Claims (1)
- 【特許請求の範囲】 1. 哺乳類の身体管腔内に管状管腔内人工器官を移植するための搬送カテーテ ルであって、 a)貫通して突出可能な長手方向の軸を有する長手のカテーテル本体であって 、該カテーテル本体が、 i)基端、末梢端、およびそこを長手方向に貫通して延びる中空管腔を有す る外側管と、 ii)基端および末梢端を有する長手の内側部材であって、該内側部材は該 外側管の管腔を長手方向に貫通して伸び、該内側部材の末梢部が該外側管の末梢 端から突出し、かつ、それを越えて伸びるようにした、内側部材とを備える、カ テーテル本体と、 b)該管腔内人工器官を半径方向に拡張する環状バルーンであって、該バルー ンは基端と末梢端を有し、該バルーンの基端は該外側管に装着され、該バルーン の末梢端は該内側部材の末梢部に装着される、環状バルーンと、 c)該カテーテル本体を長手方向に貫通して延び、バルーン膨張流体が該バル ーンに注入され、かつ、そこから引き出されるようにする、バルーン膨張流体通 路とを備え、 d)該外側管と該内側部材のうち少なくとも一方が、他方に関連して移動可能 で、バルーンが収縮された場合には、バルーンの引き締めを引き起こす、搬送カ テーテル。 2. 前記バルーンは、膨張すると、 前記長手方向の軸について同軸に配置される実質的に円筒状の側壁であって、 該側壁は基端と末梢端を有する、円筒状側壁と、 円筒状側壁の基端から該外側管の外側表面まで延びる基端壁と、 円筒状側壁の末梢端から該内側部材の外側表面まで延びる末梢端壁とを備え、 該基端壁および該末梢端壁が、カテーテル本体の長手方向の軸に直交する軸か ら10度未満を成す角度で配置される、請求項1に記載の搬送カテーテル。 3. 前記カテーテル本体に装着されて、前記外側管および前記内側部材の互い に関連のある長手方向の運動を促進するための基部コネクタ組立体を更に含む、 請求項1に記載の搬送カテーテル。 4. 前記基部コネクタ組立体は、そこに解放自在に装着されて外側管および内 側部材の互いに関連する不慮の長手方向の移動を阻止するためのスペーサ・クリ ップを含む、請求項3に記載の搬送カテーテル。 5. 基端、末梢端、およびそこを長手方向に貫通して延びる中空管腔を有する 長手のプッシャ本体を更に備え、該外側管は該プッシャ本体の管腔を長手方向に 貫通して延び、バルーンがプッシャ本体の末梢端から突出するようにした、請求 項1に記載の搬送カテーテル。 6. 前記プッシャ本体は、バルーンの基端と管腔内人工器官の基部を受容する ための長手の末梢部を含む、請求項5に記載の搬送カテーテル。 7. プッシャ本体に沿って滑動的に前進可能で、かつ、バルーンおよび管腔内 人工器官を受容する装填器を更に備える、請求項5に記載の搬送カテーテル。 8. 前記カテーテル本体に装着され、本体管腔の所望領域内の管腔内人工器官 の厳密な位置決めを促進する、少なくとも1個の放射線不透過マーカーを更に備 える、請求項1に記載の搬送カテーテル。 9. バルーンの基端に隣接するカテーテル本体の外側管に装着される第1の放 射線不透過マーカーと、 バルーンの末梢端に隣接するカテーテル本体の内側部材に装着される第2の放 射線不透過マーカーとを備え、 該第1および第2の放射線不透過マーカーは、バルーンに搭載された管腔内人 工器官の両端のそれぞれのものに隣接して位置決めされ、身体管腔の所望領域内 の管腔内人工器官の厳密な位置決めを促進する、請求項8に記載の搬送カテーテ ル。 10. 基端壁および末梢端壁は、カテーテル本体の長手方向の軸に直交する軸か ら10度未満の角度で配置される、請求項2に記載の搬送カテーテル。 11. 基端壁および末梢端壁は、カテーテル本体の長手方向の軸に直交する軸か ら5度未満の角度で配置される、請求項2に記載の搬送カテーテル。 12. 前記バルーンが収縮した時は、該バルーンに搭載され、かつ、該バルーン の膨張により半径方向に拡張状態になる、圧力拡張可能な管状の管腔内人工器官 と更に組み合わせて、請求項1の搬送カテーテルを備える、管腔内人工器官シス テム。 13. 前記バルーンに搭載される、管状の半径方向に拡張可能な管腔内人工器官 と更に組み合わせて、請求項2の搬送カテーテルを備える、管腔内人工器官シス テムであって、該管腔内人工器官は該バルーンの膨張により半径方向に拡張状態 になり、該管腔内人工器官は、該バルーンの円筒状側壁の長さと実質的に同一で ある長さを有する、管腔内人工器官システム。 14. 前記外側管および該内側管の少なくとも一方が他方に関連して長手方向に 移動可能で、バルーンが収縮した時は、バルーンがその長手方向の伸びにより引 き締められるようにする、請求項1に記載の搬送カテーテル。 15. 前記外側管と前記内側管のうち少なくとも一方が他方に関連して回転運動 可能で、バルーンが収縮した時には、バルーンのねじれにより引き締めを引き起 こすようにした、請求項1に記載の搬送カテーテル。 16. 哺乳類の身体管腔内に管状管腔内人工器官を移植するための搬送カテーテ ルであって、該搬送カテーテルが、 a)貫通して突出可能な長手方向の軸、基端、および末梢端を有する、長手の 柔軟なカテーテル本体と、 b)該管腔内人工器官を半径方向に拡張させる環状バルーンであって、該バル ーンは、 i)該カテーテル本体の長手方向の軸について同軸に配置される実質的に円 筒状の側壁であって、該側壁は基端および末梢端を有する、側壁と、 ii)該円筒状双璧の基端からカテーテル本体まで延びる基端壁と、 iii)該円筒状側壁の末梢端からカテーテル本体まで延びる末梢端壁とを有 し、 iv)該基端壁および該末梢端壁は、カテーテル本体の長手方向の軸に直交 する軸から10度未満をなす角度で配置される、バルーンとを備える、搬送カテー テル。 17. 前記バルーンの該基端壁および該末梢端壁のうち少なくとも一方が、該カ テーテル本体の長手方向の軸に関して80度ないし90度の内角を形成する、請求項 16に記載の搬送カテーテル。 18. 前記バルーンの前記基端壁および前記末梢端壁のうち少なくとも一方が、 前記カテーテル本体の長手方向の軸に関して85度ないし90度の内角を形成する、 請求項16に記載の搬送カテーテル。 19. 前記カテーテル本体は、 基端、末梢端、およびそこを長手方向に貫通して延びる中空管腔を有する外側 管と、 基端および末梢端を有する長手の内側部材であって、該内側部材は該外側管の 管腔を長手方向に貫通して伸び、該内側部材の末梢部が該外側管の末梢端から突 出し、かつ、それを越えて伸びるようにした、内側部材とを備え、 基端壁は該外側管に装着され、該末梢端壁は該内側部材に装着され、この状態 で、該外側管と該内側部材のうち少なくとも一方は他方に関して移動可能で、バ ルーンが収縮した時には、バルーンの引き締めを引き起こすようにした、請求項 16に記載の搬送カテーテル。 20. 基端、末梢端、およびそこを長手方向に貫通して延びる中空管腔を有する 長手のプッシャ本体を更に備え、該外側管は該プッシャ本体の管腔を長手方向に 貫通して延び、バルーンがプッシャ本体の末梢端から突出するようにした、請求 項19に記載の搬送カテーテル。 21. 前記プッシャ本体は、バルーンの基端と管腔内人工器官の基端を受容する ための長手末梢部を含む、請求項20に記載の搬送カテーテル。 22. プッシャ本体に沿って滑動的に前進可能で、かつ、バルーンおよび管腔内 人工器官を受容する装填器を更に備える、請求項20に記載の搬送カテーテル。 23. 前記カテーテル本体に装着されて、本体管腔の所望領域内の管腔内人工器 官の厳密な位置決めを促進する、少なくとも1個の放射線不透過マーカーを更に 備える、請求項19に記載の搬送カテーテル。 24. バルーンの基端に隣接してカテーテル本体の外側管に装着される第1の放 射線不透過マーカーと、 バルーンの末梢端に隣接してカテーテル本体の内側部材に装着される第2の放 射線不透過マーカーとを備え、 前記第1および第2の放射線不透過マーカーは、バルーンに搭載された管腔内 人工器官の両端のそれぞれのものに隣接して位置決めされ、本体管腔の所望領域 内の管腔内人工器官の厳密な位置決めを促進するようにした、請求項23に記載の 搬送カテーテル。 25. 哺乳類の身体管腔内部に半径方向に拡張可能な管腔内人工器官を移植し、 かつ、身体管腔を通って流れる内源性流体が、半径方向に拡張および移植された 管腔内人工器官の外側で漏れる、内部漏洩を検査するためのシステムであって、 該システムが、 a)貫通して突出可能な長手方向の軸、基端、および末梢端を有する長手のカ テーテルと、 b)該カテーテルに固着される、半径方向に拡張可能な管腔内人工器官であっ て、該管腔内人工器官が該カテーテルから解放されて半径方向に拡張され得る移 植位置まで該カテーテルが長手方向に前進させられるようにし、その結果、管腔 内人工器官が身体管腔と接合接触状態にあり、かつ、除去カテーテルのそれに続 く除去がそれと共に実質的に静止状態に留まる管腔内人工器官と、 c)該搬送カテーテルが該移植位置にある場合は、該管腔内人工器官の上流に ある位置で該搬送について形成される、少なくとも1個の放射線不透過コントラ スト媒体流出開口と、 d)カテーテルを長手方向に貫通して該少なくとも1個の流出開口まで延びる 、放射線不透過コントラスト媒体管腔であって、放射線不透過コントラスト媒体 は、該カテーテルが該移植位置にある時と、該環状管腔内人工器官の半径方向拡 張および移植の後は、該管腔を通して該流出開口から外へ注入され、該内部漏洩 が存在しているかどうかをX線写真法的に判定するようにした、放射線不透過コ ントラスト媒体管腔とを備える、システム。 26. 搬送カテーテルは、管腔内人工器官が移植されるべき身体管腔へ逆行様式 で前進させられるよう意図されており、該少なくとも1個のX線写真コントラス ト流出開口は、管腔内人工器官に対して末梢に在るカテーテル部分に配置され、 X線写真コントラスト媒体が、半径方向に拡張し移植された管腔内人工器官の上 流で該1個以上の流出開口から流出するようにした、請求項25に記載のシステム 。 27. 前記カテーテルは、前記身体管腔を通る内源性流体の流れと同一方向に前 進させられることが意図され、該少なくとも1個のX線写真コントラスト媒体流 出開口は、管腔内人工器官の位置に近接する位置で該カテーテルに形成され、該 1個以上の流出開口から流出するX線写真コントラスト媒体が、半径方向に拡張 し移植された管腔内人工器官の上流で導入されるようにする、請求項25に記載の システム。 28. 哺乳類の身体に挿入可能な管状カニューレであって、該カニューレは、 基端、末梢端、およびそこを長手方向に貫通して延びる中空管腔を有する管状 カニューレ本体と、 管状カニューレが挿入された時には、哺乳類体内に在る位置で該管状カニュー レ本体内部に埋設される、放射線不透過マーカーとを備える、管状カニューレ。 29. 前記放射線不透過マーカーは、該カニューレ本体の末梢端に隣接して配置 され、カニューレ本体の末梢端の位置決めが身体内で放射線的に目視可能になる ようにする、請求項28に記載のカニューレ。 30. 前記放射線不透過マーカーは、放射線不透過材料から形成されるリングで ある、請求項28に記載のカニューレ。 31. 前記放射線不透過マーカーは、 a)前記カニューレの外側表面上に放射線不透過マーカーを最初に位置決めす る工程と、 b)前記放射線不透過マーカーに隣接する該カニューレ本体の一部が溶融状態 になるのを引き起こし、融解カニューレ材が該放射線不透過マーカーを流動可能 に包囲するようにする工程と、 c)内部埋設された該放射線不透過マーカーと共に融解カニューレ材が再固化 できるようにする工程とを含む処理により、該管状カニューレ本体内部に埋設さ れる、請求項28に記載のカニューレ。 32. 前記カニューレは外径の外側表面を有し、該マーカーは、該カニューレの 外径と同一である外径を有するリングであり、該工程aは、 カニューレの外側表面の一部をその末梢端に隣接した位置で最初に除去し、該 カニューレの低減した直径の領域を作る段と、 カニューレの低減した直径の領域上で該リングを前進させる段とを含む、請求 項31に記載のカニューレ。 33. 前記工程cにおける該カニューレの溶解は、カニューレに高周波エネルギ ーを印加することにより達成される、請求項28に記載のカニューレ。
