Abstract
本发明涉及延续释放人生长激素(hGH)的组合物以及所述组合物的制备及使用方法。本发明的延续释放组合物包括生物学相容的聚合物以及生物活性并稳定的人生长激素的颗粒,其中,所述的颗粒被分散在所述的生物学相容的聚合物内。本发明的制备延续释放生物学活性hGH的组合物的方法包括将生物学相容的聚合物溶于聚合物溶液中,得到聚合物溶液,将生物学活性并金属阳离子稳定的hGH颗粒分散于该聚合物溶液中,再将聚合物固化,形成含有分散的所述hGH颗粒的聚合物基质。本发明组合物的使用方法是给对象提供治疗有效血液水平剂量的生物学活性非凝集体hGH来实现一段时间的延续释放。采用本方法,就可以给对象提供有效剂量的本发明的延续释放组合物。本发明延续释放组合物的使用方法是给对象提供治疗有效血液水平剂量的生物学活性非凝集体hGH来实现一段时间的延续释放。
Description
与本发明相关的背景技术
人生长激素(hGH)是一种由脑下垂体分泌的蛋白质,可以利用重组遗传工程来制备。hGH可以促使任何具有生长潜力的身体组织的生长。
hGH一般用于治疗垂体机能减退的患者。现在的常规方法是将人生长激素的水液对患者进行皮下注射,每周三次或每日一次,从而保证患者的适当的hGH血清水平。对于长期接受人生长激素的患者来讲,经常性地注射人生长激素水液会导致患者应变性的问题。
为了解决伴随着注射人生长激素水液的问题,已经尝试过制备含有比注射液浓度更高剂量的可控释放装置,将人生长激素包被在聚合物和/或蛋白质内,使人生长激素可以在体内释放一周或更长。
然而,这些可控释放装置一般都表现出开始时的人生长激素释放水平很高,随后的释放就减少了。进而,由于这些延续释放装置内含有高剂量的人生长激素,所以,人生长激素分子就会在几天后发生凝集的现象,形成凝集的人生长激素,这种凝集的人生长激素在体内是免疫原性质的,而且活性降低。
因此,就存在着可以在体内延续释放生物学活性的人生长激素但又不引起免疫系统在人生长激素的释放期间内发生反应的制剂和装置。
本发明的概述
本发明涉及延续释放生物学活性的并被稳定的人生长激素(hGH)的组合物和所述组合物的制备方法以及使用方法。本发明的延续释放组合物包括生物学相容的聚合物以及生物学活性金属阳离子稳定的人生长激素的颗粒,其中,所述的颗粒被分散在所述的生物学相容的聚合物内。
本发明的制备延续释放非凝集体人生长激素的组合物的方法包括将生物学相容的聚合物溶于聚合物溶剂中,形成聚合物溶液,将生物学化学的并稳定化的人生长激素颗粒分散于该聚合物溶液中,再将聚合物固化,形成含有分散的所述的人生长激素颗粒的聚合物基质。
本发明延续释放组合物的使用方法是给对象提供一定剂量的所述延续释放组合物来使该对象获得治疗有效血液水平剂量的生物学活性非凝集体hGH。
这种人生长激素的延续释放制剂的优点是提供在体内的更长时间、更为一致的血液水平的人生长激素,降低开始时候的人生长激素的冲击水平,并通过消除血清人生长激素水平的波动来提高治疗效果。它的优点还包括通过减少注射次数而使患者的接受和相容性得到提高。这些优点还包括可以使用比注射更少量的人生长激素,因为血清人生长激素的水平被维持在靠近治疗阈限值附近。本发明的技术方案
本文中所述的人生长激素(hGH),是分子形式(单体或非凝集体)生物学活性的hGH。分子形式的hGH一般都不是免疫原性的。
凝集的hGH可以引起免疫反应,引起抗hGH的抗体的产生。这就会削弱长期hGH治疗的效果。再者,凝集的hGH可以刺激自体免疫系统对外源性hGH的反应。
生物学活性的非凝集体的人生长激素的延续释放,是在比直接注射hGH水液获得的释放期更长的期间内,释放可以测量到的血清水平的生物学活性单体性hGH。优选的延续释放期将可以是约一周或更长,更为优选的是两周或更长。
从聚合物基质内延续释放生物学活性的非凝集体hGH,可以是连续的或不连续的,其释放量可以是相对恒定的,也可以是变化的。hGH释放的连续性和释放的水平可以根据多种因素,例如,使用一种或多种聚合物组合物,hGH的加载量,和/或赋形剂的选择等来达到理想效果。
本文中,稳定的人生长激素包括生物学活性的、并被至少一种来自于金属阳离子组分中的多价金属阳离子复合的hGH,所述金属阳离子为+2价或更高。在本发明的延续释放组合物中的稳定的hGH是颗粒状的。
适宜的多价金属阳离子包括在生物学相容的金属阳离子组分中的金属阳离子。生物学相容的金属阳离子组分指的是,如果金属阳离子组分对接受者不具有毒性,也不产生明显的有害作用及副作用,例如不在注射位置产生免疫反应,则该金属阳离子就是生物学相容的。
一般来讲,在金属阳离子稳定的hGH中,金属阳离子组分对hGH的摩尔比为约4∶1至约10∶1。
优选的用于稳定hGH的金属阳离子是Zn+2。在更为优选的实施方案中,含有Zn+2阳离子的金属阳离子组分对hGH的摩尔比为约6∶1。
金属阳离子对于稳定hGH的适宜性可以由本领域的技术人员采用各类鉴定稳定性的技术来获得,例如,可以使用的方法有,聚酰胺凝胶电泳、等电聚焦、反向色谱、HPLC和对经过冷冻干燥的含有金属阳离子的hGH的效力实验等,从而确定经过冷冻干燥的hGH的效力和从微球中的释放期。在将hGH与聚合物溶液接触之前,将hGH与至少一种金属阳离子复合,可以减少被稳定的hGH由于延续释放组合物的化学特性、形成该延续释放组合物的方法、水合等造成的生物学活性的损失,也可以减少在体内水合时hGH在微球内发生的凝集倾向。
稳定的hGH一般可以在整个延续释放期内保持其不发生显著的凝集。显著的凝集指的是有约15%或更多的所包被的起始hGH被凝集。优选的是,保持该凝集低于起始hGH单体剂量的约5%。更为优选的是,保持该凝集低于起始hGH单体剂量的约2%。
在hGH延续释放组合物中的hGH还可以含有其它的赋形剂,例如填充剂和另外的稳定剂,例如,在冷冻干燥期间稳定hGH的缓冲液。
填充剂一般包括惰性物质。适宜的填充剂都是本领域所熟知的。
适宜于本发明的延续释放组合物的聚合物或聚合物基质必需是生物学相容的。如果聚合物和其降解后的产物对接受者不具有毒性,也不产生明显的有害作用及副作用,例如不在注射位置产生免疫反应,则该聚合物或其聚合物基质就是生物学相容的。
在hGH延续释放组合物中的聚合物还必须是生物学降解的,即该组合物会在体内降解或腐蚀为化学小分子物质。造成降解的原因可以是酶学过程、化学过程、或物理过程。
适宜的生物学相容、生物降解的聚合物包括,例如,聚(丙交酯),聚(乙交酯),聚(丙交酯-共-乙交酯),聚(乳酸),聚(乙醇酸),聚(乳酸-共-乙酯酸),聚己内酯,聚碳酸酯,聚酰胺酯,聚酐,聚(氨基酸),聚原酸酯,聚缩醛,聚(对-二噁烷),聚(亚烷基烷基酯),生物降解的聚氨基甲酸酯,以及它们的混合物和共聚物。
另外,聚合物的终端基团可以被修饰。例如,聚合物可以被封闭,未被封闭,或者是被封闭物与未被封闭物的混合物。被封闭的聚合物指的是本领域内传统定义的被封闭物,特别是羧基终端被封闭。通常,封闭基团从聚合反应的起始物衍生而来,并且通常是酰基基团。未被封闭的聚合物如本领域内通常所定义,特别是具有自由羧基终端基团。
本发明中可用的聚合物的分子量,可以由本领域的技术人员根据所需的聚合物降解速率,物理性质如机械强度,聚合物在溶液中的溶解速率等因素来确定。通常,可接受的分子量的范围在约2,000道尔顿至约2,000,000道尔顿。在优选方案中,聚合物是生物降解的聚合物或共聚物。在更优选的方案中,聚合物是聚(丙交酯-共-乙交酯)(此后缩略为“PLGA”),且丙交酯与乙交酯的比率为约1∶1,分子量大约为5,000道尔顿至约70,000道尔顿。在更为优选的方案中,本发明中所用的PLGA的分子量为约6,000~31,000道尔顿。
延续释放组合物微球内所含有的hGH数量,或者是在另外类型延续释放装置内用含有的凝集稳定的hGH颗粒的数量是预防性或治疗性有效量,可以由本领域的技术人员根据体重,病状,使用的聚合物的种类,从聚合物中释放出来的量等因素来决定。
在一个实施方案中,hGH延续释放组合物含有约0.01%(w/w)至约50%(w/w)生物学活性稳定的hGH颗粒。hGH颗粒的使用量将根据需要hGH达到的效果,计划的释放水平,释放hGH的时间,hGH的释放期等而变化。优选的hGH颗粒装载量的范围在约0.1%(w/w)至约30%(w/w)hGH颗粒。更优选的hGH颗粒装载量的范围在约0.1%(w/w)至约20%(w/w)hGH之间。最优选的生物学活性凝集稳定的hGH颗粒的装载量为约15%(w/w)。
在另一个优选方案中,hGH延续释放组合物还含有第二种金属阳离子组分,但是,它并不含在生物学活性凝集稳定的hGH颗粒之内,而是分散在聚合物之中。优选的第二种金属阳离子组分是与稳定的hGH中所含的金属阳离子相同的金属阳离子。此外,该第二种金属阳离子组分可以含有一种或一种以上的不同的金属阳离子。
所述的第二种金属阳离子组分起着调解hGH从延续释放组合物的聚合物基质中的被释放,例如,可以起到金属阳离子库的作用,从而进一步延长释放期,即hGH被金属阳离子稳定并使其在组合物中的稳定性得到了提高。
用于调控释放的金属阳离子成分包括至少一种多价金属阳离子。适宜的调控释放hGH的金属阳离子组分的实例包括或含有Mg(OH)2、MgCO3(例如,4MgCO3·Mg(OH)2·5H2O)、ZnCO3(例如,3Zn(OH)2·2ZnCO3)、CaCO3、Zn3(C6H5O7)2、Mg(OAc)2、MgSO4、Zn(OAc)2、ZnSO4、ZnCl2、MgCl2和Mg3(C6H5O7)2。优选的第二种金属阳离子组分与聚合物的重量比例为约1∶99至约1∶2。最佳的比例取决于所使用的聚合物和金属阳离子组分。
在本申请的共悬未决美国专利申请08/237,057和共悬未决的PCT申请PCT/US 95/05511号中,有对于用含有分散的金属阳离子组分的聚合物基质调控从聚合物基质中释放生物活性剂的进一步描述,这两篇文献的教导都通过在此引述而合并于本文。
在本发明中的hGH延续释放组合物可以为膜状、片状、筒形、盘状和微颗粒状。在本文中所说的微颗粒包括直径小于约1毫米的聚合物颗粒,并有稳定的hGH分散于其中。微颗粒可以是球形的、非球形的、或非规则状的。优选微颗粒是微球形的。一般,微颗粒的尺寸应该适宜于注射。微颗粒的优选直径尺寸范围是约1至180微米。
在本发明的制备生物学活性非凝集的hGH延续释放组合物的方法中,适宜量的生物学活性稳定的hGH颗粒被分散在聚合物溶液中。
适宜的聚合物溶液含有大约1%(w/w)至约30%(w/w)的适宜的生物学相容的聚合物,其中,生物学相容的聚合物通常溶于适宜的聚合物溶液中。优选的是,聚合物溶液中含有约2%(w/v)至约20%(w/v)聚合物。最优选的聚合物溶液中含有约5%至约10%(w/w)聚合物。
适宜的聚合物溶剂指的是,聚合物在其中溶解,但是,稳定的hGH颗粒在其中基本不溶解并且不反应。适宜的聚合物溶剂的例子有,极性有机液体,例如,二氯甲烷,氯仿,乙酸乙酯和丙酮。
制备生物学活性稳定的hGH颗粒时,hGH与含有至少一种适宜的金属阳离子适宜的水液在适宜形成金属阳离子与hGH的复合体的pH条件下混合。一般,复合的hGH是絮状沉淀,悬浮于溶剂中。然而,复合的hGH也可以是溶液形式的。在优选的实施方案中,hGH与Zn+2复合。
适宜于形成复合hGH的pH条件一般为约7.0~7.4。一般,获得适宜的pH的方法是使用缓冲溶液作为溶剂,例如,用碳酸氢钠来作为溶剂。
适宜的溶剂是那些hGH及金属阳离子组分各自可以至少微溶于其中的,例如碳酸氢钠水液。对于水性溶剂,优选使用的水是共离子水或注射用水。
应该理解的是,hGH在与抗凝集剂组分接触之前,可以是固态或溶解状态的。还应该理解的是,抗凝集剂在与hGH接触之前,可以是固态或溶解状态的。在优选的技术方案中,是将hGH水溶液与抗凝集剂组分的水溶液相混合来形成稳定的混合物。
一般来讲,复合的hGH是絮状沉淀,悬浮于溶剂中。但是,复合的hGH也可以是溶液形式的。在优选的实施方案中,hGH与Zn+2复合。
对复合的hGH进行干燥,例如冷冻干燥,形成稳定的hGH颗粒。在悬浮液或溶液中的稳定的复合hGH可以成批量的冷冻干燥,也可分成小部分后干燥。在优选的技术方案中,复合的hGH悬浮液被微颗粒化,例如,使用超声喷嘴,然后冷冻干燥,形成稳定的hGH颗粒。可以被使用的对复合的hGH的混合物进行冷冻干燥的方法都是本领域技术人员所熟知的。
在优选的技术方案中,稳定的hGH的颗粒直径为约1至6微米。hGH颗粒可以破碎分开,如在共悬未决的1993年1月21日提交的美国专利申请08/006,682所描述的生产生物学活性制剂小颗粒的方法,该篇文献通过在此引述而全部合并于本文。另外,hGH颗粒可以在加入到聚合物溶液之后再破碎,例如,使用超声破碎、或超声喷嘴。
在本发明方法的一个技术方案中,聚合物溶液中还有不含在稳定hGH颗粒内的第二种金属阳离子组分,该第二种金属阳离子组分被分散在聚合物溶液中。
应该理解的是,第二种金属阳离子组分和稳定的hGH颗粒可以依次,颠倒,间隔,分别和同时加入到聚合物溶液中。另外,聚合物,第二种金属阳离子组分和稳定的hGH可以依次,颠倒,间隔,分别和同时加入到聚合物溶剂中。
调控从生物降解聚合物中释放生物活性剂的组合物的形成方法在共悬未决的美国专利08/237,057中有进一步的描述。
在该方法中,聚合物溶剂被固化,形成含有被分散的稳定hGH颗粒的聚合物基质。
一种适宜的以聚合物溶液固化形成含有凝集稳定的hGH的聚合物基质的方法是溶剂蒸发法,见于授予Schnoring等的美国专利3,737,337,授予Vranchen等的美国专利3,523,906,授予Kitajima等的美国专利3,691,090,以及授予Tice等的美国专利4,389,330,这些文献都通过在此引述而全文合并于本文。溶剂蒸发法通常都用作形成hGH延续释放微颗粒的方法。
在溶剂蒸发方法中,含有稳定的hGH颗粒悬浮液的聚合物溶液混合到或搅拌到使聚合物溶剂部分混溶的连续相中,形成乳化液。通常,乳化剂都包括在连续相中以稳定乳化液。然后,对聚合物溶剂进行数小时或更长的蒸发,使得聚合物固化,得到分散有稳定的hGH颗粒的聚合物基质。
优选的从聚合物溶液中制备凝集稳定的hGH微颗粒的方法采用了快速冷冻和溶剂萃取的方法,可见于授予Gombotz等的美国专利5,019,400和在1995年5月18日申请的共悬未决美国专利申请08/433,726,这两篇文献中的教导都通过在此引述而合并于本文。与其它方法相比,例如,与分相法相比,这种形成微球的方法可以进一步地减少制备对某种hGH的延续释放组合物的方法中的hGH的用量。
在这种方法中,含有稳定的hGH颗粒分散液的聚合物溶液被处理以生成液滴,而且这些液滴中的大部分都含有聚合物溶液和稳定的hGH颗粒。这些液滴再经过适宜的方法冷冻,形成微颗粒。对聚合物溶液分散液进行处理来生成液滴的方法的示例包括将分散液直接通过超声喷嘴,压力喷嘴,瑞利喷射(Rayleigh jet),或者其它的已知的制造溶液液滴的方法。
适宜的将液滴冷冻来形成微颗粒的方法包括将液滴放置于或使其接近于液态气体,例如,液态氩,液态氮,形成冷冻的微液滴,然后再与液态气体分离。随后,再将微液滴与非溶剂液体接触,例如,乙醇,或者是乙醇与己烷或戊烷的混合物。
冷冻的微液滴中的溶剂被以固体和/或液体的形式萃取到非溶剂中,形成含有生物学活性凝集稳定的hGH微颗粒。加入乙烷与其它的非溶剂液体例如己烷或戊烷的混合物,可以提高从聚合物中萃取溶剂的萃取率,例如,从聚(丙交酯-共-乙交酯)聚合物中萃取溶剂的比率。
通过改变液滴的大小,可以制备很宽尺寸范围的hGH延续释放微颗粒,例如,可以通过改变超声喷嘴的口径来改变液滴的大小。如果需要尺寸比较大的微颗粒,可以用注射机直接将微颗粒挤压到冷液体中来制备。增大聚合物溶液的粘度也可以增大微颗粒的尺寸。由这种方法生产的微颗粒大小可在很宽的范围内变化,例如,可在直径为1000微米至1微米或者更小的范围内变化。
从聚合物溶液中制备hGH延续释放组合物的另一种方法包括挤压成膜,例如,使用模具制备薄膜或其它形状。例如,当把含有稳定的hGH颗粒分散液的聚合物溶液放入模具后,聚合物溶剂就可以通过已知的方法去除,或者通过降低聚合物溶液的温度,直到形成具有均匀干重薄膜或其它固定形态为止。在共悬未决的美国专利申请08/237,057中,对用聚合物溶液制备含有生物活性物质的成膜方法有进一步的描述。
据信,hGH的释放有两种机制。hGH可以通过聚合物基质中的充满液体的通道渗透释放,例如,通过溶解hGH或者通过在合成延续释放组合物的过程中去除聚合物溶剂形成的空腔来完成。
第二种机制是通过聚合物的降解来释放hGH。降解的速率可以通过改变聚合物水合速率的聚合物性质来控制。这些性质包括,例如,聚合物中不同单体的比例,例如,丙交酯与乙交酯的比例;使用L-旋异构体来替代外消旋混合物;聚合物的终端基团和分子量等。这些性质影响亲水性和结晶性,从而控制聚合物的水合速率。亲水性赋形剂,例如,盐、碳水化合物、表面活性剂也可以被加入来提高水合性,从而改变聚合物的侵蚀速率。
通过改变聚合物的性质,可以控制渗透和/或聚合物降解对hGH释放的作用。例如,提高聚(丙交酯-共-乙交酯)聚合物中乙交酯成分的含量、降低聚合物的分子量都可以提高聚合物的水解,从而使聚合物腐蚀造成的hGH释放得到提高。
此外,聚合物的水解速率可以在非中性pH值的范围内得到提高。因此,可以将酸性或碱性物质加入到用于形成微球的聚合物溶液中,来改变聚合物侵蚀的速率。
本发明的组合物可使用于人和动物,可以通过注射,埋植(例如,皮下,肌肉,腹膜内,颅内,阴道内和皮内)使用,也可以施用于粘膜(例如,鼻腔内或以栓剂的方式施用),或就地施用(例如,灌肠或喷雾剂),从而根据已知的病理条件对各种需要hGH治疗的病状提供理想的hGH剂量。
本发明将由以下的实施例进一步详细描述。

实施例1
Zn+2-稳定的hGH的制备
在本实施例中使用的是人生长激素(hGH),其DNA序列在授予Goeddel等人的美国专利第4,898,830号中给予了描述。人生长激素与锌形成不溶性复合体而稳定。
hGH溶于4mM碳酸氢钠缓冲液(pH7.2)中,形成浓度为约0.1~0.5mM的hGH溶液。用乙酸锌二水合物与去离子水形成0.9mM的Zn+2溶液,然后加入到hGH溶液中形成Zn+2-hGH复合物。该Zn+2-hGH复合物的pH值用加入1%乙酸来调节到7.0~7.4之间。形成絮状沉淀,含有形成的Zn+2-稳定的hGH复合物。
该Zn+2-稳定的hGH复合物悬浮液用超声喷嘴(V1A型;Sonics and Materials,Danbury,CT)雾化,喷洒到含有液氮的聚丙烯管内(17厘米直径,8厘米深)形成冷冻颗粒。然后将该聚丙烯管放入-80℃的冷冻箱内,直至液氮挥发。含有Zn+2-稳定的hGH复合物的颗粒经过冷冻干燥得到Zn+2-稳定的hGH颗粒。
实施例2
含有生物学活性凝集稳定的人生长激素的PLGA微球的制备
含有Zn+2-稳定的hGH用亲水性的带有自由终端羧基的聚(丙胶酯-共-乙胶酯)聚合物RG502H(此后称为“未封闭的PLGA”)(50∶50 PLGA,9,300道尔顿;Boehringer Ingelheim Chemicals,Inc.),或更加疏水性的带有封闭的终端羧基的PLGA聚合物(此后称为“封闭的PLGA”)(50∶50;分子量10000道尔顿;标号#115-56-1,Birmingham Polymers,Inc.,Birmingham,AL)来制备。
室温下将聚合物溶解于二氯甲烷中。在聚合物溶液中加入冷冻干燥的hGH颗粒和碳酸锌。对该混合物进行超声处理得到均质的的悬浮液。用超声喷嘴将该悬浮液雾化到附加有液氮的冷冻乙醇床中。含有微球的容器被放在-80℃下保藏来萃取二氯甲烷,然后冻干得到自由流动的粉末。
实施例3
包被的hGH蛋白质的分析
对包被的hGH的整体性进行确定,将未水合的微球溶于二氯甲烷和丙酮中,收集蛋白质,冻干,再次溶于含有10mM EDTA的HEPES缓冲液中。同时进行对照分析,保证萃取过程没有影响蛋白质的整体性。
采用尺寸排除色谱法(size exclusion chromatography(SEC))检测在包被之后hGH中含有的hGH单体的百分数来确定包被的hGH的整体性。
对延续释放hGH微球微球的hGH整体性的SEC检测结果见于下表。

这些分析表明,包被的过程没有造成蛋白质的凝集。在提取过程所获得的蛋白质的百分数(相对于测定的微球中的氮的含量)为40~98%。发生差异的原因被认为是在检测方法中的转移步骤造成了材料的损失,因而对该提取程序进行了修正以提高蛋白质的回收率。
实施例4
碳酸锌对体外释放动力学的影响
按照实施例2制备微球,微球含有15%w/w hGH(6∶1的Zn∶hGH蛋白质复合物),分别含有0%、1%、6%、10%、20%w/w碳酸锌,以及聚(丙胶酯-共-乙胶酯)聚合物。
含有不同浓度的碳酸锌的hGH延续释放微球的体外释放动力学的确定,是将每种微球的等份试样(10mg)置于1.5ml的HEPES缓冲液(50mM Hepes,10mM KCl,0.1%NaN3)pH7.2中,然后在37℃培养。在培养开始后的第1、3、7、10、14、21、28天对缓冲液取样,取样后补充新鲜的缓冲液,然后定量样品中释放的蛋白质含量。
以释放的百分数的积累值(相对于在微球中开始的hGH含量)对时间做作图。用尺寸排除色谱对每一个时间点的释放蛋白质的样品测定其hGH单体的含量。
碳酸锌被认为是起着锌离子库的作用,因此,对形成Zn-hGH复合物是有利的,而对于解离为溶解性hGH是不利的。因为碳酸锌的水溶解性低,所以从该库中释放锌离子的速度低,从而调节了蛋白质的溶解性。
该分析发现,如果没有碳酸锌,则包被的hGH的释放速率非常高,蛋白质在很短的时间内就全部被释放。
实施例5
封闭的PLGA Zn+2-稳定的hGH微球在体内降解后的hGH分析
按照实施例2制备含有15%w/w Zn+2-稳定的hGH并分别含有0%、6%、10%、或20%碳酸锌的封闭的PLGA微球。受试的大鼠接受50mg不同hGH微球样品的皮下注射。60天后处死大鼠,剪除下注射部位的皮肤样品。将该剪除下来的皮肤样品放入10%中性缓冲的福尔马林中至少24小时。然后用剃刀去除多余的皮肤,并放入PBS中。
组织样品由Pathology Associates,Inc.(Frederick,MD)处理。将皮肤样品放入糖代异丁烯酸盐中,切块,并依照生产厂商的说明书进行染色以对hGH进行存在的检测[用HistoScan/LymphoScanStaining Kit染色(产品标号#24-408M;Accurate Chemical &Scientific Corp.,Westbury,NY)]。组织样品中没有发现染色,这就标明组织样品中不存在hGH。
所有与hGH微球注射有关的皮肤样品都有hGH存在的阳性结果,因此就标明封闭的PLGA微球在体内60天后还含有hGH。
按照实施例2的方法,将0%或15%的Zn∶hGH复合物形式的hGH,以及0%、1%或6%w/w的碳酸锌包被在封闭的和未封闭的PLGA中形成微球。
比较未封闭的和封闭的PLGA微球的体内降解,即将微球样品注射给大鼠,然后分析注射后不同时间的注射位置所残留的微球。对每一种微球样品在每一个时间的检测都是对三只大鼠进行的。在注射微球的当天,在含有50±1mg微球的药瓶中加入750μl的载体[3%羧甲基纤维素(低黏度)和1%Tween-20于生理盐水中]。然后,立即剧烈摇动该药瓶并形成悬浮液,再用不带针头的1.0cc注射器吸取药液。
大鼠(雄性Sprague-Dawley)用三氟氯溴甲烷与氧气的混合物麻醉。注射位置(肩胛区)去毛,用永久性图标进行标记以保证在取样的时候准确剪切皮肤。用18至21号注射器给每只大鼠注射一整药瓶的微球药液。
在预定的时间(对于接受封闭的PLGA微球的大鼠来讲为注射后的第15、30、59和90天,对于接受未封闭的PLGA微球的大鼠来讲为注射后的第7、14、21、28和45天),用二氧化碳使大鼠窒息致死,剪切下注射位置的皮肤(包括微球)。由于微球会在注射位置集结,因此,可以用肉眼观察辨认是否有微球存在于注射位置。
肉眼观察发现的是,未封闭的PLGA微球的降解速率比封闭的PLGA微球快很多,并且,加入碳酸锌到封闭的PLGA中明显降低了聚合物的降解。例如,在接受了含有0%hGH和0%或1%碳酸锌的未封闭的PLGA微球注射的大鼠中,在第21天时没有观察到微球。此外,对于接受了含有0%hGH和0%碳酸锌的封闭的PLGA微球注射的大鼠来讲,在第60天时观察到有少量微球,在第90天时没有观察到微球。再者,接受含有0%或15%hGH以及6%碳酸锌的封闭的PLGA微球注射的大鼠,在第90天有可观察到的微球。
实施例6
hGH延续释放微球在大鼠体内的药物动力学的研究
本研究在大鼠中进行,筛选不同的hGH微球的配方,确定静脉注射(IV)、皮下注射(SC)和SC渗透泵(Alzet)给药hGH后的药物动力学参数,评价各种hGH微球配方的体内释放速率和血清水平。
对Sprague-Dawley大鼠随机按照体重分为三只一组,每组给予一种hGH微球制剂。注射采用皮下注射,7.5mg的hGH在50mg的一种微球中,悬浮于0.75ml注射用水性载体中。载体的成分是,3%CMC(低黏度)、1%Polysorbate 20、在0.9%NaCl中。对注射的微球的剂量采用间接方法确定,即称量注射药瓶中的残留剂量并对残留注射载体进行校正。然后,用氮的分析确定微球中加载的蛋白质量,并计算出hGH剂量。
在预定的时间,即注射后30天内收取血样。在第一个24小时内收集250μl血样,在第一个24小时之后,各个取样时刻收取的血样至少为400μl。使血样凝结,用放射免疫检测方法确定血清中hGH浓度。对来自ICN的放射免疫(RIA)试剂盒进行鉴定,然后用于确定大鼠血清中hGH的水平。
为了确定药物动力学的参数,给大鼠皮下注射、静脉内注射或用埋植于皮下的渗透泵(Alzet型号2ML4)给药处于生理盐水中的hGH。
有三组大鼠接受了单一皮下注射处于0.9%NaCl中的hGH,剂量为0.5或7.0mg/kg,体积为1.0ml/kg,有两组接受了单一静脉内注射处于0.9%NaCl中的hGH,剂量为约1.0和5.0mg hGH/公斤大鼠体重,体积为1.0ml/kg。对于Alzet泵来讲,大鼠按每组三只分为四组,体重随机,对泵(Alzet型号2002,200μl,14天释放期)内加载的剂量是约20mg/ml和40mg/ml hGH处于0.9%生理盐水中,对泵(Alzet型号2ML4,2ml,28天释放期)为约4mg/ml和12mg/ml hGH处于0.9%生理盐水中。各个泵的预期释放速率分别为约2%和4%至6%的ProLease hGH剂量(约15mg/kg)每天。这些Alzet泵被埋植在肩胛区的皮下,并用无菌生理盐水预先浸泡1-2分钟。
按照实施例2合成的hGH延续释放微球制剂含有15%w/w与锌复合的hGH,比例为6∶1 Zn∶hGH;0%、1%、3%、或6%w/w碳酸锌;8k未封闭的PLGA、10k封闭的PLGA或31k未封闭的PLGA。
为了评价各种hGH延续释放制剂,采用的体内指标为Cmax、Cd5、和Cmax/Cd5,其中,Cmax是观测到的最大血清浓度,Cd5是第5天的血清浓度,大约是稳定期的浓度。结果如下

该筛选的实验结果表明,两种未封闭的(8k和31k)的体内释放动力学与原始配方即封闭的10k PLGA和6%w/w碳酸锌的释放动力学不同。一般来看,使用了未封闭的聚合物的制剂的Cmax数值比较低,指示这类制剂的体内始发冲击比较低。始发冲击指的是在注射后的第一个24小时内释放的hGH的百分数。体外始发冲击的数值为8-22%。各制剂内的碳酸锌成分看来好象没有影响始发冲击和体外释放分布型。
在第4天到第6天,未封闭的聚合物的血清浓度基本上保持稳定于基线(或采取血样之前的水平)之上,在同一时间的封闭的聚合物的血清浓度则很接近基线。7天的体外释放数据表明释放的hGH蛋白质是单体的。在第6天以后,因为大鼠产生了抗-hGH抗体,所以无法获得有用的数据。
实施例7
猕猴的药物动力学的研究
本研究的目的是评价不同的hGH延续释放制剂与传统hGH给药方式(例如,皮下注射、每日皮下注射、以及皮下注射与渗透泵的结合)之间的药物动力学。从而确定那种hGH延续释放制剂提供最佳的hGH血液浓度分布型。
接受实验的hGH延续释放微球制剂有①15%hGH(与锌按照6∶1 Zn∶hGH的比率复合)、6%w/w碳酸锌和10k封闭的PLGA;②15%hGH(与锌按照6∶1 Zn∶hGH的比率复合)、1%w/w碳酸锌和8k未封闭的PLGA(“RG502H”PLGA聚合物);和③15%hGH(与锌按照6∶1 Zn∶hGH的比率复合)、1%w/w碳酸锌和31k未封闭的PLGA(“RG503H”PLGA聚合物)。
每组中有四只猴子,每只猴子在第一天接受颈背区单一皮下注射。用20号针头将处于1.2ml注射载体中的剂量为160mg的hGH延续释放微球(24mg的hGH)注射给每一只猴子。注射载体是水性载体,含有3%w/v羧甲基纤维素(钠盐)、1%v/v Tween 20(Polysorbate 20)和0.9%氯化钠。
这样的hGH剂量是为了提供可以被检测到的hGH血清浓度来进行药物动力学分析。为了获得药物动力学参数,还包括了另外的每组四只猴子的研究组,每一组的处理如下:①单一皮下注射(24mg hGH);②每日皮下注射(24mg/28天=0.86mg hGH/天);③皮下注射(3.6mg hGH)结合Alzet渗透泵(20.4mg hGH);和④皮下注射载体作为对照(对照组只用了三只猴子)。
对于hGH、IGF1、IGFBP3和抗-hGH抗体的分析,在下述时间采取血样:-7、-5、-3、给药前、和给药后的第0.5、1、2、3、5、8、10、12、24、28、32和48小时,第4、5、6、8、11、14、17、20、23、26、29、32、35、38、41、44、47、50、53和56天。
然后测定血清中的IGF-1和hGH。使用了来自RADIM(提供者为Wein Laboratories,P.O.Box 227,Succasunna,NJ)的IRMA试剂盒来定量猴子血清中的hGH。该IRMA检测的定量性极限在PBS缓冲液中为0.1ng/mL,在合并的年幼猕猴血清中为1.5ng/mL,基准GH水平为约4ng/mL。
该IRMA检测对合并的年幼猕猴血清在1.5-75ng/mL之间为有效的。该测定的准确性和精确性在±10%内。
结果表明,hGH延续释放微球可以在一个月的时间内释放出显著的和延续性水平的hGH,而皮下注射则不能够维持同样的水平。
IGF-1血清分布型表明,在注射了微球后的第2天到第29天,血清IGF-1浓度高于基线值。这就说明有足够的hGH被从延续释放微球中释放出来并造成了药物动力效应。同时,还表明,所释放的hGH是生物学活性的,从而说明包被过程没有对hGH的生物学效力产生副作用。等同物
本领域的技术人员都将或能够在采用常规实验的条件下认识和确定本发明的具体技术方案的等同物。这些等同物都是本发明的权利要求书所要求保护的。
Claims (21)
1.一种延续释放人生长激素的组合物,包括:
(a)生物学相容的聚合物;和
(b)金属阳离子复合的人生长激素颗粒,其中所述的金属阳离子与蛋白质的摩尔比为高于4∶1,最高可达10∶1,所述的金属阳离子是从Zn+2,Ca+2或Mg+2中选取的。
2.根据权利要求1所述的组合物,其中所述的摩尔比至少为6∶1。
3.根据权利要求1或2所述的组合物,其中所述的金属阳离子是Zn(II)。
4.根据上述任何一项权利要求所述的组合物,其中所述的金属阳离子是以水溶性盐的形式加入到人生长激素中的。
5.根据权利要求4所述的组合物,其中所述的金属阳离子是以乙酸盐的形式加入到人生长激素中的。
6.根据上述任何一项权利要求所述的组合物,其中所述的金属阳离子复合的人生长激素是被分散在所述的生物学相容的聚合物中的。
7.根据权利要求6所述的组合物,其中所述的聚合物选自聚(丙交酯),聚(乙交酯),聚(丙交酯-共-乙交酯),聚(乳酸),聚(乙醇酸),聚己内酯,聚碳酸酯,聚酰胺酯,聚酐,聚(氨基酸),聚原酸酯,聚缩醛,聚(二噁烷),聚(亚烷基烷基酯),聚氨基甲酸酯,以及它们的混合物和共聚物。
8.根据权利要求7所述的组合物,其中所述的聚合物选自聚(丙交酯),聚(乙交酯),聚(丙交酯-共-乙交酯)。
9.根据前述任何一项权利要求所述的组合物,其中所述的金属阳离子复合的人生长激素颗粒在所述聚合物中的重量含量为0.1%至30%。
10.根据权利要求9所述的组合物,其中所述的金属阳离子复合的人生长素颗粒在所述的聚合物中的重量含量为0.1%至20%。
11.根据权利要求9所述的组合物,其中所述的金属阳离子复合的人生长激素颗粒在所述聚合物中的重量含量为15%。
12.根据前述任何一项权利要求所述的组合物,其中所述的生物学相容的聚合物还进一步含有分散到聚合物中的金属阳离子组分,其中金属阳离子组分是多价金属阳离子,是从Zn+2,Ca+2,和Mg+2中选取的。
13.根据权利要求12所述的组合物,其中所述的金属阳离子组分的金属阳离子是Zn(II)。
14.根据权利要求13所述的组合物,其中所述的金属阳离子组分选自ZnCO3、Zn3(C6H5O7)2、Zn(Oac)2、ZnSO4、ZnCl2。
15.根据权利要求12所述的组合物,其中所述的金属阳离子组分的金属阳离子是Mg(II)。
16.根据权利要求15所述的组合物,其中所述的金属阳离子组分选自Mg(OH)2、MgCO3、Mg3(C6H5O7)2、Mg(Oac)2、MgSO4、MgCl2。
17.根据上述权利要求13-16中的任何一项所述的组合物,其中所述的金属阳离子组分与聚合物的重量比为1∶99~1∶2。
18.根据权利要求13-16中的任何一项所述的组合物,其中所述的金属阳离子组分与聚合物的重量比为1%。
19.根据权利要求1所述的人生长激素延续释放组合物,该组合物包括:
(a)生物学相容的聚(丙胶酯-共-乙胶酯),并且其中分散有以ZnCO3形式加入的Zn(II);以及
(b)被Zn(II)复合的人生长激素颗粒,其中的Zn(II)是被以乙酸锌的形式按照Zn∶hGH为10∶1的摩尔比加入到人生长激素中的;以及
所述的Zn(II)复合的人生长激素颗粒在所述的聚合物中的含量为15%(重量比)。
20.上述任何一项权利要求所述的组合物在制备药物中的应用。
21.根据权利要求1所述的组合物,其中所述的摩尔比为10∶1。
