Description
本願は、米国仮特許出願第61/220,968号(2009年6月26日出願)の優先権を主張し、この出願の全開示は、本明細書に参考として援用される。
本発明は、概して、身体流路に埋め込む人工弁に関する。より具体的には、本発明は、現在の弁よりも少ない時間で外科的に埋め込まれるように構成される一体的な人工心臓弁に関する。
(項目1)
心臓弁輪に埋め込むための人工心臓弁であって、
流動口を画定し、流入端を有する非拡張型で非折畳み式の弁輪支持構造と、
該支持構造に取り付けられ、該流動口を横断して交互に開閉するように載置される弁尖と、
該支持構造の該流入端を囲む縫合糸透過可能輪と、
該流動口の周囲に延在する第1端を有し、該支持構造の該流入端において該弁に接続される塑性拡張型連結ステントであって、該連結ステントは、該支持構造から離れるように流入方向に突出する第2端を有し、埋込位置まで送達するための収縮状態と、弁輪と外側が接触するための該第1端よりも幅広い拡張状態とを呈することが可能である連結ステントと
を備える、人工心臓弁。
(項目2)
前記支持構造は、流出方向に突出する複数の交連柱を含み、前記弁尖は、可撓性であり、該支持構造および交連柱に付着する、項目1に記載のシステム。
(項目3)
前記縫合糸透過可能輪は、縫製輪であり、前記連結ステントは、該縫製輪に付着する、項目1に記載のシステム。
(項目4)
前記収縮状態において、前記連結ステントは、円錐形であり、前記第1端から前記第2端に向かって内向きにテーパーが付いている、項目1に記載のシステム。
(項目5)
前記拡張状態において、前記連結ステントは、円錐形であり、前記第1端から前記第2端に向かって外向きにテーパーが付いている、項目4に記載のシステム。
(項目6)
前記連結ステントは、複数の半径方向拡張型支柱を備え、そのうちの少なくともいくつかが列状に配設されており、最遠位列は、前記収縮状態から前記拡張状態までの拡張に対する最大能力を有する、項目5に記載のシステム。
(項目7)
前記人工弁は、市販の弁を備え、前記縫合糸透過可能輪は、自身の縫製輪であり、該縫製輪は、該縫製輪に前記塑性拡張型連結ステントが接続されている、項目1に記載のシステム。
(項目8)
人工心臓弁システムの送達および埋込みの方法であって、
非拡張型で非折畳み式の開口を有する人工弁を含む心臓弁を提供することであって、該心臓弁は、自身の流入端から延在する拡張型連結ステントをさらに含み、該連結ステントは、埋込位置に送達するための収縮状態と、弁輪への外側接続のために構成される拡張状態とを有する、ことと、
該弁輪に隣接する埋込位置まで、その収縮状態にある該連結ステントとともに該心臓弁を前進させることと、
該弁輪と接触し、該弁輪に接続している該拡張状態まで該連結ステントを拡張することと
を含む、方法。
(項目9)
近位ハブと、自身を通る管腔とを有するホルダ上に前記心臓弁を載置することと、自身を通る管腔を有するハンドルの遠位端上に該ホルダを載置することとをさらに含む、項目8に記載の方法であって、該方法は、該ハンドルおよび該ホルダの該管腔を通して、前記心臓弁内にバルーンカテーテルを通過させることと、前記連結ステントを拡張するために、該バルーンカテーテル上のバルーンを膨張させることとを含む、方法。
(項目10)
前記心臓弁を包装することをさらに含み、該心臓弁は、前記ハンドルおよび前記バルーンカテーテルとは別に、前記ホルダ上に載置される、項目9に記載の方法。
(項目11)
前記連結ステントの前記収縮状態は、円錐形であり、前記バルーンカテーテル上の前記バルーンは、それの近位拡張端よりも大きい遠位拡張端を有することにより、前記人工弁よりも該連結ステントに大きな拡張偏向を印加する、項目9に記載の方法。
(項目12)
前記連結ステントの前記収縮状態は、円錐形であり、該連結ステントは、複数の半径方向拡張型支柱を備え、そのうちの少なくともいくつかが列を成して配設され、前記人工弁から最も遠い列は、前記収縮状態から前記拡張状態への拡張に対して最大能力を有する、項目8に記載の方法。
(項目13)
前記連結ステントは、複数の半径方向拡張型支柱を備え、前記人工弁から最も遠い列は、交互する頂点と谷間とを有する、項目8に記載の方法であって、該方法が、該連結ステントの他の部分よりも該連結ステントの遠位端をより大きく拡張することを含むことにより、該人工弁から最も遠い列における該頂点が、前記弁輪の中へ外向きに突出する、方法。
(項目14)
前記連結ステントを塑性的に拡張することによって、前記心臓弁輪の開口サイズを1.0〜5mmだけ増加させることを含む、項目8に記載の方法。
(項目15)
前記弁構成要素の前記人工弁は、前記心臓弁輪の前記増加した開口サイズに合致する開口サイズを有するように選択される、項目14に記載の方法。
(項目16)
人工心臓弁を送達するシステムであって、該システムは、
非拡張型で非折畳み式の開口を有する人工弁を含む心臓弁であって、該心臓弁は、自身の流入端から延在する拡張型連結ステントをさらに含み、該連結ステントは、埋込位置に送達するための収縮状態と、拡張状態とを有する、心臓弁と、
該心臓弁の近位端に接続される弁ホルダと、
バルーンを有するバルーンカテーテルと、
該弁ホルダの近位端に付着するように構成され、該カテーテルの通過のための管腔を有するハンドルであって、該バルーンは、該ハンドルを通って該ホルダを越え、該心臓弁を通って遠位に延在する、ハンドルと
を備える、システム。
(項目17)
前記人工弁は、縫製輪を有する市販の弁を備え、前記連結ステントは、前記縫製輪に付着する、項目16に記載のシステム。
(項目18)
前記連結ステントの前記収縮状態は、円錐形であり、遠位方向に先細りにテーパー状である、項目16に記載のシステム。
(項目19)
前記連結ステントの前記収縮状態は、円錐形であり、遠位方向に先細りにテーパー状であり、前記バルーンカテーテルは、略円錐形のノーズコーンを自身の遠位端上にさらに含み、該ノーズコーンは、前記心臓弁を通って延在し、該収縮状態にある連結ステントの遠位端に係合する、項目16に記載のシステム。
(項目20)
前記ハンドルは、直列に連結されて連続管腔を形成し得る近位セクションと遠位セクションとを備え、該遠位セクションは、該近位ハンドルセクションとの接続の前に、前記ホルダのハブに連結して、該遠位セクションを使用した前記心臓弁の手動操作を可能にするように適合される、項目16に記載のシステム。
(項目21)
前記バルーンカテーテルおよび近位ハンドルセクションは、該近位セクションの管腔内の前記バルーンとともに包装される、項目20に記載のシステム。
(項目22)
前記ホルダ上に載置された前記心臓弁は、前記ハンドルおよび前記バルーンカテーテルとは別に包装される、項目16に記載のシステム。
本願の種々の実施形態は、ヒトの心臓の中の欠陥天然弁を置換するための人工弁および使用方法を提供する。ある実施形態は、体外循環(すなわち、バイパスポンプ)を使用して時間を最小化しながら、迅速かつ容易に心臓弁を置換するための外科的手技で使用するために、特によく適合される。
a.流動口を画定する、非拡張型で非折畳み式の弁輪支持構造であって、流出方向に突出する複数の交連柱を含む、支持構造と、
b.支持構造および交連柱に取り付けられ、流動口を横断して交互に開閉するように載置される、可撓性弁尖と、
c.支持構造の流入端を囲む縫合糸透過可能輪と、
d.流動口の周囲に延在する第1端を有し、支持構造の流入端において接続される、塑性拡張型連結ステントであって、弁支持構造から離れて流入方向に突出する第2端を有し、埋込位置への送達のための収縮状態と、弁輪と外側が接触するための第1端よりも幅広い拡張状態とを呈することが可能である、連結ステントと
を備える。
非拡張型で非折畳み式の開口を有する人工弁を含む、心臓弁を提供するステップであって、心臓弁はさらに、その流入端から延在する拡張型連結ステントを含み、連結ステントは、埋込位置に送達するための収縮状態と、弁輪への外向き接続のために構成される拡張状態とを有する、ステップと、
弁輪に隣接する埋込位置まで、その収縮状態である連結ステントとともに心臓弁を前進させるステップと、
弁輪と接触し、それに接続された拡張状態に連結ステントを塑性的に拡張するステップと
を含む、方法。
i.手技の連結ステント配備前段階で使用される、後退バルーン位置
ii.連結ステント配備に使用される前進バルーン位置。前進位置は、いったん心臓弁200が所望の大動脈起始部に配置されると使用され、連結ステントを拡張し、インプラントを適所に固定するためにバルーン拡張が必要とされる。
Claims (29)
- 人工心臓弁を送達するシステムであって、該システムは、
流動口を画定する、非拡張型で非折畳み式の弁輪支持構造を有する人工弁を含む心臓弁であって、該心臓弁は、流入端と、流出方向に突出する複数の交連柱と、該支持構造および交連柱に取り付けられ、該流動口を横断して交互に開閉するように載置される可撓性弁尖とを有し、該心臓弁は、自身の該流入端から延在する拡張型連結ステントをさらに含み、該連結ステントは、埋込位置に送達するための収縮状態と、拡張状態とを有する、心臓弁と、
該心臓弁の該流出側に接続され、かつ、該可撓性弁尖の間に延在するスリーブを有する弁ホルダであって、該ホルダとスリーブとが一緒になって該弁尖の流出側から流入側まで貫通穴を画定する、弁ホルダと、
バルーンを有するバルーンカテーテルと、
該弁ホルダの近位端に付着するように構成され、該カテーテルの通過のための管腔を有するハンドルであって、該管腔は該弁ホルダとスリーブの貫通穴と整合しており、該バルーンは、該ハンドルを通って該ホルダを越え、該心臓弁を通って遠位に延在し、該連結ステントを拡張するように設置される、ハンドルと
を備える、システム。 - 前記人工弁は、自身の前記流入端上に縫合糸透過可能輪を備える、請求項1に記載のシステム。
- 前記連結ステントの前記収縮状態は、円錐形であり、遠位方向に先細りにテーパー状である、請求項1に記載のシステム。
- 前記連結ステントの前記収縮状態は、円錐形であり、遠位方向に先細りにテーパー状であり、前記バルーンカテーテルは、略円錐形のノーズコーンを自身の遠位端上にさらに含み、該ノーズコーンは、前記心臓弁を通って延在し、該収縮状態にある連結ステントの遠位端に係合する、請求項1に記載のシステム。
- 前記ハンドルは、直列に連結されて連続管腔を形成し得る近位セクションと遠位セクションとを備え、該遠位セクションは、該近位ハンドルセクションとの接続の前に、前記ホルダのハブに連結して、該遠位セクションを使用した前記心臓弁の手動操作を可能にするように適合される、請求項1に記載のシステム。
- 前記バルーンカテーテルおよび近位ハンドルセクションは、該近位セクションの管腔内の前記バルーンとともに包装される、請求項5に記載のシステム。
- 前記ホルダ上に載置された前記心臓弁は、前記ハンドルおよび前記バルーンカテーテルとは別に包装される、請求項1に記載のシステム。
- 前記スリーブは前記弁ホルダにネジ止めされた別の要素を備える、請求項1に記載のシステム。
- 前記ホルダ上に載置された前記心臓弁は、前記スリーブとは別に包装される、請求項8に記載のシステム。
- 前記ハンドルは、前記弁ホルダの近位端にスナップ式連結で付着する、請求項1に記載のシステム。
- 前記連結ステントが、前記弁輪支持構造に連続的に縫合された流出端上の補強輪と、そこから延在する複数の相互に接続された拡張型支柱とを備え、複数の該拡張型支柱は、前記バルーンによって前記収縮状態から前記拡張状態へと変形される、請求項1に記載のシステム。
- 前記補強輪が、前記縫合糸透過可能輪の裏面の起伏のある輪郭に概して対応する頂点および谷を伴う起伏のある経路を辿る、請求項11に記載のシステム。
- 人工心臓弁を送達するシステムであって、該システムは、
流動口を画定する、非拡張型で非折畳み式の弁輪支持構造を有する人工弁を含む心臓弁であって、該心臓弁は、流入端と、流出方向に突出する複数の交連柱と、該支持構造および交連柱に取り付けられ、該流動口を横断して交互に開閉するように載置される可撓性弁尖とを有し、該心臓弁は、自身の該流入端から延在する拡張型連結ステントをさらに含み、該連結ステントは、埋込位置に送達するための収縮状態と、拡張状態とを有する、心臓弁と、
該心臓弁の該流出側に接続され、かつ、該弁尖の流出側から流入側まで延在する貫通穴を有する、弁ホルダおよびハンドルシステムと、
バルーンを有するバルーンカテーテルであって、該弁ホルダおよびハンドルシステムの貫通穴を通って遠位に、接触せずに該心臓弁尖を越えて遠位に通過する結果、該バルーンが該連結ステントを拡張するように設置される、バルーンカテーテルと、
を備える、システム。 - 前記連結ステントの前記収縮状態は、円錐形であり、遠位方向に先細りにテーパー状である、請求項13に記載のシステム。
- 前記弁ホルダとハンドルシステムは、前記心臓弁に接続された弁ホルダと、該弁ホルダに接続し、かつ、前記可撓性弁尖の間に延在するスリーブと、該弁ホルダの近位端に付着するように構成され、かつ、前記カテーテルの通過のための管腔を有するハンドルとを備える、請求項13に記載のシステム。
- 前記スリーブは前記弁ホルダにネジ止めされる、請求項15に記載のシステム。
- 前記ホルダ上に載置された前記心臓弁は、前記スリーブとは別に包装される、請求項15に記載のシステム。
- 前記ハンドルは、前記弁ホルダの近位端にスナップ式連結で付着する、請求項15に記載のシステム。
- 前記人工弁は、自身の前記流入端上に縫合糸透過可能輪を備える、請求項13に記載のシステム。
- 前記連結ステントが、前記弁輪支持構造に縫合された流出端上の補強輪と、そこから延在する複数の相互に接続された拡張型支柱とを備え、複数の該拡張型支柱は、前記バルーンによって前記収縮状態から前記拡張状態へと変形される、請求項13に記載のシステム。
- 前記補強輪が、前記縫合糸透過可能輪の裏面の起伏のある輪郭に概して対応する頂点および谷を伴う起伏のある経路を辿る、請求項20に記載のシステム。

該心臓弁の該流出側に接続される弁ホルダと、
該弁ホルダに接続され、そこから該可撓性弁尖内を遠位方向に延在するスリーブであって、貫通穴を有する、スリーブと、
