Abstract
본 발명은 로라타딘계 화합물을 제외한 비진정성 항히스타민제를 류코트리엔 D4 길항제, 5-리폭시게나제 억제제 및 FLAP 길항제로 구성된 군에서 선택된 류코트리엔 길항제, 및, 경우에 따라 통상적으로 생리학적으로 허용되는 부형제, 증량제 및 보조제와 배합한, 알러지성 또는 혈관운동성 비염 또는 알러지성 결막염을 예방 또는 치료하기 위한 국소 또는 경우 투여용 약학 배합물을 개시한다.
Description
본 발명은 비진정성 항히스타민제, 및 류코트리엔 D4 길항제, 5-리폭시게나제 억제제 및 5-리폭시게나제 활성화 단백질(FLAP) 길항제에서 선택된 류코트리엔 작용에 영향을 미치는 물질을 포함하는 신규한 약학 조성물에 관한 것이다.
본 배합물은 알러지성 및/또는 혈관운동성 비염 또는 알러지성 결막염의 국소 치료를 개선시키는데 유용하다.
항히스타민제는 붉어짐(reddening), 가려움 또는 종창(swelling)으로 표현되는 급성 증상들을 신속히 제거하는 한편, 이 증후군의 기초가 되는 염증은 본 배합물에 포함된 류코트리엔 길항제에 의해 성공적으로 조절된다.

아젤라스틴은 현재 전신으로(정제) 및 국소로(비강 분무제 및 점안제) 둘 다 이용할 수 있는 항히스타민 중의 유일한 활성 화합물이다. 따라서 매우 강하고 현저한 알러지 증상을 가진 환자도 성공적으로 치료될 수 있다. 약학 제형이 다양하므로, 환자는 증상의 성질 및 심각도에 따라 아젤라스틴으로 개별적으로 치료받을 수 있고 그리하여 이 질환의 기초가 되는 염증을 억제시킬 수 있다.

LT 수용체 길항제의 작용 메카니즘은 단순하다. 수용체 길항제로서 그들은 LT 수용체를 점유한다. 따라서 방출된 류코트리엔은 수용체에 접근할 수 있고 수용체에 의해 매개될 수 있는 그들의 작용을 나타낸다.
글루코코르티코스테로이드, 및 경우에 따라 충혈제거제(decongestant), 항알러지제, 점액용해제(mucolytic), 비마약성 진통제, 리폭시게나제 억제제 및 류코트리엔 수용체 길항제와 함께 류코트리엔 억제 특성을 갖는 항히스타민제를 포함하는 비강내 적용용 배합물이 유럽 특허 공개 제 EP 0 780 127 A1 호에 개시되어 있으며 알러지성 비결막염의 치료에 권장된다. 글루코코르티코스테로이드와 항히스타민제의 협동(cooperation)은 치료의 효과를 증가시켜야 한다.
비염의 국소 치료의 경우에 국제 특허 공개 제 WO 98/48839 호에 또한 코르티코스테로이드 형태의 항염증제와 여기에 효능을 증가시키기 위하여, 예를 들면 하나 이상의 혈관수축제, 류코트리엔 억제제, 항히스타민제, 항알러지제, 점액용해제, 마취제, 항콜린제 또는 뉴라미니다제 억제제를 첨가한 적용예가 개시되어 있다.
국제 특허 공개 제 WO 98/34611 호에 개시된 바와 같이 알러지성 천식의 국소 치료를 위한, 데스카보에톡시로라티딘, 비진정성 항히스타민 로라티딘의 대사산물, 및 류코트리엔 D4 길항제, 5-리폭시게나제 억제제 또는 FLAP 길항제일 수 있는 류코트리엔 길항제로 구성된 배합물이 제안되었다. 데스카보에톡시로라타딘의 용도는 로라타딘 및 기타 비진정성 항히스타민제의 원치 않은 다수의 부작용을 회피하기 위함이다.
문헌[A. Roquet et al., Combined antagonism of leukotrienes and Histamine produces predominant inhibition of allergen-induced early and late phase airway obstruction in asthmatics. Am. J. Respir. Crit. Care Med. 155, 1856-1863, 1997]에서는 천식 환자의 알러젠 유도 기도 폐색에서의 로라타딘, 류코트리엔 길항제 자피르루카스트 및 이들 두 활성 화합물의 배합물의 경구 투여시 작용을 조사하였다.
머크 앤드 캄파니(Merck & Co.)의 국제 특허 공개 제 WO 97/28797 호의 연구로부터 또한 5종의 선택된 류코트리엔 길항제, 몬테루카스트, 자피르루카스트, 프란루카스트, 나트륨 1-(((R)-3-(2-(6,7-디플루오로-2-퀴놀리닐)에테닐)페닐)-3-(2-(2-하이드록시-2-프로필)-페닐)티오)메틸)사이클로프로판아세테이트, 1-(((1(R)-3-(2-(2,3-디클로로티에노[3,2-b]피리딘-5-일)-(E)-에테닐)페닐)-3-(2-(1-하이드록시-1-메틸에틸)페닐)프로필)티오)메틸)사이클로프로판아세트산과 로라타딘을 천식, 알러지 및 염증에서 경구로 또는 비경구로 투여하는 방법이 알려진다.
소개한 제제들의 많은 부작용, 치료 성공의 부족 및 일부 경우의 비특이적 치료로 인하여 알러지성 비염/결막염의 치료를 위해, 높은 유효성 및 안전성을 갖는 배합물에 대한 요구가 매우 크다.
그러므로 본 발명은 알러지성 비염/결막염의 치료를 위한 유용하고 신규한 배합물을 발견하고 제조하고자 하는데 목적이 있다.

또한 본 발명은 로라타딘계 화합물을 제외한 비진정성 항히스타민제, 바람직하게는 예를 들면 아젤라스틴, 레보카바스틴, 세티리진, 펙소페나딘, 미졸라스틴 또는 아스테미졸과, 몬테루카스트, 자피르루카스트 또는 프란루카스트와 같은 류코트리엔 작용에 영향을 미치는 류코트리엔 D4 길항제; 질류톤, 피리포스트 또는 AWD 23-115와 같은 5-리폭시게나제 억제제; 또는 MK-591, MK-886, 베이 X 1005와 같은 FLAP 길항제를 배합한, 국소 또는 경구 투여가 용이한, 예를 들면 분무제, 점적제(drop) 또는 정제 형태의 적합한 개별적인 투여 제형을 제공한다.
본 발명에 따라서, 로라타딘계 화합물을 제외한 항히스타민제, 바람직하게는 예를 들면 아젤라스틴, 레보카바스틴, 세티리진, 펙소페나딘, 미졸라스틴 또는 아스테미졸과, 몬테루카스트, 자피르루카스트 또는 프란루카스트와 같은 류코트리엔 작용에 영향을 미치는 류코트리엔 D4 길항제; 질류톤, 피리포스트 또는 AWD 23-115와 같은 5-리폭시게나제 억제제; MK-591, MK-886, 베이 X 1005와 같은 FLAP 길항제; 또는 이들의 약학적으로 허용가능한 염의 신규한 배합물은 동시에, 연속하여, 서로 독립적으로 국소(비강내 또는 안내(intraocular))로 또는 경구로 고정된 배합물로서 또는 개별적인 물질로서 공급될 수 있다.
상이한 제형이 존재하는 경우에 이들은 서로간에 적합하게 제조되며, 이들이 배합물에 존재하는 경우와 동일한 양 및 상응하는 중량비로서 투여량 단위에 각각의 활성 화합물을 포함한다.
배합의 결과로서 신속한 작용 개시뿐만 아니라 높은 치료적 효능을 보이며, 이에 따라 언급한 활성 화합물의 작용 모드가 상호간에 보완적이고 또한 약물동력학적으로 유사하게 작용하므로 강한 항염증 작용이 수반된다. 작용의 장기간 지속으로 하루 2회 투여를 가능케 만든다. 활성 성분이 고정된 배합물의 형태로 존재하는 경우에 두 활성 화합물이 하나의 정제 또는 하나의 용기에 포함되기 때문에 환자에 대한 투여가 더 간단하다.
본 발명에 따라서 항히스타민제 성분의 농도는 0.001 내지 0.5%일 수 있다.
본 배합물에서 류코트리엔 길항제의 농도는 0.01 내지 5%일 수 있다.
바람직한 농도는 항히스타민제 성분이 0.05 내지 0.2%이고, 류코트리엔 길항제가 0.5 내지 2%이다.
계획된 투여를 하루 1회 또는 2회 수행한다. 국소 투여시 항히스타민제의 개별적 투여량은 50 내지 500㎍, 바람직하게는 200 내지 400㎍이다. 국소 적용시 류코트리엔 D4 길항제의 투여량은 100 내지 2000㎍, 바람직하게는 200 내지 1000㎍이다.
5-LOX 또는 FLAP 억제제는 50 내지 2000㎍, 바람직하게는 200 내지 1000㎍ 의 투여량으로 투여된다.
항히스타민제(예를 들면 아젤라스틴으로서)의 투여량은 0.5 내지 16mg/일, 바람직하게는 2 내지 8mg/일이다.
류코트리엔 D4 길항제(예를 들면 몬테루카스트)의 경우에 개별 투여량은 1 내지 50mg/일, 바람직하게는 5 내지 10mg/일이다.
질류톤과 같은 5-LOX 억제제의 경구 투여량은 0.1 내지 6g/일, 바람직하게는 0.6 내지 2g/일이다.
FLAP 억제제의 경우에 투여량은 50 내지 2000mg/일, 바람직하게는 100 내지 500mg/일이다.
언급한 항히스타민제 및 류코트리엔 길항제 화합물, 및 그의 제조 방법은 공지이다.
배합물을 제공하기 위한 화합물의 약학적 취급(handling)은 통상의 표준 방법에 따라서, 바람직하게는 항히스타민제 및 류코트리엔 길항제를 개별적으로 또는 함께, 경우에 따라서 부형제 및/또는 증량제 또는 보조제와 함께 혼합하고 수득된 혼합물을 적합한 투여 형태로 전환시켜서 수행한다.
활성 화합물은 약학적 용도의 통상의 약학적 증량제, 보조제 또는 부형제를 포함하는 혼합물의 형태로서 경구로 또는 국소로 투여된다.
경구 또는 구소 투여용 조성물은 약학적으로 허용되는 다양한 투여 형태, 예를 들면 비강 분무제, 점비제, 점안제, 정제, 캡슐제 또는 과립제로 제형화될 수 있다. 본 발명에 따른 조성물은 활성 화합물 이외에, 항균성 보존제, 삼투압조절제, 농후제, pH 조절용 보조제 또는 완충계와 같은 다양한 전형적인 약학적 형태 구성 성분을 추가로 포함할 수 있다.
항균성 보존제 물질로는 예를 들면 벤잘코늄 클로라이드, 세틸피리디늄 클로라이드/브로마이드, 클로로부탄올, 클로로헥시딘 아세테이트, 클로로헥시딘 HCl, 클로로헥시딘 디글루코네이트, 클로로크레졸, 메틸파라벤, 프로필파라벤, 페녹시에탄올, 페닐머큐리 염, 소르빈산 및 티오머잘이 포함된다.
보존 용도로서 바람직하게는 나트륨 에데테이트 및 벤잘코늄 클로라이드의 배합물이 사용된다. 나트륨 에데테이트는 0.05 내지 0.1%의 농도로 사용되고, 벤잘코늄 클로라이드는 0.005 내지 0.05%의 농도로 사용된다.
긴장성(tonicity) 또는 삼투압을 조절에 적합한 보조제로서는 염화 나트륨, 염화 칼륨, 만니톨, 글루코스, 소르비톨, 글리세롤 또는 프로필렌 글리콜이 대략 0.1 내지 10%의 농도로 사용될 수 있다.
본 조성물은 흔히 점도를 증가시키고 약제와 신체 조직의 접촉을 지속시키고 향상시키기 위하여 농후제를 포함한다. 이러한 농후제로는 메틸셀룰로스, 하이드록시프로필메틸셀룰로스, 하이드록시에틸셀룰로스, 나트륨 카복시메틸셀룰로스, 폴리비닐 알코올, 폴리비닐 피롤리돈, 폴리아크릴레이트, 폴리아크릴아미드, 덱스트란, 겔란검(gellan gum), 폴록사머 또는 셀룰로스 아세테이트 프탈레이트가 포함된다.
또한 본 발명에 따른 조성물은, pH를 조정하여 대략 4 내지 8, 바람직하게는 5.5 내지 7.5로 유지시키기 위하여 약학적으로 허용되는 완충액을 포함한다. 이러한 유형의 완충액으로는 시트레이트, 포스페이트, 트로메타민, 글리신, 보레이트 또는 아세테이트가 있다. 또한 이들 완충액은 시트르산, 1급 또는 2급 나트륨 포스페이트, 글리신, 붕산, 나트륨 테트라보레이트, 아세트산 및 나트륨 아세테이트와 같은 물질로부터 유도될 수 있다. 또한 염산 또는 수산화 나트륨과 같은 추가의 보조제를 사용하여 pH를 조정할 수 있다.

실시예 1
아젤라스틴 염산염(0.1%)을 포함하는 비강 분무제 또는 점비제:

용액의 제조:
정제수 45kg을 적당한 교반 용기 내에 도입하였다. 여기에 활성 화합물, 하이드록시프로필메틸셀룰로스, 나트륨 에데테이트, 벤잘코늄 클로라이드 및 소르비톨 용액을 연속적으로 첨가하고 교반하여 용해시켰다. 생성된 용액을 정제수로 부피 49.5리터로 만들었다. 1N 수산화 나트륨 용액을 사용하여 용액을 pH 6.0으로 조정하였다. 정제수를 사용하여 최종 부피를 50.0리터로 만들고 교반하였다. 용액을 공극 2.0㎛의 막 여과기를 통과시켜 여과하고 병에 분배하였다.

정제수 45kg을 균질화 장치가 장착된 적당한 교반 용기 내에 도입하고 그 안에서 아비셀 RC 591을 고속으로 균질화시켰다. 이어서 교반하면서 물질 폴리소르베이트 80, 소르비톨 용액, 나트륨 에데테이트 및 벤잘코늄 클로라이드를 연속적으로 용해시켰다.
이어서 활성 화합물 몬테루카스트를 고속으로 균질화시켜 균질한 현탁액을 생성시켰다. 이어서 정제수로 최종 부피 50리터로 만들고 추가로 균질화시켰다. 이어서 생성된 기포를 제거시키기 위해 현탁액을 배기시켰다. 이어서 생성된 현탁액을 병에 분배하였다.

몇 가지 항히스타민제 및 LT 길항제 또는 5-LOX 및 FLAP 억제제의 작용 스펙트럼으로부터 두 물질의 배합물이 알러지성 비결막염의 증상에 상승 작용을 나타냄을 유도해낼 수 있다.
하기 약물학적 연구조사는 갈색 노르웨이 래트의 비염 모델에서 아젤라스틴 및 몬테루카스트의 단독 및 배합시의 작용을 기술한다. 갈색 노르웨이 래트를 이틀 계속하여 생리학적 식염수중의 오발부민(ovalbumin) 및 수산화 알루미늄의 현탁액을 이중 복강내 주사하여 활성적으로 감작시켰다. 감작 3주후, 카테터를 나트륨 티오펜탈 마취하에서 직립보행위로(orthograde) 동물의 기관(trachea) 내에 결속시켜서 실험 동물의 호흡을 유지시키고 추가의 카테터를 비강의 관류를 위하여 역행 방식으로(retrograde) 기관을 통하여 후비공의 내부 개구부까지 진입시켜 고정시켰다. 그리하여 비강 관류액을 비강을 통하여 졸졸 흘리고 분획 수집기(fraction collector)로 받아낼 수 있다. 시험 물질을 타일로스(tylose)에 현탁(몬테루카스트)시키거나 생리 식염수에 용해(아젤라스틴)시키고 알러젠 유발 60분전에 복강내 주입하였다. 코로부터 점액을 세척하기 위하여 롤러 펌프를 이용하여 30분동안 비강를 통하여 PBS를 관류시켰다(관류 속도: 0.5ml/분). 국소 적용의 경우에는 시험 물질을 몰농도로 관류액에 첨가하고 용액을 알러젠 유발전 30분동안 코를 통하여 관류시킨다. 이어서 혈장 표지자 에반스 블루(evans blue)(각 동물당 PBS중의 1% 농도의 용액 1ml)를 경정맥 내로 주입하였다. 이 때 15분동안 중지시키고 관류액을 수집하였다. 이어서 60분동안 PBS중의 오발부민 용액(PBS중 오발부민 10mg/ml)으로 비강을 관류시켜서 알러젠 유발(항원투여)을 수행하고 이 동안에 분획 수집기로 15분 분획물로서 관류액을 수집하였다. 시료/동물의 총량은 5였다. 시료를 원심분리한 후 미세적정판에 적용시키고 디지스캔(Digiscan) 광도계를 이용하여 파장 620nm에서 측정하였다. 공백(blank) 값은 자동적으로 공제되었다. AUC 프로그람을 이용하여 60분동안에 걸쳐 작용의 경과를 계산하였다. 제제 군의 물질 작용을 부형제 대조군에 대한 %로서 계산하였다.
알러젠 유발후 증가된 점막 투과성을 히스타민 및 류코트리엔과 같은 전령물질(messenger) 방출의 신호로서 평가한다. 항원 접촉후 이 현상이 알러지에 걸린 사람에게서도 일어나며 체액 분비의 증가 및 코막힘에 의해 명시적으로 나타난다.

아젤라스틴을 투여량 0.01mg/kg으로 단독 투여시 11%의 낮은 도관 투과성 억제를 일으킨다. 몬데루카스트도 마찬가지로 투여량 0.1mg/kg의 복강내 투여시 7%의 억제로서 활성이 미약하다. 투여량 0.01mg/kg(복강내)의 아젤라스틴 및 투여량 0.1mg/kg(복강내)의 몬테루카스트의 복합 투여는 40%(p<0.05)의 점막 혈장 관외유출(extravasation)의 상승적인 억제를 일으켰다. FLAP 억제제 베이 X 1005는 투여량 0.1mg/kg(복강내)에서 비점막 투과성을 31% 억제한다. 투여량 0.03 내지 10mg/kg(복강내)에서 AWD 23-115, 5-LOX 억제제는 투여량에 의존하는 도관 투과성 억제(37 내지 54%)를 일으킨다.

국소 적용시 히스타민 H1 차단제 아젤라스틴은 0.003 내지 0.03μmol/l의 농도에서도 점막 혈장 관외유출을 강력히 억제한다. 5-LOX 억제제 AWD 23-115는 0.3 및 1μmol/l에서 각각 32% 및 49%의 투여량 의존적으로 도관 투과성을 억제한다. 아젤라스틴을 0.003μmol/l의 농도로 AWD 23-115(0.1μmol/l)와 공동으로 투여하는 경우에 점막 관외유출의 억제율은 31%(p<0.05)이다.
Claims (20)
- 하기 성분 a) 내지 c)를 포함하는, 개별적인 투여량의 국소 또는 경구 투여에 적합한, 알러지성 또는 혈관운동성 비염 또는 알러지성 결막염 치료용 약학 제제:a) 0.001 내지 0.5중량%의 아젤라스틴;b) 0.01 내지 5중량%의 하기 b1) 및 b2)로 구성된 군에서 선택된 류코트리엔 길항제 또는 그의 약학적으로 허용가능한 염중 하나:b1) 몬테루카스트, 자피르루카스트 및 프란루카스트로 구성된 군에서 선택된 류코트리엔 D4 길항제, 및 b2) 질류톤, 피리포스트 및 1-[4-(퀴놀린-2-일-메톡시)벤질]-5-메톡시-1H-인다졸-3-올 이염산염(AWD 23-115)으로 구성된 군에서 선택된 5-리폭시게나제 억제제; 및c) 통상적으로 생리학적으로 허용되는 부형제 또는 증량제 또는 보조제.
