Description
- Technisches Gebiet
- Die Erfindung betrifft eine neue Zusammensetzung und ein Verfahren zur Verbesserung der pharmakokinetischen Eigenschaften von Arzneistoffen, welche durch Cytochrom P450 Monooxygenase metabolisiert werden.
- Hintergrund der Erfindung
- Es wurde jüngst offenbart, dass der HIV Proteaseinhibitor Ritonavir (auch bekannt als ABT-538) in Menschen zur Hemmung einer HIV-Infektion wirksam ist.
- Es wurde auch entdeckt, dass Ritonavir ein Inhibitor des metabolischen Enzyms Cytochrom P450 Monooxygenase ist.
- Einige Arzneistoffe werden durch Cytochrom P450 Monooxygenase metabolisiert, was zu ungünstigen Pharmakokinetiken und dem Bedürfnis nach häufigeren und höheren Dosierungen führt, als es am wünschenswertesten ist. Die Verabreichung solcher Arzneistoffe mit einem Wirkstoff, der den Metabolismus durch Cytochrom P450 Monooxygenase hemmt, wird die Pharmakokinetiken des Arzneistoffs verbessern (d. h. die Halbwertszeit erhöhen, die Zeit bis zur Plasma-Peak-Konzentration erhöhen, die Blutspiegel erhöhen).
- Es wurde entdeckt, dass die gleichzeitige Verabreichung von Ritonavir mit einem Arzneistoff, der durch Cytochrom P450 Monooxygenase, insbesondere dem P450 3A4 Isoenzym, metabolisiert wird, eine Verbesserung in den Pharmakokinetiken eines solchen
- Offenbarung der Erfindung
- In Übereinstimmung mit der vorliegenden Erfindung wird ein Verfahren zur Verbesserung der Pharmakokinetiken eines Arzneistoffs (oder eines pharmazeutisch verträglichen Salzes davon) offenbart, welcher durch Cytochrom P450 Monooxygenase metabolisiert wird, umfassend die gleichzeitige Verabreichung von Ritonavir oder eines pharmazeutisch verträglichen Salzes davon. Die zwei therapeutischen Wirkstoffe können, wenn sie in Kombination verabreicht werden, als separate Zusammensetzungen formuliert werden, welche zur selben Zeit oder zu unterschiedlichen Zeiten verabreicht werden, oder die zwei therapeutischen Wirkstoffe können als eine einzelne Zusammensetzung verabreicht werden.
- Arzneistoffe, welche durch Cytochrom P450 Monooxygenase metabolisiert werden und welche durch die gleichzeitige Verabreichung mit Ritonavir einen Vorteil haben, schließen die Immunsuppressoren Cyclosporin, FK-506 und Rapamycin, die chemotherapeutischen Wirkstoffe Taxol und Taxotere und das Antibiotikum Clarithromycin ein.
- Genauer betrifft die Erfindung folgendes:
- 1. Eine Kombination aus Ritonavir oder eines pharmazeutisch verträglichen Salzes davon und eines Arzneistoffs, welcher durch Cytochrom P450 Monooxygenase 3A4 metabolisiert wird, um die Pharmakokinetiken des Arzneistoffs zu verbessern, vorausgesetzt, dass der Arzneistoff kein HIV Proteaseinhibitor ist.
- 2. Eine Kombination gemäß 1., worin Ritonavir und der Arzneistoff als eine einzelne Zusammensetzung formuliert sind.
- 3. Eine Kombination gemäß 1., worin Ritonavir und der Arzneistoff als separate Zusammensetzungen formuliert sind.
- 4. Eine Kombination gemäß irgendeinem von 1. bis 3., worin die Dosis von Ritonavir von 0,001 bis 300 mg/kg Körpergewicht des Menschen, dem sie verabreicht werden soll, ist.
- 5. Eine Kombination gemäß irgendeinem von 1. bis 4., worin der Arzneistoff Cyclosporin, FK-506, Rapamycin, Taxol, Taxotere oder Clarithromycin ist.
- Ritonavir ist (2S,3S,5S)-5-(N-(N-((N-Methyl-N-((2-isopropyl-4-thiazolyl)methyl)amino)carbonyl)-L-valinyl)amino)-2-(N-((5-thiazolyl)methoxycarbonyl)amino)-1,6-diphenyl-3-hydroxyhexan oder ein pharmazeutisch verträgliches Salz davon. Ritonavir kann durch die Verfahren, die in PCT Patentanmeldung Nr.
WO 94/14436 - Hemmund von Cytochrom P450
- Die Fähigkeit von Ritonavir Cytochrom P450 Monooxygenase Aktivität zu hemmen, wurde mit Terfenadin als dem Sondensubstrat getestet (Yun, et al., Drug Metabolism & Disposition, Band 21, 403–407 (1993)). Ritonavir hemmte die Terfenadinhydroxylase Aktivität, die die am reichlichsten verhandene Form von Cytochrom P450 (CYP3A4) darstellt, die in der menschlichen Leber vorhanden ist, mit einer IC50 von 0,25 μM.
- Pharmakokinetische Verbesserung
- Die Fähigkeit von Ritonavir, die Pharmakokinetiken einer Verbindung, die durch Cytochrom P450 Monooxygenase metabolisiert wird, zu verbessern, kann durch das unten beschriebene Testverfahren demonstriert werden, worin VX-478 als ein Beispiel verwendet wird (nicht Teil der Erfindung).
- Man ließ Ratten (männlich, Sprague-Dawley abgeleitet, 0,3 bis 0,45 kg) über Nacht vor der Dosierung fasten, aber Wasser war ihnen ad libitum erlaubt. Für die Kombinationsdosierung wurde eine einzelne Lösung, die sowohl Ritonavir als auch VX-478 (5 mg/ml jeweils) enthielt, in einem Vehikel von 20 Ethanol: 30 Propylenglycol mit einer angemessenen Anzahl an molaren Äquivalenten von Methansulfonsäure, um bei der Löslichmachung behilflich zu sein, hergestellt. Separate Lösungen von VX-478 und Ritonavir wurden ebenfalls hergestellt und diese Lösungen wurden verwendet, um die Pharmakokinetiken von VX-478 und Ritonavir zu bewerten, wenn sie als einzelne Wirkstoffe in Ratten verabreicht werden. Die Lösungen, oral durch künstliche Sondenernährung an eine Gruppe von Ratten bei einem Dosiervolumen von 2 ml/kg verabreicht, lieferte eine 10 mg/kg Dosis von jeder Verbindung. Blutproben wurden erhalten von einer Schwanzvene von jeder Ratte 0,25, 0,5, 1, 1,5, 2, 3, 4, 6 und 8 Stunden nach der Dosierung. Das Plasma wurde von den roten Blutkörperchen durch Zentrifugation abgetrennt und bis zur Analyse gefroren (–30°C). Konzentrationen von sowohl Ritonavir als auch VX-478 wurden gleichzeitig durch reverse Phase HPLC mit geringer Wellenlängen UV-Detektion nach Flüssig-Flüssig-Extraktion der Plasmaproben bestimmt. Die Peak-Plasma-Konzentration (Cmax) und die Zeit bis zur Peak-Plasma-Konzen-tration (Tmax) für jede Ratte wurde direkt aus den Plasma-Konzentrations-Daten erhalten. Die Fläche unter der Kurve wurde durch das Trapezverfahren über den Zeitverlauf der Studie berechnet. Die Plasma-Eliminations-Halbwertszeit wurde erhalten von NONLIN84 oder von einer log-linearen Regression der terminalen Plasma-Konzentrationen als einer Funktion der Zeit nach der Dosierung. Jede Kombination wurde bewertet in einer Gruppe, die mindestens drei Ratten enthielt; die berichteten Werte sind Durchschnittswerte für jede Gruppe von Tieren. Die Daten, die von der Kombination erhalten wurden, wurden verglichen mit Daten, die von einer separaten Gruppe von Ratten erhalten wurden, welche eine einzelne, separate Dosis der zu bewertenden Verbindung erhielten.
- Unten in Tabelle 1 sind die Ergebnisse aus den pharmakokinetischen Experimenten mit VX-478 und anderen HIV Proteaseinhibitoren (die nicht Teil der Erfindung sind) in Ratten gezeigt. Die Maximum-Plasma-Spiegel (Cmax), die Zeit bis zum Maximum-Plasma-Spiegel (Tmax) und die Fläche unter der Plasma-Konzentrationskurve (AUC) für einen achtstündigen Probennahme-Intervall nach der Dosierung des HIV Proteaseinhibitors alleine versus der Dosierung in Kombination mit Ritonavir, werden bereit gestellt. - Die Fähigkeit von Ritonavir die Pharmakokinetiken von Clarithromycin in Menschen zu verbessern, wurde gemäß dem unten beschriebenen Verfahren gezeigt.
- Clarithromycin (500 mg/BIAXIN® Tablette alle 12 Stunden) und eine Kombination aus Ritonavir (200 mg flüssige Formulierung alle 8 Stunden) und Clarithromycin (500 mg alle 12 Stunden) wurde Gruppen von vier gesunden menschlichen Freiwilligen verabreicht. Blutproben wurden am Tag vier der Dosierung für die HPLC-Bestimmung der Plasma-Konzentrationen von Clarithromycin genommen.
- Unten in Tabelle 2 sind die Ergebnisse von den pharmakokinetischen Experimenten mit Clarithromycin in Menschen gezeigt. Die mittleren Maximum-Plasma-Spiegel (Cmax) und die Fläche unter der Plasma-Konzentrationskurve (AUC), berechnet unter Verwendung von nicht-Kompartimentierungsverfahren für den 0 bis 24 Stunden Zeitintervall am Tag vier der Dosierung von Clarithromycin alleine versus der Dosierung in Kombination mit Ritonavir werden bereit gestellt. - Die therapeutischen Wirkstoffe der vorliegenden Erfindung können in der Form von Salzen, die von anorganischen oder organischen Säuren abgeleitet sind, verwendet werden. Diese Salze schließen ein, sind aber nicht beschränkt auf die folgenden: Acetat, Adipat, Alginat, Citrat, Aspartat, Benzoat, Benzensulfonat, Eisulfat, Butyrat, Camphorat, Camphorsulfonat, Digluconat, Cyclopentanpropionat, Dodecylsulfat, Ethansulfonat, Glucoheptanoat, Glycerophosphat, Hemisulfat, Heptanoat, Hexanoat, Fumarat, Hydrochlorid, Hydrobromid, Hydroiodid, 2-Hydroxyethansulfonat (Isethionat), Lactat, Maleat, Methansulfonat, Nicotinat, 2-Naphthalensulfonat, Oxalat, Pamoat, Pectinat, Persulfat, 3-Phenylpropionat, Picrat, Pivalat, Propionat, Succinat, Tartrat, Thiocyanat, p-Toluensulfonat und Undecanoat. Auch können die basischen Stickstoff-enthaltenden Gruppen quaternisiert werden mit solchen Wirkstoffen wie Niederalkylhalogeniden, wie zum Beispiel Methyl, Ethyl, Propyl und Butylchlorid, Bromiden und Iodiden; Dialkylsulfaten, wie Dimethyl, Diethyl, Dibutyl und Diamylsulfaten, langkettigen Halogeniden, wie zum Beispiel Decyl, Lauryl, Myristyl und Stearylchloriden, Bromiden und Iodiden, Aralkylhalogeniden, wie zum Beispiel Benzyl und Phenethylbromiden und anderen. Wasser- oder Öl-lösliche oder -dispergierbare Produkte werden dabei erhalten.
- Beispiele für Säuren, die verwendet werden können, um pharmazeutisch verträgliche Säureadditionssalze zu bilden, schließen solche anorganischen Säuren, wie zum Beispiel Salzsäure, Schwefelsäure und Phosphorsäure ein, und solche organischen Säuren, wie Oxalsäure, Maleinsäure, Bernsteinsäure und Zitronensäure. Andere Salze schließen Salze mit Alkalimetallen oder Erdalkalimetallen ein, wie zum Beispiel Natrium, Kalium, Kalzium oder Magnesium oder mit organischen Basen.
- Die Verabreichung von Ritonavir und einer Verbindung, welche durch Cytochrom P450 Monooxygenase metabolisiert wird, ist nützlich zur Verbesserung der Pharmakokinetiken der Verbindung, welche durch Cytochrom P450 Monooxygenase metabolisiert wird, in Menschen.
- Die gesamte tägliche Dosis von Ritonavir, die einem Menschen oder anderen Säugetierwirt im einzelnen oder geteilten Dosen verabreicht werden soll, kann in Mengen sein, zum Beispiel von 0,001 bis 300 mg/kg Körpergewicht täglich, und üblicher 0,1 bis 50 mg/kg und noch üblicher 0,1 bis 25 mg/kg. Dosiseinheits-Zusammensetzungen können solche Mengen von Untergruppen davon enthalten, um die tägliche Dosis auszumachen.
- Die gesamte tägliche Dosis des Arzneistoffs, der durch Cytochrom P450 Monooxygenase metabolisiert wird, der an einen Menschen oder ein anderes Säugetier verabreicht werden soll, ist wohl bekannt und kann von jemandem mit durchschnittlichem Können im Fachgebiet leicht bestimmt werden. Dosiseinheits-Zusammensetzungen können solche Mengen von Untermengen davon enthalten, um die tägliche Dosis auszumachen.
- Die Menge an aktivem Inhaltsstoff, die mit den Trägermaterialien kombiniert werden kann, um eine einzelne Dosierform von jedem Arzneistoff zu erzeugen, einzeln oder in Kombination, wird von dem behandelten Wirt und der speziellen Verabreichungsart abhängen.
- Es wird jedoch verstanden werden, dass der spezifische Dosisspiegel für irgendeinen speziellen Patienten von einer Vielzahl von Faktoren abhängen wird, einschließlich der Aktivität der spezifischen verwendeten Verbindung, dem Alter, Körpergewicht, dem allgemeinen Gesundheitszustand, dem Geschlecht, der Ernährung, der Verabreichungszeit, dem Verabreichungsweg, der Ausscheidungsrate, der Arzneistoff-Kombination und der Schwere der speziellen Krankheit, die therapiert wird.
- Die Kombination von therapeutischen Wirkstoffen der vorliegenden Erfindung (als individuelle Zusammensetzungen oder als eine einzige Zusammensetzung) können oral, parenteral, sublingual, durch Inhalationsspray, rektal oder topisch in Dosiseinheits-Formulierungen, die konventionelle nicht toxische pharmazeutisch verträgliche Träger, Hilfsstoffe und Vehikel, wie gewünscht, enthalten, verabreicht werden. Die toxische Verabreichung kann auch die Verwendung einer transdermalen Verabreichung involvieren, wie zum Beispiel transdermale Pflaster oder Iontophorese-Vorrichtungen. Der Ausdruck „parenteral", wie hierin verwendet, schließt subkutane Injektionen, intravenöse, intramuskuläre, intrasternale Injektions- oder Infusionstechniken ein.
- Injizierbare Zubereitungen, zum Beispiel sterile injizierbare wässerige oder ölige Suspensionen können gemäß dem bekannten Fachgebiet unter Verwendung von geeigneten Dispersions- oder Benetzungsmitteln und Suspendiermitteln formuliert werden. Die sterile injizierbare Zubereitung kann auch eine sterile injizierbare Lösung oder Suspension in einem nicht toxischen parenteral verträglichen Verdünnungsmittel oder Lösungsmittel sein, zum Beispiel als eine Lösung in 1,3-Propandiol. Unter den akzeptablen Vehikeln und Lösungsmitteln, die verwendet werden können, sind Wasser, Ringer-Lösung und isotonische Natriumchloridlösung. Zusätzlich werden sterile, fette Öle konventionell als ein Lösungsmittel oder Suspendiermedium verwendet. Für diesen Zweck kann jedes milde fette Öl verwendet werden, einschließlich synthetischen Mono- oder Diglyceriden. Zusätzlich finden Fettsäuren, wie zum Beispiel Ölsäure, Verwendung in der Herstellung von Injektabilia.
- Suppositorien für die rektale Verabreichung des Arzneistoffs können hergestellt werden durch Mischen des Arzneistoffs mit einem geeigneten nicht reizenden Bindemittel, wie zum Beispiel Kakaobutter und Polyethyenglycole, welche bei üblichen Temperaturen fest sind, aber flüssig bei der Reaktal-Temperatur und deshalb in dem Rektum schmelzen und den Arzneistoff freisetzen werden.
- Feste Dosierformen für die orale Verabreichung können Kapseln, Tabletten, Pillen, Pulver und Granulate einschließen. In solchen festen Dosierformen kann die aktive Verbindung mit mindestens einem inerten Verdünnungsmittel, wie zum Beispiel Saccharose, Laktose oder Stärke gemischt werden. Solche Dosierformen können auch, wie es normale Praxis ist, zusätzliche Substanzen außer den inerten Verdünnungsmitteln umfassen, zum Beispiel Schmiermittel, wie zum Beispiel Magnesiumstearat. In dem Fall von Kapseln, Tabletten und Pillen können die Dosierformen auch Puffersubstanzen umfassen. Tabletten und Pillen können zusätzlich mit Magensaft-resistenten Beschichtungen hergestellt werden.
- Flüssige Dosierformen für die orale Verabreichung können pharmazeutisch verträgliche Emulsionen, Lösungen, Suspensionen, Sirupe und Elixiere einschließen, die inerte Verdünnungsmittel enthalten, die üblicherweise im Fachgebiet verwendet werden, wie zum Beispiel Wasser. Solche Zusammensetzungen können auch Hilfsstoffe umfassen, wie zum Beispiel Benetzungsmittel, Emulgatoren und Suspendiermittel und Süßungs-, Geschmacks- und Duftstoffe.
- Die Kombination aus therapeutischen Wirkstoffen der vorliegenden Erfindung (als individuelle Zusammensetzungen oder als eine einzige Zusammensetzung) kann auch in der Form von Liposomen verabreicht werden. Wie im Fachgebiet bekannt ist, sind Liposome im Allgemeinen abgeleitet von Phsopholipiden oder anderen Lipidsubstanzen. Liposome werden durch mono- oder multilamellar hydrierte Flüssigkristalle, die in einem wässerigen Medium dispergiert werden, gebildet. Jedes nicht toxische, physiologisch verträgliche und metabolisierbare Lipid, das in der Lage ist Liposome zu bilden, kann verwendet werden. Die vorliegenden Zusammensetzungen in Liposomform können, zusätzlich zu der Verbindung der vorliegenden Erfindung, Stabilisatoren, Konservierungsmittel, Bindemittel und dergleichen enthalten. Die bevorzugten Lipide sind die Phospholipide und Phosphatidylcholine (Lecithine), sowohl natürlich als auch synthetisch.
- Verfahren zur Bildung von Liposomen sind im Fachgebiet bekannt. Siehe zum Beispiel Prescott, Hrsg., Methods in Cell Biology, BAnd XIV, Academic Press, New York, N.Y. (1976), Seite 33 und folgende.
- Bevorzugte Dosierformen für Ritonavir schließen folgende ein: (a) eine flüssige Dosierform für die orale Verabreichung, wie offenbart in U.S. Seriennummer 08/283,239, eingereicht am 29. Juli 1994 (nun
U.S. Patentnummer 5,484,801 , veröffentlicht am 16. Januar 1996), (b) eine verkapselte feste oder halbfeste Dosierform, wie offenbart in PCT-Patentanmeldung Nr.WO 95/07696 WO 95/09614
Claims (5)
- Eine Kombination aus Ritonavir oder einem pharmazeutisch verträglichen Salz davon und einem Arzneimittel, welches durch Cytochrom P450 Monooxygenase 3A4 metabolisiert wird, zur Verbesserung der Pharmakokinetik des Arzneimittels, vorausgesetzt, dass das Arzneimittel kein HIV-Protease-Inhibitor ist.
- Eine Kombination gemäß Anspruch 1, worin Ritonavir und das Arzneimittel als eine einzige Zusammensetzung formuliert werden.
- Eine Kombination gemäß Anspruch 1, worin Ritonavir und das Arzneimittel als separate Zusammensetzungen formuliert werden.
- Eine Kombination gemäß irgendeinem der obigen Ansprüche, worin die Dosis von Ritonavir von 0,001 bis 300 mg/kg Körpergewicht des Menschen beträgt, dem sie verabreicht werden soll.
- Eine Kombination gemäß irgendeinem der obigen Ansprüche, worin das Arzneimittel Cyclosporin, FK-506, Rapamycin, Taxol, Taxotere oder Clarithromycin ist.
