(発明の背景)
血管の疾患は、出血、血管の閉塞を引き起こすアテローム性動脈硬化症、血管奇形および腫瘍を含む。脳内の血管閉塞または動脈瘤の破裂は、脳卒中を引き起こす。頭蓋内動脈によって供給される動脈瘤は、脳内で、それらの塊およびサイズが、脳卒中または脳卒中の症状を引き起こし、動脈瘤の除去またはその他の治療的介入のために手術を必要とする点まで成長し得る。

冠状動脈の閉塞は、例えば、心臓麻痺の共通の原因である。疾患および妨害された冠状動脈は、心臓中の血液の流れを制限し得、そして組織虚血および壊死を引き起こす。硬化性心臓血管疾患の正確な病因はなお論議されているが、狭窄冠状動脈の処置は、さらに規定されている。冠状動脈バイパス移植片(CABG)の外科的構築は、1つまたは複数の動脈にいくつかの疾患セグメントが存在するとき、しばしば、選択の方法である。従来の開放心臓手術は、勿論、このような処置を受ける患者にとって非常に侵襲的かつ外傷性である。従って、より外傷性でない代替の方法が高度に所望される。
代替方法の1つは、折り畳まれたバルーンが、動脈を閉塞または部分的に閉塞し、そして閉塞された動脈を開放するために膨張される、狭窄中に挿入される技法であるバルーン血管形成術である。別の代替方法は、閉塞性アテロームが動脈の内面から切除される技法であるアテローム切除術である。両者の方法は、特定の%の患者で再閉塞が生じる。
血管閉塞に対して最近好まれる治療は、血管を開いて保持するために、血管の閉塞された領域内の、ステントを含む拡大可能な金属ワイヤフレームの配置である。このステントは、通常はカテーテルである送達手段によって血管系内の所望の位置に送達される。従来の血管手術に対するステント配置法の利点は、心肺バイパス、胸部の開放、および一般麻酔を含む、外科的曝露、除去、または欠陥血管をバイパスする必要性を未然に防ぐことを含む。
バルーン血管形成術による動脈の拡張の後、身体の脈管、ダクトまたは管(「脈管」)、例えば、冠状動脈中に挿入および配備されるとき、ステントは、脈管を開いて維持するための補綴具として作用する。このステントは、通常、その側壁として相互連結するストラットを備えた開放端部の管状形態を有し、このステントが脈管を辿ってそれが配備される部位に到達することを可能にするに十分小さい第1の外径から、この部位で保持のために血管の内側裏打ちと係合するに十分大きい第2の外径までのその拡大を可能にする。ステントは、代表的には、非拡大状態で身体管腔中の所望の位置に送達され、そして次に拡大される。このステントは、バルーンのような機械的デバイスの使用により拡大されるか、またはこのステントは、自己拡大性である。
通常、成功する介入的配置のための適切なステントは、相対的に非アレルギー反応、良好な放射線不透過性、磁気共鳴造影(MRI)に対する歪みのないこと、塑性的に変形可能である適切な弾性を備えた可撓性、血管反跳に対する強い耐性、血液(または心臓血管系以外の脈管中のその他の流体または材料)の流れに対する妨害を最小にするに十分な薄さ、および脈管再閉塞を避けるための生体適合性の特徴を所有すべきである。ステントが構成される材料の選択、およびステントの設計は、これらの特徴に影響する際に重要な役割を演じる。
さらに、移植可能な医療用デバイスが、異なる生物学的適用のために薬物または生体試薬の送達のために利用されている。代表的には、これら薬物または生体試薬は、これら移植可能医療用デバイスの表面に被覆されるか、またはこれら移植可能医療用デバイスの表面上に被覆されるポリマー性材料内に混合される。しかし、すべての現在利用可能な方法は、制御不能な放出、薬物の形態制限、および嵩張る外観を含む1つ以上の問題を被る。
従って、脈管内系、特に、頭蓋内血管に薬物または試薬を送達し得る移植可能な医療用デバイスが所望されている。
(発明の要旨)

1つの例では、頭蓋内ステント202は、低圧展開のために特に設計されている。このステント202は、脆弱な頭蓋内脈管の特有の環境を標的にするために適切な半径方向強度を有する。このステント202は、高いステント性能を送達すること、そして長軸方向の可撓性を絶対的に従わせることを可能にするように設計される。
ステントの低圧力展開は、4気圧(atm)未満に等しい圧力として規定される。このレベルの圧力は、ステント202が、標的脈管の外傷または破裂を導入することなく狭窄した頭蓋内脈管または動脈瘤頸部201を支持するために完全に展開されることを可能にする。このステント202は、バルーン技法または自己拡大性であることを用いて展開され得る。
ステント202は、可能な過剰拡大を制限する構造的要素を備え、脈管の内径に一致し、そして極度に正確な展開を行う。低圧力展開と組合せたこれら構造的要素のこの特徴は、脈管損傷、破裂または再狭窄を潜在的に減少する。
ステント202はまた、送達カテーテルによって提供されるのと等しいか、またはそれより良好な長軸方向の可撓性を有する。これは、このステントが、デバイスに対して増加した剛直性を付加しないことを意味する。ステント202の軌道可能性は、カテーテルの機械的性質に依存し、そしてステント202単独によっては制限されない。ステント202の長軸方向可撓性は、中立線からこのステントを曲げるための力(グラム)によって測定され得る。この力は、1mmまでのステントの曲げを8グラム未満でもたらす。現存するカテーテルは、1mmの曲げあたり20~22グラムを提供し得る。この条件はまた、特定の脈管へのステント順応性を生成するとき極度に重要であり、そして脈管を可能な外傷反力から退避する。

図6を参照して、特定の実施形態では、膜203は、デバイスの両端部で2つのリング様の短いステント302によって支持されるスリーブ301として具現化され、この膜203は、デバイスの全領域を覆う。このデバイスの中央には足場支持はない。放射線不透過性マーカー303は、ステント302の両端部に位置される。適用に依存して、上記リングは、バルーンで拡大可能であり、そしてステンレス鋼またはNiTi(形状記憶ニッケル-チタン合金)から作製されて自己拡大可能である。
膜203は、動脈瘤頸部を効率的に閉塞し、そして脈管を「再開通する」ように設計された出血ステント構造の一部である。それは、再構築脈管を可能にし、そして動脈瘤を本質的になくする。高価な(そして特に外傷性で、しばしば塊状過ぎる)コイル留めが予期される必要性はない。
このデバイスは:巨大および広い頸部の動脈瘤、2分枝および3分岐動脈瘤を処置するための好ましい解決法である。それはまた、海綿静脈洞、偽動脈瘤、小嚢性動脈瘤における破裂したccフィステルのための好ましい処置解決法である。

図7を参照して、2分枝または3分岐動脈瘤201を処置する2分枝ステント202のための膜203が提供される。動脈瘤の少なくとも30~35%が、頭蓋脈管の2分枝部位に位置される。この膜203は、片側性であり、そして周辺を取り囲まない。2分枝ステント202は、膜203によって接続され、動脈瘤頸部201を覆う。同じパターンが、自己拡大性(超弾性)またはバルーン拡大可能(ステンレス鋼、CoCr、PtIr合金)ステント202に適用され得る。

この動脈瘤201への血液供給を妨害することは、この動脈瘤201を通常の血液循環から隔離し、そしてそれによって最終的には、それを固まらせる。この動脈瘤201への完全な妨害は必ずしも必要でなくて良い。

図10を参照して、透過性膜203を作製するために、穴が中実フィルム中に穿孔されポアを形成する。ポアサイズは0.025~0.050mmの間であり、その一方、ポア間の距離は100μmより小さい。

図14を参照して、膜203はストラット上に固定され、そしてステント202から取り外すか、または引裂かれることは困難である。この膜203の厚みは、縮んだアセンブリに任意の有意なプロフィールは付加せず、すなわち、それは、縮んだステントプロフィールに0.0254mm(0.001インチ)未満寄与するに過ぎない。この膜203はまた、均一な収縮可能性を有する。
