Abstract
Description
本願は、2013年2月4日に出願された米国特許出願第61/760,577号および2013年12月11日に出願された米国特許出願第61/914,648号の利益を主張するものである。これらの特許出願は、参照によりその全体が本明細書に組み込まれる。
ヒトの心臓の関連部分が図1および図2に示される。健康な心臓は、下方尖部38に向かってテーパ状をなす略円錐形状を有する。心臓は、4つの空洞に分けられ、左心房4、右心房26、左心室6、および右心室28を備える。心臓の左側および右側は、中隔30と一般的に呼ばれる壁部により隔てられる。ヒトの心臓の天然僧帽弁2は、左心室6に左心房4を連結する。僧帽弁2は、大動脈弁14などの他の天然心臓弁とは非常に異なる解剖学的構造を有する。
天然僧帽弁が適切に機能することができない場合に、人工弁置換は、適切な機能の修復を補助することが可能である。しかし、比較的円形で堅い弁輪を有する(特に大動脈狭窄症の場合)大動脈弁14と比較すると、僧帽弁輪8は、比較的堅さに劣り、より不安定であり得る。結果として、天然僧帽弁輪8に対して押圧される人工弁の外方表面の径方向力による摩擦のみに依存することによって、主として大動脈弁用に設計された人工弁を天然僧帽弁輪8内に固定することは、不可能となり得る。したがって、本明細書において説明される人工弁は、天然僧帽弁輪8内に人工弁を固定するために、径方向摩擦力の代わりにまたはそれに加えて、心室固定部に依存し得る(例えば図23を参照)。
天然僧帽弁領域への送達および植え込みのための以下で説明される様々な方法および装置は、人工弁100に関して説明されるが、同様の方法および装置が、弁構造体104を伴わないフレーム102または他のプロテーゼ装置などの人工弁100の構成要素を送達するおよび/または植え込むために使用され得る点を理解されたい。
僧帽弁領域に人工弁100などの圧縮されたプロテーゼ装置を送達するための1つの技術は、左心室6から天然僧帽弁領域にアクセスすることを含み、一例は、経心尖アプローチである。代替的には、左心室6へのアクセスは、大動脈弁14を通して行われ得る。経心尖アプローチでは、左心室6へのアクセスは、図57に示すように胸部の切開部および心尖38の切開部を通して行われ得る。経心尖送達システムは、経心尖アプローチと共に使用され得る。
代替的には、人工弁100は、左心房4から天然僧帽弁領域に送達され得る。図63〜図67を参照すると、僧帽弁領域の心房側から人工弁を送達するための1つのアプローチは、送達カテーテル2100を使用する。初めに、人工弁100は、拡張状態から径方向圧縮状態に圧縮され、図63に示すように送達カテーテル2100の遠位端部分にて一次シース2102内に、および任意にはさらに二次シース内に装填される。送達カテーテル2100は、身体をとして左心房4内に人工弁100を案内するために使用される。人工弁100は、人工弁100の流出端部112(心室固定部126を支持する端部)がシースの遠位端部に最も近く、したがって初めに左心房4に進入し、人工弁の流入端部110(心房封止部材124)が最後に進入するように、シース2102内に配向される。次いで、シース2102は、様々な様式で左心房4に挿入され得る。一例が、図66に示す経心房アプローチであり、別の例が、図67に示す経中隔アプローチである。送達カテーテル2100が、例えば図67に示すような患者の血管系を経由して心臓にアクセスするために使用される場合には、カテーテル2100は、可撓性の操縦可能なカテーテルを備えることが可能である。
僧帽弁逆流(MR)は、天然僧帽弁が適切に閉じることができず、血液が心臓収縮の収縮期に左心室から左心房に流入する場合に発生する。MRは、心臓弁膜症の最も一般的な形態である。MRは、弁尖逸脱、機能不全の乳頭筋、および/または左室の拡張の結果として生じる僧帽弁輪の伸張などの種々の原因を有する。弁尖の中心部分におけるMRは、中心ジェットMRと呼ばれ、弁尖のある交連部により近いMRは、偏心ジェットMRと呼ばれ得る。
人工弁およびプロテーゼスペーサの両方を備える心室固定部を備えるプロテーゼデバイスのいくつかの実施形態は、心室固定部が天然僧帽弁尖10、12のA2/P2領域の背後で固定する代わりにまたはそれに加えて、天然僧帽弁2の交連部36の下方で固定するように構成され得る。図84〜図87は、天然僧帽弁2の2つの交連部36の下方で固定する心室固定部を備える例示のプロテーゼデバイス実施形態と、関連する送達方法とを示す。これらの交連部固定コンセプトは、主に人工弁と共に使用することを目的とされるが、プロテーゼスペーサを含む他のプロテーゼデバイスと共に使用することも可能である。
本明細書で開示されるプロテーゼデバイスのいくつかの実施形態は、本体の心房端部から径方向に外方に延在する心房部分を備えるが、他の実施形態は備えない。上記で説明したように、人工僧帽弁の本体の心房端部から径方向に延在する心房部分は、複数の利点をもたらし得る。心房部分は、僧帽弁輪の心房側に天然組織が位置する状態で完全に環状の接触エリアまたはシールを形成することにより、人工弁の外部と天然弁組織との間における血流を防止するまたは低減させることが可能である。これは、弁傍漏出を減少させるのを補助し得る。また、心房部分は、左心室への移動を防ぐように人工弁を保持する役割を果たすことも可能である。また、心房部分は、組織内部成長を促進することが可能であり、これは、弁傍漏出をさらに減少させ、人工弁の保持を向上させることが可能である。
図1〜図4を参照すると、ヒトの心臓の天然僧帽弁2は、他の天然心臓弁とは非常に異なる構造を有し、弁輪部分8と、前方弁尖10と、後方弁尖12と、後内側乳頭筋22および前外側乳頭筋24に弁尖10および12を繋ぎ留める腱索16とを備える。また上記のように、腱16は、領域A1、A3、P1、およびP3で弁尖10および12に付着して、領域A2およびP2は、腱付着点から比較的自由な状態に留める。これらの解剖学的特徴は、天然僧帽弁2の近位へのプロテーゼの送達に望ましいアプローチ経路を提供する。さらに、これらの解剖学的特徴により、プロテーゼのおよび腱16の固定部間の干渉の可能性を低下させるためには、A1領域、A3領域、P1領域、またはP3領域よりもA2領域およびP2領域にプロテーゼを固定することが望ましくなる。
(図47および図48に示すものなどの、本体が心室固定部と一体的には形成されないフレームを有する実施形態の場合に該当し得るように)本体に対して独立的に拡張可能ではない固定部を有するプロテーゼデバイスの実施形態については、固定部を強制的に拡張させるための機構を有する送達システムが使用され得る。図141〜図156には、固定部8052、本体8054、および心房部分8056を有する、ならびに人工弁8050の構成要素を強制的に拡張させるための機構を備える、例示の人工僧帽弁8050(図151を参照)を送達するおよび植え込むように構成された拡張支援送達システム8000の一実施形態が示される。本明細書では、送達システム8000は、例示の人工弁8050と経心尖アプローチによる送達とに関連して説明されるが、同様のシステムが、代替的な送達アプローチにより天然僧帽弁領域におよび/または代替的な展開シーケンスにより代替的なプロテーゼを送達するために使用され得る点を理解されたい。例えば、図101A〜図140で説明される心房部分実施形態のいずれかを有するプロテーゼは、送達システムおよび関連する送達方法に対する適切な変更を伴いつつ送達システム8000と共に使用され得る。したがって、図101A〜図140の心房部分実施形態は、説明の簡素化のために以下では例示の人工弁8050により一括して示される。

Claims (19)
- 植え込み可能な人工弁であって、
貫通するルーメンを画成する環状の本体、
前記本体の心室端部部分に結合された少なくとも1つの心室固定部、および
前記本体に結合されるとともに前記本体から離れるように径方向に延在し、蛇行状セグメントまたはコイル状セグメントを少なくとも1つが備える複数の径方向延在のアームを備える心房部分
を備える、径方向に収縮可能および径方向に拡張可能な環状のフレームと、
前記フレームの内部に支持される弁部材と、
を備える、植え込み可能な人工弁。 - 前記アームの少なくとも1つは、複数の実質的に直線状の平行セグメントを備える蛇行状セグメントを備える、請求項1に記載の植え込み可能な人工弁。
- 前記アームの少なくとも1つは、複数の実質的に湾曲状の部分を備える蛇行状セグメントを備える、請求項1に記載の植え込み可能な人工弁。
- 前記アームの少なくとも1つは、蛇行状セグメントを備え、前記蛇行状セグメントが、前記本体の近位のアームの部分に複数の実質的に直線状の平行セグメントを備えるとともに、前記アームの末端部分に複数の実質的に湾曲状の部分を備える、請求項1に記載の植え込み可能な人工弁。
- 前記アームの少なくとも1つは、前記アームの末端部分から前記本体の近位のアームの部分にかけて増大する厚さを有する蛇行状セグメントを備える、請求項1に記載の植え込み可能な人工弁。
- 前記複数のアームは、金属セグメントが隣接し合うアームを相互連結することなく、相互に独立して前記本体に連結される、請求項1に記載の植え込み可能な人工弁。
- 各アームは、湾曲状または円形の要素を備える自由端部分を有する、請求項1に記載の植え込み可能な人工弁。
- 前記湾曲状または円形の要素は、前記本体に向かって径方向内方に向く2つの末端部分を備える馬蹄形状要素を備える、請求項7に記載の植え込み可能な人工弁。
- 前記2つの末端部分はそれぞれ、貫通するように形成された穴を有するループを備える、請求項8に記載の植え込み可能な人工弁。
- 各アームは、前記本体に対しておよび隣接するアームに対して可撓性を有する単一金属ワイヤまたはコイルを備える、請求項1に記載の植え込み可能な人工弁。
- 前記アームの少なくとも1つは、コイル状セグメントを備える、請求項1に記載の植え込み可能な人工弁。
- 天然僧帽弁が後方天然弁尖および前方天然弁尖を有する、心臓の天然僧帽弁の領域にプロテーゼ装置を植え込むための方法であって、
送達装置の遠位端部分内の前記天然僧帽弁の領域に前記プロテーゼ装置を送達するステップと、
前記プロテーゼ装置の心室固定部を露出させるために前記送達装置の外方シースを後退させるステップと、
前記送達装置から離れるように径方向に前記心室固定部を強制的に拡張するステップと、
前記心室固定部が天然僧帽弁尖の背後に移動するように、前記天然僧帽弁を通して前記プロテーゼ装置を前進させるステップと、
前記送達装置に向かって径方向に前記心室固定部を収縮させるステップと、
前記送達装置の内方シースを後退させ、それにより前記プロテーゼ装置の本体が前記天然僧帽弁内で径方向に拡張するのを可能にするステップと、
を含む、方法。 - 前記外方シースを後退させるステップは、前記心室固定部を強制的に拡張させる固定部スプレッダを露出させる、請求項12に記載の方法。
- 前記固定部スプレッダは、前記内方シースに結合され、前記外方シースを後退させるステップは、前記固定部スプレッダが前記内方シースから径方向に弾性的に延在するのを可能にし、それにより前記心室固定部を強制的に拡張させる、請求項13に記載の方法。
- 前記内方シースを後退させるステップは、前記内方シースを部分的に後退させ、それにより、前記本体が拡張される前に、前記プロテーゼ装置の心房部分が径方向に拡張するのを可能にするステップを含む、請求項12に記載の方法。

- 患者の心臓の天然僧帽弁の領域に配向デバイスを導入するステップと、

心エコー図検査を利用して前記天然僧帽弁の領域内の前記配向デバイスの前記エコー発生アームを視認するステップと、
前記天然僧帽弁のA2領域およびP2領域に整列するように前記配向デバイスのエコー発生アームを配向するステップと、
前記エコー発生アームが前記A2領域および前記P2領域に整列されるときに、前記配向デバイスを通って延びるラインに沿って、蛍光透視鏡の蛍光透視鏡軸を整列させるステップと、
前記患者の心臓から前記配向デバイスを除去するステップと、
前記天然僧帽弁の領域にプロテーゼ装置を導入するステップと、
前記A2領域およびP2領域の一方の天然僧帽弁尖の中の1つの背後に前記プロテーゼ装置の固定部を位置決めするステップであって、前記固定部は前記蛍光透視鏡で視認可能である、ステップと、
を含む、方法。 - 前記エコー発生アームは、前記配向デバイスのシャフトの遠位端部分から離れるように径方向に延在する2つのエコー発生アームを備える、請求項17に記載の方法。
- 前記シャフトの遠位端部分は、中に配設された第1の孔および第2の孔を有する蛍光透視マーカバンドをさらに備え、
