Description
In einem Aufsatz mit dem Titel "Approaches to Prevention
of Epithelial Cancer During The Preneoplastic Period" in der Zeitschrift Cancer Research, Bd. 36 (1976), S. 2699 bis
2702, wird nachgewiesen, daß die Todesfälle für verschiedene übliche Formen maligner epithelialer Tumoren im Zeitraum von
to 1950 bis 1970 entweder zunahmen oder zumindest keine Abnahme
zeigten. Diese malignen epithelialen Tumoren traten an der Lunge und Pankreas bei Männern und Frauen, dem Colon und der
Blase bei Männern, der Brust und den Ovarien bei Frauen auf. Im allgemeinen wartet man bei malignen epithelialen Tumoren,
bis ein invasiv wachsender Tumor nachgewiesen werden kann, der dann mit zytotoxischen Chemotherapeutika, durch chirurgischen
Eingriff oder Bestrahlung behandelt wird. Keine dieser Therapien war besonders erfolgreich bei der Behandlung sämtlicher
Typen maligner epithelialer Tumoren, obwohl einige Portschrit-
20 te erzielt wurden.
Als Alternativweg zur Bewältigung des Problems der malignen epithelialen Tumoren wurde vorgeschlagen, die Krankheit als
einen Prozeß anzusehen, der viele Jahre dauert, bis er sein letztes, invasives Stadium beim Menschen erreicht, und
der durch physiologische oder pharmakologische Mechanismen während
seiner frühen Stufen gesteuert werden kann mit dem Ziel der Verhinderung der Phase invasiven Wachstums.
Retinoide spielen eine wesentliche Rolle bei der Steuerung der normalen Differenzierung von Epithelgewebe, Sie sind daher
wichtig zur Steuerung prämaligner epithelialer Zelldifferenzierung
. Es wurde festgestellt, daß Retinoide eine zelluläre Ausmerzung hyperplastischer und anaplastischer Läsionen bewirken, die
durch chemische Carcinogene verursacht werden. Ferner konnte nachgewiesen werden, daß Retinoidmangel bei Versuchstieren die

Empfindlichkeit gegen chemische Carcinogenese verstärkt. Retinoi de spielen eine wesentliche Rolle für die normale celluläre Differenzierung
der Epithelien, die für mehr als die Hälfte der gesamten primären Tumoren bei Männern und Frauen verantwortlich
sind. Diese Epithelien umfassen solche der Bronchien und Luftröhre, Magen, Darm, Uterus, Nieren und Blase, Testes, Prostata,
Pankreasgang und Haut. Bei Fehlen von Retinoiden in der Nahrung erfolgt in diesen Epithelien keine normale celluläre Differenzie
rung.
Natürliche Retinylester, wie Retinylacetat und Retinylpalmitat,
sowie Retinsäure sind in hohen Dosen sehr giftig und haben deshalb keinen praktischen Wert zur Verhinderung der Bildung maligner
Tumoren bei höheren Warmblütern. Vor kurzem wurde festgestellt, daß synthetische Retinoide, wie 13-cis-Retinsäure, wesentlich
weniger toxisch sind als Retinsäure oder die natürlichen Retinylester, und daß sie auch eine stärkere Wirkung bei
der Unterdrückung der chemischen Carcinogenese ausüben; vgl. "13-cis-Retinoic Acid:Inhibitor of Bladder Carcinogenesis in
the Rat", Science, Bd. 195 (1977), S. 487 bis 489, sowie "13-cis-Retinoic
Acid: Inhibition of Bladder Carcinogenesis Induced in Rats by N-Butyl-N-(4-hydroxybutyl)-nitrosamine", Science,
Bd. 198 (1977), S. 7^3 bis 744. Es wurde festgestellt, daß 13-cis-Retinsäure
nicht besonders wirksam ist gegen Brustkrebs im
25 nachstehend erläuterten Versuch am Rattenmodell.
Neuere Entwicklungen auf diesem Gebiet sind auch in einem Aufsatz mit dem Titel "Prevention of Chemical Carcinogenesis by
Vitamin A and its Synthetic Analogs (Retinoids)", Federation
30 Proceedings, Bd. 35 (1976), S. 1332 bis 1338, beschrieben. In diesem Aufsatz wird darauf hingewiesen, daß es ein Ziel ist,
für die praktische Anwendung beim Menschen und anderen Warmblütern, stark wirksame synthetische Retinoide zu entwickeln,
die sich durch eine niedrige Toxizität und eine hohe Gewebe-
spezifität gegen maligne Tumoren bei beliebigen Organen auszeichnen;

Der Erfindung liegt die Aufgabe zugrunde, neue all-trans-Retinsäureamide
zu entwickeln, die sich durch eine niedrige systemische Toxizität, gute Wirkung bei der Verhinderung von malignen
epithelialen Tumoren in Warmblütern bei tolerablen Dosen und ausreichende Spezifitat gegenüber einer oder mehreren
üblichen Stellen epithelialer Tumoren in Warmblütern, wie Brust, Blase, Colon, Lunge und Pankreas, auszeichnen. Diese
Aufgabe wird durch die Erfindung gelöst.
Die Erfindung betrifft somit den in den vorstehenden Patentansprüchen
gekennzeichneten Gegenstand.

Die Verbindungen der Erfindung werden nach an sich bekannten Methoden durch Acylierung von N-(4-Hydroxyphenyl)-all-transretinsäureamid
mit einer Carbonsäure der allgemeinen Formel II
30 CnH2n+1~C00H (II)
oder deren reaktionsfähigem Derivat hergestellt. Beispiele für reaktionsfähige Derivate sind die Anhydride.
Die Verbindungen der Erfindung werden systemisch, vorzugsweise oral, in üblichen Darreichungsformen gegeben. Zweckmäßig wird
der Arzneistoff in regelmäßigen Zeitabständen, im allgemeinen

zu den Mahlzeiten, oder einmal täglich gegeben. Tagesdosen in der Größenordnung von 50 mg/kg oder weniger sind wirksam zur
Verhinderung maligner epithelialer Tumoren.
Die Beispiele erläutern die Herstellung einiger erfindungsgemäßer Verbindungen.
Beispiel 1 Herstellung von N-(4-Acetoxyphenyl)-all-trans-retinsäureamid
15 In einem Dreihalskolben, der mit einer Folie abgedeckt ist, wird
eine Lösung von 5,00 g (0,0128 Mol) N-(2J-Hydroxyphenyl)-alltrans-retinsäureamid
in 10 ml Pyridin vorgelegt. Diese Lösung wird mit einer Lösung von 1,65 g (0,0162 Mol) Essigsäureanhydrid
in 5 ml Pyridin versetzt. Danach wird der Kolben eine Stunde in
20 einem ölbad auf 950C erhitzt. Während dieser Zeit wird in den
Kolben Stickstoff eingeleitet und die Lösung mit einem Magnetrührer gerührt. Nach beendeter Umsetzung wird der Kolben in
einem Eisbad auf Raumtemperatur abgekühlt und der Kolbeninhalt in 150 ml kaltes destilliertes Wasser gegossen. Der sich abschei-
25 dende gelbe Peststoff sowie die wäßrige Phase werden in einen
Scheidetrichter überführt und mit 300 ml Diäthyläther ausgeschüttelt, bis der Feststoff in Lösung gegangen ist. Die wäßrige
Phase wird verworfen und der Ätherextrakt viermal mit jeweils 50 ml kaltem destilliertem Wasser gewaschen. Danach wird der
Ätherextrakt über Natriumsulfat getrocknet, filtriert und unter vermindertem Druck eingedampft. Der gelbe feste Rückstand
wird aus Methanol umkristallisiert (40 ml/g). Die Titelverbindung
schmilzt bei 179 bis l8l°C. Das NMR-Spektrum bestätigt die
Strukturformel.

Herstellung von N-(4-Propionyloxyphenyl)-all-trans-retinsäureamld
Gemäß Beispiel 1 werden in einem Dreihalskolben 5,00 g (0,0128
Mol) N-(4-Hydroxyphenyl)-all-trans-retinsäureamid in 10 ml
Pyridin vorgelegt. Diese Lösung wird mit einer Lösung von 2,08 g (0,0l60 Mol) Propionsäureanhydrid in 5 ml Pyridin versetzt.
Sodann wird der Kolben eine Stunde in einem Ölbad auf 95°C erhitzt. Während dieser Zeit wird in den Kolben Stickstoff
eingeleitet und die Lösung mit einem Magnet rührer gerührt, Na.ch beendeter Umsetzung wird der Kolben in einem Eisbad auf Raumtemperatur
abgekühlt und der Kolbeninhalt in 150 ml kaltes de- stilliertes Wasser gegossen. Der sich abscheidende gelbe Feststoff
und die wäßrige Phase werden in einen Scheidetrichter überführt und mit 200 ml Diäthyläther ausgeschüttelt, bis der
Feststoff in Lösung gegangen ist. Die wäßrige Phase wird verworfen» Der Ätherextrakt wird viermal mit jeweils 40 ml kaltem
destilliertem Wasser gewaschen. Danach wird der Ätherextrakt .
über Natriumsulfat getrocknet, filtriert und unter vermindertem Druck eingedampft. Der gelbe feste Rückstand wird aus Methanol
umkristallisiert (31I ml/g). Die Titelverbindung schmilzt bei
170 bis 171°C. Das NMR-Spektrum bestätigt die Strukturformel.

Herstellung von N-(4-n-Butyryloxyphenyl)-all-trans-retinsäureamid
In den in Beispiel 1 beschriebenen Dreihalskolben wird eine Lösung
von 5,00 g (0,0128 Mol) N-(4-Hydroxyphenyl)-all-transretinsäureamid
in 10 ml Pyridin vorgelegt. Diese Lösung wird

mit einer Lösung von 2,53 g (O,Ol60 Mol) n-Buttersäureanhydrid
in 5 ml Pyridin versetzt. Danach wird der Kolben eine Stunde in einem ölbad auf 95°C erhitzt. Während der Umsetzung wird in den
Kolben Stickstoff eingeleitet und die Lösung mit einem Magnetrührer gerührt. Nach beendeter Umsetzung wird der Kolben in
einem Eisbad auf Raumtemperatur abgekühlt und der Kolbeninhalt in 150 ml kaltes destilliertes Wasser gegossen. Die entstandene
Fällung wird abfiltriert, mit Wasser gewaschen und unter vermindertem
Druck getrocknet» Der trockene Feststoff wird aus einem Gemisch von Chloroform und Hexan (1:1) umkristallisiert
(10 ml/g). Das NMR-Spektrum bestätigt die Struktur. Die Titelverbindung
schmilzt bei 175 bis 1760C.

Beispiel 4
N-(4-Acetoxyphenyl)-all-trans-retinsäureamld und N-^-Propionyl·
oxyphenyl)*rall-trans-retinsäureamid werden mit Retinsäure in
einem in vitro Screening-Modellversuch zur Identifizierung von . Retinoiden mit Aktivität bei der Unterdrückung der Carcinogenese
in Epithelgewebe verglichen. Die Versuchsmethodik, die in Experimental Lung Cancer: International Symposium, 1974, S.575 bis
582, beschrieben ist, besteht in der Umkehrung der Keratin!- sierung bei einer Trachea-Organkultur.
Kurz zusammengefaßt unterliegt in Abwesenheit von Retinsäure 3C) oder eines synthetischen Retinolds mit ähnlicher Aktivität die
Testorgankultur des Trachea-Epithels einer anormalen Differen-
—9 zierung. Der Zusatz geringer Mengen Retinsäure (10
Mol) verursacht eine Reversion in normales Trachea-Epithel.. Die zu untersuchenden Verbindungen werden hinsichtlich ihrer
Aktivität mit Retinsäure oder einem anderen aktiven Standard sowohl hinsichtlich der Hornzellen-Metaplasie und der Keratin-

bildung verglichen. Beide sind ein Maß für die anormale Entwicklung
der Epithelkultur.
Alle Tracheaepithelkulturen wurden die ersten drei Tage in einem Medium ohne Retinoid gezüchtet. Zu diesem Zeitpunkt werden
einige Tracheaepithelkulturen isoliert, während der Rest eine weitere Woche in einem Medium gezüchtet wurde, das entweder
kein Retinoid oder Retinoid in den angegebenen Konzentrationen enthielt. Diese Tracheaepithelkulturen werden am
10. Tag der Züchtung gesammelt. Die Kulturen werden hinsichtlich ihres Prozentsatzes des gesamten Epithels, das Hornzellen-Metaplasie
bei acht Querschnitten von der Mitte jeder Trachea zeigt, eingeteilt. Wenn mehr als 40 % der gesamten Epithellänge,
verhornt ist, wird dieser Zustand als starke Hornzellen-Metaplasie
bezeichnet. Ein Wert von 10 bis 40 % wird als ausgeprägt bezeichnet, während ein Wert von 2 bis 10 als mild und
ein Wert unter 2 % als minimal bezeichnet wird.
Die nachstehend in Tabelle I zusammengefaßten Ergebnisse zeigen, daß die beiden Verbindungen der Erfindung eine Aktivität
in der gleichen Größenordnung zeigen, wie Retinsäure bei einer Konzentration von 10" Mol. Bei einer Konzentration von 10""°
Mol ist das N-(4-Propionyloxyphenyl)-all-trans-retinsäureamid praktisch ebenso wirksam wie Retinsäure, während die Acetoxyphenylverbindung
etwas weniger aktiv ist bei der Reversion der Keratinisierung.

N-(M-Propionyloxyphenyl)-all-trans-retinsäureamid wird mit Retinylacetat
hinsichtlich seiner Wirkung beim Brustkrebs der Ratte untersucht» Die Versuchsmethodik ist in Nature, Bd. 267
(1977), S. 620 bis 621, beschrieben. Es wird die Fähigkeit zur
Hemmung der Mammacarcinogenese durch N-Methyl-N-nitrosoharnstoff
(MNU) bestimmt. Die Versuchsbedingungen und die Ergebnisse nach 8 Wochen Untersuchung sind in Tabelle II zusammengefaßt. Die
Verbindung der Erfindung ist praktisch ebenso wirksam wie Retinylacetat, jedoch wesentlich weniger giftig, wie sich aus den
Durchschnittswerten für die Körpergewichte ergibt.
