Abstract

アテローム性動脈硬化性狭窄および他の病態の治療において、管腔内で放射状の環状支持体を提供するために、血管のような体腔に弾力的に展開可能なステントを挿入することは一般的に知られている。例えば、通常の方法(例えば、バルーン血管形成術またはレーザー剥離手術)によって遮断された心血管を開通させ、展開可能なステントを用いてその血管を開口状態に維持することは、一般的に既知である。

ポリテトラフルロエチレン(PTFE)は、損傷または疾患を有する血管を置換するために用いられる血管グラフトまたは人工器官を作製するための材料として用いられている。この理由の一部は、PTFEが極めて生体適合性であり、体腔内に留置した場合に免疫原性反応をほとんどまたは全く引き起こさないためである。さらに、好ましい形状において、連続多孔質PTFE(ePTFE)が用いられている。この材料は軽く多孔性であり、生体細胞がおそらくコロニーを形成して体の永続的な一部となる。血管グラフト等級のePTFEを作製するプロセスは周知である。

通常のステントグラフトには、それらが相対的な軸方向の運動を受けないように、互いにそしてステント部分の端部から一定間隔で存在する円周方向支持部材(輪)が含まれる。隣接する支持要素間の間隔は、隣接する支持要素間の堅固な接続または架橋要素(時に、当技術分野において「ブリッジ」と呼ばれる)およびステント部分の第一の端部からステント部分の第二の端部に伸展する少なくとも一つの伸長部材によって維持される。円周方向支持部材はまた、ステント部分に沿って伸展する材料のグラフト部分に対して固定され、それによってグラフト部分は、ステント部分の長さに沿って移動することができない。これらの安定した堅固な接続のために、支持要素およびグラフト部分は、ステントの伸長部材に沿って縦方向に移動することができず、ステントグラフトはそれが留置される血管の湾曲に適合することができない。ステントグラフトの大きい展開直径に沿った通常のステント材料と通常のグラフト材料との相互作用により、通常のステントに関連した問題の他に、塑性変形能、性能および製造の問題が起こり、このことは参照として本明細書に組み入れられる、同時係属中の米国特許出願第10/100,986号において考察されている。例えば、ステントグラフトの縦方向の柔軟性が不良であるために、グラフトのねじれが起こり、最終的に血流管腔の閉塞が起こりうる。通常のステントグラフトのさらなる短所には、ステント部分のグラフト摩耗、グラフト部分とステント部分とを固定する縫合を受容するグラフト部分における縫合穴を通しての血液の漏出、および労力のかかる製造プロセスが含まれうる。
当技術分野において、縦方向に柔軟性であり、血流にとってなめらかな内表面を提供するステントグラフトが必要である。
発明の概要
本発明は、通常のステントグラフトと比較して縦方向の柔軟性が増加したステントグラフトに関する。本明細書において用いられる縦方向の柔軟性は、その主要な縦軸方向の伸展に対して移動するステントグラフト構造(またはその一部)の柔軟性に関する。ステントグラフトは、管腔を支持するためおよび管腔動脈瘤を修復するために体腔内に留置される。好ましい態様において、ステントグラフトは、大動脈瘤を修復するために血管内に配置されて展開される。
本発明は、通常のステントグラフトと比較して縦方向の柔軟性が増加したステントグラフトに関する。本明細書において用いられる縦方向の柔軟性は、その主要な縦軸方向の伸展に対して移動するステントグラフト構造(またはその一部)の柔軟性に関する。ステントグラフトは、管腔を支持するためおよび管腔動脈瘤を修復するために体腔内に留置される。好ましい態様において、ステントグラフトは、大動脈瘤を修復するために血管内に配置されて展開される。
一つの態様において、ステントグラフトは、展開可能なステント部分、展開可能なグラフト部分、および少なくとも一つの伸長したレールを含みうる。ステント部分およびグラフト部分は、レールの末端部のあいだでレールに対して移動可能であり、それによってステントグラフトはそれが留置される血管の形状に適合することができる。さらに、ステント部分の縦方向に隣接する円周方向支持要素は、少なくとも一つの架橋要素によって共に固定することができる。または、それぞれの円周方向支持要素は、縦方向に隣接する円周方向支持要素と接続されない。レールおよび架橋要素を用いることによって、支持要素を必要に応じて分離させる、血管の外部曲率半径を仮定させる、および血管の内部曲率半径を仮定させるために短くすることができる。ステントグラフトは、通常のステントグラフトに関連した不良な縦方向の柔軟性を有しない。その結果、本発明のステントグラフトは、展開後のねじれに対するより大きい抵抗性を提供し、このようにグラフト部分が血流管腔を閉塞する可能性を消失させる。その上、グラフトの摩耗は減少し、その寿命は増加する。
さらに、本発明の一つの局面に従って、ステントグラフトのグラフト部分を、ステントグラフトの末端から離れた位置で少なくとも一つの縦方向に伸展するレールに連結させる。一つの態様において、グラフト部分は、グラフト部分の中を伸展して、これらの離れた位置でのグラフト部分の液体保持完全性を損なう縫合を用いることなく、ステントグラフトの末端部から離れた位置でレールに連結させる。その代わりにグラフト部分の周囲に存在してグラフト部分に固定される円周方向連結部材がレールを受容することができる。これらの連結部材には、レールを受容する円周方向に距離を置く開口部が含まれる。または、レールは、グラフト部分の基板において機械的に作製された焼灼された穴の中を伸展する。これらの開口部および穴の中をレールが通過することによって、製造コストおよび時間が減少する。レールの通過によってまた、より大きい伸展された縦方向の柔軟性が提供され、ステント部分の先端がグラフト部分および血管の中に突出することが防止され、ならびにグラフト部分を形成する材料の摩耗が減少する。本発明に従ってグラフト部分に対してレールを固定すれば、縫合を用いる通常のステントグラフトについて典型的に認められる血液の漏出がなくなる。本明細書において考察したこの態様または任意の態様において、グラフト部分の端部は、縫合によってステント部分に固定してもよい。
発明の詳細な説明
同様の数字が図全体を通して同じ要素を示す図を参照して、図1は、本発明に従うステントグラフト10を示す。ステントグラフト10には、柔軟な伸長したレール要素50を有するグラフト部分100およびステント部分20が含まれる。ステント部分20は、ステントグラフト10が留置されて、血管腔のような哺乳類の体の一部の内部で展開状態で存在する場合にグラフト部分100に対する支持を提供する。
同様の数字が図全体を通して同じ要素を示す図を参照して、図1は、本発明に従うステントグラフト10を示す。ステントグラフト10には、柔軟な伸長したレール要素50を有するグラフト部分100およびステント部分20が含まれる。ステント部分20は、ステントグラフト10が留置されて、血管腔のような哺乳類の体の一部の内部で展開状態で存在する場合にグラフト部分100に対する支持を提供する。
ステント部分20には、複数の一定間隔で存在する円周方向に伸展する支持要素(輪)22が含まれる。それぞれの円周方向支持要素22は、一般的に図1に示される形状の輪状である。それぞれの円周方向支持要素22は、柔軟な生体適合性の材料(すなわち、例えば非反応性および/または非刺激性である材料)で構成される。一つの態様において、ステント部分20は、生体適合性の材料のチューブから形成することができる。例えば、ステント部分20は、ステント部分20およびその支持要素22等をチューブからレーザーによって切断することによって形成することができる。もう一つの態様において、それぞれの円周方向支持要素22は、例えば、ワイヤセグメントの二つの端部を互いに微細溶接することを含む既知の方法で、閉鎖環(すなわち輪状の輪として)として形成された医療用等級の金属ワイヤから形成される。
通常のステントにおいて用いられるステンレス鋼、金属合金、形状記憶合金、超弾性合金およびポリマー材料は、そこから円周ステント部分20およびその支持要素22を形成することができる材料の代表的な例である。合金には、NiTi(ニチノール)が含まれうる。円周方向支持要素22のためのポリマーは、例えばステントが除去される代わりに体内で吸収されうるように生体吸収性のポリマーであってもよい。
図1および2に示す第一の態様において、それぞれの円周方向支持要素22は、正弦波またはそうでなければ波動の形状のような波状型を有する。図1および2に示すように、支持要素22の波状型には、山12および谷13(山の後の空間)が含まれる。谷13には、それぞれの山12を形成する湾曲部材16に接続される隣接する実質的に直線の支柱14のあいだの広い空間が含まれる。それぞれの山12は、その直前または直後に続く円周方向に配置された山12の方向とは反対の方向を指す。同じことが谷13にも当てはまる。それぞれの谷13は、その直前または直後の円周方向に配置された谷とは反対方向を指す。
図1および2に示す態様において、山12は、全て一つの方向、すなわちステント20の第一の末端部54を向く。同様に、谷13は、第一の末端部とは反対のステント20の第二の末端部56に向けて全て同じ方向を向く。それぞれの円周方向支持要素22は、それぞれの架橋要素24によって縦方向に隣接する円周方向支持要素22に接続される(図1および2)。示されるように、架橋要素24は、隣接する支持要素22の隣接しているが円周上の位相のずれた山12を接続する。結果として、隣接する支持要素22は、それらが架橋要素24によって結合する領域で互いに堅固に一定距離を保つことができる。
図1および2に示す態様において、それぞれの隣接する支持要素22のあいだにはごく限られた数の架橋要素24が提供される。例えば、隣接支持要素22は、約1個〜3個の架橋要素24によって互いに接続してもよい。一つの態様において、一つのみの架橋要素24が隣接支持要素22のあいだに伸展する。隣接支持要素のあいだに提供される架橋要素24が多すぎる場合、支持要素22間の連結は、支持要素間の堅固な連結を提供する場合と類似となり、本発明に従う所望の縦方向の柔軟性が失われる。ごく限られた数の架橋要素24(一つの架橋要素24を含むがこれに限定されない)を提供することによって、得られた集合体は、完全に独立した円周方向支持要素22を用いた場合になおも良好に近似することができる。
さらに、架橋要素24がそれぞれの隣接する支持要素22のあいだで提供される周辺の位置は、縦方向の柔軟性に対して影響を及ぼす。例えば、二つの架橋要素が、支持要素22の直径上の反対側でそれぞれの対の隣接支持要素22のあいだに提供される場合、一般的に、そのあいだの縦方向の柔軟性は、架橋要素24から約90°に位置する支持要素22の直径上の反対側で最大であり、支持要素22の円周に沿ってそれぞれの架橋要素24に近づく方向で減少する。
前述の理由から、例えば、図1に示すように隣接支持要素22のあいだに一つの架橋要素24を提供することは有用であり、またはそうでなければ有益となる可能性がある。さらに、図1に同様に示すように、円周方向に縦方向に隣接する架橋要素24からそれぞれの架橋要素24をずらして配置させることはさらに有用となる可能性がある。円周方向にずれた配置は、隣接する支持要素22間のステント部分20の長さに沿って山12の一つの組によってジグザグにすることができる。または、架橋要素24は支持要素22の隣接する対に関して180°まで円周方向にずらすことができる。上記の円周方向にずらした態様は、架橋要素24を用いる構造的完全性の利点を提供するが、得られた縦方向の柔軟性の制限を分散させ、それによってステントの横方向の偏りは明らかに制限されない。
図9〜15に示すもう一つの態様において、円周方向支持要素22は、多数の接続された実質的に菱形の支持部材30によって形成される。それぞれの菱形支持部材30は、互いに円周上の反対方向を指す第一の円周方向の山32および第二の円周方向の山33を有する。それぞれの菱形支持部材30にはまた、ステント部分20の異なる末端部に向かう第一の縦方向の山34および第二の縦方向の山35も含まれる。円周方向の連続的な菱形支持部材30は、支持要素22の圧縮または成形の際に支持要素22の一部として形成される接合部36で互いに接続される。または、接合部36は、溶接、フック、または摩擦フィッティングのような通常の技術を用いて適用することができる。
図1および2に示すように、支持要素22には、支持要素22がレール50に沿って移動できるように、柔軟で伸長したレール要素50(以降「レール」と呼ぶ)が自由に取り付けられている。レール50は、第一の末端部54で末端の支持要素22の最も外側の山12と、第二の末端部56で末端の支持要素22の最も内側の山12とのあいだにステントグラフト10の長さに沿って伸長する。説明するように、末端の支持要素22は、グラフト部分100の末端部を超えて伸長することができる。
レール50は、望ましくは血管において曲げ、湾曲等に適合するように十分に柔軟性である。一つの態様において、レール50は、縦方向に膨張しない。同様に、レール50は、以下の生体適合性の材料:金属、先に考察したものを含む金属合金、ガラス、またはアクリル、および生体吸収性のポリマーを含むポリマーから作製してもよいがこれに限定されない。レール50は、任意の形をとりうる。例えば、レール50は、ワイヤまたは環状、楕円、もしくは他の既知の断面を有する押し出し物のような、固体の円柱状部材となりうる。または、レール50は、リボンまたはばねワイヤとなりうる。
一般的に同じ厚みである架橋要素24、および結合して、このように比較的非柔軟性となる円周方向支持要素22とは対照的に、レール50の厚みは、所定のステントグラフト10に対して所望の程度の柔軟性を提供するように設計することができる。各レール50は、厚さ(直径)が約0.001インチ〜約0.010インチを有しうる。一つの態様において、各レール50は、厚さが約0.0011インチ〜約0.005インチを有しうる。もう一つの態様において、各レール50は厚さ約0.005インチを有する。レール50は、参照として本明細書に組み入れられる、同時係属中の米国特許出願第10/100,986号において考察されるように、円周方向支持要素22の中を通過する、またはくねらせつつ進む。さらに、レール50は、下記において考察するように、ステント部分20とグラフト部分100の中を通過することができる。
レール50の少なくともいくつかには、円周方向支持要素22が偶然にレール50の末端部54、56を超えて通過しないように末端構造が含まれてもよい。末端構造は、参照として本明細書に組み入れられる、米国特許出願第10/100,986号に説明されるようにいくつかの形であってもよい。一例として、末端構造は、それによって最も先端の円周方向支持要素22がレール50の末端54、56に対して固定して配置される機械的押し出しまたはつかみ構造であってもよい。さらにもう一つの態様において、構造はまた、最も先端の円周方向支持要素22の一部をレール50の末端54、56に結合させるために溶接してもよい(例えば、レーザーによって作製してもよい)。
図1に示すように、ステント部分20には、末端54、56のあいだに伸展する8個のレール50が含まれうる。しかし、支持要素22の円周に沿って山12の数までのかなり多数のレール50を用いることができると企図される。例えば、支持要素22に3組の山12が含まれる場合、3個のレール50を用いることができる。支持要素に14組の山12が含まれる場合、14個までのレール50を用いることができるであろう。末端部54、56の支持要素22のあいだにおいて、架橋要素24によって互いに接続した支持要素22は、レール50に沿って自由に移動する。これらの残りの支持要素22は、ステント50が血管の形状に適合できるようにレール50に沿ってスライドする。同様に、末端支持要素22は、レール50に沿って移動することができると企図される。
図1に示した態様において、円周方向支持要素22には、その中をレール50が伸長する湾曲部材16における開口部17が含まれる。開口部17は、ステント部分20の長さと実質的に平行である方向に山12の中を伸展する。これらの開口部17は、ステントグラフト10の長さと平行な方向に支持レール50を保持してその方向に向ける。同様に、一つの態様において、レール50は、それらがステントグラフト10の外表面を超えて突出しないようにステントグラフト10の壁内(外表面内)に完全に含まれる。
ステントグラフト20の支柱14は、血管内に配置されて展開された場合にグラフト部分100と血管とを支持するために必要な強度をそれらに提供する実質的に任意の厚み半径を有しうる。それぞれの支柱14は、それが留置される血管に損傷を与えない実質的に低いプロフィールを有する。一例において、支柱14は、約0.0001インチ〜約0.020インチの厚み半径を有しうる。一つの態様において、厚み半径は約0.002インチ〜約0.008インチである。もう一つの態様において、支柱14は、厚み半径約0.004インチ〜約0.005インチを有する。これらの厚みは、グラフト100を支持するために、それが留置される血管を支持するために必要な構造および展開特性、ならびに血管の天然の伸展形状に適合するための縦方向の柔軟性をステントグラフト10に提供する。
一つの態様において、湾曲部材16の領域は、開口部17と受容レール50に適合するように支柱14と同じ厚み半径を有するように形成される。もう一つの態様において、湾曲部材16の領域は、開口部17に適合するように支柱14より大きい厚み半径を有するように形成される。例えば、湾曲部材16の厚み半径は、支柱14の厚みより約0.001インチ〜約0.006インチ大きい範囲となりうる。開口部17は、レール50を受容するために直径約0.005インチを有しうる。展開させた場合にレール50間の厚みは約0.004インチとなりうるであろう。厚み0.002インチの支柱14の壁を有するステント部分20は、レール50が通過する約0.009インチの厚み半径を有する湾曲部材16を有しうるであろう。
図9〜15および17に示した態様において、レール50は、支持要素22の第一および第二の縦方向の山34、35に存在する開口部39の中に伸展する。第一の態様において、縦方向の山34と縦方向の山35および取り巻く開口部39を形成する支持部材30の領域は、山32〜35のあいだに伸展する縦方向の支柱37の厚みと同じ厚み半径を有しうる。代わりの態様において、開口部39を取り囲む領域は、縦方向の支柱37の厚みより大きい厚み半径を有しうる。先に考察したように、開口部39周辺の領域の厚み半径は、支柱37の厚みより約0.001インチ〜約0.006インチ大きい範囲となりうる。例えば、厚み半径約0.002インチの支柱37を有する菱形支持部材30は、約0.006インチ〜約0.009インチの厚み半径を有する縦方向の山34、35を有しうるであろう。
それぞれの開口部39は、レール50をスライド可能に受容するために十分に大きい直径を有しうる。各開口部39の直径は、約0.0014インチ〜約0.012インチの範囲となりうる。一つの態様において、レール受容領域は、約0.0014インチ〜0.006インチの範囲の開口部を有する。しかし、レール50をスライド可能に受容する如何なる直径も同様に用いることができる。
図18〜20に示す代わりの態様において、レール50は、支持要素22を形成する支持部材30の表面から伸展するレール受容部材130内でスライド可能に受容される。これらのレール受容部材130は、レール50を支持要素22にスライド可能に連結させる。示されるように、レール受容部材130は、それぞれの支持部材30の縦方向の山34、35の近位に存在する。しかし、レール受容部材130は、そのそれぞれの支持要素22の長さに沿って他の位置に存在しうるであろう。上記の任意の態様には、レール受容部材130を有する支持要素22が含まれうる。
図18に説明した第一の態様において、レール受容部材130は、支持部材30の縦方向の山34、35の近位に存在する。この態様の受容部材130には、レール50を受容する溝139を有するアーム137が含まれる。溝139は、支持要素22をレール50に連結させるために大きさが十分に大きいが、なおもレール50に沿ってグラフト部分100に対して支持要素22を移動させることができるベアリング表面を有する。
図19に示す態様において、それぞれの受容部材130には、支持部材30の長さに沿って互いに枝分かれする二つの相対するアーム158が含まれうる。アーム137と同様に、それぞれのアーム158には、支持部材30をレール50に連結させる大きさであって、支持部材とステント部分20とをレール50に対してスライド運動させる溝159が含まれる。
図18および19に示すいずれかの態様において、アーム137、158はその支持部材30の一部から穿孔されることによって形成されうる、またはそうでなければ機械的に形成されうる。または、アーム137、158は、溶接または他の既知の接続技術によってそれぞれの支持部材30に固定することができる。それぞれのアーム137、158は、グラフト部分100の方向においてその支持部材30から離れて内側に伸展するように形成することができる。そのような態様において、アーム137、158は、ステントグラフト10が留置される血管の内表面を接続することを意図していない。または、受容部材130のアーム137、158は、ステントグラフト10が留置される血管の内壁に接することが意図されるステント部分100とその支持部材30の外表面から外側に離れるように突出することができる。上記の態様と同様に、溝139、159は、約0.0014インチ〜0.012インチのあいだの開口部を有するレール受容領域を提供する。一つの態様において、溝139、159のレール受容領域は、約0.0014インチ〜0.006インチの範囲の開口部を有する。
図20に示すように、レール受容部材130にはまた、その中にレール50がパチンとはめ込まれうる溝164を形成する相対する協調作用するアーム163の対が含まれうる。溝164は、支持部材30がレール50に連結されて、他の態様に関して先に考察したようにレール50に沿って縦方向に自由に移動するように、レール50を受容する大きさである。アーム163は、図18および19において説明した態様に関して先に考察したとおりに形成することができる。さらに、アーム163は、図18および19において説明した態様に関して先に考察したように、そのそれぞれの支持部材30の内表面または外表面のいずれかから伸展しうる。
上記の任意の態様において、説明されたグラフト部分100は、テレフタル酸ポリエステル(PET、以前にデュポン(Dupont)社の商標「ダクロン」として入手されているポリエステル)、ポリテトラフルオロエチレン(PTFE、テフロン)、およびフッ素添加エチレンプロピレン(FEP、溶融プロセシングのための付加物を有するテフロン)を含む織物様ポリエステルのような周知の生体適合性の材料で形成される。ポリプロピレン、多孔性ポリウレタンを含むポリウレタン等を含む他のポリマー生地を用いることができる。一つの態様において、生体適合性の材料は連続多孔質ポリテトラフルオロエチレン(ePTFE)である。ePTFEを作製する方法は当技術分野において周知であり、同様に参照として本明細書に組み入れられる、1980年2月5日にGoreに交付された米国特許第4,187,390号に記述されている。グラフト部分100は、織物材料または不織材料のいずれかによって形成することができる。
ePTFEの多孔性構造は、非常に小さい細線維によって接続されている節からなる。ePTFE材料は、人工血管グラフトとして用いた場合に多くの長所を提供する。ePTFEは、非常に生体適合性であり、優れた力学的および取り扱い特徴を有し、患者の血液を前凝固させず、移植後比較的迅速に治癒し、そして血栓抵抗性である。さらに、ePTFEは、血管系に移植すると、本来の組織の内増殖および細胞の内皮化を可能にする微孔性構造を有する。これは、長期間の治癒およびグラフトの開口に関与する。
グラフト部分100は、図1〜17に示すように、レール50とステント部分20によって取り囲まれうる。図1〜8に示される第一の態様において、ステントグラフト10には、グラフト部分100の長さに沿って互いに一定間隔で存在する複数の円周方向に伸展するレール受容連結部材60が含まれる。レール受容連結部材60によって、グラフト部分100の末端から離れた位置のグラフト部分100にステント部分20を縫合する必要がなくなる。
それぞれの連結部材60は、グラフト部分100の外表面の一部と円周および縦方向に同時伸展可能な大きさである。連結部材60は、グラフト部分100の周囲に沿って360°伸展することができ、またはグラフト部分100の円周の周りにごく部分的に伸展することができる。例えば、それぞれの連結部材60は、グラフト部分100の円周の周りをわずか約270°または180°伸展してもよい。連結部材60は、ステントグラフト10を自己展開またはバルーンのいずれかを用いて血管内で展開させた場合に、ステント部分20とグラフト部分100と共に展開する。
それぞれの連結部材60は、PTFE、ePTFE、FEP、織物様PET(DACRON)、PET被膜、またはグラフト部分100の外部に結合させることができ、以降「開口部62」と呼ぶその関連する経路62の中をレール50がなめらかに容易に通過することができる任意のポリマーを含む、グラフト部分100に関して先に記述した材料のような既知の材料で形成される。それぞれの連結部材60の材料は、グラフト部分100に関して用いられる材料に応じて変化しうる。
図6および7に示すように、開口部62は、レール50がその中を通過する前後で開口部62がその開口位置を保持するように、連結部材60の内表面とグラフト部分100の外表面104とのあいだに形成される。開口部62は、連結部材60の円周の周りに均等にまたは不均等に距離を開けて存在する。一つの態様において、開口部62は、グラフト部分100の長さに沿って軸方向に配置される。しかし、代わりの態様において、隣接する連結部材60の開口部62は、互いに対して円周上でずれて存在しうる。連結部材60の周囲の円周上に存在する開口部62の数は、ステントグラフト10に関して用いられるレールの数に等しいであろう。例えば、ステントグラフト10にレール50 5個が含まれる場合、それぞれの縦方向に間隔をあけて存在する連結部材60には、少なくとも5個の開口部62が含まれうるであろう。
一つの態様において、連結部材60の数は、グラフト部分100の周囲に伸展する支持要素22の数と等しいであろう。図5に示すように、それぞれの連結部材60は、超音波溶接、接着結合、熱融合、または他の既知の方法によってグラフト部分100の外表面に固定された材料の単層64から形成される。この態様において、レール50は、それぞれの開口部62での各連結部材60の内表面63と、グラフト部分100の外表面104とのあいだに伸展する。
代わりの態様において、連結部材60には、グラフト部分100の外表面104に固定された第一の円周方向伸展部材と、第一の部材の上に配置された第二の円周方向伸展部材とが含まれる。この態様において、開口部62は、二つの円周方向伸展部材のあいだに形成される。
図1〜8に関する上記の任意の態様において、連結部材60は、連結部材60がレール50に沿ってこれに対して移動することができるように、レール50を受けながらグラフト部分100およびステント部分20に固定される。連結部材60は、溶接または他の既知の通常の固定技術によって支持要素22に固定することができる。もう一つの態様において、連結部材60は、支持要素22における細隙の中を伸展することができ、または支持要素22の内表面上に形成された凹部に接着によって固定することができる。
図9〜11に示すもう一つの態様において、連結部材60は、ステントグラフト10の長さに沿って配置することができ、その開口部62が縦方向に隣接する連結部材66、68の開口部62から円周上にずれて存在する様な方向に向けることができる。図9に示すように、連結部材66は、それぞれのレール50が、支持部材30の縦方向の山34、35のあいだであるA点で連結部材60における開口部62の中を通過するように、円周方向に配置された支持部材30における開口部の中に存在する。連結部材60は、直ちに円周方向に隣接する支持部材30の中を伸展する円周方向に隣接するレール50の下を通過する(図9を参照されたい)。縦方向の直ちに隣接連結部材68の開口部62は、レール50がB点で隣接連結部材68の開口部62の中を通過するように、連結部材66の開口部から円周方向にずれて存在する。その結果として、縦方向の直ちに隣接する連結部材60(66、68)は、ずれた点で円周方向に配置されるレール50をスライド可能に受容する。これは、それが留置される血管の形状に対するその適合能を減少させることなく、ステントグラフト10の安定性を増加させることができる。
図10および11に示す代わりの態様において、縦方向に配置された連結部材60は、B点で支持部材30の外部でレール50を受容する。この態様において、縦方向に隣接する連結部材60の開口部62は、円周および縦方向に配置される。
図12〜15に示す態様において、レール50は、連結部材60を用いる代わりにグラフト部分100における焼灼された開口部の中を伸長することができる。したがって、これらの代わりの態様において、円周上に直ちに隣接するレール50は、図9および12に示されるように縦方向および/または円周上でずれた点(A、B)でグラフト部分100における焼灼された開口部80の中を伸展することができる。または、隣接するレール50は、図14に示されるように、円周および/または縦方向に配置された位置Bで焼灼されたグラフト部分100の開口部80の中に伸展することができるであろう。上記の任意の態様において、グラフト部分100は、支持要素22がレール50に沿って移動するにつれて支持要素22と共に移動するであろう。
図16および17に示す態様において、レール50は、以降「ループ200」と呼ばれる円周方向に伸展する保持連結部材200の中を通過する。図9に示す連結部材60とは異なり、ループ200は、グラフト部分100における開口部195の中を通過して、グラフト部分100の内表面に沿って伸展する内部領域202を有する。開口部195は、他の既知の技術を用いてループ200に溶接、焼灼、またはそうでなければ閉鎖することができる。一つの態様において、ループ200は、グラフト部分100より強い糸で形成することができる。一つの態様において、ループ200は、PETの80デニールループ糸で形成される。ループ200はまた、グラフト部分100に関して先に考察した任意の材料で形成することができる。ループ200はまた、固体ポリマー線維、編組、被膜等から形成することができる。同様に、ループ200をグラフト部分100に結合またはそうでなければ固定することも可能である。
グラフト部分100の外側の上で内部領域202間内にループ200を配置すると、グラフト部分100の外表面に沿ってアーチ210が形成される。アーチ210は、グラフト部分100がレール50に沿って支持要素22に対して移動することができるように、レール50をスライド可能に受容する。丸い形状のアーチ210が記述されているが、レール50をスライド可能に受容する任意の形状の開口部を用いることができる。例えば、アーチ210の開口部には、直角、楕円、または三角形が含まれうる。アーチ210にはそれぞれ、レール50を受容する大きさの開口部が含まれる。これらの開口部は約0.0014インチ〜約0.012インチの範囲となりうる。一つの態様において、アーチ開口部は、約0.0014インチ〜約0.006インチの範囲となりうる。一つの態様において、アーチ開口部は約0.005インチとなりうる。
それぞれのアーチ210は、隣接するレール50の円周上の距離に実質的に等しい距離によって、円周上に存在するアーチ210と離れている。隣接するアーチ210は、グラフト部分100の円周の周囲に互いに等しい間隔で存在しうる。または、隣接するアーチ210は、ステントグラフト10に対して異なる屈曲能を提供するためにグラフト部分100の円周の周囲に異なる間隔で存在しうる。それぞれのアーチ210は、隣接するアーチ210から約0.10インチ〜約0.30インチの距離離れうる。一つの態様において、隣接するアーチ210は、約0.155インチの距離互いに離れている。
支持要素22は、図9および17に示す菱形の支持部材30を含む。しかし、上記の態様の場合と同様に、その他の既知の形状を用いてもよい。図9〜15に記載した態様と同様に、図17に示す支持要素22には開口部39が含まれ、ループ200に接続していない。支持要素22(図17)は、上記のようにグラフト部分100の長さと実質的に平行である方向にレール50に沿って移動可能である。
ステントグラフト10の長さに沿ったおよびレール50およびグラフト部分100に対する支持要素22の運動は、支持要素200に隣接する縦方向の山34、35の一つまたは双方によって制限されうる。図17に示すように、ループ200のアーチ210は、支持要素22の縦方向の運動に関する停止板として作用しうる。したがって、支持要素22がレール50に沿って移動する全体的な距離は、グラフト部分100の長さに沿ってループ200のあいだに距離を置くことによって制御および制限することができる。一つの態様において、それぞれのループ200は、ステントグラフト10が留置される血管の形状にステントグラフト10を適合させる距離を、支持要素22が移動することができるように、グラフト部分100の長さに沿って隣接するループ200から連結部材60と同じ距離離れうる。隣接するループ200(および60)のあいだの間隔は、各支持要素22がステントグラフト10の長さと平行な方向に伸展する距離より短くなりうる。
他の態様(例えば、図1に示す態様)とは異なり、図17に記載されるそれぞれの支持要素22は、架橋要素によって縦方向に隣接する支持要素22に接続していない。その結果、図17に示すように、支持要素22は、グラフト部分100の長さに沿って互いに対して独立して移動することができる。同様に、上記の態様と同様に、レール50には、多数のターンを有する単一の連続した部材(図17)、グラフト部分100周囲の隣接する部材から円周方向に距離が離れた少なくとも一つのターンを有する異なる複数の部材、またはターンを含まず、隣接するレール50に対して直接の固定された結合がない異なる個々の部材が含まれうる。本明細書において用いられるように、「レール」という用語には、これらの配置のそれぞれが含まれる。
もう一つの代わりの態様において、グラフト部分100には、グラフト部分100を形成するために用いられる材料で形成されるレール50を受容する一体型の空間領域が含まれうる。これらの空間領域は、グラフト部分100の残りに関して増加した厚みを有する。
本発明には、上記のステントグラフト10を用いて、参照として本明細書に組み入れられる、米国特許出願第60/426,366号に記載される物質を含む物質を体内に導入することが含まれる。好ましい態様において、物質は、レール50またはグラフト部分100の一つまたはそれ以上によって運ばれ、既定の期間、体内において放出される。例えば、これらのステントは、治療薬および薬剤を含む一つまたはそれ以上の既知の物質を血管系の一部に接触する部位または既知のように担体から放出された際に接触する部位に輸送することができる。これらの物質には、任意の既知の治療薬、抗血小板剤、抗凝固剤、抗菌剤、抗体謝剤およびタンパク質が含まれうる。これらの物質にはまた、その双方の全文が参照として本明細書に組み入れられる、Hossainyらに交付された米国特許第6,153,252号、Donovanらに交付された米国特許第5,833,651号に開示される物質が含まれうる。これらの物質の局所輸送は、全身投与によって通常得られる場合よりステントによって輸送された場合にその有効局所濃度が非常に高いという点において有利である。
その横断方向に比較的低い弾性係数(すなわち弾性による変形に対して低い力)を有するレール50は、血管内でステントグラフト10の留置後に、物質輸送レール50に血管が接触する際に血管に適用するための上記の物質の一つまたはそれ以上を輸送してもよい。これらの物質は、参照として本明細書に組み入れられる、上記の特許および特許出願に記述されている液浸、噴霧、含浸または他の任意の技術のような既知の技術を用いて適用することができる。物質をレール50に適用することにより、従来のステントの支柱において認められる局所領域での応力が回避される。このようにして、ステントレール50に適用された薬物コーティングを、そうでなければコーティングに適していない材料で形成された支持要素と共に用いてもよい。
本発明の様々な要素は、所望の柔軟性を提供するために互いに組み合わせることができると企図される。例えば、レール50は、一つまたはそれ以上の放射線不透過性材料で形成することができる。さらに、支持要素のデザインは変更することができ、レールの通過を可能にする様々な支持要素デザインを用いることができるであろう。同様に、レール要素の数、形状、組成および空間は、ステントに異なる特性を提供するように変更することができる。さらに、装置は、様々な数および配置の架橋要素を有しうる。任意の個々のステントの特性は、支持要素、レール、および架橋要素の設計、組成、および空間の関数となるであろう。
このように、本発明の基本的な新規特徴をその好ましい態様に応用されたように示し、記述して指摘してきたが、当業者は、記述の装置の形状および詳細、その操作、ならびに説明および記述された方法に様々な省略、置換、および変更を行ってもよく、それらも本明細書において広く開示される本発明の趣旨に含まれると理解されるであろう。

Claims (52)
- 以下を含む、ステントグラフト:
軸の周囲に伸展する伸長したステント部分;
ステント部分と少なくとも部分的に同時に伸展されるグラフト部分;ならびに
ステント部分とグラフト部分の少なくとも一部がその一部に沿ってレール要素に対して自由に移動可能であるように、ステント部分および/またはグラフト部分に移動可能に連結されている、ステントグラフトの長さに沿って伸展する少なくとも一つのレール要素。 - グラフト部分が、軸に沿って距離を置く円周方向伸展連結部材を含み、少なくとも一つのレール要素が、複数の縦方向に距離を置く円周方向伸展連結部材によってグラフト部分にスライド可能に接続されている、請求項1記載のステントグラフト。
- ステント部分が、複数の円周方向支持要素を含む、請求項2記載のステントグラフト。
- 円周方向支持要素が、少なくとも一つのレール要素を受容するための開口部を含む、請求項3記載のステントグラフト。
- 隣接する支持要素が、架橋部材によって共に接続されている、請求項3記載のステントグラフト。
- 各支持要素が、隣接する支持要素に直接接続していない、請求項3記載のステントグラフト。
- 各支持要素が、円周方向伸展連結部材およびグラフト部分に直接接続していない、請求項3記載のステントグラフト。
- グラフト部分が、ステント部分の末端部のあいだを伸展する、請求項1記載のステントグラフト。
- ステント部分が、ePTFEから形成される、請求項1記載のステントグラフト。
- 少なくとも一つのレール要素が、グラフト部分における閉鎖された開口を通って伸展する、請求項2記載のステントグラフト。
- 開口部が、焼灼される、請求項10記載のステントグラフト。
- 連結部材が、グラフト部分に固定される、請求項2記載のステントグラフト。
- 支持要素のそれぞれが山と谷を含み、軸方向に距離を置いた複数の山が少なくとも一つのレール要素を受容するための開口部を含む、請求項3記載のステントグラフト。
- 各支持要素が、その円周方向の長さに沿って分布した菱形の支持部材を含む、請求項3記載のステントグラフト。
- ステント部分が軸の長さに沿って距離を置いた複数の支持要素を含み、各支持要素が複数の伸長した支柱と縦方向に隣接する支柱を接続する湾曲部分を有する複数の円周方向接続支持部材を含む、請求項1記載のステントグラフト。
- 各支持部材が、各支持部材を少なくとも一つのレール要素にスライド可能に連結させるための少なくとも一つのレール受容部材を含む、請求項15記載のステントグラフト。
- 少なくとも一つのレール受容部材が、少なくとも一つのレール要素をスライド可能に受容するための溝を有するアームを含む、請求項16記載のステントグラフト。
- 少なくとも一つのレール受容部材が相対するアーム対を含み、各アームが少なくとも一つのレール要素をスライド可能に受容するための溝を有する、請求項16記載のステントグラフト。
- 少なくとも一つのレール受容部材が、相対するアーム対によって形成されたレール受容溝を含む、請求項16記載のステントグラフト。
- 円周方向伸展連結部材が、グラフト部分の中に伸展する第一の部分と少なくとも一つのレール要素を受容するためのグラフト部分の外表面に沿ってアーチを形成する第二の部分を含む、請求項2記載のステントグラフト。
- 各円周方向伸展連結部材が、グラフト部分内の閉鎖開口部を通って伸展する糸を含む、請求項20記載のステントグラフト。
- 少なくとも一つのレール要素が、グラフト部分および円周方向伸展連結部材における開口部を通って伸展する、請求項2記載のステントグラフト。
- 円周方向伸展連結部材が、少なくとも一つのレール要素の長さに沿って支持要素の縦方向の移動を制限する、請求項3記載のステントグラフト。
- 距離を置いて隣接する円周方向伸展連結部材間の距離が、軸に平行な方向に伸展する各支持要素の距離より短い、請求項3記載のステントグラフト。
- 少なくとも一つのレール要素の少なくとも一部が、軸と実質的に平行に伸展する、請求項1記載のステントグラフト。
- ステントグラフトが以下を含む、哺乳類の体の一部の中に配置するためのステントグラフト:
伸長した軸の周囲に伸展するグラフト部分;
ステントグラフトの長さに沿って互いに距離を置いた複数の支持要素を含むステント部分であって、グラフト部分の少なくとも一部と少なくとも部分的に同時伸展し、その周囲に存在する伸長したステント部分;ならびに
ステント部分とグラフト部分とが少なくとも一つのレール要素に沿って移動可能であるように、およびステント部分がグラフト部分に対して移動可能であるようにステント部分にスライド可能に連結される、ステントグラフトの長さに沿って伸展する少なくとも一つのレール要素。 - 少なくとも一つのレール要素が、グラフト部分にスライド可能に連結している、請求項26記載のステントグラフト。
- ステント部分が、グラフト部分に直接接続していない、請求項26記載のステントグラフト。
- グラフト部分が軸に沿って距離を置いた円周方向伸展連結部材を含み、少なくとも一つのレール要素が縦方向に距離を置いた円周方向伸展連結部材によってグラフト部分にスライド可能に接続される、請求項26記載のステントグラフト。
- 支持要素が、複数の円周方向伸展支持部材を含む、請求項29記載のステントグラフト。
- 円周方向支持要素が、少なくとも一つのレール要素を受容する開口を含む、請求項30記載のステントグラフト。
- 隣接する円周方向支持要素が、架橋部材によって共に接続される、請求項26記載のステントグラフト。
- 各支持要素が、隣接する支持要素に対して直接接続していない、請求項26記載のステントグラフト。
