Abstract
Stents parcialmente encapsulados son fabricados utilizando espacios cortados en un material de cubierta de PTFEe. Stents anulares son colocados sobre un tubo interno de PTFEe (por ejemplo, soportado sobte un mandril) y son cubiertos mediante un manguito de injertacion "lacey", que es construido al cortar aberturas en un tubo de PTFEe, de tal manera que se crea una serie de tiras circunferenciales y longitudinales. Luego, este manguito "lacey" es laminado al tubo interno de PTFEe para capturar los stents. Al seleccionar el tamano y posicion de las aberturas en la cubierta de PTFEe, es posible dejar partes criticas del stent sin encapsular para facilitar la flexibilidad y expansion. ENCAPSULACION PARCIAL DE STENTS USANDO TIRAS Y BANDAS ANTECEDENTES DE LA INVENCIÓN
CAMPO DE LA INVENCIÓN La presente invención es concerniente en general con el campo de dispositivos médicos y más en particular, con la encapsulación de stents.
DESCRIPCIÓN DE LA TÉCNICA RELACIONADA Los stents y dispositivos endoluminales similares actualmente son usados por profesionales médicos para tratar porciones del sistema vascular que se pueden volver tan estrechos que se restringe el flujo sanguíneo. Tal estrechamiento (estenosis) se presenta, por ejemplo, .como resultado del proceso de enfermedad conocido como arterieesclerosis. La angioplastia de una arteria coronaria para corregir la arterieesclerosis puede estimular la proliferación de tejido en exceso, el cual luego bloquea
(restenosis) el vaso recién reabierto. Mientras que los stents son más frecuentemente usados para "mantener abiertos" los vasos sanguíneos, también pueden ser usados para reforzar estructuras tubulares plegadas o estrechadas en el sistema respiratorio, el sistema reproductor, conductos biliares o cualquier otra estructura de cuerpo tubular. REF: 131819 desarrollarse a través de las aberturas dando por resultado la restenosis del vaso. El politetrafluoroetileno (PTFE) se ha comprobado extraordinariamente ventajoso como un material a partir del cual fabricar injertos o prótesis de vaso sanguíneo, estructuras tubulares que pueden ser usadas para reemplazar vasos dañados o afectados por enfermedad. Esto es parcialmente debido a que el PTFE es extremadamente biocompatible provocando poca o nada de reacción inmunogénica cuando se coloca dentro del cuerpo humano. Esto es también porque en su forma preferida, PTFE expandido (PTFEe), el material es ligero o poroso y es colonizado potencialmente por células vivas convirtiéndose en una parte permanente del cuerpo. El proceso de fabricación de PTFEe de grado de injerto vascular es bien conocido para aquel de experiencia ordinaria en la técnica. Es suficiente decir que la etapa critica en este proceso es la expansión de la pasta de partículas de PTFE cristalinas. La expansión representa un estiramiento longitudinal controlado en el cual el PTFE es estirado hasta varios cientos por ciento de su longitud original. Durante el proceso de expansión las fibrillas de PTFE son alargadas de la partícula de PTFE agregada (nudos) , para crear asi una estructura porosa. Si los stents pudieran ser encerrados en PTFEe, la infiltración celular podria ser limitada, esperanzadamente
impidiendo o limitando la restenosis. Intentos previos para producir un stent envuelto con un PTFEe se enfocaron alrededor del uso de adhesivos o la unión fisica tal como la suturación (véase por ejemplo la patente norteamericana No, 5,405,377 expedida a Cragg). Sin embargo, tales métodos están lejos de lo ideal y la suturación, en particular, es un trabajo muy intensivo. Más recientemente, se han desarrollado métodos para encapsular un stent entre dos elementos de PTFEe tubulares, mediante los cuales el PTFEe de un elemento hace contacto y une al PTFEe del otro elemento a través de las aberturas en el stent. Estos métodos son descritos en las publicaciones recientes WO 98/38947 y WO 96/28115. Sin embargo, tal stent monolíticamente encapsulado tiende a ser más bien inflexible. En particular, la expansión radial del stent puede estirar y romper el PTFEe. Existe una necesidad continua por un stent que sea encapsulado para impedir la intrusión celular y para proporcionar un flujo de sangre en la superficie interior uniforme y más aún apto para la expansión sin rompimiento o deslaminación y que sea relativamente más flexible.
BREVE DESCRIPCIÓN DE LA INVENCIÓN La presente invención es concerniente con stents encapsulados en donde la flexibilidad del stent es retenida, a pesar de la encapsulación.
.¡.l*.ti i -í^yt:,,, . i .-, u?. .: - -* *•"** »* Un objeto de esta invención es proporcionar un dispositivo de stent que tiene flexibilidad mejorada, que aún mantiene su forma en la expansión. También un objeto de esta invención es proporcionar un stent encapsulado para impedir la infiltración celular en donde las porciones del stent pueden moverse dentro de la expansión radial sin el estiramiento o rompimiento del material de encapsulación. Estos y objetos adicionales son realizados mediante un proceso de encapsulación que deja porciones del stent libres de movimiento durante la expansión sin dañar la cubierta de PTFEe. La forma más básica de esta invención es producida mediante la colocación de un stent sobre un elemento de PTFEe interior (por ejemplo, soportado sobre un mandril) y luego al cubrir la superficie exterior del stent con un tubo de PTFEe exterior en el cual las ranuras han sido cortadas. El tubo de PTFEe exterior luego es laminado al
PTFEe interior a través de las aberturas en la estructura de stent para capturar el stent. Al seleccionar el tamaño y la localización de las ranuras es posible dejar partes criticas del stent sin encapsular para facilitar la flexibilidad y la expansión. No solamente la ranura y impide la captura del
PTFE implícito, esta forma un punto focal para que se flexione el PTFE. Una forma más compleja del proceso es colocar sobre el stent un manguito de PTFEe en el cual han
sido cortadas aberturas. Este manguito exterior "lacey" deja porciones del stent dispuestas para flexibilidad incrementada y para movimiento de las porciones de stent durante la expansión sin daño del PTFEe. Aunque puede ser usado un solo stent, estos procedimientos conducen por si mismos al uso de una pluralidad de stents anulares individuales espaciados a lo largo de una tubo de PTFEe interior y cubierto por un manguito de PTFEe "lacey". En la presente invención, los stents anulares individuales son parcialmente encapsulados usando el procedimiento resumido antes en la presente. De preferencia, los stents anulares de estructura sinusoidal en zigzag son colocados "en fase" (por ejemplo, picos y valles de un stent alineados con aquellos de un stent vecino) sobre la superficie de un injerto de PTFEe tubular soportado mediante un mandril. Un manguito de PTFEe es cortado usando láser de C02, de tal manera que son creadas las aberturas, lo que da por resultado una configuración de "lacey". Este manguito "lacey" luego es colocado sobre los stents anulares. La estructura resultante luego es sometida a calor y presión de modo que las regiones de PTFEe son laminadas o fusionadas conjuntamente donde el manguito lacey hace contacto con el injerto tubular. Además, los extremos del stent pueden ser completamente encapsulados, mediante métodos conocidos, para estabilizar la estructura en conjunto.

La referencia será hecha a las hojas adjuntas de dibujos que primero serán descritas brevemente.

DESCRIPCIÓN DETALLADA DE LA MODALIDAD PREFERIDA La presente invención satisface la necesidad para un dispositivo de stent encapsulado para impedir la restenosis, que es flexible en la expansión y contracción de tal manera que es retenida la forma estructural general. Esto es realizado mediante la encapsulación de un stent o una pluralidad de anillos de stent usando una cubierta de PTFEe en la cual han sido cortadas aberturas. Con referencia ahora a los dibujos, en los cuales los números de referencia semejantes representan estructuras similares o idénticas de principio a fin, la figura 1 ilustra una etapa inicial en la construcción del stent parcialmente encapsulado de la presente invención. Un injerto de PTFEe tubular 20 es colocado sobre un mandril para el montaje de un dispositivo 10 (figura 3) . Un stent luego es colocado sobre el injerto 20. En una modalidad preferida, como se muestra en la figura 1, una serie de stents anulares sinusoidales en zigzag 30 son colocados sobre la superficie exterior del injerto 20. Alternativamente, pueden ser usados uno o más stents en donde cada stent comprende más de un anillo o aro (por ejemplo, donde los anillos son conectados

Los stents anulares 30 pueden ser elaborados de cualquier material, pero un material preferido es metal. Los stents anulares en zigzag 30 pueden ser montados "en fase" con cada stent anulare adyacente que tiene 5 picos y valles alineados. Alternativamente, los stents individuales 30 pueden estar "fuera de fase" a diferentes grados. Será evidente que la relación de fase de los stents adyacentes 30 alterará la flexibilidad lateral asi como la capacidad de compresión longitudinal de la estructura. La
relación de fase puede ser variada a lo largo de la longitud del dispositivo 10, para alterar de esta manera las propiedades físicas en diferentes porciones del dispositivo 10. Al tener stents anulares individuales 30, contrario a un stent tubular individual, se proporciona la ventaja de que la
periodicidad o en número y forma precisa de los zigzags por anillo, fácilmente puede ser variado a lo largo de la longitud del injerto para influenciar las propiedades de flexibilidad y estabilidad de la estructura. También, el espaciamiento de los stents individuales (número de stents
por longitud unitaria) asi como la relación de fase de stent a stent puede ser variada para producir injertos de stent con propiedades deseadas. Mediante la colocación de los stents anulares 30 sobre la superficie exterior del injerto de PTFEe tubular 20, la estructura resultante tiene una superficie
interior (luminal) que es completamente uniforme o lisa para
facilitar el flujo de sangre. Sin embargo, puede haber casos en donde los stents anulares 30 u otros stents tubulares sean colocados ventajosamente en contacto con la superficie de injerto interior o sobre ambas de las superficies interior y exterior, como un experimentado ordinario en la técnica fácilmente apreciará. La figura 2 muestra la estructura de un injerto "lacey" 40 que comprende un manguito de PTFEe 42 en el cual han sido cortadas aberturas. Este injerto de "lacey" 40 es colocado sobre los stents anulares 30 en la modalidad preferida. El injerto "lacey" 40 es creado al cortar aberturas 44 en un manguito de PTFEe tubular 42. Las aberturas 44 se cortaron en el manguito mediante un láser de C02, aunque cualquier otra tecnología cortante podria ser fácilmente empleada. El injerto "lacey" 40 es deslizado sobre los stents anulares 30 y el injerto tubular implícito 20 para formar el dispositivo preferido 10 mostrado en la figura 3. manguito 42 con aberturas 44 recortadas, es una manera de llevar a cabo el objetivo de flexibilidad y estabilidad. Las aberturas 44 entre las secciones circunferenciales de PTFEe 46 pueden ser alteradas para controlar el grado de flexibilidad y estabilidad deseado. En la modalidad preferida mostrada en la figura 3, el injerto "lacey" 40 forma un número de secciones circunferenciales 46, que son propuestas para cubrir una porción de la circunferencia de cada stent anulares 30, dejando los extremos de los zigzag sin cubrir. Al cubrir circunferencialmente únicamente una porción de cada stent anulares 30, se proporciona la cantidad máxima de flexibilidad lateral. Sin embargo, al cubrir circunferencialmente los stents anulares individuales 30 sin ningún soporte longitudinal dará por resultado una estructura con poca resistencia y estabilidad longitudinal, que será propensa a la "compresión". Asi, las secciones longitudinales 48 que conectan las secciones circunferenciales de PTFEe 46 son importantes, debido a que las secciones longitudinales 48 son completamente laminadas al injerto implícito 20 y actúan como dispositivos de "anticompresión" al resistir el acortamiento de la estructura 10 (el doble espesor de PTFEe resiste la compresión de las secciones longitudinales 48). El ancho de las secciones circunferenciales 46 y las secciones longitudinales 48 controla la resistencia y estabilidad
longitudinal contra la flexibilidad lateral. Mediante el ajuste de estos parásitos, los injertos pueden ser elaborados más o menos flexibles con mayor o menor resistencia de anticompresión. En la modalidad preferida, cuatro secciones longitudinales 48 son formadas y los extremos de la estructura 10 son completamente encapsulados para mayor estabilidad. Por supuesto, un número más grande de secciones longitudinales 48 se podria formar. También las secciones longitudinales 48 pueden estar por sí mismas en zigzag o pueden ser arregladas helicoidalmente dependiendo de como las aberturas 44 son cortadas en el manguito 42. Cada estructura diferente tendrá diferentes propiedades. De manera similar, las secciones circunferenciales 46 pueden tener diferentes formas y pueden ser ondulantes. No hay nada que impida una cubierta con una configuración más compleja, donde secciones circunferenciales y secciones longitudinales son difíciles de discernir o son aún no existentes. Una segunda modalidad de la presente invención puede ser observada en las figuras 4-6. En lugar de tener una estructura de injertos de "lacey", un manguito exterior ranurado es usado para proporcionar encapsulación parcial del stent, las ranuras que proporcionan estabilidad a la estructura lo que permite al stent expandirse y retraerse más fácilmente. *«"—*-*-«-j manguito sin corte en los extremos. Las ranuras son formadas a 0°, 90°, 180° y 270° y son orientadas para pasar sobre una porción de pico de cada stent anulares en zigzag 30 (figura 6). La figura 5 muestra ranuras circunferenciales 62, en donde las ranuras son cortadas circunferencialmente alrededor del manguito 60 a intervalos espaciados, de preferencia para coincidir con un anillo de stent. En cada sección radial, dos ranuras son cortadas alrededor de la circunferencia a intervalos igualmente espaciados. En una primera sección radial, las ranuras se extienden desde 0o a 90° y desde 180° a 270°. Cada sección radial sucesiva tiene un par de ranuras que están desplazadas 90° del par previo. Así, una segunda sección radial tendrá ranuras que se extienden desde 90° a 180° y desde 270° a 0o. Además de las configuraciones mostradas en las figuras 4 y 5, son posibles un número de otras configuraciones de ranura, que incluyen diagonal y sinusoidal como será apreciado por un experimentado en la técnica. Como se muestra en la figura 6, un manguito 70 es colocado sobre los stents anulares 30 y el injerto tubular implícito 20 para formar una nueva estructura 80. Las ranuras longitudinales 72, que son cortadas en el manguito 70, difieren de las ranuras 52 mostradas en la figura 4 en que no se extienden en la longitud de la estructura 80 y son escalonadas alrededor de la circunferencia del manguito 70. Idealmente, las ranuras son alineadas sobre los picos en los
stents anulares en zigzag 30. Una vez que las ranuras 72 son cortadas en el manguito 70 usando cualquiera de los métodos conocidos, la estructura 80 es expuesta a calor y presión, tal como aquel provocado al envolver con cinta de PTFE y al calentar en un horno, para provocar así que las regiones de PTFEe del injerto ranurado 70 sean fusionadas o laminadas al injerto tubular 20. La ranura 72 en el manguito exterior ranurado 70 pueden ser formadas al usar un láser de C02, hoja de afeitar o cualquier otra técnica adecuada conocida en el arte. Las ranuras mejoran la flexibilidad de la estructura encapsulada y permiten la expansión radial sin el rompimiento del PTFEe. Además, una pluralidad de ranuras ayudan al injerto expandido a que se sujete sobre la pared del vaso. Esto es particularmente importante donde un injerto de stent encapsulado es extendido a una región de vaso dañado o debilitado como en una aneurisma. Además, durante el proceso de sanación los tejidos fácilmente se desarrollan en las ranuras para fijar adicionalmente el injerto a la pared de vaso. Una ventaja que ofrece el corte de ranuras en un manguito de PTFEe es que es un poco más fácil de fabricar que el injerto "lacey". Debido a que no es retirado material, el manguito es un poco más fuerte que un "injerto lacey". Hay una multitud de configuraciones posibles, que incluyen el corte de las ranuras de manera asimétrica para lograr
resultados deseados, tal como el uso de cortes radiales, longitudinales y diagonales simultáneamente. Además, un mayor número de ranuras pueden ser cortadas en una región de la estructura en la cual es deseada mayor expansión. Aunque los ejemplos anteriores son descritos con los injertos "lacey" y ranurados que son colocados sobre un stent el cual por si mismo es reforzado sobre un injerto tubular, esta orientación puede ser fácilmente invertida. Esto es, los injertos "lacey" o ranurados pueden ser colocados sobre un mandril; un stent o stents luego pueden ser colocados sobre los injertos "lacey" o ranurados y un injerto tubular luego puede ser colocado sobre el stent o los stents. Esto da por resultado una estructura en donde parte o mucha de la superficie luminal es proporcionada mediante el injerto exterior, lo que da por resultado la sanación superior ya que únicamente una sola capa de PTFEe separará los tejidos del cuerpo de la sangre. Similarmente, es posible una estructura con dos injertos "lacey" o ranurados. Al mantener las aberturas en un injerto fuera de fase con aquellas en el otro injerto, se obtiene una estructura hermética a la sangre. No obstante, una mayor parte del área de superficie final del dispositivo comprenderá una sola capa que separa el tejido del cuerpo de la sangre circulante. Únicamente el área efectivamente ocupada mediante el (los) stent (s) y mediante la sobreposición entre los dos injertos

Además tal estructura tendrá un perfil más pequeño cuando se comprime debido a que es reducida la cantidad total de PTFE. Del mismo modo, puede ser empleada una combinación de injerto "lacey" e injerto ranurado. Habiéndose descrito de esta manera las modalidades preferidas de la encapsulación parcial de los stents, será evidente para aquellos experimentados en la técnica como se han logrado ciertas ventajas de la presente invención. También se debe apreciar que varias modificaciones, adaptaciones y modalidades alternativas de la misma pueden ser elaboradas dentro del alcance y espíritu de la presente invención. Por ejemplo, se han ilustrado anillos de stent en zigzag, pero debe ser evidente que el concepto inventivo descrito anteriormente será igualmente aplicable a diseños sinusoidales y otros diseños de stent. Las modalidades descritas son para ser consideradas como ilustrativas restrictivas. La invención adicionalmente es definida por las siguientes reivindicaciones. Se hace constar que, con relación a esta fecha, el mejor método conocido por la solicitante para llevar a la práctica la citada invención, es el que resulta claro de la presente descripción de la invención.
Claims (18)
- REIVINDICACIONES Habiéndose descrito la invención como antecede, se reclama como propiedad, lo contenido en las siguientes reivindicaciones : 1. Un injerto vascular reforzado, expansible radialmente, que comprende una primera y una segunda capa de politetrafluoroetileno expandido y una capa de soporte expansible radialmente que comprende al menos un stent, en donde la capa de soporte es puesta en contacto con una superficie de la primera capa de politetrafluoroetileno expandida y es asegurada a la misma mediante la segunda capa d politetrafluoroetileno expandida, caracterizado porque al menos una de las capas de politetrafluoroetileno expandidas incluye una pluralidad de aberturas espaciadas cortadas directamente en la misma, adaptadas para dejar al menos una porción de la capa de soporte no asegurada.
- 2. El injerto vascular reforzado, expansible radialmente de conformidad con la reivindicación 1, caracterizado porque la segunda capa de politetrafluoroetileno expandida incluye la pluralidad de aberturas espaciadas.
- 3. El injerto vascular reforzado, expansible radialmente de conformidad con la reivindicación 1, caracterizado porque tanto la primera como la segunda capa de politetrafluoroetileno expandido incluyen una pluralidad de aberturas espaciadas, en donde las aberturas en la primera capa de politetrafluoroetileno expandido son adaptadas para ser alineadas fuera de fase con las aberturas en la segunda capa de politetrafluoroetileno expandida.
- 4. Un injerto vascular reforzado, expansible radialmente de conformidad con cualquiera de las reivindicaciones anteriores, caracterizado porque las aberturas además comprenden ranuras.
- 5. El injerto vascular reforzado, expansible radialmente de conformidad con la reivindicación 3, caracterizado porque las aberturas en la primera o segunda capa de politetrafluoroetileno expandida además comprenden ranuras .
- 6. Un injerto vascular reforzado, expansible radialmente de conformidad con las reivindicaciones 4 o 5, caracterizado porque las ranuras son orientadas longitudinalmente .
- 7. El injerto vascular reforzado, expansible radialmente de conformidad con las reivindicaciones 4 o 5, caracterizado porque las ranuras son orientadas circunferencialmente .
- 8. Un injerto vascular reforzado, expansible radialmente de conformidad con cualquiera de las reivindicaciones anteriores, caracterizado porque la capa de soporte expansible radialmente comprende una pluralidad de stents anulares.
- 9. El injerto vascular reforzado, expansible radialmente de conformidad con la reivindicación 8, caracterizado porque cada uno de los stents anulares es formado en una configuración en zigzag de picos y valles alternantes .
- 10. El injerto vascular reforzado, expansible radialmente de conformidad con la reivindicación 9, caracterizado porque los stents anulares en zigzag son adaptados para ser alineados con los picos y valles alternantes en fase.
- 11. El injerto vascular reforzado, expansible radialmente de conformidad con cualquiera de las reivindicaciones anteriores, caracterizado porque el stent es elaborado de metal.
- 12. El injerto vascular reforzado, expansible radialmente de conformidad con cualquiera de las reivindicaciones anteriores, caracterizado porque al menos un extremo del injerto vascular reforzado, expansible radialmente es completamente encapsulado.
- 13. Un método para fabricar un injerto vascular reforzado, expansible radialmente, parcialmente encapsulado, que comprende proporcionar una primera capa de material de politetrafluoroetileno expandida, proporcionar una segunda capa de material de politetrafluoroetileno expandido, disponer una capa de soporte expansible radialmente que comprende al menos un stent sobre la primera capa de politetrafluoroetileno expandida, colocar la segunda capa de 5 politetrafluoroetileno expandida sobre la capa de soporte expansible radialmente y laminar la segunda capa de politetrafluoroetileno expandida a la primera capa de politetrafluoroetileno expandida, caracterizado porque comprende: cortar una pluralidad de aberturas en al menos un 10 extremo de las capas de politetrafluoroetileno expandido y posicionar las aberturas con respecto a la capa de soporte, para dejar una porción de la capa de soporte expuesta a través de las aberturas.
- 14. El método de conformidad con la reivindicación 15 13, caracterizado porque la capa de soporte expansible radialmente comprende una pluralidad de stents anulares formados en una configuración en zigzag de picos y valles alternantes, en donde la etapa de disposición además comprende disponer los picos y valles de stents sucesivos en 20 fase.
- 15. El método de conformidad con las reivindicaciones 13 o 14, caracterizado porque además comprende la etapa de encapsular completamente al menos un extremo del injerto vascular reforzado, expansible 25 radialmente.
- 16. Un método para fabricar un injerto vascular reforzado, expansible radialmente, parcialmente encapsulado, que comprende proporcionar una primera capa de material de politetrafluoroetileno expandida, proporcionar una segunda capa de material de politetrafluoroetileno expandida, disponer una capa de soporte expansible radialmente que comprende al menos un stent sobre la primera capa de politetrafluoroetileno expandida, colocar la segunda capa de politetrafluoroetileno expandida sobre la capa de soporte expansible radialmente y laminar la segunda capa de politetrafluoroetileno expandida a la primera capa de politetrafluoroetileno expandida, caracterizado porque comprende: cortar una pluralidad de ranuras en al menos una de las capas de politetrafluoroetileno expandidas, tubulares y posicionar las ranuras para extender al menos una porción de la capa de soporte expansible radialmente.
- 17. El método de conformidad con la reivindicación 16, caracterizado porque la capa de soporte expansible radialmente comprende una pluralidad de stents anulares formados en una configuración en zigzag de picos y valles alternantes, en donde la etapa de disposición además comprende disponer los picos y valles de stents sucesivos en fase.
- 18. El método de conformidad con las reivindicaciones 16 o 17, caracterizado porque además comprende la etapa de encapsular completamente al menos un extremo del injerto vascular reforzado, expansible radialmente. 1 A,'"y RESUMEN DE LA INVENCIÓN Stents parcialmente encapsulados son fabricados utilizando espacios cortados en un material de cubierta de PTFEe. Stents anulares son colocados sobre un tubo interno de PTFEe (por ejemplo, soportado sobte un mandril) y son cubiertos mediante un manguito de injertación "lacey", que es construido al cortar aberturas en un tubo de PTFEe, de tal manera que se crea una serie de tiras circunferenciales y longitudinales. Luego, este manguito "lacey" es laminado al tubo interno de PTFEe para capturar los stents. Al seleccionar el tamaño y posición de las aberturas en la cubierta de PTFEe, es posible dejar partes críticas del stent sin encapsular para facilitar la flexibilidad y expansión. Alternativamente, los espacios pueden consistir de hendiduras cortadas en el material de cubierta de PTFEe. Estas hendiduras pueden ser cortadas en cualquier dirección, en las que se incluyen longitudinal, radial o diagonalmente.
