Katheter
Die Erfindung betrifft einen Katheter zur Einführung in einen lebenden Körper, der durch das Gefaßsysteme eines Patienten mit einem
Minimum an Beschädigung oder Trauma bezüglich des endothelialen Gewebes führbar ist.
Die Einführung von schlauchförmigem Kathetern in das Gefäßsystem
eines Körpers wird heutzutage häufig verwendet, beispielsweise werden angiographische Katheter gewöhnlich in die Oberschenkelarterie eingeführt
und durch das Arteriensystem und zum Koronaostium geführt, so daß
strahlenundurchlässige Kontrastmittel oder Medikamente als Teil eines
diagnostischen oder Behandlungsverfahrens injiziert werden können.
Typischerweise umfassen derartige Katheter langgestreckte flexible
Plastikschläuche eines vorbestimmten kleinen Durchmessers, der beträchtlich geringer als der Querschnitt des Gefässes ist, durch das der Katheter
hindurchgeführt werden muß. Andere Katheterformen sind solche, die mit Herzschrittmachern verbunden sind. Hierbei besitzt der Katheter die' Form
einer Schrittmacherleitung, die gewöhnlich durch die subklaviane Vene und durch die obere vena cava in die rechte Herzkammer geführt wird. Hierbei
nimmt der Katheter die Form eines flexiblen Mantels an, der langgestreckte Leiter umgibt, die am distalen Ende in einer oder mehreren
Oberflächenelektroden enden, die geeignet sind, mit dem endokardialen
Herzgewebe in Berührung zu stehen.
Bekannte angiographische oder andere diagnostische Katheter als auch Herzschrittmacherleitungen werden gewöhnlich aus Polyäthylen,
Polyurethan, Polypropylen oder Silicongummi oder aus einem anderen nicht trombogenen Material hergestellt. Damit sie durch das Gefäßsystem ohne
Ausbeulen oder Falten hindurchgeführt werden können, müssen sie eine Wandstärke und eine Härte aufweisen, die ausreicht, um eras vaskulare
Gewebe zu beschädigen, insbesondere, wenn das distale Ende des Katheters stumpf ist oder eine scharfe Kante besitzt. Obwohl vielleicht selten, hat es
Unfälle gegeben, wo Katheter mit kleinem Durchmesser und scharfkantigem
Ende durch die Aortawandung gedrungen sind und den Tod des Patienten hervorgerufen haben.
Aufgabe der Erfindung ist es, die genannten Nachteile zu beseitigen.
Erfindungsgemäß besitzt der Katheter ein weiches deformierbares Endstück, das an dem distalen Ende des Katheters befestigt
ist. Bei einer Ausführungsform umfaßt das deformierbare Endstück ein ringförmiges aufblasbares flexibles nicht ausdehnbares Ballonelement, das an
dem distalen Ende des ansonsten relativ starren Endabschnitts des Katheters befestigt ist. Bei dieser Anordnung ist der Katheter selbst mit
einem Lumen versehen, das sich im wesentlichen über die gesamte Länge des Katheters erstreckt und mit dem deformierbaren Endstück in
Verbindung steht, wobei ein Fluid eingeführt werden kann, um Größe, Form und Festigkeit des Endabschnitts des Katheters zu steuern. Durch
wahlweises Aufblähen des ballonartigen Endstücks kann der wirksame Bereich des Endstücks vergrößert werden, um die Gesamtkraft pro Flächeneinheit
an der Berührungsstelle zwischen dem Endstücks des Katheters und einem Blutgefäß zu reduzieren.
Bei einer zweiten Ausführungsform umfaßt das weiche deformierbare Endstück eine ringförmige Plastik- oder Gummihülse, die das
ansonsten starre Ende des Katheters umgibt und sich normalerweise um einen vorbestimmten Abstand über dieses hinaus erstreckt. Die Dicke oder
Zusammensetzung der Hülse wird um eine Umfangslinie geändert, um eine
bevorzugte Stelle zum Falten und Kollabieren der Hülse zu schaffen, wenn das terminale Ende der Hülse gegen Körpergewebe gedruckt wird. Wenn die
Hülse kollabiert, vergrößert sich der effektive Kontaktbereich zwischen dem deformierbaren Endstück und dem Körpergewebe, um den Druck oder die
Kraft pro Flächeneinheit, die auf das Gewebe ausgeübt wird, zu reduzieren.
Hierdurch wird ein verbesserter Katheter geschaffen, durch den die Wahrscheinlichkeit der Beschädigung von Körpergewebe beim
Einführen des Katheters und beim Hindurchführen durch das Gefäßsystem verringert wird. Durch das aufblähbare Endstück kann die Form und die
Festigkeit oder Steifheit des distalen Endes des Katheters durch die Einführung eines Fluids am proximalen Ende des Katheters gesteuert
werden. Wenn das weiche deformierbare Endstück eine Hülse mit einem
vorbestimmten Wandstärkenprofil ist, so daß das Endstück in vorbestimmter
Weise kollabieren kann, kann wirksam der gesamte Kontaktbereich zwischen dem distalen Ende und dem Körpergewebe vergrößert werden.
Weitere Ausgestaltungen sind der nachfolgenden Beschreibung und den Ansprüchen zu entnehmen.
Die Erfindung wird nachstehend anhand der in den beigefügten Abbildungen dargestellten Ausführungsformen näher erläutert.

In Fig. 1 ist im Längsschnitt das distale Ende eines bekannten
angiographischen Katheters dargestellt, der aus einer vorbestimmten Länge
eines flexiblen Plastikschlauch^ Io besteht, der ein zentrales Lumen 11
aufweist, das sich über die gesamte Länge des Katheters von dessen
proximalem Ende (nicht dargestellt) bis zu seinem distalen Ende 12 erstreckt. Das Lumen 11 ist vorgesehen, damit angiographische Farbstoffe
oder andere Medikamente in dem Katheter an seinem proximalen Ende außerhalb des Körpers eingeführt und durch das Lumen zu einevorbestimmten
Stelle im Körper, an dem sich das distale Ende 12 des Katheters befindet, geführt werden können.
Das distale Ende 12 des Katheters von Fig. 1 besitzt eine abrupte
Kante 13, die relativ scharf ist, was von der Art des Materials, das für
den Körper des Katheters Io verwendet wurde, abhängt und genügend
Festigkeit besitzen kann, um das vaskulare Gewebe, insbesondere die
endotheliale Schicht (tunica intima), die sich auf der Innenseite der Blutgefäße befindet, zu beschädigen, wenn der Katheter durch das
Gefäßsystem geführt wird.
Um dieses Problem zu vermeiden, ist gemäß einer ersten in Fig. 2
dargestellten Ausführungsform der Katheterkörper Io mit einem Doppellumen versehen, nämlich dem Lumen 11, zum Ermöglichen des
Durchflusses angiographischer Farbstoffe oder anderer Flüssigkeiten und einem weiteren Lumen 14, das über die gesamte Länge des Katheters
verläuft. Am distalen Ende des Katheterkörpers Io ist ein flexibles, nicht
ausdehnbares, aufblähbares ballonartiges Endstück 15, das in expandierter Form allgemein Toriodgestalt annimmt. Das Lumen IA steht mit dem
Inneren des Torus, den das Enstück 15 bildet, in Verbindung, während das Lumen 11 mit der zentralen öffnung des Toroids in Verbindung steht.
Während Flüssigkeit, die in das Lumen 11 am proxialen Ende des Katheters
eingeführt wird, schließlich durch die öffnung 17 des weichen flexiblen
Endstücks 15 ausfließen kann, strömt in das Lumen 14 eingeführtes Fluid in die ringförmige Kammer 16 und bewirkt ein Aufblähen des Endstücks 15.
Das Endstück 15 wird vorzugsweise aus einem bioaxial orientierten oder vernetzten Polyolefin-Film gebildet und kann eine Stärke in der
Größenordnung von o, .oo254 bis o,oo635 cm aufweisen. In nichtaufgeblähtem oder evakuiertem Zustand besitzt es keine Starrheit und
legt sich um und entspricht im wesentlichen der Form des Endstücks des Katheterkörpers lo. Wenn es geeignet unter Druck gesetzt wird, expandiert
es, um den gewünschten wirksamen Kontaktbereich zwischen dem Ende des Katheters und irgendwelchem Körpergewebe zu vergrößern, gegen das es
stößt, wenn es durch den Körper geführt oder in diesem positioniert wird.
Der Grad des Aufblähens bestimmt den Grad der Weichheit oder Deformierbarkeit des Endstücks. Daher besitzt der Arzt eine gute Kontrolle
über die Endstückeigenschaften des Katheters, so daß eine Gewebebeschädigung minimalisiert wird.
Das toroidale ballonartige Endstück 15 wird vorzugsweise getrennt von dem Katheterkörper geformt und dann durch thermisches oder
chemisches Verkleben daran befestigt, kann jedoch auch als Hohlraum in den Wandungen des distalen Endes des Katheters ausgebildet sein, wodurch
bewirkt wird, daß bei Füllen mit einem Druckfluid die Wandung des
Katheters selbst ausgebeult wird.
Fig. 3 zeigt eine alternative Ausführungsform, bei der eine
ausdehnbare Elastomerhülse 18 zum Steuern des Innendrucks des nicht ausdehnbaren Endstücks 15 vorgesehen ist. Die ausdehnbare Elastomerhülse
18 kann an irgendeiner Stelle längs der Länge des Katheterkörpers Io und
insbesondere am extremen proximalen Ende des Katheters an einer Stelle
angeordnet sein, die außerhalb des Körpers während des Einführens des
Katheters bleibt. Die Elastomerhülse 18 ist vorzugsweise in einer ringförmigen Ausnehmung in der Außenseite des Katheterkörpers Io
angeordnet und in geeigneter Weise fluiddicht mit dem Katheterkörper Io
an den Endkanten hiervon, wie durch die Bezugsziffern 19 bzw. 2o angedeutet, etwa durch Kleben, verbunden. Eine Radialbohrung 21 erstreckt
sich von der Ausnehmungsf lache des Katheterkörpers Io zu dem zweiter
Lumen IA. Die Radialbohrung 21 als auch der distal hierzu befindliche Teil
des Lumens 14 und die Kammer 16 des deformierbaren Endstücks 15 sind
mit einem inkompressiblen Fluid gefüllt, wobei ein geeigneter Stopfen 22 in dem Lumen 14 an einer Stelle proximal zu der Radialbohrung 21
angeordnet ist.
Wenn eine Kraft auf das Endstück 15 von Fig. 3 ausgeübt wird, wird
der Innendruck in dem Endstück 15 vergrößert, wodurch ein Ausbeulen der Elastomerhülse 18 und ein entsprechendes Zusammendrücken des Endstücks
15 bewirkt wird. Der Grad des Zusammendrückens ist proportional zu der
auf das Endstück 15 ausgeübten Kraft, die ihrerseits eine Funktion der
"Steifheit" der Elastomerhülse 18 ist. Eine Abnahme des Volumens in dem
Endstück 15 resultiert in einer Vergrößerung des Kontaktbereichs zwischen
diesem und der damit in Eingriff stehenden Fläche, gegen die es gedrückt
wird.
Wenn die Hülse 18 an einer Stelle außerhalb des Körpers ist, kann sie in geeigneter Weise mit einem Wandler gekoppelt werden, der direkt
den Fluiddruck in dem System anzeigt, der die Expansion der Hülse 18 hervorruft.
Bei dem Endstück gemäß Fig. 3 ist es zweckmäßig, zum Bilden der ringförmigen öffnung 17 einen schwereren Toroidalring 23 vorzusehen. Der
Ring 23 besitzt einen Außendurchmesser, der nicht größer als der Innendurchmesser irgendeines Einführers ist, durch den der Katheter
während der Implantierung hindurchgeführt werden muß. Bei dieser

Konstruktion wird sich das Endstück 15 an ein Volumen kleiner als der
vollständig ausgedehnte Durchmesser des Endstücks 15 anpassen, da dann, wenn die Kraft, die auf die Wandung ausgeübt wird, ansteigt, der
Innendruck in dem Endstück 15 aufgrund der Expansion der Elastomerhülse·
IB ansteigt.
In der Praxis kann das Lumen 14 einen Durchmesser im Bereich von
o,oo5 bis o,o 1 cm aufweisen und sich von dem Endstück 15 durch die
radiale Seitenöffnung 21 diese umfassend erstrecken. Das Lumen 14 kann bis zum proximalen Ende des Katheters geführt sein, wo die Elastomerhülse
18 und die eingeschlossene Seitenöffnung 21 angeordnet sein können.
Wie bei der Ausführunsform von Fig. 2 ist das Endstück 15 vorzugsweise aus einem nicht ausdehnbaren Polyolefin-Film hergestellt und
kann eine Stärke im Bereich von o,oo2.5 bis o,oo64 cm aufweisen.
Bei der in Fig. 4 dargestellten Ausführungsform besitzt der Katheter
ein daran befestigtes weiches defomierbares Endstück, das eine schlauchförmige Hülse 24 umfaßt, die das distale Ende des Katheterkörpers
Io umgibt und an diesem mittels einer Überlappungsverbindung verbunden
ist, wobei sich die Hülse 24 um eine vorbestimmte Länge über das distale
Erde hinaus erstreckt. Anstatt von konstanter Wandstärke sind die

Wandungen der flexiblen t-'lastikhülse 24 so gestaltet, daß dann, wenn das
extreme Ende 25 hiervon gegen ein stationäres Objekt mit einer vorbestimmten geringen Kraft auftrifft, das Endstück kollabiert, um die in
Fig. 5 dargestellte Konfiguration anzunehmen. Die Wandstärke des
Abschnitts des Endstücks, der sich über das Ende des Katheters Io

hinauserstreckt, ist allgemein konstant, jedoch an der Stelle 27 längs einer
ringförmigen Linie geschwächt, d.h.mit einer geringeren Wandstärke
versehen, so daß sich die Hülse um diese Umfangslinie wie ein Gelenk falten kann. Dementsprechend faltet sich das weiche kompressible
Endstück 24 längs des Außenumfangs des Endes des Katheters bei 28 wie
ein Gelenk. Anstelle der Reduzierung der Wandstärke der Hülse zum Erzeugen einer bevorzugten Umlenk- oder Faltlinie ist es auch möglich, das
gewünschte Faltverhalten dadurch zu erreichen, daß eine Diskontinuität in dem Hülsenmaterial vorgesehen wird, so daß dieses eine andere (niedrigere)
Steifheit als das benachbarte Material aufweist.
Durch geeignete Wahl der Härte des Plastikmaterials für das weiche
kollabierbare Endstück 24 besitzt dieses eine "Gedächtnis"-Eigenschaft, so
daß dann, wenn das Endstück nicht langer in ein stationäres Objekt
gedrückt wird, es in die in Fig. 4 dargestellte Form zurückkehrt. Es ist vorteilhaft, die Materialmenge, die die zentrale öffnung 29 umgibt, in dem
Endstück, wie bei 3o angedeutet, zu vergrößern. Diese Materialvergrößerung definiert einen Ring, der die Öffnung stabilisiert und verstärkt und am
Einsinken hindert.
Aus Fig. 5 ist ersichtlich, daß dann, wenn das Endstück kollabiert, cer
wirksame Kontaktbereich zwischen diesem und dem Objekt, gegen das er gedrückt wird, vergrößert wird, wodurch der effektive Druck, der auf das
Gewebe durch den Katheter ausgeübt wird, vermindert wird.
Das Lumen des Katheters kann beispielsweise einen Durchmesser von 0,114 cm und eine Wandstärke von o,o22 cm an seinem distalen Ende
aufweisen. Das weiche Endstück 24 in den Fig.4 und 5 kann vorzugsweise
etwa o,ol27 cm dick sein, sich jedoch zu etwa ο,οΐο cm Stärke an der
kreisförmigen Umfangslinie 27 verringern. Im undefomierten Zustanc kann
das distale Ende einen Querschnittsbereich von etwa o,oo77 cm2 urd irr
kollabierten Zustand gemäß Fig. 5 einen Bereich von o,o26 5 cm2 aufweisen, was eine Bereichvergrößerung um das 3,41-fache darstellt.
Bei der in Fig. 6 dargestellten Ausführungsform ist am distalen Ende
des Katheters ein schlauchförmiges zylindrisches Endstück 31 befestigt, das aus Silastic oder einem anderen weichen deformierbaren Plastikm^tenal
besteht. Wenn es auf ein Hindernis stößt, wird sich das weiche Plastikendstück durch Verbreitern und Biegen deformieren, unr einen
vergrößerten Kontaktbereich in bezug auf das Hindernis zu bilden.

Katheter nach einem der Ansprüche 7 bis lo, dadurch gekennzeichnet, daß der Außendurchmesser der Hülse der gleiche wie derjenige des langgestreckten flexiblen Schlauchs (lo) ist.
