Abstract
Description
図1〜図6は、外科手術を行うために、図1Aに示すような非関節動作状態でトロカールカニューレ通路を通って患者の体内の手術部位まで挿入するように寸法決めがされている、例示的な外科ステープル留め及び切断器具(10)を図示している。外科ステープル留め及び切断の器具(10)は、実施部分(22)へ接続されたハンドル部分(20)を含み、後者は関節動作機構(11)において遠位方向に終端するシャフト(23)と、遠位方向に装着されたエンドエフェクタ(12)と、を更に備える。一旦関節動作機構(11)及びエンドエフェクタ(12)がトロカールカニューレ通路を通って挿入されると、関節動作機構(11)は、関節動作制御部(13)によって図1Bに示すように、遠隔で関節動作されてよい。そのようにして、エンドエフェクタ(12)は、所望の角度から又は他の理由のために、臓器の背後に到達する又は組織に近づくことができる。本明細書では、「近位」及び「遠位」といった用語は、器具(10)のハンドル部分(20)を握っている臨床医を基準として使用されていることを理解されたい。したがって、エンドエフェクタ(12)は、より近位のハンドル部分(20)に対して遠位である。便宜上、また説明を明確にするため、本願では「垂直」及び「水平」といった空間的な用語を図面に対して使用する点も更に認識されるであろう。しかしながら、手術器具は、多くの配向き及び位置において使用され、これらの用語は、制限的及び絶対的であることが意図されない。
後述の実施例は、発射バーが組織を切断する間及びステープルがカートリッジから組織内へ駆動される間、添加剤材料を組織上に載置するように動作可能である例示の発射バー及びカートリッジを含む。添加剤材料は、例えば、添加剤又は手術部位において血液を凝固させかつ出血量を低減させるのに支援するフィブリン(fibrin)又はトロンビン(thrombin)などの止血薬を含んでよい。かかる添加剤の止血能力はまた、接着剤及びシーラントなどの添加剤の利用にも寄与し得る。これらの薬剤は、例えば、ステープル留めされた組織部位に沿って実質的に漏れを防止できる。かかる添加剤又は試薬は、血小板乏血漿(PPP)、血小板浮遊血漿(PPP)、でん粉、キトサン、アルギネート、フィブリン、多糖類、セルロース、コラーゲン、ウシコラーゲン、ゼラチン−レゾルシン−ホルマリン接着剤、酸化セルロース、ムラサキイガイ系接着剤、ポリ(アミノ酸)、アガロース、アミロース、ヒアルロナン、ポリヒドロキシ酪酸(PHB)、ヒアルロン酸、ポリビニルピロリドン(PVP)、ポリ(ビニルアルコール)(PVA)、ポリラクチド(PLA)、ポリグリコリド(PGA)、ポリカプロラクトン(PCL)、及びそれらのコポリマー、VICRYL(登録商標)(Ethicon,Inc.,Somerville,N.J.),MONOCRYL材料、PANACRYL(Ethicon,Inc.,Somerville,N.J.)などの薬液又はバットレス構成成分、及び/又は各材料の組み合わせを含む、生物学的材料と混合されかつ創傷又は欠陥部位へ導入されるのが好適な任意の他の材料を、更に含んでよいが、これらに限定されない。例えば、材料は次の材料:エプシロン−カプロラクトングリコリド、ウシ囲心嚢、ポリ乳酸、ポリグリコール酸、ポリグラクチン、ポリジオキサノン、ポリグリコネート、乳清蛋白質、セルロースガム、でん粉、ゼラチン、シルク、ナイロン、ポリプロピレン、編組ポリエステル、ポリブテステル、ポリエチレン、及び/又はポリエーテルエーテルケトン類、から選択されてよい。薬液又はバットレスに使用されてもよい他の好適な化合物、材料、物質などは本明細書の教示を鑑みて当業者に明らかであろう。
図7及び8は、前述したカートリッジ(37)と同様の構成要素を含む取り外し可能なカートリッジ(101)を示す。カートリッジ(101)は、テープの形態の材料(114)を含みかつ前述したエンドエフェクタ(12)の下部つかみ具(16)において例示の発射バー(103)の上に配設される。発射バー(103)は、前述した発射バー(14)と同様である。図9は、上部先端部(104)において終端する上面(102)とともに遠位ブレード(100)を含むとした発射バー(103)を示す。発射バー(103)はまた、遠位ブレード(100)から延出する細長い近位部分(106)を含む。細長い近位部分(106)の上縁部(108)は、ノッチ(110)を含む。ノッチ(110)は、材料(114)の第1のスプール(116)と第2のスプール(118)との間に配設されかつ両者を連結する材料(114)の中間部分(112)を受容するように構成される。材料(114)のスプール(116,118)は、材料(114)がスプール(116,118)から早まってほどかれるのを防止するために弾性的に付勢されてよい。
図10及び図11は、下部つかみ具(16)内に受容されるために構成される他の例示のカートリッジ(107,113)を示す。カートリッジ(107,113)は、発射バー(126)を受容する部分を更に含み、発射バー(126)は動作がカートリッジ(37)に関して前述した発射バー(14)と同様である。図12を参照すると、発射バー(126)は上部先端部(130)に終端する遠位ブレード(128)を含む。細長いサイドノッチ(136)は、上縁部(132)の下に及び発射バー(126)の反対の両面(134)の上又はそこを通して配設され、かつ例示のカートリッジ(107,113)が第1の位置にある発射バー(126)の上に、及び下記のように配設されるとき、材料(138)の受容のために構成される。
図10は、デッキ(109)の下に配設される第1の及び第2のスプール(140,142)の一部分としてテープ形態の材料(138)を含むカートリッジ(107)を示す。カートリッジ(107)が下部つかみ具(16)内へ挿入されるとき、スプール(140、142)の内側端部からの材料(138)がノッチ(136)を通って発射バー(126)の両面(134)に取り付ける。例えば、カートリッジ(107)は、スプール(140,142)間に架る材料(138)を発射バー(126)の下を及びノッチ(136)内を通過可能にするのに十分な角度で下部つかみ具(16)内へ挿入されてよい。かかる材料(138)は、カートリッジ(107)が所定位置にはまると、次いでノッチ(136)に保持されてよい。代替的に、上縁部(132)はノッチ(136)と連通するスロットを含んでよく、材料(138)が発射バー(126)の上側からノッチ(136)に進入することを可能にする。どちらの場合にしても、発射バー(126)が矢印(B)の方向に遠位に発射されるとき、スプール(140,142)から材料(138)がほどかれてかつ発射バー(126)がデッキ(109)のスロット(111)の遠位端(148)においてナイフ(144,146)に到達するまで発射バー(126)と共に遠位に発射される。スロット(111)は、構造及び動作が前述したスロット(49)と同様である。ナイフ(144、146)は、次いで発射バー(126)が材料(138)の実質的な部分を保持せずにその最初の位置に戻るように発射バー(126)の両面(134)から材料(138)を切断するように働く。追加的に、前述したように、発射バー(14)が組織(90)を切断するのと同様の方法で発射バー(126)が組織(90)を切断すると、材料(138)は、組織(90)の切断部分へ解放されることになる。
図11は、周囲部分(150)を含む材料(138)のストリップを備えたカートリッジ(113)を示す周囲部分(150)は、カートリッジ(113)のデッキ(115)の外側周辺部を移動可能に取り囲む。カートリッジ(113)が下部つかみ具(16)内に挿入されるとき、周囲部分(150)の中間部分(152)から材料(138)が発射バー(126)のノッチ(136)内へ挿入される。発射バー(126)は、前述したスロット(111)と同様である、スロット(117)に沿って矢印(C)の方向に遠位に発射されるとき、周囲部分(150)から材料(138)が引っかけられてかつ発射バー(126)と共に遠位方向に発射され、発射バー(126)がデッキ(109)のスロット(111)の遠位端(158)においてナイフ(154,156)に到達するまで材料を周囲部分(150)から矢印(D)の方向に引っ張る。ナイフ(154,156)は、前述したカートリッジ(107)のナイフ(144、146)と同様の方法で発射バー(126)から材料(138)を切断する。同様に、発射バー(126)が組織(90)を切断するとき材料(138)は組織(90)の切断部分上に解放されることになる。

カートリッジ(119)は、添加剤アプリケータ(168)の第1のバージョンを含んでよい。図13は、発射バー(162)の反対の両面(166)に回転可能に当接するように構成されるローラー(170,172)を備えるアプリケータ(168)を示す。発射バー(162)がカートリッジ(119)の垂直スロット(121)を通って矢印(E)方向に遠位に前進されるとき、発射バー(162)の前縁部(167)はローラー(170,172)間の材料(174)を切断してよい。代替的に、発射バー(162)は、ローラー(170,172)間でなにも切断せずにローラー(170,172)に沿って単純に通過してよい。発射バー(162)は遠位方向に前進し続けるにつれて、材料(174)はウィッキング作用を介して発射バー(162)の両面(166)に対して転動してかつ塗布する。特に、ローラー(170,172)は、材料(174)をローラー(170,172)からノッチ(164)内へ解放するために矢印(F、G)の方向にそれぞれ転動する。かかる作用は、フェルトペンが例えば、紙の上にインキを塗布しかつ解放するのと同様の方法で発射バー(162)の両面(166)を塗布する。材料(174)を保持するノッチ(164)は、発射バー(162)がエンドエフェクタ(12)に関して前述した方法で組織(90)を切断するとき、別のウィッキング作用を介して材料(174)を、図16に示した切断線(160)などの、切断線の頂部及び底部へ載置する。発射バー(162)は、切断線(160)を生成するために組織(90)を同時に切断しながら、このようにして材料(174)を切断線(160)に最終的に塗布する。

(13) 前記生体適合性材料が、ばね付勢されるゲルブロックを備え、前記生体適合性材料を前記発射バーの側表面上のノッチ内へ解放するために前記発射バーが遠位方向に前進するときに、前記発射バーの前記前縁部が、前記ゲルブロックを通り過ぎて遠位方向に前進するように構成される、実施態様10に記載の装置。
(14) 前記ばね付勢されるゲルブロックが、前記発射バーの両側に対して弾性的に付勢される一対のブロックを備える、実施態様13に記載の装置。
(15) 前記ばね付勢されるゲルブロックが、切断可能なブロックを備え、かつ前記発射バーの前記前縁部が前記切断可能なブロックを一対のブロック部分に切断するために遠位方向に前進するように構成され、これによって前記材料が前記一対のブロック部分の切断された区域から前記発射バーの側表面上のノッチ内へ解放されるようになる、実施態様13に記載の装置。
(a)生体適合性材料を備える前記カートリッジを前記エンドエフェクタの前記下部つかみ具に挿入する工程であって、前記発射バーが第1の位置にあるときに前記生体適合性材料が前記発射バーのノッチ内に保持されるようにする、工程と、
(b)前記カートリッジの前記デッキの前記垂直スロットを通って第2の位置まで前記ステープル留め及び切断アセンブリの前記発射バーを前進させるために前記長手方向閉鎖運動を生成する工程であって、前記発射バーの前記前進により、前記保持された生体適合性材料を同時に前進させるようにする、工程と、
を備える、方法。
(17) (a)前記カートリッジの前記デッキの前記垂直スロットの遠位端に位置付けられた一対のナイフによって前記前進された保持された生体適合性材料を切断する工程を、更に備える、実施態様16に記載の方法。
(18) (a)前記ハンドル部分及び前記シャフトによって生成された前記長手方向閉鎖運動を取り除く際、前記発射バーを前記第1の位置まで付勢させる工程を、更に備える、実施態様17に記載の方法。
(19) 外科用器具であって、
(a)ハンドル部分と、発射バーを収納するシャフトと、
(b)前記ハンドル部分及び前記シャフトによって生成される長手方向閉鎖運動に反応するステープル留め及び切断アセンブリと、
(c)ステープルカートリッジと、
(d)前記ハンドル部分に接続されたエンドエフェクタであって、前記エンドエフェクタがアンビル及び下部つかみ具を備え、前記下部つかみ具が前記ステープルカートリッジを受容するように構成される、エンドエフェクタと、
(e)前記ステープルカートリッジのハウジングに配設された複数のステープルであって、前記アンビルが前記長手方向閉鎖運動に反応して前記ステープルを形成するように構成される、複数のステープルと、
(f)前記ステープルカートリッジの上又は中に配設される生体適合性材料と、
を備え、前記発射バーがノッチを備え、前記発射バーが前記アンビルと前記カートリッジとの間を移動するように動作可能であり、かつ前記ノッチが前記生体適合性材料の一部分を保持するように構成される、外科用器具。
(20) 前記発射バーが、前記生体適合性材料を切断するように動作可能であり、かつ前記ノッチが前記切断された材料の一部分を保持するように構成され、前記生体適合性材料が接着剤、シーラント剤、治癒剤、又は抗菌剤のうちの1種又は2種以上を備える、実施態様19に記載の装置。
Claims (20)
- 外科用器具装置であって、
(a)ハンドル部分と、
(b)発射バーを収納するシャフトと、
(c)アンビルと、下部つかみ具と、前記ハンドル部分及び前記シャフトによって生成される長手方向閉鎖運動に反応するステープル留め及び切断アセンブリと、を備えるエンドエフェクタと、
(d)取り外し可能なカートリッジと、を備え、前記下部つかみ具が前記カートリッジを受容するように構成され、前記カートリッジが、
(i)ハウジングと、
(ii)前記ハウジング内に配設される複数のステープルと、
(iii)前記複数のステープルの上に配設されるデッキであって、前記デッキがアパーチャを画定し、各アパーチャが実質的に各ステープルの上に配設される、デッキと、
(iv)生体適合性材料と、
を備え、前記ステープル留め及び切断アセンブリが、組織を切断すると同時に前記組織内へステープルを駆動する間に、前記発射バーが前記生体適合性材料を前記組織に塗布するように動作可能である、外科用器具装置。 - 前記発射バーが、前記生体適合性材料を受容するように構成されたノッチを含む、請求項1に記載の装置。
- 前記生体適合性材料が、前記カートリッジの近位端において前記カートリッジの前記デッキの上又は下に配設される、請求項2に記載の装置。
- 前記生体適合性材料が、一対のスプールに巻き付けられる、請求項2に記載の装置。
- 前記スプールが、弾性的に付勢される、請求項4に記載の装置。
- 前記発射バーが前記ハンドル部分及び前記シャフトによって生成された前記長手方向閉鎖運動に反応して前記カートリッジの前記デッキのスロットを通って前進するとき、前記スプールの周りの前記生体適合性材料が前進するように構成される、請求項4に記載の装置。
- 前記カートリッジの前記デッキの前記スロットの遠位端において位置付けられた一対のナイフを、更に備える、請求項6に記載の装置。
- 前記生体適合性材料が前記スロットの前記遠位端に到達するときに、前記ナイフが、前記生体適合性材料を切断するように構成される、請求項7に記載の装置。
- 前記生体適合性材料がテープを備えるとき、前記発射バー内のノッチが前記テープを受容するように構成される、請求項1に記載の装置。
- 前記発射バーの前縁部に近位の区域が、前記生体適合性材料を受容するように構成される、請求項1に記載の装置。
- 前記発射バーが遠位方向に前進するときに前記生体適合性材料を前記発射バーの側表面上のノッチ上に解放するように構成される一対のローラーを更に備える、請求項10に記載の装置。
- 前記生体適合性材料を収納するアンプル、を更に備え、前記生体適合性材料を前記発射バーの側表面上のノッチ内に解放するために前記発射バーが遠位方向に前進するときに、前記発射バーの前記前縁部が、前記アンプルを切断するように構成される、請求項10に記載の装置。
- 前記生体適合性材料が、ばね付勢されるゲルブロックを備え、前記生体適合性材料を前記発射バーの側表面上のノッチ内へ解放するために前記発射バーが遠位方向に前進するときに、前記発射バーの前記前縁部が、前記ゲルブロックを通り過ぎて遠位方向に前進するように構成される、請求項10に記載の装置。
- 前記ばね付勢されるゲルブロックが、前記発射バーの両側に対して弾性的に付勢される一対のブロックを備える、請求項13に記載の装置。
- 前記ばね付勢されるゲルブロックが、切断可能なブロックを備え、かつ前記発射バーの前記前縁部が前記切断可能なブロックを一対のブロック部分に切断するために遠位方向に前進するように構成され、これによって前記材料が前記一対のブロック部分の切断された区域から前記発射バーの側表面上のノッチ内へ解放されるようになる、請求項13に記載の装置。
- 外科用器具から材料を解放する方法であって、前記外科用器具がエンドエフェクタと、ハンドル部分と、発射バーを収納するシャフトとを含み、前記エンドエフェクタが下部つかみ具と、アンビルと、を含み、前記下部つかみ具が取り外し可能なカートリッジを受容するように構成され、前記カートリッジが垂直スロットを有するデッキを含み、前記器具が前記ハンドル部分及び前記シャフトによって生成された長手方向閉鎖運動に反応するステープル留め及び切断アセンブリを更に含み、前記ステープル留め及び切断特徴部が前記発射バーを含み、前記方法が、
(a)生体適合性材料を備える前記カートリッジを前記エンドエフェクタの前記下部つかみ具に挿入する工程であって、前記発射バーが第1の位置にあるときに前記生体適合性材料が前記発射バーのノッチ内に保持されるようにする、工程と、
(b)前記カートリッジの前記デッキの前記垂直スロットを通って第2の位置まで前記ステープル留め及び切断アセンブリの前記発射バーを前進させるために前記長手方向閉鎖運動を生成する工程であって、前記発射バーの前記前進により、前記保持された生体適合性材料を同時に前進させるようにする、工程と、
を備える、方法。 - (a)前記カートリッジの前記デッキの前記垂直スロットの遠位端に位置付けられた一対のナイフによって前記前進された保持された生体適合性材料を切断する工程を、更に備える、請求項16に記載の方法。
- (a)前記ハンドル部分及び前記シャフトによって生成された前記長手方向閉鎖運動を取り除く際、前記発射バーを前記第1の位置まで付勢させる工程を、更に備える、請求項17に記載の方法。
- 外科用器具であって、
(a)ハンドル部分と、発射バーを収納するシャフトと、
(b)前記ハンドル部分及び前記シャフトによって生成される長手方向閉鎖運動に反応するステープル留め及び切断アセンブリと、
(c)ステープルカートリッジと、
(d)前記ハンドル部分に接続されたエンドエフェクタであって、前記エンドエフェクタがアンビル及び下部つかみ具を備え、前記下部つかみ具が前記ステープルカートリッジを受容するように構成される、エンドエフェクタと、
(e)前記ステープルカートリッジのハウジングに配設された複数のステープルであって、前記アンビルが前記長手方向閉鎖運動に反応して前記ステープルを形成するように構成される、複数のステープルと、
(f)前記ステープルカートリッジの上又は中に配設される生体適合性材料と、
を備え、前記発射バーがノッチを備え、前記発射バーが前記アンビルと前記カートリッジとの間を移動するように動作可能であり、かつ前記ノッチが前記生体適合性材料の一部分を保持するように構成される、外科用器具。 - 前記発射バーが、前記生体適合性材料を切断するように動作可能であり、かつ前記ノッチが前記切断された材料の一部分を保持するように構成され、前記生体適合性材料が接着剤、シーラント剤、治癒剤、又は抗菌剤のうちの1種又は2種以上を備える、請求項19に記載の装置。
