Description
1)バルーンは、血管壁とバルーンとが接触するような直径にまで膨らませることができ、これによりバルーンを通過する血流を部分的又は完全に遮断する。これにより、血栓除去術において血栓の一部又は全部がカテーテル内に回収される確率を低くする助けとなり得る。また、これにより、カテーテルを通じた吸引が促され、この吸引によって遠位の血管内に逆方向の流れが生じることを確実にする。
2)バルーンの膨張は、血管内でのカテーテルの安定化を助けることができ、想定外のカテーテルの動き及びその結果生じる血管の損傷が防止される。
3)本発明のバルーンの膨張はカテーテル先端部の形状をも変化させ、これによりカテーテル先端部とその中に引き込まれるあらゆる物質又は装置との間の界面の幾何形状が最適化される。詳細には、このような形状の変化は、血栓物質がカテーテル内に引き込まれる際に血栓物質に作用する剪断力を低減し、血栓が引っかかる可能性のある「デッドスペース」又はポケットを最小限に抑えるか又は排除する。
血栓捕捉カテーテルを提供することであって、カテーテルの最遠位先端部に膨張可能な部材を有する、血栓捕捉カテーテルを提供することと、
膨張可能な部材が折り畳まれた形態にある状態で、血栓捕捉カテーテルを血栓に向かって進めることと、
膨張可能な部材がカテーテルの最遠位先端部から血管壁にまで外側に延びるようにカテーテルの遠位先端部の膨張可能な部材を膨張させることと、血栓をカテーテル内に引き込むことと、
膨張可能な部材を収縮させることと、
カテーテルを引き抜くことと、を含む、手技を提供する。
カテーテルを、脈管構造を通じて血管内の標的血栓の近位側の位置にまで進めることと、
マイクロカテーテルを血栓に横断させることと、
マイクロカテーテルを通じて血栓捕捉装置を血栓の部位にまで進めることと、
マイクロカテーテルを引っ込めて血栓の下に少なくとも部分的に血栓捕捉装置を展開することと、
カテーテルの拡張部材を膨張させて血管内の流れを遅くするか又は停止することと、血栓捕捉装置をカテーテルに向かって近位方向に引っ込めることと、
注射器又はポンプをカテーテルの近位端に接続し、吸引を行って血管内の血流を逆流させることと、
血栓捕捉装置及び捕捉された血栓を吸引しながらカテーテルの開口部内に引っ込めることと、
吸引しながら血栓捕捉装置を、カテーテルを通じてカテーテルの外部へと引き続き引っ込めることと、
吸引を停止し、拡張部材を収縮させて血管への血流を回復させることと、を含む、方法を提供する。
・従来の血管内アクセス法を用いて、バルーンガイドカテーテル703(又は同様のガイド若しくはシース)を、脈管構造を通じて標的血栓の近位側の位置にまで進め、
・通常はガイドワイヤの助けにより、マイクロカテーテルを血栓に横断させ、
・このマイクロカテーテルを通じてステントリトリーバを血栓の部位にまで進め、
・マイクロカテーテルを引っ込めて血栓の下の少なくとも一部にステントリトリーバを展開し、
・バルーンガイドカテーテル703のバルーン704を膨張させて血管700内の血流を遅くするか又は停止させ、
・ステントリトリーバをバルーンガイドカテーテルに向かって近位方向に引っ込め、
・注射器又はポンプをバルーンガイドカテーテル703の近位端に接続して吸引を行い、血管700内の血流を逆流させ、
・ステントリトリーバ及び捕捉された血栓を吸引しながらバルーンガイドカテーテル703の開口部705内に引っ込め、
・吸引しながらステントリトリーバを、バルーンガイドカテーテル703を通じて外部へと引き続き引っ込め、
・吸引を停止し、バルーン704を萎ませて血管700への血流を回復させる。
(1) 近位端と、遠位端と、前記遠位端における膨張可能な拡張部材とを有する細長管状シャフトを備えた血栓捕捉カテーテルであって、前記拡張部材は折り畳まれた送達形態から拡張形態へと膨張させることが可能であり、前記拡張形態において前記拡張部材は少なくとも前記シャフトの最遠位先端部にまで延び、前記シャフトの前記最遠位先端部において前記シャフトから径方向外側に延びて開口部を画定する、血栓捕捉カテーテル。
(2) 前記拡張形態において、前記拡張部材は前記シャフトの前記最遠位先端部を超えて遠位方向に延びる、実施態様1に記載の血栓捕捉カテーテル。
(3) 前記拡張部材がバルーンを含む、実施態様1又は2に記載の血栓捕捉カテーテル。
(4) 前記バルーンが、前記拡張形態において、拡大された遠位入口開口部とより細い近位端とを有する漏斗形状である、実施態様3に記載の血栓捕捉カテーテル。
(5) 前記バルーンは前記カテーテルシャフトの前記遠位先端部と一体である、実施態様3又は4に記載の血栓捕捉カテーテル。
(7) 前記バルーンが互いに異なる性質を有する複数の領域を有する、実施態様3から6のいずれかに記載の血栓捕捉カテーテル。
(8) 前記バルーンは、近位領域と、遠位領域と、前記近位領域と前記遠位領域との間の中間領域とを有し、前記拡張形態において前記遠位領域は前記近位領域よりも大きな程度まで拡張する、実施態様7に記載の血栓捕捉カテーテル。
(9) 少なくとも1つの領域が少なくとも1つの他の領域と異なる壁厚を有する、実施態様7又は8に記載の血栓捕捉カテーテル。
(10) 前記近位及び遠位領域の壁厚が前記中間領域の壁厚よりも大きい、実施態様9に記載の血栓捕捉カテーテル。

血栓捕捉カテーテルを提供することであって、該カテーテルの最遠位先端部に膨張可能な部材を有する、血栓捕捉カテーテルを提供することと、
前記膨張可能な部材が折り畳まれた形態にある状態で、前記血栓捕捉カテーテルを血栓に向かって進めることと、
前記膨張可能な部材が前記カテーテルの前記最遠位先端部から血管壁にまで外側に延びるように前記カテーテルの前記遠位先端部の前記膨張可能な部材を膨張させることと、
前記血栓を前記カテーテル内に引き込むことと、
前記膨張可能な部材を収縮させることと、
前記カテーテルを引き抜くことと、を含む、手技。
(72) 前記血栓にステントリトリーバなどの機械的装置を係合させて前記血栓を前記カテーテル内に引き込むことを含む、実施態様70又は71に記載の血栓捕捉手技。
(73) マイクロカテーテルを提供することと、前記血栓捕捉カテーテルを通じて前記マイクロカテーテルを進めることと、前記カテーテルから前記血栓係合装置を展開することと、を含む、実施態様72に記載の血栓捕捉手技。
(74) 血栓を捕捉するための方法であって、
実施態様1から67のいずれかに記載のカテーテルを、脈管構造を通じて血管内の標的血栓の近位側の位置にまで進めることと、
マイクロカテーテルを前記血栓に横断させることと、
前記マイクロカテーテルを通じて血栓捕捉装置を前記血栓の部位にまで進めることと、
前記マイクロカテーテルを引っ込めて前記血栓の下に少なくとも部分的に前記血栓捕捉装置を展開することと、
前記カテーテルの前記拡張部材を膨張させて前記血管内の流れを遅くするか又は停止することと、
前記血栓捕捉装置を前記カテーテルに向かって近位方向に引っ込めることと、
注射器又はポンプを前記カテーテルの近位端に接続し、吸引を行って前記血管内の血流を逆流させることと、
前記血栓捕捉装置及び捕捉された血栓を吸引しながら前記カテーテルの前記開口部内に引っ込めることと、
吸引しながら前記血栓捕捉装置を、前記カテーテルを通じて前記カテーテルの外部へと引き続き引っ込めることと、
吸引を停止し、前記拡張部材を収縮させて前記血管への血流を回復させることと、を含む、方法。
Claims (19)
- 近位端と、遠位端と、前記遠位端における膨張可能な拡張部材とを有する細長管状シャフトを備えた血栓捕捉カテーテルであって、前記拡張部材は折り畳まれた送達形態から拡張形態へと膨張させることが可能であり、前記拡張形態において前記拡張部材は少なくとも前記細長管状シャフトの最遠位先端部にまで延び、前記細長管状シャフトの前記最遠位先端部において前記細長管状シャフトから径方向外側に延びて開口部を画定し、
前記拡張部材の拡張によって前記細長管状シャフトの前記最遠位先端部に張力がかかり、前記細長管状シャフトの前記最遠位先端部が漏斗形状になる、血栓捕捉カテーテル。 - 前記拡張部材がバルーンを含む、請求項1に記載の血栓捕捉カテーテル。
- 前記バルーンは前記細長管状シャフトの前記最遠位先端部と一体である、請求項2に記載の血栓捕捉カテーテル。
- 前記拡張部材は、前記バルーンと前記細長管状シャフトとの遠位接合部が前記バルーンの内部に位置するように反転されたポリマーチューブで形成されている、請求項3に記載の血栓捕捉カテーテル。
- 前記バルーンが互いに異なる性質を有する複数の領域を有する、請求項2から4のいずれか一項に記載の血栓捕捉カテーテル。
- 前記細長管状シャフトは、主内側管腔と、前記拡張部材を膨張させるための膨張管腔とを有する、請求項1から5のいずれか一項に記載の血栓捕捉カテーテル。
- 前記膨張管腔は、前記細長管状シャフトの壁の内部に延びる、請求項6に記載の血栓捕捉カテーテル。
- 前記膨張管腔と前記主内側管腔とが偏心配置にある、請求項6又は7に記載の血栓捕捉カテーテル。
- 前記膨張管腔と前記主内側管腔とが同心配置にある、請求項6又は7に記載の血栓捕捉カテーテル。
- 前記膨張可能な拡張部材が非晶質エラストマー性ポリマーを含む、請求項1から9のいずれか一項に記載の血栓捕捉カテーテル。
- 前記非晶質エラストマー性ポリマーが熱可塑性ポリウレタンエラストマーである、請求項10に記載の血栓捕捉カテーテル。
- 前記細長管状シャフトの一部が前記膨張可能な拡張部材を含む、請求項1から11のいずれか一項に記載の血栓捕捉カテーテル。
- 前記細長管状シャフトの遠位領域が非晶質エラストマー性ポリマーを含む、請求項11又は12に記載の血栓捕捉カテーテル。
- 前記細長管状シャフトの前記遠位領域の前記非晶質エラストマー性ポリマーは熱可塑性ポリウレタンエラストマーである、請求項13に記載の血栓捕捉カテーテル。
- 前記細長管状シャフトの遠位領域は第1の非晶質エラストマー性ポリマーを含み、前記膨張可能な拡張部材は、前記第1の非晶質エラストマー性ポリマーとは異なる第2の非晶質エラストマー性ポリマーを含む、請求項11から14のいずれか一項に記載の血栓捕捉カテーテル。
- 前記拡張部材が近位接合領域において前記細長管状シャフトに接合され、前記細長管状シャフトは、外側スリーブ、中間層、内側ライナ、及び補強編組又はコイルを含む複数の層を有する、請求項1から15のいずれか一項に記載の血栓捕捉カテーテル。
- 近位端と、遠位端と、前記遠位端における膨張可能な拡張部材とを有する細長管状シャフトを備えた血栓捕捉カテーテルであって、前記拡張部材は折り畳まれた送達形態から拡張形態へと膨張させることが可能であり、前記拡張形態において前記拡張部材は少なくとも前記細長管状シャフトの最遠位先端部にまで延び、前記細長管状シャフトの前記最遠位先端部において前記細長管状シャフトから径方向外側に延びて開口部を画定し、
前記細長管状シャフトは、外側スリーブ、中間層、内側ライナ、及び補強編組又はコイルを含む複数の層を有し、
前記拡張部材の近位端は近位接合領域において前記細長管状シャフトに接合され、前記拡張部材の他端は、前記拡張部材を形成するほぼ管状のポリマー部材の中央部分が前記細長管状シャフトの前記遠位端に配置されるように反転されて接合部において前記中間層に接合され、前記ほぼ管状のポリマー部材の、前記接合部と前記中央部分との間に位置する部分は、前記細長管状シャフトの前記最遠位先端部に接合されている、血栓捕捉カテーテル。 - 請求項1から17のいずれか一項に記載の血栓捕捉カテーテル、血栓係合装置、及び該血栓係合装置用のマイクロカテーテルを含む、キット。
- 前記血栓係合装置はステントリトリーバ装置である、請求項18に記載のキット。
