC 또는 활성화 단백질 C는 재조합된 활성화 단백질 C를 말한다. aPC는 인간 단백질 C를 포함하고, 완전 단백질 C 단백분해, 아미드분해, 에스테르분해, 및 생물학적 (항응혈 또는 프로피브리놀린용해) 활성을 가지는 다른 종이나 유도체들을 또한 포함할 수 있지만 인간 단백질 C가 바람직하다. 단백질 C 유도체들의 예는 본원 명세서에서 인용된 참고 문헌인 게르리츠 (Gerlitz) 등의 미국 특허 제 5,453,373호 및 포스터 (Foster) 등의 미국 특허 제 5,516,650호에 기재되어 있다. 재조합된 활성화 단백질 C는 시험관내에서 재조합 인간 단백질 C 자이모겐을 활성화하거나 단백질 C의 활성화 형태를 직접 분비하여 얻을 수 있다. 단백질 C는 예를 들어 인간 신장 293 세포로부터 자이모겐으로서 분비된 후, 숙련된 당업자에게 공지된 기술에 의해 정제되고 활성화되는 것을 포함하는 세포, 진핵세포, 형질전환 동물 또는 형질전환 식물에서 생산될 수 있다.

본 발명의 방법하에 전적으로 활성화되고 조작가능하기 위해서, 이들 방법 중 임의의 방법으로 제조된 aPC는 9 개의 감마-카르복시-글루탐산의 첨가 (감마-카르복실화 즉, Gla 함유), 1 개의 에리쓰로-베타-히드록시-Asp의 첨가 (베타-히드록실화), 4 개의 Asn-연결된 과당류의 첨가 (포도당화), 리더 서열 (42개 아미노산 잔기)의 제거 및 디펩티드 Lys 156-Arg 157의 제거와 같은 번역후 변형을 거쳐야 한다. 번역후 변형 없이는 aPC가 충분히 기능적이지 못하거나 비기능적이다.
aPC는 약리학적으로 유용한 조성물을 제공하는 공지된 방법에 따라서 제형화될 수 있다. aPC는 적합한 투여량을 약 24 시간 내지 약 144 시간 동안의 연속 주입액으로 주사함으로써 유효 형태로 혈류 내에 전달되도록 비경구적으로 투여될 것이다. 투여된 aPC의 양은 약 20 ㎍/kg/hr 내지 약 50 ㎍/kg/hr이다. 더욱 바람직하게는 투여된 aPC의 양은 약 22 ㎍/kg/hr 내지 약 40 ㎍/kg/hr이다. 더욱더 바람직하게는 투여된 aPC의 양은 약 22 ㎍/kg/hr 내지 약 30 ㎍/kg/hr이다. 가장 바람직한 aPC의 투여량은 약 24 ㎍/kg/hr 내지 약 30 ㎍/kg/hr이다.
또는, aPC를 약 5 분 내지 약 120 분의 시간 동안 볼러스 주사로 시간 당 적합한 투여량의 일부 (1/3 내지 1/2)를 주사한 후, 약 23 시간 내지 약 144 시간 동안 적합한 투여량을 연속 주입하여 24 시간 내지 144 시간에 걸쳐 투여되는 적합한 투여량을 만들 수 있다.
주의깊게 대조된 임상 연구와 철저한 실험 연구 후에서야 본 출원인들은 효 과적인 치료법은 약 24 시간 내지 약 144 시간 동안 연속 주입되는 약 20 ㎍/kg/hr 내지 약 50 ㎍/kg/hr의 투여량 수준이 효과적 치료법임을 밝혀내었다. 후천성 응고 항진 상태 또는 후천성 단백질 C 결핍을 앓고 있는 인간 환자를 치료하는데 투여되는 aPC의 가장 바람직한 투여량 수준은 본 명세서에서 개시한 바에 따르면 약 24 ㎍/kg/hr이다.
제조예 1
인간 단백질 C 제조
재조합 인간 단백질 C (r-hPC)를 인간 신장 293 세포에서 얀 (Yan)의 미국 특허 제4,981,952호 (본원에 전문이 참고 문헌으로 인용됨)에 개시된 바와 같은 당업자들에게 널리 공지된 기술로 생산하였다. 유전자 암호화 인간 단백질 C는 방 (Bang)의 미국 특허 제4,775,624호 (본원에 전문이 참고 문헌으로 인용됨)에 개시되고 청구되었다. 293 세포에서 인간 단백질 C를 발현시키는데 사용된 플라스미드는 방의 미국 특허 제4,992,373호 (본원에 전문이 참고 문헌으로 인용됨)에 개시된 플라스미드 pLPC이었다. 플라스미드 pLPC의 구성은 또한 유럽 특허 공개 제 0 445 939호 및 [그린넬 (Grinnell) 등의 1987 년 Bio/Technology 5:1189-1192] (본원에 전문이 참고 문헌으로 인용됨)에 기재되어 있다. 간략하게, 플라스미드를 293 세포로 형질감염시키고, 이어서 안정한 형질전환체를 동정하고 무혈청 배지에서 계대배양하고 성장시켰다. 무세포 배지는 발효시킨 후 한외여과로 얻었다.
인간 단백질 C를 얀의 미국 특허 제4,981,952호 (본원에 전문이 참고 문헌으 로 인용됨)의 기술을 응용하여 배양액으로부터 분리하였다. EDTA 중의 4 mM로 청정화 배지를 만들어 음이온 교환 수지 (패스트-플로우 Q, 파마시아)에 흡수시켰다. Tris 20 mM, NaCl 200 mM (pH 7.4)의 4 컬럼 부피 및 Tris 20 mM, NaCl 150 mM (pH 7.4)의 2 컬럼 부피로 세척한 후 결합된 재조합 인간 단백질 C 자이모겐을 Tris 20 mM, NaCl 150 mM 및 CaCl2 10 mM (pH 7.4)로 용출시켰다. 용출된 단백질은 SDS-폴리아크릴아미드 겔 전기영동법으로 판단할 때 용출 후 95% 이상 순수해졌다.
NaCl 중에 단백질을 3 M로 만든 후 소수성 상호작용 수지에 흡착시키고 (도요펄 페닐 (Toyopearl Phenyl) 650 M, 도소하스 (TosoHaas)), Tris 20 mM, NaCl 3 M 및 CaCl2 10 mM (pH 7.4)로 평형화시킴으로써 단백질 추가 정제를 수행하였다. CaCl2가 없는 평형화 완충액 2 컬럼 부피로 세척한 후 재조합 인간 단백질 C를 Tris 20 mM (pH 7.4)로 용출시켰다.
용출된 단백질을 활성화를 위해 잔류 칼슘을 제거시켰다. 재조합 인간 단백질 C를 금속 친화성 컬럼 (켈렉스 (Chelex)-100, 바이오-라드)에 통과시켜 칼슘을 제거하고 다시 음이온 교환기에 결합시켰다 (패스트 플로우 Q, 파마시아). 이 두 컬럼 모두를 직렬로 배열하고, Tris 20 mM, NaCl 150 mM 및 EDTA 5 mM (pH 6.5)로 평형화시켰다. 단백질을 적재한 후, 켈렉스-100 컬럼을 같은 완충액의 1 컬럼 부피로 세척하고 직렬 연결에서 분리하였다. 평형화 완충액 3 컬럼 부피로 세척한 후 음이온 교환 컬럼을 Tris-아세테이트 20 mM, NaCl 0.4 M (pH 6.5)로 용출시켰다. 재조합 인간 단백질 C와 재조합 활성화 단백질 C 용액의 단백질 농도를 UV 280 nm에서 흡광도를 측정하였다 (각각의 E0.1%는 1.85 또는 1.95).
제조예 2
재조합 인간 단백질 C의 활성화
소 트롬빈을 4℃에서 HEPES 50 mM (pH 7.5) 존재 하에 활성화된 CH-세파로스 (Sepharose) 4B (파마시아)와 커플링 반응시켰다. 커플링 반응은 약 5000 단위 트롬빈/㎖ 수지를 사용하여 컬럼에 이미 채워진 수지상에서 수행하였다. 트롬빈 용액을 약 3 시간 동안 컬럼을 순환시킨 후, 순환 용액 1 ℓ 당 0.6 ㎖의 농도로 MEA를 첨가하였다. 수지상의 반응하지 않은 아민들을 완벽하게 차단하기 위해 MEA를 포함한 용액을 10 내지 12 시간 동안 더 순환시켰다. 차단한 후, 트롬빈-결합된 수지를 10 컬럼 부피의 NaCl 1 M, Tris 20 mM (pH 6.5)로 세척하여 모든 비특이적으로 결합된 단백질을 제거하고 활성화 완충액 중에서 평형화시킨 후 활성 반응에 사용했다.
정제된 r-hPC를 EDTA 중에 5 mM로 만들고 (임의의 잔류 칼슘을 착물화시킴), Tris 20 mM (pH 7.4) 또는 Tris-아세테이트 20 mM (pH 6.5)를 사용하여 2 ㎎/㎖의 농도로 희석시켰다. 이 물질을 NaCl 50 mM 및 Tris 20 mM (pH 7.4) 또는 Tris-아세테이트 20 mM (pH 6.5) 중 하나와 함께 37℃에서 평형을 이룬 트롬빈 컬럼에 통과시켰다. 유속은 r-hPC와 트롬빈 수지 사이에 접촉 시간이 대략 20 분이 되도록 조절하였다. 유출물을 수집하고 즉시 아미드분해 활성에 대해 분석하였다. 만일 물질이 aPC의 확립된 표준물과 비교해 특이적 활성 (아미드분해)을 갖지 않는다면, r-hPC를 완벽하게 활성화시키기 위해 트롬빈 컬럼에 재순환시켰다. 이어서 다음 공정 단계를 기다리는 동안 aPC가 더 낮은 농도로 유지되도록 pH 7.4 내지 6.0의 임의의 pH (자동 분해 (autodegradation)를 방지하기 위해 보다 낮은 pH가 바람직함)인 상기의 완충액 20 mM로 물질을 1:1로 희석시켰다.
aPC 재료로부터 침출된 트롬빈을 제거하기 위해 pH 7.4의 Tris 20 mM 또는 바람직하게는 pH 6.5의 Tris-아세테이트 20 mM의 활성화 완충액 중에서 NaCl 150 mM로 평형화시킨 음이온 교환 수지 (패스트-플로우 Q, 파마시아)에 aPC를 결합시켰다. 평형화 완충액 20 mM로 2 내지 6 컬럼 부피를 세척하는 동안 트롬빈을 컬럼에 통과시키고 용출시켰다. 결합된 aPC는 pH 7.4의 Tris 20 mM 또는 pH 6.5의 Tris-아세테이트 5 mM에서 NaCl 0.4 M을 사용하여 단계구배액으로 용출시켰다. 컬럼을 더 많은 부피로 세척하면 도데카펩티드를 보다 완벽하고 쉽게 제거할 수 있었다. 이 컬럼으로부터 용출된 물질은 냉동액 (-20℃) 중에 또는 동결건조 분말로 보관하였다.
aPC의 아미드분해 활성도 (AU)는 베크만 DU-7400 다이오드 어레이 분광기를 사용하여 카비 비트럼 (Kabi Vitrum)으로부터 구입한 합성 기질 H-D-Phe-Pip-Arg-p-니트로아닐라이드 (S-2238)로부터 p-니트로아닐린을 방출시킴으로써 결정하였다. 활성화 단백질 C의 1 단위는 9620 M-1㎝-1 의 405 ㎚에서의 p-니트로아닐린 흡광계수를 이용하여 pH 7.4, 25℃에서 1 분에 p-니트로아닐린 1 μ㏖을 방출하는 데 필요한 효소의 양으로 정의된다.
활성화 단백질 C의 항응혈 활성은 활성화 부분 트롬보플라스틴 시간 (APTT) 응혈 분석 중 응혈 시간의 지속성을 측정하여 결정하였다. 단백질 C 농도 범위가 125 내지 1000 ng/㎖인 희석 완충액 (방사성 면역분석법 (radioimmunoassay) 등급 BSA 1 ㎎/㎖, Tris 20 mM, pH 7.4, NaCl 150 mM 및 NaN3 0.02%)에서 기준 곡선을 제작하였고, 이 농도 범위에서 몇 개의 희석 용액을 취하여 샘플을 제조하였다. 각 샘플 큐베트에 냉각된 말 혈장 50 ㎕와 재구성된 활성화 부분 트롬보플라스틴 시간 제제 (APTT 제제, 시그마) 50 ㎕를 첨가하고, 5 분 동안 37℃에서 인큐베이션시켰다. 인큐베이션 후, 적합한 샘플 또는 표준물 50 ㎕를 각 큐베트에 첨가하였다. 희석 완충액을 샘플 또는 표준물 대신 사용하여 기저 응혈 시간을 결정하였다. 각 샘플 또는 표준물에 37℃의 CaCl2 30 mM 50 ㎕를 첨가한 후, 피브로메타의 타이머 (CoA 스크리너 헤모스타시스 분석계, 아메리칸 레이버)를 작동시켰다. 샘플 중의 활성화 단백질 C 농도를 기준 곡선의 선형 회귀 방정식으로 계산하였다. 본원에 보고된 응혈 시간은 기준 곡선 샘플들을 포함하여 3 회 반복 시험의 최소량의 평균이다.
당업자라면 상기 기술 내용으로부터 aPC를 제조하고 이를 패혈증, 이식, 화상, 임신, 대수술/외상 및 ARDS (상기에 제한되지 않음)와 연관된 응고 항진 상태 또는 후천성 단백질 C 결핍의 치료에 사용할 수 있을 것이다.
실시예 1
aPC의 인간 혈장 수준
6 명의 인간 환자들에게 24 시간에 걸쳐 1 ㎎/㎡/hr 또는 약 0.024 ㎎/kg/hr 으로 aPC를 정맥 주입하였다. 투여된 aPC는 물 2 ㎖를 넣어 pH 6.5로 조정된 aPC 10 ㎎, Tris-아세테이트 5 mM 완충액 및 NaCl 100 mM을 함유하는 동결건조 제형이었다.
aPC의 혈장 농도는 면역포획 (Immunocapture)-아미드분해 측정법을 이용하여 측정하였다. 혈액은 시트레이트 항응혈제 및 aPC의 가역 억제제인 베즈아미딘의 존재하에 수집하였다. 효소는 미세적정 플레이트에 고정된 aPC 특이성인 쥐의 단클론성 항체 (C3)에 의해 혈장으로부터 얻었다. 억제제는 세척하여 제거하였고 aPC의 아미드분해 활성은 올리고펩티드 색원체 기질을 이용하여 측정하였다. 37℃에서 16 내지 20 시간 동안 인큐베이션시킨 후, 405 ㎚에서 흡광도를 측정하였고 데이타는 중선형 커브-피팅 (curve-fitting) 연산으로 분석하였다. 0 내지 100 ng/㎖의 농도 범위의 기준 곡선으로부터 aPC 농도를 얻었다. 분석의 정량 한계는 1.0 ng/㎖이었다. aPC 투여 수준과 혈장 농도를 약 24 시간에 측정하였다. 0.024 ㎎/㎏/hr의 투여량으로 24 시간 동안 약 50 ng/㎖의 혈장 농도를 얻을 수 있다.
실시예 2
패혈증인 인간 환자에 대한 이중 맹검 위약-대조 실험, 단계 1
본 프로토콜은 심한 패혈증 환자 중의 두 단계 이중 맹검 위약 대조 시험이다. 1 단계에서는 전체 72 명의 환자에게 재조합 인간 활성화 단백질 C (r-aPC)를 48 시간 동안 주입하였다.
입회 기준은 통상적으로 인정되는 패혈증의 4 가지 판단 기준 (심장 RATE, 호흡성 효과, 증가/감소 온도, 증가/감소 백혈구 수) 중에 3가지를 포함시켰다. 또한, 쇼크, 감소된 뇨 배출량 또는 저산소증으로 정의되는 환자는 어느 정도의 장기 부전을 보이는 환자만을 선택하였다. 4 개의 다른 투여량을 사용하였다; 12, 18, 24, 30 ㎍/㎏/hr. r-aPC를 연속 주입 방법으로 48 시간 동안 주입하였다. 본 연구의 1차 목표점은 투여량 및 투여 지속기간의 함수로서의 안전성, 투여량 및 투여 지속기간의 함수로서의 응고 장애를 고치는 r-aPC의 능력이었다.
사망률 정보는 달리 지시되지 않는 한 모든 투여량과 최저 투여량까지도 포함한다. 위약 사망률이 예상된 위약 사망률과 일치했다는 사실이 중요하다. 모두 사망한 28 일이 위약을 받은 환자들 대 r-aPC을 받은 환자들의 사망률의 종점이었다.
전체 관찰된 위약 사망률은 38% (10/26)이고 전체 관찰된 r-aPC 사망률은 20% (9/46)이었다. 위약 환자에 대한 r-aPC의 상위 두개의 투여량 (24 및 30 ㎍/㎏/hr)만을 포함한 하위집단의 관찰된 사망률은 13% (3/24)이었다.
두 번째 하위그룹 분석에서는 단백질 C 활성도의 기저값이 60% 이하로 정의되는 후천성 단백질 C 결핍인 환자를 포함하였다. 유용한 단백질 C 활성도의 기저값을 가진 64 명의 환자중 61 명의 환자 또는 95%가 연구에 입회된 시점에서 후천성 단백질 C 결핍이 있었다. 단백질 C 결핍 환자에서 관찰된 위약 사망률은 41% (9/22)였고, 단백질 C 결핍 환자에게서 관찰된 r-aPC 사망률은 18% (7/39)였다.
위약 환자에 대한 치료된 환자들의 사망에 이르는 평균 시간이 r-aPC의 낮은 투여량 치료가 중증 패혈증인 환자에게 유익하다고 제안하는 데 상당 부분 작용한 다. 위약 군에서 사망한 10 명의 환자들 중 사망에 이르는 평균 시간은 6 일이었다. r-aPC로 치료된 환자들 중 사망에 이르는 평균 시간은 14 일이었다. 그밖에, r-aPC로 치료 중 사망한 9 명의 환자 중 4 명이 21 일 이상 생존했고, 이후 이들은 패혈증의 첫번째 에피소드에 관여되지 않은 사건으로 사망했다. 늦게 사망한 4 명 중 2 명은 낮은 투여량 군 (12 ㎍/㎏/hr)에서 발생하였다. 이러한 환자들 모두가 중환자실에 체류하였고 그들의 사망까지 본 연구의 전 기간 (27 일)동안 기계적 환기를 시켰다. 늦게 사망한 다른 두 환자는 높은 투여량 군이었다 (30 ㎍/㎏/hr). 이 환자들 모두 초기에는 개선을 보였다. 2 주 이내에 두 환자 모두 기계적 환기를 마치고, 중환자실로 옮겨졌다. 한 환자는 "소생술을 실시하지 말라"는 지시의 정지를 요구한 후 패혈증으로 유발된 호흡곤란증 때문에 1 주 후에 사망했다. 두 번째 환자는 패혈증의 두 번째 에피소드와 연관된 폐 부전증의 에피소드를 겪은 지 28 일 후 사망했다. 이 환자는 또한 요구된 DNR 상태였으며 그러므로 재삽관하지 않았다. 28 일간의 연구동안 중증 패혈증의 두 번째 에피소드가 발병한 환자를 r-aPC로 재치료하는 것은 치료 프로토콜하에 승인되지 않았음을 주지해야 한다.
본 연구에서 사망률 정보는 놀랍게도 예상 밖의 것이었다. 다른 이중 맹검, 위약 대조된 패혈증 연구에서는 모두 사망한 28 일에 뚜렷한 감소를 보인 데이터가 얻어지지 않았다.
실시예 3
활성화 단백질 C의 제형
활성화 단백질 C 약 2.5 ㎎/㎖, 슈크로오스 약 15 ㎎/㎖, NaCl 약 20 ㎎/㎖ 및 pH 5.5 초과 pH 6.5 미만인 시트르산나트륨 완충액을 포함하는 용액을 동결건조시키는 것을 포함하는 공정으로 활성화 단백질 C의 안정한 동결건조 제형을 제조하였다. 또한, 활성화 단백질 C 약 5 ㎎/㎖, 슈크로오스 약 30 ㎎/㎖, NaCl 약 38 ㎎/㎖ 및 pH 5.5 초과 pH 6.5 미만인 시트르산나트륨 완충액을 포함하는 용액을 동결건조시켜 활성화 단백질 C의 안정한 동결건조 제형을 제조하였다.
aPC:염:증량제 (w:w:w)의 비율은 동결 건조 공정에 적합한 제형화에 중요한 인자이다. 비율은 aPC의 농도, 염 선택 및 농도, 증량제 선택 및 농도에 따라 다르다. 특히, 활성화 단백질 C 약 1:염 약 7.6:증량제 약 6의 비율이 바람직하다.
연속 주입에 의한 투여에 적합한 활성화 단백질 C의 단일 투여량 제형은 활성화 단백질 C, NaCl, 슈크로오스 및 시트르산나트륨 완충액을 혼합해서 제조하였다. 혼합 후, 용액 4 ㎖를 단일 투여량 용기에 옮기고 동결 건조시켰다. 환자들에게 약 0.02 ㎎/㎏/hr 내지 약 0.05 ㎎/㎏/hr의 투여량을 투여하는데 적합한 활성화 단백질 C의 약 5 ㎎ 내지 약 20 ㎎을 포함하는 허용된 단일 투여량 용기를 밀폐시키고 저장하였다.
Claims (18)
- 인간 배아 신장 (HEK) 293 세포 내의 단백질 C 자이모겐의 발현에 이어서 시험관내 활성화에 의해 얻어진 인간 활성화 단백질 C를 포함하며 이 단백질이 약 24 시간 내지 약 144 시간 동안의 연속 주입에 의해 약 20 ㎍/kg/hr 내지 약 50 ㎍/kg/hr의 투여량으로 인간 환자에게 투여되도록 제조된, 패혈증과 관련된 후천성 응고 항진 상태의 치료 또는 예방을 위한 제약 제제.

- 제13항에 있어서, 인간 활성화 단백질 C가 약 22 ㎍/kg/hr 내지 약 30 ㎍/kg/hr의 투여량으로 투여되는 제약 제제.
- 제14항에 있어서, 인간 활성화 단백질 C가 약 24 ㎍/kg/hr의 투여량으로 투여되는 제약 제제.
- 인간 배아 신장 293 세포 내의 단백질 C 자이모겐의 발현에 이어서 시험관내 활성화에 의해 얻어진 유효량의 인간 활성화 단백질 C를 포함하며 약 24 시간 내지 약 144 시간 동안의 연속 주입에 의해 인간 환자에서 상기 단백질의 혈장 수준이 2 ng/㎖ 내지 200 ng/㎖의 범위를 달성하도록 제조된, 패혈증과 관련된 후천성 응고 항진 상태의 치료 또는 예방을 위한 제약 제제.
