Abstract
Cuando se encuentra en la posición no operativa, el médico puede empujar o halar el botón de control (84) para mover la cuchilla (78) en una vía lineal dentro del catéter (ver Fig. 20). Al empujar el botón de control (84), el médico puede mover la cuchilla (78) hacia fuera del catéter (68), donde puede hacer girar ésta hasta alcanzar la posición operativa, (ver Fig. 21). Cuando se encuentra en la posición operativa, al empujar y halar el botón de control (84) se acciona la cuchilla en golpes lineales contra la masa de tejido circundante. Al usarse, el catéter (68) es también llevado mediante deslizamiento y rotación hacia dentro de la funda guía o cánula (34), de la misma manera que la mostrada en la Fig. 4. El médico puede deslizar libremente el catéter (68) axialmente dentro de la funda guía (34) para desplegar la herramienta (56) en el área de tratamiento seleccionada. Al ser desplegada en el área que se trabajará, el médico puede desplegar la cuchilla (78) en el estado operativo fuera del catéter (68) y deslizar la cuchilla (78) a lo largo del tejido en una vía lineal. El movimiento lineal de la cuchilla (78) a lo largo del tejido corta el tejido. El médico también puede hacer girar tanto el catéter (68) dentro de la funda guía (34), como la cuchilla (78) dentro del catéter (68) para ajustar la orientación y el trayecto de la cuchilla (78). La cuchilla (78) puede poseer uno o más marcadores radiológicos (86), según lo descrito anteriormente, para permitir la visualización radiológica de la cuchilla (78) dentro del área de tratamiento seleccionada. terapéuticos requieren la formación de una cavidad en una zona del
interior del cuerpo.
Por ejemplo, como se muestra en las Patentes de
US 4.969.888 y 5.108.404, un cuerpo expansible, se despliega para
formar una cavidad en un tejido corporal esponjoso, como parte del
procedimiento terapéutico que fija estructuras u otros procesos
óseos anormales, tanto de origen osteoporóticos como no
osteoporóticos. El cuerpo expansible comprime el hueso esponjoso
para formar una cavidad interior. La cavidad recibe un material de
relleno que proporciona un apoyo estructural interno renovado al
hueso cortical.
Este procedimiento puede usarse para tratar el
hueso cortical, el cual debido a la osteoporosis, una necrosis
vascular, cáncer o trauma, se fractura o está predispuesto para una
fractura por compresión o un colapso. Estos procesos, si no se
tratan exitosamente, pueden traer como consecuencia deformidades,
complicaciones crónicas, y un impacto general adverso sobre la
calidad de vida.
Las patentes de US 4, 969,888 y 5, 108,404
pueden formar cavidades en el hueso y en otras zonas del interior
del cuerpo de forma segura y eficaz.
La WO 97.31577 muestra una herramienta cortante
flexible que contiene un cable de fibras superelásticas para
heridas helicoidales que posee una punta de taladro en su extremo
distal. Un motor hace girar el cable en una dirección que tiende a
tensar las fibras del cable.
La US 3.181.533 muestra un lazo quirúrgico que
consta de un asa de alambre quirúrgico atado a un mecanismo de
"rachet". Al usarse, el lazo se coloca alrededor de un
cartílago u otro miembro y se aprieta manualmente hasta que se
efectúa el corte.
La US 5.591.170 muestra una sierra de corte del
hueso intramedular que contiene una hoja cortante giratoria.
Existe una demanda de sistemas o métodos
alternativos los cuales, a semejanza del cuerpo expansible mostrado
en las Patentes US 4, 969,888 y 5, 108,404, son capaces de formar
cavidades en el hueso y en otras zonas del interior del cuerpo en
forma segura y eficaz.
Según la presente invención, se proporciona un
sistema según la reivindicación 1 y un equipo según la
reivindicación 11.
La invención proporciona nuevas herramientas
para crear cavidades en huesos esponjosos. Las herramientas
contienen estructuras que cortan el hueso esponjoso para formar una
cavidad.
En una realización, la estructura consta de un
filamento, el cual puede formarse como un asa o como un conjunto
que forma una brocha. La manipulación del filamento cuando está
dentro del hueso corta el hueso esponjoso para crear una cavidad.
En este documento se muestra también una estructura que consta de
una cuchilla que corta el hueso esponjoso mediante cualquier
movimiento lateral, movimiento rotacional o ambos. También se
muestra en este documento una estructura que consta de un transmisor
de energía que corta el hueso esponjoso para crear la
cavidad.

La invención también proporciona un equipo que
contiene indicaciones para usar una herramienta seleccionada según
un método que comprende los pasos de despliegue de la herramienta
dentro del hueso y la manipulación de la estructura para cortar el
hueso esponjoso y formar la cavidad. Las instrucciones también
pueden indicar el llenado de la cavidad con un material como por
ejemplo, cemento óseo, material para homo trasplantes, sustituto de
hueso sintético, un medicamento o un material que fluya y se
endurezca al depositarse.
Características y ventajas de las invenciones se
muestran en la siguiente Descripción y Dibujos, así como en
las Reivindicaciones adjuntas.

La invención puede ser presentada en varias
formas sin alejarse de sus características esenciales. El alcance
de la invención está definido en las reivindicaciones adjuntas más
que en la descripción específica que las precede. Todas las
realizaciones que se encuentran dentro del significado y rango de
equivalencia de las reivindicaciones tienen el propósito, por lo
tanto, de estar incluidas en las reivindicaciones.
Los sistemas y métodos relacionados con la
invención pueden adaptarse para ser usados casi en cualquier zona
del interior del cuerpo, donde se requiera la formación de una
cavidad dentro del tejido con fines terapéuticos o diagnósticos.
Las realizaciones de preferencia muestran la invención en asociación
con sistemas y métodos usados para tratar huesos. Esto se debe a
que los sistemas y métodos de los que trata la invención se adaptan
muy bien a este entorno. Debe destacarse que los sistemas y métodos
que se refieren a características de la invención pueden también
ser utilizados en otras zonas del interior del cuerpo.
La Fig. 1 muestra una herramienta rotativa (10)
que puede formar una cavidad en un área de tratamiento determinada.
La herramienta (10) consta de un catéter (12) que tiene un extremo
proximal y uno distal, respectivamente (14 y 16). El catéter (12)
incluye preferiblemente una agarradera (18) para ayudar a asir y
maniobrar el catéter (12). La agarradera (18) puede hacerse de un
material de espuma asegurado al catéter (12).
El catéter (12) posee una estructura formadora
de cavidades (20) en su extremo distal (16). En la realización
ilustrada, la estructura (20) consta de un filamento (22) hecho de
un material elástico inerte, el cual ha sido doblado hacia atrás
hasta poner sus extremos paralelos y preformado con la memoria
resiliente para formar un asa.
El material del cual está hecho el filamento
(22) puede ser alambre resiliente e inerte como el acero inoxidable.
Alternativamente, el plástico inerte elástico moldeado por
inyección o con la memoria estructural del material, como el
titanio-níquel (comercialmente disponible como el
material Nitinol ™) también pueden ser usados. El filamento (22)
puede ser, en un corte transversal, redondo, rectilíneo o tener otra
configuración.
Según se muestra en la Fig. 1A, el filamento
(22) irradia desde ranuras (24) en una base (26) colocada en el
extremo distal (16) de un catéter (12). Los extremos libres (28) del
filamento (22) se extienden a través del catéter (12) y están
conectados a un controlador de corredera (30) cerca de la agarradera
(18).
Según se muestra en la Fig. 2, al deslizar el
controlador (30) posterior (flecha A) se recoge el filamento (22) a
través de las ranuras (24), las cuales disminuyen progresivamente
las dimensiones de la estructura de asa (20). Como se muestra en la
Fig. 2, en su posición posterior más alejada, el filamento (22)
está esencialmente recogido por completo y no sobresale a una
distancia significativa por detrás del extremo distal (16) del
catéter (12).
Según muestra la Fig. 3, al deslizar (30) el
controlador hacia delante (flecha F), hace que el filamento (22) se
deslice hacia delante a través de las ranuras (24). Se forma la
estructura de asa (20), la cual sobresale más allá del extremo
distal (16) del catéter (12). A medida que avanza progresivamente
hacia adelante a través de las ranuras (24), las dimensiones de la
estructura de asa (20) aumentan progresivamente (compare la Fig. 1
con la Fig. 3). El controlador (30) puede incluir datos (32), a
través de los cuales el médico puede calcular las dimensiones de la
estructura de asa (20).
Al usarse (ver Fig. 4), el catéter (12) es
llevado mediante un movimiento axial y rotacional hacia dentro de
una funda guía (34). El médico puede entonces deslizar libremente el
catéter (12) axialmente dentro de la funda guía (34) (flecha S en
la Fig. 4). Según se muestra en la Fig. 4, cuando se encuentra
completamente dentro de la funda guía (34), la estructura de asa
(20), si sobresale a una distancia significativa del extremo distal
(16), es plegada por la funda (34) que la rodea. Al liberarse de la
funda (34), la estructura de asa (20) se abre como un resorte para
asumir su dimensión normal. A partir de ahora, el médico puede
accionar el controlador (30) para alterar la dimensión de la
estructura de asa (20) a voluntad.
Al estar libre de la funda guía (34), el médico
también puede rotar la estructura de asa (20) desplegada, mediante
la rotación del catéter (12) dentro de la funda guía (34) (flecha R
en la Fig. 4). Como se describirá más detalladamente
posteriormente, la rotación de la estructura de asa (20) rebana o
corta a través de la masa de tejido circundante.
Los materiales para el catéter (12) son
seleccionados para facilitar el avance y rotación de la estructura
de asa (20). El catéter (12) puede construirse, por ejemplo, usando
materiales plásticos flexibles típicos aprobados para el uso
médico, como el vinil, el nylon, polietilenos, ionómero, poliuretano
y tetraftalato de polietileno (PET). El catéter (12) puede también
incluir materiales más rígidos para imprimir mayor rigidez y por lo
tanto facilitar su manipulación y la capacidad de transmisión de
torsión. Los materiales más rígidos que pueden ser usados para este
fin incluyen el acero inoxidable, las aleaciones
níquel-titanio, (material Nitinol™) y otras
aleaciones de metales.
El filamento (22) posee preferiblemente uno o
más marcadores radiológicos (36). Los marcadores (36) están hechos
de materiales radio-opacos conocidos, como platino,
oro, calcio, tantalio y otros metales pesados. Al menos un marcador
(36) es colocado en o cerca del extremo distal de la estructura de
asa (20), mientras otros marcadores pueden ser colocados de forma
espaciada en puntos ubicados en la estructura de asa (20). El
extremo distal (16) del catéter (12) también puede tener marcadores.
Los marcadores (36) permiten la visualización radiológica de la
estructura de asa (20) y del catéter (12) dentro del área de
tratamiento escogida.
Por supuesto, otras formas de marcadores pueden
ser utilizadas para permitir al médico visualizar la localización y
forma de la estructura de asa (20) dentro del área de tratamiento
escogida.
La Fig. 5 muestra una realización alternativa de
una herramienta (38) giratoria que puede formar una cavidad en el
área de tratamiento escogida. La herramienta (38) consta de un árbol
motor (40), el cual puede hacerse de materiales más rígidos en aras
de una buena capacidad de transmisión de torsión, por ejemplo, acero
inoxidable, aleaciones níquel-titanio (material
Nitinol™) y otras aleaciones de metales.
El extremo distal (42) del árbol motor porta una
estructura que forma una cavidad (44), la cual consta de un sistema
de filamentos que forman cerdas (46). Como se muestra en la Fig. 5A,
las cerdas (46) se extienden desde ranuras (48) colocadas de forma
espaciada en una base (50) colocada en el extremo distal (42) del
árbol motor (40).
El material del cual están formadas las cerdas
(46) puede ser acero inoxidable, o plástico inerte moldeado por
inyección, o materiales con memoria estructural, como el titanio.
Las cerdas (46) pueden, en un corte transversal, ser redondas,
rectilíneas o tener otra configuración.
El extremo proximal (52) del árbol motor (40)
porta un accesorio (54) que, al estar en uso, es acoplado a un
motor eléctrico (56) para que haga girar al árbol motor (40) y con
ello, las cerdas (46) (flechas R en las Figs. 7 y 8). Al ser
giradas por el motor (46), las cerdas se separan y se abren (según
se muestra en la Fig. 7), bajo la influencia de la fuerza
centrífuga, formando una estructura semejante a una brocha (44). La
estructura de brocha (44), al girar, corta la masa de tejido
circundante en el área de tratamiento seleccionada.
Los extremos libres (58) de las cerdas (46) se
extienden a través del árbol motor (40) y están comúnmente
conectados al controlador de corredera (60). Como se muestra en la
Fig. 6, el controlador de corredera (60) posterior (flecha A en la
Fig. 6) acorta la distancia a la que se encuentran las cerdas (46)
de la base (50). Según lo mostrado en las Figs. 7 y 8, al deslizar
el controlador de corredera (60) hacia adelante (flecha F en la
Fig. 8), se alarga la distancia de extensión de las cerdas (46).
Usando el controlador (60), el médico puede ajustar la dimensión
del área de corte (compare la Fig. 7 y la Fig. 8).
El sistema de cerdas (46) incluye
preferiblemente uno o más marcadores radiológicos (62), según se
describió previamente. Los marcadores (62) permiten la
visualización radiológica de la estructura de brocha (44) al usarse
dentro del área de tratamiento escogida. El controlador (60) puede
también incluir indicios (64) a partir del cual el médico puede
calcular visualmente la distancia de extensión de las cerdas. El
extremo distal (42) del árbol motor (40) también puede poseer uno o
más marcadores (62).
El árbol motor (40) de la herramienta (38), al
usarse, es llevado por el movimiento axial y giratorio hacia dentro
de la funda guía o cánula (34), de la misma manera que se muestra
para la herramienta (10) en la Fig. 4. El médico puede deslizar
libremente el árbol motor (40) axialmente dentro de la funda guía y
desplegarlo en el área de tratamiento escogida. Una vez conectado al
motor de transmisión (56), el árbol motor (40) está libre para
girar dentro de la funda guía (34) para formar una estructura de
brocha (44).
La Fig. 9 muestra una realización alternativa de
una herramienta giratoria (138) que posee un sistema de filamentos
que forman cerdas (140), la cual puede formar una cavidad en un área
de tratamiento escogida. La herramienta (138) incluye un árbol
motor flexible (142), el cual está hecho, por ejemplo, de filamentos
de alambre torcido, tales como acero inoxidable, aleaciones de
níquel-titanio (material Nitinol™) y otras
aleaciones de metales.
Las cerdas (140) se extienden radialmente desde
el árbol motor (142), cerca de su extremo distal. Las cerdas (140)
pueden estar hechas de, por ejemplo, acero inoxidable resiliente, o
plástico inerte moldeado por inyección, o materiales con memoria
estructural como el níquel-titanio. Las cerdas (140)
pueden, en un corte transversal, ser circulares, rectilíneas o
tener otra configuración.
Como muestra la Fig. 10, la herramienta (138) se
introduce en el tejido de la zona seleccionada mediante una cánula
(144). Cuando se encuentran dentro de la cánula (144), las cerdas
resilientes (144) son comprimidas hacia atrás hasta lograr un
perfil bajo, permitiendo el paso a través de la cánula. Al liberarse
de la cánula (144), las cerdas resilientes (140) se abren
radialmente como resortes, listas para ser usadas.
El extremo proximal del árbol motor (142) posee
un accesorio (146) que, al usarse, se acopla a un motor eléctrico
(148). El motor (148) hace girar el árbol motor (142) (flecha R en
la Fig. 11) y, con éste, las cerdas (140).
Según se muestra en la Fig. 11, al desplegarse
dentro de una cavidad interna del cuerpo con tejido blando (S) (por
ejemplo, el hueso esponjoso rodeado por tejido duro (H) (por
ejemplo, hueso cortical), el médico puede guiar la herramienta
(138) a través del tejido blando (S) permitiendo que las cerdas en
rotación (140) presionen sobre el tejido duro (H). El árbol motor
flexible (142) se curva para seguir el contorno del tejido duro
(H), mientras que las cerdas en rotación (140) cortan el tejido
blando (S) circundante, formando una cavidad C.

(142) porta una cuchilla angular (151) en su extremo distal. La
cuchilla (151) gira con el árbol motor (142). Al trabar el tejido,
la cuchilla (151) genera una fuerza que hala hacia delante, la cual
ayuda a adelantar el árbol motor (142) y las cerdas (140) a través
de la masa de tejido blando.
En la realización ilustrada, las cerdas (140), o
la cánula (144) o ambas incluyen uno o más marcadores radiológicos
(153), según lo descrito previamente. Los marcadores (153) permiten
la visualización radiológica de las cerdas (140) al rotar y van
moviéndose hacia delante dentro del área de tratamiento
seleccionada.
La Fig. 12 muestra una herramienta giratoria
comparativa (106) que puede formar una cavidad en un área de
tratamiento seleccionada. La herramienta 106, al igual que la
herramienta 38, consta de un árbol motor rígido (108) hecho de, por
ejemplo, acero inoxidable, aleaciones de
níquel-titanio (material Nitinol™) y otras
aleaciones de metales, para obtener una buena capacidad de
transmisión de torsión.
El extremo distal del árbol motor (108) porta
una estructura para formar una cavidades (110) que consta de una
hoja cortante. La cuchilla (110) puede tener varias formas.
En las Figs. 12 y 13, la cuchilla (110)
generalmente tiene forma de L, poseyendo un brazo principal (112) y
un brazo corto (116). En el dispositivo ilustrado, el brazo
principal (112) de la cuchilla (110) es movido angularmente en un
plano vertical radialmente hacia delante del eje del árbol motor
(114), en un pequeño ángulo hacia fuera y adelante, perpendicular
al árbol motor. El brazo principal (112) puede poseer una
configuración generalmente derecha (según muestra la Fig. 12), o,
alternativamente, puede presentar una superficie generalmente curva
(según muestra la Fig. 13) en el dispositivo ilustrado, el brazo
corto (116) de la cuchilla (110) también es movido en un pequeño
ángulo con inclinación hacia delante con respecto al brazo principal
(112), con una amplitud ligeramente superior a los
90°.

En la Fig. 14, la cuchilla (110) toma la forma
de un anillo continuo (126). Según se ilustra, el anillo (126) es
movido angularmente ligeramente hacia delante, por ejemplo, en un
ángulo ligeramente superior a los 90° en relación con el eje del
árbol motor (114).
El material del cual está hecha la cuchilla
(110) puede ser acero inoxidable, o plástico inerte moldeado por
inyección. Los brazos (112 y 116) de la cuchilla (110) mostrados en
las Figs. 12 y 13, y el anillo (126) mostrado en la Fig. 14,
pueden, en un corte transversal, ser redondos, rectilíneos o tener
otra configuración.
Al ser girada (flecha R), la cuchilla (110)
corta y crea una vía generalmente cilíndrica a través de la masa de
tejido circundante. El movimiento angular hacia delante de la
cuchilla (110) reduce la torsión y proporciona estabilidad y
control a medida que la cuchilla avanza (110), mientras rota, a
través de la masa de tejido.
La rotación de la cuchilla (110) puede obtenerse
manualmente o a mayor velocidad mediante el uso de un motor. En la
realización ilustrada, el extremo proximal del árbol motor (108) de
la herramienta (106), posee un accesorio (118). El accesorio (118)
está acoplado a un motor eléctrico (120) para hacer girar al árbol
motor (108), y, con ello, a la cuchilla (110).
Según muestra la Fig. 15, el árbol motor (108)
de la herramienta (108?/118) es desplegado subcutáneamente hacia
dentro del área de tejido seleccionada a través de la funda guía o
cánula (124). Al conectarse el motor transmisor (120), el árbol
motor (108) gira dentro de la funda guía (34), haciendo, por lo
tanto, rotar la cuchilla (110) para cortar y abrir una vía
cilíndrica (P) en la masa de tejido circundante TM. La cuchilla
(110) puede llevarse hacia delante o recogerse, mientras gira, en
una vía recíproca (flechas F y A), mediante la aplicación de
fuerzas de empuje y de tracción sobre el árbol motor (108). La
cuchilla (110) también puede ser retirada hacia adentro de la cánula
(124). De esta forma, pueden abrirse sucesivas vías cilíndricas
cortando a través de la masa de tejido, mediante la rotación y el
vaivén (110), y así crear una cavidad con la forma deseada.
La cuchilla (110), o el extremo de la cánula
(124), o ambas, pueden portar uno o más marcadores radiológicos
(122), según lo descrito anteriormente. Los marcadores (122)
permiten la visualización radiológica de la cuchilla (110) y su
posición con respecto a la cánula (34) al usarse dentro del área de
tratamiento seleccionada.
Como muestra la Figura 16, cualquiera de las
herramientas 10, 38, 106 o 138 pueden incluir un lumen interno
(128). El lumen (128) esta acoplado mediante una válvula en Y (132)
a una fuente externa de fluido (130) y a una fuente externa de vacío
(134).
Un líquido de enjuague (136), por ejemplo,
solución salina, puede introducirse desde la fuente (130) a través
del lumen (128) hacia el área de tejido seleccionada a medida que
las herramientas (10, 38, ó 106) giran y cortan la masa de tejido
(TM). El líquido de enjuague (136) reduce la fricción y conduce el
calor lejos del tejido durante la operación de corte. El líquido de
enjuague (136) puede ser introducido de forma continua o
intermitente mientras se corta la masa de tejido. El líquido de
enjuague (136) puede también incluir un anticoagulante.
Mediante el acoplamiento periódico del lumen
(128) a la fuente de vacío (134), se pueden aspirar los líquidos y
los desechos del área de tejido seleccionada a través del lumen
(128).
Las Figs. 17 a la 21 muestran una herramienta
comparativa de movimiento lineal (65) que puede formar una cavidad
en un área de tratamiento escogida. Al igual que la herramienta 10,
la herramienta 66 consta de un catéter (68) que posee una agarradera
(70) (ver Fig. 20) en su extremo proximal (72) para facilitar el
agarre y manipulación del catéter.
El catéter (68) posee una estructura formadora
de cavidades (74) de movimiento lineal en su extremo distal (76).
En el dispositivo ilustrado, la estructura (56/86?) consta de una
cuchilla generalmente rígida (78), que sobresale en un ángulo
lateral al extremo distal (76) (ver Figs. 17 y 21). La cuchilla (78)
puede hacerse de acero inoxidable o plástico vaciado o moldeado.
Una sonda (80) es llevada por un carril interior
(82) que se encuentre dentro del catéter (68) (ver Figs. 18 y 19).
El carril (82) se extiende a lo largo del eje del catéter (68). La
sonda (80) está libre para moverse en una vía lineal posterior
(flecha A en la Fig. 20) y una vía lineal anterior (fecha F en la
Fig. 21) dentro del carril (82) (flecha R en la Fig. 17).
El extremo más distante de la sonda (80) está
acoplado a la cuchilla (78). El extremo más cercano de la sonda
(80) posee un botón de control (84). Mediante la rotación del botón
de control (84), el médico hace girar la cuchilla (78) entre ambos
extremos y la posición no operativa, mostrada en las Figs. 19 y 20,
y una posición operativa, mostrada en las Figs. 17, 18 y 21. Cuando
se encuentra en la posición no operativa, el médico puede empujar o
halar el botón de control (84) para mover la cuchilla (78) en una
vía lineal dentro del catéter (ver Fig. 20). Al empujar el botón de
control (84), el médico puede mover la cuchilla (78) hacia fuera del
catéter (68), donde puede hacer girar ésta hasta alcanzar la
posición operativa, (ver Fig. 21). Cuando se encuentra en la
posición operativa, al empujar y halar el botón de control (84) se
acciona la cuchilla en golpes lineales contra la masa de tejido
circundante.
Al usarse, el catéter (68) es también llevado
mediante deslizamiento y rotación hacia dentro de la funda guía o
cánula (34), de la misma manera que la mostrada en la Fig. 4. El
médico puede deslizar libremente el catéter (68) axialmente dentro
de la funda guía (34) para desplegar la herramienta (56) en el área
de tratamiento seleccionada. Al ser desplegada en el área que se
trabajará, el médico puede desplegar la cuchilla (78) en el estado
operativo fuera del catéter (68) y deslizar la cuchilla (78) a lo
largo del tejido en una vía lineal. El movimiento lineal de la
cuchilla (78) a lo largo del tejido corta el tejido. El médico
también puede hacer girar tanto el catéter (68) dentro de la funda
guía (34), como la cuchilla (78) dentro del catéter (68) para
ajustar la orientación y el trayecto de la cuchilla (78).
La cuchilla (78) puede poseer uno o más
marcadores radiológicos (86), según lo descrito anteriormente, para
permitir la visualización radiológica de la cuchilla (78) dentro del
área de tratamiento seleccionada. Los datos (88) sobre la sonda
(80) también pueden permitir que el médico visualice aproximadamente
la extensión de movimiento lineal o rotacional de la cuchilla (78).
El extremo distal (76) del catéter (68) también puede poseer uno o
más marcadores (86).
La Fig. 22 muestra una herramienta de movimiento
lineal (90) que puede formar una cavidad en un área de tratamiento
seleccionada. La herramienta (90) está físicamente construida de la
misma forma que la herramienta de movimiento lineal (66) que se
acaba de describir, por lo que se han asignado números de referencia
comunes.
Sin embargo, en la herramienta (90) mostrada en
la Fig. 22, el extremo más distante de la sonda (80) posee, no una
cuchilla (78), sino un transmisor (92) que puede transmitir energía
que corta el tejido (mostrado por las líneas 100 en la Fig. 22). Un
conector (94) conecta el transmisor (92) a la fuente (96) de
energía, mediante un controlador de energía apropiado (98).
El tipo de energía (100) que el transmisor (92)
propaga para cortar el tejido en el área de tratamiento puede
variar. Por ejemplo, el transmisor (92) puede propagar energía
ultrasónica en frecuencias harmónicas apropiadas para cortar el
tejido determinado. Alternativamente, el transmisor (92) puede
propagar energía de láser a una frecuencia apropiada para cortar el
tejido.
Según lo descrito anteriormente, el extremo más
cercano de la sonda (80) incluye un botón de control (84). Al usar
el botón de control (84), el médico puede mover el transmisor (92)
en una vía lineal (flechas A y F en la Fig. 22) entre una posición
recogida, alojado dentro del catéter (68) - como la cuchilla (78)
mostrada en la Fig. 20 -, y una gama de posiciones que se extienden
fuera del catéter (68) según se muestra en la Fig. 22.
Como también se describió anteriormente, al
usarse el catéter (68) de la herramienta 90, es llevado por
deslizamiento y rotación al interior de la funda guía o cánula
(34). El médico desliza el catéter (68) axialmente dentro de la
funda guía (34) para el despliegue de la herramienta (90) en el área
de tratamiento escogida. Al ser desplegada en el área escogida, el
médico opera el botón de control (84) para mover y hacer girar
linealmente el transmisor (92) para alcanzar la posición deseada en
el área de tratamiento escogida. El médico también puede hacer
girar el catéter (68) y así ajustar posteriormente la ubicación del
transmisor (92).
El transmisor (92) o sonda (80) puede portar uno
o más marcadores radiológicos (86), como se ha descrito previamente,
para permitir la visualización radiológica de la posición del
transmisor (92) dentro del área de tratamiento escogida. Los datos
(88) sobre la sonda (80) también pueden permitir al médico calcular
visualmente la posición del transmisor (92). El extremo distal (76)
del catéter (68) también puede poseer uno o más marcadores (86).
El uso de varias herramientas (10 - Figs. 1 a
4), (38 - Figs. 5 a 8), 138 - Figs. 9 a 11), (106 - Figs. 12 a 15),
(66 - Figs. 17 a 21) y (90 - Fig. 22) será descrito a continuación
en el contexto de despliegue en una vértebra humana (150).
La Fig. 23 muestra la vértebra (150) en una
vista coronal (superior), y la Fig. 24 muestra la vértebra (150)
en una vista lateral (de lado). Debe apreciarse, sin embargo, que la
herramienta no está limitada en su aplicación a la vértebra. Las
herramientas 10, 38, 138, 106, 66 y 90 pueden ser desplegadas
igualmente con la misma eficacia en los huesos largos y otros tipos
de huesos.
Como muestran las Figs. 23 y 24, la vértebra
(150) incluye el cuerpo de la vértebra (152), el cual se extiende
por la cara anterior de la vértebra (152) (o sea, dorsales o del
pecho). El cuerpo de la vértebra (152) incluye un exterior formado
de hueso cortical compacto (158). El hueso cortical (158) rodea un
volumen interior de hueso reticulado esponjoso (160) - también
llamado hueso medular o trabecular.
El cuerpo vertebral (152) tiene la forma de un
disco oval. Según muestran las Figs. 23 y 24, se puede lograr el
acceso al volumen interior del cuerpo de la vértebra (152), por
ejemplo, barrenando un portal de acceso (162) a través de la pared
del cuerpo de la vértebra (152), lo cual es llamado un acceso
postero-lateral. El portal (162) para el
acercamiento postero-lateral entra por una pared
posterior del cuerpo (152) y se extiende en un ángulo hacia adentro
del interior del cuerpo (152). El portal (162) puede efectuarse, ya
sea mediante un procedimiento de cirugía de mínimo acceso o con un
procedimiento abierto.
Alternativamente, el acceso al volumen interior
puede lograrse barrenando un portal de acceso a través de cualquier
pedículo (164) (identificado en la Fig. 23). Este es llamado un
acercamiento transpedicular. Es el médico quien decide en última
instancia cuál vía de acceso es la indicada.
Según muestran las Figs. 23 y 24, la funda guía
(34) (mostrada anteriormente en la Fig. 4), está ubicada en el
portal de acceso (162). Bajo monitoreo radiológico o de TA, una de
las herramientas siguientes seleccionadas: 10, 38, 66 ó 90, puede
ser introducida a través de la funda guía (34).
Cuando, por ejemplo, se usa la herramienta de
asa (10), si la estructura de asa (20), está desplegada, es plegada
por la funda guía (34) (según se muestra en la Fig. 4), o recogida
dentro del catéter (12) (según muestra la Fig. 2) durante el paso
a través de la funda guía (34).
Con referencia a la Fig. 25, cuando la
herramienta de asa (10) es desplegada fuera de la funda guía (34) en
el hueso esponjoso (160), el médico opera el controlador (30) de la
forma descrita previamente para obtener la dimensión deseada para
la estructura de asa (20), la cual puede calibrarse mediante
monitoreo radiológico usando los marcadores (36) que se encuentran
en la pizarra. El médico hace girar manualmente la estructura de
asa (20) a través del hueso esponjoso que la rodea (160) (como
indican las flechas R en la Fig. 25). La estructura de asa en
rotación (20) corta el hueso esponjoso (160) formando de esta manera
una cavidad (C). Una sonda de succión (102), también desplegada a
través de la funda guía (34) extrae el hueso esponjoso cortado por
la estructura de asa (20). Alternativamente, el catéter (12) también
puede incluir un lumen interior (128) (según se muestra en la Fig.
16) para servir como sonda de succión, así como para llevar el
líquido de enjuague hacia la cavidad a medida que ésta se va
formando.
La rotación y operación sincronizada del
controlador (30) para aumentar las dimensiones de la estructura de
asa (20) durante el proceso le permite al médico lograr crear una
cavidad (C) de la dimensión deseada. Dimensiones representativas
para una cavidad (C), serán discutidas más detalladamente
posteriormente.
Cuando, por ejemplo, la herramienta de brocha
(38) es usada, el médico preferiblemente recoge las cerdas (46)
durante su paso por la funda guía (34), de la forma mostrada en la
Fig. 6.
En referencia a la Fig. 26, cuando la
herramienta de brocha (38) es desplegada en el hueso esponjoso (160)
libre de la funda guía (34), el médico hace avanzar las cerdas (46)
a una distancia deseada (según se muestra en la Fig. 5), ayudado
por el monitoreo radiológico de los marcadores (62), o por los datos
(32) descritos previamente o por ambos. El médico conecta el eje
transmisor (40) al motor (56) para hacer girar las cerdas (46),
creando la estructura de brocha (44). Como muestra la Fig. 26, la
estructura de brocha en rotación (44) corta el hueso esponjoso
(160) y forma una cavidad (C). La sonda de succión (102) - o un
lumen (128) en el árbol motor (40) como se muestra en la Fig. 16 -
introduce un fluido de enjuague (con un anticoagulante si se desea)
y extrae el hueso esponjoso cortado por la estructura de brocha
(44). Mediante la detención periódica de la estructura de brocha
(44) y la operación del controlador (60) - descrita previamente -
para aumentar la extensión frontal de las cerdas (46), el médico
puede crear una cavidad (C) que posea las dimensiones deseadas.
Cuando, por ejemplo, una de las herramientas
comparativas de movimiento lineal (66 y 90) es utilizada, el médico
preferiblemente extrae la cuchilla (78) o el transmisor (92) dentro
del catéter (68) de la manera mostrada en las Figs. 20, hasta que
el extremo distal (76) del catéter (68) esta libre de la funda guía
(34).
Con referencia a la Fig. 27, al usar la
herramienta de cuchilla (66), el médico acciona la sonda (80) hacia
delante (flecha F) y hacia atrás (flecha A) para mover la cuchilla
(78) en una vía lineal a través del hueso esponjoso (169). La
cuchilla (78) raspa y corta el hueso esponjoso (160) a medida que va
avanzando en su trayecto, y éste es extraído por la sonda de
succión (102). De esta manera se forma una cavidad (C). La rotación
sincronizada (flecha R) y el movimiento lineal (fechas F y A) de la
cuchilla (78) permiten al médico crear una cavidad (C) que posea las
dimensiones
deseadas.

Con respecto a la Fig. 28, usando la herramienta
comparativa transmisora de energía (90), el médico hace girar
(flecha R) y empuja o hala la sonda (80) (flechas F y A) para
posicionar el transmisor de energía (92) en los lugares deseados en
el hueso esponjoso (160). Los marcadores (86) ayudan en el proceso
de ubicación. La transmisión por el transmisor (92) de la energía
seleccionada corta el hueso esponjoso (160) para ser extraído por
la sonda de succión (102). De esta manera se forma una cavidad (C).
Mediante la manipulación adecuada del transmisor (92), el médico
logra formar una cavidad (C) que posea las dimensiones deseadas.
Una vez formada la cavidad (C) deseada, la
herramienta seleccionada (10, 38, 66, 90, 106 ó 138) es extraída a
través de la funda guía (34). Según muestra la Fig. 29, otra
herramienta (104) puede ser desplegada ahora a través de la funda
guía (34) dentro de la cavidad (C) formada. La segunda herramienta
(104) puede, por ejemplo, realizar un procedimiento diagnóstico.
Alternativamente, la segunda herramienta (104) puede efectuar un
procedimiento terapéutico, por ejemplo, suministrando un material
(106) en la cavidad (C), tal como, por ejemplo, cemento óseo,
material para homo trasplantes, sustituto sintético de hueso o algún
material que fluya y al depositarse se endurezca. Más detalles de
la inyección de tales materiales (106) en la cavidad (C) con fines
terapéuticos se encuentran en las Patentes EU 4, 969,888 y 5,
108,404 y en la solicitud de Patentes US pendiente Serie No.
08/485,394.
El tamaño de la cavidad (C) varía según el
procedimiento terapéutico o diagnóstico realizado.
Al menos cerca de un 30% del volumen del hueso
esponjoso necesita ser extraído en casos en los que la enfermedad
ósea causante de fractura (o riesgo de fractura) es la pérdida de la
masa de hueso esponjoso (como en la osteoporosis). El rango de
preferencia está aproximadamente entre el 30% y el 90% del volumen
del hueso esponjoso. La extracción de menos volumen de hueso
esponjoso puede dejar demasiado hueso esponjoso dañado en la zona de
tratamiento. El hueso esponjoso dañado permanece débil y puede
colapsar posteriormente, causando fractura, a pesar del
tratamiento.
Sin embargo, hay momentos en que una cantidad
inferior de hueso esponjoso es extraída según lo indicado. Por
ejemplo, cuando el hueso enfermo que está siendo tratado es
localizado, como en una necrosis vascular, o donde la pérdida de
sangre local está matando al hueso en un área limitada, la
herramienta seleccionada (10, 38, 66, 90, 106 ó 138) puede extraer
un volumen menor del total del hueso. Esto es debido a que el área
enferma necesitada de tratamiento es más pequeña.
Otra excepción se encuentra en el uso de una
herramienta seleccionada (10, 38, 66, 90, 106 ó 138) para mejorar
la inserción de materiales sólidos en estructuras definidas, como el
hidróxido de fluorofosfato de calcio y componentes usados en
reemplazo total de articulaciones. En estos casos, la cantidad de
tejido que necesita ser extraído está definido por el tamaño del
material que está siendo insertado.
Otra excepción establece el uso de una
herramienta seleccionada (10, 38, 66, 90, 106 ó 138) en los huesos
para crear cavidades con el fin de proporcionar sustancias
terapéuticas, como se muestra en la solicitud pendiente de patente
EU, serie No. 08/485,394. En este caso, el hueso esponjoso puede o
no estar enfermo o afectado adversamente. El hueso esponjoso sano
puede sacrificarse mediante la compresión significativa para mejorar
la administración del medicamento o factor de crecimiento que tiene
un importante propósito terapéutico. En esta aplicación, el tamaño
de la cavidad es escogido por la cantidad de sustancia terapéutica
que se desea administrar. En este caso, el hueso con el medicamento
dentro está apoyado mientras el medicamento trabaja, y el hueso
sana mediante el enyesado exterior o los dispositivos corrientes de
fijación interna o externa.
Un uso único de cualquiera de las herramientas
(10, 38, 66, 90, 106 ó 138) establece un contacto con el hueso
cortical y esponjoso. Este contacto puede dañar las herramientas,
creando zonas localizadas de debilidad que pueden escapar a la
detección. La existencia de las regiones localizadas de debilidad
puede impredeciblemente causar fallos estructurales generales
durante un uso subsiguiente.
Además, la exposición a la sangre y al tejido al
usarse tan solo una vez, puede atrapar los componentes biológicos
sobre o dentro de las herramientas. A pesar de la limpieza y
posterior esterilización, la presencia de los componentes
biológicos atrapados puede conducir a reacciones pirogénicas
inaceptables.
Como resultado, posterior a su primer uso, las
herramientas pueden no cumplir las especificaciones establecidas de
comportamiento y esterilización. Los efectos de la fuerza y el daño
causado durante la vez que fueron usadas, unido a la posibilidad de
reacciones pirogénicas, incluso después de la
re-esterilización, justifican razonablemente que se
imponga una restricción para que las herramientas de despliegue en
los huesos sean usadas una sola vez.
Para proteger a los pacientes de las
consecuencias potenciales adversas ocasionadas por el uso múltiple,
las cuales incluyen transmisión de enfermedades o tensión del
material e inestabilidad o disminución o comportamiento
impredecible, cada herramienta de uso único (10, 38, 66, 90, 106 ó
138) está envasada en un equipo estéril (500) (ver Figs. 30 y 31)
previamente a su despliegue en el hueso.
Como muestran las Figs. 30 y 31, el equipo (500)
incluye una bandeja interior (508). La bandeja (508) sostiene la
herramienta formadora de cavidades en particular (genéricamente
designada 502) en una posición desplegada horizontal durante la
esterilización y almacenamiento antes de su primer uso. La bandeja
(508) puede estar formada por una o más lengüetas de cartón
troquelado o material plástico formado térmicamente. La bandeja
(508) incluye una o más lengüetas (510) espaciadas, que sostienen la
herramienta (502) en la posición desplegada horizontal deseada.
El equipo (500) incluye una envoltura interna
(512), la cual está sellada periféricamente mediante calor o algo
similar, para aislar la bandeja (508) del contacto con el medio
ambiente externo. Un extremo de la envoltura interna (512) incluye
un sello de apertura rápida (514) (ver Fig. 31) para proporcionar un
acceso rápido a la bandeja (508) en el momento de uso, lo cual
preferiblemente ocurre en un medio estéril, tal como el de un salón
de operaciones.
El equipo (500) también incluye una envoltura
externa (516), la cual está preferiblemente sellada por calor o
algo similar, para cubrir la envoltura interna (512). Un extremo de
la envoltura externa (516) incluye un sello de apertura rápida
convencional (518) (ver Fig. 31), para proporcionar acceso a la
envoltura interna (512), el cual puede retirarse de la envoltura
externa (516) previo al uso inminente de la herramienta (502), sin
comprometer la esterilidad de la herramienta (502) propiamente.
Cada una de las envolturas, la interna y la
externa (512 y 516) (ver Fig. 31), incluye una lámina en su parte
superior (520) y una lámina en la parte inferior (522). En la
realización ilustrada, la lámina superior (520) está hecha de una
película plástica transparente, como el polietileno o el material
MYLAR™, para permitir la identificación visual del contenido del
equipo (500). La lámina inferior (522) está hecha de un material que
es permeable al gas de esterilización EtO, por ejemplo, el material
plástico TYVEC™ (disponible en DuPont).
El equipo estéril (500) también tiene una
etiqueta o encarte (506), que incluye la declaración "Para usar
con solo un paciente" (o alguna frase similar en idioma
comparativo) para prevenir contra la reutilización de los
contenidos del equipo (500). La etiqueta (506) también instruye
preferiblemente al médico o a quien va a utilizar la herramienta a
desechar la herramienta (502) y todo el contenido del equipo (500)
al ser usado según los procedimientos aplicables a los desechos
biológicos. La presencia de la herramienta (502) empacada en el
equipo (500) le verifica al médico o a quien la va a utilizar que la
herramienta (502) cumple las normas de comportamiento y esterilidad
y no ha sido expuesto a un uso previo, así como que tendrá la
configuración deseada al ser desplegada para su uso.
El equipo (500) también preferiblemente incluye
instrucciones para su uso (524), las cuales instruyen al médico con
respecto al uso de la herramienta (502) para crear una cavidad en un
hueso esponjoso de las formas descritas anteriormente. Por ejemplo,
las instrucciones (524) instruyen al médico a desplegar y manipular
la herramienta (502) dentro del hueso para cortar el hueso
esponjoso y formar una cavidad. Las instrucciones (524) también
instruyen al médico a llenar la cavidad con un material, por
ejemplo, cemento óseo, material para homo trasplantes, sustituto
sintético para huesos, un medicamento, o un material que fluya y que
al depositarse se endurezca.
Las características de la invención se describen
en las siguientes reivindicaciones.
Claims (12)
1. Un sistema (10, 38, 138) para crear una
cavidad (C) en un hueso esponjoso (160) que consta de:
- una cánula (34, 144) que posee un eje que crea una vía percutánea hacia el hueso;
- un vástago (12, 40, 142) que tiene una porción extrema distal (16, 42) que posee elementos alongados (22, 46, 140) que pueden desplegarse desde el vástago (12, 40, 142) para crear una estructura para formar una cavidad (20, 44, 140),
- estando el vástago calibrado (12, 40, 142) para pasar por la cánula (34, 144) hacia el hueso antes del despliegue de los elementos alongados (22, 46, 140);
- un controlador (30, 60) para extender los elementos alongados (22, 46, 140) desde el vástago (12, 40, 142) in situ dentro del hueso esponjoso (160) para crear la estructura formadora de cavidades (20, 44, 140) y
- siendo el vástago desplazable en relación con el eje de la cánula (34, 144) para mover la estructura formadora de cavidades (20, 140) al ser desplegada dentro del hueso esponjoso (160) para formar una cavidad (C) en el hueso esponjoso (160).
2. Un sistema (10, 38, 138) según la
Reivindicación 1 en el cual los elementos alongados (22, 46, 140)
forman un asa (20).
3. Un sistema (10, 38, 138) según la

esponjoso (160) mediante el despliegue de la cánula (34, 144)
percutáneamente para establecer una vía de acceso hacia el hueso,
introduciendo el vástago (12, 40, 142) mediante el movimiento hacia
dentro y a lo largo del eje de la cánula (34, 144) para colocar los
elementos alongados (22, 46, 140) dentro del hueso esponjoso (160),
desplegando la estructura formadora de cavidades (20, 44, 140) in
situ dentro del hueso esponjoso (160) desde el vástago (12, 40,
142) y moviendo el vástago (12, 40, 142) para formar una cavidad (C)
en el hueso esponjoso (160).
12. Un equipo (500) según la Reivindicación 11,
que además consta de una herramienta (104) calibrada para el paso a
través de la cánula (34, 144), pudiendo la herramienta (104)
suministrar un material de relleno (106) hacia la cavidad (C), e
instrucciones (524) para introducir la herramienta (104) mediante el
movimiento hacia dentro y a lo largo del eje de la cánula (34, 144),
y administrar material de relleno (106) a través de la herramienta
(104) hacia la cavidad (C).
