Description

【発明の詳細な説明】 「技術分野」 本発明は、椎間板ヘルニアを外科的に治療する場合に、
椎間板の切除部に移植される椎体スペーサに関する。
「従来技術およびその間通点」 椎間板ヘルニアは、椎間板の退向変性によって中央の髄
核が逸出してを髄神経を圧迫し、激痛を起こすことで良
く知られている。最も一般的には、腰部椎間板ヘルニア
が知られているが、頚部の椎間板ヘルニアも転落、交通
事故等、外傷によって起こり、また日中あるいは夜間突
然発症することもある。
椎間板ヘルニアは、軽い症状であれば、固定することに
より治療可能であるが、重い症状の場合には、手術治療
を行なう。従来の手術では、病変椎間板を切除した上で
、上下の椎体の間に骨移植を行なう。骨移植には、患者
自身の腰骨の一部を移植するか、またはアルミナ、アパ
タイト等の人工骨を移植する方法がある。しかしながら
、これらの方法では、いずれにしても、本来可動性であ
る椎体が完全に固定されてしまう。
したがって、椎体の間で可動な移植物の開発が望まれて
いる。
「発明の目的」 本発明は、上記従来技術の欠点を解消し、椎体の間に容
易に移植でき、移植後に椎体の動きを可能ならしめ、異
和感を与えず、生体親和性の椎体スペーサを提供するこ
とを目的とする。
「発明の概要」 本発明の椎体スペーサは、外側表面にアパタイト層を有
する一対の端部体の間に、医療用合成ゴム成形体が結合
部材を介して挟持されていることを特徴とする。
本発明の椎体スペーサは、強度を持たせるため、端部体
は金属材料から作成するが、これを生体親和性とするた
めに、その表面にアパタイト層を設ける。このアパタイ
ト層は例えば溶射にょって設けることができる。このた
めアパタイトと金属とが同等の熱膨張率を有することが
望ましく、そのような金属としてチタンを用いることが
好ましい。
そしてこの端部体は、椎体に適切に嵌合し、かつ外れな
いように、突出部を有することが望ましい。
本発明において、医療用合成ゴムとしては、生体親和性
を有するゴムであればよく、例えばシリコーンゴム、ポ
リビニルアルコール、ポリウレタン等を使用することが
できる。
端部体と合成ゴムを結合する結合部材は、チタン、ステ
ンレス鋼等、適当な強度を有する任意の材料から形成さ
れていてよいが、チタンは高価であり、結合部材は合成
ゴム成形体中に埋め込まれてしまうので、ステンレス鋼
から形成するのが実際的である。
「発明の実施例」 以下図示実施例について本発明を説明する。本発明の椎
体スペーサは、一対の端部体１の間に、結合部材２によ
フて合成ゴム成形体３を挟持してなり、端部体１の外表
面にはアパタイト層４が設けられている。
チタンからなる端部体１は、偏平な直方体状をなしてい
て、その中心に外方に向かう逆円錐体状の突起１ａを有
している。この突起１ａは、手術によフて、隣り合う椎
体に対向させて形成した孔内に埋め込まれる。逆円錐体
の突起１ａは、椎体から外れるのを防止するために効果
的である。
チタンまたはステンレス鋼からなる結合部材２は、端部
体１よりやや小さい偏平直方体状の埋込部２ａと、この
埋込体２ａの中心部に設けられ、端部体１に当接する円
柱状突起２ｂと、この円柱状突起２ｂに設けられ、端部
体１に螺合されるねじ２Ｃとを有している。
この椎体スペーサの端部体１の外表面に設けるアパタイ
ト層４は、例えば溶射によって形成される。アパタイト
層４の厚さは、一般に０．１〜０．５ｍｍである。
結合部材２とねじ２Ｃによって結合された一対の端部体
１は、適切な間隔を置いて対向して型内に設置される。
そしてその間に合成ゴムラテックスを流し込み、常法に
より硬化させると、内部に結合部材２を埋め込んだ合成
ゴム成形体３が形成され、本椎体スペーサが得られる。
合成ゴム成形体３は、結合部材２の埋込部２ａと端部体
１との間に進入し、埋込部２ａは、端部体１とほぼ相似
形で、これより僅かに小さいだけなので、端部体１と強
固に結合され、椎間板に埋め込まれた後も剥離するおそ
れがない。
本発明の椎体スペーサの可動性は、ゴムの柔軟性を利用
するので、合成ゴム成形体３はある程度の厚さを必要と
し、スペーサ全体の大きさや結合部材２の大きさによっ
ても左右されるが、一般に５〜３５ＩＩＩｍ、好ましく
は、１０〜３０ＩＩＩｍの厚さとするのがよい。
なお端部体１および結合部材２の形状は、種々変形が可
能であり、図示した形状に限定されるものではない。
「発明の効果」 本発明による椎体スペーサは、柔軟性を有する合成ゴム
成形体が端部体の間に挟まれてし）るので、椎体の動き
に適合して弾性変形する。したかって本発明の椎体スペ
ーサを移植したを鎖部分は固定されず、自由に運動する
ことができ、健常な椎体に影響を及ぼさない。この可動
式椎体スペーサは、特に頚部に移植した場合に好適であ
り、患者はそれ程異和感を感じることなく首を動かすこ
とができる。さらに本発明の椎体スペーサは、端部体の
外表面にアパタイト層を設けて生体親和性とし、ゴム成
形体として医療用合成ゴムを用いているので、生体組織
に害を及ぼさない。

Claims (5)

    【特許請求の範囲】
  1. （１）外側表面にアパタイト層を有する一対の端部体の
    間に、医療用合成ゴム成形体が結合部材を介して挟持さ
    れていることを特徴とする可動式椎体スペーサ。
  2. （２）特許請求の範囲第１項において、端部体はチタン
    からなっている可動式椎体スペーサ。
  3. （３）特許請求の範囲第１項または第２項において、結
    合部材がチタンまたはステンレス鋼からなり、端部体に
    ねじ結合されている可動式椎体スペーサ。
  4. （４）特許請求の範囲第１項ないし第３項のいずれか一
    において、端部体は椎体と係合する突出部を有する可動
    式椎体スペーサ。
  5. （５）特許請求の範囲第１項ないし第４項のいずれか一
    において、医療用合成ゴムがシリコーンゴム、ポリビニ
    ルアルコール、ポリウレタンから選択されたものである
    可動式椎体スペーサ。
