Abstract

Description
本願は、2016年12月2日出願の米国特許仮出願第62/429,694号、及び2017年3月9日出願の同第62/469,440号の優先権を主張する。これらの出願のそれぞれは、この参照によりその全体が本明細書に組み込まれる。

ドライアイ疾患(「DED」)は、世界中で数百万もの人々に影響を与えている病態である。北米の4000万人を超える人々が、いくつかの形態のドライアイを有しており、更に世界中で数百万人が悩まされている。DEDは、眼の表面上の天然涙膜の破壊による結果であり、眼の不快感、視覚的障害、及び視力に関連する生活の質の低下をもたらし得る。運転、コンピュータ使用、家事、及び読書などの日常生活の活動もまた、DEDによって悪影響を受けることが示されている。DEDの重度の症例を有する患者は、角膜潰瘍などの重篤な眼の健康欠損のリスクがあり、中等度の狭心症に匹敵する生活の質の欠損を経験し得る。
DEDは本質的に進行性であり、基本的には、眼の表面上の不十分な涙液被覆によるものである。この不十分な涙液被覆は、眼表面の健康なガス交換及び栄養輸送を妨げ、細胞の乾燥を促進し、視力のための低屈折性表面を作り出す。不十分な涙液被覆は、典型的には、1)涙腺からの不十分な涙液産生(例えば、閉経後ホルモン欠乏症、自己免疫疾患、LASIK手術など)、及び/又は2)マイボーム腺の機能不全から生じる涙液の過剰な蒸発から生じる。低涙量は、眼表面の炎症状態を誘発する高浸透圧性環境を引き起こす。この炎症反応は、表面細胞のアポトーシスを誘発し、これは次に、眼表面上の涙膜の適切な分布を妨げ、その結果、任意の所与の涙量をもたらす効果が低下する。これにより、更なる炎症が続いて起こり、より多くの表面細胞損傷などを引き起こす可能性のある、悪循環が始まる。加えて、眼の表面内の感覚ニューロンが損傷しているため、反射性の涙の活性化を制御する神経制御ループは破壊される。結果として、より少ない涙が分泌され、疾患の更なる進行をもたらす第2の悪循環が発生する(より少ない涙は神経細胞の損失を引き起こし、これにより、涙などはより少なくなる)。

システム

一例として、ドライアイ予測は、予測された涙液蒸発の量に少なくとも部分的に基づいてよい。予測された涙液蒸発量は、環境及び患者固有の特性など、本明細書に記載される特性、並びに空気と接している涙液の表面積、及び涙液温度の推定値を使用して計算されてよい。判定された特性から予測された涙液蒸発量を推定するための任意の好適な方法が、ドライアイ予測を形成する際に使用されてよい。例えば、Eが蒸発速度(Kg/min)であるとき、Pは周囲温度(kPa)での水の蒸気圧であり、Vは水面上方での空気の速度(m/s)であり、Aは水の表面積(m2)であり、Taは周囲温度(K)であり、Mは水の分子量(18.02g/mol又は無次元)であり、Rは一般ガス定数(8314.5Pa・m3/(kmol・K))であり、φは蒸発係数(Kg/m2*h)であり、Xsは飽和空気中の湿度比(Kgの水/湿潤空気のKg)であり、Xは空気中の湿度比(Kgの水/乾燥空気のKg)であり、蒸発速度は、以下の式のうちの1つに従って推定されてよい。

例えば、図4A〜図4Dは、第1の特性が温度であり、第2の特性が相対湿度であり、第3の特性が圧力であり、第4の特性が風速であるときの予測された涙液蒸発量の例を示している。これらの図は、上の第3の式を使用し、温度、相対湿度、圧力、及び風速の患者に近接した環境特性に基づいた、計算された平均蒸発速度の散布図を示している。
図4A〜図4Bは、第1の地理的位置に対する、平均相対湿度(%)(図4A)及び平均温度(セ氏)(図4B)に対する蒸発速度(g/s)の散布図である。図4C〜図4Dは、第2の地理的位置に対する、平均相対湿度(%)(図4C)及び平均温度(セ氏)(図4D)に対する蒸発速度(g/s)の散布図である。各データポイントは、特定の日に対するものである。涙液の表面積は2cm2であると仮定し、判定された環境特性(温度、相対湿度、圧力、及び風速)は各日での平均値とした。各位置における平均蒸発速度(g/s)の範囲を以下の表1に示す。
各位置における各日のドライアイ予測は、計算された蒸発速度に基づいてよい。例えば、いくつかの変形例では、ドライアイ予測は、蒸発速度に等しいか、又は蒸発速度と正の相関を持つ値であってよく、より高い値はより重いドライアイ症状に対応し、より低い値はより軽いドライアイ症状に対応する。
他の変形例では、予測された涙液蒸発量は、患者に近接した空気の水蒸気圧と涙膜の水蒸気圧との間の関係を使用して、他のモデルに少なくとも部分的に基づいてよい。例えば、患者及び涙膜は、体温、即ち37℃(310.15ケルビン)、に保たれたヒートシンクであると仮定されてよい。患者の局所環境は、温度T及び相対湿度RHを有する、独立したヒートシンクであると仮定されてよい。空気混合は、瞬間的かつ完全であると仮定されてよく、その結果、温かい空気の層は眼の上に形成されない。これらの仮定に基づいて、空気の水蒸気圧(表面に到達する水分子によって及ぼされる圧力)は以下の式に等しく、

別の例として、患者が飛行機で移動している場合、相対湿度は約20%であり得、温度は約20℃であり得、結果として92のドライアイ予測値が得られる。本発明の他の変形例では、完璧な混合気形成を仮定しない、したがって、移行層をモデル化する方法を含む、他の好適な方法を使用して、涙液蒸発量がモデル化されてよいことを理解されたい。本発明のいくつかの変形例では、ドライアイ症状強度に関する患者フィードバックを使用して、判定された特性からのドライアイ予測の形成が精緻化されてよいことも理解されたい。例えば、患者は、自身のドライアイ症状の重症度を(例えば、患者デバイス上の)ユーザインターフェース上で示してよく、ドライアイ症状のより高い評価は、より高いドライアイ予測値(又は以下に記載される調整されたドライアイ予測値)に寄与する。1つの例示的な変形例では、患者は、自身のドライアイ症状の重症度を数値スケール(例えば、1〜5。1は重篤でないこと、5は極めて重篤であることを表す)で評価してよく、評価は、上記のドライアイ予測式(A)(又は以下に記載される調整されたドライアイ予測値の式)を介して得られるドライアイ予測値に対する乗数に相関付けられてよい。

追加的に又は代替的に、刺激は、他の好適な刺激デバイスによって手動で及び/又は自動的に実行されてよい。例えば、電気刺激は、この参照により全体が本明細書に組み込まれる、2012年4月6日出願の「STIMULATION DEVICES AND METHODS」と題された米国特許出願第13/441,806号に記載されているものなど、マイクロ刺激器インプラントによって送達され得る。更に、他の治療は、超音波刺激(例えば、超音波トランスデューサを介して)、化学的刺激、又は任意の好適な刺激を含んでよい。

前記第1の特性及び前記第2の特性に基づいてドライアイ予測を形成し、前記ドライアイ予測に基づいて治療勧告を選択し、コンピュータネットワークを介して前記マシンに接続されているデバイスに前記治療勧告を供給する命令を記憶している、メモリと、を含む、
マシン。 - 前記第1の特性が、前記患者に近接した湿度又は相対湿度である、請求項1に記載のマシン。
- 前記第2の特性が、前記患者に近接した周囲温度である、請求項1に記載のマシン。
