DE 4138756 beskriver 5HT1A-reseptoragonister, aminometylkroman-derivater, som forsterker den nevrobeskyttende aktiviteten til 5-HT2-reseptor-antagonister så som ketanserin, ritanserin og andre 4-fluorfenylderivater. (Bode-Gruel, K., "Kombination mit Neuroprotektiver Wirkung", tysk patent nr. DE 4 138 756 5/27/93).
Samtidig administrering av ipsapiron, en forbindelse med 5-HT1A-agonsit-aktivitet og ketanserin, en forbindelse med 5-HT2-antagonistaktivitet, gir mer nevrobeskyttelse i en dyremodell for ischemi enn hvilket som helst middel alene.

Den nevrobeskyttende aktiviteten til en forbindelse kan tilskrives mer en ett aspekt av dens reseptoraktivitetsprofil. For eksempel er detfremsatt hypoteser som omfatter at for 5-HT1 A-agonisten Bay R 1531 er det ikke dets 5-HT1A-aktivitet, men dets lave bindingsaffiniteter for 5HT2, D2 og sigma-reseptorer som kan spille en viktig rolle i dets nevrobeskyttende effektivitet. Bode-Grueul, ovenfor.
Omtrent 5 - 6 millioner amerikanere er rammet av kronisk eller akutte nevrodegenerative sykdommer. Det er følgelig et behov for en effektiv forbindelse for å behandle og forebygge nevrodegenerative tilstander. Foreliggende oppfinnelse
tilveiebringer et nyttig middel for behandling og forebygging av nevrodegenerative forstyrrelser.
Beskrivelse av tegningene
Fig. 1 er en skjematisk presentasjon av effektene til adatanserin på azid-indusert ischemisk strøm av glutamat i hippocampalsnitt i rotter. Effekten av tre konsentrasjoner av adatanserin, 10 (nedover diagonalt) 100 (oppover diagonalt) og 1000 nM (skravert), ble sammenlignet med en kontroll (fylt). Ved konsentrasjoner på 100 og 1000 nM ble en statistisk signifikant reduksjon av glutamat-konsentrasjonen (henholdsvis 72 % og 71 %) observert.
Foreliggende oppfinnelse vedrører anvendelse av adatanserin eller et farmasøytisk salt derav for fremstilling av et medikament for behandling av akutte neurodegenerative forstyrrelser.
Det er følgelig N-[2-[4-(2-pyirmidinyl)-1 -
piperazinyl]etylltricyklo[3.3.3.13,7]dekan-1-karboksamid, eller adatanserin og farmasøytiske salter derav som anvendes.
Foreliggende oppfinnelse tilveiebringer videre anvendelse ifølge krav 1, idet den neurodegenerative forstyrrelse er slag. Nevnte slag er akutt tromboembolisk slag, fokal ischemi, global ischemi eller forbigående ischemisk angrep.
Egnede salter kan bli dannet fra farmasøytisk akseptable organiske og uorganiske syrer så som saltsyre, hydrobromsyre, surfonsyre, svovelsyre, fosforsyre, salpetersyre, maleinsyre, fumarsyre, benzosyre, askorbinsyre, pamoinsyre, ravsyre, metansulfonsyre, eddiksyre, propionsyre, vinsyre, sitronsyre, melkesyre, maleinsyre, mandelsyre, kanelsyre, palmitinsyre, itaconsyre og benzensurfonsyre.
Anatanserin og farmasøytiske salter derav kan også bli anvendt ved mediering eller inhibering av glutamataktivrteten assosiert med forstyrrelser kjent som kroniske, nevropatiske eller vedvarende smerte.
US-patent nr. 5 380 725 og US-patent nr 5 482 940 omfatter N-[2-I4-{2-pyrimidyl)-1-piperazinyl]etyl]tricyklo[3.3.3.13,7]dekan-1 -karboksamid, eller adatanserin, fremgangsmåte for fremstilling av disse og flere anvendelser derav. Anvendelsene beskrevet er for behandling av depresjon, angst, psykose.r så som paranoia og schizofreni, reduksjon av overvekt og reduksjon av omfattende etanol-konsum. Patentene er herved inkorporert som referanse.
Dysfunksjonen glutamatfirgjøring, og spesielt omfattende glutamatfirgjøring, er assosiert med patofysiologien til akutte og kroniske nevrodegenerative forstyrrelser. Adatanserin er blitt funnet å inhibere glutamatfrigjøringen og er følgelig nyttig for behandling og forebygging av akutte og kroniske nevrodegenerative forstyrrelser for å lindre eller eliminere symptomer. En terapeutisk effektiv mengde som anvendt heri, er en mengde som er tilstrekkelig for å tilveiebringe en grad av nevrobeskyttelse, eller for å behandle, inhibere eller lindre symptomer assosiert med nevrodegenerasjon, kronisk smerte eller omfattende eller dysfunk-sjonelt glutamatfrigjøring.
Kroniske nevrodegenerative forstyrrelser er f.eks. Alzheimers sykdom, Huntingtons sykdom, Parkinsons sykdom, epilepsi, amyotrofisk lateral sklerose, AIDS-demens og netthinnesykdom.
Akutte nevrodegenerative forstyrrelser innbefatter, men er ikke begrenset til, slag, hode- eller spinaltraume og asfyksi.
Slag innbefatter akutt tromboembolisk slag, fokal og global ischemi, forbigående cerebral ischemiangrep og andre cerebralvaskulære problemer ledsaget av cerebral ischemi.
Andre akutte nevrodegenerative tilstander er assosiert med hodetraume, spinaltraume, generell anoksi, hypoksi, inkludert føtal hypoksi, hypoglykemi, hypotensjon samt lignende skader som fremkommer i løpet av prosedyrer fra embole, hyperfusjon og hypoksi.
Kronisk, nevropatisk eller vedvarende smerte innbefatter fibromyalgi, post-herpetisk nevralgi, reflekssympatetisk dystrofi og diabetisk perifer nevropati.
Terapeutisk effektive mengder av adatanserin eller farmasøytiske salter derav kan bli administrert oralt eller parenteralt, alene eller i kombinasjon med konvensjonelle farmasøytiske bærere. Anvendbare faste bærere kan innbefatte én eller flere forbindelser som også kan virke som smaksmidler, smøremidler, oppløsningsmidler, suspenderingsmidler, fyllstoffer, glidemidler, kompresjons-hjelpemidler, bindemidler, tablettoppløsende midler eller innkapslingsmaterialer. I pulvere er bæreren et finfordelt faststoff som er blandet sammen med den fin-fordelte aktive ingrediens. I tabletter blir den aktive ingrediensen blandet med en bærer som har de nødvendige kompresjonsegenskapene i egnede andeler, og komprimert i form og størrelse som er ønskelig. Pulvere og tabletter kan inneholde opptil 99 % av den aktive ingrediensen. Egnede faste bærere innbefatter f.eks. kalsiumfosfat, magnesiumstearat, talk, sukker, laktose, dekstrin, stivelse, gelatin, cellulose, metylcellulose, natriumkarboksymetylcellulose, polyvinyl-pyrrolidin, lavtsmeltende voks og ionebytteharpikser. Flytende bærere kan bli anvendt for fremstilling av løsninger, suspensjoner, emulsjoner, siruper og eliksirer. Den aktive ingrediensen ifølge denne oppfinnelsen kan bli oppløst eller suspendert i en farmasøytisk akseptabel flytende bærer så som vann, et organisk løsningsmiddel, en blanding av begge, eller farmasøytiske akseptable oljer eller fett. Flytende bærere kan inneholde andre egnede farmasøytiske additiver så som oppløsningsmidler, emulgeringsmidler, buffere, konserveringsmidler, søtnings-midler, smaksstoffer, suspenderingsmidler, fortykningsmidler, farver, viskositets-regulerende midler, stabiliserende midler eller osmoregulerende midler. Passende eksempler på flytende bærere for oral eller parenteral administrering innbefatter vann (spesielt inneholdende additiver som ovenfor, f.eks. cellulosederivater, fortrinnsvis natriumkarboksymetylcelluloseløsning), alkoholer (inkludert mono-hydriske alkoholer og polyhydriske alkoholer, f.eks. glykoler) og derivater derav, og oljer (f.eks. separert kokosolje og arachisolje). For parenteral administrering kan bæreren også være en oljeholdig ester, så som etyloleat og isopropylmyristat. Sterile, vandige bærere blir anvendt i preparater med steril, flytende form for parenteral administrering. Flytende farmasøytiske preparater, som er sterile løsninger eller suspensjoner, kan bli anvendt ved f.eks. intramuskulær, intra-peritoneal eller subkutan injeksjon. Sterile løsninger kan også bli administrert intravenøst. Oral administrering kan være enten i flytende eller fast sammen-setningsform. Det er foretrukket at det farmasøytiske preparatet inneholdende foreliggende forbindelser er i enhetsdoseirngsform, f.eks. som tabletter eller kapsler. I en slik form blir sammensetningen delt inn i enhetsdoseringer inneholdende hensiktsmessige mengder av de aktive ingrediensene. Enhets-doseringsformene kan være pakkede sammensetninger, f.eks. pakkede pulvere, beholdere, ampuller, på forhånd fylte sprøyter, eller puter inneholdende væsker. Alternativt kan enhetsdoseringsformen være f.eks. en kapsel eller selve tabletten, eller den kan være det hensiktsmessige antallet av hvilke som helst slike sammensetninger i pakkeform. Den terapeutisk effektive doseringen som blir anvendt ved behandling av en spesifikk sykdom eller tilstand må subjektivt bestemmes av behandlende lege. Generelt kan en daglig dosering til mennesker på fra omtrent 100 mg til omtrent 1500 mg per dag bli administrert, fortrinnsvis mellom omtrent 300 mg og omtrent 1200 mg per dag, mer foretrukket mellom omtrent 500 mg og 1000 mg per dag. Variabler involvert i bestemmelsen av en hensiktsmessig terapeutisk dose innbefatter den spesifikke betingelsen(e) som blir behandlet og størrelse, alder og responsmønstret til pasienten.
Nytten av adatanserin og farmasøytiske salter derav, som midler for behandling og forebygging av nevrodegenerative forstyrrelser, er demonstrert i standard eksperimentelle prosedyrer.
Eksempel 1
Reduksjon av ischemisk strøm av glutamat i hippocampalsnittet hos
rotter
Adatanserin ble vurdert for dets evne til å redusere azid-indusert ischemisk strøm ("effluks") av glutamat i hippocampalsnittet hos rotter. Tre Krebs-buffere ble anvendt i eksperimentene. Normal Krebs-buffer besto av en løsning inneholdende som følger: (122 mM NaCI, 3 mM KCI, 24 mM NaCH03,10 mM glukose, 0,315 mM K2HPO4,1,2 mM MgS04, 4 mM CaCI2). Aglycemisk Krebs-buffer var lik normalbufferen, med unntagelse av at glukose ikke ble tilsatt. Ischemisk Krebs-buffer var den aglycemiske løsningen som inneholdt varierende konsentrasjoner av natriumazid (0 - 30 mM).
Hippocampi av rotter ble dissekert på en kald plattform og suspendert i iskald oksygenert normal Krebs-buffer. Vevet ble kryss-siiittet ved 350 u/n på en Mcllwain vevssnittsinnretning, deretter resuspendert og vasket tre ganger med iskald, normal Krebs-buffer. Omtrent 80 mg av vevet (en hel hippocampus) i 130 ml fluid ble tilsatt til en brønn i et Brandel superperfusjonsapparatur. Prøvene bie
perfusert med oksygenert normal Krebs-buffer og latt bli ekvilibrert i 30 minutter ved en strømningsrate på 0,4 ml/minutt. Tre 10-minutters fraksjoner ble samlet og deretter ble ischemi indusert med anvendelse av ischemisk Krebs-buffer (inneholdende natriumazid). Tre ytterligere 10-minutters fraksjoner ble deretter samlet.
For eksperimenter hvor testforbindelsen ble undersøkt, ble medikament tilsatt til en fraksjon før induksjon av ischemi, oppløst i aglycemisk Krebs-buffer. Amino-syrekonsentrasjoner ble analysert med reversfase HPLC på en katacholamin-kolonne (Keystone Scientific, 150 x 3 mm) ved anvendelse av en 0,05 M acetat/- metanolgradient. Alfa-aminoadipinsyre ble anvendt som en indre standard. Aminosyrer ble visualisert ved derivatisering med naftalen-2,3-dikarboksaldehyd og fiuorimetrisk deteksjon (Dawson, L. A. et al., "Improved Temporal Resolution of Microdialysis Measurement of Glutamate and Aspartate Using Capillary Electro-phoresis with Laser Induced Fluorescense Detection", J. Chromatogr. B 1997, 694,1204 -1212). Aminosyrekonsentrasjonene av de første tre likevektsfrak-sjonene ble tatt et gjennomsnitt av, og alle påfølgende verdier ble uttrykt som en prosentandel av dette gjennomsnittet ved anvendelse av arealet under kurven (AUC) for hvert individuelt eksperiment som ble dannet av trapezoid-metoden. Resultatene er presentert i fig. 1.
Som vist i fig. 1 reduserte adatanserin betydelig azidindusert ischemisk strøm av glutamat i hippocampalsnittet hos rotter. Konsentrasjoner på 100 nM og 1 |iM reduserte glutamatfrigjøringen med henholdsvis 72 % og 71 %.
Claims (6)
1. Anvendelse av adatanserin eller et farmasøytisk salt derav for fremstilling av et medikament for behandling av akutte neurodegenerative forstyrrelser.
2. Anvendelse ifølge krav 1, idet den neurodegenerative forstyrrelsen er slag.
3. Anvendelse ifølge krav 2, idet slaget er akutt tromboembolisk slag, fokal ischemi, global ischemi eller forbigående ischemisk angrep.
4. Anvendelse ifølge krav 1, idet den neurodegenerative forstyrrelsen er ischemi som er et resultat av en kirurgisk teknikk som involverer forlenget stans i blodstrømning til hjernen.
5. Anvendelse ifølge krav 1, idet den neurodegenerative forstyrrelsen er hodetraume, spinaltraume eller hypoksi.
6. Anvendelse ifølge krav 5, hvori hypoksi er føtal hypoksi.
