Description
Foreliggende oppfinnelse vedrører farmasøytiske sammensetninger og tabletter inneholdende som aktiv ingrediens en mikronisert form av fast nebivolol eller et farmasøytisk akseptabelt syreaddisjonssalt derav.
Nebivolol er en fellesbetegnelse for (±)-[R<*>[S<*>[S<*->
(S*)] ] ]-a,a'-[iminobis(metylen)-bis-[6-fluor-3,4-dihydro-2H-l-benzopyran-2-metanol]. Den generelle struktur av nebivolol vises som formel (I). Strukturen med formel (I) har fire stereogene sentere som hver er merket med en asterisk.
Nebivolol er en blanding av like deler av 2 enantiomerer med henholdsvis SRRR- og RSSS-konfigurasjon. SRRR-konfigu-rasjonen nevnes SR3-nebivolol (d-nebivolol), og RSSS-konfi-gurasjonen nevnes RS3-nebivolol {1-nebivolol). SR3-nebivolol er en sterk og selektiv px-adrenergisk antagonist både in vitro og in vivo. Nebivolol skiller seg fra andre p-adrenergiske antagonister idet den senker blodtrykket akutt hos spontant hypertensiv rotte, senker den totale perifere vaskulære motstand og øker slagvolumet hos bedøvet hund. Disse hemodynamiske virkninger kan til stor del tilskrives RS3-nebivolol. Det er også oppdaget at RS3-nebivolol virker styrkende på en rekke antihypertensive midler, så som atenolol, propanolol, prazosin, hydralazin og interessant nok også på sin egen enantiomer, dvs. SR3-nebivolol. Flere kliniske forsøk har dessuten demonstrert nebivololens terapeutiske potensial som en Pi-selektiv betablokker og som antihypertensivt middel.
EP-O.145.067 beskriver generelt 2,2<1->imino-bis-etanolderi-vater som er nyttige ved behandling og/eller forebyggelse av lidelser i det koronære vaskulære system. EP-0.334.429 beskriver [imino-bis-metylen]-bis-[3,4-dihydro-2H-l-benzo-pyran-2-metanol]derivater, også nebivolol.
Nebivolol kan fremstilles ifølge fremgangsmåtene beskrevet i EP-0.145.067 og nærmere i EP-0.334.429. Nebivolol har basiske egenskaper, og kan omdannes til sine farmasøytisk akseptable syreaddisjonssaltformer ved behandling med egnede syrer. Egnede syrer er f.eks. uorganiske syrer, så som hydrohalogensyre, f.eks. saltsyre, hydrobromsyre og lignende, og svovelsyre, salpetersyre, fosforsyre; eller organiske syrer, f.eks. eddiksyre, propansyre, hydroksy-eddiksyre, 2-hydroksypropansyre, 2-oksopropansyre, etan-disyre, propandisyre, butandisyre, (Z)-2-butendisyre, (E) - 2-butendisyre, 2-hydroksybutandisyre, 2,3-dihydroksybutan-disyre, 2-hydroksy-1,2,3-propantrikarboksylsyre, metansul-fonsyre, etansulfonsyre, benzensulfonsyre, 4-metylbenzen-sulfonsyre, cykloheksansulfamsyre, 2-hydroksybenzosyre, 4-amino-2-hydroksybenzosyre og lignende syrer. Det foretrukne syreaddisjonssalt ved utøvelse av foreliggende oppfinnelse er saltsyreaddisjonssaltet.
Farmasøytiske sammensetninger ifølge foreliggende oppfinnelse er faste eller halvfaste farmasøytiske sammensetninger. Interessante faste farmasøytiske sammensetninger er f.eks. pulvere, piller, kapsler, tabletter og lignende. Begrepet "halvfast farmasøytisk sammensetning" vedrører farmasøytiske sammensetninger som i alt vesentlig består av en dispersjon av fast aktiv ingrediens in et (høy)viskøst formuleringsmiddel. Interessante halvfaste farmasøytiske sammensetninger er f.eks. suppositorier, kremer, geler, salver og lignende.
En interessant fast farmasøytisk sammensetning er en enkeltenhetsdoseform, dvs. en ikke flerdelt doseringsform.
Den faste doseringsform som foretrekkes ved utøvelse av foreliggende oppfinnelse, er en tablett. Fagpersonen må ta hensyn til tablettenes kjennetegn under letingen etter en egnet sammensetning. Tabletters spesielle kjennetegn er form, oppløsningstid, og spesielt hardhet.
Oral administrasjon er den generelt foretrukne administra-sjonsform for farmasøytiske midler på grunn av at denne rute er spesielt bekvem og akseptabel for pasientene. Fremstilling av en fast doseform for oral administrasjon med alle de riktige egenskaper kan imidlertid iblant være en alvorlig utfordring for en person med kunnskaper innen faget. For at et middel skal være virksomt, må det oppnå egnede konsentrasjoner i pasientens blodstrøm innen et akseptabelt tidsrom etter inntak. Ned andre ord må midlet ha en akseptabel biotilgjengelignet.
En meget viktig faktor som påvirker midlers biotilgjengelighet etter oralt inntak, er oppløsningen, dvs. midlets oppløsningshastighet, spesielt i mavesyre. Det er oppfattet at oppløsningen av den faste doseform ifølge foreliggende oppfinnelse bør være minst 75% på 45 minutter i 0,1N HC1 ved en temperatur på 37°C. Denne oppløsning måles ifølge testprosedyren beskrevet i Eksempel 5 nedenfor. Denne testprosedyre er analog med testprosedyrene nevnt i offisi-elle farmakopéer, f.eks. U.S. Pharmacopoeia XXII.
Personer med kunnskaper innen faget å utvikle farmasøytiske sammensetninger stilles overfor problemet å fremstille en fast doseform egnet for oral administrasjon for at forbin-delsen med formel (I) har en akseptabel oppløsning. Videre bindes fagpersonen av andre begrensende betingelser. Den av ham utviklede farmasøytiske sammensetning vil fremstilles i industriell skala og må møte kravene for intern og ekstern kvalitetskontroll.
Oral administrasjon av nebivololhydroklorid hindres av den dårlige oppløsning når den er i normal krystallinsk form. I løpet av undersøkelser for å forbedre nebivololhydroklorid-ets biotilgjengelighet ble produktet mikronisert. Som vist i Eksempel 3, er dessverre oppløseligheten av mikronisert nebivololhydroklorid enda dårligere enn oppløsningen av nebivololhydroklorid i normal krystallinsk form.
Det ble imidlertid på uventet måte funnet at når nebivololhydroklorid i mikronisert form formuleres i en sammensetning med formuleringsmidler som er kjent innen faget, som beskrevet nedenfor, har det en egnet oppløsning og møter interne og eksterne kvalitetskontrollkrav.
Oppfinnelsen frembringer derfor en spesielt fordelaktig formulering av nebivololhydroklorid. Det frembringes såle-des ifølge oppfinnelsen en farmasøytisk sammensetning inneholdende et farmasøytisk akseptabelt bærerstoff og som aktiv ingrediens nebivolol eller et farmasøytisk akseptabelt salt derav, særpreget ved at den aktive ingrediens foreligger i mikronisert fast form og forholdet (vekt/vekt) av fuktemiddel og aktiv ingrediens varierer mellom 0,025 og 0,5.
Mikroniserte former av nebivololhydroklorid kan fremstilles ved mikroniseringsteknikker som er kjent innen faget, f.eks. ved å male i egnede møller og å sile gjennom egnede sikter.
Det spesifikke areal av det mikroniserte material bør være minst ca. 23 x IO<3>cm<2>/g (2,3 x 10<3> m2/kg) ; det spesifikke areal bør fortrinnsvis være mer enn 25 x IO<3> cm<2>/g (2,5 x 10<3> m2/kg) , mer foretrukket mer enn 28 x IO<3> cm<2>/g (2,8 x 10<3> m2/kg) , og mest foretrukket mer enn 31 x IO<3> cm<2>/g (3,1 x 10<3> m2/kg) .
Ifølge foreliggende oppfinnelse er kjennetegnene for mikronisert nebivololhydrokloridet som følger. Maksimalt 50% av partiklene kan ha en større diameter enn 10 /im, dvs. DL50 har en maksimal verdi på 10 /im. DLS0 bør fortrinnsvis være mindre enn 8 /im. Maksimalt 10% av partiklene får ha en større diameter enn 20 /im, dvs. DL10 har en maksimal verdi på 20 /im. DL10 bør fortrinnsvis være mindre enn 18 /im.
Sammensetninger ifølge foreliggende oppfinnelse vil fortrinnsvis inneholde farmasøytisk akseptable bærerstoffer og eksipienser, så som fyllstoff, f.eks. laktose, sakkarose, mannasukker, maisstivelse, mikrokrystallinsk cellulose eller kalsiumhydrogenfosfat; smøremidler, f.eks. stearin-syre, polyetylenglykol, magnesiumstearat, talkum eller ki-selgel; oppløsningsmidler, f.eks. ris-, potet- eller maisstivelse, natriumstivelseglykolat eller kroskarmellosenatrium (dvs. natriumkarboksymetylcellulose); bindemidler, f.eks. pregelatinisert maisstivelse, polyvinylpyrrolidon eller hydroksypropylmetylcellulose, og fuktemidler, f.eks. natriumdioktylsulfosuccinat og polysorbater.
Interessante fyllstoffer er laktose, sakkarose eller mikrokrystallinsk cellulose; fortrinnsvis laktose og mikrokrystallinsk cellulose. Interessante smøremidler er stearin-syre, polyetylenglykol, hydrogenert vegetabilsk olje, nat-riumstearylfumarat eller magnesiumstearat, fortrinnsvis magnesiumstearat. Interessante oppløsningsmidler er ris-, potet- eller maisstivelse, fortrinnsvis kroskarmellosenatrium. Det foretrukne bindemiddel er hydroksypropylmetylcellulose.
Det ble funnet at polysorbater er det mest egnede fuktemiddel. Interessante fuktemidler er polysorbat 20 ("Tween 20"), polysorbat 40 ("Tween 40"), polysorbat 60 ("Tween 60"), polysorbat 80 ("Tween 80"), polysorbat 65 ("Tween 65"), polysorbat 85 ("Tween 85"). Mer interessante fuktemidler er polysorbat 20, polysorbat 40, polysorbat 60, polysorbat 80. Det foretrukne fuktemiddel er polysorbat 80.

For fremstilling av sammensetninger ifølge oppfinnelsen, blandes mikronisert nebivololhydroklorid med egnede eksipienser og granuleres. Fortrinnsvis granuleres nebivololhydrokloridet med fyllstoffet eller fyllstoffene før blanding med de andre eksipienser. Mest foretrukket er de an-vendte fyllstoffer laktose og maisstivelse.
Forholdet (vekt/vekt) mellom fuktemiddel og nebivololhydroklorid er en viktig faktor. For å erholde en god oppløsning må den aktive ingrediens fuktes tilstrekkelig. Når på den andre side fuktemidlets andel er for høy i sammensetningen, har de erholdte tabletter ikke den passende hardhet, og derfor er disse tabletter ikke egnet for industriell fremstilling.
Forholdet (vekt/vekt) mellom fuktemiddel og nebivololhydroklorid kan variere mellom ca. 0,025 og 0,5. Dette forhold varierer fortrinnsvis mellom ca. 0,025 og ca, 0,3. Mer foretrukket varierer forholdet mellom ca. 0,04 og ca. 0,25. Mest foretrukket varierer forholdet mellom ca. 0,06 og 0,1.
Tabletter ifølge foreliggende oppfinnelse kan være rette sirkulære sylindere eller ha formen av en stav, hvis ende-overflater kan være flate eller konvekse og hvis kanter kan være jevnet ut. Disse tabletter kan ha streker eller brudd-1injer og kan være preget med et symbol eller andre merker.
Foreliggende oppfinnelse er egnet ved behandling av en
pasient som lider av tilstander forbundet med koronære forstyrrelser og hypertensjon, hvilken behandling omfatter oral administrasjon av en farmasøytisk sammensetning i henhold til oppfinnelsen.
Det vil innses at den eksakte terapeutiske dose av den aktive ingrediens vil være avhengig av alderen og tilstan-den av pasienten og naturen av lidelsen som skal behandles, og den endelige avgjørelse om dosen vil tas av den behand-lende lege.
Generelt er imidlertid virksomme doser for behandling av tilstander forbundet med koronære forstyrrelser og hypertensjon i området fra ca. 0,1 til ca. 50 mg, mest foretrukket ca. 1 til ca. 10 mg, f.eks. ca. 5 mg aktiv ingrediens per enhetsdose som kan administreres i en enkelt dose eller
oppdelte doser, f.eks. 1-4 ganger daglig.
Eksperimentell del
Eksempel 1: Fremstilling av nebivololhydroklorid
(±)-[2R<*>[1S<*>,5S<*>(S<*>)]]- + [2R<*>[IS<*>,5R<*>(R<*>)]]-a,a"-[imino-bis-(metylen)]-bis-[6-fluor-3,4-dihydro-2H-l-benzopyran-2-metanol] (142 g) ble omdannet til saltsyresaltet i etanol (1000 ml). Krystallene ble avfiltrert og krystallisert fra etanol. Den andre fraksjon fra krystallisasjonen ble om-krystallisert fra etanol, hvilket gav 10,3 g (6,6%) (±)-[2R<*>[IS<*>,5S<*>(S<*>)]]-a,a'-[imino-bis-(metylen)]-bis-[6-fluor-3,4-dihydro-2H-l-benzopyran-2-metanol]hydroklorid; smp. 224,9°C, nebivololhydroklorid (krystallinsk forbin-delse l) .
Eksempel 2: Mikronisering av nebivololh<y>droklorid
En mengde på 11 kg nebivololhydroklorid ble mikronisert
under anvendelse av en "Air Classifying Mill" med en mølle-plate utstyrt med steiner. Den optimale arbeidshastighet er 13.500 omdreininger per minutt. Når partiklene er tilstrekkelig små tas de opp med luftstrømmen og føres til en
vindsil hvor de samles, hvilket gir den mikroniserte for-bindelse 1.

Fremstilling av bindemiddelløsningen
92 g hydroksypropylmetylcellulose 2910 15 eps og 9,2 g polysorbat 80 ble oppløst i 1.840 g avmineralisert vann under magnetisk omrøring ved en temperatur på 90°C.
Fremstilling av granulatet
109 g nebivololhydroklorid, 138 g kroskarmellosenatrium, 2.835 g laktose og 920 g maisstivelse blandes i en
granulator med fluidisert sjikt under et arbeidstrykk på 5-6 bar. Luftens innløpstemperatur er 60°C. Blandingen fort-settes inntil luftens utløpstemperatur har nådd 30°C. Deretter sprayes bindemiddelløsningen på pulverblandingen. Etter sprayingen tørkes granulatet ved en luftinnløpstempe-ratur på 75°C.
Fremstilling av sammenpressingsblandingen
Det tørkede granulat, 322 g mikrokrystallinsk cellulose, 138 g kroskarmellosenatrium, 13 g kolloidalt vannfritt silisiumdioksyd og magnesiumstearat siles gjennom en rust-fri stålrammesikt (mesh: 0,95 mm) og blandes i en planet-pulverblandemaskin inntil det erholdes en homogen blanding.
Fremstilling av tablettene
Fra ovennevnte sammenpressingsblanding fremstilles det
tabletter på 230 mg under anvendelse av en rotasjons-tablettpresse.
Eksempel 5: Oppløsningstest
Fremstilling av standardløsningen
Ca. 54,5 mg nebivololhydroklorid ble nøyaktig oppmålt i en 50 ml målekolbe. Denne mengde nebivololhydroklorid ble opp-løst i metanol og fortynnet til volumet (50 ml) med metanol .
Fremstilling av referanseløsningen
En mengde på 5 ml standardløsning (jfr. ovenfor) ble pi-pettert i en 500 ml målekolbe. En placebotablett samt 3 00 ml saltsyre 0,1N ble tilsatt. Denne løsning ble oppvarmet til 37°C og rystet mekanisk i 30 minutter. Løsningen ble videre fortynnet til et volum på 500 ml med saltsyre 0,1N. Deretter ble løsningen filtrert gjennom et 15 fim filter.
Fremstilling av prøveløsningen
En tablett inneholdende mikronisert nebivololhydroklorid (fremstilt som beskrevet i Eksempel 4} ble plassert i et oppløsningskar av Paddleapparatet beskrevet i European Pharmacopoeia med en rotasjonshastighet justert til 50 ± 2 omdreininger per minutt. Oppløsningsmediet var saltsyre 0,1N. Temperaturen var konstant 37°C ± 0t5°C.
Måling
Etter omrøring i 45 minutter i oppløsningskaret ble en prøve på 6 ml fjernet fra oppløsningskaret og filtrert gjennom et 15 fim reagensfilter. Prøvens absorbans ble målt med et spektrofotometer (etter en andre filtrering gjennom et 0,2 nm filter) ved et maksimum nær 280 nm, i en 10 mm celle mot en "ren løsning" ("blank solution") bestående av saltsyre 0,1N.

Tabletter fremstilt ifølge Eksempel 4 viste en oppløsning på 75%, dvs. 75% oppløst, etter 45 minutter.
Eksempel 6: Oppløsningen av tabletter inneholdende kr<y>s-tallinsk sammenlignet med mikronisert nebivolol
Oppløsningshastigheten for tablettene ble målt under anvendelse av en analog fremgangsmåte som beskrevet i Eksempel 5. Tablettene ble plassert i et oppløsningskar i Paddleapparatet med rotasjonshastigheten justert til ca. 100 omdreininger per minutt, hvor oppløsningsmediet var kunstig mavesyre, og temperaturen ble holdt ved 37°C. Oppløsnings-hastigheten for tabletten inneholdende krystallinsk nebivolol (Tablett 1) var mindre enn 50% etter 45 minutter, mens oppløsningshastigheten for tabletten med mikronisert nebivolol (Tablett 2) var over 75% etter 45 minutter.
