Abstract
Description
イコトリエンD4拮抗薬、5リポキシゲナーゼ阻害薬、もしくは5リポキシゲナーゼ
活性化タンパク質(FLAP)拮抗薬でありうる物質を含む、新規な医薬の組み合わせ
の組成物に関連する。組み合わせは、アレルギー性及び/もしくは血管神経性の
鼻炎又はアレルギー性結膜炎の局所的な治療の為に働く。当該抗ヒスタミン薬は
、充血、かゆみ、もしくは腫大として現れる急性症状の急速な除去を供すこの症
状の基となる炎症は、この組み合わせに伴うロイコトリエン拮抗薬を使用するこ
とでうまくコントロールされる。
び青年の平均7.5%が鼻・結膜炎(目の症状と結びつく枯草熱)を患っていると調査
が示している(Worldwide variation in prevalanace of symptoms of asthma,al

32,1998)。西ヨーロッパにおいて、流行は14%であり、より著しく高い(Annesi-M

応という意味深い炎症過程であるという認識である。ヒスタミンはなおも、早期
の最も重要な伝達物質として、及び充血、すすり、かゆみ及び分泌過多、鼻漏や
流涙のような症状の重要な引き金であるとみなされており、そのうえ、ロイコト
リエンのような伝達物質は、鼻詰まり、分泌中に及び炎症の進行(例えば、前炎
症性細胞の誘引、細胞浸潤の促進など)に連坐される。それゆえ、治療の狙いが
対症療法から、アレルギー性疾患の基となる炎症の作用による付加的な抗炎症性
の治療に移ってきている。ヒスタミンおよびロイコトリエンはアレルギー性の早
期及び晩期において放出される。
第1及び第2世代の古典的な抗ヒスタミンの助けにより容易にコントロールできる
。しかしながらそれらは、疾患の基となり、たいてい進行性である、炎症に対す
る治療に関連する作用をほとんど持たない。しばしば、アレルギー性疾患(鼻・
結膜炎症状)は、患者によって及び医師の双方よって些細な疾患とみなされ、結
果的には不十分な治療しかされない。いわゆる段階の変化が起こり、すなはち、
比較的害のない鼻炎から発展して重病として扱われる気管支喘息がもたらされる
。この理由のために、アレルギー性の鼻・結膜炎でさえ、充分かつ徹底的な治療
が不可欠である。そのときにだけ、患者は症状がなく生活でき、そのときにだけ
一定の環境の下で生命を脅かす段階の変化を防ぐことができる。

独立して−マスト細胞もしくは塩基性顆粒の脱顆粒及び細胞内の貯蔵が空である
ことが目立った。これは細胞内もしくは外カルシウムによりコントロールされる
プロセスである。

シゲナーゼの阻害に加えて、5-リポキシゲナーゼ活性化タンパク質(FLAP)は又ロ
イコトリエンの合成を導くことができる。5-リポキシゲナーゼの阻害に加えて、
5-リポキシゲナーゼ活性化タンパク質(FLAP)の阻害も又、ロイコトリエンの合性
阻害を導くことができる。数あるLT拮抗薬の中で、いくつか、ザフィルルカスト
(zafirlukast)、モンテルカスト(montelukast)、プランルカスト(pranlukast)な
どは気管支喘息における治療に使用されている。5-LOX阻害薬のジリュートン(zi
leuton)は既に市場にある。いわゆるFLAP阻害薬など、例えばMK-591、Bay×1005

リエンは鼻詰まりに対してはるか更に重要であるようだ。

レルギー反応の後6−8時間持続する。ヒスタミンとは対照的にすすり及びかゆみ
がLT刺激の後は起こらない(Okuda,M.,T.watase A.Mazewa,C.M.Liu.The role of

ら、LTD4による刺激の後は、好酸性顆粒球の長く持続する浸潤があり、たいてい
の場合のそれはアレルギー性炎症の原因である(Fujika,M et al. see Above)。
これらいわゆる晩期反応(例えば 鼻詰まり)はザフィルカストのようなLT拮抗を

て為されるそれらの作用を披露することができる。
あれば充血除去剤、抗アレルギー薬、ムコリティクス(mucolytics)、ノノピオイ
ド(nonopioid)、鎮痛薬、リポキシゲナーゼ阻害薬並びにロイコトリエンレセプ
ター拮抗薬と一緒に、ロイコトリエン阻害特性を持つ抗ヒスタミン薬を含むもの
は、EP0780127A1にて開示されており、アレルギー性鼻・結膜炎の治療に宛てて
推薦されている。抗ヒスタミン薬と糖質コルチコイドとの共同作用は治療の効率
性を増加させるだろう。
縮薬、ロイコトリエン阻害薬、抗ヒスタミン薬、抗アレルギー薬、ムコリティク
ス、強壮薬、抗コリン作用薬もしくはノイラミニターゼ阻害薬が加えられた。糖
質コルチコイドの形態の抗炎症剤の用途をWO98/48839が開示する。
組み合わせが提案され、それは非鎮静抗ヒスタミンのロラタジンの代謝産物であ
るデカルボエトキシロラタジン(descarboethoxyloratidine)及び、ロイコトリエ
ンD4拮抗物質であるロイコトリエン拮抗薬、5-リポキシゲナーゼ阻害薬もしくは
FLAP拮抗薬から成る。デカルボエトキシロラタジンの使用はロラタジン及び他の
抗ヒスタミン薬の数多くの所望されぬ副作用を防ぐであろう。

ウム1-(((R)-3-(2-(6,7-ジフロロ-2-キノリニル)エテニル)フェニル)-3-(2-(2-
ヒドロキシ-2-プロピル)-フェニル)チオ)メチル)シクロプロパンアセテート、1-
(((1R)-3-(2-(2,3-ジクロロチエノ[3,2-b]ピリジン-5-イル)-(E)-エテニル)フェ
ニル)-3-(2-(1-ヒドロキシ-1-メチルエチル)フェニル)-プロピル)チオ)メチル
)シクロプロパン酢酸を喘息、アレルギー及び炎症において経口的にもしくは非
経口的に投与することがまた知られている。

独立して局所的(鼻腔内もしくは眼内)又は経口的に本発明に従って付与されうる
。もし独立した製剤が存在すれば、これらは互いに仕上げられ、並びにそれらが
組み合わせでもって存在しうる時と同じ容量及び対応の重量比での服投与単位に
おいて、それぞれの活性化合物を包含する。
方式が相互に相補的であり、又薬物動力学的に同じように振舞うため、強い抗炎
症反応が付随する高い治療上の効率ももたらす。作用の長期の持続は毎日2回の
投与を可能にした。もし活性化合物が固定化された組み合わせの形態で存在すれ
ば、双方の活性化合物が一つの錠剤もしくは一つのコンテナに包含される為、患
者にとって投与が簡素になる。本発明に係る抗ヒスタミン薬の成分の濃度は0.00
1%から0.5%の範囲にありうる。

イコトリエン拮抗薬を個々に、もし適切なら、賦形剤及び/もしくは伸展剤もし
くは補形薬と一緒に好適に混合し、並びにこのようにして得た混合物を最適な投
与形態に転化することによる、慣習の標準的な方法に従って行われる。
医薬の伸展剤、補形薬もしくは賦形剤を医薬的な目的の為に含んでいる。
形態、例えば、鼻のスプレー、鼻滴、点眼薬、錠剤、カプセルまたは微粒子であ
る。活性化合物とは別に、本発明に係る組成は、様々な典型的な製薬の形態成分
である例えば、抗微生物保存剤、浸透圧剤、増粘剤、pH調整もしくは緩衝系のた
めの補形薬を更に含むことができる。

薬はpHの調整の為に使用される。
チルセルロース、エデト酸ナトリウム、塩化ベンザルコニウム及びソルビトール
溶液を逐次加え、次いで攪拌し溶かす。生じた溶液を純水で体積49.5Lにする。1
Nの水酸化ナトリウム溶液を使い溶液のpHを6.0に整える。純水で最終体積を50.0
Lにして攪拌する。孔径0.2μmのフィルター膜を通して溶液を濾し、ボトルに分
注する。
RC 591を高スピードでホモジナイズする。次いで逐次、物質、Polysorbate 80
、ソルビト−ル溶液、エデト酸ナトリウム及び塩化ベンザルコニウムを攪拌し溶
かす。その後、活性化合物のモンテルカストを均一な懸濁物が生じる迄高スピー
ドでホモジナイズする。それから、純水で最終体積を50Lに仕立て、更にホモジ
ナイズする。今度は、生じている気泡を取り除く為に、懸濁物を真空にかける。
生じた懸濁物はその後ボトルに分注する。

純水で最終体積を50Lにし仕立て、更にホモジナイズする。今度は、生じている
気泡を取り除く為に、懸濁物を真空にかける。生じた懸濁物はその後ボトルに分
注する。

が示す相乗効果を推理できる。

する。本茶色ノルウェーラットを、2日連続で生理学的食塩溶液中の卵白アルブ
ミン及び水酸化アルミニウムの懸濁物の2度の腹腔注射により活発に感作した。
感作の後3週間、動物の呼吸を維持する為チオペンタールナトリウム麻酔の下で
、カテーテルを動物の気管に直立歩行(orthograde)様式につなぎこんだ。そして
更に、カテーテルを鼻孔の局所灌流に備えて、気管の中に後鼻孔の開口部迄逆行
性式に進めて固定した。鼻の灌流液はこのように鼻孔を通して滴り落ち、フラク
ションコレクターによって受け入れられる。当該試験物質はチロースに浮遊させ
た(モンテルカスト)か、生理学的食塩水に溶かし(アゼラスチン)、アレルゲン刺
激の60分前に腹膜内に注射した。鼻から粘液を洗い流す為、ローラーポンプ(灌
流レート0.5ml/min)を使い鼻孔に30分に渡り灌流した。局所的な適用の場合、当
該試験物質をモル濃度において灌流液に添加し、溶液をアレルギー性刺激の30分
前に鼻の中に灌流した。エバンスブルー(Evans Blue)血しょうマーカー (1ml/動
物 それぞれPBS中の1%強の溶液)を、次いで頚動脈に注射した。次いで灌流にお
ける15分間の休憩があり、その間に流体を収集した。アレルギゲン刺激(負荷)を
、その後60分に渡るPBS中の卵白アルブミン(PBSにおける卵白アルブミン10mg/ml
)の溶液で鼻孔の灌流によって実行し、この間、灌流液はフラクションコレクタ
ーにおいて15分における分画で収集した。試料/動物の全量は5つであった。当該
試料を遠心し、次いでマイクロタイタープレートに(microtiter plate)に固相化
し、デジスキャン光度計(Digiscan photoeter)を使い620nmの波長で測定した。
盲検の値は自動的に差し引かれた。60分の間の反応の推移はAUCプログラムを使
用して計算した。試料群の物質の作用は賦形剤の対照に対して%で計算した。
のようなメッセンジャーの放出の信号として評価される。抗原が接触した後、ア
レルギーのある人々においてさえもこの現象は起こり、増加した流体の分泌及び
鼻の遮断状態によって明示される。
経の透過性について小さな阻害を引き起こす。モンテルカストは同様に、腹腔内
の0.1mg/kg投与において7%阻害のわずかな作用がある。腹腔内に0.01mg/kgのア
ゼラスチンの投与量及び腹腔内に0.1mg/kgのモンテルカストの投与量における組
み合わせ投与は40%の粘膜の血しょう溢出の超相加的な阻害を引き起こした(p<0.
05)。
性を31%阻害した。AWD 23-115、5-LOX阻害薬は、腹腔内の投与量の範囲0.03から
10mg/kgで、投与量に依存する血管神経の浸透性の阻害(37−54%)を引き越した
。
.03μmol/Lの濃度であっても粘膜の血しょう溢出の強い阻害をみせる。5-LOX阻
害薬であるAWD23-115は投与量依存性の様式において、血管神経の透過性を0.3及
び1μmol/lで其々32%及び49%近く阻害する。もし、アゼラスチンが0.003μmol
/lの濃度でAWD 23-115(0.1μmol/l)と組み合わせて与えられれば、粘膜の溢出阻
害は31%である(p<0.05)。
Claims (20)
- 【請求項1】 以下の成分を別々にもしくは一緒に含み、それぞれの用量が
局所的もしくは経口的な投与に最適であって、アレルギー性及び/もしくは血管
神経性の鼻炎又はアレルギー性結膜炎の治療に対する医薬製剤: a)ロラタジン(loratadine)型の化合物を除いた、有効な量の、非鎮静抗ヒスタミ
ン薬もしくはその医薬的に許容される塩の一つ、 b)有効な量の、ロイコトリエン拮抗薬又は、 b1)ロイコトリエンD4拮抗薬もしくは b2)5-リポキシゲナーゼ阻害薬もしくは b3)FLAP拮抗薬 から成る群から選択される医薬的に許容される塩の一つ、 c)慣用の生理学的に許容される賦形剤及び/もしくは伸展剤または補形剤。 - 【請求項2】 非鎮静抗ヒスタミン薬がアゼラスチンのみならずレボカバス
チン、セチリジン、フェキソフェナジン(fexofenadine)、ミゾラスチン、アステ
ミゾールを含んで成る請求項1記載の医薬製剤。 - 【請求項3】 ロイコトリエンD4拮抗薬がモンテルカスト、ザフィルルカス
ト、又はプランルカストを含んで成る請求項1記載の医薬製剤。 - 【請求項4】 5-リポキシゲナーゼ阻害薬がジリュートン、ピリポスト又は
AWD23-115を含んで成る請求項1記載の医薬製剤。 - 【請求項5】 FLAP拮抗薬がMK-591,MK-886もしくはBay×1005を含んで成る
請求項1記載の医薬製剤。 - 【請求項6】 抗ヒスタミン薬の成分について0.001%から0.5%を含む請求項
1及び2記載の医薬製剤。 - 【請求項7】 成分ロイコトリエンD4拮抗薬0.01%から5%を含む請求項1及
び3記載の医薬製剤。 - 【請求項8】 成分5-リポキシゲナーゼ阻害薬0.01%から5%を含む請求項1
及び4記載の医薬製剤。 - 【請求項9】 成分FLAP拮抗薬0.01%から5%を含む請求項1及び5記載の医
薬製剤。 - 【請求項10】 局所的な投与形態における請求項1から8記載の医薬製剤
。 - 【請求項11】 経口的な投与形態における請求項1から8記載の医薬製剤
。 - 【請求項12】 スプレーの局所的な投与形態における請求項1から8及び1
0記載の医薬製剤。 - 【請求項13】 鼻滴もしくは点眼薬の局所的な投与形態における請求項1
から8及び10記載の医薬製剤。 - 【請求項14】 同時、連続又は独立に局所的もしくは経口的に投与する為
のアレルギー性及び/もしくは血管神経性の鼻炎又はアレルギー性結膜炎の予防
及び治療の為、医薬生産の為の、以下の成分を含む医薬の組み合わせの利用: a)ロラタジン型の化合物を除いた、有効な量の、非鎮静抗ヒスタミン薬もしくは
その医薬的に許容される塩の一つ、 b)有効な量の、ロイコトリエン拮抗薬又は、 b1)ロイコトリエンD4拮抗薬もしくは b2)5-リポキシゲナーゼ阻害薬もしくは b3)FLAP拮抗薬 から成る群から選択されるその医薬的に許容される塩の一つ 並びに c)慣用の生理学的に許容される賦形剤及び/もしくは伸展剤または補形剤。 - 【請求項15】 局所的もしくは経口的投与の為のアレルギー性及び/もし
くは血管神経性の鼻炎又はアレルギー性結膜炎の予防及び治療の為の、医薬の製
造の為の前記非鎮静抗ヒスタミンがアゼラスチンのみならずレボカバスチン、セ
チリジン、フェキソフェナジン、ミゾラスチン、アステミゾールを含んで成る、
請求項14記載の医薬の組み合わせの利用。 - 【請求項16】 局所的もしくは経口的投与の為のアレルギー性及び/もし
くは血管神経性の鼻炎又はアレルギー性結膜炎の予防及び治療為の医薬の製造の
為の -ロイコトリエンD4拮抗薬がザフィルルカスト、モンテルカストもしくはプラン
ルカストを含んで成る -5-リポキシゲナーゼ阻害薬がジリュートン、ピリポスト、もしくはAWD23-115を
含んで成る -FLAP拮抗薬がMK-591、MK-886もしくはBay×1005を含んで成る請求項14記載の
医薬物質の組み合わせの利用。 - 【請求項17】 以下の成分を、固定化もしくは遊離した組み合わせにおい
て含んでいる、アレルギー性及び/もしくは血管神経性の鼻炎又はアレルギー性
結膜炎に対して作用を持つ局所的又は経口的投与の為の医薬: a)ロラタジン型の化合物を除いた、有効な量の、非鎮静性抗ヒスタミン薬もしく
はその医薬的に許容される塩の一つ、 b)有効な量の、ロイコトリエン拮抗薬又は、 b1)ロイコトリエンD4拮抗薬もしくは b2)5-リポキシゲナーゼ阻害薬もしくは b3)FLAP拮抗薬 から成る群から選択されるその医薬的に許容される塩の一つ、並びに c)もし適切ならば、慣用の生理学的に無難な賦形剤及び/もしくは伸展剤または
補形剤。 - 【請求項18】 非鎮静抗ヒスタミン薬がアゼラスチンのみならずレボカバ
スチン、セチリジン、フェキソフェナジン、ミゾラスチン、アステミゾールを含
んで成る、請求項17記載の医薬。 - 【請求項19】 -ロイコトリエンD4拮抗薬がザフィルルカスト、モンテル
カストもしくはプランルカストを含んで成り、 -5リポキシゲナーゼ阻害薬がジリュートン、ピリポスト、もしくはAWD23-115を
含んで成り、 -FLAP拮抗薬がMK-591、MK-886もしくはBay×1005を含んで成る 請求項17記載の医薬。 - 【請求項20】 抗ヒスタミン薬及びロイコトリエン拮抗薬を独立に又は一
緒に、もし適切であれば賦形剤もしくは伸展剤と一緒に加工し、そしてこのよう
にして得られた混合物を適切な投与形態に転換することを特徴とする、請求項1
から13及び17から19記載の医薬の生産の為の方法。
