Fijación ósea para prótesis de disco intervertebral
Campo de la invención
La presente invención se refiere a una prótesis de disco intervertebral, destinada a sustitución por discos fibrocartilaginosos que garantizan la unión entre las vértebras y la columna vertebral.
Se conocen diversos tipos de prótesis de disco intervertebral en la técnica anterior. Numerosas prótesis, tales como por ejemplo en la solicitud de patente WO 02 089 701 y en el documento WO 2004/041129, están formadas por una placa inferior y una placa superior que forma un tipo de jaula alrededor de un núcleo central. Una parte de estas prótesis permite que la placa superior gire con respecto al núcleo central y opcionalmente permite que el núcleo central se deslice con respecto a la placa inferior. Este deslizamiento del núcleo central con relación a la placa inferior es una característica esencial, ya que debe permitir el posicionamiento espontáneo del núcleo en la posición ideal para absorber las limitaciones impuestas sobre la prótesis, durante los movimientos realizados por el paciente que lleva la prótesis. El desplazamiento del núcleo, en co-operación con al menos una placa alrededor de una superficie no uniforme, permite una inclinación entre las placas de la prótesis, lo cual facilita la movilidad del paciente que lleva la prótesis. El desplazamiento del núcleo también evita la deformación por fatiga en condiciones de carga, cuando se somete a limitaciones principales. Una parte de estas prótesis tiene un medio de fijación ósea que permite la unión de estas prótesis a las vértebras entre las cuales se pretende la inserción.
No obstante, el tamaño de las vértebras varía en gran medida de una persona a otra, para una misma vértebra en una posición concreta de la columna vertebral, pero también para una persona concreta dependiendo de la posición de las vértebras de la columna vertebral entre las cuales se pretende la inserción de la prótesis. Las prótesis de disco intervertebral deben ser de un tamaño apropiado para las vértebras entre las cuales se pretende la inserción, dependiendo de la persona y de la posición de estas vértebras en la columna vertebral. Además, dependiendo del trastorno de la columna vertebral del paciente que lleva la prótesis, en ocasiones, es preferible que la prótesis permita una corrección de este trastorno. De este modo, las prótesis se pueden usar para corregir un defecto de inclinación de las vértebras, tal como, por ejemplo, lordosis. Con el fin de disponer de prótesis que sean apropiadas en la mayoría de los casos posibles, se debe prever un gran número de prótesis con diferentes tamaños de placa e inclinaciones. Esta multiplicidad de prótesis tiene el inconveniente principal de los elevados costes de fabricación y elevados niveles de reserva. En este contexto, resulta beneficioso proponer una prótesis que tenga una estructura que permita la adaptación a tamaños diferentes de vértebras y que permita inclinaciones diferentes de las placas. Dicha prótesis reduciría los niveles de reserva y los costes de fabricación.
El documento DE 43 28 690 A1 muestra un implante que se fija por medio de una fijación denominada grapa que se fabrica a partir de una banda ancha y plegada. La grapa posee dos patas unidas por una parte curvada que permite que la grapa se bloquee sobre un puente del implante. La grapa también posee una limitación que permite que la grapa mantenga el puente del implante en la parte curvada de la grapa por medio de un efecto de gancho. Dicha grapa es especialmente apropiada para implantes abiertos que tienen un puente que une dos partes de un implante
o que se puede proporcionar con un puente separado para la grapa. Este tipo de implante no posee placas que forman un tipo de jaula alrededor de un núcleo central por lo que, de este modo, no proporciona una solución para prótesis que incluyen placas. Además, la forma de la pata de la grapa no resulta eficaz para la fijación del implante sobre el hueso.
El objetivo de la presente invención es proponer una prótesis de disco intervertebral que permita un movimiento limitado de las diferentes piezas de la prótesis, unas y otras, y que comprenda un núcleo usado para restringir su desplazamiento en al menos una dirección. Una misma prótesis se puede adaptar a diferentes tamaños de vértebras a bajo coste.
La invención de la presente solicitud de división se define en la reivindicación 1. Se definen realizaciones adicionales en las reivindicaciones dependientes.

La prótesis de disco intervertebral descrita en la presente memoria está formada por una placa superior (1)
25 articulada con relación a una placa inferior (2) por medio de un núcleo (3) y cada una de las placas (1, 2) está equipada con un elemento de adaptación anatómica (11, 22) que permite ajustar el tamaño global de la prótesis al tamaño de las vértebras entre las cuales tiene lugar la inserción de la misma. De este modo, gracias a los elementos de adaptación anatómica (11, 22), una unidad sencilla constituye dos placas (1, 2) y el núcleo (3) se puede usar para diferentes tamaños de vértebras, lo cual tiene la ventaja de reducir sustancialmente el coste de fabricación de las
30 prótesis y sus variaciones. La ventaja de la prótesis es que comprende partes simples cuyos elementos de adaptación anatómica (11, 22) se pueden dimensionar para que se adapten a las diferentes vértebras de la columna vertebral, por ejemplo, el ajuste del espesor de la prótesis al espacio intervertebral y/o el ajuste de la inclinación de las placas (1, 2) de la prótesis a la inclinación de las vértebras del paciente. Aunque los elementos de adaptación anatómica (11, 22) permiten por sí mismos el ajuste de la prótesis a diferentes tamaños de vértebras, naturalmente
35 se pueden usar las placas (1, 2) y el núcleo (3) de diferentes tamaños y formas, en caso de ser necesario.
Los dos elementos de adaptación anatómica (11, 22) de la prótesis consisten en un elemento superior (11) y un elemento inferior (12). El elemento superior (11) tiene, por una parte, una superficie superior (110) de la cual al menos una parte presenta una superficie en contacto con una superficie inferior de una primera vértebra y tiene, por otra parte, una superficie inferior (111) de la cual al menos una parte presenta una superficie en contacto con una
40 parte de la placa superior (1). El elemento inferior (22) tiene, por una parte, una superficie inferior (220) de la cual al menos una parte presenta una superficie en contacto con una superficie superior de una segunda vértebra y tiene, por otra parte, una superficie superior (222) de la cual al menos una parte presenta una superficie en contacto con una parte de la placa inferior (2). Cada uno de los dos elementos de adaptación anatómica (11, 22) se fija sobre las placas (1, 2) gracias a los respectivos medios de fijación (113, 223).

En estas posibles realizaciones diferentes (no mostradas) de la invención, la superficie convexa o no cóncava del núcleo (3) puede ser cóncava o convexa respectivamente, por ejemplo de manera ligera.
En las realizaciones que se ilustran en las figuras 1 a 9, la parte cóncava (140) de la superficie inferior (14) de la placa superior (1) complementaria con la parte convexa de la superficie superior (34) del núcleo (3) permiten inclinar la placa superior (1) cuando el paciente que lleva la prótesis se agacha. La cooperación entre la superficie cóncava
(140) y la superficie convexa (34) presenta una superficie de articulación con la prótesis, gracias a la inclinación de la placa superior (1) con relación al núcleo (3). De forma natural, el centro de esta articulación está en la punta de la superficie convexa (34) del núcleo (3). En las realizaciones ilustradas, la superficie inferior del núcleo (3) y la superficie superior de la placa inferior (2) son planas para permitir el espacio libre del núcleo (3) con relación a la placa inferior (2), ambos en traslación de acuerdo con un eje sustancialmente paralelo a la placa inferior (2), y en rotación alrededor de un eje sustancialmente perpendicular a la placa inferior (2). Durante los movimientos del paciente que lleva la prótesis, esta inclinación de la placa superior (1) y este espacio libre del núcleo permiten el desplazamiento del núcleo (3) hacia la posición ideal para absorber las limitaciones aplicadas a la prótesis. El movimiento entre la placa superior (1) y el núcleo (3), así como también el espacio libre del núcleo (3) con respecto a la placa inferior (2) permiten de este modo que el paciente se mueva, opcionalmente, para eliminar los defectos de posicionamiento de la prótesis. De igual forma, el espacio libre tiene la ventaja de evitar el desgaste prematuro debido a las limitaciones aplicadas a la prótesis.
Independientemente de la realización, el núcleo (3) también tiene un medio de co-operación macho o hembra (33) complementario con el medio de co-operación hembra o macho (23) respectivo presente sobre al menos una de las placas (1, 2). Estos medios de co-operación macho y hembra (23, 33), situados en las proximidades de los bordes de al menos una placa (1, 2) y el núcleo (3) limitan, sin excesiva fricción, los movimientos de traslación del núcleo (3) con relación de esta placa (1, 2), de acuerdo con un eje sustancialmente paralelo a esta placa (1, 2), y limitan o evitan el movimiento de rotación del núcleo (3) con respecto a esta placa (1, 2), aproximadamente un eje sustancialmente perpendicular a esta placa (1, 2). Las dimensiones de cada medio de co-operación macho (33) pueden ser ligeramente menores que las de cada medio de co-operación hembra (23), para permitir un ligero espacio libre entre el núcleo (3) y la placa (1, 2) equipada con estos medios de co-operación. Por el contrario, las dimensiones de cada medio de cooperación macho (33) también pueden ser sustancialmente las mismas que las de cada medio de co-operación hembra (23) para evitar cualquier espacio libre entre el núcleo (3) y la placa (1, 2) equipada con estos medios de co-operación.
En la realización de las figuras 1 a 3, el núcleo (3) tiene un medio de co-operación macho (33) complementario con el medio de co-operación hembra (23) presente sobre la placa inferior (2). Los medios de co-operación macho (33) del núcleo (3) son, por ejemplo, pasadores o bloques con forma sustancialmente de paralelepípedo, presentes sobre los bordes laterales del núcleo (3), como resulta particularmente visible en las figuras 1 a 3. El medio de cooperación hembra (23) puede consistir, por ejemplo, en cuatro paredes situadas, por pares, sobre cada uno de los dos bordes laterales de la placa inferior (2). Estas paredes pueden curvarse hacia el centro de la prótesis, para cubrir al menos una parte del medio de co-operación macho (33) del núcleo (33) y evitar el desplazamiento del núcleo (3) y la placa superior (1). Estos medios de co-operación (23, 33) también evitan que el núcleo (3) se salga de la prótesis, en caso de limitación excesiva de la prótesis. En una realización alternativa (no mostrada), las dimensiones de cada medio de co-operación macho (33) del núcleo (3) son sustancialmente las mismas que las de cada medio de co-operación hembra (23) de la placa inferior (2), para evitar cualquier espacio libre del núcleo (3) con relación a la placa inferior (2), tanto en traslación como en rotación. En el último caso, el único movimiento permitido de la prótesis es la inclinación de la placa (1) superior con relación al núcleo (3). En una realización alternativa (no mostrada), el núcleo (3) tiene un medio de co-operación hembra que consiste, por ejemplo, en rebajes complementarios del medio macho presente en la placa (2) inferior. Estos medios macho de la placa (2) inferior pueden consistir, por ejemplo, en dos bloques o dos puntas, por ejemplo curvados hacia el interior de la prótesis y que miran uno hacia el otro, sobre los bordes de la placa (2) inferior. Las puntas pueden, por ejemplo, sustituirse por un bloque con un orificio sobre el cual se fija un pasador por medio de un perno que penetra en el orificio. En otra realización alternativa (no mostrada), la placa inferior (2) tiene medios dientes. A modo de complemento, el núcleo (3) tiene paredes bajo su superficie inferior. Las dimensiones de los medios dientes de la placa inferior (2) y las paredes del núcleo (3) se adaptan, según elección, por medio de un ligero espacio libre del núcleo (3) en traslación y en rotación o por medio de la ausencia de espacio libre, de acuerdo con el resultado deseado. En otras alternativas (no mostradas), el medio de co-operación puede estar ubicado sobre el núcleo (3) y sobre la placa (1), en lugar de la placa inferior (2).
Se considera ahora la descripción de una primera realización en referencia a la figura 1. En esta realización, los elementos de adaptación anatómica superior (11) e inferior (12) consisten en placas, denominadas anatómicas, que cubren respectivamente las placas superior (1) e inferior (2). Las superficies (222) e inferior (111) de los elementos de adaptación anatómica inferior (22) y superior (11) pueden tener un refuerzo en el que se albergan las placas inferior (2) y superior (1), respectivamente. En otra realización alternativa, los elementos de adaptación pueden ser planos y comprender obturadores que, como los refuerzos anteriormente mencionados, evitan que las placas inferior
(2) y superior (1) se muevan respectivamente con relación a los elementos de adaptación anatómica. superficies de contacto de la prótesis con las vértebras adyacentes que son más grandes que cuando no existen elementos de adaptación anatómica (11, 22). Se pueden adaptar diferentes tamaños de las placas anatómicas de los elementos de adaptación anatómica (11, 22) a una unidad individual creada por las dos placas (1, 2) y el núcleo (3), para proporcionar un buen contacto entre la prótesis y las vértebras de diferentes tamaños.
En la realización de la prótesis ilustrada en la figura 2, los elementos de adaptación anatómica (11, 22) consisten en coronas que rodean a las placas superior (1) e inferior (2). En esta realización, los bordes de las superficies superior
(10) e inferior (20) de las respectivas placas superior (1) e inferior (2) son biselados y complementarios con respecto a los bordes interiores inferior (111) y superior (222) de las respectivas coronas superior (11) e inferior (22). Esta forma inclinada de los bordes de las placas (1, 2) y de las coronas de adaptación anatómica (11, 22) co-opera con los medios de fijación (113, 223) de los elementos de adaptación anatómica con el fin de mantener las coronas de adaptación anatómica (11, 22) fijadas en el plano de las respectivas placas superior (1) e inferior (2) de la prótesis. Las coronas de adaptación anatómica (11, 22) prolongan respectivamente las superficies superior (10) e inferior (20) de las placas superior (1) e inferior (2) respectivamente, para presentar superficies de contacto de la prótesis con las vértebras adyacentes que sean más grandes que cuando no existen elementos de adaptación anatómica (11, 22). De la misma forma que para las placas anatómicas (11, 22) anteriormente mencionadas, de este modo, se puede ajustar una unida individual creada por las dos placas (1, 2) y el núcleo (3) a las vértebras de diferente tamaño, gracias a las diferentes zonas de coronas de los elementos de adaptación anatómica (11, 22).
En todas las realizaciones, los elementos de adaptación anatómica (11, 22) pueden prolongar, simétrica o asimétricamente, las superficies superior (10) e inferior (20) de las placas superior (1) e inferior (2), respectivamente. De este modo, por ejemplo, el borde anterior de los elementos de adaptación anatómica (11, 22) puede tener una superficie de contacto con las vértebras más grande que su borde posterior, de forma que el centro de la articulación de la prótesis (descrito anteriormente) se encuentre centrado con respecto al eje natural de la columna vertebral, lo que significa el centro de la parte trasera de las vértebras de una sección 2/3-1/3.
De acuerdo con las realizaciones escogidas, la prótesis de discos intervertebrales de acuerdo con la invención permite, por ejemplo, corregir los defectos de la lordosis. La presencia de un ángulo entre las superficies superior e inferior de la prótesis, en contacto con las vértebras adyacentes, podría resultar deseable. Dicho ángulo podría obtenerse creando un ángulo en la placa superior (1), cuyos planos medios representen sus superficies inferior (14) y superior (10). Otra posibilidad consiste en la creación de un ángulo de la placa inferior (2), cuyos planos medios representen sus superficies inferior (20) y superior (24). Otra posibilidad consiste en que al menos uno de los elementos de adaptación anatómica (11, 22), cuyos planos medios representen sus superficies inferior y superior, forme un ángulo. De este modo, se puede usar una unidad individual formada por dos placas (1, 2) y el núcleo (3), por ejemplo, para inducir o no lordosis, dependiendo de que los elementos de adaptación anatómica (11, 22) se encuentren asociados a la misma. En la realización ilustrada en la figura 3, la superficie inferior (220) de la placa anatómica inferior (22) forma un ángulo con su superficie superior (222). Otra posibilidad para obtener dicho ángulo consiste en una posición ligeramente desviada del núcleo (3) con respecto al centro de la prótesis. Esta posición ligeramente desviada del núcleo (3) puede, por ejemplo, mantenerse gracias a un posicionamiento ajustable de los medios de co-operación macho y hembra (23, 33) entre ellos. Si el cirujano desea, por ejemplo, que la prótesis induzca lordosis que se encuentre dentro de un intervalo de valores, se selecciona una prótesis cuyo núcleo (3) pueda tener un ligero espacio libre en traslación y en rotación con respecto a la placa inferior (2), pero aproximadamente una posición que impone una inclinación permanente y ligera de al menos una de las placas, gracias a un ajuste preciso de los medios de co-operación (23, 33) entre el núcleo (3) y la placa inferior (2). De este modo, de acuerdo con la realización escogida, los planos medios que representan las superficies superior (110, 222) e inferior (111, 220) de cada uno de los elementos de adaptación anatómica (11, 22) pueden ser sustancialmente paralelos o formar un ángulo agudo. La inclinación obtenida por medio de dicho ángulo permite adaptar la forma global de la prótesis a la anatomía de la columna vertebral o, posiblemente, corregir los defectos de inclinación de las vértebras del paciente al cual va destinada la prótesis. Se pueden conectar los mismos elementos de adaptación anatómica (11, 22) con diferentes placas (1, 2) cuyas superficies superior (10, 24) e inferior (14, 20) forman diferentes ángulos. Por el contrario, las placas (1, 2) cuyas superficies superior (10, 24) e inferior (14, 20) son paralelas, se conectan con los elementos de adaptación anatómica (11, 22) cuyas superficies superior (110, 222) e inferior (111, 220) forman ángulos diferentes. Este ángulo entre la superficie superior (10) y la placa superior (1) y la superficie inferior (20) y la placa inferior (2) se puede imponer bien mediante el hecho de que los planos medios que representan las superficies inferior (20, 14) y superior (24, 10) de la placa inferior (2) y/o la placa superior (1) formen un ángulo, o bien restringiendo, gracias a los medios de co-operación (23, 33), los movimientos del núcleo (3) alrededor de una posición que impone una inclinación de al menos una de las placas (1, 2).
Las Figuras 1 a 3 ilustran los medios móviles de fijación ósea (60) de acuerdo con la invención de los elementos de adaptación anatómica (11, 22). Ventajosamente, estos medios de fijación ósea (60) se pueden fijar sobre los elementos de adaptación anatómica (11, 22) tras la fijación de los mismos sobre las placas (1, 2) y, la mayoría de ellos, tras la inserción de la prótesis entre las vértebras. Esta característica permite al cirujano ubicar fácilmente la prótesis entre las vértebras y posteriormente insertar los medios de fijación ósea (60) una vez que la prótesis se ha colocado de forma correcta. En la realización presentada en la figura 1, estos medios móviles de fijación ósea (60) consisten en una placa (61) equipada con dientes (62) orientados para ejercer resistencia frente a la retirada de la placa (61) una vez que se ha insertado en la vértebra. Esta placa (62), por supuesto, puede ser con o sin dientes

curvada (63) para plegarse sobre sí misma. Esta parte curvada forma un tipo de gancho destinado al enclavamiento sobre un borde (16, 26) de una abertura realizada en las proximidades de la periferia de los elementos de adaptación anatómica (11, 22). Este borde (16, 26) de la abertura crea un tipo de vástago sobre el cual se enclavan los medios de fijación ósea (60). De hecho, la parte curvada (63) permite la sujeción del medio de fijación ósea (60) sobre el vástago (16, 26) de los elementos de adaptación anatómica (11, 21). Por supuesto, este vástago se puede sustituir por cualquier medio equivalente que permita sujetar el medio de fijación ósea (60). En las realizaciones ilustradas en las figuras 1 a 9, el vástago (16, 26) se ubica sobre el borde anterior de los elementos de adaptación anatómica (11, 22), para permitir el acceso al cirujano una vez que se ha insertado la prótesis entre las vértebras gracias al medio anterior (a través del acceso a las vértebras desde su cara anterior). Si la implantación de la prótesis se hace gracias a un medio posterior, los elementos de adaptación anatómica (11, 22) pueden tener un vástago (16, 26) ubicado sobre el borde posterior. Si la implantación de la prótesis se realiza gracias a un medio lateral, los elementos (11, 22) de adaptación anatómica puede tener un vástago (16, 26) ubicado sobre al menos uno de sus bordes. En la realización ilustrada en las figuras 2 y 3, la parte con gancho (63), curvada para plegarse sobre sí misma, de la placa dentada (61) del medio móvil de fijación ósea (60) de los elementos de adaptación anatómica (11, 22) se prolonga con una segunda placa (61) también equipada con dientes (62) orientados para ejercer resistencia frente a la retirada de la placa (61), una vez que se ha insertado en la vértebra. En la realización ilustrada en la figura 2, esta segunda placa (61) es más corta que la primera placa y en la realización ilustrada en la figura 3, es tan larga como la primera placa. El hecho de que el medio de fijación ósea (60) se cierre sobre el vástago (16, 26) permite disponer de un ángulo variable que facilita la unión de la prótesis. De hecho, dependiendo de su impedimento, el cirujano podrá elegir ángulos según desee introducir el medio de fijación ósea (60) en el interior de la vértebra. Además, el hecho de que el medio de fijación ósea (60) se pueda insertar tras colocar la prótesis entre las vértebras, permite ajustar la posición relativa de los diferentes elementos (1, 2, 3) de la prótesis. De hecho, la inserción de la prótesis genera restricciones sobre los elementos de la misma, que son móviles unos con respecto a otros, y presentan riesgo de una mala ubicación conjunta. Posteriormente, el cirujano puede, gracias a la invención, ajustar la posición de la prótesis entre las vértebras y ajustar la posición relativa de los elementos de la prótesis entre las mismas antes de unir la prótesis de forma definitiva.
Resulta obvio que la prótesis anteriormente descrita puede comprender otros medios de fijación ósea (60) diferentes de los de la presente invención. Para proporcionar ejemplos no limitantes, dicho medio de fijación ósea (60) puede consistir en aletas fijadas sobre la prótesis como en la Solicitud de Patente WO03/039400 o en un saliente clavado en la vértebra a través de los elementos de adaptación anatómica como en la Solicitud de Patente WO04/041129. Las Figuras 10A, 10B, 11A y 11b presentan una realización del medio de fijación (60). El medio de fijación ósea (60) de acuerdo con la presente realización consiste en aletas que comprenden una parte de gancho (63), curvada para plegarse sobre sí misma, de forma que las aletas se puedan adaptar sobre los medios de adaptación anatómica. La parte de gancho (63) de la aleta, particularmente visible en la figura 10B, permite que el medio de fijación (60) se enclave sobre el borde (16, 26) de la abertura realizada en las proximidades de la periferia de los elementos de adaptación anatómica (11, 22), de forma tan particularmente visible como en las figuras 11A y 11B. Este borde (16,26) de la abertura crea un tipo de vástago sobre el cual se enclava el medio de fijación ósea (60), como se ha descrito con anterioridad. La aleta además comprende un perno (64) (o un pasador) adaptado para su inserción en una hendidura (65) presente sobre la superficie de la placa y/o elemento de adaptación anatómica sobre el cual se fija la aleta, como resulta particularmente visible en la figura 11B. La hendidura (65) y el perno (64) tienen dimensiones adaptadas para que el perno (64) se fije en el interior de la hendidura (65). Por ejemplo, el perno (64) puede tener una forma sustancialmente cónica, estando el diámetro más largo del cono en la base del perno y estando el diámetro más pequeño en su extremo. La hendidura (65) puede tener sus paredes laterales adaptadas para cooperar con la forma cónica del perno (64) de forma que el perno encaje ligeramente en el interior de la hendidura y, de este modo, fije el medio de fijación ósea (60) sobre los elementos de adaptación anatómica (11, 22). Por ejemplo, la anchura de la hendidura (65) puede ser mayor en su superficie que en su parte inferior. El medio de fijación ósea (60) se fija de este modo sobre la prótesis de la presente invención, en primer lugar enclavando la parte con gancho (63) sobre el vástago (16, 26) de los elementos de adaptación anatómica (11, 22) y por medio de rotación del medio de fijación ósea (60) alrededor del vástago hasta que el perno (64) penetre de forma ajustada en el interior de la hendidura (65) del elemento de adaptación anatómica (11, 22) y/o la placa (1, 2). La aleta (60) puede tener un tamaño convencional para todas las prótesis realizadas de acuerdo con la presente invención y la posición del perno (64) de la aleta (60) en el interior de la hendidura (65) varía en función del tamaño de los elementos de adaptación anatómica (11, 22). Dependiendo del espesor de los elementos de adaptación anatómica (11, 22), el perno (64) penetra en el interior del elemento de adaptación anatómica (11, 22) únicamente o puede atravesar el elemento de adaptación anatómica (11, 22) y penetra en la hendidura (65) en las placas (1, 2), como se muestra por ejemplo en la figura 11B para la placa superior (1). Debido a que el elemento de adaptación anatómica (11, 22) varía en cuanto a tamaño (diámetro), su hendidura puede tener longitudes variables y se puede sustituir por un orificio que tenga una distancia variable desde el vástago (16, 26), de manera que el orificio se adapte para recibir el perno (64), pero cuando se diseña el perno para penetrar también las placas, las placas tienen que incluir una hendidura ya que la distancia entre el perno y la periferia de las placas varía dependiendo del tamaño del elemento de adaptación anatómica (11, 22). Una vez fijadas sobre los elementos de adaptación anatómica (11, 22), las aletas (60) se adaptan para cooperar con una hendidura horadada en las superficies de las vértebras adyacentes con las cuales se encuentra en contacto. De este modo, el cirujano puede llevar a cabo una hendidura en las superficies de las vértebras entre las cuales se pretende insertar la prótesis. Esta hendidura en las vértebras tendrá, de forma natural, una orientación relativa con respecto al plano sagital que depende de la posición y orientación de la aleta. Esta

orientación será pre-determinada y establece y fija la orientación de la prótesis. Similarmente, la profundidad de la hendidura en las vértebras y su distancia desde la periferia están pre-determinadas, como una función del tamaño de la aleta (60) y permite al cirujano ajustar la posición relativa de los diversos elementos de la prótesis y predecir la posición de la misma con respecto al eje natural de las vértebras. Las aletas comprenden dientes (66) sobre sus superficies que se pretende que estén en contacto con la parte inferior de la hendidura realizada en las vértebras. Estos dientes (66) de las aletas (60) ejercen resistencia frente a la salida de la prótesis del interior de su alojamiento entre las vértebras, por ejemplo cuando se aplican restricciones intensas a la prótesis. Resulta obvio (en particular a partir de la figura 11B que muestra ambas realizaciones), que la parte con gancho (63) de las aletas (60) se puede orientar de forma que se enclave sobre el vástago (16, 26) por medio de inserción en el interior de la abertura realizada en las proximidades de la periferia de los elementos de adaptación anatómica (11, 22) o por medio de inserción desde fuera de esta abertura.
Resulta obvio que el medio de fijación ósea (60) descrito anteriormente se adapta de forma particular a los elementos de adaptación anatómica (11, 22), pero también se puede adaptar a las placas u otros tipos de prótesis de disco intervertebral que tienen placas que comprenden una abertura en las proximidades de su periferia. El borde (16, 26) de dicha abertura en las placas crea un tipo de vástago (16, 26) sobre el cual se puede enclavar la parte con gancho (63) de ambas realizaciones desprendibles del medio de fijación ósea (60).
Las Figuras 4 y 9 ilustran las placas (1, 2) de la prótesis transversal equipada con sus elementos de adaptación anatómica (11, 22) y definen diferentes realizaciones de los medios de fijación (113, 223, 15, 25) de estos elementos de adaptación anatómica (11, 22) sobre las placas (1, 2). Estos medios de fijación (113, 223, 15, 25) son reversibles, lo que significa que los elementos de adaptación anatómica (11, 22) se pueden unir de forma sencilla y retirar de las placas (1, 2) de la prótesis. Estos medios de fijación (113, 223, 15, 25) permiten el cambio de los elemento de adaptación anatómica (11, 22), fijados de manera móvil a las placas (1, 2), Los medios de fijación (113, 223, 15, 25) de los elementos de adaptación anatómica (11, 22) sobre las placas (1, 2) consisten en los medios de fijación (113, 223) presente sobre los elementos de adaptación anatómica (11, 22) y complementarios con los medios de fijación (15, 25) presentes sobre las placas (1, 2) de la prótesis. Los elementos de adaptación anatómica (11, 22) se fijan sobre las placas (1, 2), por una parte, por medio de contacto de al menos parte de su superficie inferior (111) y superior (222) con al menos una parte de las respectivas placas superior (1) e inferior (2) y por otra parte, por medio de contacto de los medios de fijación (113, 223) con los medios de fijación complementarios (15, 25) presentes sobre las placas (1, 2) de la prótesis. Para las placas anatómicas (11, 22), tal como las que se ilustran, por ejemplo, en las figuras 4A a 4D, se fijan los elementos de adaptación anatómica (11, 22) sobre las placas (1, 2) gracias al hecho de que las superficies superior (10) e inferior (20) de las respectivas placas superior (1) e inferior (2) se unen firmemente a los refuerzos presentes sobre las superficies inferior (111) y superior (222) de las placas anatómicas superior (11) e inferior (22), gracias a los medios de fijación (113, 223, 15, 25). Para las coronas anatómicas (11, 22), tal como las que se ilustran, por ejemplo, en las figuras 5A a 5C, se fijan los elementos de adaptación anatómica (11, 22) sobre las placas (1, 2), gracias al hecho de que las partes biseladas de las superficies superior (10) e inferior (20) de las respectivas placas superior (1) e inferior (2) se unen firmemente a las partes biseladas de las superficies (111) e inferior (222) de las respectivas placas anatómicas superior (11) e inferior (22), gracias a los medios de fijación (113, 223, 15, 25). Las diferentes realizaciones de los medios de fijación (113, 223, 15, 25) de los elementos de adaptación anatómica (11, 22) sobre las placas (1, 2) se describen ahora con referencia a las figuras 4 a 9. Resulta obvio que estos medios de fijación se proporcionan a modo de ilustración y se pueden sustituir por cualquier medio equivalente sin abandonar el alcance de la invención. De igual modo, la invención permite el uso de cualquier combinación de medios de fijación diferentes (113, 223, 15, 25) descritos con anterioridad.
En varias realizaciones, los medios de fijación (113, 223, 15, 25) de los elementos de adaptación anatómica (11, 22) sobre las placas (1, 2) consisten en medios de fijación macho (113, 223) presentes sobre los elementos de adaptación anatómica (11, 22) y co-operan con los medios de fijación hembra (15, 25) presentes sobre las placas (1, 2) de la prótesis. Los medios de fijación hembra (15, 25) presentes sobre las placas (1, 2) de la prótesis pueden consistir, por ejemplo, en superficies planas (15, 25) presentes sobre los bordes de las placas (1, 2) de la prótesis o en rebajes (15, 25) bien realizados en los bordes de las placas (1, 2) de la prótesis, o en los bordes del medio de cooperación hembra (23) de las placas (1, 2) de la prótesis.
En una realización ilustrada en las Figuras 4A y 4B, el medio de fijación (113) de la placa anatómica superior (11) consiste, en el borde posterior de su superficie inferior (111), en puntas cuyas formas y dimensiones se pretende que alberguen una sección (15) del borde posterior de la superficie inferior (14) de la placa superior (1). Sobre el borde anterior de su superficie inferior (111), el medio de fijación (113) de la placa anatómica superior (11) consiste en pestillos constituidos en un eje de rotación sobre el cual se monta un pasador destinado a pivotar alrededor de este eje y albergar la sección (15) del borde posterior de la superficie inferior (14) de la placa superior (1), como resulta particularmente visible en las figuras 4A y 4B. Los pestillos a mano derecha de las figuras 4A a 4C se ilustran en posición abierta y los pestillos a mano izquierda están en posición cerrada. En las realizaciones que se ilustran en las figuras 4C y 4D, el medio de fijación (223) de la placa anatómica inferior (22) consiste, en el borde posterior de su superficie inferior (222), en puntas que tienen forma y dimensiones destinadas a encajar en una abertura (25) realizada en el medio de co-operación (23) de la placa inferior (2). Sobre el borde anterior de su superficie inferior (222), el medio de fijación (223) de la placa anatómica superior (22) consiste en pestillos constituidos en un eje de rotación sobre el cual se monta un pasador destinado a albergar un rebaje (25) realizado en una parte del medio de co-operación (23) presente sobre el borde posterior de la placa inferior (2). En las realizaciones de las figuras 6A a 9D, los medios de fijación (113, 223, 15, 25) de los elementos de adaptación anatómica (11, 22) sobre las placas (1, 2) consisten en medios de fijación hembra (113, 223) presentes sobre los elementos de adaptación anatómica (11, 22) y que co-operan con el medio intermedio macho (50) que también puede co-operar con medios de fijación hembra (15, 25) presentes sobre las placas (1, 2) de la prótesis. Los elementos de adaptación anatómica (1, 2) se fijan sobre las placas (1, 2), por una parte por medio del contacto de al menos una de sus parte superiores (111) y la superficie inferior (222) con al menos una parte de las respectivas partes superior (1) e inferior (2) y por otra parte, el contacto del medio intermedio macho (50) con medios de fijación hembra (113, 223) presentes sobre los elementos de adaptación anatómica (11, 22) y con medios de fijación hembra (15, 25) presentes sobre las placas (1, 2) de la prótesis. El medio intermedio macho (50) tiene un medio de fijación (55) que bloquea el medio intermedio macho (50) en la posición en la que co-operan por un lado con los medios de fijación hembra (113, 223) de los elementos de adaptación anatómica (11, 22) y por otra, con los medios de fijación hembra (15, 25) presentes sobre las placas (1, 2) de la prótesis. Este medio de fijación (55) consiste, por ejemplo, en al menos una irregularidad formal (55) presente sobre al menos un lado de esta placa (50) y está destinado a co-operar con al menos una abertura (550) realizada en los medios de fijación hembra (113, 223) del elemento de adaptación anatómica (11, 22) y/o en los medios de fijación hembra (15, 25) de las placas (1, 2). La abertura (550) puede tener una forma complementaria al medio intermedio macho (50) o su medio de fijación, como se ilustra en las figuras 6A y 6B.
En la realización que se ilustra en la figura 6A, la placa que constituye el medio intermedio macho (50) se ensancha hacia su extremo posterior y la irregularidad formal que constituye el medio de fijación (55) consiste en una rendija sobre la mitad posterior de la placa (50). Esta rendija (55) comprime el extremo posterior de la placa (50) cuando se introduce en los medios de fijación hembra (113, 223) de los elementos de adaptación anatómica superior y/o inferior (11, 22), como se ilustra para la placa a mano izquierda de la figura 6A. Cuando la placa (50) alcanza su extremo de freno de la rueda creada por los medios hembra (113, 223) de los elementos de adaptación anatómica (11, 22) y los medios (15, 25) de las placas (1, 2), significa que cuando co-opera con estos dos medios hembra al mismo tiempo, las aberturas (550) realizadas, por ejemplo, en los medios hembra (113, 223) de los elementos de adaptación anatómica (11, 22) separan la placa (50) del lugar en el que se encuentra, como queda ilustrado por medio de la placa a mano derecha de la figura 6A. La Figura 7B ilustra una vista en perspectiva de esta realización del medio de fijación en el que se pretende que la placa (50) quede albergada en el medio hembra (25) realizado en el medio de co-operación (23) de la placa inferior (2). Esta figura muestra también de forma notable el hecho de que el refuerzo presente, por ejemplo, sobre la placa anatómica inferior (22) puede ser más profundo que el espesor de la placa inferior (2). Dependiendo del tamaño de los medios de co-operación (23, 33) de la placa inferior (2) del núcleo (3), los bordes de este refuerzo proporcionan un estribo periférico que posiblemente limita el desplazamiento del núcleo (3), en relación con la placa inferior (2). En la realización que se ilustra en la figura 6B, las irregularidades formales que constituyen el medio de fijación (55) del medio intermedio macho (50) consisten en pasadores presentes sobre los bordes laterales de la placa (50). Las Figuras 7A y 8A a 8D ilustran otra realización alternativa del medio intermedio macho (50). En esta realización, las irregularidades formales de la placa (50) que constituyen el medio de fijación (55) de la placa (50), consisten en una perforación en el medio intermedio macho (50), prolongada por una perforación (550) en los medios de fijación hembra (113, 223) de los elementos de fijación anatómica (11, 22), como resulta particularmente visible en la figura 8B. Resulta evidente para los especialistas que la invención permite realizaciones en otras numerosas formas
10 específicas sin que ello suponga apartarse del alcance de la aplicación de la invención tal y como se reivindica. Como consecuencia de ello, las realizaciones deben considerarse a modo de ilustración, pero se pueden modificar dentro del alcance definido por el intervalo de las reivindicaciones adjuntas, y la invención no tiene que limitarse a los detalles presentados con anterioridad.

- 1.
- Fijación ósea (60) para fijar un implante intervertebral en el tejido óseo de una vértebra, en la que la fijación incluye una placa (61) que tiene un extremo distante que soporta una parte (63) curvada para plegarse sobre sí misma, que es capaz de ser enclavada sobre un medio para sujetar la fijación ósea (60) sobre dicho implante, para así retener dicho implante contra dicho tejido óseo, al tiempo que se permite la retirada de la fijación ósea (60) del implante, caracterizada porque la placa tiene una forma total rectangular alargada de anchura y espesor sustancialmente constantes.

- Fijación ósea (60) de acuerdo con la reivindicación 1, caracterizada porque dicha parte curvada (63) es capaz de ser enclavada sobre dicho medio para la sujeción de dicho implante formado por medio de un borde (16,26) de una abertura formada en las proximidades de la periferia de dicho implante.

- Fijación ósea (60) de acuerdo con la reivindicación 2, caracterizada porque dicha placa (61) es capaz de penetrar en el tejido óseo a través de dicha abertura de dicho implante.

- Fijación ósea (60) de acuerdo con cualquiera de las reivindicaciones 1 a 3, caracterizada porque dicha parte curvada (63) es capaz de ser enclavada sobre dicho medio de sujeción sobre dicho implante formado por un vástago en un borde (16, 26) de una abertura formada en las proximidades de la periferia de dicho implante, de manera que la fijación ósea (60) se pueda mover y permita ajustar el ángulo de dicha fijación ósea (60) con respecto al implante, por medio de la rotación de dicho vástago.

- Fijación ósea (60) de acuerdo con cualquiera de las reivindicaciones 1 a 3, caracterizada porque dicha parte curvada (63) se prolonga con una segunda placa (61).

- Fijación ósea de acuerdo con la reivindicación 5, caracterizada porque la segunda placa es más pequeña que la primera placa.

- Fijación ósea (60) de acuerdo con una cualquiera de las reivindicaciones 1 a 6, caracterizada porque las placas primera y/o segunda (61) está/están equipada(s) con muescas (62) orientadas para ejercer resistencia frente a la retirada de la fijación ósea (60) de la vértebra.

- Prótesis de disco intervertebral y fijación ósea (60) para fijar dicha prótesis de disco intervertebral, caracterizada porque dicha fijación ósea es una fijación ósea de acuerdo con la reivindicación 1 a 4 y porque la anchura de la placa (61) corresponde a la anchura de una abertura realizada en las proximidades de la periferia de la prótesis de disco intervertebral y que tiene un borde (16, 26) que forma dicho medio para sujetar la fijación ósea (60) sobre dicho implante.

- Prótesis de disco intervertebral y fijación ósea (60) para fijar dicha prótesis de disco intervertebral caracterizada porque dicha fijación ósea es una fijación ósea de acuerdo con cualquiera de las reivindicaciones 5 o 6, y porque la anchura de al menos una de las placas (61) corresponde a la anchura de una abertura realizada en las proximidades de la periferia de la prótesis de disco intervertebral y que tiene un borde (16, 26) que forma dicho medio de sujeción de la fijación ósea (60) sobre dicho implante.

- Prótesis de disco intervertebral y fijación ósea (60) de acuerdo con la reivindicación 7 u 8, caracterizada porque se pretende que dichos medios de sujeción de dicho implante estén en contacto con el tejido óseo, de forma que dicha placa (61) de la fijación penetre en el interior del tejido óseo hasta su extremo distante.

- Prótesis de disco intervertebral y fijación ósea (60) de acuerdo con cualquiera de las reivindicaciones 7 a 9, caracterizada porque dicho implante es un elemento de adaptación anatómica (11, 22) que amplia el diámetro total y/o la altura total de una prótesis de disco intervertebral y que tiene medios de sujeción sobre los cuales puede ser enclavada dicha parte curvada (63).
