Description
- Die vorliegende Erfindung bezieht sich auf ein Arzneimittel zur Prophylaxe und Behandlung von Glaukom. Genauer bezieht sich die vorliegende Erfindung auf ein Arzneimittel zur Prophylaxe und Behandlung von Glaukom, das eine Verbindung mit einer Rho-Kinasehemmaktivität als aktiven Bestandteil umfaßt.
- Stand der Technik
- Glaukom wird durch einen abnormal hohen Innendruck des Augapfels verursacht, wobei der abnormal hohe Druck das Auge trübe werden läßt oder das Auge verletzt, was wiederum das Auge möglicherweise ganz allmählich erblinden läßt. Normalerweise zirkuliert im Augapfel fortwährend das Kammerwasser und hält einen konstanten Augeninnendruck (10–20 mmHg) aufrecht. Der Druck wird durch den Kreislauf des Bluts und Lymphozyten, die Elastizität der Augapfelwand, das Leistungsverhalten der Kontrollnerven und dergleichen aufrechterhalten. Eine Abnormalität bei einem derselben führt zu einem Anstieg des Augeninnendrucks, der Glaukom entwickeln kann.
- Mit der Absicht des Verhinderns eines Ansteigens des Augeninnendrucks oder des Erniedrigens eines Augeninnendrucks, der angestiegen ist, sind zur Prophylaxe und Therapie von Glaukom verschiedene Wirkstoffe verwendet worden. Bekannte Augentropfen zur Therapie von Glaukom schließen sympathische Agonisten wie etwa Epinephrin, Dipivefrin und dergleichen ein. Aufgrund der mydriatischen Wirkung verstärken diese Augentropfen jedoch das Winkelglaukom beim Verabreichen zum Behandeln eines Engwinkelglaukoms und können nicht nur einen akuten Anstieg des Augeninnendrucks, sondern auch Bluthochdruck und eine Pigmentierungsablagerung verursachen. Außerdem verursachen parasympathische Agonisten wie etwa Pilocarpin und dergleichen Nebenwirkungen wie etwa ein Dunkelgesichtsfeld aufgrund einer Miosis und eines Stauungsauges, Iriszysten, hintere Synechia, Katarakt, Netzhautablösung und dergleichen nach einer Langzeitverwendung. Weiterhin werden β-Adrenalinblocker wie etwa Timolol, Pindolol und dergleichen weit verbreitet verwendet, da sie den Augeninnendruck durch Hemmen der Kammerwasserproduktion erniedrigen, ohne auf die Pupillen zu wirken. Ihre Verwendung ist jedoch beschränkt, da von β-Adrenalinblockern berichtet wurde, daß sie sowohl Nebenwirkungen wie etwa ein örtliches Trockengefühl des Auges, allergische Blepharitis, oberflächliche Keratitis und dergleichen als auch systemische Nebenwirkungen wie etwa Bradykardie, Herzversagen, Asthmaanfälle und dergleichen hervorrufen. Diese Nebenwirkungen verhindern die Anwendung der Blocker bei an diesen Symptomen leidenden Patienten. Eine kürzliche Anregung einer den Abfluß des Kammerwassers fördernden Wirkung von α1-Adrenalinblockern schlägt auch die mögliche Verwendung von Bunazosinhydrochlorid und dergleichen als neues therapeutisches Mittel für Glaukom vor (Ikuo Azuma, Folia Ophthalmol. Jpn., 42, 710–714, 1991). Die α1-Adrenalinblocker sind jedoch unvermeidlich mit einer Konjunktivalinjektion und einer Miosis aufgrund ihrer gefäßerweiternden Wirkung verbunden.
- Inzwischen ist von einer Verbindung mit einer Rho-Kinasehemmaktivität berichtet worden, daß sie bei verschiedenen Bluthochdruck-Tiermodellen eine blutdrucksenkende Wirkung zeigt (Masayoshi Uehata et al., Nature 389, 990–994, 1997). Von der Rho-Kinase wurde bestätigt, daß sie in Hornhautepithelzellen vorkommt (Nirmala SundarRaj. et al., IOVS, 39 (7), 1266–1272, 1998). Es ist jedoch unbekannt, ob Rho-Kinase in anderem Augengewebe vorkommt.
- Die pharmazeutische Verwendung der Verbindung mit einer Rho-Kinasehemmaktivität wird in der WO98/06433 offenbart und es wird als Verwendung auf ophthalmologischem Gebiet gelehrt, daß sie bei Retinopathie nützlich ist. Die WO98/06433 offenbart jedoch nicht ihre Brauchbarkeit gegen Glaukom oder eine die Wirkung nahe legende Beschreibung.
- Als Verbindung mit einer Rho-Kinasehemmaktivität ist über eine später anzuführende Verbindung der Formel (I') berichtet worden (WO98/06433). Von der Verbindung der Formel (I') ist bereits bekannt, daß sie als Mittel zur Prophylaxe und Behandlung von Störungen von Kreislauforganen wie etwa Koronar-, Hirn-, Nieren-, peripheren Arterien und dergleichen (z. B. als therapeutisches Mittel bei Bluthochdruck, therapeutisches Mittel bei Angina pectoris, therapeutisches Mittel bei Nieren- und peripherer Kreislaufstörung, Suppressivum von zerebrovaskulärer Kontraktion und dergleichen) brauchbar ist, das wirksam und lang anhaltend ist und auch als Therapeutikum bei Asthma (JP-A-62-89679, JP-A-3-218356, JP-A-4-273821, JP-A-5-194401, JP-A-6-41080 und WO95/28387).
- Die JP-A-8 198 876 beschreibt trans-N-(1HN-Pyrazolo[3,4-b]pyridin-4-yl)-4-guanidinomethylcyclohexancarbonsäureamid-1-methansulfonsäuresalz-1-hydrat als therapeutisches Mittel bei Glaukom.
- Diese Verbindungen der Formel (I') werden jedoch nicht als bei Glaukom brauchbar offenbart und es gibt keine eine derartige Brauchbarkeit nahe legende Beschreibung.
- Offenbarung der Erfindung
- Die vorliegende Erfindung beabsichtigt das Lösen der vorstehend angeführten Probleme und stellt ein neues Arzneimittel zur Prophylaxe und Behandlung von Glaukom bereit, das in der prophylaktischen und therapeutischen Wirkung bei Glaukom überlegen ist.
- Die Erfinder haben eingehende Untersuchungen durchgeführt und fanden, daß eine Verbindung mit Rho-Kinasehemmaktivität auch eine den Augeninnendruck senkende Wirkung, eine den Blutstrom in den Papillen verbessernde Wirkung und eine den wäßrigen Abfluß fördernde Wirkung aufweist und daß sie zur Prophylaxe und Behandlung verschiedener Typen Glaukom brauchbar ist, was zum Abschluß der vorliegenden Erfindung führte.
- Demgemäß stellt die vorliegende Erfindung Folgendes bereit:
- (1) Verwendung einer Verbindung mit einer Rho-Kinasehemmaktivität, wobei die Verbindung mit einer Rho-Kinasehemmaktivität eine Amidverbindung der folgenden Formel (I') ist: worin Ra' eine Gruppe der Formel ist, worin R' Wasserstoff, Alkyl oder Cycloalkyl, Cycloalkylalkyl, Phenyl oder Aralkyl ist, das gegebenenfalls einen Substituenten am Ring aufweist, R1 Wasserstoff, Alkyl oder Cycloalkyl, Cycloalkylalkyl, Phenyl oder Aralkyl ist, das gegebenenfalls einen Substituenten am Ring aufweist oder R' und R1 in kombinierter Form zusammen mit dem benachbarten Stickstoffatom eine Gruppe bilden, die einen Heterocyclus mit gegebenenfalls einem Sauerstoffatom, Schwefelatom oder gegebenenfalls substituierten Stickstoffatom im Ring bildet, R2 Wasserstoff oder Alkyl ist, A eine Gruppe der Formel ist, worin R10 und R11 gleich oder verschieden sind und jeweils Wasserstoff, Alkyl, Halogenalkyl, Aralkyl, Hydroxyalkyl, Carboxy oder Alkoxycarbonyl sind oder R10 und R11 eine Gruppe darstellen, die in Kombination Cycloalkyl bildet, und l, m und n jeweils 0 oder eine ganze Zahl 1–3 sind, Rb Wasserstoff, Alkyl, Aralkyl, Aminoalkyl oder Mono- oder Dialkylaminoalkyl ist und Rc ein gegebenenfalls substituierter, stickstoffhaltiger Heterocyclus ist, eines Isomers davon und/oder eines pharmazeutisch annehmbaren Säureadditionssalzes davon zur Herstellung eines Arzneimittels zur Prophylaxe und Behandlung von Glaukom.
- (2) Verwendung von (1), wobei die Verbindung mit einer Rho-Kinasehemm-aktivität (+)-trans-4-(1-Aminoethyl)-1-(4-pyridylcarbamoyl)cyclohexan, (+)-trans-N-(1H-Pyrrolo[2,3-b]pyridin-4-yl)-4-(1-aminoethyl)cyclohexancarbonsäureamid und/oder ein pharmazeutisch annehmbares Säureadditionssalz davon ist.
- (3) Verwendung von (1) oder (2), wobei das pharmazeutisch annehmbare Säureadditionssalz ein Salz ist, bei dem die Säure durch eine anorganische Säure wie etwa Salzsäure, Bromwasserstoffsäure und Schwefelsäure oder eine organische Säure wie etwa Methansulfonsäure, Fumarsäure, Maleinsäure, Mandelsäure, Citronensäure, Weinsäure und Salicyclsäure veranschaulicht wird.
- (4) Verwendung von (1) bis (3), wobei das Säureadditionssalz ein Salzsäure additonssalz ist.
- (5) Verwendung von (1), wobei das Arzneimittel zur Prophylaxe und Behandlung von Glaukom zur lokalen Verabreichung am Auge bestimmt ist.
- (6) Verwendung von (5), wobei sich das Arzneimittel zur Prophylaxe und Behandlung von Glaukom in Form von Augentropfen befindet.

- Bei der vorliegenden Erfindung wird Glaukom durch primäres Offenwinkelglaukom, Normaldruckglaukom, Hypersekretionsglaukom, okuläre Hypertension, akutes Winkelblockglaukom, chronisches Winkelblockglaukom, Plateau-Iris-Syndrom, Glaukom mit kombiniertem Mechanismus, Steroidglaukom, Kapselglaukom, Pigmentglaukom, mit Amyloidose verbundenes sekundäres Glaukom, neovaskuläres Glaukom und malignes Glaukom veranschaulicht.
- Bei der vorliegenden Erfindung bedeutet Rho-Kinase zusammen mit der Aktivierung von Rho aktivierte Serin/Threonin-Kinase. Zum Beispiel werden ROKα (ROCKII: Leung, T. et al., J. Biol. Chem., 270, 29051–29054, 1995), P 160 ROCK (ROKβ, ROCK-I: Ishizaki, T. et al., The EMBO J., 15 (8), 1885–1893, 1996) und andere Proteine mit einer Serin/Threonin-Kinaseaktivität veranschaulicht.
- Die in der vorliegenden Erfindung als aktiver Bestandteil verwendete Verbindung weist eine Rho-Kinasehemmaktivität auf. Diese Verbindungen sind von der Formel (I'). In der vorliegenden Erfindung kann eine Verbindung der Formel (I') mit einer Art Rho-Kinasehemmaktivität allein verwendet werden oder wenn nötig können mehrere Arten Verbindungen verwendet werden.
- In der vorliegenden Beschreibung ist jedes Symbol der Formeln (I') wie folgt definiert.
- Alkyl als R' und R1 ist gerades oder verzweigtes Alkyl mit 1 bis 10 Kohlenstoffatomen, das durch Methyl, Ethyl, Propyl, Isopropyl, Butyl, Isobutyl, sec-Butyl, tert-Butyl, Pentyl, Hexyl, Heptyl, Octyl, Nonyl und Decyl veranschaulicht wird, wobei Alkyl mit 1 bis 4 Kohlenstoffatomen der Vorzug gegeben wird.
- Cycloalkyl als R' und R1 weist 3 bis 7 Kohlenstoffatome auf und wird durch Cyclopropyl, Cyclobutyl, Cyclopentyl, Cyclohexyl und Cycloheptyl veranschaulicht.
- Cycloalkylalkyl R' und R1 ist das, bei dem die Cycloalkylstruktureinheit das vorstehend angeführte Cycloalkyl mit 3 bis 7 Kohlenstoffatomen ist und die Alkylstruktureinheit gerade oder verzweigtes Alkyl mit 1 bis 6 Kohlenstoffatomen (z. B. Methyl, Ethyl, Propyl, Isopropyl, Butyl, Pentyl, Hexyl) ist, das durch Cyclopropylmethyl, Cyclobutylmethyl, Cyclopentylmethyl, Cycohexylmethyl, Cycloheptylmethyl, Cyclopropylethyl, Cyclopentylethyl, Cyclohexylethyl, Cycloheptylethyl, Cyclopropylpropyl, Cyclopentylpropyl, Cyclohexylpropyl, Cycloheptylpropyl, Cyclopropylbutyl, Cyclopentylbutyl, Cyclohexylbutyl, Cycloheptylbutyl, Cyclopropylhexyl, Cyclopentylhexyl, Cyclohexylhexyl und Cycloheptylhexyl veranschaulicht wird.
- Aralkyl als R' und R1 ist das, bei dem die Alkylstruktureinheit Alkyl mit 1 bis 4 Kohlenstoffatomen ist und wird durch Phenylalkyl wie etwa Benzyl, 1-Phenylethyl, 2-Phenylethyl, 3-Phenylpropyl und 4-Phenylbutyl veranschaulicht.
- Der Substituent des gegebenenfalls substituierten Cycloalkyl, Cycloalkylalkyl, Phenyl und Aralkyl am Ring als R' und R1 ist Halogen (z. B. Chlor, Brom, Fluor und Iod), Alkyl (dasselbe wie Alkyl als R' und R1), Alkoxy (gerades oder verzweigtes Alkoxy mit 1 bis 6 Kohlenstoffatomen wie etwa Methoxy, Ethoxy, Propoxy, Isopropoxy, Butoxy, Isobutoxy, sec-Butoxy, tert-Butoxy, Pentyloxy, Hexyloxy), Aralkyl (dasselbe wie Aralkyl als R' und R1) oder Halogenalkyl (Alkyl als R' und R1, das durch 1–5 Halogen substituiert ist und durch Fluormethyl, Difluormethyl, Trifluormethyl, 2,2,2-Trifluorethyl und 2,2,3,3,3-Pentafluorpropyl veranschaulicht wird), Nitro, Amino, Cyan und Azid.
- Die durch R' und R1 in Kombination zusammen mit dem benachbarten Stickstoffatom gebildete Gruppe, die einen Heterocyclus mit gegebenenfalls einem Sauerstoffatom, Schwefelatom oder gegebenenfalls substituierten Stickstoffatom im Ring bildet, ist vorzugsweise ein 5- oder 6gliedriger Ring und daran gebundener Ring. Beispiele davon schließen 1-Pyrrolidinyl, Piperidino, 1-Piperazinyl, Morpholino, Thiomorpholino, 1-Imidazolyl und 2,3-Dihydrothiazol-3-yl ein. Der Substituent des gegebenenfalls substituierten Stickstoffatoms wird durch Alkyl, Aralkyl und Halogenalkyl veranschaulicht. Hierin verwendet sind Alkyl, Aralkyl und Halogenalkyl wie für R' und R1 definiert.
- Alkyl als R2 ist wie für R' und R1 definiert.
- Hydroxyalkyl als R10 und R11 ist gerades oder verzweigtes Alkyl mit 1 bis 6 Kohlenstoffatomen, das durch 1 bis 3 Hydroxy substituiert ist und durch Hydroxymethyl, 2-Hydroxyethyl, 1-Hydroxyethyl, 3-Hydroxypropyl und 4-Hydroxybutyl veranschaulicht wird. Alkyl als R10 und R11 ist wie für R' und R1 definiert; Halogenalkyl und Alkoxycarbonyl als R10 und R11 ist wie für R' und R1 definiert; Aralkyl als R10 und R11 ist wie für R' und R1 definiert und durch R10 und R11 in Kombination gebildetes Cycloalkyl ist dasselbe wie Cycloalkyl als R' und R1.
- Alkyl als Rb ist wie für R' und R1 definiert.
- Aralkyl als Rb ist wie für R' und R1 definiert.
- Aminoalkyl als Rb ist ein gerades oder verzweigtes Alkyl mit 1 bis 6 Kohlenstoffatomen, das durch Amino substituiert ist und wird durch Aminomethyl, 2-Aminoethyl, 1-Aminoethyl, 3-Aminopropyl, 4-Aminobutyl, 5-Aminopentyl und 6-Aminohexyl veranschaulicht.
- Mono- oder Dialkylaminoalkyl als Rb ist mono- oder disubstituiertes Aminoalkyl mit Alkyl, das 1 bis 4 Kohlenstoffatome aufweist und wird durch Methylaminomethyl, Dimethylaminomethyl, Ethylaminomethyl, Diethylaminomethyl, Propylaminomethyl, Dipropylaminomethyl, Butylaminomethyl, Dibutylaminomethyl, 2-Dimethylaminoethyl und 2-Diethylaminoethyl veranschaulicht.
- Wenn der Heterocyclus als Rc ein einzelner stickstoffhaltiger Ring ist, ist er Pyridin, Pyrimidin, Pyridazin, Triazin, Pyrazol oder Triazol und wenn er ein kondensierter Ring ist, wird er durch Pyrrolopyridin (z. B. 1H-Pyrrolo[2,3-b]pyridin, 1H-Pyrrolo[3,2-b]pyridin, 1H-Pyrrolo[3,4-b]pyridin), Pyrazolopyridin (z. B. 1H-Pyrazolo[3,4-b]pyridin, 1H-Pyrazolo[4,3-b]pyridin), Imidazopyridin (z. Wenn diese Ringe einen hydrierten aromatischen Ring bilden, kann das Kohlenstoffatom in dem Ring Carbonyl sein und schließt zum Beispiel 2,3-Dihydro-2-oxopyrrolopyridin, 2,3-Dihydro-2,3-dioxopyrrolopyridin, 7,8-Dihydro-7-oxo-1,8-naphthyridin und 5,6,7,8-Tetrahydro-7-oxo-1,8-naphthyridin ein, wobei Pyridin und Pyrrolopyridin der Vorzug gegeben wird.
- Diese Ringe können durch einen Substituenten wie etwa Halogen, Alkyl, Alkoxy, Aralkyl, Halogenalkyl, Nitro, Amino, Alkylamino, Cyan, Formyl, Acyl, Aminoalkyl, Mono- oder Dialkylaminoalkyl, Azid, Carboxy, Alkoxycarbonyl, Carbamoyl, Alkylcarbamoyl, Alkoxyalkyl (z. B. Methoxymethyl, Methoxyethyl, Methoxypropyl, Ethoxymethyl, Ethoxyethyl, Ethoxypropyl) und gegebenenfalls substituiertes Hydrazino substituiert sein.
- Hierin verwendet schließt der Substituent des gegebenenfalls substituierten Hydrazino Alkyl, Aralkyl, Nitro und Cyan ein, wobei Alkyl und Aralkyl wie für R' und R1 definiert sind und wird durch Methylhydrazino, Ethylhydrazino und Benzylhydrazino veranschaulicht.

- Bevorzugt sind die Verbindungen (80), (109), (110), (112) und (115).
- Die Verbindung der Formel (I') mit einer Rho-Kinasehemmaktivität kann ein pharmazeutisch annehmbares Säureadditionssalz sein, wobei die Säure durch eine anorganische Säure wie etwa Salzsäure, Bromwasserstoffsäure und Schwefelsäure und eine organische Säure wie etwa Methansulfonsäure, Fumarsäure, Maleinsäure, Mandelsäure, Citronensäure, Weinsäure und Salicylsäure veranschaulicht wird. Eine Verbindung mit einer Carboxygruppe kann mit einem Metall wie etwa Natrium, Kalium, Calcium, Magnesium und Aluminium in ein Salz und mit einer Aminosäure wie etwa Lysin in ein Salz umgewandelt werden. Weiter wird ein Monohydrat, Dihydrat, 1/2-Hydrat, 1/3-Hydrat, 1/4-Hydrat, 2/3-Hydrat, 3/2-Hydrat, 6/5-Hydrat und dergleichen von der vorliegenden Erfindung umfaßt.
- Die Verbindung der Formel (I') kann durch ein in zum Beispiel der JP-A-62 89 679, JP-A-3 218 356, JP-A-5 194 401, JP-A-6 41 080, WO95/28387 und WO98/06433 beschriebenes Verfahren synthetisiert werden.
- Wenn die vorstehend angeführte Verbindung mit einer Rho-Kinasehemmaktivität ein optisches Isomer aufweist können ihr Racemat oder cis-trans-Isomere alle in der vorliegenden Erfindung verwendet werden. Diese Isomeren können durch ein herkömmliches Verfahren isoliert werden oder können durch Verwenden von Ausgangsmaterialien des Isomers hergestellt werden.
- Eine Verbindung der Formel (I') mit einer Rho-Kinasehemmaktivität, ein Isomer davon und/oder ein pharmazeutisch annehmbares Säureadditionssalz davon weisen eine den Augeninnendruck erniedrigende Wirkung, die Blutströmung in den Papillen verbessernde Wirkung und den wäßrigen Abfluß fördernde Wirkung bei Säugern einschließlich Mensch, Kuh, Pferd, Hund, Maus und Ratte auf. Daher können sie als Mittel zur Prophylaxe und Behandlung verschiedener Typen Glaukom wie etwa primäres Offenwinkelglaukom, Normaldruckglaukom, Hypersekretionsglaukom, okuläre Hypertension, akutes Winkelblockglaukom, chronisches Winkelblockglaukom, Plateau-Iris-Syndrom, Glaukom mit kombiniertem Mechanismus, Steroidglaukom, Kapselglaukom, Pigmentglaukom, mit Amyloidose verbundenes sekundäres Glaukom, neovaskuläres Glaukom und malignes Glaukom verwendet werden.
- Das Mittel zur Prophylaxe und Behandlung von Glaukom der vorliegenden Erfindung wird oral oder parenteral verabfolgt. Die Dosierung kann zum Beispiel eine orale Zubereitung wie etwa Tablette, Kapsel oder Sirup oder parenterale Zubereitung wie etwa flüssige Injektion (z. B. Lösung, Emulsion, Suspension) oder ein äußerliches Mittel [z. B. Salbe (insbesondere Augensalbe), Augentropfen] sein. In Anbetracht des Einflusses und der Wirkung auf andere Kreislaufsysteme ist die Dosierungsform einer Verabreichung auf eine lokale Stelle im Auge bevorzugt. Die Dosierungsform von Augentropfen oder einer Augensalbe ist besonders bevorzugt.
- Eine Zubereitung mit der vorstehend angeführten Dosierungsform kann durch Mischen der erfindungsgemäßen Verbindung mit einem zum Formulieren einer Zubereitung notwendigen Additiv wie etwa einem typischen Träger, Arzneimittelträger, Bindemittel oder Stabilisator und Folgen einem herkömmlichen Verfahren hergestellt werden. Zum Beispiel wird die Verbindung der Formel (I') mit einer Rho-Kinasehemmaktivität mit einem pharmazeutisch annehmbaren Träger (z. B. Arzneimittelträger, Bindemittel, Zerfallhilfsmittel, Korrektivum, Korrigentium, Emulgator, Verdünnungsmittel, Löslichmacher) unter Ergeben einer pharmazeutischen Zusammensetzung oder einer pharmazeutischen Zubereitung in Form einer Tablette, Pille, Pulvers, Granulats, Kapsel, Pastille, Sirups, Flüssigkeit, Emulsion, Suspension, Injektion (z. B. Flüssigkeit, Suspension), Suppositoriums, Inhalationsmittels, perkutanen Absorbers, Augentropfen und Augensalbe in einer für eine orale oder parenterale Zubereitung geeigneten Form gemischt.
- Wenn eine feste Zubereitung hergestellt wird, werden Additive wie etwa Sucrose, Lactose, Cellulosezucker, D-Mannit, Maltit, Dextran, Stärken, Agar, Arginate, Chi tine, Chitosane, Pektine, Tragacanthgummi, Gummiarabicum, Gelatinen, Kollagene, Casein, Albumin, Calciumphosphat, Sorbit, Glycin, Carboxymethylcellulose, Polyvinylpyrrolidon, Hydroxypropylcellulose, Hydroxypropylmethylcellulose, Glycerin, Polyethylenglykol, Natriumhydrogencarbonat, Magnesiumstearat oder Talk verwendet. Nötigenfalls kann auf Tabletten eine typische Beschichtung unter Ergeben zuckerbeschichteter Tabletten, magensaftresistenter Tabletten, filmbeschichteter Tabletten, Zweischichttabletten und Mehrschichttabletten aufgebracht werden.
- Wenn eine halbfeste Zubereitung hergestellt wird, werden Tier- und Pflanzenfette und -öle (z. B. Olivenöl, Maisöl, Rizinusöl), Mineralfette und -öle (z. B. Petrolatum, weißes Petrolatum, festes Paraffin), Wachs (z. B. Jojobaöl, Carnaubawachs, Bienenwachs) und teil- oder vollsynthetische Glycerinfettsäureester (z. B. Laurinsäure, Myristinsäure, Palmitinsäure) verwendet. Beispiele davon im Handel erhältlicher Produkte schließen Witepsol (hergestellt von Dynamit Nobel Ltd.) und Farmazol (NOF Corporation) ein.
- Wenn eine flüssige Zubereitung hergestellt wird, wird ein Additiv wie etwa Natriumchlorid, Glucose, Sorbit, Glycerin, Olivenöl, Propylenglykol oder Ethylalkohol verwendet.
- Die flüssige Zubereitung kann zum Beispiel eine Injektion oder Augentropfen sein.
- Wenn eine Injektion hergestellt wird, wird eine sterile wäßrige Lösung wie etwa physiologische Kochsalzlösung, isotone Lösung oder Öl (z. B. Sesamöl und Sojabohnenöl) verwendet. Wenn nötig kann ein geeignetes Suspendiermittel wie etwa Natriumcarboxymethylcellulose, ein nichtionisches Tensid und Löslichmacher (z. B. Benzylbenzoat und Benzylalkohol) gleichzeitig verwendet werden.
- Wenn weiterhin Augentropfen hergestellt werden, wird eine wäßrige Flüssigkeit oder Lösung verwendet, die insbesondere eine sterile injizierbare wäßrige Lösung ist. Die Augentropfen können entsprechend verschiedene Additive wie etwa Puffer, Stabilisatoren, Feuchtemittel, Emulgatoren, Suspendiermittel, Tenside, Isotoniemittel, Konservierungsmittel und Verdickungsmittel enthalten.
- Der Puffer kann zum Beispiel ein Phosphatpuffer, Boratpuffer, Citratpuffer, Tartratpuffer, Acetatpuffer oder Aminosäure sein.
- Der Stabilisator kann zum Beispiel Natriumedetat oder Citronensäure sein.
- Das Feuchtemittel kann zum Beispiel Glycerin sein.
- Der Emulgator kann zum Beispiel Polyvinylpyrrolidon sein.
- Das Suspendiermittel kann zum Beispiel Hydroxypropylmethylcellulose oder Methylcellulose sein.
- Das Tensid kann zum Beispiel Polysorbat 80 oder Polyoxyethylen-hydriertes Rizinusöl sein.
- Das Isotoniemittel kann zum Beispiel Saccharide wie etwa Sorbit, Glucose, Mannit, mehrwertige Alkohole wie etwa Glycerin, Propylenglykol oder Salze wie etwa Natriumchlorid sein.
- Das Konservierungsmittel kann zum Beispiel ein quaternäres Ammoniumsalz wie etwa Benzalkoniumchlorid, Benzethoniumchlorid, p-Hydroxybenzoat wie etwa Methyl-p-hydroxybenzoat, Ethyl-p-hydroxybenzoat, Benzylalkohol, Phenethylalkohol, Sorbinsäure und Salze davon, Thimerosal oder Chlorbutanol sein.
- Das Verdickungsmittel kann zum Beispiel Hydroxyethylcellulose, Hydroxypropylcellulose, Methylcellulose, Hydroxypropylmethylcellulose, Carboxymethylcellulose oder Salze davon sein.
- Beim Gebrauch als Augentropfen wird der pH vorzugsweise im allgemeinen auf etwa 4–9, vorzugsweise etwa 6–8,5 eingestellt.
- Wenn die Zubereitung eine Augensalbe ist, werden eine Salbengrundlage (z. B. Petrolatum, Lanolin, Plastibase) und ein Konservierungsmittel (z. B. Benzalkoniumchlorid, p-Hydroxybenzoat, Chlorbutanol) geeignet ausgewählt und zur Her stellung verwendet.
- Das Arzneimittel der vorliegenden Erfindung zur Prophylaxe und Behandlung von Glaukom enthält einen aktiven Bestandteil in einem Anteil von 0,0001–100 Gew.-%, geeigneterweise 0,001–50 Gew.-% der Zubereitung. Obschon die Dosis und Verabreichungshäufigkeit in Abhängigkeit vom Symptom, Alter, Körpergewicht und der Verabreichungsform schwanken, wird eine Zubereitung, die eine Verbindung der Formel (I') mit einer Rho-Kinasehemmaktivität in einem Anteil von 0,0001–10 Gew./Vol.-%, vorzugsweise 0,001–1 Gew./Vol.-%, mehrmals täglich, vorzugsweise 1–6 Mal täglich jedes Mal in mehreren Tropfen, vorzugsweise 1–3 Tropfen verabreicht, wenn sie als Augentropfen für einen Erwachsenen verwendet wird. Wenn eine diese Verbindung in einem Anteil von 0,0001–10 Gew./Vol.-%, vorzugsweise 0,001–1 Gew./Vol.-% enthaltende Zubereitung als Augensalbe verwendet wird, kann sie mehrmals täglich, vorzugsweise 1–6 Mal täglich angewendet werden.
- Beispiele
- Die vorliegende Erfindung wird unter Bezug auf Beispiele und Versuchsbeispiele genauer erläutert.
- Beispiel 1: Augentropfen 1
- (+)-trans-4-(1-Aminoethyl)-1-(4-pyridylcarbamoyl)cyclohexan-2HCl-1H2O (hierin nachstehend Verbindung A), die eine Verbindung mit einer Rho-Kinasehemmaktivität ist, wurde in destilliertem Wasser zur Injektion gelöst. Der pH wurde mit Natriumhydroxid auf 7 eingestellt und Augentropfen mit der folgenden Zusammensetzung wurden hergestellt.
Verbindung A 0,5 g Natriumdihydrogenphosphat-2-hydrat 0,1 g Natriumchlorid 0,9 g destilliertes Wasser zur Injektion erforderliche Menge Gesamtmenge 100 ml - Beispiel 2: Augentropfen 2
- Auf dieselbe Weise wie bei Beispiel 1 wurden die Verbindung A in einer Konzent ration von 0,1% enthaltende Augentropfen hergestellt.
- Beispiel 3: Augentropfen 3
- Auf dieselbe Weise wie bei Beispiel 1 wurden die Verbindung A in einer Konzentration von 0,03% enthaltende Augentropfen hergestellt.
- Beispiel 4: Augentropfen 4
- Auf dieselbe Weise wie bei Beispiel 1 wurden (+)-trans-N-(1H-Pyrrolo[2,3-b]pyridin-4-yl)-4-(1-aminoethyl)cyclohexancarbonsäureamid-2HCl-6/5H2O (hierin nachstehend Verbindung B), die eine Verbindung mit einer Rho-Kinasehemm-aktivität ist, in einer Konzentration von 0,03% enthaltende Augentropfen hergestellt.
- Beispiel 5: Tabletten
- Die Verbindung A, Lactose, Maisstärke und kristalline Cellulose wurden gemischt, mit Polyvinylpyrrolidon-K30-Pastenlösung geknetet und zur Granulierung durch ein 20-Mesh-Sieb geführt. Nach 2 Stunden Trocknen bei 50°C wurde das Granulat durch ein 24-Mesh-Sieb geführt und Talk und Magnesiumstearat wurden zugefügt. Mittels eines Stempels mit 7 mm ø wurden 120 mg je Tablette wiegende Tabletten hergestellt.
Verbindung A 10,0 mg Lactose 50,0 mg Maisstärke 20,0 mg kristalline Cellulose 29,7 mg Polyvinylpyrrolidon K30 5,0 mg Talk 5,0 mg Magnesiumstearat 0,3 mg 120,0 mg - Formulierungsbeispiel 6: Kapseln
- Die Verbindung A, Lactose und Maisstärke wurden gemischt, mit Polyvinylpyrrolidon-K30-Pastenlösung geknetet und zur Granulierung durch ein 20-Mesh-Sieb geführt. Nach 2 Stunden Trocknen bei 50°C wurde das Granulat durch ein 24-Mesh-Sieb geführt und Talk und Magnesiumstearat wurden zugefügt. Das Ge misch wurde unter Ergeben 120 mg wiegender Kapseln in Hartkapseln (Nr. 4) gefüllt.
Verbindung A 10,0 mg Lactose 70,0 mg Maisstärke 35,0 mg Polyvinylpyrrolidon K30 2,0 mg Talk 2,7 mg Magnesiumstearat 0,3 mg 120,0 mg - Versuchsbeispiel 1: Wirkung auf den normalen Augeninnendruck farbiger Kaninchen
- Versuchsverfahren
- Männliche farbige Holländer-Kaninchen (Körpergewicht etwa 2 kg) wurden verwendet. Die Kaninchen wurden eine Woche vor dem Test 3–5 h täglich zur Eingewöhnung in eine Haltekiste gesetzt. Die Kaninchen, die einen durch ein Tonometer [Pneumatonograph (hergestellt durch Alcon Lab. Inc.)] gemessenen gleichbleibenden Augeninnendruck zeigten, wurden ausgewählt und für den Test verwendet. Nach dem Messen des Anfangswerts des Augeninnendrucks wurden die Augentropfen (50 μl) des Beispiels 1 in ein Auge eingeträufelt und eine Grundlage, die die Augentropfen des Beispiels 1 außer Verbindung A war, wurde auf dieselbe Weise in das andere Auge eingeträufelt und als Kontrollauge angenommen. Der Augeninnendruck wurde mit der Zeit 30, 60, 90 und 120 min nach dem Einträufeln und danach in Abständen von 60 min gemessen, bis der Augeninnendruck wieder auf seinen Anfangswert zurückgekehrt war und die Wirkungsdauer wurde untersucht.
- Versuchsergebnis
- Die Wirkung der Augentropfen des Beispiels 1 auf den normalen Augeninnendruck wird in
1 dargestellt. Verglichen mit dem Kontrollauge 60 min nach dem Einträufeln wurde eine höchste signifikante, den Augeninnendruck erniedrigende Wirkung von 5 mmHg gemessen. Es wurde 180 min lang nach dem Einträufeln eine signifikante, den Augeninnendruck erniedrigende Wirkung verglichen mit dem Kontrollauge gefunden. 360 min nach dem Einträufeln war der Au geninnendruck nahezu derselbe wie bei dem Kontrollauge und kehrte auf seinen Anfangswert zurück. - Versuchsbeispiel 7: Wirkung auf die Blutströmung einer normalen Papille farbiger Kaninchen
- Versuchsverfahren
- Männliche farbige Holländer-Kaninchen (Körpergewicht etwa 2 kg) wurden verwendet. Die Augentropfen (50 μl) des Beispiels 1 wurden in ein Auge eingeträufelt und eine Grundlage, die die Augentropfen des Beispiels 1 außer Verbindung A war, wurde auf dieselbe Weise in das andere Auge eingeträufelt und als Kontrollauge angenommen. Mittels eines Laser-Speckle-Mikrozirkulationsanalysengeräts wurde die Blutströmung der Papille 30, 60, 90 und 120 min nach dem Einträufeln und danach in Abständen von 60 min bis 300 min nach dem Einträufeln gemessen.
- Versuchsergebnis
- Die Wirkung der Augentropfen des Beispiels 1 auf die Blutströmungskinetik der Papille wird in
2 dargestellt. Verglichen mit dem Kontrollauge wurde eine die Blutströmung erhöhende Wirkung von 11% 30 min nach dem Einträufeln gefunden und eine signifikante, die Blutströmung erhöhende Wirkung von 15% wurde 60 min nach dem Einträufeln gefunden. Die Blutströmung erhöhte sich 120 min nach dem Einträufeln am höchsten (18%). Die Wirkung nahm danach allmählich ab, aber eine signifikante, die Blutströmung erhöhende Wirkung wurde 180 min nach dem Einträufeln der Verbindung verglichen mit dem Kontrollauge beobachtet. - Versuchsbeispiel 3: Wirkung auf die Carbacholkontraktion des extrahierten Ziliarmuskels weißer Kaninchen
- Versuchsverfahren
- Männliche weiße japanische Kaninchen (Körpergewicht etwa 2 kg) wurden durch intravenöse Verabreichung eines Überschusses an Pentobarbitalnatrium eingeschläfert. Der Augapfel wurde unmittelbar danach entkernt und in Krebs-Lösung (112 mM NaCl, 5,9 mM KCl, 2,0 mM CaCl2 2H2O, 1,2 mM MgCl2 6H2O, 1,2 mM NaH2PO4 2H2O, 25 mM NaHCO3, 11,5 mM Glucose) konserviert. Der vom Augapfel abgetrennte Ziliarkörper wurde in mit der Krebs-Lösung gefülltes Magnus-Bad gehängt und unter 20–30 mg Ruhespannung äquilibriert. Die Änderungen der Spannung bei der Präparation wurden mit einem Meßwertaufnehmer gemessen und über einen Verstärker auf einem Schreiber aufgezeichnet. Als Kontraktionswirkstoff wurde Carbachol verwendet und die Hemmwirkung auf die dosisabhängige Reaktion der phasischen Kontraktion wurde untersucht. Der Testwirkstoff war (+)-trans-4-(1-Aminoethyl)-1-(4-pyridylcarbamoyl)cyclohexan-2HCl-1H2O (Verbindung A), die dem Magnus-Bad 5 min vor der Carbacholzugabe zugefügt wurde.
- Versuchsergebnis
- Die Wirkung von Verbindung A auf die Carbacholkontraktion wird in
3 dargestellt. Der Ziliarmuskel zeigte eine dosisabhängige Kontraktion durch 10–6–3 × 10–4 M Carbachol und Verbindung A zeigte einen nicht-kompetitiven Antagonismus gegenüber der Carbacholkontraktion. Die IC50 von Verbindung A gegenüber der Carbacholkontraktion war 2,8 × 10–6 M. - Die Kontraktion und Relaxation des Ziliarmuskels spielt eine wichtige Rolle beim wäßrigen Abfluß. Durch die Relaxation des Ziliarmuskels kann der wäßrige Abfluß über das Trabekelwerk gehemmt werden, aber der über die Uveosklera wird gefördert (Takeshi Yoshitomi, Neuroophthalmol. Jp., 15 (1), 76–81, 1998). Von der Relaxation des Ziliarmuskels, die den wäßrigen Abfluß fördert, wird angenommen, daß sie zu einer Erniedrigung des Augeninnendrucks führt.
- Im allgemeinen wird angenommen, daß 1/1000 eines Augentropfens in die Vorderkammer überführt wird (Kouji Honda: Practical Ophthalmology, Guide of ophthalmic drug, Bunkodo Co. Ltd., Tokio, 387–392, 1994). Wenn 0,5% Verbindung A in 50 μl eingeträufelt werden, wird ein in die Vorderkammer zu überführendes 1/1000 davon zu 1,5 × 10–5 berechnet. Daher wird von diesen Testergebnissen angenommen, daß sie auch die in vivo ausreichend wirksame Konzentration darstellen.
- Versuchsbeispiel 4: Wirkung auf den normalen Augeninnendruck weißer Kaninchen
- Testwirkstoff
- Verbindung A (+)-trans-4-(1-Aminoethyl)-1-(4-pyridylcarbamoyl)cyclohexan-2HCl-1H2O
- Verbindung B (+)-trans-N-(1H-Pyrrolo[2,3-b]pyridin-4-yl)-4-(1-aminoethyl)cyclohexancarbonsäureamid-2HCl-6/5H2O
- Bei diesem Versuch wurden 0,1%ige und 0,03%ige, Verbindung A enthaltende Augentropfen (jeweils in Beispiel 2 und Beispiel 3 hergestellt) und 0,03%ige, Verbindung B enthaltende Augentropfen (hergestellt in Beispiel 4) verwendet.
- Versuchsverfahren
- Von Japan Laboratory Animals, INC., erworbene japanische weiße Kaninchen (Körpergewicht etwa 2 kg) wurden verwendet. Diese Tiere wurden in einer auf eine Temperatur von 23 ± 3°C und Feuchtigkeit von 55 ± 10% eingestellten Aufzuchtkammer gehalten und mit einer eingeschränkten Menge von 100 g täglich eines festen Futters (Labo R Stock, Nihon-Nosan Kogyo K. K.) gefüttert. Ihnen wurde der freie Zugang zu Leitungswasser gestattet. Die Kaninchen wurden 2 Tage vor dem Test 5 h täglich zur Eingewöhnung in eine Haltekiste gesetzt. Die Kaninchen, die einen durch ein Tonometer [Pneumatonograph (hergestellt durch Alcon Lab. Inc.)] gemessenen gleichbleibenden Augeninnendruck zeigten, wurden ausgewählt und für den Test verwendet. Nach dem Messen des Anfangswerts des Augeninnendrucks wurden verschiedene Augentropfen (20 μl) in ein Auge eingeträufelt und eine Grundlage, die irgendwelche der verschiedenen Augentropfen außer dem Testwirkstoff war, wurde auf dieselbe Weise in das andere Auge eingeträufelt und als Kontrollauge angenommen. Der Augeninnendruck wurde mit der Zeit 30, 60, 90, 120, 150 und 180 min nach dem Einträufeln und danach in Abständen von einer Stunde gemessen, bis der Augeninnendruck wieder auf seinen Anfangswert zurückgekehrt war und anschließend wurde die Wirkungsdauer untersucht.
- Versuchsergebnis
- Die Wirkungen der jeden Testwirkstoff in einer Konzentration von 0,1% enthaltenden Augentropfen auf den normalen Augeninnendruck werden in
4 (Beispiel 2) dargestellt. Die Wirkungen jeden Testwirkstoff in einer Konzentration von 0,03% enthaltender Augentropfen auf den normalen Augeninnendruck werden in5 dargestellt (Beispiel 3, 4). In jedem Fall wurde eine signifikante, den Augeninnendruck erniedrigende Wirkung gefunden. Insbesondere Verbindung A (Beispiel 2, 3) zeigte in den frühen Stadien nach dem Einträufeln ein den Augeninnendruck erniedrigende Wirkung. - Versuchsbeispiel 5: Wirkung auf die Blutströmung der normalen Papille weißer Kaninchen
- Testverbindung
- Verbindung A (+)-trans-4-(1-Aminoethyl)-1-(4-pyridylcarbamoyl)cyclohexan-2HCl-1H2O
- Bei diesem Versuch wurden 0,1% Verbindung A enthaltende Augentropfen (hergestellt in Beispiel 2) verwendet.
- Versuchsverfahren
- Von Japan Laboratory Animals, INC., erworbene japanische weiße Kaninchen (Körpergewicht etwa 2 kg) wurden verwendet. Diese Tiere wurden in einer auf eine Temperatur von 23 ± 3°C und Feuchtigkeit von 55 ± 10% eingestellten Aufzuchtkammer gehalten und mit einer eingeschränkten Menge von 100 g täglich eines festen Futters (Labo R Stock, Nihon-Nosan Kogyo K. K.) gefüttert. Ihnen wurde der freie Zugang zu Leitungswasser gestattet. Auf dieselbe Weise wie bei Beispiel 4 wurde jeder Testwirkstoff verabreicht. Mittels eines Laser-Speckle-Mikrozirkulationsanalysengeräts wurde die Blutströmung der Papille 30, 60, 90, 120, 150 und 180 min nach dem Einträufeln und danach in Abständen von einer Stunde bis 300 min nach dem Einträufeln gemessen.
- Versuchsergebnis
- Die Ergebnisse werden in
7 dargestellt. In jedem Fall wurde eine signifikante, die Blutströmung erhöhende Wirkung 30 min nach dem Einträufeln beobachtet. Insbesondere als Verbindung A (Beispiel 2) eingeträufelt wurde, war die Wirkung länger anhaltend. - In Anbetracht der Ergebnisse des Versuchsbeispiels 2 wurde angenommen, daß diese die Papillenblutströmung erhöhende Wirkung der Gefäßerweiterung zuzuschreiben ist, die durch die Dephosphorylierung vaskulären Glattmuskelmyosins leichter Kette aufgrund der Aktivierung von Myosinphosphatase durch eine Verbindung mit einer Rho-Kinasehemmaktivität (Masayoshi Uehata et al., Nature 389, 990–994, 1997) und der begleitenden Zunahme des Augenperfusionsdrucks (Blutdruck – Augeninnendruck) zuzuschreiben ist.
- Versuchsbeispiel 6: durch 8faches tägliches Einträufeln in weiße Kaninchen verursachte Augenstörung
- Testwirkstoff
- Verbindung A (+)-trans-4-(1-Aminoethyl)-1-(4-pyridylcarbamoyl)cyclohexan-2HCl-1H2O
- Die Testwirkstoffe, Verbindung A, wurden jeweils in der folgenden Grundlage in einer Konzentration von 0,125, 0,25, 0,5 und 1,0% gelöst und zur Verwendung in diesem Versuch auf pH 7 eingestellt. Grundlagenformulierung
Natriumdihydrogenphosphat-2-hydrat 0,1 g Natriumchlorid 0,9 g Natriumhydroxid erforderliche Menge destilliertes Wasser zur Injektion erforderliche Menge Gesamtmenge 100 ml - Versuchsverfahren
- Von Japan Laboratory Animals, INC., erworbene japanische weiße Kaninchen (Körpergewicht etwa 2 kg) wurden verwendet. Diese Tiere wurden in einer auf eine Temperatur von 23 ± 3°C und Feuchtigkeit von 55 ± 10% eingestellten Aufzuchtkammer gehalten und mit einer eingeschränkten Menge von 100 g täglich eines festen Futters (Labo R Stock, Nihon-Nosan Kogyo K. K.) gefüttert. Ihnen wurde der freie Zugang zu Leitungswasser gestattet. Einträufeln: mittels einer Mikropipette wurde jede Testverbindung (100 μl) in das rechte Auge jedes Tiers 8 Mal im Abstand von einer Stunde eingeträufelt. In das linke Auge wurde eine Grundlage auf dieselbe Weise eingeträufelt. Beobachtung: der vordere Abschnitt des Auges wurde vor dem Einträufeln und 30 min nach der 2., 4., 6. und 8. Verabreichung gemäß den in Tabelle 1 dargestellten makroskopischen Kriterien für Augenläsionen makroskopisch beobachtet (Naruyuki Fukui, Fumihiko Ikemoto, Gendai no Rinshou 4, 277–289, 1970). Außerdem wurde vor dem Einträufeln und nach der 8. Verabreichung ein die Hornhaut verfärbender Fleck beobachtet.
- Die Ergebnisse der makroskopischen Beobachtung des vorderen Augenabschnitts nach der Verabreichung von Verbindung A werden in Tabelle 2 dargestellt.
- Entsprechend der Beobachtung des die Hornhaut verfärbenden Flecks, führte die Verabfolgung von Verbindung A in jeder Konzentration nicht zu Abnormalitäten.
- Industrielle Anwendbarkeit
- Da eine Verbindung der Formel (I') in dem Arzneimittel der vorliegenden Erfin dung zur Prophylaxe und Behandlung von Glaukom eine den Augeninnendruck erniedrigende Aktivität, eine die Papillenblutströmung verbessernde Wirkung und eine den wäßrigen Abfluß fördernde Aktivität zeigt, ist bei das Arzneimittel zur Prophylaxe und Behandlung verschiedener Glaukomtypen wie etwa primäres Offenwinkelglaukom, Normaldruckglaukom, Hypersekretionsglaukom, okuläre Hypertension, akutes Winkelblockglaukom, chronisches Winkelblockglaukom, Plateau-Iris-Syndrom, Glaukom mit kombiniertem Mechanismus, Steroidglaukom, Kapselglaukom, Pigmentglaukom, mit Amyloidose verbundenes sekundäres Glaukom, neovaskuläres Glaukom und malignes Glaukom brauchbar.
- Insoweit die Verbindung der Formel (I') mit einer Rho-Kinasehemmaktivität die Kontraktion des Ziliarmuskels hemmt, ist sie als Mittel zur Prophylaxe und Behandlung von durch eine anhaltende, abnormale Spannung des Ziliarmuskels verursachte Asthenopie und Pseudomyopie und dergleichen brauchbar.
Claims (6)
- Verwendung einer Verbindung mit einer Rho-Kinasehemmaktivität zur Herstellung eines Arzneimittels zur Prophylaxe und Behandlung von Glaukom, wobei die Verbindung mit einer Rho-Kinasehemmaktivität eine Amidverbindung der folgenden Formel (I') ist: worin Ra' eine Gruppe der Formel ist, worin R' Wasserstoff, Alkyl oder Cycloalkyl, Cycloalkylalkyl, Phenyl oder Aralkyl ist, das gegebenenfalls einen Substituenten am Ring aufweist, R1 Wasserstoff, Alkyl oder Cycloalkyl, Cycloalkylalkyl, Phenyl oder Aralkyl ist, das gegebenenfalls einen Substituenten am Ring aufweist oder R' und R1 in kombinierter Form zusammen mit dem benachbarten Stickstoffatom eine Gruppe bilden, die einen Heterocyclus mit gegebenenfalls einem Sauerstoffatom, Schwefelatom oder gegebenenfalls substituierten Stickstoffatom im Ring bildet, R2 Wasserstoff oder Alkyl ist, A eine Gruppe der Formel ist, worin R10 und R11 gleich oder verschieden sind und jeweils Wasserstoff, Alkyl, Halogenalkyl, Aralkyl, Hydroxyalkyl, Carboxy oder Alkoxycarbonyl sind oder R10 und R11 eine Gruppe darstellen, die in Kombination Cycloalkyl bildet, und l, m und n jeweils 0 oder eine ganze Zahl 1–3 sind, Rb Wasserstoff, Alkyl, Aralkyl, Aminoalkyl oder Mono- oder Dialkylaminoalkyl ist und Rc ein gegebenenfalls substituierter, stickstoffhaltiger Heterocyclus ist, ein Isomer davon und/oder ein pharmazeutisch annehmbares Säureadditionssalz davon.
- Verwendung des Anspruchs 1, wobei die Verbindung mit einer Rho-Kinasehemmaktivität (+)-trans-4-(1-Aminoethyl)-1-(4-pyridylcarbamoyl)cyclohexan, (+)-trans-N-(1H-Pyrrolo[2,3-b]pyridin-4-yl)-4-(1-aminoethyl)cyclohexancarbonsäureamid und/oder ein pharmazeutisch annehmbares Säureadditionssalz davon ist.
- Verwendung des Anspruchs 1 oder 2, wobei das pharmazeutisch annehmbare Säureadditionssalz ein Salz ist, bei dem die Säure durch eine anorganische Säure wie etwa Salzsäure, Bromwasserstoffsäure und Schwefelsäure oder eine organische Säure wie etwa Methansulfonsäure, Fumarsäure, Maleinsäure, Mandelsäure, Citronensäure, Weinsäure und Salicyclsäure veranschaulicht wird.
- Verwendung der Ansprüche 1 bis 3, wobei das Säureadditionssalz ein Salzsäureadditonssalz ist.
- Verwendung des Anspruchs 1, wobei das Arzneimittel zur Prophylaxe und Behandlung von Glaukom zur lokalen Verabreichung am Auge bestimmt ist.
- Verwendung des Anspruchs 5, wobei sich das Arzneimittel zur Prophylaxe und Behandlung von Glaukom in Form von Augentropfen befindet.
