Abstract
Description
p53は、異常な増殖シグナル、DNA損傷、紫外線及びプロテインキナーゼ阻害剤等の多くの潜在的な腫瘍形成過程により誘発及び活性化され(Millard M,et al.Curr Pharm Design 2011;17:536-559)、細胞増殖停止、DNA修復、アポトーシス及び血管新生を制御する遺伝子を調節する(Bullock AN&Fersht AR.Nat Rev Cancer 2001;1:68-76;Vogelstein B,et al.Nature Education 2010;3(9):6)。
ベネトクラックスは、抗アポトーシスタンパク質であるB細胞リンパ腫(BCL)-2の強力な選択的阻害剤である。ベネトクラックスは、BCL-2のBH3結合溝に直接結合し、BIMのようなBH3モチーフ含有アポトーシス促進性タンパク質に取って代わり、ミトコンドリア外膜透過性上昇(MOMP)、カスパーゼ活性化及びプログラム細胞死を開始させる。Bcl-2は、とりわけMcl-1及びBcl-xLを含むタンパク質のBcl-2ファミリーの抗アポトーシスタンパク質である。Mcl-1及びBcl-xLのように、Bcl-2は、Bax/Bakなどのアポトーシス促進性エフェクター分子に結合し、それらのオリゴマー化及びミトコンドリア膜上での膜貫通孔の形成を妨害し得る。そのようにすることにより、Bcl-2は、チトクロムCの放出を妨げ、それによりカスパーゼ活性化を阻止し、続いてアポトーシスを阻害する。アポトーシス回避は、癌の特徴の1つであり、従って、この回避の促進因子であるBcl-2などの抗アポトーシスタンパク質は、納得できる治療標的に相当する(Certo et al.2006)。ベネトクラックスは、Bcl-2依存性白血病及びリンパ腫細胞株で活性を示したBcl-2の非常に選択的な経口生体利用可能な小分子阻害剤である(Souers et al.2013)。ベネトクラックスは、両方ともインビトロであるAML/MDS細胞株、原発性AML/MDS対象試料及びマウス異種移植モデルにおいてアポトーシス細胞死を誘導する(Pan et al.2014)。
癌単剤療法は、持続的な有効性の欠如及び/又は安全性の問題により左右されることが多い。相乗効果を示す組み合わせパートナーに基づく組み合わせ癌療法は、実質的な長期有効性の向上及び安全性プロファイルの改善という長所を提供する。このため、抗癌薬の組み合わせについての研究が依然として望まれている。

HDM201処置は、少なくとも3回の28日処置サイクルから構成され、
1回目及び2回目の処置サイクル(即ち誘導サイクル)のためのHDM201の1日薬物用量は、50mg~100mg、好ましくは50mg~80mg、より好ましくは60mg~80mg、さらにより好ましくは60mgであり、且つ3回目及び任意のそれ以降の処置サイクル(即ち地固めサイクル)のための1日のHDM201用量は、10mg~45mg、好ましくは20mg~40mg、より好ましくは30mg~40mg、さらにより好ましくは40mgである、実施形態3~6の何れか1つに記載の癌の処置での使用のための組み合わせ。
8.HDM201は、28日処置サイクルの最初の5日のそれぞれに投与され、
HDM201処置は、少なくとも3回の28日処置サイクルから構成され、
誘導サイクル(サイクル1及び2)の1日のHDM201用量は、60mg~80mgであり、地固めサイクル(サイクル3及びそれ以降)の1日のHDM201用量は、40mgである、実施形態3~6の何れか1つに記載の癌の処置での使用のための組み合わせ。
9.ベネトクラックスは、20mg~1000mg、好ましくは50mg~600mg、より好ましくは300mg~600mg、さらにより好ましくは400mg~600mg、さらにより好ましくは400mg又は600mgの1日用量で投与される、実施形態3~8の何れか1つに記載の癌の処置での使用のための組み合わせ。
10.HDM201は、28日処置サイクルの最初の5日のそれぞれに投与され、HDM201処置は、少なくとも3回の28日処置サイクルから構成され、誘導サイクル(サイクル1及び2)の1日のHDM201用量は、60mg~80mgであり、地固めサイクル(サイクル3及びそれ以降)の1日のHDM201用量は、40mgであり、
ベネトクラックスは、400mg又は600mgの1日用量で投与される、実施形態3~6の何れか1つに記載の癌の処置での使用のための組み合わせ。
11.HDM201は、非共有結合誘導体として存在し、好ましくは、前記非共有結合誘導体は、塩、溶媒和物、水和物、錯体及び共結晶からなる群から選択され、より好ましくは、非共有結合誘導体は、共結晶であり、さらにより好ましくはコハク酸共結晶として、さらにより好ましくは1:1(モル比)コハク酸:HDM201共結晶として存在する、実施形態1~10の何れか1つに記載の組み合わせ又は癌の処置での使用のための組み合わせ。
12.1つ以上の他の抗癌剤をさらに含み、好ましくは、前記抗癌剤は、癌免疫薬(例えば、PD-1[例えば、PDR001(Novartis、INN スパルタリズマブ)]、PD-L1、LAG-3、TIM-3[例えば、MBG453(Novartis)]、GTIR、TGF-ベータ、IL15阻害剤)、FLT3阻害剤(例えば、ギルテリニブ(gilterinib)、キザルチニブ、ミドスタウリン)、BCL2阻害剤(例えば、ナビトクラックス、ベネトクラックス)、他のHDM2阻害剤(例えば、イダサヌトリン、AMG232、DS-3032B、ALRN6924/ATSP7041)、低メチル化剤(HMA)(例えば、Vidaza[アザシチジン、5-アザシチジン]、Dacogen[デシタビン]、グアデシタビン)、アントラサイクリン(例えば、イダルビシン、ダウノルビシン、ドキソルビシン、エピルビシン、ルビドマイシン);抗CD33抗体(例えばマイロターグ[ゲムツズマブ]、バダスツキシマブ)及び他の薬剤(例えば、AraC[シタラビン、アラシチン])から選択される、実施形態1~11の何れか1つに記載の組み合わせ又は癌の処置での使用のための組み合わせ。
13.1つ以上の他の抗癌剤をさらに含み、好ましくは、この抗癌剤は、シタラビン(Ara-C)、アントラサイクリン、ダウノルビシン、イダルビシン、ルビドマイシン、イダマイシン、ミドスタウリン及びアザシチジンから選択される、実施形態1~12の何れか1つに記載の組み合わせ又は癌の処置での使用のための組み合わせ。
以下において且つ本願全体を通してさらなる用語を定義する。
血小板モデル
臨床試験CHDM201X2101の集団PK/PDデータに基づき、疾患が血小板産生の制御に影響を与えることを認識するAML患者血小板モデルを開発した。以下の図画は、このモデルを明らかにする。
実施例1及び2の集団PK/PDモデルを使用して、個体間の変動性とともに経時的にPK、血小板及び芽球プロファイルのシミュレーションを行った。
重要な指標は、以下のとおりであった:経時的な血小板数がある一定の閾値を下回る/上回る患者の割合、PK閾値を上回る患者の割合、芽球値がベースラインを下回る日数。
・長期血小板枯渇、及び
・長期処置(>6カ月)は、用量減量又は中断なく持続可能ではない:
- 処置サイクル増加に伴う血小板数の漸減
- サイクル3又は4を超えた際の芽球における薬物効果を制限する疾患抵抗性。
・累積的な血小板毒性
・地固めでの投与を妨げる造血回復の遅れがこの適応症に対するリスクをもたらす。
誘導の1又は2、好ましくは2サイクル後の用量減量。
実施例2:HDM201及びベネトクラックスの組み合わせのインビボでの薬理学
HDM201は、複数のAML患者由来の同所性モデルにおいてインビボで選択的Bcl-2阻害剤ベネトクラックスの抗腫瘍活性を促進することが示された。突然変異IDH1/FLT3-ITDを有するマウスにおいて、HDM201処置単独が示す抗癌活性は、最小限のものであった(92%T/C、p>0.05)。対照的に、ベネトクラックスと組み合わせたHDM201は、完全な腫瘍退縮を誘導した(-100%Reg)一方、ベネトクラックス単独では部分的な腫瘍退縮のみが認められた(-9~-52%Reg)。図8及び9を参照されたい。
ベネトクラックスと組み合わせたHDM201の用量/レジメン及び処置の持続時間に対する根拠及び設計
これは、AML又は高リスクMDSの対象でのベネトクラックスと組み合わせたHDM201の第1b相試験、多群非盲検試験である。
サイクル1中のベネトクラックスの増量(RU)(処置群2:HDM201+ベネトクラックス)
HDM201及びベネトクラックスを組み合わせるための根拠は、以下の証拠に基づく:
・前臨床のエビデンスは、MDM2及びBcl-2の同時遮断がp53wt AML対象試料において相乗的な活性を有し、異なるAML異種移植マウスモデル(実施例2)において相乗的な抗腫瘍活性を発揮することを示す。
・ベネトクラックスとの組み合わせにおけるMDM2阻害剤イダサヌトリンの早期臨床データは、AML対象において予備的な活性を報告する(Daver N,Pollyea DA,Yee K,et al(2017)Preliminary Results from a Phase Ib Study Evaluating BCL-2 Inhibitor Venetoclax in Combination with MEK Inhibitor Cobimetinib or MDM2 Inhibitor Idasanutlin in Patients with Relapsed or Refractory(R/R)AML.The American Society of Hematology-59th Annual Meeting and Exposition,Atlanta,Georgia)。
本試験は、以下のTP53wt成人患者において行う:
以前のレジメンが≧1回、奏効しなかったR/R AML、又は
標準的誘導化学療法に不適切な初回AML、又は
低メチル化剤療法が奏効しなかった高リスクMDS。
この試験の対象となる組み入れ患者は、以下の基準全てに合致しなければならない:
1.以下の1つを呈する、インフォームドコンセント用紙への署名日に≧18歳の男性又は女性患者:
a.AML患者に対するシタラビン又は他の確立された化学療法レジメンでの再誘導を含む、難治性疾患を再発したか又はそれを呈しており(一次無効)、標準的治療に対する候補ではないと治験責任者によりみなされる、≧1回の以前の治療(しかし、≦3回の以前の治療)後の再発/難治性AML(標準的な再誘導化学療法又は造血幹細胞移植に適切であり、それを受ける意思がある患者は、排除される)。一実施形態では、AMLは、難治性疾患を再発したか又は呈した患者(一次無効)における1回以上の以前の治療後の再発/難治性AMLである。
b.初回AML患者は、標準的な誘導化学療法に不適切である(新規及び二次AMLの両方を含む)。造血性幹細胞移植に適切であり、それを受ける意思がある患者を除外する。別の実施形態では、AMLは、特に標準的な誘導化学療法に適さない患者における初回AMLである(AMLは、新規及び二次AMLの両方を含む)。
c.低メチル化剤療法が奏効しなかった高リスクMDS患者(rIPSSによる高及び超高リスク群)。別の実施形態では、MDSは、高リスクMDS患者(rIPSSによる高及び超高リスク群)、特に低メチル化剤療法が奏効しなかった患者である。2.Eastern Cooperative Oncology Group(ECOG)パフォーマンスステータスが≦1。
3.患者の腫瘍がTP53wtである。TP53遺伝子の少なくともエクソン5、6、7及び8が配列決定され、突然変異を含有しないと判定されなければならない。TP53の状態は、主要なICFに署名する前3カ月以内に骨髄試料から得られ、回収されなければならない。
4.患者は、本研究所の指針に従う連続的な骨髄穿刺及び/又は生検に対する候補であり、スクリーニング時、本研究の治療中、治療終了時に骨髄穿刺及び/又は生検を行う。
この試験に適格な患者は、以下の規準の何れにも合致してはならない:
・同じ作用方式を有する化合物での以前の組み合わせ処置:
- Bcl-2阻害剤と組み合わせたmdm2又はmdm4阻害剤
・試験薬物及び他のモノクローナル抗体(mAb)及び/又はそれらの賦形剤のあらゆる成分に対する重篤な過敏反応の病歴。
・PML-RARAがある急性前骨髄球性白血病の患者。
・過去6カ月以内の、同種幹細胞移植(HSCT)及び/又は全身免疫抑制療法を必要とする活動性GvHD。
・経口HDM201又はベネトクラックスの吸収に影響を及ぼすGI障害。
・活動性出血又は出血素因又は重度の凝固障害(家族性を含む)のエビデンス。
この試験に対して、「治験薬」又は「試験薬物」という用語は、HDM201又はベネトクラックスを指す。「処置群」又は「試験処置」は、具体的な組み合わせ処置、即ちHDM201+ベネトクラックスを指す。この試験で使用される治験薬は、以下のとおりである:
HDM201、10mg、20mg、40mg、経口使用のためのカプセル、20mg(出発用量)、第1日~第5日(28日サイクル)、オープンラベル患者固有;ボトル。
出発用量
用量漸増におけるHDM201に対する出発用量及びレジメン選択は、AML/MDSの対象における単剤としてのHDM201の以前の第I相用量漸増及び拡大試験(CHDM201X2101)に基づき、その試験では、45mg/日の用量(第1~7日/28日サイクル)がRDであることが決定された。この試験において、用量漸増に対する20mg/日HDM201の出発用量及びレジメン(第1~5日/28日サイクル)を選択した。用量及びレジメンの選択は、腫瘍保有ラットの単剤トランスレーショナル前臨床モデリング及び血小板減少症の集団PK/PDモデリング及びAML/MDS対象におけるCHDM201X2101試験からの骨髄芽球データにより裏付けられた。この出発用量は、HDM201単剤RD(45mg/日(第1~7日/28日サイクル)又は315mg/サイクルでCHDM201X2101において評価される場合)の累積用量を約315%下回る量に相当する。この用量レベルで、対象の~15%が、いくつかの見込まれる臨床活性(骨髄芽球減少)及び標的骨髄抑制の制限とともに、1サイクルあたりのHDM201の前臨床由来の平均標的有効濃度を達成すると予想される。さらに、20mg HDM201出発用量のPBPK SimCypモデリングは、最初の増量相中及び定常状態でのベネトクラックス曝露の約1.20倍の上昇を予測した。
以下の表実施例3-1は、HDM201+ベネトクラックスの組み合わせ(1サイクル=28日)中に評価され得るHDM201の出発用量及び投与レジメンを記載する。

Claims (13)
- HDM2-p53相互作用阻害薬(S)-5-(5-クロロ-1-メチル-2-オキソ-1,2-ジヒドロ-ピリジン-3-イル)-6-(4-クロロ-フェニル)-2-(2,4-ジメトキシ-ピリミジン-5-イル)-1-イソプロピル-5,6-ジヒドロ-1H-ピロロ[3,4-d]イミダゾール-4-オン[HDM201]又はその薬学的に許容可能な非共有結合誘導体(塩、溶媒和物、水和物、錯体、共結晶を含む)と、BCL2阻害剤4-(4-{[2-(4-クロロフェニル)-4,4-ジメチルシクロヘキス-1-エン-1-イル]メチル}ピペラジン-1-イル)-N-[(3-ニトロ-4-{[(オキサン-4イル)メチル]アミノ}フェニル)スルホニル]-2-[(1H-ピロロ[2,3-b]ピリジン-5-イル)オキシ]ベンズアミド[ベネトクラックス]又はその薬学的に許容可能な非共有結合誘導体(塩、溶媒和物、水和物、錯体、共結晶を含む)との組み合わせ。
- 癌の処置での使用のための、請求項1に記載の組み合わせ。
- 前記癌は、血液腫瘍である、請求項2に記載の癌の処置での使用のための組み合わせ。
- 前記血液腫瘍は、急性骨髄性白血病(AML)、好ましくは再発/難治性AML又は初回(1L)AML(新規及び二次AMLの両方を含む)である、請求項3に記載の癌の処置での使用のための組み合わせ。
- 前記血液腫瘍は、骨髄異形成症候群(MDS)、好ましくは高リスクMDS(rIPSS(改定国際予後判定システム)による高及び超高リスクMDSを含む)である、請求項3に記載の癌の処置での使用のための組み合わせ。
- 前記癌は、TP53野生型腫瘍である、請求項2~5の何れか一項に記載の癌の処置での使用のための組み合わせ。
