Description
Pflaster zur Behandlung von Nagelmykosen
Die Erfindung betrifft ein Pflaster zur Behandlung von Nagelmy¬ kosen.
Pilzerkrankungen der Nägel sind eine weit verbreitete Erschei¬ nung. In der medizinischen Fachliteratur findet man Hinweise, daß 8 % der Bevölkerung daran leiden. Hierbei spielt eine Rolle, daß in feuchter Umgebung eine relativ hohe Infektionsgefahr ge¬ geben ist (beispielsweise in Badeanstalten oder beim Tragen von Schuhen oder Schutzhandschuhen) . Es handelt sich um eine Erkran¬ kung, die nur schlecht abheilt und einer Behandlung bedarf.
Als Behandlungsmethode gibt es die Möglichkeit, den befallenen Nagel nach entsprechender Vorbehandlung abzulösen, was aber von vielen Betroffenen wegen der damit verbundenen Unannehmlichkei¬ ten abgelehnt wird. Außerdem sind in den letzten Jahren moderne Antimykotika entwickelt worden, die auch nach oraler Einnahme gegen Nagelpilzerkrankungen wirken. Die Behandlung muß jedoch über eine lange Zeit von einigen Monaten durchgeführt werden, wobei bei der oralen Behandlung Nebenwirkungen auftreten, da der ganze Organismus mit dem Medikament in Kontakt kommt. Die Folge ist, daß der Patient die Therapie abbricht und somit die Pilzer¬ krankung nicht abheilt .
Es wurden auch Lösungen und sogar Nagellack entwickelt, die pilzhemmende Wirkstoffe enthalten, um sie nur lokal am Ort der Erkrankung freizusetzen, und die damit die Nebenwirkungen der oralen Therapie nicht aufweisen. Allerdings liegt die Pilzbe¬ siedlung im Nagelbett unter der Nagelplatte (vgl. Charlet, Kos¬ metik für Apotheker, Wissenschaf liche Verlagsgesellschaft mbH, Stuttgart 1989) , durch die der Wirkstoff zunächst hindurchgelan¬ gen muß. Lösungen werden zu rasch abgespült, beispielsweise beim Waschen oder Duschen, und auch ein Nagellack kann nicht genügend Wirkstoff und Permeationsförderer bzw. Enhancer aufnehmen, also Substanzen, die den Wirkstoff durch den Nagel zum Ort der Pilz¬ erkrankung schleusen. Insofern ist eine wirksame lokale Behand¬ lung bis heute ein Problem.
Erfindungsgemäß wird nun ein Pflaster zur Behandlung von Nagel- mykosen vorgeschlagen, das durch Clotrimazol und/oder Bifonazol als antimykotische Substanz (Wirkstoff) gekennzeichnet ist.
Bevorzugte Ausführungsformen ergeben sich aus den Unteransprüchen.
Es handelt sich dabei um ein Pflaster mit einem Antimykotikum, das auf den befallenen Nagel aufgeklebt wird. Der pilzhemmende Wirkstoff kann dazu in eine Polymermatrix eingebettet werden, die beispielsweise aus einem Acrylat-Mischpolymer besteht. Ferner können in die Matrix Substanzen eingearbeitet werden, die den Wirkstoff aus der Matrix heraus- und/oder durch den Nagel hindurchbringen können, also sogenannte Permeationsförderer oder Enhancer, die speziell proteinreiche Barrieren der Nägel beein¬ flussen, beispielsweise das Keratin.
Darüberhinaus können Zusätze zugesetzt werden, beispielsweise Aerosil bzw. für eine Acrylat-Copolymerisation übliche Ver¬ netzer, die die Klebeeigenschaften regulieren. Damit ist das System einschichtig, was den Herstellungsprozess vereinfacht. Die Matrix kann mit einer Abdeckfolie versehen sein, damit durch
die klebenden Eigenschaften der Matrix keine Beeinträchtigung beim Tragen erfolgt und die Matrix mit den in ihr enthaltenen Substanzen beim Waschen nicht abgetragen wird.
Ein weiterer Vorteil des erfindungsgemäßen Pflasters ist in der Möglichkeit zu sehen, ausreichend viel Wirkstoff in die Matrix einzubetten, um den Fluß des Wirkstoffs durch den Nagel lange aufrechtzuerhalten, was eine zur Pilzbekämpfung wichtige Eigen¬ schaft darstellt.
Ein erfindungsgemäßes Pflaster kann 1,1 bis 1,7 mg Wirkstoff pro cm2 enthalten, so daß auf den Nagel je nach individueller Größe in der Regel eine Dosis von 3 bis 6 mg appliziert wird. Das er¬ findungsgemäße Pflaster kann dabei so konzipiert werden, daß das Pflaster mehrere Tage auf dem Nagel verbleiben kann, was der Pa- tienten-Compliance entgegenkommt und die Behandlungskosten senkt .
Bei der Herstellung eines erfindungsgemäßen Pflasters wird be¬ sonderes Augenmerk auf die Auswahl der Polymere , der Per¬ meationsförderer, der Klebeeigenschaft und der Abdeckfolie ge¬ legt . Die Polymere sollen eine ausreichende Menge des Wirk¬ stoffs in gelöster Form aufnehmen' und die Permeationsförderer müssen die Permeation des Wirkstoffs durch den Nagel verbessern. Die wirkstoffhaltige Klebermatrix weist erfindungsgemäß eine starke Klebkraft auf dem Nagel auf, ist nach dem Tragen jedoch abziehbar. Erfindungsgemäß wird eine kosmetisch einwandfreie Ab¬ deckfolie vorgesehen, die eine genügende Flexibilität aufweist, um sich dem Nagel gut anzupassen, auch wenn der Nagel eine unre¬ gelmäßige Oberfläche besitzt. Die Abdeckfolie besteht vorzugs¬ weise aus Weich-PVC, das insbesondere hautfarben pigmentiert ist. Auf diese Folie kann gegebenenfalls handelsüblicher kosme¬ tischer Nagellack aufgetragen werden, ohne daß dadurch die The¬ rapie der Nagelmykose beeinflußt wird.
Nachstehend wird die Erfindung durch Figuren und Beispiele näher erläutert. Es zeigen:

Beispiel 1
Zur Herstellung von Nagelpflastern mit einer Gesamtfläche von 1.800 cm2 werden 3,06 g Clotrimazol in ein 125-ml-Schraubglas eingewogen und 35,45 ml Ethanol hinzugegeben. Man rührt mit einem Magnetrührer, bis Lösung erfolgt ist. Anschließend werden 1,0 g Milchsäure, 1,0 g Aerosil, 0,05 g Aluminium-acetylacetonat und 62,8 g einer 41,5-proz. Durotak 380-2819-Lösung hinzugefügt. Man rührt etwa 30 min lang. Die Lösung wird auf eine Polyester¬ folie von 75 μm Dicke aufgetragen, so daß die Naßschichtdicke der Lösung 500 μm beträgt. Die Lösungsmittel werden durch 1- stündiges Vortrocknen im Trockenschrank bei 40 °C und 1-stündi¬ ges Haupttrocknen bei 80 °C entfernt. Anschließend wird nach dem Abkühlen mit einem PVC-Film (Guttagena WK68) von 80 μm Dicke ka¬ schiert. Die einzelnen Nagelpflaster werden danach mit einer Fläche von 4,5 cm ausgestanzt. Beim Applizieren schneidet der Patient das Pflaster auf die jeweilige Größe des Nagels zu.
Beispiel 2
Zur Herstellung von 1.800 ciri Pflasterfläche werden 3,06 g Clo¬ trimazol soweit möglich in Ethanol unter Rühren gelöst . Man führt 0,9 g Dimethylsulfoxid zu, rührt erneut, gibt sodann 30 g einer 41-proz. Lösung von Durotak 380-2819 und 80,7 g einer 35- proz . Lösung von Durotak 380-1781 hinzu und rührt bis zu Homoge¬ nität. Danach wird auf eine beidseitig silikonisierte Polyester¬ folie (100 μm Dicke) ausgestrichen, so daß die Naßfilmdicke 500 μm beträgt. Nach dem Trocknen über 2 h im Trockenschrank bei 60 °C und Lagerung bei 25 °C über 12 h wird mit einem Polyolefin- Film einer Dicke von 50 μm (Cotran 9722) kaschiert. Anschließend werden mit einer Stanze 4,5 cm2 große Pflaster ausgestanzt. Beim
Applizieren schneidet der Patient das Pflaster auf die jeweilige Größe des Nagels zu.
Beispiel 3
Es werden mehrere Enhancer in vitro (modifizierte Franzzellen) durch Permeationsmessungen an Rinderhufschnitten von ca. 0,4 mm Dicke untersucht. Das Hornmaterial von Rinderhufen ist wie der menschliche Nagel aus Hartkeratin aufgebaut. Das Ergebnis ist anhand von Flux-Werten (ermittelt im Steady State) in der fol¬ genden Tabelle dargestellt. Besonders effektiv erweist sich bei dieser Methode Milchsäure als Permeationsenhancer.

Beispiel 4
Grafik 1 ist die Freisetzung in vitro über 5 Tage aus einem Pflaster von 4,5 cm" und mit einem Gehalt von 4,5 mg Clotrimazol zu entnehmen.
Ferner wird gezeigt, daß der Wirkstoff, der mit dem erfindungs¬ gemäßen Pflaster auf einen mykotischen, vor der mechanischen Ex¬ traktion unbehandelten Nagel, geklebt wird, in vitro (modifi¬ zierte Franzzelle) eine Permeation durch die Nagelplatte in das Akzeptormedium über einen Zeitraum von 8 Tagen erreicht; vgl. Grafik 2. Die Konzentration im Akzeptormedium liegt deutlich über der minimalen Hemmkonzentration des Wirkstoffs auf Derma- tophyten, den häufigsten Erregern der Nagelmykose.
Claims
1. Pflaster zur Behandlung von Nagelmykosen, gekennzeichnet durch Clotrimazol und/oder Bifonazol als antimykotisch wirksame Substanz (Wirkstoff) .
2. Pflaster nach Anspruch 1, gekennzeichnet durch eine Abdeckfo¬ lie, eine Polymermatrix mit dem Wirkstoff und gegebenenfalls eine die Permeation des Wirkstoffs durch den Nagel fördernde Substanz (Permeationsförderer) sowie eine Schutz¬ schicht .
3. Pflaster nach Anspruch 2, gekennzeichnet durch eine Polymer¬ matrix auf Acrylatbasis, erhältlich durch radikalische Polymerisation von 2-Ethylhexylacrylat, Butylacryla , Methyl- acrylat, Acrylsäure, Vinylacetat, Hydroxyethylacrylat und/oder Glycidylmethacrylat oder Mischungen daraus sowie gegebenenfalls anderen Substanzen, insbesondere Aerosil, Vernetzern und/oder Weichmachern, in einer Menge von bis zu 5 % (auf Gewichtsbasis und bezogen auf die Polymermatrix ohne Wirkstoff und Permea¬ tionsförderer) .

Pflaster nach Anspruch 3, gekennzeichnet durch eine Polymer¬ matrix auf Acrylatbasis, erhältlich durch radikalische Polyme- risation von 50 bis 80 % 2-Ethylhexylacrylat, 5 bis 20 % Bu- tylacrylat, 20 bis 40 % Methylacrylat, 2 bis 8 % Acrylsäure, 2 bis 30% Vinylacetat, 0,5 bis 3 % Hydroxyethylacrylat und 0,02 bis 0,1 % Glycidylmethacrylat (jeweils auf Gewichtsbasis und bezogen auf die Polymermatrix ohne Wirkstoff und Permeationsfδrderer) .
5. Pflaster nach Anspruch 3, gekennzeichnet durch eine Polymer¬ matrix auf Acrylatbasis für ein Pflaster von 3 bis 6 cm Fläche, erhältlich durch Polymerisation von 70 bis 80 % 2-Ethylhexyl- acrylat, 12 bis 18 % Butylacrylat, 2,8 bis 6,8 % Acrylsäure und 3 bis 7 % Vinylacetat.

Pflaster nach Anspruch 3, gekennzeichnet durch eine Polymer¬ matrix auf Acrylatbasis für ein Pflaster von 3 bis 6 cm Fläche, erhältlich durch Polymerisation von 72 bis 83 % 2-Ethylhexyl- acrylat, 1,5 bis 3,3 % Acrylsäure, 15 bis 25 % Vinylacetat und gegebenenfalls 0,1 bis 0,8 % Aluminium-acetylacetonat und/oder 0,1 bis 0,35 % Titan-acetylacetonat.
7. Pflaster nach einem der Ansprüche 1 bis 6, dadurch gekenn¬ zeichnet, daß als Permeationsfδrderer verwendet werden können Sulfoxide, insbesondere DMSO und Decylmethylsulfoxid; Säure- amide, insbesondere Pyrrolidone, Dimethylformamid und Dimethyl- acetamid; Alkylsulfonate, insbesondere Natriumdodecylsulfat; N- Acetyl-L-cystein; Polyalkohole, insbesondere Propylenglycol; Keratolytica, insbesondere Salicylsäure; Harnstoff; und/oder Milchsäure.
8. Pflaster nach einem der Ansprüche 1 bis 7, dadurch gekenn¬ zeichnet, daß als Permeationsförderer verwendet werden Dimethyl- sulfoxid in einer Konzentration von 1 bis 6 % oder Milchsäure in einer Konzentration von 1 bis 6 % (jeweils Konzentration in der Matrix) .
