Description
成形された、射出成形された、インサート成形された、機械加工された、引抜き加工された外科用グレードss管状材料及び他のパーツの組合せを使用して、送達装置を製造することができる。一貫した作用システムの組立てを容易にするために、(締まりばめ、接着、溶接、タブ、スロット、熱接着等の)機械的、熱的、又は、化学的接着といった種々の手段を使用して、装置の構成要素をはめ込むことができる。
当該送達装置は、上記の方法のうちいずれからでも導入することができる。十分な軸上及び放射状の位置決めが達成されると、展開手順が始まる。弁は、近位端から先又は遠位端から先に展開することができる。大動脈弁の場合では(弁の設計及び/又は幾何学に応じて)、漏斗状に開かせるために、弁の近位端を第一に展開することが好ましい。そうすることで、抵抗が感じられるまで、送達装置をわずかに前に進めることができる。オペレータに対するこの触覚フィードバックは、弁の端が本来の弁のほぼ環状の端であるため、最適な軸上配列が達成されたことのしるしである。弁の軸上の位置を維持するよう送達装置に対する前方へのわずかな圧力を維持しながら、遠位外筒を前方に進めることによって遠位端をここで展開することができる。近位及び/又は遠位端が展開している間、感温ニチノール弁フレームの緩やかな制御された展開を容易にするよう温度が調節された塩水を注入することができる。これは、移植部位にて切開又は他のダメージを引き起こし得るため望まれていない場合がある弁の突然の「スナップオープン」を防ぐことができる。塩水の温度は、最終的に体温に達するまで緩やかに上げることができる。これは、弁がその完全に拡張した最適な幾何学まで拡張するのを可能にする。
−折りたたみ可能/拡張可能な弁を折りたたむ、維持する、送達する、展開する、再配置する、及び、(所望であれば)回収するための薄型送達システム。
−送達方法(逆行性又は順行性)に対して送達システムを微調整することができる。
−蛍光透視又は他の可視化システム下での弁及び送達システムの遠位端の案内並びに可視化のために、放射線不透過性マーカーを戦略的に重要な領域、及び、弁の周りに置くことができる。
−再配置又は回収のために完全に回復させる場合に、弁を完全に展開するが解放しない能力が所望される。
−ガイドワイヤ、バルーンカテーテル、塞栓保護装置、流体送達(フラッシング又は可視化)等の補助装置を使用することによって処置上の支持を促進するための多数の内腔の能力を有した送達システム。
−以下のように互いに対して移動するいくつかのシャフト/内腔:(1)外側のシャフト:弁を覆い、折りたたみ、さらに、解放する;弁の周りに流体を流すための導管を生じる。(2)中間シャフト:外側及び内側のシャフトに対してクリンプされた弁を進める、及び、引っ込める。(3)内側のシャフト:弁の遠位端の展開のために遠位の外筒を進める;流体送達のための導管だけでなく、ワイヤ上での前進を可能にする内腔も有している。
−外筒及び保持装置を使用して弁は所定の位置に固定される。保持装置は、中間シャフトに装備され、弁と共に移動する。外筒は、クリンプされた弁上を移動する。
−弁の保持/折りたたみ機構として機能する移動可能な外筒。折りたたまれた状態で弁を維持及び保護することもできる。温度が調節された塩水等の流体のための導管の外縁を生じる。
−独特な分離外筒設計(近位及び遠位の外筒の半分)。遠位の半分は先端部に取り付けられ、近位端にて制御及び作動させられる細長いシャフトにも取り付けられる。この外筒を遠位方向に押すことによって、弁の遠位端が展開される。外筒の他の半分は、送達装置の残りに取り付けられ、弁に対して近位方向に移動し、弁の近位端を展開する。
−(例えば、本来の弁の交連と合わせて並べるため、及び、冠状動脈の閉塞を回避するために)正確な放射状の向きで展開することができるように送達システム内での弁の回転(角度)の向きを制御する機能。
−マニホールドコネクタに戻った内腔の出入口のうちいずれに対するアクセスも開け閉めする能力。
−送達装置のシャフトを適切な所で互いに相対してロックし、その適切な所でロックすることに加えて、導管をふさぐ能力。
−近位の弁保持装置:(1)近位の外筒を主要な送達装置の縦軸上の中央に置く。(2)送達装置の準備中及び温度が調節された塩水の適用中の流体の通過を可能にする。(3)前進している/引っ込んでいる要素を縁がじゃま/捕らえるのを防ぐためのテーパエンド。(4)弁の近位端に対する静止場所を亢進することができる。
−遠位の弁保持装置:(1)遠位の調圧環及び接続支柱の一部を保持及び制御する。(2)最終的な解放のために覆いを取り払われるまで、弁の軸上の移動を防ぐ。(3)保持装置の幾何学は、特定の弁フレームの幾何学と一致し、必要に応じて解放する。(4)前進している/引っ込んでいる要素を縁がじゃま/捕らえるのを防ぐテーパエンド。(5)展開手順が始まるに従い、最終的な解放中に弁が跳ねるのを防ぐ。
−近位及び/又は遠位の保持装置は、再度折りたたむ要素の通過を容易にする孔を含み得る。前記要素は、弁の近位又は遠位部分(若しくはその両方)に取り付けられ、再配置又は回収のために弁を再度折りたたむよう使用される。
−近位及び遠位の外筒インターフェースは、塞ぐ及び連結するよう設計することができる。
−描写されてはいないけれども、送達システムは、遠位端にてシャフトの外まで延び、弁の展開及び解放に先立ち、石灰化した本来のリーフレットをこじあけることができる機能を組み入れることができる。
−種々の設計の変形が可能であるが、いくつかの設計のみが例証用に含まれている。例えば、近位の外筒が遠位の外筒を完全に内部に封じ込める別の設計を参照されたい。これは、遠位外筒に対する保護を提供し、同時に、2つの外筒IDサイズにさらなる設計の変形を提供する。これは、遠位端よりもわずかに大きい折りたたまれた直径を弁の近位端が有することを可能にし、より空間を、従って、より少ないリーフレットの組織の圧縮を提供する。近位外筒の内側にある遠位外筒におけるさらなる可能な利点は、経心尖部方法等の順行性の方法中のスムーザーシステムの引っ込みである。当該システムから弁が解放されると、近位外筒を、リーフレットを超えるまで前に進めることができる。送達システムの遠位外筒を、次に、近位外筒の内側まで引っ込めることができ、弁を介してシステム全体を戻し除くことができる。引っ込み中、リーフレットは、システム全体を取り除く前に、リーフレットを介して近位外筒の内側に遠位外筒の引っ張ることを試みるシナリオと比較して、滑らかで継続する管の表面にのみ直面することに留意されたい。
Claims (19)
- 折り畳み可能で再拡張可能な人工心臓弁を患者の移植部位まで送達するための装置であって、
折り畳み状態において前記弁を周囲に配置する弁支持構造体と、
該弁支持構造体上の前記弁を取り囲む外筒構造体と、
前記移植部位での拡張のために前記弁の覆いを取るよう、前記弁支持構造体に対して前記外筒構造体を移動させる手段とを含み、
前記弁は、第一の表面部分及び第二の表面部分を有し、前記第一の表面部分及び前記第二の表面部分は、前記弁支持構造体上で前記弁を周囲に配置する軸に沿って、それぞれ反対の第一方向及び第二方向に面し、
前記弁支持構造体は、第三の表面部分及び第四の表面部分を有し、前記第三の表面部分及び前記第四の表面部分は、それぞれ前記第二方向及び前記第一方向に面し、
前記弁が前記折り畳み状態において前記弁支持構造体の周囲に配置される間の前記軸に沿った前記弁と前記弁支持構造体との相対的な移動を実質的に防止するよう、前記第一の表面部分及び前記第三の表面部分は、互いに隣接して向き合うよう配置され、前記第二の表面部分及び前記第四の表面部分は、互いに隣接して向き合うよう配置され、
前記弁支持構造体は、弁支持構造体より下からの流体が通過し得るよう、前記弁の近位にある第一の位置から前記弁の遠位にある第二の位置まで延びる通路を定め、前記第一の位置は、前記第二の位置よりも当該装置のオペレータに近い、
装置。 - 前記外筒構造体は、前記折り畳み状態において前記弁の第一の軸方向端部を覆う第一の外筒部分と、前記折り畳み状態において前記弁の第二の軸方向端部を覆う第二の外筒部分とを含む、請求項1に記載の装置。
- 前記移動させる手段は、前記外筒部分の一方を前記外筒部分の他方に対する移動を可能にさせる、請求項2に記載の装置。
- 前記移動させる手段は、前記第一の外筒部分を前記弁の前記第二の軸方向端部から離れる第一の方向に移動させる第一の手段と、前記第二の外筒部分を前記弁の前記第一の軸方向端部から離れる第二の方向に移動させる第二の手段とを含む、請求項3に記載の装置。
- 前記第一及び第二の外筒部分は、互いに部分的に重なり合う、請求項2に記載の装置。
- 前記弁支持構造体は、前記折り畳み状態の弁と係合して、前記弁が前記弁支持構造体の周りを回転するのを実質的に防止する、請求項1に記載の装置。
- 前記外筒構造体による覆いを取られた前記弁の一部を前記弁支持構造体に向かって放射状に内側に引く手段を更に含む、請求項1に記載の装置。
- 前記外筒構造体が前記弁支持構造体を覆うように、前記移動させる手段は、前記弁の拡張後に、前記外筒構造体が前記弁支持構造体に対して再び移動させられるのを追加的に可能にする、請求項1に記載の装置。
- 前記弁支持構造体は、前記弁の近位にある位置から前記弁までの流体連絡のための通路を定め、前記位置は、前記弁よりも当該装置のオペレータに近い、請求項1に記載の装置。
- 前記弁の軸方向端部の一方は、弁リーフレットを含み、前記弁の他方の軸方向端部は、リーフレットを備えない弁フレーム構造を含む、請求項2に記載の装置。
- 当該装置のオペレータから最も遠い前記外筒構造体の一部に固定される遠位端構造体を更に含み、前記遠位端構造体は、前記外筒構造体の内部の脱気を容易にするために前記外筒構造体の内側から当該装置の外側まで続く孔を有する、請求項1に記載の装置。
- 当該装置のオペレータから最も遠い前記外筒構造体の一部に固定される遠位端構造体と、前記遠位端構造体と前記外筒構造体の一部とを前記弁支持体から離れるよう動かすことを可能にするシャフトを更に含み、該シャフトは、前記外筒構造体の一部に近接した結合部位を含む、請求項1に記載の装置。
- 折り畳み可能で再拡張可能な人工心臓弁を患者の移植部位まで送達するための装置であって、
折り畳み状態において前記弁を周囲に配置する弁支持構造体と、
該弁支持構造体上の前記弁を取り囲む外筒構造体と、
前記移植部位での拡張のために前記弁の覆いを完全に取るよう、前記外筒構造体を前記弁支持構造体に対して遠位方向に移動させる手段とを含み、
前記弁支持構造体は、軸方向に離れた近位部分と、遠位部分とを有し、前記近位部分は、前記遠位部分よりも当該装置のオペレータに近く、
前記弁支持構造体は、弁支持構造体より下からの流体が通過し得るよう、前記弁の近位にある第一の位置から前記弁の遠位にある第二の位置まで延びる通路を定め、前記第一の位置は、前記第二の位置よりも当該装置のオペレータに近い、
装置。 - 前記外筒構造体を取り囲む第二の外筒構造体と、前記外筒構造体の覆いを取るよう、弁支持構造体に対して近位方向に前記第二の外筒構造体を移動させる第二の手段とを更に含む、請求項13に記載の装置。
- 前記弁支持構造体は、前記折り畳まれた弁のフレーム構造体が配置される管状の幾何学的な形状に放射状に外側に延びる要素を含み、該要素は、前記弁支持体に対して、前記管状の幾何学的な形状の縦軸と平行な前記弁の動きを妨害するよう置かれる、請求項13に記載の装置。
- 前記移動させる手段は、前記移動させる手段が前記弁の覆いを完全に取るよう前記外筒構造体を移動させるときに、前記外筒構造体の近位にあるが前記弁支持構造体の遠位にある結合部位を含む、請求項13に記載の装置。
- 前記外筒構造体の遠位端に固定される遠位端構造体を更に含み、該遠位端構造体は、前記外筒構造体の内側から当該装置の外側まで続く孔を定める、請求項13に記載の装置。
- 前記弁支持構造体は、前記弁の近位にある位置から前記弁までの流体連絡のための通路を定め、前記位置は、前記弁よりも当該装置のオペレータに近い、請求項13に記載の装置。
