<1.第1実施形態>
(1-1 曝露防止カバーの構成)
カバー本体1は、容器ホルダ2と針体3の一部分を挟み込み、開口部1Bを残し、周縁を熱溶着することにより一体形成している。カバー本体の溶着に関しては、熱溶着に限るものではなく、接着剤等も利用可能である。図において熱溶着部分に関しては、認識しやすいように網目模様にて表記している。なお、図示はしないが、溶着部はカバー本体の端部とせず、端部から若干離間させた部位とすることで、カバー本体の端部を接着しないで残すこともできる。このようにすることで、カバー本体の端縁が溶着によって堅い状態となることを防ぎ、角部のエッジによって医療従事者の手などを当接する事態を回避することもできる。
この実施形態に係るカバー本体1の形状は、図に示すように全体を略長方形の形状としつつ、その右側上部に、突出部1Aを有している。この突出部1Aは、薬液容器を挿入する開口部1Bを、後述する閉塞部材にて閉塞するために設けている。この開口部1Bを、上述の通り突出部1Aの端縁に設けることで、カバー本体の端縁の全体でなく、カバー本体1の一部のみを部分的に閉塞しやすくできる。カバー本体は、開口部以外の周縁を、上述したように熱溶着で接合している。なお、カバー本体は、上記形状に限るものではなく、収容する薬液容器に合わせることができ、例えば、正方形、楕円形、台形等の形状とすることもできる。一方、カバー本体1は、図10に示すように、針先及び針カバーの上部に容易に開封可能な簡易仕切部を設けることもできる。この簡易仕切部は、カバー内面同士を熱溶着によりイージーピールシールとして作成している。これにより、曝露防止カバーを使用するまでは、針先及び針カバーが外気との接触を避けることができ、無菌状態を保つことができる。
また、曝露防止カバーは、薬液容器を懸吊するための保持部として容器ホルダ2を備えており、また、容器ホルダ2は、曝露防止カバーを点滴台等に懸吊させる懸吊部を兼ねている。図1に示す容器ホルダ2は、カバー本体1の内部にハンガー状の形状で薬液容器を懸吊可能な構成としている。曝露防止カバーは、カバーの内部にハンガー状の幅をもった支持体2Aを備えており、この支持体2Aは、図1に示すようにその左側にU字型に湾曲された上保持部2Dを形成している。また、支持体2Aの右端部は、図の右側に示すように下方に直角に屈折させ、さらに一定の長さで支持体2Aに直交する形で手前に直角に突出させた係止部2Bを形成している。上保持部2Dは、長さを支持体2Aの上部の幅より若干長く形成し、上保持部2Dの右端が係止部2Bに係止できるようになっている。この上保持部2Dは、薬液容器を保持する保持部として機能する。さらに、係止部2Bの端縁には、上保持部2Dの係止が外れないように突出片2Cを上向きに形成している。上保持部2Dの長さを長くすることで、容器ホルダ2に複数の薬液容器を懸吊し保持することができる。さらにまた、上保持部が二箇所以上ある薬液容器を保持できるように上保持部2Dを二本以上とすることもできる。
次に、針カバー4付き針体3の構成を図2の拡大斜視図により説明する。この針体3は、曝露防止カバー内に懸吊された薬液容器の接続口と接続可能な接続部である。針体3は、針先3A、突出管3B及びカバー本体1に固定される固定部3Cを備えている。この針先3Aは、カバー本体1の内部に配置されている。また針体3は、針先3Aの縦方向の中央部付近に針先口があり、その針先口から突出管3Bの下端部へ空洞を形成している。これにより、針体3は、抗がん剤等の薬液を通過させることができる。
また、上記針体3には、針先3Aの周囲を覆う形状で針カバー4を装着している。この針カバー4は、上方を開口した有底筒状の形状をなし、底面のほぼ中心が針先3Aの根元の部分で固定されている。また、針カバー4は、針先3Aの高さより高く形成されている。これにより、針体3の鋭利な先端がカバー本体1で覆われるため、この先端がカバー本体1に接触することを阻止し、先端がカバー本体1に刺さって破損することを防止できる。また、針体3周辺の無菌性を担保するため、針カバー4の上端を封止してカバー内部から隔離することもできる。
次に、曝露防止カバー内に薬液容器を懸吊し開口部をクリップにて閉塞した状態の外観斜視図を図4にて示す。薬液容器10は、カバー本体1の開口部1Bより挿入され、上保持部2Dに懸吊させる。この図の曝露防止カバーは、内部に薬液容器10を三本、上保持部2Dに懸吊し、上保持部2Dを係止部2Bに係止し固定させている。これにより、容器ホルダ2は、上保持部2Dにより三本の薬液容器を懸吊し、係止部2Bに係止させたことにより、薬液容器を安定的に保持することができる。
また本発明の曝露防止カバーの底部には、吸水シート等の吸水材を設けてもよい。一実施形態として、吸収材を設けた暴露防止カバーの外観斜視図を図8にて示す。この曝露防止カバーのカバー本体1には、内部の底部に吸水シート7を設けている。この吸水シート7は、吸水紙、澱粉膜及びポリエチレン等による多層構造体で形成可能である。また、当該シートは透明であることが好ましいが、不透明であっても、曝露防止カバー内の目視を妨げない箇所に設けることで好適に使用できる。図8のようにカバー本体1の底部に吸水シート7を設けることによって、不透明な吸水シートを使用する場合でも薬液容器全体としての透明性を確保することができる。このカバー本体1は、薬液容器10より漏洩した薬液の溜まりを吸水シート7により吸水することができる。さらに、医療廃棄物として排出時の取り扱い時に万が一カバー本体を損傷したとしても、薬液は吸水シート内に貯留しているため、薬液を外部に飛散させることを避けることができる。
次に、別の実施形態として、抗がん剤等の薬液が充填された容器を封入する第二実施形態に係る曝露防止カバーについて、図9の外観正面図により構成を説明する。この曝露防止カバーは、薬液容器を個別に収納できるように、仕切りを設けた複数のカバー本体31により構成している。なお、このカバー本体31は、上記第一実施形態と同様の材質等を使用することができることから、以下の説明を第二実施形態の特有な点に関してのみ行い、第一実施形態と共通の部材については詳細説明を省略する。
この図に示すカバー本体31は、3つの連結した形状の曝露防止カバーを表している。各カバー本体は薬液容器を1つのみ収納可能な大きさとしている。なお、連結される枚数は、3枚に限定されるものではなく、必要な薬液の量や種類等に応じてさらに複数のカバー本体31を連結させておくことができる。このような曝露防止カバーは、例えば複数を予め連結したシート状に構成すると共に、各連結部分において切り離しが可能な破断線39を形成しておき、使用される薬液容器の数に応じて破断線にて破断することにより、連結数を任意に調整することができる。例えば、一個の薬液容器を用いる場合には、カバー本体31を1枚のみ判断線39により切り離して利用することができる。
このカバー本体31は、図9の網目模様で示す部分を熱溶着等で接合している。この底部には、針体33の一部を熱溶着している。さらに、カバー本体31内部には、針体33と一体化している針先33A及び針カバー34が封入されている。このカバー本体31は、針先33A及び針カバー34の上部に容易に開封可能な簡易仕切部38を設けている。この簡易仕切部38は、カバー内面同士を熱溶着によりイージーピールシールとして作成している。これにより、曝露防止カバーを使用するまでは、針先33A及び針カバー34を外気との接触を避けることができ、無菌状態を保つことができる。なお、この例では簡易仕切部38を、カバー本体同士を部分的に溶着して線状に形成しているが、別部材で面状に形成することもできる。さらに、図示しないが、上述の通り、溶着部をカバー本体の端部から離間させることで、エッジ部分が堅くなることを防ぎ医療従事者の作業性を妨げないようにすることもできる。
次に、各連結コネクタ42を接続した後で、針体33の針先33Aと薬液容器50の接続口51を接続することにより、順次それぞれの薬液容器50の薬液を送出することができる。また針体33には、各薬液容器50への逆流を防ぐために、逆止弁を有することが好ましい。一方、三方活栓40及び連結コネクタ42には、誤った連結を防止するために、色分けを施している。
また、狭持孔35を設けずに薬液容器50の接続口51周縁を指で挟むことのできる変形例のカバー本体31’を図11に示す。このカバー本体31’には、上記カバー本体31と同様に、接続口51近傍の薬液容器50を保持する下保持部32’を2箇所に形成している。このカバー本体31’は、下保持部32’下方の外形を予め細く形成しておくことができる。これにより、カバー本体31’に収納された薬液容器の接続口周縁を外部より手で挟み込み狭持することができる。
以下、本発明の第3実施形態について、図12を参照しつつ説明する。図12は、本実施形態に係るカバーモジュールの正面図である。
図12に示すように、本実施形態の曝露防止カバー40には、後述する逆止弁47及び中継チューブ46が取り付けられており、これらを含めたカバーモジュール4として使用される。曝露防止カバー40は、上部に開口を有するカバー本体41を備えており、この開口をスライド式のチャック42によって閉じることができるように構成されている。すなわち、チャックのスライド部材を移動させることで、開口が閉じるように構成されている。また、カバー本体41は、上部開口の径が大きく、下部411は収容される薬液容器の外形にフィットするような内径を有している。これにより、カバー本体41の上部開口から挿入された薬液容器は、カバー本体41の下部において固定され、薬液容器内の薬液の水面が水平に保持される。このようなカバー本体41の下部411の形態が、本発明の保持部に相当する。したがって、薬液の残量を正確に視認することができる。また、カバー本体41の上部には、懸吊孔43が形成されている。
次に、生理食塩水等の輸液が収容された輸液容器21について、図13を参照しつつ説明する。図13は、本実施形態に係る生理食塩水用の薬液容器及びその付属品の正面図である。ここで用いられる輸液容器21は、第1実施形態の図6で示されたものとほぼ同じである。第1実施形態と相違する点は、図13に示すように、三方活栓26(連結部)の構成であり、上述した中継チューブ46の先端の接続アダプタ461を取り付け可能になっている。すなわち、中継チューブ46を取り付けたときに、接続アダプタ461の薄膜のカバーを破ることができるように中空の針体(図示省略)が取り付けられている。
次に、上記のように構成された曝露防止カバー40を用いた薬液の供給方法について、図14から図18も参照しつつ説明する。まず、必要な数の薬液が収容された薬液容器10A,10Bと、これを収容するカバーモジュール4,5を準備する。そして、各薬液容器10A,10Bをカバーモジュール4,5の曝露防止カバー40,50に収容し、密閉する。各薬液容器10A,10Bは曝露防止カバー40,50の下部411,511に保持され、カバー内で液面が水平となるように固定される。このとき、曝露防止カバー40,50の針体44,54が、薬液容器10A,10Bの接続口11A,11Bに刺入しないように注意する。ここでは、2つの薬液容器10A,10Bを用いた例を示し、はじめに接続するカバーモジュールを第1カバーモジュール4、2つ目に接続するカバーモジュールを第2カバーモジュール5(追加モジュール)と称して説明を行う。
この実施形態では、2つのカバーモジュールを用いたが、その数は特には限定されない。また、中継チューブ、逆止弁の構成は特には限定されず、公知のものを適宜使用可能である。3以上のカバーモジュールを用いる場合には、それぞれの中継チューブを直列的に接続する。すなわち、上述したように、一方の中継チューブの先端部を、他方の中継チューブの基端部、つまり暴露防止カバーの近傍の連結用ポートに接続し、すべての薬液容器の薬液が、1つの経路を通過して三方活栓に流れるようにする。

前記薬液ラインと輸液ラインとを三股状に接続することで、前記輸液及び薬液の少なくとも一方を排出可能な供給ラインを形成するステップと、を備え、
前記排出ステップに先立って、
前記輸液容器から、前記供給ラインに向かって輸液を排出する第1プライミングステップと、
前記輸液容器から、前記曝露防止カバーの接続部へ向かって輸液を流す第2プライミングステップと、
さらに備えている、請求項23に記載の薬液供給方法。 - 前記排出ステップの後に、
前記薬液ラインにおける前記接続部の近傍から、輸液を供給し、前記薬液ラインへ排出するフラッシュステップをさらに備えている、請求項21または22に記載の薬液供給方法。 - 前記第1準備ステップにおいて、複数の薬液容器を準備し、
前記第2準備ステップにおいて、前記複数の薬液容器をそれぞれ前記曝露防止カバーに収容して密閉し、
前記第3準備ステップにおいて、前記各曝露防止カバーの接続部に、前記薬液ラインをそれぞれ接続するとともに、当該複数の薬液ラインを直列的に接続し、
前記排出ステップでは、前記各薬液容器から順に薬液を排出する、請求項23に記載の薬液供給方法。 - 輸液が収容された輸液容器を準備する、第4準備ステップと、
前記輸液容器に、当該輸液容器から輸液を排出する輸液ラインを接続する、第5準備ステップと、
前記輸液ラインと先頭に配置された前記薬液ラインとを三股状に接続することで、前記輸液及び薬液の少なくとも一方を排出可能な供給ラインを形成するステップと、を備え、
前記先頭の薬液ラインへ排出するステップに先立って、前記輸液容器から、供給ラインに向かって輸液を排出する第1プライミングステップを実行し、
前記各薬液ラインへ排出するステップに先立って、前記輸液容器から、前記各曝露防止カバーの接続部へ向かって輸液を流す第2プライミングステップを実行する、請求項26に記載の薬液供給方法。 - 前記排出ステップにおける前記各薬液容器からの薬液の排出の後それぞれに、
前記各薬液ラインにおける前記接続部の近傍から、輸液を供給し、前記各薬液ラインへ排出するフラッシュステップをさらに備えている、請求項26または27に記載の薬液供給方法。 - 前記第2準備ステップでは、前記曝露防止カバーに、複数の前記薬液容器を収容して密閉し、
前記各薬液容器に対し、前記排出ステップを実行する、請求項23に記載の薬液供給方法。 - 輸液が収容された輸液容器を準備する、第4準備ステップと、
前記輸液容器に、当該輸液容器から輸液を排出する輸液ラインを接続する、第5準備ステップと、
前記薬液ラインと輸液ラインとを三股状に接続することで、前記輸液及び薬液の少なくとも一方を排出可能な排出部を形成するステップと、を備え、
前記先頭の薬液ラインへ排出するステップに先立って、前記輸液容器から、排出部に向かって輸液を排出する第1プライミングステップを実行し、
前記各薬液容器から薬液を排出するステップの前にそれぞれ、前記輸液容器から、前記曝露防止カバーの接続部へ向かって輸液を流す第2プライミングステップを実行する、請求項29に記載の薬液供給方法。 - 前記排出ステップにおける前記各薬液容器からの薬液の排出の後それぞれに、
前記各薬液ラインにおける前記接続部の近傍から、輸液を供給し、前記各薬液ラインへ排出するフラッシュステップをさらに備えている、請求項29または30に記載の薬液供給方法。 - 輸液が収容された輸液容器を準備する、第4準備ステップと、
前記輸液容器に、当該輸液容器から輸液を排出する輸液ラインを接続する、第5準備ステップと、
前記薬液ラインと輸液ラインとを三股状に接続することで、前記輸液及び薬液の少なくとも一方を排出可能な供給ラインを形成するステップと、
前記薬液排出ラインに、前記薬液容器側から流れる薬液は通過するが、空気が通過しないフィルターを取付けるステップと、をさらに備え、
前記排出ステップに先立って、
前記輸液容器から、前記供給ラインに向かって輸液を排出する第1プライミングステップ、をさらに備えている、請求項21に記載の薬液供給方法。
