Abstract

Description
本发明涉及溶菌酵素二聚体新的医疗用途及其含有这样的二聚体的制剂。所述的新用途涉及对自然防御机制的特定的机能变常的治疗。
很久以来人们就知道,单节形酶对治疗各种疾病有疗效。
溶菌酵素是Fleming于1922年发现的,但在1950年以后,人们才刚刚发现它具有酶的机能。从那时起,这种化合物已成为人们致力研究的一个对象,并且其各种疗效也被报道出来。其中有抗病毒、抗菌、消炎和抗组胺的效果。不过,由于发现单节形的溶菌酵素具有细胞毒副作用,其在治疗方面的应用受到很大的限制。
在80年代后期,当人们发现游离的二聚体型酶在保留了已知的单节型酶的所有有益的性能的同时并没有表现出细胞毒性副作用时,对溶菌酵素和其它具有疗效活性酶实际使用的这方面的限制已经得到克服。国际专利申请PCT/US88/01785中公开了包括作为活性组分的溶菌酵素二聚体或其它二聚体化的酶的抗病毒和抗菌制剂。根据该申请的报道,在体外试验中,溶菌酵素二聚体抑制了从病人体采集的样品中在5-20mg/ml的培养浓度下培养的多种菌株的增生。此外,按1-2mg/ml体重的剂量每天口服两次二聚体,可以治疗犬牙的细小病毒的感染。
随着研究工作的继续,现已发现溶菌酵素二聚体还具有其它有吸引力的特性,同时也开发出该药物的新的治疗用途。
在为证实溶菌酵素二聚体的抗菌和抗病毒作用而进行的临床试验中,令人鼓舞地发现该溶菌酵素二聚体对治疗消化道和呼吸道的急性疾病有出人意料的效力。相应地,选择新的研究方向以确定溶菌酵素二聚体对天然防御机制失效的不同疾病的各个阶段的病人的疗效。
众所周知,细菌毒素构成了一大类多种互性反应因子,细菌就是通过这些毒性反应因子而致病。近来在细菌毒素方面认识的进展涉及到其与宿主免疫系统的相互作用。这种相互作用首先导致免疫调节,然后释放出细胞毒素(cytokines)和其它间介,用这种相互作用可以解释由毒素引起的许多生理紊乱。现已参照外毒素的作用而对后一种作用进行了专门的研究,所述的外毒素对格兰阴性脓毒症的发病机制起了重要的作用(见Bayston,D.F.,Cohen,J.:Bacterial endotoxins and current concepts in the diagnosis and retreatment of endotoxaemia;J.Med.Microbiol.1990,31:73-83)。虽然长期以来外毒素对由葡萄球菌和酿脓链球菌所致的感染的作用已为人所知,现在人们已公认,正是由于葡萄球菌中毒性休克综合症才导致了人们对这些机制所产生的外毒素有了越来越浓厚的兴趣。
中毒性休克是一种严重的疾病,症状是高热、高血压、渗透压下降(capillary leak)、扩散性红皮病、粘膜红斑、肾缺损、低血钙、血白蛋白减少以及红皮疹所致的脱屑。许多中毒性休克综合症的病例与在月经期使用阴道塞有关,但越来越多的报道表明,该综合症经常发生在经手术而把填充材料放置到位(如鼻成形术或严重的鼻出血后所进行的鼻填充)之后,而与性别或是否处于月经期无关。现已证明从患中毒性休克综合症(TSS)的患者阴道中分离出的葡萄球菌株可产生中毒性休克综合症毒素1(TSST-1),但是产生TSST-1的微生物源也可能是一种不显性感染。初发的菌血症可能是不显性的,但几周或几个月以后可能发展成局部感染。在出现上述感染的同时,有证据表明出现了脓毒性综合症或脓毒性休克。较为罕见的但却是更加严重的菌血症可不发生在任何门户或者可与局部感染有关,在这些情况下,休克、心内膜炎、散布性血管凝血病和多组织衰竭会更加突出(见Stevens,D.L.等人:Gram-positive shock;Current Opinion in Infectious Diseases 1992,5:355-363)。
与之类似的观察还涉及到其它革兰阳性菌。例如,酿脓链球菌感染与休克有关,并且死亡率达到30%。肺炎双球菌是肺炎的一个病因,据文献报道,它对青霉素有高度的抗性,并且易发展为休克综合症。此外,爱滋(AIDS)病人比总体人群发生肺炎球菌感染的可能性要高得多。
革兰阴性菌感染还可导致脓毒症和脓毒性休克。革兰阴性杆菌和弧菌是大多数重要的肠毒素的源。肠毒素是革兰阴性菌细胞壁的外膜的一种脂多糖类(LPS)。肠毒素主要影响消化道,通常引起腹泻。在动物和人中发生的最多的革兰阴性菌感染是大肠杆菌感染。伴随这类感染的严重的脱水可使受感染的个体死亡。根据世界卫生组织(WHO)的报告,每年在发展中国家因急性腹泻而死亡的儿童约为320万。在所有的脓毒症当中,约有30%是由革兰阴性菌引起的。
由革兰阳性菌和革兰阴性菌感染引起的脓毒症在所有国家中都是一个严重的问题。美国每年发生约40000例,其中死亡率约为50%。
近年来,脓毒症和脓毒性休克已成为许多出版物的主题。对脓毒性休克的病理生理学研究已发现,内毒血症和其它细菌性中毒间介器起了重要的作用。它们包括肿瘤坏死因子TNF、白细胞介素-1、干扰素血小板活性因子和eicosanoids(arac hidomic酸的衍生物),其中最重要的是TNF,它能作用于代谢以及免疫和噬细胞系统(见Berkowitz,F.E.:Bacterial Toxins in phatogenesis of infections;Current opinion in Infectious Diseases,1991 4:332-337)。文献表明,脓毒性休克的死者体内的TNF和白细胞介素-1的浓度很高。许多论文的作者都报道了在脓毒性休克患者的血浆和血液中的TNF-α含量有所升高,此外,TNF-α的毒性作用依赖于TNF浓度的程度不如依赖于其在体内的持久性的程度大。
许多作者都研究了用脓毒症和脓毒性休克中的细胞毒素级联放大进行高节的可能性。所报道的可行性作法涉及使用单克隆抗TNF抗体和用抗脂多糖类对脂多糖类进行中和。不过,抗体并没有提高细菌的清除率。当使用试剂(如通过巨噬细胞而阻滞TNF产生的地塞米松和pentoxifylline)时,观察到了局部的正向作用。
此外还已知,其它细胞毒素也是对脓毒性休克有贡献的因素。在这种情形中,调节脓毒性休克中细胞毒素级联的治疗方法有希望用遏制感染的方式进行干扰,这是因为宿主的防御依赖于这些同样炎性的细胞毒素。由于对大量的患者有潜在的益处,找出预防脓毒性休克的手段是当务之最急。而控制TNF的水平似乎又是为达到这些目的所必需。
同样关键的是TNF在另一个防御机制“发热”中的作用,发热是通常所有高级动物和人类对感染的生理反应。目前已确定有5个致热细胞毒素(白细胞介素-1、TNF、干扰素、白细胞介素-2和白细胞介素-6)是发热反应主要的内生间介器,它们可抑制通常促进热量流失和抑制在人体中产生热量的对热敏感的视前叶神经。发热及其间介物能同时损害入侵的生物和宿主。近年来所积累的大量数据使人想到白细胞介素-1、TNF和白细胞介素-6间介了感染的病理生理学异常过程。由于内生热原对各类感染的病理过程有作用,间介物和发热反应二者都对宿主有潜在的损害。在这方面最为有力的证据来自对革兰阴性脓毒症的研究。还有证据表明,内生热原间介了革兰阳性菌所致的脓毒症、AIDS、螺旋体感染脑膜炎、成人呼吸痛苦综合症、suppurative化脓和分支杆菌病等病症的系统的和局部的表现。虽然所引用的数据与所观察到的发热反应本身提高了实验动物感染的抵抗力,不过,种的保存、更多个体的存活才是进化过程的实质。令人信服的是,可以利用热原细胞毒素对压倒性的感染(如革兰阴性脓毒症)有害的系统性的作用作为暴发性较弱的感染中有益的局部发热效应。采用这种方式,种的总体可以免受流行病的伤害(见Mackowiak,P.A.:Mechanism of Fever;Current Opinions in Infectious Diseases,1992,5:348-354)。
有关发热的病原体的一个基本概念是:外原性热原(不论其来源或结构)通过诱导宿主细胞(主要是巨噬细胞)产生内生热原而发热。相应于此,基于采用抗内生热原抗体和内生热原受体对抗剂的治疗方法可能有效。一种可能性是阻滞TNF的生物合成。动物研究表明,TNF可以在对感染的反应级联中的IL-1和其它细胞毒素之前而产生。按照许多科学家的观点,抑制TNF的生物合成还意味着阻滞某些暴发性和无希望的感染的有害的作用。
TNF更加有名的作用在于对AIDS本身。AIDS的特征在于意义深奥的免疫缺损。AIDS的发病特征是CD4+淋巴细胞数量持续减少。即使在AIDS患者的外周血液单核细胞(PBMC)中,受HIV(AIDS的病因试剂)感染的细胞数也较小(100-1000中≤1)。在CD4+淋巴细胞被优先感染的同时,这些细胞并不是HIV感染的排除性靶。最近的证据表明,HIV靶细胞的范围很宽。现已发现,在单核细胞/巨噬细胞对T淋巴细胞中出现HIV感染的情况有很大不同。T淋巴细胞易被摧毁,而单核细胞/巨噬细胞则允许持续地受感染。于是HIV可以象体内的其它细胞一样作为贮主而靠在单核细胞/巨噬细胞附近:这类单核细胞/巨噬细胞对HIV感染的反应可以解释所发现的其在宿主中的潜伏性;这种反应还可通过受感染的细胞所产生的可溶性因子所致的病原体后遗症(见Toshifumi Matsuyama等人:Cytokines and Hivinfection:Is AIDS a Tumor Necrosis Factor disease?;AIDS 1991,5:1405-1417)。据许多科学家报道,受HTLV-1感染的人类T细胞线极容易受HIV感染,表现出与HIV复制水平提高有关的惊人的细胞病理作用。此外,受HIV感染的细胞易于被这些细胞的上层清液所损伤。在用此上层清液处理后所进行的病毒滴定测试表明,T细胞(MT-2)产生的因子促进了HIV的复制。经鉴定,该因子为TNF-β,并且这个发现与T细胞(MT-2)产生TNF-β的报道相一致。当使用TNF-α时,观察到了同样的效应。TNF-α和TNF-β选择性地杀死受HIV感染的细胞,并且提高了HIV的复制水平。此外据报道,受HIV感染的T细胞线和刚从HIV感染的个体中分离出的新鲜的PBMC对TNF有反应,含量有所提高。这使人想到HIV在活体中的表达也可能同样地提高。实际上TNF活性的提高可以被抗TNF抗体中和。用TNF-α和TNF-β处理后HIV复制的提高高达10±1倍(见Yakarnam,A.等人:Tumor necrosis factors(α,β) induced by HIV-1 in peripheral blood monocellular cells potentate virus replication;AIDS 1990,421-427)。
另外,现已证实,有很多种细胞毒素可以影响HIV的产生。通过采用从正常的外周血液中提纯的单核噬细胞,当暴露在HIV病毒儿小时内都观察到了IL-6和TNF-α诱导。基于许多观察和Toshifumi Matsuyama等人(op.cit.)的观点,可以肯定,AIDS代表了一种细胞毒素或TNF疾病。在AIDS的细胞毒素网络中,TNF-α和TNF-β似乎是提高HIV的复制和诱导它们本身及其它细胞毒素表达的一个关键分子。TNF-α已被证明可刺激各种类型的细胞中释放出其它细胞毒素,因此它是第一防御机制中细胞毒素级联中的一个关键数。
现有人提出,与AIDS有关的许多症状可以用具有不同的生物功能的细胞毒素的释放来解释。用IL-1和TNF-α这两个有名的热原的诱导产生可以解释AIDS病人中所见的高热。TNF-α和-β的作用都是单细胞间介的细胞毒性中的免疫调节器和效应器分子。此外,TNF激活了免疫反应,并可直接杀灭被HIV感染的细胞,并因此提高了HIV的复制。还有人提出了免疫机制来解释CD4-T细胞在AIDS病人中的排除(Matsuyama等人,op.cit.)。还有人报道,与AIDS有关的卡波济氏肉瘤也是由TNF-α诱导出的:TNF-α可以通过生理学刺激(如紫外光)而从角质细胞中产生,它有助于在皮肤中诱导出IL-6,并促进AIDS中卡波济氏肉瘤的生长。在体外试验中,TNF-α可以损害髓磷脂和少突神经胶质细胞,并且一些神经胶质瘤诱导的细胞线被证明易受TNF-α的抗增生作用,因此可以得出结论,AIDS病人中枢神经系统的机能变常是TNF-α介入的结果。有几份报告表明,TNF-α和IL-1的血清水平随AIDS和ARC(与AIDS有关的综合症)病情的发展而显著提高,而在HIV的无症状载体的血清试验中,它们回落到健康的对照值范围内。根据Mat suyama等人(op.cit.)的观点,AIDS作为TNF疾病程度与HIV疾病的程度相同。现已证明,提高对TNF诱导的控制水平有助于有效地治疗AIDS病人。
下列基本的发现使我们可以解决上述问题,并为溶菌酵素二聚体在上述病理状况中找到新的治疗用途提供新的思路:
1.溶菌酵素二聚体抑制TNF的合成,
2.溶菌酵素二聚体刺激TNF-α的合成,
3.溶菌酵素二聚体提高了噬细胞的活性。
于是,本发明的一个目的是提供多种用于治疗与上述的TNF(肿瘤坏死因子)水平过高有关的疾病的新的药物配方。
本发明的另一个目的是提供多种用于与上述TNF水平过量有关的profilaxis疾病的新的药物配方。
本发明的又一个目的是提供用于治疗和预防与TNF水平升高和过高有关的疾病的配方和卫生用品。
根据本发明,上述目的可以通过下列二聚体型溶菌酵素新用途和含有溶菌酵素二聚体活性成份的新的药物制剂来实现:
用二聚体型溶菌酵素来制造一种用于抑制动物体和人体中肿瘤坏死因子的生物合成的药物。
用二聚体型溶菌酵素来制造一种用于治疗与肿瘤坏死因子水平过高有关的疾病的制剂。
用二聚体型溶菌酵素来制造一种用于预防与肿瘤坏死因子水平过高有关的疾病的制剂。
用二聚体型溶菌酵素来制造用于控制无症状载体和与AIDS有关的综合症患者中肿瘤坏死因子受HIV诱导而释放的药物。
用二聚体型溶菌酵素来制造用于治疗AIDS的药物组合物。
用二聚体型溶菌酵素来制造用于预防和治疗脓毒症和脓毒性休克的药物制剂。
用二聚体型溶菌酵素来制造用于预防和治疗恶病质的药物制剂。
用二聚体型溶菌酵素来制造用于预防和治疗发热的药物制剂。
注射二聚体型溶菌酵素含量在0.01-10mg/ml(最好在0.1-1.0mg/ml)的一种不致热的无菌制剂,所述的制剂含有生理相容的溶剂和已被批准使用于药物的保存剂。
注射上述药物,方式是:静泳注射一次或多次,剂量为0.02mg/kg体重。
使用浸渍了有效剂量的二聚体型溶菌酵素的塞和绷带来预防脓毒症和脓毒性休克。
在月经期使用浸渍了有效剂量的二聚体型溶菌酵素的阴道塞。
对二聚体型溶菌酵素,最好使用分离纯化的溶菌酵素二聚体。对某些用途而言,也可能采用的是除溶菌酵素二聚体以外还含有少量的溶菌酵素的三聚体或更高的低聚体的酶。
根据本发明,也可以进行肌肉内和皮下注射。在某些用途中,适宜的投药方式也可能是同时(或自主性地)在子宫内和乳房内给予同样的液体制剂或局部给予其它局部制剂。
包括生理接受的溶剂和批准使用于药物的保存剂的优选的不致热无菌制剂由作为一种溶剂的不致热无菌水或PBS水溶液和作为批准用于蛋白质药物制剂的保存剂的硫柳汞组成。
二聚体型溶菌酵素是经下面的工艺获得的:受控聚合酶单体,然后从反应后混合物中仔细提纯,特别要分离开具有所报道的毒副作用的溶菌酵素的单体以及三聚体或更高的低聚体部分。可以采用任何已知的、可用于 获得二聚体型酶的方法。国际专利申请PCT/US90/PP00140中公开了包括提纯步骤在内的复合生产工艺。
以前的溶菌酵素二聚体的临床前试验表明,没有出现诱变电位,没有畸形形成效应,没有耐受性效应或该效应很弱小,口服或皮下投药时的一次剂量毒性LD50无法测量(>2000mg/kg),静脉内注射时LD50>1000mg/kg。
这里所公开的溶菌酵素二聚体的新用途经活体外试验证明有坚实的基础,用于活体内临床场合也有效果。另外还进行了一些比较研究。
据信溶菌酵素二聚体对TNF释放的抑制作用如此之强是因为它的活性范围(即其诱导IFN释放)很宽并且它增强了对吞噬作用的作用的缘帮。上面刚刚提到的两个性质是天然防御机制中的重要因素。相应于此,溶菌酵素二聚体的上述新用途的治疗和预防作用因其同时增强了自然防御机制而又反过来得到自然防御机制的支持。
在下面的例子中对本发明进行进一步的说明,其中在附图中用参考例图示了特别关心的内容之外的一些试验结果。图1示出了在不同的稀释物中存在溶菌酵素二聚体的情况下用ConA最适度下地刺激的淋巴细胞培养物TNF的释放在活体外受到抑制的情况。图2示出了实验测定的TNF在颅盖血液中的IFN水平,并且图3示出了在一个根据本发明的药物的疗效的比较试验中,实验测出的颅盖血液中的IFN水平。
例1
为确定溶菌酵素二聚体的免疫活性,采用了用FACS分析人类外周血液淋巴细胞的测定法。
在人类外周淋巴细胞中进行致有丝分裂的刺激是一种试验免疫系统中大多数重要细胞的反应性的较完善的方法。为试验治疗物质对从健康血液捐献者采集的淋巴细胞的活性和增生的影响,以最适度下的剂量加入致有丝分裂物,最后定量测量淋巴细胞反应的免疫相关参数。将结果与不加入药物的对照值进行比较。
为刺激淋巴细胞,使用了浓度为每毫升介质20μg/ml的ConA。初始细胞浓度为106细胞/ml。在1天(淋巴细胞和HLA-Dr上的IL-2受体试验)或2天(所有其它的试验)中在一个CO2保温器中进行短期培养。作为细胞活性的判据,测量下列参数:
-Neopterin(免疫活性的标志)
-β-2小球蛋白(同样是一种活性标志)
-白细胞介素-2(从autocrine和paracrine物质中衍生的T辅助淋巴细胞)
-白细胞介素-6(细胞分化激素)
-肿瘤坏死因子-TNF(作用于血管的多效力白细胞介素)
-干扰素-α(特别作用于B淋巴细胞的分化因子)
-Tymidine Kinase(酶,在增生细胞中被向上调节)
-淋巴细胞白细胞介素-2感受器(用于autocrine和paracrine受体分子)
-淋巴细胞Ki-67(在被激活的细胞和增生细胞中表达的抗原)
-淋巴细胞HLA-Dr(在免疫反应中被向上调节的第二类组织适合性抗原)。
在培养上层清液的过程中,观察到了下列有关细胞产物的现象:在免疫反应过程中和刺激培养中被淋巴细胞产生的neopterin在本实验中没有被溶菌酵素二聚体显著提高到从不加试验的二聚体的ConA刺激细胞中所得到的对照值的水平之上。随着所试验的二聚体浓度的加大,neopterin值也略微增高。与此相似,在溶菌酵素二聚体所有的浓度情形中,β-2小球蛋白值在对照值上下波动。在培养过程中从淋巴细胞表面散布出并给出了整个IL-2受体更新情况的IL-2受体与淋巴细胞表面上的IL-2受体(下面列出)处于一个相当的水平,表明在所试验的二聚体浓度上的最高处出现了明显的抑制现象。白细胞介素-6清楚地表明,在较高的浓度下,依赖于剂量的值也较高。此分子在hematopoiesis细胞分化和免疫反应中起着重要的作用。在试验中,前三个值不得不外推,这是因为最高的标准仅为2000pg/ml。与剩下的分子有关的结果示于下面的表1中。与白细胞介素-6不同,TNF-α在培养的上层清液中的浓度出人意料地急剧降低,只有最后的两个稀释步骤除外,在这两个步骤中,它对TNF浓度没有抑制作用。在对细胞小丸进行冷冻和解冷后,测量淋巴细胞胞质中的thymidine kinaze。在分裂细胞中,thymidine kinase的数据上升,使之成为细胞增生的一个良好的标志。从表1的数据中可见,在所试验的溶菌酵索二聚体的最高浓度处数据出现下凹,而在其它稀释步骤中没有看见这种趋势。反过来,干扰素-α在同样的条件下的数值高于ConA单独处于较高浓度时的对照值。从第二值到第三次稀释之间可以看到数值在增加。关于淋巴细胞标专,所观察到的情况如下:在免疫反应其间,作为组织适合性标志的HLA-Dr/CD3表达在活化的T淋巴细胞表面。获得的结果表明,在对照培养物中有一定百分比分比的活化的T细胞,并且在带有不同浓度的溶菌酵素二聚体的培养液中,数值在此对照值上下波动。IL-2接受在淋巴细胞表面。众所周知,白细胞介素-2是通过IL-1活化后由T辅助淋巴细胞所产生的细胞激素。T辅助淋巴细胞不但产生IL-2,而且还受此分子刺激而增生。T辅助淋巴细胞表面的IL-2受体在激活时会向上调节。从表1中可见,在溶菌酵素二聚体的浓度最高处淋巴细胞上的IL-2受体受到显著的遏制,而其余的值在生物带宽范围内并没有显著的差别。表1中还含有一个与Ki-67/CD8、Ki-67/CD4有关的数据。Ki-67是一种出现在正在进行有丝分裂的细胞中的增生细胞。Ki-667是评价受刺激的细胞和进行肿瘤诊断的一个重要参数。在所报告的结果中,细胞增生在带有最高的两个剂量的溶菌酵素二聚体的Ki-67+抑制因子(CD8)细胞中受到轻微的抑制。在带有最大剂量的辅助(CD4)淋巴细胞中,阳性细胞的百分比显著增加。剂量较低时,表达Ki-67/CD4的细胞的百分比略高于对照值。

用以测量被测物对最适度下的ConA刺激的人类外周淋巴细胞的影响为基础的方法分析上面所列出并且根据其在免疫反应中的电位重要性所选择的免疫参数,该方法如此完备和灵敏,以致可以用来评价许多不同的参数。在某些溶菌酵素二聚体浓度处,试验结果与只用ConA刺激的淋巴细胞值相比有很大的区别,对于如TNF和IFN-α而言,所观察到的作用的趋势明显地处于所有试验的稀释物的范围之中。
例2
进行实验室试验以确定溶菌酵素二聚体对体外的乳和血液细胞的噬细胞活性影响。
以前就已确定,在标准的试验中,体外的溶菌酵素二聚体不会抑制从感染的母牛乳腺中分离出的微生物的增生。由于临床上使用乳腺内并且同时静脉内给予溶菌酵素二聚体的方法有效地消除了母牛乳腺的感染,显然在母牛乳腺中的主要的抗菌机制是吞噬作用。为进行比较,采用同样浓度的试验物质以及同样的培养时间,并采用从健康的以及受感染的母牛(或者甚至采用从乳腺受感染的部分以及同一母牛的一个健康部分)中分离出来的细胞进行实验,以消除个体反应差异。
在所进行的试验中,将试验的物质(提纯为二聚体型的溶菌酵素并且还有二聚体和少量的三聚体和更高的低聚物型溶菌酵素)以25-0.25μg/ml的浓度加入到健康的和受感染的母牛的血液或乳中,并且将混合物在37DU下培养0.5-24小时。确定每个样品的噬细胞的百分比、吞噬作用指数(按照Wisniewski等人的方法)以及NBT阳性粒细胞的百分比(按照Park的方法)。
在体外试验中,溶菌酵素二聚体提高了淋巴细胞的噬细胞活性。此作用的强弱依赖于剂量和孵化时间。需要更高浓度的溶菌酵素二聚体来活化乳淋巴细胞,然后才是血液淋巴细胞。过高浓度的二聚体会略微降低噬细胞的体外活性。为示意起见,在下面的表2和表3中有选择地给出了一些结果(主要是为了表明:只要后聚合反应混合物不含有细胞互性的单体型的二聚体,从高度提纯的及纯度不高的二聚体型溶菌酵素得出的结果可与之相比)。

在上述的表2和表3中,用“溶菌酵素二聚体+”来表示含有如在说明书的前面所提到的少量的溶菌酵素的三聚体及更高的低聚体。
很显然,溶菌酵素二聚体的提纯程度对所观察到的乳淋巴细胞的噬细胞活性没有显然的影响。此外还可看出,效应器细胞可以是粒细胞。
在后面的进一步的例子中报告了体内研究的结果。为进行临床试验,使用了10ml的PBS溶液中含有2mg溶菌酵素二聚体的制剂。这种制剂称为KLP-602。
例3
当静脉注射时,KLP-602表现出刺激健康和有病的小牛以及健康的驹和刚进行乳房内投药的母牛中血液粒细胞的噬细胞活性。这个作用是通过中性白细胞数增加和吸收葡萄球菌的能力增强以及降低了NBT而表现出来的。这种现象主要发生在注射制剂后的前12-24小时中。KLP-602对乳房中噬细胞活性的作用依赖于药物的剂量和剂型以及动物个体的反应。
用溶菌酵素二聚体来治疗母牛、猪、马和狗的感染性疾病。在不同的剂量的投药中,在不同的时间间隔处,进行静脉内、肌肉内、皮下、乳房内和子宫内投药。对346只母牛、274只小牛、110只母猪和食用猪、294只小猪、709只乳猪、35个驹及107只狗进行投药。作用对照,进行了另外的处理。由于试验动物所处状态的原因,未进行针对死亡原因的治疗处理,这些动物的任何一个都不留下。不过,与兽医文献中所知的治疗疾病的临床图象进行比较可以得出结论。
例4
在所进行的对小牛的试验中,测定一组健康的动物和受感染、然后进行治疗(先进行12小时的感染,再进行抗菌治疗,并且用高度
化的剂型溶菌酵素二聚体治愈)的动物的INF和TNF在血液中的水平。在图2和3中图示了所获得的结果,结果分别表明了TNF和IFN的水平。这两个图代表从4只经个体鉴别的母牛的血样中获得的真实结果。在这两个图上,线1表示从健康的母牛血液中获得的结果,线2表示从感染过但未经治疗的母牛处获得的结果(在12小时时IFN水平为45U/ml在图上未示出,但一条线指示了线2方向的值);对动物进行方式已知的治疗,在12小时处,线3表示用抗菌素治疗的母牛的结果并且线4表示从用KLP-602治疗的母牛获得的结果。
在用溶菌酵素二聚体治疗的组中,一次或两次KLP-602注射治愈了90%以上的患胃肠炎的母牛和85%以上的患支气管肺炎的母牛。发热和腹泻之类的症状突然消失显得非常突出。用这种药物进行治疗不需要对动物补充流体。与用抗菌素治疗的对照组相比,恢复时间和恢复率的结果更好。
例5
KLP-602对猪的疾病的疗效最好。100%或接近100%的动物从下列疾病中恢复过来:产后无乳(MMA综合症)、痢疾、脓性子宫炎、流感和大肠杆菌病。此药物用于水肿病和支气管肺炎的结果比上面的结果略差,但仍显著好于用其它方法治疗所获得的结果。需要强调的是,腹泻(通常在前24小时内)和发热迅速退缩并且重新出现乳汁分泌,这对患产后乳房和子宫发炎的病例特别重要(解救了猪的生命)。下面的表4中示出了与其它治疗制剂相比KLP-602的疗效:

当用KLP-602治疗时,100%患肠炎的驹和83.3%患支气管肺炎的驹恢复得比用已知的制剂治疗的对照组快。
例7
测试KLP-602对治疗某些狗的疾病的治疗效果,如毛囊炎(100%有效)、上呼吸道和下呼吸道感染和通过腹泻表现出的胃肠道感染。在此组中流行的是微小病毒症(parvovirosis),这是一种目前很难治的疾病。不过,在微小病毒症病例中,约75%得到恢复。
在对自然发病的动物所进行的试验中,如上所述,得到一些重要的发现:
1.证明此疗法非常容易施行,并且对多因子病因和发病机理的疾病有疗效(这些疾病在动物种群中非常流行,并且在饲养农场中很难治疗,因为这些流行疾病在那里扩散得非常快)。这些发现证明溶菌酵素二聚体对自然防御机制有调节作用。
2.证明此疗法对处于自然病程的疾病有效。内毒素休克或其它失调症(如高热和长期持续的高热)的出现会在恢复后的长时间内影响动物的状态,并且因此对极度损害这些动物,例如作为种畜或竞赛而饲养的马(驹),以及主要供应食品工业所饲养的动物,在这种情况下生产成本是关键因素。快速恢复将显著降低治疗这些疾病的成本。
体内试验所发现的结果证明,用溶菌酵素二聚体治疗各种自然发生的感染可以降低TNF的水平,因此支持了本发明的权利要求。

1、用二聚体型溶菌酵素来制造一种用于抑制动物体和人体中肿瘤坏死因子的生物合成的药物。

9、注射二聚体型溶菌酵素含量在0.01-10mg/ml(最好在0.1-1.0mg/ml)的一种不致热的无菌制剂,所述的制剂含有生理相容的溶剂和已被批准使用于药物的保存剂。
10、注射上述药物,方式是:静泳注射一次或多次,剂量为0.02mg/kg体重。
11、使用浸渍了有效剂量的二聚体型溶菌酵素的塞和抗脓毒衣和的来预防脓毒症和脓毒性休克。
12、在月经期使用浸渍了有效剂量的二聚体型溶菌酵素的阴道塞。
