常规的医用敷料应用于多个方面,如伤口敷料、术后敷料和其它专用方面。常规的医用敷料的功能可以包括保护伤口部位和防御微生物。在一些情况下,当伤员在边远地区遭到严重的外伤或战伤时可以使用常规的医用敷料。例如,常规的医用敷料可以在就医前对伤员使用,其可能在所述伤口部位内带来由周围环境中的俘获碎片、灰尘和始终存在的污染的微生物引发的问题。另一个问题在向常规的医用敷料中加入抗微生物剂时发生。所述加入的抗微生物剂趋于过度强烈,引发组织损伤,或过于温和,不能防御大范围的微生物。抗微生物剂在医用敷料中的用途已经面临挑战。
在其它情况下,抗微生物剂用于护理手术部位。通常,所述抗微生物剂的使用包括先利用酒精再利用聚维酮碘清洗皮肤,或替换地先利用酒精再利用氯己定清洗皮肤。然而,一旦以这种方式进行护理,所述手术部位易受到通过接触传播或空气传播的微生物的再次污染,这样可能导致所述微生物传入患者的手术部位、组织或可能是血液中。因此,在术前部位周围预防可能的细菌污染已经面临挑战。
然而,由细菌污染带来的另一个挑战与导管和引流管有关。导管可以使流体引流、流体注射或通过手术器材进入,以及导流管使流体引流。持续使用所述导管或引流管会发生问题,会使进入点(即导管部位或引流部位)处易于感染。紧邻于导管部位或引流部位的皮肤可能受到皮肤细菌(如绿脓杆菌、金黄色葡萄球菌等)的污染。在所述导管部位或引流部位周围皮肤中的细菌可能进入患者血流中并可能表现为威胁生命的挑战。因此,在导管部位或引流部位预防可能的细菌污染面临另一个挑战。
发明内容
本公开文本描述了通过在医用敷料中使用感染抑制溶液和抗微生物剂来防御微生物。在一个实施方案中,所述感染抑制溶液含有各种浓度下的过氧化氢、过氧化脲、乙二胺四乙酸、柠檬酸钠或醇的组合物。
在另一个实施方案中,医用敷料包括底层、覆盖在所述底层上的透气膜层、设置在所述透气膜层内的防腐剂以及覆盖在所述透气膜层上的吸收材料层。所述防腐剂含有过氧化氢、过氧化脲、乙二胺四乙酸、柠檬酸钠或醇。
在又一个实施方案中,本公开文本描述了在导管部位抑制感染的方法。该方法包括提供包括放置在透气膜层内的防腐剂的敷料。所述防腐剂含有过氧化氢、乙二胺四乙酸(EDTA)、EDTA二钠或乙醇。而且,所述方法描述了通过裂缝将导管插入所述敷料的中心开口中,并将所述敷料的前表面贴附于患者皮肤上。

具体实施方式
概述
在一个实施方案中描述了感染抑制溶液,在另一个实施方案中描述了含有抗微生物剂混合物的抗微生物医用敷料。所述感染抑制溶液还含有抗微生物剂。所述溶液中的抗微生物剂和所述敷料中的抗微生物剂的混合物以各种浓度用于防御微生物、吸收分泌物、和/或促进愈合。用于所述感染抑制溶液中和用于所述抗微生物医用敷料中的抗微生物剂可以含有各种浓度下的过氧化氢、过氧化脲、乙二胺四乙酸、柠檬酸钠或醇。
所述感染抑制溶液和所述抗微生物医用敷料都提供了对微生物的防御以达到不同目的。例如,各种浓度的抗微生物剂可以提供用于防御不同阶段外伤或不同阶段愈合的微生物。例如,所述抗微生物医用敷料可以初始应用于外伤,如战伤、烧伤、割伤、擦伤等。所述抗微生物医用敷料中的抗微生物剂可以以逐步减少的形式涂覆,以调节患者在不同的愈合阶段对所述抗微生物剂的需要。
在另一个实施例中,抗微生物剂或感染抑制溶液可以在使用导管和引流管的过程中防御微生物带来的污染。特殊地,所述抗微生物医用敷料可以位于导管部位,以提供对所述导管部位处(倾向于延长置留)的微生物的防御。所述抗微生物医用敷料降低或消除了所述微生物的邻近部位污染,并减少或消除了在导管口和内腔上形成生物膜的微生物。
在又一个实施例中,抗微生物医用敷料可以用于手术前和手术过程中,以获得并保持无菌部位。所述抗微生物医用敷料可以继续对微生物防御,而同时进入手术部位。所述抗微生物医用敷料具有很多其它可能的用途。
描述的技术方案可以以任何数目的不同医用敷料和/或组合物实施,实施方案在下面示出的抗微生物医用敫料的内容中进行描述。
抗微生物医用敷料的实施例
本申请中描述了抗微生物医用敷料、预防伤口感染的方法、预防导管部位感染的方法和感染抑制或感染预防溶液。大体参照图1-9描述示例性的抗微生物医用敷料。
图1示出示例性的抗微生物医用敷料10的后视图。所述抗微生物医用敷料10包括后侧12。所述抗微生物医用敷料10显示为正方形,不过可以考虑用以下形状代替,例如,圆形、矩形、椭圆形、多边形等。
图2示出图1中显示的抗微生物医用敷料10的剖视图。所述剖视图沿着图1的线2-2,后侧12沿着图2的底部设置。如初始提供的,所述抗微生物医用敷料10具有与后侧12相对的上表面14。
所述抗微生物医用敷料10包括背衬层16,其可以是防潮层(vaporbarrier)或物理屏蔽层。所述背衬层16另外可以作为氧气屏蔽发挥作用。用于背衬层16的示例性材料包括但不限于聚乙烯、铝箔、氧化铝、涂覆氧化硅的聚合物膜、聚丙烯、有机硅基聚合物(硅树脂)、聚四氟乙烯(铁氟龙)和聚氯乙烯。另外,基本磨碎的或粉末形式的无机陶瓷材料可以用于控制地释放所述抗微生物剂,所述无机陶瓷材料含有吸收的、装入微胶囊的或其它形式并入的抗微生物剂。在实施方案中,所述背衬层16可以是双层或三层的这些材料的两种或多种混合物。如图2所示,背衬层16可以最初设置为抗微生物医用敷料10的上表面。所述背衬层16的上面区域(孔线18上方)可以具有与背衬层16下方区域不同的组合物,所述上方区域和下方区域材料选自例如上面描述的材料或其它材料。
所述抗微生物医用敷料10也可以包括设置为覆盖在背衬层16的下方区域上的膜20,使得如最初提供的,如图2所示,膜20完全被背衬层16围住。膜20优选为由允许氧气和流体扩散的聚合物材料组成。替换地,膜20可以是透气膜层。可以用于膜20的示例性的材料包括但不限于凝胶材料,如淀粉聚合物、纤维素凝胶、聚乙烯泡棉和硅树脂开孔泡棉。可以利用具有抗微生物性质的溶液或凝胶将膜20灌注、涂覆或浸渍。这样的溶液或凝胶可以含有过氧化氢、过氧化脲、其它氧化剂、乙醇、乙二胺四乙酸(EDTA)、EDTA的盐类、柠檬酸钠、其它螯合剂、清洁剂和水的一种或多种。在另一个实施方案中,所述溶液可以含有各种浓度下的过氧化氢、EDTA、乙醇和水。
如图2所示,可以在背衬层16的上部分和下部分之间提供孔线或划线。如图3所示,所述孔线或划线用于移除背衬层16的上部分,以暴露出膜20的上表面22。连接背衬材料16的上层和下层的可替换方法是可以考虑的,如粘附、热熔等,以连接两个所述背衬层以保持沿着连接部位的防潮层。
图3示出示例性的抗微生物医用敷料10的剖视图。将显示的膜20的上表面22设置为与伤口部位接触。上表面22可以含有透明的、通透的或半透明的材料,用以在防御微生物的同时进入手术部位。通过将膜20的上表面22贴附于所述伤口部位上,使浸渍的感染抑制的或防御的溶液与患者或人的皮肤接触,以提供感染抑制的效果或感染预防的效果。术语“感染抑制溶液”可以与术语“感染预防溶液”互换使用,以表示含有各种浓度下的抗微生物剂的溶液,所述抗微生物剂用于防御微生物、吸收分泌物、和/或促进愈合。
抗微生物医用敷料可以用于使皮肤破裂、刺穿皮肤或透入皮肤的伤口。示例性的伤口类型包括但不限于刺伤、烧伤、割伤和擦伤。另外,所述抗微生物医用敷料可以用于要求将设备插入到患者身体中的医疗过程。医疗过程的示例性种类包括手术切口、穿刺部位、导管部位(例如,血管内或尿透析导管)、腹膜透析部位、腹腔镜手术进入部位、血管内线口、引流管部位等。而且,所述抗微生物医用敷料可以用于将发生医用过程的预治疗部位。例如,可以在进行医用过程之前,可以识别穿刺部位、切口部位或导管部位并且施用抗微生物医用敷料。这样的部位护理可以利用抗微生物感染抑制溶液预处理所述部位,以在医疗过程中预防或使感染发生的危险最小化。含有抗微生物剂的防护敷料可以置于原位直到进行所述外科手术。附加的防护敷料可以用于后续过程中以进一步将感染的危险最小化。另外,在膜20内使用抗微生物溶液中的螯合剂,可以抑制基质金属蛋白酶(MMP)活性。二价辅助因子(锌)的键合降低了破坏性的催化高炎症反应。
螯合剂的存在可以用于治疗烧伤。对金属蛋白酶类(metalloproteinase)或金属蛋白酶类(metalloprotease)(其一或两者,MMP)的抑制效果可以降低或避免由MMP的升高水平导致的进一步的组织破坏。在烧伤的情况下,释放的MMP活性可以增加烧伤的严重性(在深度和面积上)。因此,利用含有合适浓度的适中螯合剂的抗微生物医用敷料或感染预防溶液治疗烧伤可以降低或预防由MMP导致的潜在的组织破坏。适中的螯合剂可以包括但不限于柠檬酸盐、EDTA或其盐类,其它代替的化合物,如水杨酸或水杨酸酯和很多本领域熟知的其它已知物质。随着螯合剂的强度、稳定性和其它变量发生改变,但浓度倾向于在一个范围中。伤口情况也发生相当大的改变。实际上,浓度可以基于对愈合过程或缺乏愈合过程的观察情况。
抗微生物剂具有很多目的。例如,杀菌剂是指杀死微生物(例如,杀孢子的、杀病毒的、杀细菌的和杀原生动物的)的试剂。微生物抑制剂是指抑制微生物生长(例如,抑制细菌的、抑制真菌的和抑制孢子的)试剂。另一个目的是微生物防御机制的破坏。例如,微生物防御机制的破坏系统保护了对伤口感染或手术预防施用的抗微生物剂。如氨基糖苷类的分子(即庆大霉素、妥布霉素和丁胺卡那霉素)可以使药物治疗水平表现为抗细菌。这样可能对于高抗性革兰氏阴性杆状(即假单胞菌种(亚种)、不动杆菌亚种和大肠杆菌亚种)的细菌尤其重要。上述的抗微生物剂可能使微生物酶(即碳酸化酶类、氨基乙酰基转移酶类)失活,从而保持所述氨基糖苷类抗细菌的活性。例如,一些反防御系统β-内酰胺(青霉素类似物)、头孢菌素和碳青霉烯抗生素类的生物化学机制是已知的。
图4图解了根据抗微生物医用敷料的可替换的方案的抗微生物医用敫料10的剖视图。图4中示出的实施方案含有图2的实施方案的所有要素,并且按照前面的图将这样的特征排号。为附加的特征赋予新的标示符。在这个实施方案中,膜20具有最外沿23。在显示的实施方案中,将粘附材料30设置在膜20的最外沿23外的抗微生物医用敷料10的外围区域。

图6图解了图5中示出的示例性的抗微生物医用敷料10的俯视图。图5中示出的剖视图沿着图6的线5-5。如图6所示,可以将粘附材料30设置为围绕抗微生物医用敷料10的整个外围区域。替换地,可以将粘附材料30间歇地围绕所述外围设置,并不是沿着所有侧面,而只是在所述外围的角落处等(未显示)。
所述抗微生物医用敷料10的尺寸不限于任何特定尺寸。在含有或不含有粘附剂的情况下,以多种尺寸设置抗微生物医用敷料10。可以将抗微生物医用敷料10设置为小到覆盖单针穿透部位面积的尺寸,或大到覆盖大面积身体烧伤的尺寸。
图7图解了另一个示例性的抗微生物医用敷料10的剖视图。为前面标识的结构给予同样的标号,并给予附加的特征新的标号。如图所示,附加层40由可以设置为覆盖膜20的上表面22的材料组成。在这种情况下,将最外侧表面42设置为与患者皮肤接触。最外侧表面42优选为吸收性材料,其为海绵状的并可以是毡制品型的或纤维质材料。这样的材料的存在可以有益于吸收从皮肤流出的液体,如汗液、血液或伤口渗液。
图8和9图解了设置为位于导管部位的示例性的抗微生物医用敷料。图8图解了抗微生物医用敷料10的后视图。所述抗微生物医用敷料10显示为环形的,不过替换的形状也可以考虑。例如,所述替代的形状可以包括但不限于椭圆形、矩形、方形、多边形和设计为适合指定身体部位的形状。另外,基本不是平面而是三维的替换的形状也可以考虑,即凸面状、凹面状或一些它们的结合,用以好的适应指定的身体部位,例如治疗头部伤口、肘部伤口等。所述抗微生物医用敷料10包括外沿50。中心开口52可以位于整体穿过抗微生物医用敷料10的中心。可以将狭缝54设置为从抗微生物医用敷料10的中心开口52延伸到外沿50。所述狭缝54穿过抗微生物医用敷料10的所有层。
图9图解了图8中示例性的抗微生物医用敷料的剖视图。图9中的剖视图沿着图8的线9-9。如图9所示,中心开口52也延伸通过膜20的所用层。这个实施方案的抗微生物医用敷料10可以应用于导管部位,通过将导管由狭缝54放置穿过中心开口52并将上表面22放置为紧贴患者皮肤。可以利用含有抗微生物剂的溶液或凝胶灌注、浸渍或涂覆膜20。所述溶液可以含有过氧化氢、过氧化脲、其它氧化剂、乙醇、EDTA、柠檬酸钠、其它螯合剂、清洁剂、苄基过氧化物和水的一种或多种。在另一个实施方案中,凝胶形式的溶液或组合物可以包括各种浓度的过氧化氢、EDTA、乙醇和水。注意到任何或所有描述的任选特征(如粘附剂和任选的吸收层)都可以与显示的特征结合使用。图8和9中示出的抗微生物医用敷料的尺寸不限于任何特定尺寸;然而,对于导管应用,一些实施方案可以使用2-3英寸直径的抗微生物医用敷料。
在一些实施方案中,可以将所述抗微生物医用敷料构造为具有狭缝、孔洞、嵌片或其它开口。所述开口可以覆盖在装置插入患者处的皮肤区域上,如注射部位、导管部位、切口部位、供皮移植部位等。在所述医疗过程进行中,伴随着所述过程穿过所述开口进行,所述抗微生物医用敷料可以保持到位。由于在所述医疗过程进行过程中使暴露于环境中的组织最小化,因此使所述部位能尽可能保持无菌。
在使用之前,图1-9中描述的抗微生物医用敷料可以含在外部保护包装中。所述外部保护包装包括但不限于灭菌的袋子、套管和/或包皮。所述外部保护包装中的抗微生物医用敷料的灭菌可以通过伽玛射线、电子束射线、汽化的过氧化氢或其它常规的灭菌方法进行。
感染抑制溶液应用的实施例
所述抗微生物医用敷料另外包括用于清洁伤口部位、血管内线口等的感染抑制溶液。例如,可以将所述感染抑制溶液灌注、浸渍或涂覆在抗微生物医用敷料10的膜20上。总体说来,所述感染抑制溶液包括不同浓度下的水、乙二胺四乙酸(EDMA)、乙醇和过氧化氢。所述感染抑制溶液可以包括水(即H2O)、烈性并无毒的螯合剂如EDTA、短链一元醇(带有分子式C2H5OH和实验式C2H6O的乙醇)以及烈性、小分子氧化剂如过氧化氢(即H2O2)。在特殊的情况下,所述溶液可以优选主要由水、EDTA、乙醇和过氧化氢组成。
所述抗微生物剂可以是液体形式,如溶液或凝胶形式,每种都包括至少两种化学物种。除了需要pH调节剂(即过氧化氢)之外,所述两种或多种化学物种可以是彼此兼容的(即混合时没有不良反应发生),并优选彼此之间没有显著的反应活性。所述化学物种的混合能够形成稳定的悬浮液或优选为稳定的溶液。所述抗微生物剂可以是在含水介质或纯水中可溶混的或至少中度可溶的。而且,所述抗微生物剂中的化学物种的每种在所有浓度下,可以是对人体分开无毒的并且合并无毒的,其可以用于所述抗微生物剂中。另外,所述溶液中的化学物种可以包括小分子化合物,因此易于完全通过所述敷料的半透膜扩散。同时,所述化学物种中的至少一种具有缓冲作用,用以将所述溶液的pH调节到希望的最终pH,并保持这个pH一段时间,足以使所述敷料完全发挥作用。总体说来,希望的pH是身体的生理参数,大约7.4,但其它的pH值在指定或异常情况下可能更理想。进一步地,所述抗微生物剂中的化学物种可以具有不同程度的亲脂性和疏脂性,因此使所述抗微生物剂被“设计”为指定的溶剂特性,根据对指定用途的需要不同而定。例如,接近纯的或浓缩形式的醇,作为含在所述抗微生物溶液中的组分,为高亲脂性的。然而,当以小浓度存在于水中时,醇也是疏脂性的。
含有所述抗微生物剂的溶液通常含有小于或等于以体积计大约70%乙醇,并优选含有小于或等于以体积计大约50%乙醇。在再一个实施方案中,所述溶液可以含有大约5%-30%的乙醇。所述溶液还含有大约5-50mg/ml的乙二胺四乙酸(EDTA)或其盐类,如Na、K、Ca或EDTA二钠。所述溶液可以含有小于或等于以体积计大约7.5%的过氧化氢。在另一个实施方案中,所述溶液可以含有大约0.5%-5%过氧化氢。所述溶液的剩余体积由水组成。
对于一些用途,需要高(40%或更高)浓度醇。在这样的用途中,EDTA浓度应该尽量高。然而,EDTA在乙醇中几乎不可溶。因此,所述抗微生物剂的两种高度希望的特性-高乙醇浓度和高EDTA浓度,不能在简单的和稳定的溶液中获得。然而,在实验过程中,在过氧化氢存在下产生了出乎意料的结果。即使在此处描述的抗微生物剂中使用的高乙醇溶液浓度下,也可以避免固体EDTA的沉淀。潜在的抗微生物剂浓度的范围显著增加。因此,所述过氧化氢的功用价值从提供抗微生物性质扩展到提供制备高乙醇和EDTA浓度的溶液的方法。在一个实施方案中,所述溶液包括大约1.5%-6%浓度的过氧化氢、达到大约20mg/ml的EDTA、达到大约70%的乙醇,剩余的溶液体积由水组成。在加入乙醇之前,加入所述溶液的过氧化氢组分,使含有过氧化氢、EDTA和水的混合物的溶液稳定。当pH接近中性范围时,含在所述溶液中的EDTA的浓度比乙醇、水和EDTA可以混合存在的浓度高得多。
在另一个实施方案中,所述感染抑制溶液或抗微生物溶液含有EDTA和乙醇,其中由于加入过氧化氢,所述EDTA和乙醇以稳定的浓度共存(mutual concentrations),否则如果不存在过氧化氢,所述共存的浓度的量是不稳定的。
在一个实施方案中,所述溶液含有由于存在过氧化氢而以共存的浓度存在的EDTA和乙醇,否则如果不存在过氧化氢,所述共存的浓度的量是不稳定的。在又一个实施方案中,制备所述溶液含有如下步骤:将干燥的EDTA加入到容器中,加入大约20%-30%过氧化氢,加入最少量的水,加入大约0.1-1.0M稀释量的NaOH,同时在pH电极存在下搅拌,其中搅拌继续直到pH达到最小值并保持稳定大约1分钟;搅拌并监控pH的同时加入一定量乙醇以达到希望的百分比;加入更多的NaOH以达到希望的pH值;并加入更多的水以使所述感染抑制溶液达到希望的浓度。所述步骤可以以任何顺序进行并且可以针对各种抗微生物剂。
本发明的感染抑制溶液可以用于医疗过程,如用以预处理针注射部位、导管部位或切口部位。方法包括确认针注射部位、导管部位或切口部位并将所述感染抑制溶液施用于所确认部位的皮肤。所述溶液还可以用于清洁过程后的这些部位。
由于多种原因,所述感染抑制溶液也可以在所述抗微生物医用敷料中作为防腐剂使用。可以将其用于初始伤口愈合,并且随着愈合进行,对于所述敷料中强有力的抗微生物剂的需要可能仍然存在,但由于微生物负荷的降低需要降到较小的程度。所述抗微生物医用敷料的希望的特征可以是调节浓度、量或组合物的能力,用以满足患者在愈合的当前阶段的抗微生物剂的需要。基于浓度、量或组合物,可以推出逐步降低抗微生物作用。特殊地,愈合组织具有生物化学的需要,如依赖于愈合阶段而变化的通常的化学信号和营养需求、生长因素等。在愈合过程后期的强有力的抗微生物治疗由于干扰了修复机制、酶机制和组织化学机制,可能是有害的。因此,在某些情况下,最初释放感染抑制溶液中较高浓度的抗微生物剂,以及基于治疗的每个阶段逐渐降低抗微生物作用是足够的。在某些情况下,稍微降低所述抗微生物剂的总浓度是必要的。在其它实施方案中,通过降低一种或多种组分的相对浓度改变所述抗微生物剂的组合物而保持其它组分不变可能更有效。
在一个实施方案中,所述感染抑制溶液在所述抗微生物医用敷料中作为防腐剂使用。所述组合物可以含有EDTA、乙醇、水和过氧化氢。上述浓度下的EDTA具有使可能破坏过氧化氢的细菌酶失活的能力。所述细菌酶包括过氧化物酶、超氧化物歧化酶和谷胱甘肽还原酶。EDTA具有螯合用于酶活性的金属离子(即Fe+2、Zn+2)的能力。EDTA抑制血液或血清中类似的酶,从而增强了过氧化氢对皮肤和软组织的活性。从而增强了延迟的释放抗微生物剂和抗生物膜效果。
在另一个实施方案中,在外科手术开始之前可以将所述抗微生物医用敷料施用于手术区域,在使手术进行和可见的同时建立并保持未受污染的皮肤区域。所述抗微生物医用敷料可以通过抗微生物作用建立未受污染区域。相反,这样可以比现行方式更加有效,因为可以在所述外科手术之前的充足时间下施用,用以使所述抗微生物作用更有效进行。与在开始开刀之前通过利用简单的局部给药进行极强抗微生物相比,使用所述抗微生物医用敷料渗入皮肤和组织层更深。可以以这样的方式构造所述抗微生物医用敷料,以使所述外科手术进行,而在进行手术的同时所述抗微生物医用敫料在所述部位上保持就位。由于这样的原因,使靠近皮肤(即贴近患者皮肤)的层由透明或半透明、弹性材料制成是有好处的。而且,所述材料是半渗透的,使小分子试剂从吸收池传输,所述材料如附图中表示的膜,含有所述抗微生物剂,叠压在所述抗微生物医用敷料的末端部分内。所述传输将所述抗微生物剂连续地并控制地供应到所述部位中。
在又一个实施方案中,所述抗微生物医用敷料可以包括部分开口以观测所述部位进展,而保持所述部位和周围皮肤处于受保护状态。而且,在对所述部位进行观测之后,所述抗微生物医用敷料可以容易地恢复保护功能,在保持所述敷料通畅的同时使敷料变化以及该变化伴随的危险最小化。优选地,在不干扰贴近的半透层的情况下,所述观测可以完成,但需要可逆地向后折叠最末端持久的保护层,最优选为同时并可逆地向后折叠所述吸收层。
在另一个实施方案中,所述感染抑制溶液可以为了各种应用混合到凝胶材料中。所述凝胶材料可以包括但不限于多元醇或其它惰性凝胶材料。所述凝胶可以在进行医疗过程之前护理伤口部位,并在过程之后,护理导管部位、切口部位等周围。还可以通过使用拭子或聚合物泡棉来应用所述感染抑制溶液和凝胶。在又一个实施方案中,可以通过凝胶或泡棉形式的洗手乳来应用所述感染抑制溶液和凝胶。
接下来将讨论所述感染抑制溶液应用于抑制、降低或预防细菌生长。紧邻所述导管入口点(即,所述“部位”)处的皮肤经常受到普通皮肤细菌污染。很多这些普通的微生物制剂很少对完整皮肤有害,但如果进入患者深层组织或乃至进入血流中,危害很严重,甚至威胁生命。所述微生物可以包括金黄色葡萄球菌和万古霉素耐药肠球菌。所述微生物也可以在可用的表面上形成保护的生物膜,如可以在包括导管口和内腔的医疗装置的聚合物表面,在一些最近发现的情况下,在伤口组织本身形成生物膜。所述微生物形成了甚至更好地保护其中的微生物的生物膜,并显著减低对抗微生物剂的敏感性。生物膜是可以通过形成耐性的“黏液”层可以粘附在表面上并自我保护的微生物的聚集体,所述“黏液”层基本由表多糖和大量其它组分(主要包括蛋白质、来自死细菌的内毒素、DNA、纤维材料以及宿主蛋白)组成。
如上面描述的,本申请中描述的含有所述抗微生物剂的感染抑制溶液用于所述抗微生物医用敷料中。特殊地,在所述导管部位含有感染抑制溶液的抗微生物医用敷料具有几种用途。术语“在所述导管部位含有感染抑制溶液的抗微生物医用敷料”可以与术语“用于所述导管部位的抗微生物医用敷料”交换使用,用以表示位于所述导管部位的含有所述溶液的敷料。
用于所述导管部位的抗微生物医用敷料减轻或消除了微生物、尤其是那些能够形成生物膜微生物的邻近污染。而且,用于所述导管部位的抗微生物医用敷料在导管部位附近形成非污染皮肤区域。这个区域可以保持非污染状态至少1-3天,而不需要更换抗微生物医用敷料。进一步地,用于所述导管部位的抗微生物医用敷料使每天进入所述导管多次,而保持所述非污染的皮肤区域。
表中显示可以用于不同用途的溶液的各种浓度。为了方便考虑,表中示出每种浓度大致的量。所述大致的量示为mg/ml、以体积计、以百分比计,以及需要获得pH=7.0-7.8的稀释量。
微生物系统(以生物膜形式或作为浮游(自由游动)生物形式)使用生物化学反应的复杂系统以维持其生命过程。例如,形成生物膜的微生物,尤其是革兰氏阴性菌,使用酰基高丝氨酸内酯(AHL)衍生的化合物作为群体感应化合物。所述AHL含有几个脂肪酸酯基团。作为羧酸和醇之间的反应产物的酯可以进行将所述酸脱离醇的反应。
该反应可以包括水解反应,或替换地,酯基转移,其中起始的醇被新的醇代替,生成不同的羧酸酯。方程式如下:

上述任一反应都生成不再适于激发生物膜生成的初始功能的化合物。因此生物膜形成的可能性降低了。
返回来讨论内毒素,该霉素是所述细菌的结构分子。两种通常已知的是脂多糖类(LPS类)和脂寡糖类(LOS类),通过革兰氏阴性菌的死亡及其细胞壁溶解将其释放。所述内毒素毒性极大,可以引发严重疾病,例如,微克水平或更少的量就能导致成人中毒性休克或死亡。所述内毒素由脂类(主要为脂肪酸)、多糖链以及通常情况下一些蛋白质成分组成。将所述多糖连接到所述脂肪酸上的键为酯键,其中糖基(sugar-moiety)提供了醇功能。
LPS可以发生常规的水解反应和酯基转移反应,在任一情况下,产物都是低毒或无毒的并通过普通的代谢过程分解为较简单的分子。

上述任一反应都使该物质分解。因此,所述感染抑制溶液用于降低释放内毒素的潜在危险效果。
EDTA(螯合剂)在所述感染抑制溶液中的作用可以以几种方式阻止微生物生长:EDTA螯合剂本身可以通过下面的反应式表示,其中Ca+2是二价钙离子,EDTA为二价阴离子状态,即EDTA-2:
螯合作用:EDTA-2+Ca+2→EDTA-Ca(EDTA-单钙络合物)
已经显示表皮葡萄球菌细菌浓度可以通过EDTA降低,仅稍微低于由万古霉素引起的量。在其它情况下,EDTA螯合金属离子的能力可能阻碍生物膜形成,从而释放内置的细菌。因此,所述内置的细菌变为易被由于所述感染抑制溶液中的乙醇和过氧化氢而杀死。
同时也已知EDTA螯合铁离子。例如,铁离子(Fe+2和Fe+3)对于细菌和真菌生长是普遍需要的。EDTA与这两种形式都容易螯合。接下来,加入与Fe螯合的EDTA,从生长的介质中移出Fe,也防止了细菌和真菌生长,否则细菌和真菌将会生长。所述铁的螯合作用如下:

没有细菌或真菌生长
如上所述,一些种类的细菌和真菌微生物具有形成孢子的能力,其高度保护了所述微生物本身。然而,随着形成非常强耐性的孢壁,所述微生物形成孢子的能力极大地使毁坏所述孢子的问题加剧。已经显示出这样的产孢菌需要二价锰离子Mn+2作为孢子形成过程的一部分。已知EDTA能够螯合所述二价Mn+2离子,从而从所述溶液中将其移出并延迟或完全阻止孢子形成。所述过程如下:

没有细菌或真菌产生孢子
同时也已知EDTA具有额外的好处,推测其可能与螯合作用相关。这种额外的效果是由于EDTA的存在,细菌孢子不能转化为植物状态(vegetative state)。在EDTA存在下,这些孢子的电子显微测试未能检测到超微结构损坏。在EDTA存在下,除了不能发芽使其成为无活力之外,所述孢子在所有方面都与普通的孢子相同。
如上所述,所述感染抑制溶液用于所述抗微生物医用敷料中。特殊地,含有所述感染抑制溶液的抗微生物医用敷料具有几方面用途。所述含有感染抑制溶液的抗微生物医用敷料在手术前、手术中、手术后以及各个阶段愈合中减少或防止微生物形成。而且,所述含有感染抑制溶液的抗微生物医用敷料从伤口移除死亡或老化组织以促进愈合。
当受损组织愈合时,一个前提是应该移除老化的、患病的或甚至死亡的组织。为了完成这项工作,愈合体使用了多种试剂,尤其是酶成分来分解细胞组分。一种这样的酶,如蛋白酶类,是通常发现的。典型的蛋白酶类是基质金属蛋白酶(MMP类)。MMP类含有金属离子,通常Zn+2在该分子的活性部分的中心。已经显示Zn+2离子位于所述活性位置,但必须为实现其功能而部分暴露。认为MMP类对于治疗是有活性的,但可能过度生成并因此使所述治疗过程过度反应。通过伸出的Zn+2离子的暴露部分的螯合,EDTA可以协助控制所述过度反应。

所述抗微生物剂(如在所述感染抑制溶液中指定的)在伤口愈合中具有活性功能,所述功能不限于毁坏或削弱微生物生长的那些。由于大部分生物圈共有的生物化学性质的普遍特性,所述抗微生物剂也可以与所述伤口处存在的化学物种(即,从微生物角度看,宿主)相互作用。由于这个原因,如在所述感染抑制溶液中指定的抗微生物剂也可以作为局部试剂,用以改变所有生物化学行为的较大区域。例如,在普通的伤口愈合过程中,细胞因子和/或趋化因子被带入所述愈合区域中。然而,所述细胞因子和/或趋化因子的过度产生可能是有害的。所述细胞因子和/或趋化因子与过氧化氢自身或与乙醇以及所述螯合剂结合的相互作用可以反应,用以改变所述细胞因子和/或趋化因子的结构组成。例如,通过使所述细胞因子和/或趋化因子失活,其浓度降到更有效的水平。
如所述感染抑制溶液中指定的抗微生物剂-过氧化氢,具有额外的不限于毁坏或削弱微生物生长的那些功能。大量生物化学键通过浓度减小状态(即含有最小量的氧)下的原子形成。过氧化氢或由过氧化氢衍生的化学基团通常可以将氧原子加入到还原的分子中,改变其化学结构和化学性质。在很多情况下,所述氧化的分子不能表现通常的功能,并且发生细菌死亡。很多蛋白质分子具有络合的三维结构,其通过-S-S-基团稳定。单个S原子来自半胱氨酸氨基酸残基,所述半胱氨酸氨基酸残基位于彼此远离的蛋白质氨基酸链上的位置,但由于折叠、扭曲或所述链的其它变形能够结合在一起,使其相互邻近。将S结合在一起形成-S-S-基团在蛋白质的3-D形状上保持了折叠的或扭曲特征。所述指定的形状基本上是所述蛋白质分子表现酶(催化)功能的绝对特征。小量的氧化剂(如过氧化氢)在基本3-D形状上引发主要变化,并使所述酶失活。可以通过反应式进行图解,使用ABC和DEF作为通过化学键(利用短的竖直或水平线表示)结合到一起的氨基酸链:

在一些情况下,所述较少的络合形状还可以改为如-SOH或-SO2H的硫的更高的氧化态。若失去了关键的形状,所述酶将不再能够催化反应,并且将可能发生细菌死亡。
也已知过氧化氢作为蒸汽形式的杀菌剂并通常用在大量可商用的杀菌剂装置中。用于所述杀菌剂装置中的过氧化氢具有优秀的渗透到微生物体内的功能,并通过包括但不限于上面描述的各种机理将其杀死。也已知液体过氧化氢以较高的浓度(7.5%或更高)具有有效的抗微生物作用。然而,这些浓度不适合于人体使用,因为7.5%或更高的浓度在暴露的皮肤引起腐蚀性损伤。较高的浓度倾向于使所述抗微生物溶液中的其它化合物不稳定,因为稳定性要求所述溶液高的酸性,通常低于pH3.0并甚至接近2.0。
所述感染抑制溶液以生理pH值或接近生理pH值(例如,大约pH7.4)提供稳定溶液。在实施方案中,pH值可以为大约6.8-7.8。可以进行皮肤应用以及甚至上皮组织,包括子宫颈、阴道、肛门和口腔组织的应用是有效的。
在很多情况下,所述分子结构包括提供化学“弱键”的官能团,即所述分子遭到在所述抗微生物混合物中所述试剂的溶菌作用的地方。通常,当那些官能团分解为其较简单的组分时,新释放的组分具有重组初始连接的可能,因此重新获得初始的分子及其性质。这对于整个愈合过程起反作用。例如,通过水解的酯的分裂形成两个较小的分子,醇和羧酸。在一些情况下通过相反过程重新结合形成所述酯:
酯的水解:R-COOEt+H2O→R-CO2H+EtOH
重新酯化:R-CO2H+EtOH→R-COOEt+H2O(式2)
更具体的实施例是R-基团为长链脂肪酸并且所述醇基本为式1所示的糖分子情况下的AHL类。糖含有几个氧基官能团,并在一些情况下含有可以被氧化的其它基团。希望阻止式2所示的逆反应发生,在式2中水解产物发生逆反应。所述逆反应重新形成功能性的AHL,并使群体感应和生物膜的形成继续进行。过氧化氢可以由几个途径改变分子中可氧化位点的结构。例如,通过将-OH基团连接在所述糖上,如果所述结构特征允许,可以形成酮或醛基团。在任一情况下,所述过氧化氢可以通过改变所述初始反应产物防止反应的可逆性(式2)。因此即使再次酯化反应发生,该反应的产物不能是通常的AHL,并因此不能发挥功能以发出生物膜信号。

在伤口愈合过程中,非常不希望出现组织低氧。一些伤口病原体(可以包括厌氧细菌或产气荚膜梭菌)只能在低氧压的环境下生长。由于局部缺血和组织缺氧,严重的或甚至灾难性的气性坏疽会导致肢体丧失或死亡。所述感染抑制溶液是高氧化性的,主要由于所述过氧化氢的含量。由于其小分子的特性、其在含水介质中特别的分子溶解性以及在乙醇的帮助下穿过细胞屏障的能力,因此所述过氧化氢在所述伤口区域内建立了被高度氧化的环境。这样的氧化的环境,在湿润或干燥条件下基本上都积极刺激伤口愈合,避免了局部缺血的负面影响。
组织低氧也引发宿主白细胞的吞噬功能的障碍。所述过氧化氢在宿主白细胞中建立了促进正常吞噬功能恢复并增强吞噬细胞恢复正常的杀菌作用的富氧环境。
结论
尽管本主题已经对结构特征和/或组合物进行具体描述,但理解为所附权利要求限定的主题不必要受限于具体描述的特征或组合物。相反,所具体描述的特征和组合物作为实施本权利要求的示例性形式来公开。
Claims (23)
1.一种医用敷料,包括
包括防潮层的底层,该底层含有铝箔、氧化铝或涂覆氧化硅的聚合物膜、聚乙烯或聚丙烯中的至少一种或多种;
覆盖在所述底层上的透气膜层,该透气膜层具有构造为与人皮肤接触的上表面;以及
设置在所述透气膜层内的防腐剂,该防腐剂含有过氧化氢、过氧化脲、乙二胺四乙酸、柠檬酸钠或醇中的一种或多种。
2.根据权利要求1所述的医用敷料,其特征在于,还包括可移动的顶层,该顶层包括防潮层,所述防潮层含有铝箔、氧化铝、涂覆氧化硅的聚合物膜、聚乙烯、聚丙烯、聚硅氧烷、聚氯乙烯、聚四氟乙烯或聚酯树脂中的至少一种或多种。
3.根据权利要求2所述的医用敷料,其特征在于,所述可移动的顶层和所述底层由相同的材料形成。
4.根据权利要求2所述的医用敷料,其特征在于,所述可移动的顶层与所述底层可移动地连接,并且所述可移动的顶层被构造为通过撕开位于所述底层和所述可移动的顶层之间的孔线移除。
5.根据权利要求2所述的医用敷料,其特征在于,还包括围绕所述顶层周围的粘附材料区域。
6.一种医用敷料,包括

7.根据权利要求6所述的医用敷料,其特征在于,所述医用敷料具有穿过所述医用敷料的中心开口,以及从所述中心开口延伸通过所述医用敷料的外沿的狭缝。
8.根据权利要求6所述的医用敷料,其特征在于,还包括围绕所述医用敷料周围的粘附材料区域。
9.根据权利要求6所述的医用敷料,其特征在于,所述上表面含有透明材料、通透材料或半透明材料中的至少一种。
10.根据权利要求6所述的医用敷料,其特征在于,所述医用敷料包括可观察到人皮肤上的部位进展情况的部分开口。
11.一种抑制伤口感染的方法,包括
识别伤口部位;和
将抗微生物敷料贴附于所述伤口部位,该抗微生物敷料包括:
包括防潮层的底层,该底层含有铝箔、氧化铝、涂覆氧化硅的聚合物膜、聚乙烯、聚丙烯、聚硅氧烷、聚氯乙烯、聚四氟乙烯或聚酯树脂中的至少一种或多种;
覆盖在所述底层上方的透气膜层;
设置在所述透气膜层内的防腐剂,该防腐剂含有过氧化氢、过氧化脲、乙二胺四乙酸(EDTA)、EDTA的盐类、柠檬酸钠或醇的至少一种或多种;以及
覆盖在所述透气膜层上方的吸收材料层,该吸收材料层具有被构造为与患者皮肤接触的上表面。
12.根据权利要求11所述的方法,其特征在于,所述伤口部位是开刀的部位或放置导管的部位,其中,在开刀或放置所述导管以预处理所述伤口部位之前进行识别和贴附。
13.根据权利要求12所述的方法,其特征在于,还包括防御微生物的同时通过所述抗微生物敷料进入所述伤口部位以进行外科手术。
14.一种在导管部位抑制感染的方法,包括:
提供包括设置在透气膜层内的防腐剂的敷料,该防腐剂含有过氧化氢、乙二胺四乙酸(EDTA)、EDTA二钠、水或乙醇中的至少一种或多种;
将导管通过狭缝插入所述敷料的中心开口中;以及
将所述敷料的前表面贴附在患者的皮肤上。
15.根据权利要求14所述的方法,其特征在于,所述敷料包括:
包括防潮层的底层,该底层含有铝箔、铝箔、氧化铝、涂覆氧化硅的聚合物膜、聚乙烯、聚丙烯、聚硅氧烷、聚氯乙烯、聚四氟乙烯或聚酯树脂中的至少一种或多种;
覆盖在所述底层上的透气膜层;以及
覆盖在所述透气膜层上方的吸收材料层,该吸收材料层具有被构造为与人皮肤接触的前表面。
16.根据权利要求14所述的方法,其特征在于,所述敷料还包括将螯合剂加入到设置在所述透气膜层内的防腐剂中。
17.根据权利要求14所述的方法,其特征在于,所述敷料还包括将苄基过氧化物加入到设置在所述透气膜层内的防腐剂中。
18.一种感染抑制溶液,包括:
大约5-50mg/ml的乙二胺四乙酸(EDTA);
以体积计,小于或等于大约70%的乙醇;
以体积计,小于或等于大约7.5%的过氧化氢;以及
水。
19.根据权利要求18所述的溶液,其特征在于,包括以体积计,小于或等于大约3%的过氧化氢。
20.根据权利要求18所述的溶液,其特征在于,在医用敷料、拭子、聚合物泡棉或洗手乳中的至少一种中的用途。
21.根据权利要求18所述的溶液,其特征在于,含有由于存在过氧化氢而以共存的浓度存在的EDTA和乙醇,其中在不存在过氧化氢时,所述共存的浓度的量否则是不稳定的。
22.根据权利要求18所述的溶液,其特征在于,还包括
将干燥的EDTA加入到容器中;
加入大约20%-30%的量的过氧化氢;
加入最少量的水;
加入大约0.1-1.0M的稀释量的NaOH,同时在pH电极存在下搅拌,其中搅拌继续直到pH达到最小值并保持稳定大约1分钟;
加入乙醇的量到希望的百分比,同时搅拌并监控pH;
加入更多的NaOH到希望的pH值;以及
加入更多的水以使所述感染抑制溶液达到希望的浓度。
23.一种护理针注射部位或导管部位的方法,该方法包括:
识别所述针注射部位或所述导管部位;以及
在所述识别的部位将医用敷料中的感染抑制溶液施用于患者皮肤,该感染抑制溶液含有:
水;
乙醇;
乙二胺四乙酸;以及
过氧化氢。
