ii.バルーンガイドカテーテルを挿入し、バルーンを膨張させ、標準的な結腸内視鏡を取り外す(バルーンカテーテルおよび膨張させたバルーンは定位置に残す)。バルーンガイドカテーテルは、例示的な装置/デバイス1を容易に挿入できるようにするためのガイドワイヤとして使用することができる。
iii.チャンバが病変の近位に位置するまで、バルーンガイドカテーテルを介して例示的な装置/デバイス1を挿入する。
iv.例示的な装置/デバイス1のチャンバ10を展開して、好ましい寸法に調節する。必要であれば、処置中にチャンバ10を調節し直す。
v.提供された吸引カテーテルを用いて手術領域を洗浄する。望まれる場合には、近位バルーン、遠位バルーン、または近位バルーンと遠位バルーンの両方を、治療領域を隔離するために膨張させる。
vi.ツールチャネルを挿入する。
vii.器具/ツールをツールチャネル内に挿入する。病変への器具/ツールの接近を最適化して容易にするようにツールチャネルを操作する。
viii.処置を行う。例えば、結腸穿孔を閉じる、大きな結腸ポリープもしくは腫瘍を切除する、出血を止める、憩室を閉じる、虫垂を切除する、または他の体腔病変を治療する。
Claims (33)
- 少なくとも1つの解剖学的組織に影響を及ぼすためのデバイスであって、
伸張可能であり、少なくとも1つの開口または作業空間を(i)有するか、または(ii)形成するかの少なくともどちらかである構造を備える構成を備え、前記少なくとも1つの開口または作業空間を通して、前記少なくとも1つの解剖学的組織が前記構造内に配置され、前記構造が、
a.伸張される前に、少なくとも1つの部分的に剛性の部分を有すること、
b.一部もしくは完全に伸張したときに、複数の形状を有するように制御可能であること、または
c.複数の形状もしくは複数のサイズの少なくとも一方を作業空間に与えるように制御可能であること
のうちの少なくとも1つを含む、デバイス。 - 前記構造が伸張される前に、前記構造が、少なくとも1つの部分的に剛性の部分を有し、前記部分が、非円筒形状の作業領域を形成するように伸張可能である、請求項1に記載のデバイス。
- 前記作業領域が非対称である、請求項2に記載のデバイス。
- 内視鏡装置をさらに備え、前記内視鏡装置が、前記作業領域内に提供されるように構造化され、前記作業領域内部での前記内視鏡装置の先端部分の関節運動を容易にするさらなる構成を含む、請求項2に記載のデバイス。
- 前記さらなる構成が機械的な屈曲アームを含み、前記屈曲アームが、前記作業領域内で前記先端部分を容易に移動させることができるようにして、前記作業領域内の少なくとも1つの物体の視覚化を容易にする、請求項4に記載のデバイス。
- さらに、前記構造に結合され、(i)少なくとも1つのルーメン、または(ii)前記作業領域に達するように前記少なくとも1つのルーメンを通る少なくとも1つの器具の少なくとも一方を提供する装置を備え、前記装置の先端と、前記装置から最も遠く離れた前記構造の遠位部分との間の距離が、前記作業領域の形状またはサイズの少なくとも一方を調節するように制御可能である、請求項2に記載のデバイス。
- 前記構造の一部または完全な伸張時に、複数の形状を有するように前記構造を制御することができる、請求項1に記載のデバイス。
- 前記構造が、複数の形状または複数のサイズの少なくとも一方を前記作業空間に与えるように制御可能である請求項1に記載のデバイス。
- 前記構造が、伸張部分と非伸張部分を有し、前記デバイスの長手軸から前記伸張部分の最高点までの第1の距離が、非伸張部分までの距離とは異なる請求項1に記載のデバイス。
- 前記第1の距離が前記第2の距離よりも大きい、請求項9に記載のデバイス。
- 前記第2の距離をほぼ一定に保ちながら前記第1の距離を調節するように前記構造を制御することができる、請求項10に記載のデバイス。
- 非伸張状態で、前記構成が、複数の方向でその関節運動を行えるように制御可能である、請求項1に記載のデバイス。
- さらに、
前記構成および前記少なくとも1つの解剖学的構造からある距離だけ離して提供される第1の装置と、
前記第1の装置と前記構成の間に提供され、前記第1の装置に接続された少なくとも1つのルーメンを有する第2の装置と、
前記少なくとも1つのルーメンを通って少なくとも1つの解剖学的構造またはその近くに移動するように構造化され、前記構造内に提供されるように構成された第3の装置と、
を備える、請求項1に記載のデバイス。 - 前記少なくとも1つのルーメンがマルチチャネル管を備え、前記構造が、前記マルチチャネル管を通って移動可能であるように構造化され、またマルチチャネル管にしっかりと接続されて、前記マルチチャネル管に対する前記構造の移動を制限または低減する、請求項13に記載のデバイス。
- さらに、前記構成の内部またはその近くに提供され、前記マルチチャネル管を通して移動可能である、少なくとも1つの可動カメラおよび照明装置を備える、請求項14に記載のデバイス。
- 前記構造の内部またはその近くに提供され、前記マルチチャネル管を通して移動可能である、少なくとも1つの可動真空カテーテルまたは少なくとも1つの潅流カテーテルをさらに備える、請求項14に記載のデバイス。
- 前記少なくとも1つのルーメンが、管チャネルまたはツールチャネルの少なくとも一方を備え、前記管チャネルまたはツールチャネルが前記ルーメン内部で移動可能である、請求項13に記載のデバイス。
- 前記ツールチャネルが、軸方向での移動、回転、または屈曲の少なくとも1つを行うことができ、前記ツールチャネルを屈曲するように構成された少なくとも1本のワイヤを含む、請求項17に記載のデバイス。
- 前記ツールチャネルの遠位端が、前記構造の内部またはその近くの任意の点に達するように構成される、請求項17に記載のデバイス。
- 前記ツールチャネルが、少なくとも1本のワイヤを含み、前記ワイヤを使用して、少なくとも1方向で、0〜180度の間の少なくとも1つの角度で前記管または前記ツールチャネルを屈曲することができる、請求項17に記載のデバイス。
- 前記作業チャネルと前記構造の間の距離が、前記作業チャネル内で、前記構造に向かう方向または前記構造から離れる方向の少なくとも一方で少なくとも1本のワイヤを移動させることによって制御可能である、請求項14に記載のデバイス。
- 前記構成の内部またはその近くに提供され、前記作業空間に達するように前記マルチチャネル管を通して移動可能である内視鏡をさらに備える、請求項14に記載のデバイス。
- 前記内視鏡が、前記少なくとも1つの組織の少なくとも一部を視覚化するために可撓性シャフトに設けられた少なくとも1つの画像センサを含む、請求項22に記載のデバイス。
- 前記構造が、少なくとも1本の可撓性ストリップまたはワイヤを有する、請求項1に記載のデバイス。
- 前記構造が、2本以上の可撓性ストリップまたはワイヤを有する、請求項1に記載のデバイス。
- 所望の幾何形状の前記作業空間を提供するために、前記ストリップまたはワイヤの少なくとも1本が、予め成形された形状を有する、請求項25に記載のデバイス。
- 少なくとも1つのバルーンをさらに備える、請求項1に記載のデバイス。
- 2つ以上のバルーンをさらに備え、前記バルーンの少なくとも1つが非対称である、請求項1に記載のデバイス。
- 2つ以上のバルーンをさらに備え、前記バルーンの少なくとも1つが対称形状を有する、請求項1に記載のデバイス。
- 前記構造の近位にある少なくとも1つのバルーンをさらに備える、請求項1に記載のデバイス。
- 第1のバルーンおよび第2のバルーンをさらに備え、前記第1のバルーンが、前記構造に関して遠位に提供され、前記第2のバルーンが、前記構造に関して近位に提供される、請求項1に記載のデバイス。
- 前記構造が、ワイヤまたはメッシュの少なくとも一方から構成され、
a.伸張される前に、少なくとも1つの部分的に剛性の部分を有する、
b.一部もしくは完全に伸張したときに、複数の形状を有するように制御可能である、または
c.複数の形状もしくは複数のサイズの少なくとも一方を作業空間に設けるように制御可能である、請求項1に記載のデバイス。 - 少なくとも1つの解剖学的組織に影響を及ぼすための方法であって、
体腔または体内空洞内にデバイスを挿入するステップであって、前記デバイスが、伸張可能であり、少なくとも1つの開口または作業空間を(i)有するか、または(ii)形成するかの少なくともどちらかである構造を備え、前記少なくとも1つの開口または作業空間を通して、前記少なくとも1つの解剖学的組織が前記構造内に配置されるステップと、
前記少なくとも1つの解剖学的構造またはその近くに、前記構造を展開していない状態で位置決めするステップと、
前記少なくとも1つの解剖学的構造が前記開口または前記作業空間内に移動されるように、前記構造を少なくとも部分的に伸張させることによって前記構造を展開するステップと、
少なくとも1つの器具を、前記開口または前記作業空間に達するように前記デバイスの少なくとも1つのルーメン内に挿入するステップと、
前記少なくとも1つの器具を使用して、前記開口または前記作業空間内で前記少なくとも1つの解剖学的構造の少なくとも1つの変更を行うステップと、を含み、
前記構造が、
a.伸張される前に、少なくとも1つの部分的に剛性の部分を有すること、
b.一部もしくは完全に伸張したときに、複数の形状を有するように制御可能であること、または
c.複数の形状もしくは複数のサイズの少なくとも一方を作業空間に設けるように制御可能であること
のうちの少なくとも1つを含む、方法。
