Abstract
Description
本願は、米国仮特許出願第61/139,398号(2008年12月19日出願)の米国特許法第119条第(e)項の優先権を主張する。
本発明は、概して、身体流路に埋め込むための人工弁に関する。より具体的には、本発明は、現在の弁よりも少ない時間で外科的に埋め込まれるように構成される人工心臓弁に関する。
心臓弁輪に隣接する埋込み位置まで基礎ステントを前進させるステップと、
基礎ステントを心臓弁輪に固着するステップと、
拡張不可能で折畳み不可能な開口を有する、人工弁を含む弁構成要素を提供するステップであって、弁構成要素は、その流入端から延在する拡張可能連結ステントをさらに含み、連結ステントは、埋込み位置に送達するための収縮状態と、基礎ステントへの外向き接続のために構成される拡張状態とを有する、ステップと、
基礎ステントに隣接する埋込み位置まで、その収縮状態である連結ステントとともに弁構成要素を前進させるステップと、
基礎ステントと接触し、それに接続された拡張状態に連結ステントを拡張するステップと
を含む。
Claims (27)
- 人工心臓弁システムであって、
心臓弁輪に対して固着するように適合され、自身の中に開口を画定する基礎ステントと、
人工弁を含む弁構成要素であって、該人工弁は、自身の中に拡張不可能で折畳み不可能な開口を画定し、該弁構成要素は、自身の流入端から延在する拡張可能連結ステントをさらに含み、該連結ステントは、埋込み位置への送達のための収縮状態と、該基礎ステントへの外向きの接続のために構成される拡張状態とを有する、弁構成要素と
を備える、人工心臓弁システム。 - 前記基礎ステントは、拡張可能であり、心臓弁輪に隣接する埋込み位置への送達のための収縮状態と、該心臓弁輪に接触し、該心臓弁輪に対して固着するようにサイズ決定された拡張状態とを有する、請求項1に記載のシステム。
- 前記基礎ステントは、塑性的に拡張可能である、請求項2に記載のシステム。
- 前記連結ステントは、塑性的に拡張可能である、請求項1に記載のシステム。
- 前記人工弁は、縫製輪を有する市販の弁を備え、前記連結ステントは、該縫製輪に取り付く、請求項1に記載のシステム。
- 前記連結ステントの前記収縮状態は、円錐形であり、遠位方向に先細りになる、請求項1に記載のシステム。
- 前記連結ステントは、複数の半径方向に拡張可能な支柱を備え、該支柱のうちの少なくともいくつかが複数の列を成して配設され、最遠位の列は、前記収縮状態から前記拡張状態への拡張に対して最大能力を有する、請求項6に記載のシステム。
- 前記連結ステントは、複数の半径方向に拡張可能な支柱を備え、前記人工弁から最も遠い列は、交互する頂点と谷間とを有し、前記基礎ステントは、開口を含み、該連結ステントの頂点が該開口の中に突出して、該2つのステントを相互に係止し得る、請求項1に記載のシステム。
- 前記基礎ステントは、軸方向に配向した支柱の間に複数の半径方向に拡張可能な支柱を含み、該軸方向に配向した支柱のうちの少なくともいくつかは、該ステントの周囲の場所の境界を定める上部突起を有する、請求項1に記載のシステム。
- 人工心臓弁システムの送達および埋込みの方法であって、
基礎ステントを心臓弁輪に隣接する埋込み位置まで前進させることと、
該基礎ステントを該心臓弁輪に固着することと、
人工弁を含む弁構成要素を提供することであって、該人工弁は、拡張不可能で折畳み不可能な開口を有し、該弁構成要素は、自身の流入端から延在する拡張可能な連結ステントをさらに含み、該連結ステントは、埋込み位置への送達のための収縮状態と、該基礎ステントへの外向き接続のために構成される拡張状態とを有する、ことと、
該基礎ステントに隣接する埋込み位置まで、収縮状態にある該連結ステントとともに該弁構成要素を前進させることと、
該基礎ステントと接触した状態で、それに接続される該拡張状態まで該連結ステントを拡張することと
を含む、方法。 - 前記基礎ステントは、塑性的に拡張可能であり、
収縮状態にある該拡張可能な基礎ステントを前記埋込み位置まで前進させることと、
前記心臓弁輪の開口サイズを少なくとも10%だけ増加させる過程において、該心臓弁輪に接触し、それに固着した拡張状態まで該基礎ステントを塑性的に拡張することと
をさらに含む、請求項10に記載の方法。 - 前記弁構成要素の前記人工弁は、前記心臓弁輪の前記増加した開口サイズに合致する開口サイズを有するように選択される、請求項11に記載の方法。
- 機械的拡張器上に前記基礎ステントを載置することと、該機械的拡張器を使用して前記心臓弁輪において該基礎ステントを配備することとをさらに含む、請求項11に記載の方法。
- 近位ハブと、自身を通る管腔とを有するホルダ上に前記弁構成要素を載置することと、ハンドルの遠位端に該ホルダを載置することであって、該ハンドルは自身を通る管腔を有する、こととをさらに含み、該方法は、該ハンドルの管腔および該ホルダを通して、該弁構成要素内においてバルーンカテーテルを通過させることと、該バルーンカテーテル上のバルーンを膨張させて、該連結ステントを拡張することとを含む、請求項10に記載の方法。
- 前記ハンドルおよび前記バルーンカテーテルとは別に、前記ホルダ上に載置された前記弁構成要素を包装することをさらに含む、請求項14に記載の方法。
- 前記連結ステントの前記収縮状態は、円錐形であり、前記バルーンカテーテル上の前記バルーンは、それの近位拡張端よりも大きい遠位拡張端を有することによって、前記人工弁によりも該連結ステントに、より多大な拡張偏向を印加する、請求項14に記載の方法。
- 前記連結ステントの前記収縮状態は、円錐形であり、該連結ステントは、複数の半径方向に拡張可能な支柱を備え、該支柱のうちの少なくともいくつかが列を成して配設され、該人工弁から最も遠い列は、該収縮状態から前記拡張状態への拡張に対して最大能力を有する、請求項10に記載の方法。
- 前記連結ステントは、複数の半径方向に拡張可能な支柱を備え、前記人工弁から最も遠い列は、交互する頂点と谷間とを有し、前記方法は、該連結ステントの遠位端を該連結ステントの他の部分よりも拡張することにより、該人工弁から最も遠い列における該頂点が、前記基礎ステントの開口の中へ外向きに突出する、こととを含む、請求項10に記載の方法。
- 前記基礎ステントおよび前記連結ステントの両方は、複数の軸方向に配向した支柱の間に複数の半径方向に拡張可能な支柱を有し、前記方法は、該連結ステントを配向することにより、それの軸方向に配向した支柱が、該基礎ステントの支柱との位相がずれて、その間での保持を増加させる、ことを含む、請求項10に記載の方法。
- 前記基礎ステントを塑性的に拡張することによって、前記心臓弁輪の開口サイズを1.5乃至5mmだけ増加させることを含む、請求項10に記載の方法。
- 人工心臓弁を送達するシステムであって、
人工弁を含む弁構成要素であって、該人工弁は、前記拡張不可能で折畳み不可能な開口を有し、該弁構成要素は、自身の流入端から延在する拡張可能な連結ステントをさらに含み、該連結ステントは、埋込み位置への送達のための収縮状態と、拡張状態とを有する、弁構成要素と、
該弁構成要素の近位端に接続される弁ホルダと、
バルーンを有するバルーンカテーテルと、
該弁ホルダの近位端に取り付くように構成され、該カテーテルの通過のための管腔を有するハンドルであって、該バルーンは、該ハンドルを通り、該ホルダを越え、該弁構成要素を通って遠位に延在する、ハンドルと
を備える、システム。 - 前記人工弁は、縫製輪を有する市販の弁を備え、前記連結ステントは、該縫製輪に取り付く、請求項21に記載のシステム。
- 前記連結ステントの前記収縮状態は、円錐形であり、遠位方向に先細りになる、請求項21に記載のシステム。
- 前記連結ステントの前記収縮状態は、円錐形で、遠位方向に先細りになり、前記バルーンカテーテルは、自身の遠位端に略円錐形のノーズコーンを含み、前記弁構成要素を通って延伸し、収縮状態にある該連結ステントの遠位端に係合する、請求項21に記載のシステム。
- 前記ハンドルは、連続管腔を形成するようにともに直列に連結され得る近位セクションと遠位セクションとを備え、該遠位セクションは、該近位ハンドルセクションとの接続の前に前記ホルダのハブに連結して、該遠位セクションを使用する前記弁構成要素の手動操作を可能にするように適合される、請求項21に記載のシステム。
- 前記バルーンカテーテルおよび近位ハンドルセクションは、前記近位セクション管腔内の前記バルーンとともに包装される、請求項25に記載のシステム。
- 前記ホルダ上に載置された前記弁構成要素は、前記ハンドルおよび前記バルーンカテーテルとは別に包装される、請求項21に記載のシステム。
