Description
Oppfinnelsens bakgrunn
Søknaden angår preparat for antisense-behandling av cancer som gjør bruk av et antisense-oligonukleotid som binder til "testosteron-repressed prostate message-2"
(TRPM-2).
Prostatacancer er den mest vanlige cancer som påvirker menn, og den nest hyppigste årsak til cancerdødsfall hos menn i den vestlige verden. På grunn av at prostatacancer er en androgensensitiv tumor, utnyttes fjerning av androgen, for eksempel via kastrering, ved noen terapeutiske regimer for pasienter med langtkommet prostatacancer. Fjerning av androgen fører til uttalt apoptose i prostatatumoren, og følgelig en regresjon av sykdommen. Imidlertid er den kastreringsinduserte apoptose ikke fullstendig, og en progresjon av overlevende tumorceller til androgen uavhengighet oppstår til slutt. Denne progresjon er hovedhindringen for forbedring av overlevelse og livskvalitet, og forsøk er derfor blitt utført med hensyn på målsøking av androgen uavhengige celler. Disse forsøk har fokusert på ikke-hormonelle behandlingsformer målrettet mot androgen uavhengige tumorceller (Yagoda et al., Cancer 71 (supp. 3): 1098-1109
(1993); Oh et al, J. Uroi. 60-1220-1229 (1998), så langt har imidlertid ingen ikke-hormonelle midler forbedret overlevelsen. Alternative fremgangsmåter antydes derfor.
Det er observert at flere proteiner uttrykkes i økte mengder av prostatatumorceller etter fjerning av androgen. I alle fall antas noen proteiner å være assosiert med den observerte apoptopiske celledød som observeres ved fjerning av androgen (Raffo et al., Cancer Res. : 4448-4445 (1995):Krajewska et al., Am. J. Pathol. 148: 1567-1576
(1996);McDonnell et al, Cancer Res. 52: 6940-6944 (1992)). Funksjonene til mange av proteinene er imidlertid ikke lydelig forstått. TRPM-2 (også kjent som sulfatert glykoprotein-2 (SGP-2) eller clusterin) finnes i denne sistnevnte kategori.
TRPM-2 er et allestedsnærværende protein, med et variert spekter av foreslåtte aktiviteter. I prostataepitelceller øker ekspresjon av TRPM-2 umiddelbart etter kastrering, og når et toppnivå i rotteprostataceller 3 til 4 dager etter kastrering, sammenfallende med starten på en massiv celledød. Disse resultater har ledet noen forskere til den konklusjon at TRPM-2 er en markør for celledød, og en promoter for apoptose. Observasjonen at certoli-celler og noen epitelceller uttrykker høye nivåer av TRPM-2 uten økte nivåer av celledød reiser på den annen side spørsmål om denne konklusjon er korrekt. Sensibar et al., Cancer Research 55: 2431-2437 (1995) rapporterte om in vitro eksperimenter utført i den hensikt å tydelig utlede rollen til TRPM-2 ved prostatacelledød. De anvender LNCaP-celler transfektert med et gen som koder for TRPM-2 og observerer om ekspresjonen av dette protein endrer effektene av tumornekrose faktor a (TNFa), for hvilken LNCaP-celler er svært sensitive, med celledød normalt inntreffende innen 12 timer. Behandling av transfekterte LNCaP-celler med TNFa ble vist å resultere i en forbigående økning i TRPM-2-nivåer for en periode på noen få timer, men disse nivåer var forsvunnet ved det tidspunkt DNA-fragmentering forut for celledød ble observert. Ved anvendelse av et antisense-molekyl tilsvarende basene 1-21 til TRPM-2-sekvensen, men ingen andre TRPM-2 -sekvensen, men ingen andre TRPM-2-antisenseoligonukleotider, resulterte i en betydelig reduksjon i ekspresjon av TRPM-2, og en økning i apoptopisk celledød i LNCaP-celler eksponert for TNFa. Dette førte Sensibar et al. til hypotesen at overekspresjon av TRPM-2 kunne beskytte celler fra cytotoksiske effekter av TNF, og at fjerning av TRPM-2 er ansvarlig for start av celledød, selv om virkningsmekanismene fortsatt er uklare.
Mens Sensibar et al. frembringer informasjon om den mulige rolle til TRPM-2, beskrives ikke desto mindre resultater fra bare et modellsystem der ekspresjon av TRPM-2 baseres på et transfektert gen. Videre er ekspresjonsnivået av TRPM-2 svært lavt eller fraværende i LNCaP-celler dyrket ved andre laboratorier. Den situasjon som oppstår in vivo når prostatatumorceller utsettes for fjerning av androgen er mye mer kompleks, med mange proteiner som endrer ekspresjonsnivåer som et resultat. Følgelig er det ikke mulig fra Sensibar et al. data å forutsi om TRPM-2 ville utøve den samme funksjon når den er til stede i kombinasjon med andre proteiner, eller om endringer i nået av TRPM-2 etter fjerning av androgen in vivo kunne frembringe noen terapeutiske fordeler. Det faktum at TRPM-2 uttrykkes i vesentlige mengder i prostatatumorceller ved ulike stadier etter androgenfjerning, inkludert de stadier der betydelig apoptopisk celledød forekommer antyder faktisk at rollen til TRPM-2 in vivo kan være mye mer komplisert. Mens teknikken frembringer data angående visse aspekter av apoptopisk celledød i prostatatumorceller gir den allikevel ikke noen kunnskap eller antakelser om en metodologi som frembringer en forsinkelse i starten på androgenuavhengigheten.
Følgelig er det viktig for foreliggende oppfinnelse å frembringe en slik fremgangsmåte.
Et aspekt ifølge oppfinnelsen å frembringe terapeutiske antisense-molekyler for å forsinke starten på androgenuavhengighet i prostatatumorceller.
Det er også mulig å frembringe en fremgangsmåte for å øke kjemisensitiviteten eller strålingssensitiviteten til cancerceller fra en cancer som uttrykker TRPM-2.
Det er ytterligere et aspekt av oppfinnelsen å frembringe terapeutiske antisense-molekyler for å inhibere ekspresjon av TRPM-2.
Foreliggende oppfinnelse omfatter preparat for å forsinke progresjon av prostatatumorceller til en androgen-uavhengig tilstand, kjennetegnet ved at det omfatter et antisense-oligonukleotid som inhiberer ekspresjon av TRPM-2 fra tumorcellene, som medfører at når prostatatumorcellene behandles med preparatet forsinkes progresjonen til androgen-uavhengighet.
Omfattet av oppfinnelsen er også anvendelse av et preparat ifølge ethvert av kravene 1 til 6 ved formulering av et farmasøytisk preparat for å forsinke progresjon av prostatatumorceller til en androgen uavhengig tilstand.
Videre omfatter oppfinnelsen oligonukleotid kjennetegnet ved at det har sekvensen vist i SEQ ID nr. 4 eller SEQ ID nr. 12.
Til slutt omfatter oppfinnelsen farmasøytisk preparat kjennetegnet ved at det omfatter et antisense-oligonukleotid som er komplementært til TRPM-2-mRNA og som omfatter en sekvens vist i enhver av sekvensene SEQ ID nr. 4, 5 og 12 og en farmasøytisk akseptabel bærer for å tilby oligonukleotidet til et pattedyrindivid for å redusere ekspresjon av TRPM-2.
Buttyan et al. Molecular and Cellular Biology Vol 9, No 8, 3474-3481 (1989) rapporterte at RNA og protein produkter kodet av TRPM-2 genet, ble indusert til høye nivåer og var koordinert med starten på celle død, i tallrike gnagermodeller med induserbar vevsødeleggelse. Buttyan et al, innrømmet imidlertid at funksjonen(e) til produktene kodet for av TRPM-2 var uklar(e). Buttyan et al. nevnte ikke de terapeutiske fordelene av å inhibere TRPM-2 ekspresjon, heller ikke foreslo de anvendelsen av terapeutiske midler som besto av antisense oligonukleotider mot TRPM-2 for å behandle cancer slik det fremgår av foreliggende oppfinnelse.
Milner et al. Nature Biotechnology 15: 537-541 (1997) rapporterte om en fremgangsmåte for å måle potensialet til oligonukleotider for å danne heteroduplex med kanin p-globin mRNA på en kombinatorisk oligonukleotid "array". De viste imidlertid ikke identifiseringen av en antisense oligonukleotid som målsøker TRPM-2 eller målsøking av TRPM-2 med en antisense oligonukleotid.
Oppsummering av oppfinnelsen
Ifølge foreliggende oppfinnelse er det nå vist at antisense-terapi som reduserer ekspresjon av TRPM-2 frembringer terapeutiske fordeler ved behandling av cancer. I særdeleshet kan slik antisense-terapi anvendes ved behandling av prostatacancer og nyrecellecancer.
Tilsetting av antisense-TRPM-2-oligodeoksynukleotid (ODN) til prostatatumorceller in vivo forsinker effektivt starten av androgen uavhengighet. I et aspekt frembringer følgelig foreliggende oppfinnelse et preparat for å behandle prostatacancer i et individ som lider av prostatacancer, omfattende trinnene av å initiere androgenfjernelse for å indusere apoptopisk celledød av prostatatumorceller i individet, og administrering til individet av et preparat som er effektivt til å inhibere ekspresjon av TRPM-2 fra tumorcellene, dermed forsinkes progresjonen av prostatatumorceller til en androgen uavhengig tilstand i et individ. Videre øker kombinert anvendelse av antisense-TRPM-2 pluss cytotoksisk kjemoterapi (for eksempel taxaner) synergistisk kjemosensitivitet i hormonmotstandsdyktige prostatacancere. I et annet aspekt ifølge oppfinnelsen administreres en annen antisense-ODN som inhiberer ekspresjon av et anti-apoptopisk protein forskjellig fra TRPM-2 samtidig med antisense-TRPM-2-ODN.
Det er også funnet at antisense-TRPM-2 har gunstige effekter for andre cancertyper. Spesielt øker antisense-TRPM-2-ODN kjemosensitiviteten i humane nyrecellecancere, en normalt kjemoresistent sykdom der ingen aktive kjemoterapeutiske midler har en objektiv responsrate høyere enn 10 %. Strålesensitiviteten er også øket når celler som uttrykker TRPM-2 behandles med anstisens-TRPM-2-ODN. Følgelig kan antisense-TRPM-2-ODN anvendes for å behandle flere cancertyper der ekspresjon av TRPM-2 er observert.

Detaljert beskrivelse av oppfinnelsen
Foreliggende oppfinnelse angår antisense-TRPM-2-ODN og anvendelse av disse preparater for behandling av cancer. Oppfinnelsen kan anvendes ved behandling av cancere der cancerceller uttrykker TRPM-2. Tre viktige klasser av cancerceller som uttrykker TRPM-2 er prostatacancerceller, humane nyrecancerceller (RCC) og noen brystcancerceller.
En fremgangsmåte for å øke kastreringsindusert tumorcelledød og forsinkelse av progresjon av prostatatumorceller til androgen uavhengighet; en terapeutisk fremgangsmåte for behandling av individer, omfattende mennesker, som lider av prostatacancer; og terapeutiske midler effektive for anvendelse ved slike metoder blir beskrevet. Den terapeutiske fremgangsmåte vil oftest anvendes ved behandling av individer med langtkommet prostatacancer.
Økning av kastreringsindusert tumorcelledød og forsinket progresjon av androgensensitive prostatacancerceller til androgen uavhengighet oppnås ved å inhibere ekspresjonen av TRPM-2 i cellene. Eksperimenter ble utført i tre modellsystemer, in vivo Shionogi-tumormodellen, den humane TRPM-2-transfekterte LNCaP-modellen og den humane PC-3-modellen, som til sammen viser at slik inhibisjon som leder til forsinket androgen uavhengighet kan oppnås ved å behandle andogen-sensitive prostatatumorceller med antisense-oligodeoksynukleotider (ODN).
I det første eksperimentet ble evnen til et muse-TRPM-2-antisensemolekyl (SEQ ID nr. 1) til å forsinke starten av androgen uavhengighet i Shionogi-tumormodellen undersøkt. Shionogi-tumormodellen er en xenotransplantasjon av en androgen-avhengig muse-brystkarsinom som vokser subkutant i syngeneiske hannverter. Shionogi-tumorceller er svært tumorgene og lokalt invasive. Cellene er vist å respondere på androgen fjernelse på en måte som etterlikner den observerte adferd til prostatatumorceller, og aksepteres som en god modell for prostatacancer hos menneske. (Bruchovsky et al., Cancer Res.
50: 2275-2282 (1990); Rennie et al, Cancer Res. 48: 6309-6312 (1988); Bruchovsky et al., Cell 13: 272-280 (1978); Gleave et al., i genitourinary Oncology, s 367-378, Lange et al. red. Lippencott (1997); Gleave et al, J. Uroi. 157: 1727-1730 (1997); Bruchovsky et al., The Prostate 6: 13-21 (1996)). Følgelig fremskynder androgenfjernelse apoptose og tumorregresjon på en svært reproduserbar måte. Videre er endringene i ekspresjon av TRPM-2 og Bcl-2 i human prostatacancer etter kastrering og under progresjon til androgen uavhengighet tilsvarende de som observeres i Shionogi-tumorceller. Følgelig
etterlikner Shionogi-tumormodellen mange egenskaper til prostatacancerceller. Videre frembringer Shionogi-tumormodellen en svært nyttig modell for evaluering av den evne forbindelser har til å forsinke starten av androgen uavhengighet. På tross av fullstendig tumorregresjon etter kastrering, oppstår stadig rasktvoksende androgenuavhengig Shionogi-tumorer etter en måned, hvilket gir et pålitelig endepunkt for å evaluere midler som kan forsinke progresjonen mot androgen uavhengighet. Hendelser som oppstår i Shionogi-tumormodellen innen en måned oppstår vanligvis i humane pasienter innen ca.
2 år.
Antisense-ODN som inhiberer ekspresjon av TRPM-2 sin evne til å forsinke starten av androgenuavhengighet ble evaluert ved å måle tumorvolum etter kastrering i Shionogi-tumormodellen. Forsøksdyr (n=7) ble behandlet intraperitonealt en gang daglig med 12,5 mg/kg gjentagende doser av antisense-TRPM-2-ODN (SEQ ID nr. 1) i en bufret saltløsning. Som kontroll ble dyr (n=7) behandlet med en feiltilpasset ODN (SEQ ID nr. 2). Som vist i fig. 1 viste både test- og kontrollgruppene den forventede nedgang i tumorvolum umiddelbart etter kastrering, men tumorene hos antisense-TRPM-2-ODN-behandlede mus gikk raskere tilbake enn kontrollene. Kontrollgruppen viste også den forventede økning i tumorvolum som er assosiert med utvikling av androgenuavhengighet. I motsetning ved 49 dager etter kastrering, forekom liten tumorgjenvekst hos mus behandlet med antisense-TRPM-2-ODN. Tumorer gjenoppstod etter hvert i de antisense-TRPM-2-ODN-behandlede mus, men mediantiden for tilbakefall var omtrent dobbelt så lang som for kontrollgruppen. Følgelig er inhibisjon av TRPM-2 effektivt ikke bare for å øke mengden celledød som oppstår umiddelbart etter androgenfjernelse, men også for å forsinke starten på androgenuavhengighet. Den raskere nedgang i tumorvolum hos mus behandlet med antisense-TRPM-2-ODN skyldtes den tidligere start og mer uttalt kastreringsindusert apoptose. Dette ble bekreftet ved å påvise poly(ADP-ribose)-polymerase (PARP)-spaltingsprodukter i Shionogi-tumorprøver.
For å evaluere hvilke humane antisense-ODN komplementære med TRPM-2-mRNA-sekvenser som er mest effektive i denne hensikt, ble en serie på ti antisense-fosfortioat-ODN fremstilt spredt over ulike mRNA-områder som vist i fig. 2. Sekvensene til disse ti ODN er vist i de vedlagte sekvenslister som SEQ ID No 3-12. De ti humane antisense-ODN ble evaluert ved anvendelse av TRPM-2-transfekterte LNCaP-celler og humane prostatacancer-PC-3-celler for deres evne til å inhibere ekspresjon av TRPM-2 - mRNA. Som vist i fig. 3 frembrakte de testede antisense-ODN ulike nivåer av inhibisjon av TRPM-2-mRNA-ekspresjon, med det beste resultat oppnådd med SEQ ID nr. 4, 5 og 12. SEQ ID Nr. 5 tilsvarte sekvensen anvendt av Sensibar et al. som frembrakte inhibisjon av TRPM-2-ekspresjon i LNCaP-celler og er komplementær til de første 21 basene av TRPM-2-mRNA. Den mest effektive nedregulering forekom med SEQ DD No 4. Felles for alle de effektive sekvensene er en overlapp med enten initierings- eller termmineringssetene til TRPM-2-mRNA. Generelt kan følgelig fremgangsmåten ifølge oppfinnelsen utøves med antisense-oligonukleotider som er komplementære til et område av TRPM-2-mRNA som spenner enten fra translasjonsinitieringssetet eller termineringssetet.
Terapeutisk behandling av individer kan oppnås, inkludert humane individer, som lider av prostatacancer ved å initiere androgen-fjernelse for å indusere apoptopisk celledød av prostatatumorceller hos individet og administrering til individet av et preparat som effektivt inhiberer ekspresjon av TRPM-2 i tumorceller, dermed forsinkes progresjonen av prostataceller til en androgenuavhengig tilstand i individet.
Initiering av androgen fjernelse kan utføres via kirurgi (fjerning av begge testikler) eller medisinsk (medikamentindusert suppresjon av testosteron) kastrering, som for tiden anvises ved behandling av prostatacancer. Medisinsk kastrering kan oppnås ved ulike regimer, inkludert LHRH-midler eller antiandrogener. (Gleave et al., CMAJ160: 225-232 (1999)). Periodevis behandling der reversibel androgenfjernelse utøves er beskrevet i Gleave et al. Eur. Uroi. (supp. 3): 37-41 (1998).
Inhibisjon av TRPM-2-ekspresjon kan være forbigående, og bør ideelt opptre samtidig med androgenfjernelse. Hos menneske betyr dette at inhibering av ekspresjon bør være effektivt startende en dag eller to etter androgenfjernelse og med varighet ca. 3 til 6 måneder. Flere doser kan være nødvendig for å oppnå dette. Imidlertid vil det forstås at tidsperioden kan forlenges, ved å starte før kastrering og vare en god stund etter uten å avvike fra rammen av oppfinnelsen.
Antisense-TRPM-2-ODN er også blitt vist å øke kjemosensitiviteten i humane nyrecellecancere (RCC). RCC er en kjemoresistent sykdom der ingen aktive kjemoterapeutiske midler oppnår en høyere responsrate enn 10 %. Økt TRPM-2-ekspresjon i renale proksimale nyretubuli ("Proximal convoluted cells") som gjennomgår apoptose er observert etter ulike stimuli inkludert obstruksjon av ureter og aminoglykosider. Imidlertid er den funksjonelle betydelighet av TRPM-2-ekspresjon i RCC ikke vel dokumentert. Disse resultater viser imidlertid at antisense-TRPM-2-ODN øker kjemosensitiviteten i humane RCC CaKi-2-celler (se eksempel 6, in/ ra). Antisense-TRPM-2-ODN ble også runnet å øke sensitiviteten overfor stråling (se eksempel 7 og fig. 8).
Inhibering av ekspresjon av TRPM-2 kan utføres ved administrering av antisense-ODN, spesielt antisense-ODN som er komplementær til et område av TRPM-2-mRNA som spenner over enten translasjonsinitieringssetet eller termineringssetet. For behandling av prostatacancer hos menneske er særskilt nyttige sekvenser de som vises i SEQ ID nr. 4, 5 og 12.
De anvendte ODN kan modifiseres for å øke stabiliteten av ODN in vivo. For eksempel kan ODN anvendes som fosfortiolatderivater (erstatning av en ikke-brobyggende) forsforyloksygenatom med et svovelatom) som har økt resistens mot nukleasespalting. MOE-modifisering (ISIS)-skjelett) er også effektivt.
Administrering av antisense-ODN kan utføres ved anvendelse av ulike mekanismer kjent innen teknikken, en naken administrering eller administrering i farmasøytisk akseptable lipidbærere. Lipidbærere for antisense avlevering er for eksempel beskrevet i US patent nr. 5 855 911 og 5417 978. Antisense administreres vanligvis intravenøst, intraperitonealt, subkutant eller oralt, eller ved direkte lokal injeksjon i tumor. Ut i fra eksperimentene utført ved anvendelse av Shionogi-musemodellen, synes det som om antisense-ODN fortrinnsvis er aktiv i tumorcellene. TRPM-2-ekspresjon i ikke-tumorvev var virkelig hovedsakelig uaffisert, og ingen bivirkninger av antisense-ODN-administrering ble observert.
Mengden antisense-ODN som administrerers er en mengde som effektivt inhiberer ekspresjonen av TRPM-2 i prostataceller. Det vil forstås at denne mengden vil variere både med effektiviteten av den anvendte antisense-ODN, og med egenskapene til enhver bærer som anvendes. Bestemmelse av passende mengder av et hvilket som helst gitt preparat er innenfor teknikken, via standardserier av tester utformet for å vurdere tilfredsstillende terapeutiske nivåer.
Fremgangsmåten for behandling av prostatacancer kan videre inkludere administrering av kjemoterapeutiske midler og/eller ytterligere antisense-ODN rettet mot ulike mål. Det er for eksempel funnet ved anvendelse av Shionogi-tumormodellen at anatisense-TRPM-2-ODN øker sensitiviteten overfor konvensjonelle kjemoterapeutiske midler slik som taksaner (paclitaxel eller docetaxel) og mitoxantron (fig. 12A og 12B). Som vist i fig. 12A og 12B resulterte behandling med antisense-TRPM-2-ODN i nærvær av taksol eller mitoxantron i et redusert tumorvolum sammenliknet med kombinasjon av taksol eller mitoxantron med den feiltilpassede (MM) ODN. Andre midler som sannsynligvis viser synergistisk aktivitet omfatter andre cytotoksiske midler (for eksempel syklofosfamid, topoisomeraseinhibitorer), angiogeneseinhibitorer, differenseringsstoffer og signaltranduksjonsinhibitorer. Kombinasjoner av TRPM-2-antisense med andre antisenseprøver slik som antisense-Bcl-2-ODN virket på tilsvarende måte bedre til å drepe Shionogi-celler in vitro enn en av ODN alene. Følgelig kan TRPM-2 virke sammen med andre antisense-molekyler, slik at antisense-Bcl-2-, Bcl-xl- og c-myc-ODN frembringer større effektivitet.
Oppfinnelsen vil nå bli ytterligere beskrevet med henvisning til de følgende ikke-begrensende eksempler.
EKSEMPEL 1
Shionogi-tumormodelleksperimenter ble utført ved anvendelse av celler fra Toronto-sublinjen av transplanterbare SC-115-AD-musebrystkarsinom. For in vivo studier ble omtrent 5 x IO6 celler Shionogi-karsinom injisert subkutant i voksne hannmus av DD/S-stammen. Når Shionogi-tumorene ble 1 til 2 cm i diameter, vanligvis 2 til 3 uker etter injeksjon, ble kastrering utført gjennom et abdominalsnitt under metoksyflurananestesi. Detaljer vedrørende behandling av musene, tumorstammen og operasjonsprosedyrene er tidligere beskrevet. Mus ble tilfeldig utvalgt for behandling med murine fosfortioat-antisense-TRPM-2-ODN (SEQ ID nr. 1) eller en feiltilpasset kontroll (SEQ ID nr. 2) som er to baser forskjellig i sekvensen fra antisense-TRPM-2-ODN. Hver eksperimentgruppe bestod av 7 mus. En dag etter kastrering ble 12,5 mg/kg antisense-TRPM-2- eller feiltilpasset kontroll-ODN oppløst i fosfatbufret saltvann injisert intraperitonealt en gang daglig til hver mus i 40 dager. Tumorvolum ble målt to ganger ukentlig, og beregnet etter formelen lengde x bredde x dybde x 0,5236. Gleave et al., Cancer Res. 52: 1598-1605
(1992). Datapunktene ble rapportert som gjennomsnittlig tumorvolum ± standard avvik.
Resultatene fra denne studie vises i figur 1. Som vist gikk Shionogi-tumorer tilbake raskere og fullstendig regresjon opptrådte tidligere i mus behandlet med antisense-TRPM-2-ODN. Videre forsinket behandling med antisense-TRPM-2-ODN tydelig starten på androgen-uavhengighet som gjenspeiles ved økningen i tumorvolum etter 21 dager hos kontrollmusene. Ingen bivirkninger forbundet med antisense-TRPM-2 eller den feiltilpassede kontroll ble observert.
For å undersøke effektene av in vivo ODN behandling på nivåene av TRPM-2-mRNA, ble Northern blot analyser utført på Shionogi-tumorvev fra mus. Musene ble behandlet daglig med 12,5 mg/kg av antisense-TRPM-2-ODN (n=6) eller den feiltilpassede kontroll (n=6) ved intraperitoneal injeksjon som startet en dag etter kastrering. På den fjerde dagen etter kastrering ble tumorvev høstet og analysert ved Northern blot for TRPM-2-mRNA. Antisense-TRPM-2-ODN resulterte i en 75 % reduksjon i TRPM-2-mRNA-nivåene i Shionogi-tumorer sammenliknet med tumorene behandlet med den feiltilpassede kontroll-ODN (fig. 3).
Sammenliknbare analyser ble utført på normale museorgan. Prøver fira milt, nyre, prostata og hjerne ble høstet fra Shionogi-tumormus behandlet med antisense-TRPM-2-ODN og feiltilpasset kontroll under det samme behandlingsregime, og analysert ved Northern blot. Selv om TRPM-2-mRNA-nivåene var betydelig lavere i tumorvev, hadde antisense-TRPM-2-ODN ingen effekt på TRPM-2-mRNA-nivåer i de normale organ.
EKSEMPEL 2
Sekvensselektiviteten til antisense-TRPM-2-ODN (SEQ ID nr. 1) ble bekreftet ved å sammenlikne ekspresjonsnivåene av TRPM-2-mRNA i Shionogi-celler opprettholdt in vitro, etter behandling med ulike nivåer av antisense-TRPM-2-ODN eller en feiltilpasset kontroll (SEQ ID nr. 2). For å lettet opptaket av ODN i cellene ble ODN formulert i en kationisk lipidbærer ("Lipofectin", (Life Technologies, Inc.)). Cellene ble behandlet to ganger over en periode på to dager ved den følgende protokoll. Celler ble preinkubert i 20 minutter med 4 ug/ml lipofectin i serumfritt "OPTI-MEM" (Life Technologies, Inc.) og deretter innkubert med medium inneholdende den utvalgte konsentrasjon av ODN og lipofectin i fire timer. Mediet ble deretter erstattet med standard kulturmedium.
Mengden TRPM-2-mRNA i cellene ble evaluert ved hjelp av Northern blot analyser. Som vist i fig. 4 reduserte behandling av Shionogi-celler med antisens-TRPM-2-ODN TRPM-2-mRNA-nivåene på en doseavhengig måte. I motsetning ble TRPM-2-mRNA- nivåene ikke påvirket av den feiltilpassede ODN (SEQ ID nr. 2) ved noen av de anvendte konsentrasjoner. Følgelig er effekten av antisense-TRPM-2-ODN tilsynelatende sekvensspesifikk.
EKSEMPEL 3
Shionogi-celler opprettholdt in vitro ble behandlet med ulike mengder taksol alene eller i kombinasjon med 500 nM antisense-TRPM-2-ODN (SEQ ID nr. 1) eller den feiltilpassede kontroll. (SEQ ID nr. 2). Cellene ble behandlet to ganger, som beskrevet i eksempel 2, og prosentandelen av gjenværende levedyktige celler ble bestemt. Resultatene er oppsummert i fig. 5. Som vist endret inkludering av antisense-TRPM-2-ODN doseresponskurven mot venstre, ved å redusere IC50med en faktor fra 5 til 10. Tilsvarende resultater ble oppnådd ved anvendelse av mitoxantron i stedet for paclitaxel (fig. 12A og 12B).
EKSEMPEL 4
Eksperimentet fira eksempel 3 ble gjentatt, med tilsetting av antisense-Bcl-2-ODN (SEQ ID nr. 13) eller en feiltilpasset Bcl-2-ODN (SEQ ID nr. 14) i ulike kombinasjoner med antisense/feiltilpasset TRPM-2-ODN og taksol. Resultatene vises i fig. 6. Kombinasjonen av antisense-TRPM-2-ODN med antisense-Bcl-2-ODN og taksol økte ytterligere de cytotoksiske effekter av taksol. Følgelig synes målrettingen av ytterligere anti-apoptopiske midler å frembringe terapeutiske fordeler.
EKSEMPEL 5
For å identifisere tilfredsstillende antisense-TRPM-2-ODN-sekvenser for anvendelse i human terapi, ble antisense-ODN-sekvenser rettet mot 10 ulike seter av det humane TRPM-2-genet (fig. 2, SEQ ID nr. 3-12) syntetisert og testet for deres evne til å redusere TRPM-2-genekspresjon i human prostatacancer PC-3-celler og transfekterte LNCaP-celler som overuttrykker TRPM-2 ved anvendelse av den samme behandlingsprotokoll som beskrevet i eksempel 2. Resultatene er oppsummert i fig. 3. Som vist er sekvensene 4, 5 og 12 aktive i å redusere TRPM-2-ekspresjon. Disse tre sekvenser overlapper og er umiddelbart naboer til translasjonsinitierings- eller termineringssetene.
EKSEMPEL 6
Immunhistokjemisk farging ble anvendt for å karakterisere clusterinekspresjon i 17 RCC og normale nyrevev skaffet fra radikale nefrektomiprøver. TRPM-2-ekspresjon i humane nyrecancercellelinjer ACHN, CaKi-1 og CaKi-2 ble evaluert ved Northern og Western blot analyser. Northern blot analyser ble anvendt for å vurdere endringer i TRPM-2-mRNA-ekspresjon etter antisense-TRPM-2-ODN-behandling. Effektene av kombinert antisense-TRPM-2-ODN og taksolbehandling på CaKi-2-cellevekst ble undersøkt i en MTT-analyse.
Immunfarging viste en økt clusterinekspresjon ill RCC-prøver sammenliknet med det tilstøtende normale nyrevev. I de øvrige 6 tilfeller ble ingen forskjell observert mellom malignt og normalt vev. Både TRPM-2-mRNA- og proteinekspresjon var påvisbart i alle tre humane RCC-cellelinjer, med høyest nivå for CaKi-2. Antisense-TRPM-2-ODN (SEQ ID nr. 1), men ikke den feiltilpassede kontroll-ODN (SEQ ID nr. 2) inhiberte TRPM-2-ekspresjon i CaKi-2-celler på en doseavhengig og sekvensspesifikk måte (fig. 7A). Videre økte antisense-TRPM-2-ODN tydelig taksolkjemosensitivitet, ved å redusere IC50 for taksol med 1 log (500 nM til 50nM) sammenliknet med den feiltilpassede kontroll ODN (fig. 7B). Disse data viser at TRPM-2 og dens protein clusterin uttrykkes ved høyere nivåer i RCC sammenliknet med normalt nyrevev, og at antisense-TRPM-2-ODN kan være nyttig ved å øke de cytotoksiske effekter ved konvensjonell kjemoterapi ved fremskreden RCC.
EKSEMPEL 7
Antisense-TRPM-2-ODN øker strålesensitiviteten til cancerceller som uttrykker TRPM-2. Ved anvendelse av Northern-analyser fant vi at stråleterapi resulterer i dose- og tidsavhengig økning i TRPM-2-genekspresjon i humane prostatacancer PC-3-celler (fig. 8). Overekspresjon av TRPM-2 resulterer i økt resistens mot stråleindusert celledød. Humane prostata LNCaP-celler som overuttrykker TRPM-2 (LNCaP/Tl) er mer resistente mot stråleterapi (fig. 9A og B). Behandling av humane prostatacancer PC-3-celler med 100 og 500 nM antisense-TRPM-2-ODN (SEQ ID nr. 1) reduserer betydelig celleoverlevelse etter en enkelt behandling med 4 Gy stråleterapi sammenliknet med feiltilpasset ODN (SEQ ID nr. 2) -behandling. (Fig. 10). Fig. 1 IA og B viser doseavhengig strålesensibilisering av humane prostatacancer PC-3-celler etter behandling med 10, 50 og 100 nM antisense-TRPM-2-oligo in vitro.
EKSEMPEL 8
For å avgjøre om behandling med human antisense-TRPM-2-ODN øker

Claims (15)
1.
Preparat for å forsinke progresjon av prostatatumorceller til en androgen-uavhengig tilstand,karakterisert vedat det omfatter et antisense-oligonukleotid som inhiberer ekspresjon av TRPM-2 fra tumorcellene, som medfører at når prostatatumorcellene behandles med preparatet forsinkes progresjonen til androgen-uavhengighet.
2.
Preparat for å øke kjemo- eller strålesensitiviteten til cancerceller hos et individ som lider av en cancer som uttrykker TRPM-2 i mengder forskjellig fra normalt vev av samme type,karakterisert vedat det omfatter et antisense-oligonukleotid som effektivt inhiberer ekspresjon av TRPM-2 fra cancercellen.
3.
Preparat ifølge krav 1 eller 2,karakterisert vedat antisense-oligonukleotidet er komplementært til et område av TRPM-2-mRNA som inkluderer translasjonsinitierings- eller termineringssetet.
4.
Preparat ifølge krav 3,karakterisert vedat antisense-oligonukleotidet har sekvensen gitt ved SEQ ID nr. 4, SEQ ID nr. 5 eller SEQ ID nr. 12.

Preparat ifølge ethvert av kravene 1-4,karakterisertv e d at det ytterligere omfatter et annet antisense-oligodeoksynukleotid som inhiberer ekspresjon av et anti-apoptopisk protein forskjellig fra TRPM-2.

Preparat ifølge krav 5,karakterisert vedat det andre antisense-oligodeoksynukleotid er antisense-Bcl-2-oligodeoksynukleotid.
7.
Anvendelse av et preparat ifølge ethvert av kravene 1 til 6 ved formulering av et farmasøytisk preparat for å forsinke progresjon av prostatatumorceller til en androgen uavhengig tilstand.
8.
Oligonukleotidkarakterisert vedat det har sekvensen vist i SEQ ID nr. 4 eller SEQ ID nr. 12.
9.
Oligonukleotid ifølge krav 8,karakterisert vedat det har sekvensen vist i SEQ ID nr. 4.
10.
Oligonukleotid bestående av sekvensen fremsatt i et hvilket som helst av kravene 4, 8 eller 9,karakterisert vedat oligonukleotidet er modifisert for å øke dets stabilitet in vivo.
11.
Farmasøytisk preparatkarakterisert vedat det omfatter et antisense-oligonukleotid som er komplementært til TRPM-2-mRNA og som omfatter en sekvens vist i enhver av sekvensene SEQ ID nr. 4, 5 og 12 og en farmasøytisk akseptabel bærer for å tilby oligonukleotidet til et pattedyrindivid for å redusere ekspresjon av TRPM-2.

Farmasøytisk preparat ifølge krav 14,karakterisertv e d at det ytterligere oligonukleotid består av sekvensen beskrevet i SEQ ID nr. 13.
