Description
[000104]本書に記載されるような、および特に高い柔軟性、高い柱強度を有する細長い反転支持カテーテル(トラクタチューブによりその遠位端からの比較的小さな圧縮力下に置いても硬化し得る)を有する反転式機械的血栓除去装置のトラッキングに関する問題の1つの解決策は、反転式機械的血栓除去装置の一部が中間カテーテルの本体内で保護され、一方で中間カテーテル部分の残りの部分がガイドワイヤ上および/またはガイドワイヤに沿って前進されるように、反転式機械的血栓除去装置を中間カテーテル内に事前装填することである。例えば、図3Aは、蛇行した解剖学的構造を通して送達されるように構成された、血管から血餅を除去するための、事前装填された反転トラクタ式機械的血栓除去装置の一例を示す。この例では、装置は、遠位端305を有する中間カテーテル303を具え得る。中間カテーテル(IC)は、反転式機械的血栓除去装置の一部と見なすことができるが、他の変形例では、反転式機械的血栓除去装置と共に使用される別個の構成要素と見なすことができる。装置はまた、中間カテーテルの管腔内に細長い反転支持カテーテル307(図3ではプッシュカテーテル「880デバイス」とも呼ばれる)を具える。この細長い反転支持カテーテル307は、遠位端311および遠位端開口部を有する。装置はまた、細長い反転支持カテーテル内を遠位方向に延びるプラー319と、プラーの遠位端領域から近位方向に延びるフレキシブルトラクタチューブ315とを具える。プラーは、中間カテーテルの遠位端および細長い反転支持カテーテルの遠位端開口部から延在している。
[000140]本書に記載の機械的血栓除去装置のいずれも、大きな血餅を除去するように適合され得る。一般に、大きな血餅は、直径(外径)と長さのいずれかまたは両方が大きく、機械的血栓除去装置と比べて大きい場合もある。例えば、血餅は、装置の直径よりも大きい(例えば、血餅を捕捉するフレキシブルチューブの拡張した直径よりも大きい)直径を有する場合がある。したがって、装置は、血餅を捕捉し圧縮して血管の内腔から中間カテーテルおよび/または反転支持カテーテルの管腔へと引き込めるように構成することができる。装置は、フレキシブルチューブが保持する能力よりも長い血餅を捕捉して除去するように構成することもできる。
[000161]本書に記載の反転式チューブ装置のいずれも、反転式チューブ装置を中間カテーテル内に引っ込める(例えば、再被覆する)ことができるように構成することができる。特に、反転フレキシブルチューブの一端(例えば、遠位端)にカフを具える反転チューブ装置のいずれかは、血餅の全部または一部を捕捉した後を含む展開後に、中間カテーテルに再被覆されるようにしてもよい。図27A~27Cは、フレキシブルチューブの一端にカフを有する反転式チューブ装置の動作を示す。図27Aに示すように、反転式チューブ装置は、反転支持カテーテル2707、反転支持カテーテルの管腔内のプラー2705、および第1の(例えば、近位)端部でプラーに連結され反転支持カテーテルのオープンな遠位端部上で巻回して反転するように構成されたフレキシブルチューブ2709を具える。いくつかの例では、フレキシブルチューブ2709は、織られるか編まれた材料であり得る。フレキシブルチューブ2709の第2の(例えば、遠位)端部はカフ2711を具え、これは当該カフに隣接するフレキシブルチューブの領域2715によりも可撓性が低くなっている。図27Aでは、装置は、展開構成または部分的に展開された構成で示され、フレキシブルチューブ2709および反転支持カテーテルが、シースとも呼ばれる中間カテーテル2713の遠位端から突出している。上述のように、装置は血餅に送達され、中間支持カテーテルの外へと展開され、ここでガイドワイヤ(図示せず)が用いられてもよい。
Claims (18)
- 血管から血餅を除去するための反転式チューブ装置であって、
反転支持カテーテルと、
前記反転支持カテーテルの管腔内のプラーと、
前記反転支持カテーテルの遠位端上に延在するフレキシブルチューブとを具え、前記フレキシブルチューブは、前記プラーの遠位端領域に結合された第1の端部と、隣接するフレキシブルチューブの領域よりも柔軟性が低いカフを有する第2の端部とを有し、前記フレキシブルチューブは、前記プラーを近位方向に引くことによって前記反転支持カテーテル内へと近位方向に引っ張られ、それにより前記フレキシブルチューブが反転支持カテーテルの遠位端上でロールし反転するように構成されており、
前記カフは、その長さに沿って近位から遠位方向に延びる1以上のスリットを有し、
前記フレキシブルチューブがニットを含むことを特徴とする反転式チューブ装置。 - 請求項1に記載の装置において、前記カフは、ポリマー材料を含む、装置。
- 請求項1又は2に記載の装置において、前記カフが前記反転支持カテーテルの遠位端部を越えて延びるのを防ぐように構成されたカフ保持具をさらに具える、装置。
- 請求項3に記載の装置において、前記カフ保持具は、前記カフを前記反転支持カテーテルに結合する1以上のフィラメントを含む、装置。

- 請求項3に記載の装置において、前記カフ保持具は、前記反転支持カテーテルの横方向開口部を通して、前記カフを前記プラーに接続する1以上のフィラメントを含む、装置。
- 請求項3に記載の装置において、前記カフの近位に面する端部が先細である、装置。
- 請求項3に記載の装置において、前記カフの近位に面する端部は、少なくとも部分的に前記カフの周囲に延在する薄型足場を含む、装置。
- 請求項1から8のいずれかに記載の装置において、中間カテーテルであって、当該中間カテーテルの内部の管腔内に前記反転支持カテーテルが配置された中間カテーテルと、前記プラーを通して真空を適用するために真空源と連結するように構成された第1の近位ポートと、をさらに具え、
前記中間カテーテルと前記反転支持カテーテルとの間に真空を適用するために真空源と連結するように構成された第2の近位ポートをさらに具える、装置。 - 請求項1から9のいずれかに記載の装置において、中間カテーテルであって、当該中間カテーテルの内部の管腔内に前記反転支持カテーテルが配置された中間カテーテルをさらに具え、前記中間カテーテルは、当該中間カテーテルが前記カフを越えて遠位方向に進められるときに、前記反転支持カテーテルの遠位端部を越えて前記カフを押すように構成される、装置。
- 請求項1から10のいずれかに記載の装置において、前記カフが放射線不透過性である、装置。
- 血管から血餅を除去するための反転式チューブ装置であって、
反転支持カテーテルと、
前記反転支持カテーテルの管腔内のプラーと、
前記反転支持カテーテルの遠位端上に延在するフレキシブルチューブとを具え、前記フレキシブルチューブは、前記プラーの遠位端領域に結合された第1の端部と、隣接するフレキシブルチューブの領域よりも柔軟性が低いカフを有する第2の端部とを有し、前記フレキシブルチューブは、前記プラーを近位方向に引くことによって前記反転支持カテーテル内へと近位方向に引っ張られ、それにより前記フレキシブルチューブが反転支持カテーテルの遠位端上でロールし反転するように構成されており、
前記カフは、その長さに沿って近位から遠位方向に延びる1以上のスリットを有し、
前記反転式チューブ装置が、さらに、
