Description

生物可相容之傷口敷料
本發明大體而言係關於促進組織成長之系統或方法,且更詳言之,是關於當暴露於減壓下時可提升組織生長之組織生長介質之多層傷口敷料。
開放性傷口藉真空引致痊癒最近已經由San Antonio,TeX.的Kinetic Concepts,Inc.經由其量產之V.A.C.產品線推廣。真空引致之痊癒過程已敘述於共同讓與Zamierowski之1990年11月13日頒予之美國專利號4,969,880以及其連續以及部份連續申請案之於1992年3月31日頒予之美國專利號5,100,396、1993年11月16日頒予之美國專利號5,261,893及1996年6月18日頒予之美國專利號5,527,293中。該等揭示併入本文供參考。真空引致之痊癒過程之進一步改善及改良亦敘述於2000年6月6日頒予Zamierowski之美國專利號6,071,267、以及分別於1997年6月10日頒予Argenta等人之美國專利號5,636,643及5,645,081,該等揭示全文併入本文供參考。

如熟悉本技藝者所習知,表面傷口之閉合涉及傷口附近之上皮及皮下組織之向內部移行。此移行通常經過發炎過程協助,因而使血液流動增加且使各種功能性細胞種類活化。經由該發炎過程,流經受損或破裂血管之血液因毛細管程度之閉塞而停止;隨後,開始清除及重建操作。不幸地是,當傷口大且被感染時會使該過程受到阻礙。此傷口中,在接近傷口表面會形成鬱血區(亦即,其中組織之局部膨潤限制了血液流經組織之區域)。
因血液流動不足,因此傷口周圍之上皮及皮下組織不僅接受到較少量之氧氣及營養份,而且無法成功的抵抗細菌感染且因此無法使傷口自然閉合。在真空引致之療法出現之前,此種不易處理之傷口僅可經由使用縫合或緊固帶(staples)加以解決。雖然此種機械閉合技術仍被廣泛使用且經常有效,但其主要缺點為對傷口附近之皮膚組織產生張力。尤其,使用縫合或緊固帶達到閉合所需之張力可能在縫合或緊固帶插入點處造成極高之局部應力。此等應力通常會造成插入點處之組織破裂,最終會造成傷口裂開及額外之組織損失。
此外,有些傷口因感染而變硬及發炎至藉緊固帶或縫合無法進行閉合之程度。藉由縫合或緊固帶無法挽回之傷口通常需要延長住院時間,而伴隨產生高經濟負擔及需要主 要的手術程序如周圍組織移植。不易藉緊固帶或縫合處置之傷口實例包含大且深的開放性傷口;褥瘡;源自慢性骨髓炎之潰瘍;及部分表層厚度燒傷其隨後發展成全皮層厚度燒傷。
機械閉合裝置之該等及其他缺點的結果,而發展出藉施加連續負壓而使傷口排膿之方法及裝置。當對該傷口之大面積施加此等負壓發現可促進表皮及皮下組織朝向傷口內部移行。實務上,對傷口施加負壓已由本受讓人或其母案以稱為"真空輔助性閉合"(或"V.A..C.")療法加以商業化,典型上包含傷口之似機械性收斂同時移除過量體液。此方式中,V.A.C.療法擴大了身體的自然發炎過程同時減緩了許多已知的固有副作用,例如對適當的靜脈血回流缺乏必要血管結構而因增加血流引起之紅腫產生。
雖然V.A..C.療法在促進傷口閉合方面已相當成功,但對於先前經過大部分處置不足之許多傷口的癒合仍留有些許困難。由於V.A..C.療法必然性質為在大氣中之癒合傷口部位,因此此療法經常必須進行至排除其他有利益且因此所需的傷口處置形式。迄今被排除的形式中,其一為藉由就地提供細胞生長提升基質而促使細胞生長。
在經常更換傷口敷料方面也有其他困難。由於傷口閉合,可能發生細胞組織與該傷口敷料結合。使用傳統的V.A..C.療法必然需要定期更換敷料。若細胞組織過度生長至該敷料中,則魯莽地更換敷料可能導致傷口有些組織損傷。
現有組織敷料存在之問題係經由本發明之系統及方法獲得解決。依據本發明之一具體例,係提供一種減壓輸送系統,係對組織部位施加減壓組織治療。該減壓輸送系統包含具有組織接觸層、釋離層及集流腔層之多層減壓輸送裝置。該組織接觸層包含適合與組織部位接觸之支架。該釋離層包含水凝膠形成材料及複數個流動通道,且該集流腔層包含分配集流腔。該釋離層位在組織接觸層及集流腔層之間,且釋離層之水凝膠形成材料與組織接觸層及集流腔層之至少一者結合。減壓輸送管與集流腔層流體相通以對組織部位傳送減壓。
依據本發明另一具體例,多層減壓輸送裝置包含具有適合與組織部位接觸之支架之第一層及具有水凝膠形成材料及複數個流動通道之第二層。該水凝膠形成材料與支架接觸。該減壓輸送裝置又包含具有與水凝膠形成材料接觸之分配集流腔之第三層。
本發明又另一具體例中,多層減壓輸送裝置包含組織接觸、集流腔層及釋離層。該組織接觸層包含適合與組織部位接觸以自組織部位接收生長中之新組織之支架。該組織接觸層又包含第一複數個流動通道。該集流腔層包含孔狀材料及第三複數個流動通道,該孔狀材料可將減壓分配於組織部位。該釋離層位在組織接觸層及集流腔層之間且包含與組織接觸層及集流腔層之至少一者接觸之水凝膠形成材料。水凝膠形成材料適於在吸收流體後形成水凝膠而釋 離組織接觸層及集流腔層之至少一者。該釋離層又包含使流體與第一及第三複數個流動通道相通之第二複數個流動通道。
依據本發明又另一具體例,減壓輸送裝置係用於對組織部位施加減壓組織治療。該減壓輸送裝置包含:適合與組織部位接觸以自組織部位接收生長中之新組織之支架;適於經由支架將減壓分配於組織部位之分配集流腔;及位在支架及分配集流腔之間且與該等接觸以實質上防止支架與分部集流腔之配置有釋離材料之區域彼此接觸。
且依據本發明,減壓輸送裝置之另一具體例包含具有分部集流腔、支架及水凝膠形成材料之減壓輸送裝置。該分配集流腔分配一減壓,且支架激勵自組織部位生長中之新組織。該分配集流腔及支架係以位在分配集流腔及支架間之水凝膠材料結合在一起。該系統又包含一減壓輸送管,其具有與分配集流腔流體相通之遠端以經由該分配集流腔及支架將減壓輸送至組織部位。
依據本發明另一具體例,係提供一種促進組織部位之新組織生長之組織生長套組。該組織生長套組包含具有第一側及第二側之支架,該第一側適合與組織部位接觸;水凝膠形成材料適合與支架之第二側接觸;且分配集流腔適合與水凝膠形成裝置接觸,而可經該支架將減壓分配於組織部位。
本發明又另一具體例中,係一種促進組織部位之新組織生長之方法,其包含配置支架使之與該組織部位接觸,配 置水凝膠形成材料使之與該支架接觸,及配置集流腔使之與該水凝膠形成材料接觸。經該集流腔及支架對組織部位施加減壓。
本發明又另一具體例中,係一種促進組織部位之新組織生長之方法,其包含配置與該組織部位接觸之多層減壓輸送裝置。該多層減壓輸送裝置包含具有適合與組織部位接觸之支架之組織接觸層及具有分配集流腔之集流腔層。該減壓輸送裝置又包含具有水凝膠形成材料及複數個流動通道之釋離層。該釋離層位在該組織接觸層及該集流腔層之間,且釋離層之水凝膠形成材料與該組織接觸層及該集流腔層之至少一者結合。該多層減壓輸送裝置係經定向使得該組織接觸層與該組織部位接觸。經由該分配集流腔、該流動通道及支架對組織部位施加減壓。
本發明另一具體例中,係一種促進組織部位之新組織生長之方法,其包含配置多層減壓輸送裝置使其與組織部位接觸。該多層減壓輸送裝置包含具有適於與組織部位接觸之支架之第一層,及具有水凝膠形成材料及複數個流動通道之第二層。該水凝膠形成材料與支架接觸。具有分配集流腔之第三層與水凝膠形成材料接觸。該多層減壓輸送裝置係經定向使得該組織接觸層與該組織部位接觸,且經由該分配集流腔、該流動通道及該支架對組織部位施加減壓。
依據本發明又另一具體例,係一種促進組織部位之新組織生長之方法,其包含配置支架使之與該組織部位接觸, 配置水凝膠形成材料使其與該支架接觸,且配置分配集流腔使其與水凝膠形成材料接觸。藉由經該集流腔及支架對組織部位施加減壓而刺激該組織部位之新組織生長。

下列較佳具體例之詳細敘述係參考形成其部份之附圖,且以執行本發明之最佳具體例之說明顯示。此等具體例之詳細敘述足使熟悉本技藝者有能力執行本發明,且應了解可使用其他具體例且可在不脫離本發明之精神或範圍內進行邏輯上之結構、機械、電性及化學性改變。為了避免對於熟悉本技藝者在執行本發明上不必要之細節,在敘述上可略去某些熟悉本技藝者已知之資料。因此下列詳細敘述並非用於限制且本發明之範圍僅以附屬之申請專利範圍加以界定。
本文所用之名詞"集流腔"通常係指有助於施加減壓或輸送流體至組織部位或自組織部位移除流體所提供之材料或結構。集流腔通常包含複數個相通之流動通道或路徑,以改善施加於集流腔四周之組織區域之流體分配或自該組織區域移除流體。集流腔實例可包含(但不限於)具有排列以形成流動通道之結構元件、孔窩狀發泡體(如:開放孔窩狀發泡體)、多孔組織集合體及包含流動通道或硬化使之包含流動通道之液體、凝膠及發泡體之裝置。
本文所用之名詞"減壓"通常係指對欲進行治療之組織部 位的壓力低於周圍壓力之壓力。大部分情況下,該減壓將比病患所處之大氣壓低。雖然可使用名詞"真空"及"負壓"敘述施加於組織部位之壓力,但施加於組織部位處之實際壓力可能明顯低於完全真空自然產生之壓力。減壓最初造成流體在管中及組織部位區域流動。當組織四週之壓力接近所需減壓時,可能停止流動且接著維持著該減壓。除非另有說明,本文所列之壓力值為錶壓。
本文所用名詞"支架"係指用於提升或促進細胞生長及/或組織形成之材料或結構。支架通常為對細胞生長提供模板之三維多孔結構。該支架可注入有、包覆有或包括有細胞、生長因子、細胞外基質成分、營養素、整合素或其他促進細胞生長之材料。支架可用作本文所述具體例之集流腔,以對組織部位施與減壓組織治療。
本文所用名詞"組織部位"係指位在任何組織上或組織內之傷口或缺陷,包含(但不限於)骨骼組織、脂肪組織、肌肉組織、真皮組織、血管組織、結締組織、軟骨組織、肌腱或韌帶。名詞"組織部位"可進一步代表任何組織區域,其非必為受傷或缺陷區域,但代之為需要加入或促進額外組織生長之區域。例如,減壓組織治療可用於使額外組織生長,其可被收取或移植至另一組織部位。
本發明係用於負壓療法之生物可相容之傷口敷料。本文所用之名詞"傷口"可包含灼傷、刀傷、切開傷口、潰瘍、外傷、及慢性開放性傷口。至於本文所用之名詞"墊片"係指發泡體、篩網、其他似多孔材料。名詞"習知墊片"係指 慣用於V.A.C.療法之聚胺基甲酸酯(PU)或聚乙烯醇(PVA)發泡體。本文所用之名詞"V.A.C.療法"係指代理商或其母公司商業化且進一步敘述於前述專利及專利案之負壓傷口療法。
參考附圖,本發明之多層減壓輸送裝置10顯示通常包括實質上插入傷口部位12之發泡墊片11及將傷口部位12處之發泡墊片11密封住之傷口覆蓋片13。依據本發明,係將發泡墊片11改良成含有促進細胞生長之基質或晶胞14,因而所需之高度多孔性細胞生長提升基質可導入傷口部位12及其附近。插入傷口部位12且以且以傷口覆蓋片13密封後,該發泡墊片11係以與真空源流體相通配置以促進流體排放,如熟悉本技藝者已知。發泡墊片11係自先前技藝之墊片加以改良,其中該墊片11包括對已知之V.A..C.療法為非侵入性之基質14且因此不需要其改良。

如'832申請案中之詳述,該發泡墊片11較好包括抽氣時 對受傷流體具有良好滲透性之高度網狀、開放孔窩狀之聚胺基甲酸酯或聚醚發泡體。亦如'832申請案中之詳述,發泡墊片11較好經由塑膠或類似材料之軟管15與真空源流體相通地配置,其較好包括經由介插之疏水性薄膜過濾器,經過與真空泵浦流體相通而在真空下安全的配置之罐。最後,該'832申請案亦詳述該傷口覆蓋片13,其較好包括至少周邊覆蓋有感壓性、丙烯酸系黏著劑之彈性材料用以在傷口部位12上施加密封。
依據本發明之較佳方法,該'832申請案中描述之該等組件通常如本技藝中已知般使用,但發泡墊片11係設有基質14。該基質14顯示包括已形成為複數個區段17之多孔材料16。該材料16係於該發泡墊片11之基底18處灌注入該發泡墊片11中。因為此需要修整該發泡墊片11用於製備供V.A.C.療法之傷口治療,因此材料16較好放置在墊片11之中心部份。本申請者並不欲經由使用名詞"基質"限制材料16或區段17本身而限制其本身為規則或對稱排列。

當放置其中埋有基質14及/或自其突起之基質14及/或包括生物可吸收之分支聚合物之墊片11後,將該傷口覆蓋片13施加至該墊片上,在傷口部位上形成氣密封。使用時,該V.A..C.療法係如已知般進行,且若需要則藉由簡單提供包括材料16之基質14而加上細胞生長提升療法。依該方 式,細胞生長提升療法宜與既有之V.A.C.療法併用,而不會損及V.A..C.療法之效能且不會造成不方便或過度增加成本。
實例I
上述開放孔窩狀之發泡體形成為墊片。遵循1998年8月18日頒予Totakura等人之美國專利號5,795,584第5欄第5至42行中敘述之一般理論以建立重疊在墊片底部上之結構。孔洞係配置在非生物可吸收受質之相對遠離生物可吸收細胞成長提升基質受質之該等部分。該基質覆蓋住位於欲治療傷口邊界之墊片之一部份。該墊片接著以氣密覆蓋片完全覆蓋,且使之處於大氣壓力以下,如同利用V.A.C.療法之標準實務般。該基質在該墊片之預期可用壽命內被吸收,因此當移開該墊片時,該基質已被吸收,且不會干擾成長中之細胞。在認為必要治療下,若需要可以習知墊片或以含基質之墊片替換該墊片。

實例V
將生物可吸收之聚合物(類似於頒予Bennet等人之美國專利號5,578,662中所述)形成該墊片。依循V.A.C.療法之一般理論,以該生物可吸收之分支聚合物墊片取代該習知墊片。在該墊片之預期工作週期期間,該墊片被生長中之細胞吸收,因此並不需要更換該墊片且不干擾傷口部位。若認為需要進一步處理,可在該傷口部位放置習知墊片或 額外之含基質之墊片或額外之生物可吸收之分支聚合物墊片,且繼續V.A.C.療法。
參照圖2及3,本發明具體例之減壓輸送系統211包含生物可相容之傷口或組織敷料,或減壓輸送裝置213、減壓輸送管217及減壓源219。該減壓輸送系統211係用於對人類組織部位221施加減壓組織治療。該組織部位可包含灼傷或其他傷口,或者為需要促進新組織生長之健康組織。減壓源219係與減壓輸送管217之遠端流體相連接,且該減壓輸送裝置213與該減壓輸送管217之近端流體相連接。圖2中,該減壓輸送管217藉由類似圖1說明之管連接器220與該減壓輸送裝置213流體連接。圖3中,將該減壓輸送管217直接放置在該減壓輸送裝置213內部或與之相鄰且可能包含複數個與該減壓輸送裝置213相通之裝置222。該減壓源經減壓輸送管217將減壓送到減壓輸送裝置213,且該減壓輸送裝置213將減壓分配至組織部位221。薄膜224係配置在該減壓輸送裝置213上且與組織部位221周圍之組織密貼。該薄膜224可降低組織部位221之污染且有助於保持組織部位221為減壓。
該減壓輸送裝置213為一種多層裝置,其具有第一層或組織接觸層223;第二層或釋離層225;及第三層或集流腔層227。該第一層223包含支架233及第一複數個流動通道234。該第二層225包含釋離材料235如水凝膠形成材料或水溶性聚合物。該第二層225又包含第二複數個流動通道236。該第三層227包含分配集流腔237及第三複數個流動 通道238。該第三層係配置成使該第二層225位在該第一層223及該第三層227之間,該第一層223與該第二層225相鄰,該第二層225與該第一及第三層223、227相鄰,且該第三層227與該第二層225相鄰。
一具體例中,各層223、225、227係藉適合各層材料種類之任何連接方式與相鄰層連接。例如,第三層227可與第二層225黏合,或第一層223可與第二層225黏合,或可使全部三層223、225、227黏合在一起。黏合可藉由對其一層、兩層或所有層之界面處加熱,且施力而將該等層壓製成黏合連接而達成。或者,可使用黏著劑或機械固定器使該等層彼此連接,只要該固定或黏合方式不會對通過該等層之壓力分配造成實質且負面之影響即可。另一具體例中,在用於減壓組織治療之前及/或期間,該等層223、225、227可不彼此連接,而是使該等層彼此簡單接觸放置。或者,該等層中之二層可彼此黏合,且該等層之第三層與該二黏合層之一接觸放置。例如,該第二層225可以前述方式與該第三層227連接,且該第一層223可與該第二層225接觸放置但不連接。或者,該第二層225可與該第一層223連接,且該第二層225可與該第三層227接觸放置但不連接。
該第一、第二及第三層223、225、227中分別設置第一、第二及第三複數個流動通道234、236、238,以使減壓分配於減壓輸送裝置213中且分配於組織部位221。各層中提供之流動通道可為該層中所提供材料之固有特性(例 如天然多孔性材料),或該流動通道可在該三層223、225、227組裝之前或之後經化學、機械或者其他方式於材料中形成。該等層223、225、227彼此相鄰配置使其一層中之流動通道與相鄰層中之流動通道流體相通。例如,上述該等層之相對位置或連接可使該第一複數個流動通道234與第二複數個流動通道236流體相通,後者又可與第三複數個流動通道238流體相通。

該支架233中之孔隙尺寸通常在約50至500微米之間,且更好在約100至約400微米之間。孔隙尺寸小於50微米將抑制或阻止組織成長。一具體例中,該支架中孔隙之較佳平均孔隙尺寸為約100微米。
該支架可以薄片或墊薄片料提供。支架厚度係將減壓輸送裝置與組織部位相鄰放置時,沿著與組織部位垂直之方向測量。材料厚度可改變,但一具體例中,該支架厚度約1至4 mm。與厚度尺寸垂直之平面中之薄片或墊片附著薄片材料之尺寸會隨著欲治療之組織部位大小而變。墊片或薄片材料可以大尺寸提供,然後再剪裁至適合該組織部位。
該第二層之釋離材料235,或釋離層225使該第一層223及該第三層227間之接觸點為最小。一具體例中,該釋離材料235可防止該第一層及該第三層223、227間之任何接觸。經由使該第一層及該第三層223、227間之接觸最小化,該釋離材料235可用為生長中組織之障壁物以防止自支架233生長到該第三層227之分配集流腔237。
該釋離材料235亦可作為該第一層及第三層223、227之黏合劑及釋離劑。減壓組織治療之前及期間,可如先前所述般使用該釋離材料235,使該第一層與第三層黏合。該釋離材料235較好為水凝膠形成材料或水溶性聚合物。
作為水凝膠形成材料之該釋離材料235在暴露於水或其他液體之後可形成液體及/或凝膠。該減壓輸送裝置213施加於該組織部位之初期及施 減壓組織治療期間,該水凝膠形成材料較好為固體,"非-水合"狀態。換言之,該水凝膠形成材料尚未轉化成液態及/或似凝膠態。當施加減壓組織治療時,該水凝膠形成材料可能暴露於傷口滲出液及其他自傷口部位吸出或施於傷口部位之流體中,但在減壓影響之下壓縮該第一層、該第二層及該第三層時,最好可減低或消除該水凝膠形成材料所吸收之流體。此使得該水凝膠形成材料保持在固態,直到停止減壓輸送為止。停止減壓療法之後,該水凝膠形成材料可經由添加水、鹽水溶液或其他流體而水合成水凝膠形成材料,或經由以傷口滲出液使水凝膠形成材料水合。當該水凝膠形成材料水合時,該材料轉化成液體及/或凝膠態,使得第三層227易於自第一層223釋離。
該水凝膠形成材料為在暴露於水或其他流體中特定時間後可接受及/或形成液體或似凝膠材料之任何適宜材料。該水凝膠形成材料通常為交聯聚合物,然而,該材料位必須為交聯。適宜之水凝膠形成材料可包含(但不限於)交聯聚乙二醇、親水性聚醚、聚乙烯醇、聚乙酸乙烯酯、聚丙烯酸酯、聚磺酸酯、聚磷腈水凝膠、膠原、明膠、玻尿酸、葡糖胺聚糖、甲殼素及藻膠酸鹽。亦可使用具有輸水性及親水性部分之未交聯聚合物,如乙二醇與乳酸之共聚物或具有極長親水性軟段之聚胺基甲酸酯。
該釋離材料235亦可為水溶性聚合物。若為水溶性聚合物,則釋離材料235可在水或其他液體存在下溶解。該減壓輸送裝置213施加於組織部位之初期及施加減壓組織治療期間,該水溶性聚合物較好為"非-水合"形式。換言之,該聚合物尚未吸收水或其他液體。當施加減壓組織治療時,該水溶性聚合物會暴露於傷口滲出液中且其他流體會自組織部位吸出或施加於組織部位,但在減壓影響之下,壓縮該第一、該第二及該第三層較好降低該水溶性聚合物之溶解度,此可避免水溶性聚合物過早溶解。此使得水溶性聚合物保持在固態直到停止減壓輸送為止。停止減壓療法之後,該水溶性聚合物可經由添加流體於該聚合物,或經由以傷口滲出液使該聚合物水合。當該水溶性聚合物水合時,該聚合物溶解成水合液體,其使得該第三層227易於自該第一層223釋離。
該水溶性聚合物可包含(但不限於)未交聯聚乙二醇、親水性聚醚、聚乙烯醇、聚乙酸乙烯酯、聚丙烯酸酯、聚磺酸酯、聚磷腈、膠原、凝膠、玻尿酸、葡糖胺聚糖、甲殼素及去氧核醣核酸(DNA)。
該第二層225中存在之第二複數個流動通道236可使減壓自分配集流腔237分配至支架233。該第二複數個流動通道236可進一步使流入組織部位221或自組織部位221流出之任何流體通過。雖然第二複數個流動通道236可為該釋離材料235之固有特性(亦即在該材料本身中之互連孔隙或其他流動通道),更可能的是該第二複數個流動通道236為經 機械、化學或其他方式於釋離材料中235形成。例如,參照圖4,該第二複數個流動通道236可由以似柵欄方式排列之釋離材料235相鄰線股間之孔洞加以界定。或者,參照圖5,該釋離材料235可以該第一及該第三層223、227間之材料珠粒使用。此特定結構中,該第二複數個流動通道236係由該釋離材料235相鄰珠粒間之孔洞所界定。又另一結構中,參照圖6,該釋離材料235薄片可於該薄片中提供有形成之縫隙,以界定該第二複數個流動通道236。上述縫隙及孔洞之形狀、大小及部位均可改變,且在一具體例中可任意改變。
不管是否使用孔隙、孔洞、縫隙或其某些組合以界定該第二複數個流動通道236,該第二層225之孔隙度可能小於該支架233之孔隙度,以使生長中組織進入該第二層225為最小。該第二層225之孔隙度可經由限制孔隙、孔洞或縫隙之尺寸而控制,或藉由控制該第二層225中之孔隙、孔洞或縫隙數量(亦即密度)而控制。然而,該第二層225之孔隙度必須保持夠高以足以分配減壓且使流體流經該第二層225。
如同該支架233,該釋離材料235可以薄片或墊薄片料(參閱圖6)提供。或者,釋離材料235可以格狀線股、點狀或其他單獨片之材料提供(參閱圖4及5)。此等個別薄片材料之形狀、大小及分配可改變且在某些情況下,該形狀、大小及分配可隨意。該釋離材料235之厚度係在減壓輸送裝置放置與組織部位相鄰時測量與組織部位垂直之方向。 雖然並非必要,但該釋離材料235之厚度通常小於該支架厚度,以節省材料成本費用。一具體例中,該釋離材料235厚度在吸收流體之前約為200至500微米。該釋離材料經水合後,厚度可能膨潤至約500微米至1毫米。與厚度尺寸垂直之面中該釋離材料之薄片或墊片之尺寸會隨著欲治療組織部位之大小而變,但大小通常會與該支架之長度及寬度相同。可提供較大尺寸之材料墊片或薄片,且接著修整至適合該組織部位。
該第三層或集流腔層227之分配集流腔237有助於使接收自減壓輸送管217之減壓分配。該集流腔又可用於使導入該組織部位之流體分配或使自組織部位收集之傷口滲出液及其他流體分配。集流腔237可為可達成該等任務之任何多孔性材料,且一具體例中,該集流腔係由孔窩狀材料如開放孔窩狀發泡體形成。該材料較好包含與相鄰孔窩狀流體連接之孔窩。該第三複數個流動通道238係由該孔窩狀材料之"開放孔窩狀"形成且於其間形成。該流動通道使流體在具有開放孔窩狀之全部孔窩狀材料部分相通。該孔窩狀及流動通道形狀及大小可為均勻,或可包含形狀及大小之圖案或隨意改變。該孔窩狀材料之孔窩狀形狀及大小改變導致該第三複數個流動通道238之改變,且此等特性可用於使流經該孔窩狀材料之流體特性改變。該孔窩狀材料又可包含含"密閉孔窩狀"之部分。孔窩狀材料之此等密閉孔窩狀部份含複數個孔窩狀,其大多數與相鄰孔窩狀流體上不連接。該孔窩狀材料之密閉孔窩狀部份可選擇性的與 開放孔窩狀部分組合,以防止流體經由集流腔237之選擇部份傳送。
一具體例中,該集流腔237係由孔隙尺寸在約400-600微米間之開放孔窩狀之網狀聚胺基甲酸酯發泡體製成。該發泡体之實例可包含由San Antonio, Texas之Kinetic Concepts, Inc.製造之GranuFoam。該集流腔237亦可為聚胺基甲酸酯發泡體、聚乙烯醇發泡體、聚乙烯發泡體、膨脹之聚四氟乙烯、聚矽氧發泡體、絲瓜泡綿(loofa sponge)、海泡綿、網紗、毛氈墊或可提供經由複數個三維流動通道而提供流體相通之任何其他生物可相容之材料。
如同該支架233及該釋離材料235,該集流腔237可由材料薄片或墊片形成。該集流腔237厚度可改變,但一具體例中,該厚度將至少如該支架233厚度般厚。與厚度方向垂直之面中之集流腔尺寸亦可隨著欲治療組織部位之尺寸而變。該集流腔材料之墊片或薄片可以較大尺寸提供且再修整至適合組織部位。
操作時,若需要,可將減壓輸送裝置213修整至使之與該組織部位221之形狀及大小相符。許多情況下,該組織部位221可為外傷,或其他受損組織,但當需要生長額外組織時,該組織部位221類似地可為含健康組織之部位。該減壓輸送裝置213係與組織部位221相鄰配置,使得該第一層223與該組織部位221接觸。如前所述,可使該減壓輸送裝置213之多層積層、黏合或者連接,但亦可使該等層彼此分離。若某些該等層彼此不連接,則各種層可個別放 置,使得該第一層223與該組織部位接觸,該第二層225與該第一層223接觸,且該第三層227與該第二層225接觸。
設置減壓輸送裝置213後,自減壓源219經由減壓輸送管217輸送減壓至該第三層227之該集流腔237中。該減壓經由與集流腔237有關之第三複數個流動通道238分配至與第二層225有關之第二複數個流動通道236。接著使減壓分配至與該第一層223之支架233有關之第一複數個流動通道234中。當減壓到達該組織部位221時,該組織部位221之流體如傷口滲出液可經由該第一、該第二及該第三複數個流動通道234、236、238吸出,且自該減壓輸送裝置213移除。該減壓輸送裝置213及減壓源219間可設置儲槽(未顯示)及各種過濾器,以收集滲出液且保護該減壓源219。除了使減壓分配且抽取自該組織部位221之流體之外,該第一、該第二及該第三複數個通道234、236、238可用於使流體如沖洗流體、醫藥、抗微生物劑、抗菌劑、抗病毒劑及成長因子分配於該組織部位221中。
對該組織部位219施加減壓引發新組織生長。促進新組織生長之有些機制包含組織之微變形、上皮移行及改善血液流動。此等因子可增加組織部位之粒化組織發展,其導致新組織生長。雖然提供減壓組織治療之討論通常係指"輸送"減壓至該組織部位,但熟悉本技藝者應了解輸送減壓通常涉及在壓力源219及該組織部位221間產生壓差。該壓差(因該減壓源219之壓力低於該組織部位221之壓力)產生自該組織部位221流向該減壓源219之起始流體。當該組 織部位221之壓力接近或等於該減壓源219之壓力時,因為與減壓源219相通之流體及薄膜224之密封功能使組織部位得以維持減壓。
當在減壓之影響下形成新組織時,可使新組織生長至該支架233中。該支架233之選用材料較好支撐並激勵新組織生長。因為該支架在施行減壓組織治療後將留在組織處,因此較好使新組織儘可能滲入該支架中。已發現在減壓之影響下,新組織可在兩天期間滲入該支架中達1 mm(厚度)。因為有些具體例中該支架233厚度可能僅約1至4 mm,因此可能需要移除該減壓輸送裝置213之該第二及該第三層225、227,且以含該第一、該第二及該第三層223、225、227之新敷料替換該層。換言之,可將新的支架233放在舊的支架233上,接著移除該第二及該第三層225、227。藉由僅移除一部份之減壓輸送裝置213並留下該支架233,當新支架233堆疊在已透入新組織生長之事先插入的支架233時,可對該組織部位增量地加入新組織生長。
該第二及該第三層225、227自該第一層223釋離因該釋離材料235之存在而變簡單。施加減壓且自該組織部位221移除流體期間中,該釋離材料235較好維持為固態,因此使該第二複數個流動通道236維持暢通。雖然該釋離材料一般會轉化成液體或凝膠或在吸水或其他流體之後將溶解,但此改變在對減壓輸送裝置213施加減壓期間明顯降低。減壓導致該減壓輸送裝置213壓縮,降低暴露於流經 該第一、該第二及該第三複數個流動通道234、236、238之流體中之該釋離材料表面積。因此可使該釋離材料吸收之流體為最少直到停止減壓輸送為止。
施加減壓期間,該釋離材料較好使該第一及該第三層223、227間之接觸為最小或防止其接觸。生長至該支架233之新組織因該支架233及該集流腔237間之分隔且因該釋離材料235本身而阻礙於集流腔237中之生長。雖然可能發生組織成長至集流腔237,但該生長為最小化,使移除集流腔237時病患之疼痛為最小。
施加減壓一段選定時間後,可經由將該減壓輸送裝置213浸泡在水、鹽水溶液或其他流體中使釋離材料水合。或者,可使該減壓輸送裝置213擱置,直到組織部位之體液使釋離材料235水合為止。若釋離材料235為水凝膠形成材料,則釋離材料235轉化成似凝膠態且通常因如其水合而膨脹。此使集流腔237可更易於自支架233移除。移除集流腔237後留下之任何水凝膠形成材料(或水凝膠)可手動移除或導入額外流體而溶解。或者,若釋離材料235為水溶性聚合物,則會在吸收水或其他流體後溶解,因此使該第三層227自該第一層223釋離。

參閱圖9,依本發明具體例之促進組織部位之新組織生長之組織生長套組911包含支架913、釋離材料915及分配集流腔917。該支架913包含第一側及第二側,該支架913之第一側適於與組織部位接觸。該支架913與前述參閱圖2及3之支架233類似。該釋離材料915適於與支架913之第二側接觸且與前述參閱圖2及3之釋離材料235類似。該分配集流腔917適於與釋離材料915接觸,以經由支架913將減壓分配於組織部位。該分配集流腔917與前述參閱圖2及3之集流腔237類似。組織生長套組911又包含一容器921,用以在該等組件使用之前,包納該支架913、釋離材料915及分配集流腔917。該容器921可為軟性袋、盒或適用於存放該支架913、釋離材料915及分配集流腔917之任何其他容器。
雖然本文揭示之多層減壓輸送裝置係與減壓輸送源連用,以對組織部位提供減壓組織治療,但該減壓輸送裝置亦可在未施加減壓下單獨用作先進之組織敷料。該先進組織敷料中可使用相同材料、相對位置及層間連接性。類似於本文所述之減壓輸送裝置,該先進之組織敷料可包含第 一層以促進並接受新組織之生長,第三層以助於將流體導離組織部位,及第二層以助於在選定時間下自第一層移除第三層。該先進組織敷料之第三層替代具有"集流腔"之層,而可視同包含收集自傷口滲出之流體並予以保留之流體儲槽。本文所述適於分配集流腔材料之材料類似於第三層之儲槽材料。該儲槽唯一需求為其係由可儲存組織部位產生或存在之流體之材料。
雖然本發明之系統及方法已參考人類病患之組織生長及痊癒描述,但應了解用於施加減壓組織治療之此等系統及方法可用於需要促進組織生長或痊癒之任何活有機體。類似地,本發明之系統及方法可適於任何組織,包含(但不限於)骨骼組織、脂肪組織、肌肉組織、上皮組織、血管組織、結締組織、軟骨、肌腱或韌帶。雖然組織痊癒可能是使用本文所述之減壓組織治療之一焦點,但施加減壓組織治療亦可用於產生沒有生病、缺陷或損傷之組織之組織生長。例如,可能需要施加減壓組織治療,使組織部位生長出額外組織,隨後收取。所收取之組織可被移植到另一組織部位處,以替代生病或受損之組織,或者可將收取之組織移植到另一病患身上。

Claims (109)

  1. 一種用以對組織部位施加減壓組織治療之減壓輸送系統,包括:多層減壓輸送裝置,其具有組織接觸層、釋離層及集流腔層,該組織接觸層包含適於與組織部位接觸之支架,該釋離層包含水凝膠形成材料及複數個流動通道,該集流腔層包含分配集流腔,該釋離層位在該組織接觸層與該集流腔層之間,該釋離層之水凝膠形成材料與該組織接觸層及該集流腔層之至少一者結合;及減壓輸送管,其與該集流腔層流體相通,以對該組織部位傳送減壓。
  2. 如請求項1之減壓輸送系統,其中該水凝膠形成材料係位在該支架與該分配集流腔之間,以實質上防止支架與分配集流腔中已配置有水凝膠形成材料之區域的接觸。
