本明細書に記載の機械的血栓除去システムおよび装置をより良く理解することを目的として、それらの使用方法を説明する。1つのそのような方法において、機械的血栓除去装置を血管内で遠位に進め、トラクタを用いて、装置の前に遠位に延在させ、場合によっては(例えば、血管壁に対して)引っ張って、カテーテルを遠位に前へ誘導または引き込むことにより、遠位に位置する凝血塊を捕えて、吸い込むことができる。この進める方法を、血管内の「インチワーム」または「ワーム状」の動きと称してもよい。例えば、図2A〜2Eは、機械的血栓除去装置を進める、および/または、機械的血栓除去装置を用いて凝血塊を除去する、第1の方法を示す。この例において、装置は、近位から遠位に延在するカテーテル205と、カテーテル内に遠位に延在するプーラー207(細い管または皮下管として示されるが、上述のとおり、ワイヤまたは棒でもよい)と、可撓管状トラクタ209とを含む。トラクタがカテーテルの外側(外径)の上で近位に延在するよう、トラクタはプーラー217の遠位端領域に結合され、トラクタは、カテーテル219の遠位端開口部上で反転される。作動中、図2B〜2Dに示すように、装置を、血管211内に遠位に(例えば、凝血塊213に向かって)進めることができる。図2Bにおいて、トラクタがカテーテル218の遠位端開口部を越えて、プーラーから遠位に延在して、トラクタとカテーテルの遠位端開口部との間にギャップ219を形成するように、プーラーを遠位に押し出し(220)(左側に矢印で示す)、カテーテル内および血管内で凝血塊に向かって進める。このギャップは、トラクタで形成された環状、遠位面のキャビティ(例えば、袋)で、図2Cに示すように、カテーテルの遠位端開口部をそこに進めることができる。図2Cにおいて、カテーテルが、プーラー上、およびギャップ内に遠位に進められる。このステップの結果、トラクタがカテーテルの更に外側に延在し、血管211の壁と接触するが、必ずしもそのようにする必要はない。膨張したトラクタの外径は、血管の内径(ID)より狭くてもよく、または、血管のIDよりも大きくてもよい(そのため、それらに接触していてもよい)。
Claims (6)
- 血管から凝血塊を除去する機械的血栓除去装置であって、
遠位端および遠位端開口部を有する可撓性カテーテルと、
前記カテーテル内に延在し、前記カテーテルの遠位端上で反転し、前記カテーテルの外径に沿って延在する可撓性ベルトを有するトラクタと、
前記カテーテルの遠位端開口部上で反転するように、前記カテーテルの周りで前記トラクタを駆動するように構成された、前記可撓性カテーテルの近位端の動力駆動部と、
ガイドワイヤを通すように構成された、前記カテーテルおよびトラクタを通るガイドワイヤ管腔とを具え、
前記動力駆動部は、前記カテーテルおよびトラクタを取り囲む環状リングを具えることを特徴とする、機械的血栓除去装置。 - 血管から凝血塊を除去する機械的血栓除去装置であって、
遠位端および遠位端開口部を有する可撓性カテーテルと、
前記カテーテル内に延在し、前記カテーテルの遠位端上で反転し、前記カテーテルの外径に沿って延在する可撓性ベルトを有するトラクタと、
前記カテーテルの遠位端開口部上で反転するように、前記カテーテルの周りで前記トラクタを駆動するように構成された、前記可撓性カテーテルの近位端の動力駆動部と、
ガイドワイヤを通すように構成された、前記カテーテルおよびトラクタを通るガイドワイヤ管腔とを具え、
前記動力駆動部は、前記カテーテルの外面で前記可撓性ベルトと係合するように構成されていることを特徴とする、機械的血栓除去装置。 - 請求項1に記載の装置において、前記可撓性ベルトが、可撓管を具えることを特徴とする、装置。
- 請求項1または2に記載の装置において、前記トラクタが、複数の可撓性ベルトを具え、前記複数の可撓性ベルトが、それぞれ前記カテーテル内に延在し、前記カテーテルの遠位端開口部上で反転し、前記カテーテルの外径に沿って延在することを特徴とする、装置。
- 請求項2に記載の装置において、前記動力駆動部は、前記カテーテルおよびトラクタを取り囲む環状リングを具えることを特徴とする、装置。
- 請求項1または2に記載の装置において、前記可撓性カテーテルおよびトラクタを取り囲むように構成された外側カテーテルを更に具え、前記可撓性カテーテルおよびトラクタは、前記外側カテーテル内にある状態で体内に挿入され得ることを特徴とする、装置。
