Abstract
Description
本発明は、体内の2つの管腔構造の間に吻合を形成することができる、内視鏡検査または内視鏡的超音波検査の誘導下での経管的診断用デバイスに関する。さらに具体的には、固着し、流体導管を形成し、拡張可能な中空ステントを使用して管腔構造に接近するためのデバイスおよび方法を記載する。
中央部材は、管腔構造の液体間の漏洩、および腹腔または他の解剖学的空間の液体を抑制する流体導管を提供するために中央部材を密閉するように設計され、それに配置される薄い被覆または膜4160を有してもよい。これは図24に示される。これは、中央部材およびアンカが完全に展開され、有意な液体の喪失なく1つの管腔構造から別の管腔構造へ液体が通過し得るように、安定した流体導管が確立されるときに好適である。被覆4160は、中央部材が拡張し、縮小するときに、拡張または縮小することができる弾性被覆であってもよい。中央部材が拡張可能なステントの場合は、被覆は、ステントの編組間の隙間を密閉し、それによってステントの外側を密閉し、ステントの内側の直径を液体導通可能にさせるように設計されなければならない。
Claims (25)
- 第一および第二の管腔構造の間に吻合を形成する装置であって、
遠位端および近位端を有する中空の中央部材と、
該中央部材の該遠位端に接続され、該第一の管腔構造の壁および該第二の管腔構造の壁を貫通し、該第二の管腔構造の中または周辺に配置されるように適合される、遠位アンカと、
該中央部材の該近位端に接続され、該第一の管腔構造の中または周辺に配置されるように適合される、近位アンカと
を備え、
該遠位アンカと該近位アンカとは、相互に向かって移動し、該2つの管腔構造に接近することが可能である、装置。 - 前記遠位アンカは、第一の状態から増大した半径を有する拡張した第二の状態に拡張させられることが可能であり、前記中央部材は、組織を貫通するように適合される貫通部材の通過を許容するように適合され、該遠位アンカは、一端において該中央部材の遠位部に接続される第一および第二の端部を有する、第一の実質的に直線状の脚部材を含み、該第一の状態から拡張状態への拡張は、該脚部材の遠位半径の増大を引き起こすように、該実質的に直線状の脚部材を半径方向に屈曲させることを含む、請求項1に記載の装置。
- 前記近位アンカは、第一の状態から増大した半径を有する第二の状態に拡張させられることが可能であり、該近位アンカは、一端において前記中央部材の近位部に接続される第一および第二の端部を有する、第一の実質的に直線状の脚部材を含み、
該第一の状態から第二の状態への拡張は、該脚部材の遠位半径の増大を引き起こすように、該実質的に直線状の脚部材を半径方向に屈曲させることを含む、請求項2に記載の装置。 - 前記中空の中央部材は、前記第一と第二の管腔構造との間に流体連通路を提供するように適合され、該中央部材は、第二のより大きい直径に拡張することが可能である、第一の直径を有する、請求項3に記載の装置。
- 前記装置は、前記第一および第二の管腔構造から取り外し可能である、請求項4に記載の装置。
- 前記第一の管腔構造から前記第二の管腔構造への流体流動を制限するように構成される、前記中央部材の前記近位端に接続される弁部材をさらに備える、請求項4に記載の装置。
- 遠位管腔構造と近位管腔構造との間に流体導管を形成するための方法であって、
該遠位管腔構造の中または周辺に、遠位保持部材を有する中央部材の一部を配置することと、
該近位管腔構造の中または周辺に、近位保持部材を有する該中央部材の一部を配置することと、
該管腔構造の一部が、相互に密接に接近して保持されるように、該遠位と近位の保持部材とを相互に向かって引き寄せることと
を含む、方法。 - 第一の倒壊状態から増大した半径を有する第二の拡張状態まで、前記遠位保持部材を拡張することをさらに含み、前記中央部材は、組織を貫通するように適合される貫通部材の通過を許容するように適合され、該遠位保持部材は、一端において該中央部材の遠位部分に接続される第一および第二の端部を有する、第一の実質的に直線状の部材を含み、
該拡張することは、前記実質的に直線状の脚部材を半径方向に屈曲させることにより、該脚部材の遠位半径の増大を引き起こすことを含む、請求項7に記載の方法。 - 第一の倒壊状態から増大した半径を有する拡張状態まで、前記近位管腔構造の少なくとも一部の中の前記近位保持部材を拡張することをさらに含み、
該近位保持部材は、一端において前記中央部材の近位部分に第一および第二の端部を有する、第一の実質的に直線状の部材を備え、
該拡張することは、前記実質的に直線状の脚部材を半径方向に屈曲させることにより、該脚部材の近位半径を増大させることを含む、請求項8に記載の方法。 - 前記近位保持部材は、一端において前記中央部材の近位部分に接続される第一および第二の端部を有する、少なくとも1つの第一の実質的に直線状の部材を含み、
前記拡張することは、前記実質的に直線状の脚部材を半径方向に広げることにより、該脚部材の近位半径の増加を引き起こすことを含む、請求項8に記載の方法。 - 前記遠位保持部材と近位保持部材とを相互に向かって引き寄せることは、前記中央部材の2つの端部を相互に向かって移動させ、それにより、該中央部材の長さを短縮することを含み、該中央部材は、2つの同心の中空管を備え、該短縮することは、一方の中空管を他方の中空間の内側で摺動させることを含む、請求項9に記載の方法。

- 遠位および近位の管腔構造から前記流体導管を除去することをさらに含む、請求項7に記載の方法。
- 第一と第二の管腔構造との間に吻合を形成するための方法であって、
該管腔構造の間に被覆導管を配置することと、
該遠位の管腔構造に該導管の遠位端を少なくとも一時的に固着し、該近位の管腔構造に該導管の近位端を少なくとも一時的に固着することと、
該管腔構造を接近させ、それらの間に流体チャネルを形成するように該導管の長さを短縮することと
を含む、方法。 - 前記遠位管腔構造に前記導管の遠位端を固着することは、該導管の該遠位端部分の周辺に配置される遠位アンカ部材の半径を拡張させることを含み、該導管は、第一の崩壊状態から第二の拡張状態まで、一端において該導管の遠位部分に第一および第二の端部が接続される、少なくとも1つの実質的に直線状の部材を有し、該拡張させることは、該実質的に直線状の脚部材を半径方向に屈曲させることにより、該脚部材の遠位半径の増大を引き起こすことを含む、請求項16に記載の方法。
- 前記近位管腔構造に前記導管の近位端を固着することは、該導管の該近位端部分の周辺に配置される近位アンカ部材の半径を拡張させることを含み、該導管は、第一の崩壊状態から第二の拡張状態まで、一端において該導管の近位部分に第一および第二の端部が接続される、少なくとも1つの実質的に直線状の部材を有し、該拡張させることは、該実質的に直線状の脚部材を半径方向に屈曲させることにより、該脚部材の遠位半径の増大を引き起こすことを含む、請求項17に記載の方法。
- 前記導管が自己拡張できるように、収縮性シースを引き出すことにより該導管の直径を拡張させることをさらに含み、該導管は、被覆ステントまたは被覆編組管である、請求項16に記載の方法。
- 前記直径を拡張させることは、膨張性バルーンを該導管に挿入し、該導管の内側で該バルーンを膨張させることを含む、請求項16に記載の方法。
- 熱エネルギーを前記導管に適用することによって、該導管の直径を拡張させることをさらに含み、該導管は、該熱エネルギーに応じて直径が拡張する形状記憶合金を含む、請求項16に記載の方法。
- 管腔を形成し、第一と第二の中空本体との間に流体経路を形成するためのシステムであって、該システムは、遠位および近位の端部分を有する中空の中央部材と、該遠位端部分に連結される遠位保持部材と、該近位端部分に連結される近位保持部材とを備えており、該遠位保持部材が該第二の中空本体の内側に固着するように適合され、該近位保持部材は、該第一の中空本体の内側に固着するように適合され、それにより、軸方向の圧縮力に応じて、該中央部材の長さが減少し、該管腔の直径が拡張するときに、該第一と第二の中空本体とが相互に向かって引き寄せられる、システム。

- 前記中央部材は、組織を貫通するように適合される貫通部材の通過を可能にするように適合され、前記遠位保持部材は、一端において該中央部材の遠位部分に連結される第一および第二の端部を有する、第一の実質的に直線状の脚部材を含み、前記第一の状態から拡張状態への拡張は、該脚部材の遠位半径の増大をもたらすように、該実質的に直線状の脚部材を半径方向に屈曲させることを含む、請求項23に記載の装置。
- 前記近位保持部材は、第一の状態から増大した半径を有する第二の拡張状態に拡張させられることが可能であり、
該近位保持部材は、一端において前記中央部材の近位部分に連結される第一および第二の端部を有する、第一の実質的に直線状の脚部材を含み、該第一の状態から第二の拡張状態への拡張は、該脚部材の遠位半径の増大を引き起こすように、該実質的に直線状の脚部材を半径方向に屈曲させることを含む、請求項24に記載の装置。
