背景技术
本公开的实施例涉及用于以负压或减压疗法或局部负压(TNP)疗法来敷料并处理伤口的方法和设备。具体而言但不加限制,本文中公开的实施例涉及负压治疗装置、用于控制TNP系统的操作的方法以及使用TNP系统的方法。
发明内容
在一些实施例中,公开了一种用于向伤口施加负压的设备。所述设备可以包括:伤口敷料,所述伤口敷料被配置成放置在患者的伤口上;负压源,所述负压源设置在所述伤口敷料上或所述伤口敷料内,所述负压源被配置成经由流体流动路径向所述伤口敷料提供负压;开关,所述开关设置在所述伤口敷料上或所述伤口敷料内,所述开关被配置成接收第一用户输入;界面元件,所述界面元件设置在所述伤口敷料上或所述伤口敷料内,所述界面元件被配置成接收第二用户输入;和控制电路。所述控制电路可以电耦合到所述开关和所述界面元件。当在第一模式时,所述控制电路可以:在所述负压源不供应负压时,响应于接收所述第一用户输入用所述负压源供应负压,在所述负压源正供应负压时,响应于接收所述第一用户输入防止用所述负压源供应负压,以及响应于接收所述第二用户输入,从所述第一模式变成不同于所述第一模式的第二模式。当在所述第二模式时,所述控制电路可以禁止用所述负压源供应负压。

具体实施方式
本公开涉及用于以减小的压力疗法或局部负压(TNP)疗法敷料并处理伤口的方法和设备。具体而言但不加限制,本公开的实施例涉及负压治疗设备、用于控制TNP系统的操作的方法和使用TNP系统的方法。所述方法和设备可包括或实施下面描述的特征的任何组合。
已知用于帮助人或动物的愈合过程的许多不同类型的伤口敷料。这些不同类型的伤口敷料包括许多不同类型的材料和层,例如,纱布、衬垫、泡沫垫或多层伤口敷料。TNP疗法有时称作真空辅助闭合、负压伤口疗法或减压伤口疗法,其可能是用于促进伤口的愈合率的有益机制。这种疗法适用于宽范围的伤口,例如切口、开放性伤口和腹部伤口等。
TNP疗法可有助于通过减少组织水肿来闭合和愈合伤口;促进血液流动;刺激肉芽组织的形成;去除多余的渗出液,且可减少细菌负载(且因此减少伤口感染)。而且,TNP疗法可以允许伤口受到较少的外部干扰,促进更快速的愈合。
如本文所用,减压水平或负压水平(诸如-X mmHg)表示低于大气压的压力水平,它通常对应于760mmHg(或者1atm、29.93inHg、101.325kPa、14.696psi等)。因此,-X mmHg的负压值反映低于大气压X mmHg的压力,例如(760–X)mmHg的压力。此外,比-X mmHg“更少”或“更小”的负压对应于更接近大气压的压力(例如,-40mmHg比-60mmHg小)。比-X mmHg“更多”或“更大”的负压对应于更远离大气压的压力(例如,-80mmHg比-60mmHg大)。
概览
一些TNP设备的用户界面可具有有限的元件,用户可通过这些元件提供用户输入。在某些情况下,特定的用户界面可以仅包括由用户可用于停止和开始负压递送的单个元件,并且用户可能不能够用另一元件替换或交换该单个元件。这些特定的用户界面可以理想地比具有许多元件的更复杂用户界面更容易构建和操作。然而,特定的用户界面可以在单个元件经历故障(例如,失效)时出现问题,并且不再能够用来接收用户输入。如果负压尚未被提供,那么特定用户界面的用户可能不期望不能开始负压递送,以及如果正提供负压时可能不期望不能停止负压递送。
用户不能停止负压递送的情形可能另外对患者伤口愈合引入风险。如果患者在负压递送期间经历伤口敷料不适,且单个元件体验不再能够用来接受用户输入,则患者可能被迫去除伤口敷料,以终止负压递送。伤口敷料的移除可能会损伤患者伤口,并阻碍已经进行中的任何愈合轨迹,以及由于失去伤口敷料的保护,使伤口暴露到外部污染物。
针对用户不能停止负压递送的情况,具有可由用户使用以停止和开始负压递送的单个元件的TNP设备可包括另一机构,例如另一冗余机构,以停止负压递送。在一些实施方式中,可使用具有四个电路的头电路(header circuit),且每对电路可用于通过一个或多个手段停用操作。一对电路可用于连接到TNP设备的电源,且另一对电路可用于连接到启动信号(例如,控制电路启动信号)。另外或替代地,可使用表面安装技术(SMT)排针。例如,另一种机制可以是模制在可焊接到伤口敷料的膜中的激活零件,使得激活机构不可轻易失去。另外或替代地,零插入力(ZIF)连接器的锁定机构可用于改进保持力。在另一个实例中,激活机构可以是印刷电路板(PCB),例如挠性PCB,其内置到膜中以便插入。在又一实例中,激活机构可包括导电标签,所述导电标签在被附接时完成电路且可移除以停止负压递送。在一些实施方式中,可以另外或替代地使用突片,并且突片可以例如被拉动以破坏TNP设备的电源(例如,移除电池),或者拉动以撕开伤口敷料中的孔隙(诸如通过在伤口敷料外部拉动突片)以迫使大泄露,导致终止负压递送。
减压治疗系统和方法
图1图示了包括TNP设备11和伤口14的负压治疗系统100。TNP设备11可以用来处理伤口14。TNP设备11可包括控制电路12A、存储器12B、负压源12C、用户界面12D、电源12E、第一压力传感器12F、第二压力传感器12G和皮肤检测器12H,他们都被配置成相互电通信。另外,TNP设备11可包括伤口敷料13。电源12E可将功率提供至TNP设备11的一个或多个部件。

控制电路12A可包括控制器、激活电路、升压转换器、电流限制器、反馈调理电路和H-桥逆变器中的一个或多个。一个或多个控制器可以至少根据存储在存储器装置12B中的指令控制TNP设备11的一个或多个其它部件的操作。一个或多个控制器可例如通过一个或多个H-桥逆变器的信号输入(例如,信号的脉冲宽度调制)控制负压源12C的操作,所述一个或多个H-桥逆变器接着将来自电源12E的电力驱动到负压源12C。
负压源12C可包括泵,例如但不限于旋转隔膜泵或其它隔膜泵,压电泵,蠕动泵,活塞泵,旋转叶片泵,液体环泵,旋涡泵,由压电换能器操作的泵,音圈泵或者任何其它适合的泵或微型泵或前述的任何组合。
用户界面12D可包括接收用户输入或者将用户输出提供至患者或护理人员的一个或多个元件。接收用户输入的一个或多个元件可包括按钮、开关、拨号盘、触摸屏等,提供用户输出的一个或多个元件可包括发光二极管(LED)或显示器的一个或多个像素的激活或扬声器等的激活。在一个实例中,用户界面12D可包括接收第一用户输入(例如负压激活或去激活输入)的开关,接收第二用户输入(例如,负压禁止输入)的界面元件和指示TNP设备11的操作状态(例如工作正常,处于故障状态或等待用户输入)的两个LED。
第一压力传感器12F可以用来监测伤口敷料13下面的压力,例如,连接负压源12C和伤口14的流体流动路径中的压力,伤口14处的压力或者负压源12C中的压力。第二压力传感器12G可以用来监测伤口敷料13外部的压力。伤口敷料外部的压力可以是大气压;然而,大气压可根据例如使用的高度或可以使用TNP设备11的加压环境而变化。
控制电路12A可以根据至少由第一压力传感器12F监测的压力和由第二压力传感器12G监测的压力之间的比较,控制负压源12C的负压供应。控制电路12A可包括控制器,例如微控制器或微处理器。

伤口敷料13可包括伤口接触层、隔件层和吸收层。伤口接触层可与伤口14接触。伤口接触层可以在面向患者侧上包括粘合剂,用于将敷料固定到伤口14周围皮肤;或者在顶侧上包括粘合剂,用于将伤口接触层固定到伤口敷料13的覆盖层或其他层。在操作中,伤口接触层可以提供单向流动,以便有利于从伤口移除渗出液,同时阻挡或基本上防止渗出液返回伤口14。隔件层可有助于在伤口部位之上分配负压,并且有助于促进伤口渗出液与流体输送到伤口敷料13中。此外,吸收层可吸收并保持从伤口14吸出的渗出液。
在一些情况下,控制电路12A可防止用负压源12C供应负压。例如,控制电路12A可通过停用负压源的操作、打开位于流体流动路径中的通风口以及关闭位于流体流动路径中的阀门来防止供应负压。

当TNP设备11可以用来治疗伤口14时,伤口敷料13可以在伤口13周围及在伤口敷料13下面产生基本密封或封闭的空间,第一压力传感器12F可周期性或连续地测量或监测在此空间中的压力水平。控制电路12A可以将此空间中的压力水平控制在第一负压设定点极限和至少第二负压设定点极限之间。在一些情况下,第一设定点极限可以近似为–70mmHg,或者从近似–60mmHg或者更小到近似–80mmHg或者更大。在一些情况下,第二设定点极限可以近似为–90mmHg,或者从近似–80mmHg或者更小到近似–100mmHg或者更大。

在负压治疗系统200中,TNP设备11的伤口敷料13显示为附接到伤口14。箭头描绘了通过伤口敷料13的空气流动和来自伤口14的伤口渗出液。TNP设备11可包括排气26和部件区25,例如用于TNP设备11的部件(例如控制电路12A、存储器装置12B、负压源12C、用户界面12D、电源12E、第一压力传感器12F、第二压力传感器12G和皮肤传感器12H中的一个或多个)的部件外壳或储存区。
负压治疗系统200的用户界面12D可包括开关21(例如圆顶开关(dome switch))、界面元件22(例如电触点)、第一指示器23(例如第一LED)和第二指示器24(例如第二LED)。开关21可接收负压激活或去激活用户输入(例如响应于在例如从0.5秒到5秒的时段按压开关21,接收激活或去激活用户输入)。界面元件22可接收负压禁止用户输入。第一指示器23和第二指示器24可以指示如工作正常,处于故障状态或等待用户输入的操作状态。在一些实施方式中,开关21或界面元件22可以耦合到负压源12C或控制电路12A(例如,控制电路12A的控制器)的电源连接或者负压源12C或控制电路12A的启动信号,以激活或去激活负压供应或者禁止负压供应。另外或替代地,SMT排针可用于激活或去激活负压供应或禁止负压供应。
负压治疗系统200的伤口敷料13的部件部分图示为包括气锁层27、吸收层28和接触层29。气锁层27可以使空气流动。吸收层28可吸收伤口渗出液。接触层29可以是软的,且包括硅,并用来将TNP设备11耦合到患者。
图3图示了负压治疗系统300的俯视图,其可以是负压治疗系统200的更详细示例性实施方式。如所示的界面元件22可包括激活零件31,其可以模制在膜中,所述膜在位置32处焊接到伤口敷料13。激活零件31可用于接收界面元件22的用户输入。
图4图示了负压治疗系统400的俯视图,其可以是负压治疗系统200的更详细示例性实施方式。如所示的界面元件22可以包括印刷电路板(PCB)41,其可以是挠性的,并且内置到膜中以在位置42处插入和焊接到伤口敷料13。印刷电路板41可用于接收界面元件22的用户输入。此外,零插入力(ZIF)连接器的锁定机构可用于改进保持力。
图5A和5B图示了负压治疗系统500的俯视图,其可以是负压治疗系统200的更详细示例性实施方式。如所示的界面元件22可以包括导电标签51,其可以用于在附接时完成电触点(参见图5B),并且在移除时断开电触点以接收界面元件22的用户输入。此外,ZIF连接器的锁定机构可用于改进保持力。
图6图示了可用于控制设备(例如TNP设备11)的负压治疗的递送的治疗控制过程600。为了方便,在TNP设备11的背景下描述治疗控制过程600,但该治疗控制过程可以在本文描述的其他系统中实施或者由未示出的其他系统实施。在一些情况下,治疗控制过程600可由TNP设备11的控制电路12A执行。
在框602处,治疗控制过程600可确定是否从用户接收禁止输入。禁止输入例如可以经由界面元件22从用户接收。在一些实施方式中,除了经由界面元件22之外,禁止输入可以不由TNP设备11的任何用户输入提供。
如果接收到禁止输入,则在框604处,治疗控制过程600可禁止负压供应。负压供应可例如通过停用负压源12C或控制电路12A的操作、打开位于流体流动路径中的通风口以及关闭位于流体流动路径中的阀门而被禁止。在框604之后,治疗控制过程600可结束。在一些实施方式中,在框604之后,TNP设备11可不再被开关21的用户输入激活,且用户因此不再能够使TNP设备11产生负压。
如果未接收到禁止输入,在框606处,治疗控制过程600可确定是否从用户接收到激活输入。例如,可以经由开关21从用户接收激活输入。在一些实施方式中,除了经由开关21之外,激活输入可以不由TNP设备11的任何用户输入提供。
如果未接收到激活输入,则治疗控制过程600可返回到框602,并再次确定是否已从用户接收禁止输入。
另一方面,如果接收到激活输入,则在框608处,治疗控制过程600可供应负压。负压供应可由负压源12C执行,且负压可经由流体流动路径供应到伤口敷料13。
在框610处,治疗控制过程600可确定是否从用户接收去激活输入。例如,可经由开关21从用户接收去激活输入。在一些实施方式中,除经由开关21以外,去激活输入可以不由TNP设备11的任何用户输入提供。
如果接收到去激活输入,则在框612处,治疗控制过程600可防止负压供应。可例如通过停用负压源12C的操作、打开位于流体流动路径中的通风口以及关闭位于流体流动路径中的阀门中的一个或多个防止负压供应。在框612之后,治疗控制过程600可返回到框602,并再次确定是否从用户接收禁止输入。
如果接收到去激活输入,则在框614处,治疗控制过程600可确定是否从用户接收禁止输入。禁止输入例如可以经由界面元件22从用户接收。在一些实施方式中,除了经由界面元件22之外,禁止输入可以不由TNP设备11的任何用户输入提供。在一些实施例中,周期性地执行框614,同时TNP设备11提供负压伤口治疗,以便确定负压的供应是否应被禁止。
如果未接收到禁止输入,则治疗控制过程600可返回到框608,且负压供应可继续。
如果接收到禁止输入,则在框616处,治疗控制过程600可禁止负压供应。负压供应可例如通过停用负压源12C或控制电路12A的操作、打开位于流体流动路径中的通风口以及关闭位于流体流动路径中的阀门而被禁止。在框616之后,治疗控制过程600可结束。在一些实施方式中,在框616之后,TNP设备11可不再被开关21的用户输入激活,且用户因此不再能够使TNP设备11产生负压。另外或替代地,TNP设备11可不再通过开关21的用户输入激活,直到例如经由界面元件22从用户接收启动输入。在一些实施方式中,除了经由界面元件22之外,启动输入可以不由TNP设备11的任何用户输入提供。
在治疗控制过程600的一些实施方式中,负压供应可以不由除了去激活输入或禁止输入之外的任何用户输入停止。
图7A、7B、7C图示了可与本文中所描述的负压系统的实施例中的任一个一起使用的连接器。图7A图示了具有四个电路(或连接器)70A、70B、70C和70D的头部700A,以用于将例如开关21和界面元件22连接到每对电路。图7B图示了用于连接例如开关21和界面元件22的SMT排针700B。在一些实施方式中,开关21可连接到连接器72A,且界面元件22可连接到连接器72B或反之亦然。图7C图示了具有端子74的ZIF连接器700C,开关21或界面元件22可连接到所述端子。在一些实施例中,两个ZIF连接器700C可用于连接开关21和界面元件22中的每一个。

Claims (12)
1.一种用于向伤口施加负压的设备,所述设备包括:
伤口敷料,所述伤口敷料被配置成放置在患者的伤口上;
负压源,所述负压源设置在所述伤口敷料上或所述伤口敷料内,所述负压源被配置成经由流体流动路径向所述伤口敷料提供负压;
开关,所述开关设置在所述伤口敷料上或所述伤口敷料内,所述开关被配置成接收第一用户输入以控制负压的递送;
界面元件,所述界面元件设置在所述伤口敷料上或所述伤口敷料内,所述界面元件被配置成接收第二用户输入,用作所述开关的冗余机构以在通过所述开关无法停止负压递送时停止负压递送;和
控制电路,所述控制电路电耦合到所述开关和所述界面元件,所述控制电路被配置成:
当在第一模式时,
在所述负压源不供应负压时,响应于接收所述第一用户输入用所述负压源供应负压,
在所述负压源正供应负压时,响应于接收所述第一用户输入防止用所述负压源供应负压,以及
响应于接收所述第二用户输入,从所述第一模式变成不同于所述第一模式的第二模式,
当在所述第二模式时,禁止用所述负压源供应负压。
2.根据权利要求1所述的设备,其中,当所述开关经历故障并且不再能够接收所述第一用户输入时,所述控制电路还被配置成响应于没有接收除了所述第二用户输入之外的用户输入而防止或禁止用所述负压源供应负压。
3.根据权利要求1所述的设备,其中,所述控制电路还被配置成响应于没有接收除了所述第一用户输入之外的用户输入而用所述负压源供应负压。
4.根据权利要求1所述的设备,其中,当所述负压源正在供应负压时,所述控制电路还被配置成响应于没有接收除了所述第一用户输入和所述第二用户输入之外的用户输入而防止或禁止用所述负压源供应负压。
5.根据权利要求1所述的设备,其中,当所述控制电路处于所述第二模式时,所述控制电路响应于接收所述第二用户输入还被配置成:
从所述第二模式变成所述第一模式;以及
用所述负压源供应负压。
6.根据权利要求1所述的设备,其中,所述控制电路被配置成通过以下步骤禁止用所述负压源供应负压:停用所述负压源或所述控制电路的操作;打开位于所述流体流动路径中的通风口;或关闭位于所述流体流动路径中的阀门。
7.根据权利要求6所述的设备,其中,所述控制电路被配置成通过(i)断开所述负压源或所述控制电路的电力或(ii)撤回提供至所述负压源或所述控制电路的启动信号来停用所述负压源或所述控制电路的操作。
8.根据权利要求1所述的设备,其中,所述控制电路被配置成通过以下步骤防止用所述负压源供应负压:停用所述负压源的操作;打开位于所述流体流动路径中的通风口;或关闭位于所述流体流动路径中的阀门。
9.根据权利要求1所述的设备,其中,所述界面元件模制在与所述伤口敷料联接的膜中。
10.根据权利要求1所述的设备,其中,所述界面元件包括被配置成接收所述第二用户输入的电触点。
11.根据权利要求1所述的设备,其中,所述开关被配置成响应于按压所述开关一时间段接收所述第一用户输入。
12.根据权利要求11所述的设备,其中,所述时间段在0.5秒到5秒之间。
