Description
[0005] 止血性および血栓性の疾病において、トロンビンは極めて重要な役割を果たす。静脈の血栓性の疾病において、このことはかなり以前から認識されており(1)、静脈性血栓症の予防および治療の効果が、トロンビンの活性を減少させることによって、あるいは直接的な阻害(ヒルジン、メラガストラン)によって、または合成の減少(ビタミンKアンタゴニスト)によって、または減衰の増加(ヘパリン)によって最も得られるという事実から確証的に実証されている。ここ10年間において、トロンビンは、静脈の疾病と同様、動脈の疾病においても重要であることがますます明らかになってきた。臨床試験は、ビタミンKアンタゴニスト(2)およびヘパリン(3)が、心筋梗塞の再発率を減少させることを示している。出血におけるトロンビンの役割は、トロンビンの生成が、経口抗凝血剤(4)またはヘパリン(5)の激しい過剰投与において大きく影響を受けたときに観察される延長された出血時間によって示される。また、血友病は、トロンビン形成系の疾病である(6)。
[0006] 現代の研究は、トロンビンが血液および血漿の決まった成分の協力を通して形成されるという認識に基づいている。赤血球細胞(RBC)は、これらの数パーセントにおいて、その外膜が凝血促進活性を示すけれども、上述した血液成分の協力関係の中では最も活性が低い(7)。さらに重要なことは、白血球細胞が組織因子の活性を担うということである。この活性は通常暗号化されるが、病変の中で血小板との相互作用によって明白になる(8, 9)。主要な因子は、疑いなく血小板および血漿凝固系である。教科書では、血小板は、主要な止血および動脈血栓症の原因である一方、血漿の凝固が止血栓の硬化に役立ち、かつこれが静脈血栓症後のメカニズムであることが記されている。この視点は、血漿と血小板が互いに別々に研究されてきたという事実に起因している。実際には、血小板と血漿との協力および血液の他の細胞が、一次的および二次的止血の両方において、ならびに動脈および静脈血栓症において不可欠である。血小板による栓の形成は、トロンビン生成の役割を果たす。つまり、血小板凝集塊における隙間が不動のニッシェ(unstirred niche)を形成するので、この中において流れる血液により押し流されることなくトロンビンを形成することができるのである。これが、凝固全血におけるトロンビンの生成を測定する理由であり、生理学的現実に非常に近似しているのである。
[0009] 増加したTGは、それがアンチトロンビンの欠乏に起因しようと、あるいはプロトロンビンの過多に起因しようと、常に血栓症の危険性を示す。また、タンパク質C経路が障害されると(タンパク質SおよびC、因子VLeidenの欠乏)、トロンビンの生成は通常よりも多くなる。これは、血漿の凝固を維持するが、タンパク質C経路がトロンボモジュリンによって活性化される場合(図1)に特に明らかになる。経口避妊薬によって誘導されたトロンビンの傾向は、TMまたはAPCが添加されるときにより明らかになるトロンビンの生成の10%増加を引き起こす活性タンパク質Cに対する獲得した耐性によるものとすることができる(18, 19)。
[0014] 本発明によって解決されるべき問題に取り組む場合、以下の知見をトロンビン生成のメカニズムに関して考慮するべきである。
[0017] 止血および血栓症の系の機能を測定する必要性は、前世紀を過ぎても医療従事者の関心を逃していない。この問題の解決策は1990年代まで本質的には変わっておらず、実用的ではあるが不適当な手段、あるいは適当ではあるが非実用的な手段が提供されてきた。
[0019] 1950年代以降、H&T機能を評価するために血液を凝固させるトロンビンの時間的経過を測定することが最良であること認識されてきた(35-37)。 1992年まで、TGを測定する唯一の方法は、凝固している血液または血漿からサンプルを取り出し、その中のトロンビンの含量を決定することであった。これは、1曲線当り1人・時を要し、従って、研究目的には適しているかもしれないが、現代の臨床および疫学の使用には適しているとはいえない。
[0020] 1990年のHemker and Beguin et al. (EP-B1- 0 420 332)は、トロンビンについて高い特異性を有するが、低いターンオーバー速度(低Kcat)およびトロンビンについての弱い結合親和性(高Km)を有する凝固する血液に色素産生(色を生ずる)物質を加える手法に着手した。このような物質は、TGの全プロセス中に存在し、経時にわたるトロンビン活性の合計(すなわち、積分)は、形成された生成物の全量から測定することができる。理想的には、これは内因性トロンビンポテンシャル(ETP)、すなわち、トロンビン生成下の領域(AUC)を測定する。
[0024] 色素産生方法は、血液が半透明ではないために、全血で使用することができないことは明らかである。蛍光基質は、全血中でTGを測定するのに適用可能であることが報告されている(44)。現状において、この公開された方法は、確立した副標本をとる方法で知られるように、少なからず、トロンビン生成の経過に類似しない誤ったシグナルを産生する(図2)。また、得られたシグナルと存在するトロンビンの量との間の定量的関係が実験毎に変化する。要するに、記載された方法は、再現可能かつ定量可能な結果を提供していない。

化学物質

光学デバイスで光を集める
[00105] この実験のために、例1におけるのと同様のグリッドおよびカバーを有する一つのウェルを、Fluostar 最適蛍光計(BMG Labtech, オッフェンバッハ、ドイツ) を用いて測定した。サンプルからの光をホイヘンス接眼鏡(10×の倍率)に集め、この光学デバイスを通過後に読み取る。その結果は図8において示す。
トロンビンに対する任意単位
[00106] この試験を、主に、例えば、メッシュおよびカバーを有する例1では、既知の量(10 nM)のAMCを、基質Z-Gly-Gly-Arg-AMCに加えることによって行った。赤血球の沈降のために、存在するAMCからのシグナルが増加するので、測定が行われる体積は増加する。凝固の瞬間、「凍結」状態に至り、さらなる沈降は起こらない。その瞬間、シグナルの急激な増加から、我々は、加えた10 nMのAMCのために蛍光の量を測定する。この方法において、我々は、蛍光の単位(F)をAMCの濃度に転換する方法を知る。従って、測定されたdF/dtを、d[AMC]/dtに転換することができる。独立した試験から、我々は、d[AMC]/dtのいかなる数値がトロンビン濃度のいかなる数値と対応するを知る。従って、蛍光の変化の速度(図9、黒線)を、サンプル中のトロンビンの濃度に変換することができる。
血液サンプルを含むデバイス
基質およびCa2+イオンを含むフィルター紙セルを、50μLの100mM DMSO溶液の蛍光原基質および100μLの1M CaCl2溶液を5850μLエタノールに加えることによって調製する。11μLのこの溶液を、7×9mmの一片の固体マトリックス(Whatman 1MM クロマトグラフィー紙)上に広げ、窒素下で乾燥させる。次に、図11に示したように、プラスチック片(QPCRのサーモスプリント オプティカル クリーン シーリング テープ(Bilatec AG, マンハイム、ドイツ))で覆った。
2つのフィルター紙セル、蛍光原基質およびカルシウムイオンを含むもの(A)および基質のみを含むもの(B)を、上述したように新たに調製した。

Claims (33)
- 生物学的サンプル中のトロンビン活性をインビトロで決定する方法であって、前記サンプルが全血または血漿サンプルであり、かつトロンビンの生成が以下の工程によって測定される方法:
- 前記サンプルの層をトロンビンの蛍光原基質と接触させる工程であって、前記層は0.05〜5 mmの範囲内の厚みを有し、10〜500 mm2の範囲内の表面積を有し、前記サンプルは前記サンプルの分散を助ける手段を含む容器内に充填されている工程と;
- トロンビンを前記サンプル中において生成させる工程と;
- 前記蛍光原基質上で生成されたトロンビンの酵素作用の結果として蛍光原基質から放出された蛍光基によって、前記層の表面から放射される蛍光を測定する工程。 - 前記アッセイの間に生成されたトロンビンの濃度が、放出された蛍光基による測定された蛍光の関数として決定される、請求項1に記載の方法。
- 前記全血サンプルが、最大10倍の範囲内で稀釈される請求項1または2に記載の方法。
- 前記サンプルの層の厚みが2mm以下である請求項1〜3のいずれか一項に記載の方法。
- 前記アッセイのために、前記血液サンプルが、50〜500μmのメッシュサイズを有するグリッドを含むプレートのウェル内に充填される請求項1〜4のいずれか一項に記載の方法。
- 前記アッセイのために、前記血液サンプルが、マイクロビーズを含むプレートのウェル内に充填される請求項1〜5のいずれか一項に記載の方法。
- 前記血液サンプルを含むウェルが、トロンビン活性の決定のために覆われる請求項1〜6のいずれか一項に記載の方法。
- フルオロフォアが、トロンビンの蛍光原基質に加えられる請求項1〜7のいずれか一項に記載の方法(加えられたフルオロフォアの割合はトロンビン基質に結合した蛍光分子の量の1〜10%の範囲内にある)。
- 前記サンプルに加えられたトロンビン基質の量が、50〜1000μMの範囲内にある請求項1〜8のいずれか一項に記載の方法。
- 前記蛍光原基質が、蛍光分子とカップリングしたトロンビンについての合成基質である、請求項1〜8のいずれか一項に記載の方法。
- 前記トロンビンの基質が、蛍光分子とカップリングした有機化学物質である請求項10に記載の方法。
- 前記蛍光原基質が、蛍光分子とカップリングした2〜30のアミノ酸残基の配列を有するオリゴペプチドである請求項11に記載の方法。
- 前記オリゴペプチドが、蛍光分子とカップリングするための末端のリシンまたはアルギニンを有する請求項12に記載の方法。
- 前記蛍光分子が、AMC(7-アミノ-4-メチルクマリン)である請求項10〜13のいずれか一項に記載の方法。
- 前記ウェルがさらに、ゲルを含む、請求項1〜14のいずれか一項に記載の方法。
- 前記ゲルが、カルシウムイオンを含む請求項15に記載の方法。
- 組織因子およびカルシウムイオンが、トロンビンの生成を起こすことができる量において前記サンプルに加えられる請求項1〜16のいずれか一項に記載の方法。
- 前記サンプルが、全血のサンプルである請求項1〜16のいずれか一項に記載の方法。
- 前記サンプルが、血漿のサンプルである請求項1〜16のいずれか一項に記載の方法。
- 前記全血のサンプルが、クエン酸付加である請求項18に記載の方法。
- 止血性の疾病または血栓性の疾病を検出またはモニタリングするために使用される請求項1〜20のいずれか一項に記載の方法。
- 全血サンプルにおけるトロンビン活性上で決定された物質の相互作用を検出またはモニタリングするために使用される請求項21に記載の方法(前記決定された物質は、分析対象のサンプルに加えられるかまたはトロンビンの生成の間に加えられる)。
- 凝固因子または薬剤の相互作用をモニタリングするために使用される請求項21に記載の方法。
- 物質の相互作用能をトロンビンの生成で決定する、物質をスクリーニングするために使用される請求項21に記載の方法。
- 全血サンプルの内因性トロンビンポテンシャル(ETP)の測定のために使用される、請求項1〜21のいずれか一項に記載の方法。
- トロンビンのピークまでの時間の測定のために使用される、請求項1〜22のいずれか一項に記載の方法。
- 凝固時間の測定のために使用される、請求項1〜23のいずれか一項に記載の方法。
- 生成されたトロンビンのピークのレベルの測定のために使用される、請求項1〜24のいずれか一項に記載の方法。
- 較正ステップを含む、請求項1〜25のいずれか一項に記載の方法。
- 請求項1〜29のいずれか一項に記載の方法を行うためのキットであって、以下のものを含むキット:
- トロンビンのための蛍光原基質、
- トロンビン生成を可能にする組織因子およびカルシウムイオン、
- 前記容器内におけるサンプルの分散を助ける手段。 - 請求項30に記載のキットであって、前記サンプルの分散を助ける手段が、グリッドまたはマイクロビーズであるキット。
- 請求項30または31に記載のキットであって、さらにゲルを含むキット。
