Abstract

技术领域
本发明涉及在医学病症治疗中的功能性食物和补充物的领域。更具体地,本发明涉及用包含植物甾醇的脂质混合物治疗病症。
背景技术
整个本申请提及的所有出版物以引用方式全部并入本文中,包括其中所引用的全部参考文献。
植物固醇是存在于饮食中的天然存在的物质,作为植物油的微量组分。植物固醇在植物中所起的作用类似于胆固醇在哺乳动物中所起的作用,如,形成细胞膜结构。在人类营养物中,植物固醇和植物甾烷醇(stanols)在降低血浆总胆固醇和LDL-胆固醇水平方面均有效。术语“植物甾醇”包含植物固醇和植物甾烷醇。
植物甾醇是正常饮食的组分,主要来源于植物,如,植物油、种子、坚果以及基于谷物的产品。在西方饮食中,植物固醇的通常消耗量在200mg/d到400mg/d的范围内,而植物甾烷醇(phytostanols)(植物固醇的饱和形式)的摄取是很少的。
植物固醇和植物甾烷醇的较高的消耗量通过抑制饮食和由小肠内源产生的胆固醇的吸收而降低了血液胆固醇水平。这种抑制与植物固醇和甾烷醇与胆固醇的理化性质的相似性有关。
植物固醇/甾烷醇是吸收很差的化合物。对吸收、分布、代谢和排泄进行的研究表明植物固醇很少从肠道吸收(1-10%)。
自从二十世纪五十年代早期就已经认识到植物甾醇的降血胆固醇作用(Jones et al.(1997)Can.J.Physiol.Pharmacol.75,217-227)。已经在涉及2400多受试者的大量的临床试验中,研究了植物固醇的降低血液胆固醇作用,所用的剂量高达25g每天,持续的时间长达三年。在植物固醇的医学上监管的临床功效试验或一般临床应用中的整个这几十年中,没有观察到显著的副作用。此外,药物Cytellin(主要为β-谷甾醇)已经为处方用药达二十多年,并且具有良好的安全记录。此外,已经对植物固醇和植物甾烷醇进行了严格的毒理学评估。
植物固醇或植物甾烷醇,以及它们的脂肪酸酯和其它衍生物,通常是作为饮食补充物或各种健康促进食物和饮料中的功能性成分、以及饮食补充物的一般产品,所有这些是为了减少血液总胆固醇的水平,尤其是LDL(低密度脂蛋白)胆固醇的水平。
高胆固醇血症(高血液胆固醇)(其可以通过植物甾醇治疗或控制),是心血管疾病(CVD)的主要风险因素。CVD是全球死亡的主导原因之一。高胆固醇血症还导致动脉内斑块沉积物的形成和聚集,其导致动脉粥样硬化,该病理过程可导致冠心病(CHD)(一种心血管疾病)。
2型糖尿病和相关疾病(称为糖尿肥胖病和代谢综合征)与CVD有关,其发生率从上个世纪以来也显著提高。在大多数情况下,心血管疾病被这类代谢疾病提高甚至触发,或者其发作和发展被所述综合征加速。这些也与异常血脂特征的风险因素密切相关,尤其是高水平的总胆固醇和非HDL胆固醇种类,主要是LDL胆固醇。
具有发展代谢综合征、糖尿病、或肥胖症、以及CVD或CHD风险的人,或者已经被分类为患有所述疾病的患者的人,均被警告要监控他们的血脂情况,尤其是高水平的血液胆固醇。一种最有效的解决这个为许多健康疾病和风险所共有的风险因素的方法是摄取植物固醇或甾烷醇或它们的衍生物。
已经被用于解决这样的风险因素的有效的治疗方法是补充抑制素和植物甾醇。两种活性成分均可以解决高水平的血液胆固醇的问题,其中每种利用不同的作用机制。基于抑制素的药物成分抑制胆固醇的从头合成,而植物甾醇抑制饮食和内源性产生的胆固醇的吸收。已有资料显示采用抑制素治疗的患者(其仍具有提高的LDL胆固醇水平)消耗植物甾烷醇酯可以进一步降低总胆固醇和LDL胆固醇的水平,降低幅度分别为高达12%和17%(Blair et al.(2000)Am.J.Cardiol.86,46-52)。此外,用含有抑制素和植物甾醇酯的食物治疗患有原发性高胆固醇血症的患者,产生了LDL胆固醇减少的相加效应(Simons et al.(2002)Am.J.Cardiol.90,737-740)。实际上,Simons等人已经得出了结论:将植物甾醇酯加入到抑制素治疗中可以提供与双倍剂量的抑制素相当的LDL胆固醇降低。类似的结果也显示在其它研究资料中(Vuorio et al.(2000)Arterioscler.Thromb.Vasc.Biol.500-506)。
绝经后的女性表现出一些典型的症状(如潮热、盗汗、失眠、阴道干燥)和疾病(如骨质疏松症、CVD、以及中枢神经系统相关的改变(忧虑和情绪不稳定、抑郁和认知下降))。关于这类人群中的CVD并发症,已经完全证实心肌梗死率提高到三倍并且脑血管疾病迅速增加。相信雌激素在保护内皮细胞壁并可能地防止血栓形成方面起作用,因此,其缺乏后出现前述症状。激素取代疗法(HRT)对治疗绝经症状和骨质疏松症具有明确的作用。然而,来自女性健康倡导会(Women Health Initiative,WHI)的最近发现表明采用HRT提高了女性患心脏病、卒中、以及肺栓塞的风险。最近,服用含有人造黄油的植物甾烷醇6周的绝经后女性(年龄50-55岁)表现出LDL胆固醇与基线相比显著的降低,同时HDL胆固醇水平升高,因此LDL/HDL胆固醇的比值下降(Gylling and Miettinen(1999)Metabolism 48:575-580)。然而,在不同的甾烷醇介入后,α-胡萝卜素和β-胡萝卜素的浓度显著下降,而维生素A、维生素D和脂质标准化的α-维生素E的水平相当于基线值。
植物甾醇已经被联系并表现出具有抗癌作用。流行病学研究和实验研究表明:饮食中的植物固醇可以防止西方社会最常见的癌症,如结肠癌、乳腺癌、以及前列腺癌。乳腺癌风险低的人群比乳腺癌风险高的人群消耗更多的饮食中的植物固醇。许多体外研究表明植物固醇对人类乳腺癌和前列腺癌细胞是有毒的。体内动物实验已经表明植物固醇对结肠肿瘤、乳腺癌、以及前列腺癌的发生、生长、以及转移具有抑制作用(Awad,A.B.and Fink C.S.(2000)J.Nutr.130:2127-2130;Moreaua,R.A.et al.(2002)Progress in Lipid Research,41:457-500)。虽然有证据表明植物甾醇具有抗癌性能,但是仍不知道其抑制肿瘤生长的机制。然而,植物甾醇提供这种防护的可能机制包括:对肿瘤和宿主组织的膜结构和功能、以及调节肿瘤生长和凋亡的信号转导途径的直接作用(Awad and Fink(2000)id ibid)。
CVD和CHD、以及代谢疾病(如2型糖尿病)的一个主要风险因素是氧化性应激,自由基的相对增加。这些可以引起,例如,氧化的LDL的形成增加,氧化的LDL以相对较高水平存在时,引起泡沫细胞的形成(早期动脉粥样硬化的标志)。由于多种原因,一些环境原因(如污染增加)或营养不良,或者前氧化性营养物的摄取、或者不同的抗氧化性营养物的摄取不足,自由基可以在血液和其他组织中以高水平存在。此外,高自由基水平在代谢疾病(如2型糖尿病、糖尿肥胖症以及代谢综合征)中普遍存在,并伴随持续的高血糖状态。可以通过各种具有抗氧化活性的活性成分(基本上中和身体内存在的自由基)来处理或者甚至可以防止氧化性应激。
植物甾醇可能具有防止炎性相关疾病或病症的作用,所述炎性相关疾病或病症包括:淋巴细胞增生反应、肺结核、人类免疫缺陷病毒(HIV)、应激导致的免疫抑制、类风湿性关节炎、以及过敏性鼻炎/鼻窦炎(Bouic(2001)Opin Clin Nutr.Metab.Care 4:471-475)。植物固醇的抗炎活性的机制被认为是包括抑制炎性介质(如白介素6和单核细胞分泌的肿瘤坏死因子-α)的分泌;然而,大多数研究工作是利用动物模型进行的,因此,特定的机制仍需阐明。
植物甾醇还表现出抗溃疡活性,防止幽门螺杆菌感染。在最近的研究中,horse gram(一种药草,Dolichos属,印度栽培,用作食物和草料)中的植物甾醇酯,但不是固醇,在溃疡的幽门结扎模型(pyloric ligation model)中具有防护作用(Jayaraj et al.(2003)Phytother Res 17:391-398)。与固醇类和磷脂类一起,这些饮食中的食物脂质可以是一些地理区域中十二指肠溃疡低流行的原因,尽管存在一致的幽门螺杆菌感染的高流行。
针对所述疾病和/或病症的任何治疗均需伴随健康平衡的营养。这种营养当然应该为患者提供所有的必需营养物,这些维生素也在其中。这些维生素中的许多是亲脂性的,并且在这些维生素中对上述患者重要的那些维生素是维生素A和维生素E。这些维生素还具有抗氧化活性,其对于保护身体防止氧化性应激是很重要的并且可能防止自由基活性的损伤。维生素A,也称为视黄醇,是类胡萝卜素维生素(参见图1)。维生素A在肝脏中从不同的前维生素类胡萝卜素生成,尤其是从β-胡萝卜素生成,其是最重要的前维生素A,并且本身是一种抗氧化剂。维生素E,也称为α-生育酚,主要存在于植物来源,并且也是有效的抗氧化剂。这种亲脂性维生素通常伴随生育酚骨架的其它衍生物,如β-生育酚和γ-生育酚。
虽然植物甾醇和植物甾烷醇,以及它们的脂肪酸酯,单独或结合抑制素或其它成分时,是一种最有效的高胆固醇血症风险因素的治疗方法,但是最近的研究表明使用这些植物性成分会带来不良反应。关于补充植物甾醇的主要问题在于它可以影响脂溶性维生素和类胡萝卜素的吸收和循环水平。饮食植物甾醇抑制饮食和胆汁胆固醇的吸收,其本身又降低类胡萝卜素和脂溶性维生素的吸收(Noakes et al.(2002)Am.J.Clin.Nutr.75(1):79-86)。这会出现问题,因为如上所述,存在许多与类胡萝卜素和维生素的饮食消耗相关的健康益处(Hendriks et al.(1999)Eur.J.Clin.Nutr.53(4):319-327)。
此外,证实了一些临床实验,其中植物甾醇的消耗据报道导致血浆类胡萝卜素水平的显著降低(Katan et al.(2003)MayoClin.Proc.78(8):965-978)。在消耗含0.83g至3.6g植物甾醇/天的涂敷物(spread)或油达3到52周后会发生这些降低。最近,报道了在消耗2.2g植物甾醇/天一年后在轻度超重的男性志愿者中β-胡萝卜素和α-生育酚的生物利用度下降(Richelle et al.(2004)Am.J.Clin.Nutr.80(1):171-177)。然而,在这个试验中,植物固醇酯比游离植物固醇更多地降低β-胡萝卜素和α-生育酚的生物利用度。β-胡萝卜素水平的降低高达50%。
此外,虽然证明了植物甾醇和抑制素组合治疗的有利效果(Vuorio(2000)id ibid),但对维生素A、维生素E、以及α-胡萝卜素和β-胡萝卜素的血液水平的观察表明该治疗导致所有维生素和前维生素的血清水平降低,幅度从10%到50%。这些作用在由于遗传疾病经历治疗的儿童中尤其明显(参见综述Berger etal.(2004)Lipids Health Dis.3:5-24)。
欧洲联盟食品科学委员会(SCF)得出结论:包含植物甾醇的食品销售应当伴随可能的不良健康作用的研究,该不良健康作用除了其它之外,还包括对血浆β-胡萝卜素水平的影响。UK食品咨询委员会(FAC)建议:消费者应当被告知含有植物甾醇酯的产品对于少年儿童和哺乳女性在营养上是不合适的,因为他们不需要降低他们的血液胆固醇水平,并且存在影响维生素A水平的可能性。欧洲食品安全管理局(EFSA)的针对饮食产品、营养和变态反应的科学小组强调:需要风险管理措施以便使每天摄取超过3g植物甾醇/植物甾烷醇的可能性最小化,给消费者提供合适的信息,该信息是关于需要经常消耗水果和蔬菜以便应对该产品潜在的β-胡萝卜素降低作用。
Lichtenstein和其同事(Lichtenstein et al.(2001)Circulation.103:1177-1179)指出了与使用含有植物固烷醇/甾醇酯的脂肪相关的潜在风险,其中观察到血浆中α-胡萝卜素、β-胡萝卜素、α-生育酚、和/或番茄红素的水平降低,这是因为消耗包含甾烷醇酯和固醇酯的食物。
由于含有植物固醇的食物产品可能在进食期间被所有家庭成员分享,因此由非高胆固醇血症的个体摄取的潜力是重要的。因此,美国心脏协会建议应当采取进一步的研究和大规模的监视以确定含有植物固醇/甾烷醇酯的食物在血液胆固醇正常和高胆固醇血症的成年人以及儿童中的长期安全性。
如同药物活性成分之间的不良相互作用一样,不同活性成分或营养物(药物的或饮食的)之间的这种不良相互作用应当被避免,其可以被利用或者其被需要以应对CVD和如上所述的其他健康疾病的健康需要。这样的相互作用可以降低治疗中使用的一种或多种成分的有效性或摄取。在一些情况下,所述疾病的治疗中所用的成分的相互作用可以引起其它饮食营养物或成分的有效性或摄取降低,所述营养物或成分被个体需要以维持他的一般健康并且能够应对特定风险因素(为此他被治疗)。一般来说,所观察到的施加植物甾醇对维生素A和β-胡萝卜素、以及维生素E的摄取的不良影响还可能与其它饮食营养物、活性药物成分或饮食活性成分有关(其象类胡萝卜素和生育酚一样是亲脂性的)。这样的亲脂性饮食营养物、活性药物成分或饮食活性成分(其摄取或有效性可以受到平行给予植物甾醇的进一步损害)包括药物抗异常脂质血症成分、天然抗氧化剂、营养性或活性脂质等。图1和图2示出了若干亲脂性活性成分和饮食营养物以及维生素A和E的化学结构。

已经开发了以控制高水平的血胆固醇为目标的药物活性成分,并且该成分被广泛用于治疗具有上述疾病的风险的人群。也许最普遍的方法依赖抑制素化学家族的活性成分,已知其抑制胆固醇生产的生物合成途径,因此导致较低水平的血胆固醇水平。所用的其它活性成分是胆汁酸结合树脂、胆固醇吸收抑制剂、组合胆固醇吸收抑制剂和抑制素、甘油酸类(fibrates)、以及尼克酸。
各种合成的抗氧化剂已经用于工业和食品目的。目前的方法更优选使用天然抗氧化剂,主要是植物来源的。这样的植物来源的抗氧化剂包括番茄红素(主要存在于红色植物中,尤其是西红柿中),以及黄体素。其它植物来源的抗氧化剂包括玉米黄质和β-胡萝卜素。所有这些抗氧化剂可以分类为高度亲脂性的,其是它们的共同的类胡萝卜素骨架的直接结果。其它的抗氧化剂包括那些多酚化学性质的抗氧化剂。这样的类胡萝卜素、多酚和其它亲脂性天然抗氧化剂均可以用于治疗上述疾病,并且它们平行施加于植物甾醇应该是有利的。
ω-3长链多不饱和脂肪酸(LC-PUFA),尤其是二十二碳六烯酸(DHA)和二十碳五烯酸(EPA)已经被认为促进心血管健康,部分是由于抗高甘油三酯血症活性。美国食品及药物管理局(FDA)已经在2004年9月宣布这些脂肪酸已经获得了有资格的健康权利,用于通过消耗包含EPA和DHA的食物或饮食补充物来减少CHD的风险。确实,这些脂肪酸的消耗对于遭受上述疾病或病症的人群或希望预防或抑制所述疾病发作的人群会是有利的。
所有这些成分、以及其他成分,可以结合或单独使用,以便应对CVD、CHD、代谢疾病患者、高风险人群、癌症患者等的风险因素。
其它亲脂性药物活性成分,以及其它亲脂性饮食补充物(主要为提取或衍生自不同植物来源的植物成分),有待发现或在开发中以应对各种健康疾病,这些疾病需要或可以受益于植物甾醇和/或它们的衍生物的平行施加或摄取,并由此面对它们的摄取被所述植物甾醇施加所损害的风险。
因此,根据植物甾醇作为治疗高胆固醇血症的方法的有益效果(或与其它成分结合,这些成分应对各种CVD、CHD、代谢紊乱的健康风险、包括高甘油三酯血症、高血糖以及氧化性应激、以及其它健康疾病,如癌症),本发明的目的是提供一种治疗方法,其能够充分利用植物甾醇/植物甾烷醇及其酯的有益效果,而没有抑制其他重要成分(如维生素、抗氧化剂、以及甚至药物活性成分)的摄取的可能的不良作用。
超重或肥胖极大地增加了来自许多疾病的发病风险,包括2型糖尿病(DM)、高血压、异常脂质血症、冠心病、充血性心力衰竭、卒中、胆囊疾病、肝脏脂肪变性、骨关节炎、睡眠呼吸暂停、以及子宫内膜癌、乳腺癌、前列腺癌、以及结肠癌。所有原因的死亡率的增加也与更高的体重相关。通过利用植物甾醇/植物甾烷醇及其酯可以治疗或预防这些疾病中的许多疾病。
本发明的又一目的是提供一种治疗方法,该方法提供了植物甾醇/植物甾烷醇及其酯对高胆固醇血症的有益效果,同时还控制患者体重,甚至促进患者体重的减少。
因此,本发明的目的是提供一种需要植物甾醇治疗的疾病的治疗方法,而不会不利地影响任何一种亲脂性维生素、亲脂性药物、亲脂性营养物、或任何亲脂性药物活性成分的生物利用度,该方法是通过将PS-E(植物甾醇酯)和DAG(主要为1,3-甘油二酯)的混合物(溶解或分散在食用油或脂肪中)、或者包括该混合物的组合物或食物物品给予需要的患者。
通过下面的描述本发明的这个和其它目的将变得显而易见。
发明内容
在第一方面,本发明提供了一种治疗疾病的方法,该疾病需要不会不利地影响下述物质的任何一种的生物利用度的植物甾醇治疗,所述物质包括亲脂性维生素、亲脂性药物、亲脂性营养物、或任何亲脂性药物活性成分,所述方法包括将有效治疗量的PS-E(植物甾醇酯)和DAG(主要为1,3-甘油二酯)的混合物(可选地溶解或分散在食用油或脂肪中)、或包含该混合物的组合物或食物给予需要的患者。
主要地,本发明提供了一种治疗疾病的方法,该疾病需要不会不利地影响下述物质的任何一种的生物利用度的植物甾醇治疗,所述物质包括亲脂性维生素、亲脂性药物、亲脂性营养物、或任何亲脂性药物活性成分,所述方法包括将溶解或分散在食用油或脂肪中的有效治疗量的PS-E和DAG(主要为1,3-甘油二酯)的混合物、或包含该混合物的组合物或食物给予需要的患者。
在本发明的方法的一个具体实施方式中,所述亲脂性维生素为类胡萝卜素和生育酚,优选分别为维生素A和维生素E。
在本发明的方法的另一个具体实施方式中,所述亲脂性活性成分为植物甾醇。
需要植物甾醇治疗的所述疾病可以选自包括以下疾病的组:癌症、良性前列腺肥大、溃疡、前列腺炎、心血管疾病(CVD)、冠心病(CHD)、动脉粥样硬化、肺结核、人类免疫缺陷病毒(HIV)、应激所致的免疫抑制、类风湿性关节炎、以及变应性鼻炎/鼻窦炎。

具体实施方式
饮食的植物甾醇抑制饮食和胆汁胆固醇的吸收,并且长期每日饮食消耗植物甾醇导致血浆胆固醇和LDL胆固醇浓度的降低。这些降低与一些因素有关,包括:消耗的植物甾醇的剂量、植物甾醇的酯化、植物甾醇溶解于其中的基质、背景饮食、以及遗传因素。给予植物甾醇的主要的不希望的作用是它们可以干扰许多其它脂溶性饮食成分,如维生素(尤其是类胡萝卜素)以及脂溶性药物的吸收。此外,植物甾醇溶解于其中的基质可以明确地影响植物甾醇降低LDL胆固醇的效能。猜想在肠内的吸收过程中,植物甾醇不仅可以置换胆固醇而且可以置换其它亲脂性分子,并且将它们联合置入混合的胶束中。酯化作用和载体(其中植物甾醇被消耗)的其他成分可以影响进入混合的胶束相中的分配物。
Nestlé’s研究中心的科学家已经报道:加入食物中以减少胆固醇吸收的植物固醇还能够降低身体吸收β-胡萝卜素和维生素E的能力(Richelle(2004)id ibid)。和消耗游离固醇相比,消耗固醇酯时,这种不希望的副作用更大。
本申请的发明人观察到:在饮食中提供的植物甾醇和/或植物甾烷醇酯(PS-E)和DAG(主要为1,3-二酰甘油)的组合作为治疗例如高胆固醇血症的方法的一部分,具有有利的健康作用,而不会损害类胡萝卜素尤其是维生素A的摄取或血清水平。
DAG是指1,2-甘油二酯和1,3-甘油二酯的混合物,主要包含1,3-甘油二酯(约80%)。
本文所述的甾醇酯和DAG的组合物在其消耗后不会不利地影响生育酚或类胡萝卜素的摄取。相反,它维持该摄取在稳态水平。
当提及生育酚时,可以理解为包括α-生育酚和β-生育酚,以及维生素E和其它营养性或健康有利的衍生物。同样地,当提及类胡萝卜素时,可以理解为包括α-胡萝卜素和β-胡萝卜素、番茄红素、维生素A和其它营养性或健康有利的衍生物。
本申请的发明人先前已经表明:在实验动物中,包括溶解在菜子油、橄榄油或鱼油中的DAG(主要为1,3-二酰甘油)和PS-E的组合物具有下述性能:降低血中胆固醇和甘油三酯的水平以及降低巨噬细胞、血清和血清LDL的氧化水平(WO 2004/069150)。此外,该组合物还抑制泡沫细胞的形成(WO 2004/069150)。
在人类中进行的本研究中,在鱼油处理(LC-PUFA)后,观察到总胆固醇浓度的大幅度降低,而对照饮食消耗导致显著但是较小的降血胆固醇(hypocholesterolemic)作用(表2)。因此,与不含补充的植物固醇的饮食相比,以及与包含ω-3的处理相比,用PS-E+DAG-鱼(制剂A)治疗导致总胆固醇浓度的降低。
已经进一步证明PS-E+DAG-鱼(制剂A)具有有效的降血胆固醇作用和降血甘油三酯(hypotriglyceridemic)作用,并且适于治疗异常脂质血症(dyslipidemic)的个体(显示出对代谢综合征和糖尿病的易感性)。
然而,由于它们的作用机制,植物固醇酯丰富的产品可以影响脂溶性维生素的吸收。在消耗植物甾烷醇后较低浓度的血清α-生育酚并不是预料不到的结果,因为大多数生育酚在LDL颗粒中运载,在许多研究中表明其在消耗植物固醇后降低。同样地,若干类胡萝卜素,尤其是α-胡萝卜素和β-胡萝卜素的水平也容易随着植物固醇酯消耗而以相同方式变化。
令人惊奇地,本研究表明由本申请的发明人开发的PS-E-DAG组合物可以保护循环维生素的水平。
如表4和图1所示,与PS-E相比,用PS-E+DAG-鱼(制剂A)处理导致维生素A吸收水平的提高(P值=0.05)。与PS-E进行成对比较,视黄醇的血液水平在用PS-E+DAG-鱼(制剂A)治疗的患者中表现出5.1%的较高浓度。图1将这些显著差异表示为离对照饮食中位值水平的变化百分数。这些结果表明在包含DAG的基质中以相同剂量提供的植物固醇酯比单独的PS-E对维生素A的生物利用度具有更大的影响。
因此,PS-E+DAG-鱼(制剂A)除了其降血胆固醇作用外还维持脂溶性维生素的血液水平,这是非常重要的。这使得本文所述的组合物在基于植物甾醇的治疗领域具有重要的改进。如前所述,植物甾醇已经广泛用于降低循环胆固醇。然而,由于它们“吸收”循环脂质分子的能力,植物甾醇还能够影响其它循环脂质或亲脂性分子的水平,并且因此变得对身体有害。但是,本文所提供的组合物没有这种不良作用。
因此,本发明提供的组合物提供了超越目前市场上可获得的类似组合物的优势,因为它促进保护循环维生素的水平。类似地,可以预期在消耗本发明提供的PS-E-DAG组合物后,其它亲脂性分子(如亲脂性药物或其他药物活性成分,包括营养物)的水平将受到保护。
本发明的治疗性或药物组合物的量(其可有效治疗特定疾病、病症或失调)依赖疾病、病症或失调的性质并且可以通过标准的临床技术加以确定。此外,可以可选地采用体外测定以及体内试验来帮助确定最佳剂量范围。有效剂量可以从由体外或动物模型实验系统获得的剂量-反应曲线加以推断。
为了本发明目的的有效治疗量是通过本领域已知的这类考虑(方法)确定的量。该量必需,例如,足以能够获得植物甾醇的胆固醇降低效应,同时不会不利地影响其它亲脂性分子如维生素、或药物活性成分如抑制素、以及其他亲脂性药物或饮食成分的生物利用度。所述亲脂性分子,尤其是亲脂性药物,可以通过口服、或者通过其它方式,如静脉内、皮下、腹膜内、肌内或透皮(通过皮肤贴剂)来获得。
如本文所述,本发明提供的各种治疗方法以及改善体重管理的方法涉及给予有效治疗量的溶解或分散在食用油或脂肪中的PS-E和DAG的混合物、或包括其的组合物或食物制品。给予混合物、组合物或食物制品(视情况而定)的优选方法是口服。然而,尤其是对于混合物和组合物,其它给予方法也是可行的,如通过静脉内(i.v.)、皮下(s.c.)、腹膜内(i.p.)、肌内(i.m.)或透皮(t.d.,皮肤贴剂(skin patch))。食物制品可以是固体食物制品、饮料、或糊状物。
换句话说,本发明所述的组合物或混合物主要用于治疗需要植物甾醇治疗的疾病,并且在那种情况下维持亲脂性循环分子(如维生素、亲脂性药物、亲脂性营养物等)的生物利用度。可替换地,所述组合物还用于平行于其它疾病的治疗或与其它疾病的治疗组合,所述其它疾病的治疗需要使用亲脂性药物,如,一些镇痛药、麻醉药(anesthaetics)、免疫抑制剂、HIV蛋白酶抑制剂、抗疟药物、精神药物等。
本文所述的组合物还显示了具有与治疗肥胖症相关的性质。肥胖症是世界性逐步增加的问题并且是动脉粥样硬化、高血压和2型糖尿病的主要风险因素。如实施例4所示,平均BMI水平在用PS-E+DAG-橄榄(制剂B)组合物治疗后显示出降低的趋势,尤其是在女性中。如图2所示,与对照相比,最大和最显著的降低BMI水平的效果可以因为PS-E+DAG-橄榄(制剂B)补充而获得。
到目前为止,还没有报道在植物甾醇介入研究后BMI减少的相当的结果,包括最近的出版物(其中类似地限制性饮食提供给糖尿病患者和非糖尿病患者)(Lau et al.(2005)AmJ.Clin.Nutr.81:1351-1358)。虽然肥胖、异常脂质血症和糖尿病之间的联系已经有很多报道,但是仍然没有报道这样的药物或饮食补充物,该药物或饮食补充物提供了降低的血脂水平、BMI,同时维持脂溶性维生素水平。
因此,本发明的目的是提供一种治疗需要应对高胆固醇血症疾病的多种病症的方法或者人群的预防/维持治疗的方法,对于这些人群,高胆固醇血症的发展或其增加可能造成所述人群倾向于发展更严重的健康疾病,如CVD、CHD、动脉粥样硬化或代谢综合征,如2型糖尿病或肥胖。此外,下面的疾病也可以用本申请所述的组合物治疗:癌症、良性前列腺肥大、溃疡、前列腺炎、肺结核、人类免疫缺陷病毒(HIV)、应激所致的免疫抑制、类风湿性关节炎、以及变应性鼻炎/鼻窦炎。所述治疗方法包括提供植物甾醇(优选植物甾醇酯),连同甘油二酯(可选地甘油三酯),以引起血胆固醇水平降低同时维持其它亲脂性活性成分和/或营养物的正常摄取水平。所述活性成分可以是药物成分,如抑制素,其通过抑制胆固醇生产的生物合成途径来降低胆固醇水平。已经显示利用抑制素和植物甾醇治疗具有相加的有利效果。此外,所述成分可以为抗氧化剂、优选天然类胡萝卜素抗氧化剂,如黄体素、β-胡萝卜素、以及番茄红素。这样的抗氧化剂的使用在本发明的范围中是有利的,因为本文所述的治疗方法的最大的目标人群遭受或易于遭受氧化性应激,其可能使他们的心血管疾病或代谢综合征疾病恶化。其他成分还可以包括ω-3LC-PUFA、尤其是DHA和EPA。
实施例4所述的结果表明通过改变饮食可以影响植物甾醇的循环水平。此外,与PA-E饮食相比,本发明的PS-E+DAG组合物的消耗提高了植物甾醇和甾烷醇(如β-谷甾醇)的生物利用度。因此,本文所述的组合物可以明确地提高植物甾醇作为抗癌剂的效果,以及它们的其它健康益处。
此外,许多疾病可以起因于脂溶性维生素的水平降低,并且因此遭受或易于发展所述疾病的患者对于用本文所述的组合物进行治疗性和/或预防性治疗是非常好的候选者,该组合物可以维持循环维生素的水平。所述疾病包括卒中、糖尿病、乳腺癌、免疫缺陷、免疫抑制、骨质疏松症、骨质减少、光照老化、皮肤肿瘤、光敏反应、白内障生成和白内障、视网膜光退化、红细胞光化学分解、光照性红斑、光致癌作用、克罗恩氏病、短肠综合征、阿尔茨海默病、痴呆、婴儿非酒精性脂肪肝炎、多发性硬化。
特别是,亲脂性活性成分如饮食亲脂性维生素,以及其它饮食亲脂性营养物(其正常摄取对于维持良好的一般健康状况是非常重要的)是本文所述的治疗方法的重要靶标。在一个优选的具体实施方式中,所述饮食亲脂性维生素是基于类胡萝卜素或生育酚的,并且包括维生素A、维生素E、它们的前维生素以及它们的异构体和衍生物。在一些实例中,所述类胡萝卜素和/或生育酚维生素被认为是抗氧化剂。
一般来说,各种其它亲脂性分子,如药物成分和植物成分(其分配进入肠油/胶束相)的吸收受到植物甾醇的溶解性的损害。因此,本发明的治疗方法提供了治疗高胆固醇血症疾病或前高胆固醇血症疾病的方法,而不会抑制其他活性成分或饮食营养物的摄取或生物利用度,而不管这些活性成分或饮食营养物是否被需要以治疗疾病(CVD、CHD、代谢综合征)或其他健康疾病或病症的相同或相关方面,而为它们的治疗所需要的活性成分的摄取可能会受到用植物甾醇或它们的酯进行平行治疗的损害。
因此,本发明的又一方面提供了一种治疗和/或预防与代谢所致超重相关的疾病的方法,所述方法包括给予有效治疗量的饮食营养物或食物补充物、或本发明的组合物,如前所述。所述疾病可以是肥胖、糖尿病、高胆固醇血症和代谢综合征中的任何一种。
因此,根据本发明,作为治疗方法的一部分,所述植物甾醇酯的组合,在应对高胆固醇血症并维持其他亲脂性活性成分或饮食营养物的足够的摄取水平的同时,还提供了体重控制甚至体重减轻。如上所述,体重控制和/或减轻是通过这种治疗方法应对的健康疾病的治疗和/或预防的重要部分。

方法
人类临床研究设计
本研究被设计为随机化、单盲、交叉临床介入实验。这个实验涉及测试包含包括在正常饮食(与非补充饮食相比)中的特定脂肪酸和/或植物固醇混合物的饮食基质在中度超重的患者中(具有增加的血清脂质水平)对维生素吸收和体重增加的作用,对于这些患者,饮食调整是主要的,并且有时仅是治疗建议。
患者
募集了LDL胆固醇浓度>130mg/dL的24个志愿者(11个男性、13个女性、年龄为30-65岁)进行这项研究。患者的身体质量指数(BMI)为23-32kg/m2,除了其中3个患者BMI在21.4到23kg/m2之间。21个志愿者完成了这项研究(11个男性、10个女性)。
饮食
所有的志愿者经历根据半随机的拉丁方交叉设计的饮食实验(包含5个饮食阶段,每个持续4周)。首先提供对照饮食,随后进行随机化的其它阶段。每个进食期被4周间歇期(washout interval)分开,在该间歇期志愿者消耗他们通常的饮食而没有限制。这些饮食的组成在食物和营养物含量方面是类似的。基础饮食包含30%作为脂肪的能量(参见对照饮食),80mg胆固醇/1000千卡,12g纤维/1000千卡,15%作为蛋白质的能量,以及55%作为碳水化合物。可变的成分是处理油。

所有的膳食是能产生同等热量的,并且早餐在监管下每天食用。在监管下,在临床上,每天食用的早餐内给予治疗油。志愿者被指示仅食用和饮用由临床营养研究机构(Clinical NutritionResearch Unit(McGill University,Montreal,Canada))给予他们的物质,除了水。鼓励研究志愿者维持他们通常的身体活动水平。利用Mifflin方程式并且乘以1.7的活动系数来估计能量需要量。在最初的2周期间内调整能量摄取以维持稳定的体重并且在余下的4周和其它的治疗阶段保持不变。
血脂分析
在第1,2,28,29天,在禁食状态获得血液样品。立即分离血浆并储存在-80℃直到分析。测量在每个阶段的第1,2,28,29天所取的样品中的总胆固醇、植物固醇、以及甘油三酯水平。利用酶性试剂(Abbott A-GENT),在Abbott Spectrum CCX分析仪上(Abbott,Dallas,TX)通过自动化的方法一式两份地确定血浆总胆固醇和甘油三酯的水平。通过GLC(HP5890系列II:Hewlett-Peckard)确定血浆植物甾醇浓度。测量从第1天和(到)第28天的样品中维生素E(α-生育酚)和维生素A(视黄醇)水平,并且通过HPLC进行量化。在每个阶段的最后通过放射免疫测定来确定血浆瘦蛋白水平。
人体测量的测定法:
在五个饮食阶段的每个阶段的开始(第1,2,3天)和结尾(第27,28,29天)一式三份地进行下面的测定。着轻便服装,不穿鞋,每天早餐前测量体重。利用固定的测距仪测量高度,其是作为从头部顶端到脚部底端(不穿鞋)的距离。通过重量(kg)除以高度(m)的平方来计算BMI。
实施例1
PS-E+DAG-鱼(制剂A)的降血胆固醇作用
高胆固醇血症的超重志愿者被供应四周的对照饮食、LC-PUFA油、PS-E+DAG-鱼(制剂A)或PS-E,随后四周的冲洗(间隔,washout)和相反补充。如方法中所述在每个阶段的开始和末尾检测总胆固醇水平。结果示在表2中。数值表示21位患者在所示时间点的总胆固醇水平的平均数±S.E.M以及变化百分数。不同处理之间的统计显著性为P值<0.1,该统计显著性是通过单因素ANOVA,随后通过变化值的百分数之间的Tukey’s post hoc试验来确定。具有不同上标字母的平均值是显著不同的,P<0.05。
如表2所示,通过不同的饮食阶段消耗的所有患者中,总胆固醇浓度的基线和终点是相当的(P值分别=0.99和0.61)。然而,所有这些处理导致总胆固醇浓度的百分数的显著降低,虽然降低程度不同(P值=0.057)。Post Hoc评估表明PS-E+DAG-鱼(制剂A)在这个参数(即降低总胆固醇水平)上显示出最显著的阳性结果。

实施例2
PS-E+DAG-鱼(制剂A)的降血甘油三酯作用

如前针对这些志愿者的包含血浆胆固醇的组分所示,基线值实质上相似(P值=0.33)。给予这些治疗的最终结果显示对于降低这些高胆固醇血症患者的甘油三酯水平是有利的。然而,在含有ω-3处理的效果和其它脂肪酸饮食的效果之间具有明确的差别(P值=0.0036)。不起反应的鱼油降低了血浆禁食甘油三酯达37.1%(P值=0.007),而本发明的ω-3酯混合物(PA-E+DAG-鱼(制剂A))的同等效果是42.9%的降低(P值=0.00002),然而,仅存在轻度的趋势,其表明这些补充物的效果之间的差别(Student’s t检验成对分析,P值=0.112)。将这些补充物对禁食甘油三酯水平的作用和对照饮食(-5.3%;P值=0.04)和PS-E(-8.7%;P值=0.04)的相当温和的作用相比,提供了鱼油与植物油的消耗之间的统计学显著的差异(参见表3)。

血浆脂溶性维生素水平

补充植物甾醇饮食即PS-E,导致维生素A(视黄醇)的生物利用度与对照橄榄油相比降低(-5%)(分别为65.0±2.3μg/dL和68.2±2.5μg/dL)。然而,补充制剂A(鱼油中植物甾醇和甘油二酯的组合物)表现出与对照饮食水平相当(P值=0.41),并且与PS-E饮食的消耗相比,在血浆维生素A浓度方面表现出小的,然而显著的增加(分别为69.1±2.3μg/dL和65.0±2.5μg/dL;P值=0.05)。与这种倾向一致,在PS-E和PS-E+DAG-鱼(制剂A)饮食后,终点水平之间的高胆固醇血症患者维生素A浓度的成对变化表明:后者饮食消耗导致5.1%的更高的血浆维生素A水平。将维生素A水平与对照饮食中位值相比也得到类似的观察结果(参见图1)。
实施例4
PS-E+DAG-鱼(制剂A)对血浆植物甾醇浓度的影响
高胆固醇血症的超重志愿者被供应四周的对照饮食、LC-PUFA油、PS-E+DAG-鱼(制剂A)或PS-E,随后四周的冲洗和相反补充。如方法中所述,在每个阶段的开始和末尾检测植物甾醇水平。结果示在表5中。数值表示在所示时间点从21位患者获得的β-谷甾醇水平的平均数±S.E.M以及变化百分数。不同处理之间的统计显著性为P值<0.05,该统计显著性是通过单因素ANOVA,随后通过终点值之间的Tukey’post hoc试验来确定,以及变化的百分数之间的统计显著性为P值<0.001。具有不同上标字母的平均值是显著不同的,P<0.05。
在基线不同处理组之间没有差异(P值=0.44)。然而,处理后的值显示所示饮食之间显著的差异(P值=0.03)。这些差异在变化百分数中更显著,如表5所示(P值=0.0003)。包含两种植物固醇的饮食表现出在这种参数上显著的增加,然而,如通过Student’s t检验成对分析所评估的,在这些补充物的效果之间存在明显的倾向(P值=0.058)。当β-谷甾醇值被标准化为在这些饮食阶段终点的胆固醇水平时,获得相当的倾向。

人类身体不会内源性合成植物甾醇。循环植物甾醇仅通过肠道吸收获得。人类中血清植物甾醇的水平范围是从6到41μmol/L。因此,β-谷甾醇浓度的绝对变化在制剂A饮食介入组高于PS-E组,这个事实清楚地表明固醇水平受所消耗的饮食影响。这些结果表明植物甾醇的循环水平可以被饮食变化影响。此外,与PS-E饮食相比,本发明的固醇酯和DAG组合物的消耗可以提高植物甾醇和甾烷醇(如β-谷甾醇)的生物利用度,因此提高它们作为抗癌剂的效果以及其它健康益处。
实施例5
体重增加
高胆固醇血症的温和超重志愿者被供应四周的对照饮食、LC-PUFA油、PS-E+DAG-鱼(制剂A)、PS-E或PS-E+DAG-橄榄(油)(制剂B),随后四周的冲洗(间隔,washout)和相反补充。如方法中所述,在每个阶段的开始和末尾检测体重水平,随后计算BMI值。女性中BMI结果的变化百分数示于图2中。数值表示10位女性患者的BMI变化百分数的平均数±S.E.M。如通过Student’s t检验成对分析所发现的,所示处理之间的统计显著性为P值<0.05。
如方法中所示,21位患者完成了全部5个饮食阶段。在5个饮食组中能量摄取方面没有差别。利用标准秤测量体重,并通过体重(kg)除以高度的平方(m2)来计算BMI。最初和终点BMI水平计算为在5个饮食阶段的每个阶段的开始(第1、2、3天)和末端(第27、28、29天)所获得的BMI测量结果的平均数。
在本研究中,在全部5个饮食阶段中,从基线到终点BMI值存在微小但显著的降低(对照26.00±0.62到25.81±0.59,P值=0.01;LC-PUFA 26.31±0.69到26.17±0.65,P值=0.10;PS-E+DAG-鱼(制剂A)26.10±0.69到26.02±0.68,P值=0.10;PS-E 26.27±0.64到26.14±0.63,P值=0.10;制剂B 26.18±0.63到26.00±0.62,P值=0.001)。
男性和女性中BMI值从基线到终点水平之间的变化的进一步评估表明:基于这些植物油的植物固醇处理在降低女性BMI水平方面比在略微超重男性中的可比较效果的这些处理更有效;在对照、PS-E和PS-E+DAG-橄榄(制剂B)中从基线到终点的平均变化,女性中分别为-0.09±0.02,-0.22±0.02和-0.18±0.02,而男性中分别为-0.29±0.03,-0.14±0.02和-0.09±0.02。利用成对分析来比较这些女性患者(n=10)之间的BMI值的百分数变化,比较表明与对照橄榄油饮食相比,在补充PS-E+DAG-橄榄(制剂B)后更显著(P值=0.03;参见图2)。与对照和PS-E补充物相比,没有获得女性中BMI水平的变化%的明确和显著的降低(P值=0.28),表明饮食基质PS-E+DAG-橄榄(制剂B)在降低体重方面比PS-E更有效(图2)。有趣的是,与对照终点水平相比,由包含植物固醇的饮食所提供的在男性和女性之间对BMI水平的这些离散的影响进一步与女性中禁食瘦蛋白水平的相应降低有关(对于制剂B和PS-E为-8.6±6.3%和-6.9±7.1%),而对于男性的类似分析得到相反的结果(对于制剂B和PS-E为2.1±6.3%和6.5±4.6%)。
Claims (18)
1.溶解或分散在食用油或脂肪中的PS-E和DAG的混合物在制备维持需要植物甾醇治疗的患者中循环的亲脂性维生素、亲脂性药物、亲脂性营养物、或任何亲脂性药物活性成分的至少一种的生物利用度和/或血液水平的食用组合物中的应用,其中所述混合物中所述PS-E和所述DAG之间的重量比为约15∶1到1∶1。
2.溶解或分散在食用油或脂肪中的PS-E和DAG的混合物在制备提高需要植物甾醇治疗的患者中循环的亲脂性维生素、亲脂性药物、亲脂性营养物、或任何亲脂性药物活性成分的至少一种的生物利用度和/或血浆浓度的食用组合物中的应用,其中所述混合物中所述PS-E和所述DAG之间的重量比为约15∶1到1∶1。
3.溶解或分散在食用油或脂肪中的PS-E和DAG的混合物在制备改进需要其的患者的体重管理的食用组合物中的应用,其中所述混合物中所述PS-E和所述DAG之间的重量比为约15∶1到1∶1。
4.溶解或分散在食用油或脂肪中的PS-E和DAG的混合物在制备治疗导致超重的代谢疾病的食用组合物中的应用,其中所述混合物中所述PS-E和所述DAG之间的重量比为约15∶1到1∶1。
5.根据权利要求1和2中任一项所述的应用,其中,所述需要植物甾醇治疗的患者患有选自由癌症、良性前列腺肥大、溃疡、前列腺炎、肺结核、人类免疫缺陷病毒(HIV)、应激所致的免疫抑制、类风湿性关节炎、以及变应性鼻炎/鼻窦炎组成的组中的一种疾病。
6.根据权利要求1至4中任一权项所述的应用,其中,所述油能够选自由橄榄油、鱼油、菜子油、大豆油、向日葵油、红花油、棕榈油、鳄梨油、芝麻油、亚麻油、以及琉璃苣油组成的组。
7.根据权利要求1至4中任一权项所述的应用,其中,所述食用油能够为任何天然脂肪,选自由乳脂、无水乳脂、可可油和猪油组成的组。
8.根据权利要求1至4中任一权项所述的应用,其中,所述PS-E和DAG的脂肪酸残基相应于衍生它们的油的脂肪酸残基。
9.根据权利要求8所述的应用,其中,所述脂肪酸残基是油酸残基、棕榈酸残基、棕榈油酸残基、硬脂酸残基、亚油酸残基、亚麻酸残基、二十烷酸残基、二十二碳六烯酸(DHA)残基和二十碳五烯酸(EPA)残基中的任一种。
10.根据权利要求1至4中任一权项所述的应用,其中,所述PS-E为豆固醇、谷甾醇、β-谷甾醇、菜子甾醇、菜油甾醇、5-燕麦甾醇、β-谷甾烷醇、菜油甾烷醇和/或豆甾烷醇及其异构体和衍生物的脂肪酸酯。
11.根据权利要求1至4中任一权项所述的应用,其中,所述混合物进一步包括营养组合物的常见成分。
12.根据权利要求1至4中任一权项所述的应用,其中,所述重量比为约10∶1到约1∶1、约5∶1到1∶1和约2∶1中的任一个。
13.根据权利要求1至4中任一权项所述的应用,其中,所述混合物中DAG的量为至少1wt%。
14.根据权利要求1至4中任一权项所述的应用,其中,所述混合物中PSE的量为至少1wt%。
15.根据权利要求1至4中任一权项所述的应用,其中,所述混合物中的DAG的量为约1到约99wt%、从约4到约70wt%、从约7到约48wt%和从约10到约22wt%中的任一个,并且所述混合物中的PSE的量为约1到约99wt%、从约5到约70wt%、从约7到约60wt%、从约10到约60wt%、从约7到约35wt%和从约20到约35wt%中的任一个。
16.根据权利要求1至4中任一权项所述的应用,其中,所述混合物包括溶解或分散在橄榄油、菜子油和鱼油之一中的15wt%DAG和25wt%总PS-E。
17.根据权利要求16所述的应用,其中,所述油为鱼油。
18.根据权利要求1至4中任一权项所述的应用,其中,所述食用组合物被进一步合并入饮食营养物或食物补充物中。
