技术领域
本发明总体上主要涉及一种用于在子宫的子宫镜检查中使用的医疗装置。更具体地,一些实施方式涉及一种具有一体式可视化和子宫内膜取样部件的医疗装置。
背景技术
基于办公室的子宫内膜活组织检查是妇科医生所使用的标准诊断程序。尽管在癌症的检查上很有效,子宫内膜活组织检查常常不检查子宫内膜息肉、粘膜下肌瘤、以及其他子宫内膜病状。内窥镜或者子宫内部(文中被称作“子宫腔”和/或“子宫内膜腔”)的直接影像已经被示出以大幅地提高诊断的准确性。但是,由于所需设备和供应的复杂和昂贵,很少妇科医生做办公室子宫镜检查。尽管通过具有操作通道的一些子宫镜可以进行微量活组织检查,但是,外科医生通常需要除去子宫镜,然后用不同的仪器进行子宫内膜活组织检查。多个仪器在患者子宫腔中重复的插入和移出可能使患者感到不适和/或可能与在没有不同仪器重复的插入和移出的情况下执行两个程序相比延长了完成子宫镜检查和子宫内膜取样程序所需的时间。并且,用于相同检查/活组织检查程序的多个仪器的这种使用需要购买,储存这些仪器并且给这些仪器消毒的花费和不便。
本文中要求保护的主题不限于解决任何缺点或者仅在例如上文描述的环境中操作的实施方式。相反地,此背景仅设置成示出文中描述的一些实施方式可以实践的一个示例性技术领域。
发明内容
根据一些实施方式,描述了用于检查子宫组织的一体式的内窥镜器械。所述器械包括:细长构件,所述细长构件具有近端、远端,并且定尺寸从而有助于远端插入通过患者的子宫颈并且进到子宫中;光传输系统,所述光传输系统适合于照亮正在进行检查的子宫组织;侧面取样开口,所述侧面取样开口位于细长构件中并且定尺寸从而有助于子宫内膜组织的收集;电子成像模块,所述电子成像模块位于细长构件的远端上;以及面向远端流体开口,所述面向远端流体开口位于细长构件的远端上从而通过允许流体在靠近透镜的远端方向上流动由此减小靠近成像模块的碎片来改进使用电子成像模块的视觉检查。根据一些实施方式,细长构件包括用于侧面开口和面向远端开口的分开的流体路径和/或内部特征从而增强从细长构件通过面向远端流体开口的流体流动。根据一些实施方式,电子成像模块包括固态COMS传感器,以及与传感器基本上共面的一体式的视频处理电路,从而输出标准视频信号。根据一些实施方式,器械包括把手和一体式的电子显示监视器。
根据一些实施方式,描述了用于检查子宫组织的一体式的内窥镜器械,其包括:细长构件,细长构件具有近端、远端,并且定尺寸从而有助于远端插入通过患者的子宫颈并且进到子宫中;基于LED的光传输系统位于远端附近并且适合于从分开大于1mm的至少两点发射光,由此照亮正在进行检查的子宫组织;侧面取样开口,侧面取样开口位于细长构件中,并且定尺寸成有助于子宫内膜组织的收集;电子成像模块,电子成像模块定位于细长构件的远端上;以及流体开口,流体开口定位于细长构件的远端上以便通过允许流体在靠近透镜的远端方向上流动由此减少靠近成像模块的碎片以改进使用电子成像模块的视觉检查。根据一些实施方式,电子成像模块包括居中定位的孔,光通过居中定位的孔进入成像模块,并且光传输系统包括位于远端上、在孔的彼此相反的两侧上的两个LED。根据一些实施方式,光传输系统包括围绕孔定位的环形LED模块。

根据一些实施方式,描述了一种用于与用户相互作用的方法,方法包括:在触摸感应电子显示监视器上向用户显示多个屏,监视器与内窥镜器械形成一体。根据一些实施方式,接收在触摸感应显示器上的用户输入,所述用户输入指示由使用者选择的用户将观察的存储的捕获的图像文件(例如静态或者视频图像)。响应于所接收的用户选择,将来自所选择的存储的图像文件的内容显示在触摸感应显示器上。

具体实施方式
下文中提供了优选实施方式的示例的详细描述。尽管已经描述了一些实施方式,但是应当理解,此专利说明书中描述的新主题不限制于文中描述的任一实施方式或者实施方式的组合,而是包含多个变形、改型、以及等同物。另外,尽管在下列描述中阐明了多个具体细节以便提供彻底理解效果,但是,一些实施方式能够在没有这些细节中的一些或者全部的情况下得以实现。另外,处于清楚的目的,相关领域中已知的某些技术材料未作具体描述以避免不必要地误解文中描述的新主题。应当清楚,文中描述的具体实施方式中的一个或者多个的单个特征能够用于与特征或者其他所描述的实施方式进行组合。另外,不同的附图中的相似的附图标记和名称表示相似的元件。
图1为根据一些实施方式的用于组合式子宫镜检查和子宫内膜活组织检查的器械的左侧视图。图1中示出的子宫镜100的实施方式的许多元件与共同转让的结合的申请中所描述的实施方式中所讨论的子宫镜的元件相同或者相似,并且可以不对这些元件进行描述或者仅对这些元件进行简要地描述。还应当理解:在共同转让的结合的申请中所描述的实施方式的方面也可以应用于文中所描述的实施方式。
器械100特别有助于使医师执行有效的组合式子宫镜检查和子宫内膜活组织检查,但是应当理解,子宫镜100的其他用途在本教导的范围内。子宫镜100能够在保持设备成本低并且保持执行程序所需时间适度的方面产生很高的效率,但是同时提供比传统“盲”子宫内膜样品采集方法更好的子宫内膜样品质量的可能性。子宫镜100包括套管102、流体枢纽104、滑动连接器106、把手主体108、显示器支架112以及显示器110。套管102由远端尖端120和轴122构成。流体枢纽包括一个或者更多个流体口114,一个或者更多个流体口114用于将流体传输到器械中并且由此进到子宫中和/或用于实施抽吸以从子宫抽出流体和组织样品。如图所示,轴122在其远端附近变弯,所述变弯例如如图所示具有25度弯曲。根据一些实施方式,已经发现在远端附近的在15至35度之间的弯曲适合于许多应用。远端尖端120包括摄影机组件、照明元件以及用于流入(即,流出器械100并且进到患者中)和流出(即,进到器械100中并且离开患者)的流体口。在远端尖端120的上侧上的采样口还包括切割部,切割部有助于组织样品收集,在下文中将更详细地进行描述。根据一些实施方式,尖端120包括由丙烯酸制成的壳体主体。根据一些实施方式,轴122由尼龙制成。根据一些实施方式,显示器110是触屏显示器,并且能够向上和向下倾斜例如约45度。根据一些实施方式,在如文中其他图一样的图1中,示出了已发现适合于多个应用的多个尺寸,但是,本领域中的普通技术人员可以在不脱离本专利说明书的教导的情况下改变这些尺寸。根据一些实施方式,(包括结合到远端尖端120中的摄像机组件、LED灯以及流体口的)套管102、流体枢纽104以及滑动连接器106设计成单次使用。根据这些实施方式,套管102、流体枢纽104和连接器106在预消毒包中传输至医生并且用于可在单次使用之后丢弃,同时把手108和显示器110设计成多次重复使用。
图2为根据一些实施方式的用于组合式子宫镜检查和子宫内膜活组织检查的器械的俯视平面图。在此图中,在把手主体108上示出了三个控制按钮。具体地,开/关按钮210用于触发器械100打开或者关闭。根据一些实施方式,电源开/关按钮210通过使用两种不同颜色的LED产生背光以向用户指示可充电电池220的状态。例如绿色背光能够用于指示电池水平是很好的并且红色背光能够用于指示电池220是低电量的(例如剩余容量小于30%,例如使用下文图42中示出的图标4218)。根据一些实施方式,电池220的容量约2500mAh。根据一些实施方式,按钮210的LED灯还能够用于在电池220从外部电源的再充电过程中,例如当对接至例如下文图36-37中示出的基站时,或者当通过使用如下文图3-4中示出的口312连接至USB电源时指示电池充电状态。在此情形中,背光LED示出红色同时向电池充电,并且当电池220完全充满时示出绿色。根据一些实施方式,开/关按钮210双击作为“主页”按钮,使得按钮210的较短的按压,例如1秒或者更少时间的按压,产生在显示器110上的如下文图41中所示的主屏菜单,然而,较长的按压将关闭所述单元。
LED亮度控制按钮212用于控制在远端尖端120上的LED的亮度。根据一些实施方式,已经发现总共四个不同的LED照明等级是合适的并且单个按钮212通过循环通过等级,每次按钮按压改变照明等级来控制等级。快照/视频按钮214用于从尖端120中的摄像机捕获静态图像和/或视频。根据一些实施方式,按压快照/视频按钮214持续三秒或者更短时间来捕获单个静态图片,而按压按钮214持续长于三秒开始视频记录。当视频正在记录时,按钮214的单次按压停止视频捕获。参照下文图41-46,对包括按钮210、212和214以及交互式触屏显示器110的用户界面的其他细节进行描述。根据一些实施方式,声音确认信号与按钮210、212以及214的按压相关联。例如,除了当按压快照/视频按钮214或者OK软件按钮被按压时两声嘟之外,当按压(包括诸如下文图41和43-46中示出的软件按钮的)任一按钮时,发出单个的“嘟”的声音。
图3为根据一些实施方式的用于组合式子宫镜检查和子宫内膜活组织检查的器械的右侧视图。橡胶盖310在显示器110的一侧上,橡胶盖310覆盖微型USB口312和SD卡槽314。盖310形成围绕开口的边缘的流体密封。在盖310之下,微型USB口312用作多个目的,这些目的包括到外部显示器的视频输出、到用于向可充电电池220充电的AC适配器的连接器、和/或作为到用于下载和上传图像、视频和/或设定的主机、以及用于向可充电电池220充电的接口。SD卡槽314用于接纳用于存储图像、视频和/或用于器械100的设定的闪存卡。根据一些实施方式,提供标准尺寸大容量(SDHC或者SDXC)槽,但是能够使用更小的外形尺寸,例如微型SD卡或者小型SD卡,或者其他类型的存储载体。
图4为根据一些实施方式的用于组合式子宫镜检查和子宫内膜活组织检查的器械的远端视图。能够看到尖端120和轴122、以及流体枢纽104、流体口114以及把手主体108。SD卡/USB接口盖310也在显示器主体的一侧上示出。另外如图所示示出根据一些实施方式的照片/视频处理电路410,所述照片/视频处理电路410能够用于增强或者另外处理从在尖端120中的摄像机模块接收的标准视频信号和/或图像。
图5为根据一些实施方式的用于组合式子宫镜检查和子宫内膜活组织检查的器械的近端视图。触摸感应屏110在尺寸上(在对角上)优选地为3.5英寸。
图6为根据一些实施方式的用于组合式子宫镜检查和子宫内膜活组织检查的器械的远端尖端的立体图。根据一些实施方式,远端尖端120包括由丙烯酸制成的尖端壳体600。采样口610在尖端120的尖端侧上,采样口610用于将流体从患者子宫吸出并且收集组织。采样口610包括切割边缘612,切割边缘612是锋利的并且定位成有助于通过刮擦实现的子宫内膜样品的收集。摄像机组件640在尖端120的远端上。两个LED630和632定位于摄像机上方和下方以均匀地照亮子宫的组织用以视觉检查。遮光罩642作为透镜遮光罩并且遮光罩642保护来自LED630和632的光防止进入摄像机640的孔。
在执行子宫内膜组织的视觉检查并且特别是在由自由组织、松散附接的组织和/或流体组成的子宫内膜介质相对较粘稠的情形中的一个问题在于,从靠近透镜漂浮的组织颗粒反射的光会过亮地显示,并且因此削弱其他组织表面的成像。根据一些实施方式,两个朝前的流体口620和622设置成允许流体离开尖端并且趋于将悬浮的颗粒物质从摄像机推开从而增强由摄像机640捕获的图像和视频。在一些情形中,一些组织碎片可以聚集在远端表面上,使得成像在朝前口在清除这些所聚集的组织是有用的情形中被削弱。另外,已经发现,朝前口有助于在多种情形中帮助套管的插入,因为在插入过程中,流体提供润滑以及仅在远端尖端前面的组织的部分扩大。由于朝前口提高了可视化,意外损伤子宫的风险大幅降低。
图7为根据一些实施方式的用于组合式子宫镜检查和子宫内膜活组织检查的器械的远端尖端的左侧视图。尖端120的丙烯酸的主体600优选地包括一个或者更多个隆起部710以有助于将尖端120牢固地固定至轴122(未示出)。
图8A为根据一些实施方式的用于组合式子宫镜检查和子宫内膜活组织检查的器械的远端尖端组件的右侧视图。在这些实施方式中,前面流入(离开器械)流体口连接至分开的流体通道以增强在流进和流出器械100的流体上的控制。在此情形中,尖端120包括用于流体流入和流出的分开的流体通道。具体地,沿上右侧流动的分开的流体通道810连接至朝前的流体口620,并且未示出的另一流体通道连接至未示出的另一朝前的流体口622。中央流体通道820连接至侧取样口610。
图8B-8E为根据一些实施方式的用于组合式子宫镜检查和子宫内膜活组织检查的远端尖端组件的另一视图。图8B为图8A中示出的尖端120的近端视图。尖端主体或者壳体600包括两个流入(离开器械并且进到患者中)通道810和812和中央通道820,两个流入通道810和812流体连接至两个流入朝前口,中央通道820流体连接至取样口610。中央通道820还用于从摄像机组件向把手和显示器延伸视频和控制电缆。图8C为沿图8A中示出的线A-A’截取的远端的截面图。根据一些实施方式,注意到,尖端壳体600由外套筒830和芯832制成。图8D为沿图8A中示出的线B-B’截取的远端尖端的截面图,并且示出了在中央流体通道820与取样口610之间的连接。图8E为图8A中示出的尖端组件120的远端视图。根据一些实施方式,尖端120外部尺寸朝向远端稍较大。例如,尖端主体600在如图8B中所示的近端处测量为3.8mm×4.6mm,并且在如图8E中所示的远端处测量为4.2mm×4.8mm。

图10和图11示出了根据一些实施方式的用于组合式子宫镜检查和子宫内膜活组织检查的器械的具有分开的流体通道的轴的内部结构的细节。图10为轴122的远端的立体图。轴122的远端1030具有内部结构,内部结构被除去从而能够与尖端120的近端相匹配。轴122具有分开的通道1010和1012,所述通道1010和1012是对准的以便分别与在尖端120上的通道810和812流体匹配。上中央通道1020用于流出流体(即到器械中)以将流体从子宫移出和/或提供用于组织样品收集的负压。由此,通道1020定位成与中央通道820流体匹配。还包括单独的通道1022,单独的通道1022用于容置视频和照相机控制电缆,视频和照相机控制电缆还穿过尖端120的中央通道820。图11为轴122的截面。根据一些实施方式,轴122由挤出的尼龙制成。

传统内窥镜通常受限制并且不便于使用。它们需要熟练的工作人员来操作和维修。这使得特别难于在时间紧张的情况下,例如急诊室、手术室、以及多个器械和仪器同时使用的其他医疗设施的区域进行。根据一些实施方式,例如在图1-5中示出的器械100是手持的、紧凑的单次使用的内窥镜。在这些情形中,内窥镜100设置在无菌包中,因此适合于在无需对诊断或者治疗程序作任何准备的情况下立即使用。根据一些实施方式,单次使用器械100不需要复杂的连接器,使得整个内窥镜在无菌包中提供备用。
图31-33示出了根据不同实施方式的用于组合式子宫镜检查和子宫内膜活组织检查的单次使用的器械。在图31的情形中,器械100包括用于观察图像和/或视频的外部监视器3110。将图像和视频传输到外部监视器的无菌线3112附接至器械100并且与器械100包装在一起。
在图32的器械100的情形中,图像和视频由无线连接进行传输。把手100包括无线发射器3212并且外部监视器3210包括无线接收器。根据一些实施方式,使用无线宽带技术。根据一些实施方式,例如智能手机3220、平板电脑3222、移动计算机、或者具有无线和显示能力的其他移动设备的设备被用于观看图像和/或视频。
在图33的情形中,器械100包括内嵌的回转仪模块3310以提供恒定的方向参考。视频处理器用于记录在诸如设备3210、3110、3220以及3222之类的设备上的竖直显示的图像。

图47-48为示出根据一些实施方式的用于组合式子宫镜检查和子宫内膜活组织检查的器械的远端尖端的形状的细节的侧视图。已经发现,器械的远端尖端优选地出于多个原因应该倒圆。第一,尖端倒圆大幅地降低了意外损伤子宫,例如在使用中刺穿或者刺破脆弱的子宫组织的风险。第二,尖端倒圆影响远端尖端收集物质的阻力,这能够阻挡尖端和遮挡摄像机视线。已经发现,尖端的边缘应当优选地以至少0.25mm的半径倒圆。在示出为图47的尖端120的示例中,例如在区域4702中示出的远端尖端的边缘以0.5mm的半径倒圆。另外,已经发现,远端尖端的前面倒圆也是有利的。通过使前面4710突出,尖端几乎不可能聚集可能阻塞视场或者使其很难从摄像机模块获得清晰图像的其他物质或者组织碎片。在图47的示例中,前面4710优选地以约10mm的半径倒圆。在图48的示例中,远端尖端120优选地具有被以约2.5mm的半径倒圆的外区域4812围绕的基本上是平的中央部4810。边缘部4802以约0.44mm的半径倒圆。已经发现,使中央部4810基本上是平的(其在此示例中小于总的前面积的约30%)能够用于减小在由摄像机模块捕获的图像中的变形,同时较大的弯曲部4812和4802提供足够的圆角来避免组织收集并且减小组织损伤风险。根据一些实施方式,已经发现,占前部面积的至少50%的外部区域应当基本上倒圆。

Claims (74)

1.一种用于对子宫组织进行组合式子宫镜检查和子宫内膜活组织检查的一体式的内窥镜器械,所述内窥镜器械包括:
细长构件,所述细长构件具有近端、远端,并且所述细长构件定尺寸成以及构造成有助于所述远端插入通过患者的子宫颈并且进到子宫中;
光传输系统,所述光传输系统适合于照亮正在进行检查的所述子宫组织;
侧面取样开口,所述侧面取样开口位于所述细长构件中,并且定位成以及定尺寸成以及构造成有助于子宫内膜组织的收集;
电子成像模块,所述电子成像模块定位于所述细长构件的所述远端处;以及
第一面向远端流体开口,所述第一面向远端流体开口定位于所述细长构件的所述远端处,从而通过将流体传输成在远端方向上流动并由此迫使碎片远离所述成像模块来改进使用所述电子成像模块的子宫组织的视觉检查。
2.根据权利要求1所述的器械,其中,所述细长构件包括用于所述侧面开口和所述面向远端开口的分开的流体路径。
3.根据权利要求1所述的器械,还包括一个或者更多个内部构件,所述一个或者更多个内部构件定尺寸成以及定位成处于所述细长构件内,从而对从所述细长构件通过所述面向远端流体开口的流体流动进行增强。

6.根据权利要求5所述的器械,其中,所述电子成像模块包括一体式的视频处理电路,所述一体式的视频处理电路适配成以及编制有程序以输出标准视频信号,所述视频处理电路定位于与所述COMS传感器基本上相同的平面上。
7.根据权利要求1所述的器械,其中,所述侧面取样开口定位成距离所述细长构件的所述远端2mm与15mm之间。
8.根据权利要求1所述的器械,其还包括把手组件,所述把手组件包括把手和一体式的电子显示监视器,所述显示监视器与所述电子成像模块电通信。
9.根据权利要求8所述的器械,其中,所述显示监视器、把手、细长构件以及成像模块以固定关系安装,从而对齐地绕所述细长构件的纵向轴线旋转。

侧面取样开口,所述侧面取样开口位于所述细长构件中,并且定位成以及定尺寸成以及构造成有助于子宫内膜组织的收集;
电子成像模块,所述电子成像模块定位于所述细长构件的所述远端处;以及
流体开口,所述流体开口定位于所述细长构件的所述远端处,从而通过将流体传输成在远端方向上流动并由此清除靠近所述成像模块的碎片来改进使用所述电子成像模块的视觉检查。

光传输系统,所述光传输系统构造成照亮正在进行检查的所述子宫组织;
侧面取样开口,所述侧面取样开口位于所述细长构件中,并且定位成以及定尺寸成以及构造成有助于子宫内膜组织的收集;以及
电子成像模块,所述电子成像模块定位于所述细长构件的所述远端处。

形成远端尖端主体和电子成像模块,所述远端尖端主体定尺寸成容置光传输系统,所述光传输系统适合于照亮正在进行检查的所述子宫组织,所述电子成像模块定位于所述尖端主体的远端处,所述远端尖端主体也形成为提供侧面取样开口,所述侧面取样开口位于所述尖端主体中并且定尺寸成有助于子宫内膜组织的收集;
形成细长轴构件;以及
将所述远端尖端主体牢固地附接至所述细长轴构件,由此形成一体式的内窥镜的细长构件,所述细长构件定尺寸成以及构造成有助于所述远端插入通过患者的子宫颈并且进到所述子宫中。
