Foreliggende oppfinnelse vedrører antimikrobielt aktive blandinger, legemidler inneholdende disse samt anvendelse av blandingene for fremstilling av legemidler
Publikasjonene EP-A-571 870, DOS 43 02 155, JP 021 747 53 A2 og J. Antiboit.
(1991), 44 (5), 546-9 beskriver cyklopentan- og -penten-6-aminosyre. Slike B-aminosyreforbindelser virker antimikrobielt, særlig antimykotisk. De er imidlertid ikke fri for bivirkninger.
Ohki H., et al., "Synthesis and antifungal activity of FRI09615 analogs", J. Antibiot. 44(5):546-49 (1991) beskriver fremstilling og antimykotisk effekt av cyklopentan-P-aminosyrer og dipeptider av disse. Det angis blant annet at dipeptider av cyklopentan-P-aminosyrer viser potent aktivitet mot candida-soppinfeksjoner. Artikkelen viser at det er en nær sammenheng mellom aminosyrenes struktur og deres antimykotiske aktivitet.
Overraskende er det ved oppfinnelsen funnet antimikrobielt aktive blandinger, kjennetegnet ved at de inneholder a-aminosyrer og cyklopentan-6-aminosyrer fra gruppen bestående av "(S)-isoleucin med 2-amino-4-metylencyklopentan-l-karboksylsyre, (S)-alanin med 2-amino-4-metylencyklopenten-l-karboksylsyre, (S)-prolin med 1,2-cis-aminocyklo-pentan-l-karboksylsyre" eller isomere former derav og eventuelt en eller flere farmasøytisk akseptable bærestoffer.
Disse blandingene viser ikke, eller bare i ubetydelig grad, bivirkninger og det oppnås derved en forbedret godtagbarhet ved varmblodige dyr.
Blandingene ifølge oppfinnelsen er følgelig: blandinger av (S)-isoleucin med 2-amino-4-metylencyklopentan-l-karboksylsyre, (S)-alanin med 2-amino-4-metylen-cyklopentan-l-karboksylsyre eller (S)-prolin med 1,2-cis-aminocyklopentan-l-karboksylsyre.
Det molare blandingsforhold mellom a-aminosyrer og cyklopentan-8-aminosyre ligger i området fra 1:99 til 99:1, fortrinnsvis 1:10 til 10:1, særlig foretrukket 1:5 til 5:1 og helt spesielt foretrukket 1:3 til 3:1.
Blandingene ifølge oppfinnelsen blir vanligvis oppnådd ved blanding av de foretrukkede finpulveriserte enkeltkomponentene.
Blandingene ifølge oppfinnelsen kan bestå av i det vesentlige rene stereoisomerer eller stereoisomerblandinger.
De foranbeskrevne a-aminosyrene og cyklopentan-if-aminosyrene kan også foreligge i form av deres salter. Generelt skal det her nevnes salter med organiske eller uorganiske baser eller syrer så vel som indre salter.
Til syrene som kan bli tilsatt hører fortrinnsvis halogenhydrogensyre, som f.eks. hydrogenklorid og hydrogenbromid, særlig hydrogenklorid, videre fosforsyre, salpetersyre, svovelsyre, mono- og bifunksjonelle karboksylsyrer og hydroksykarboksylsyre, som f.eks. eddiksyre, maleinsyre, malonsyre, oksalsyre, glukonsyre, ravsyre, fumarsyre, vinsyre, sitronsyre, salicylsyre, sorbinsyre og melkesyre såvel som sulfonsyrer, som f.eks. p-toluensulfonsyre, 1,5-naftalindisulfonsyre eller kamfersulfonsyre.
Fysiologisk godtagbare salter kan likeledes være metall- eller ammoniumsalter av forbindelsene ifølge oppfinnelsen, som besitter en fri karboksylgrurjpe. Spesielt foretrukket er natrium-, kalium-, magnesium- eller kalsiumsalter, såvel som ammoniumsalter som er avledet av ammoniak eller organiske aminer som f.eks. etylamin, di- hhv. trietylamin, di- hhv. trietanolamin, dicykloheksylamin, dimetylaminoetanol, arginin, lysin, etylendiamin eller fenyletylamin.
Blandingene ifølge oppfinnelsen kan eksistere i stereoisomere former, f.eks. enten som bilde og speilbilde (enantiomere), eller slike som ikke forholder seg som bilde og speilbilde (diastereomere), hhv. som diastereomerblandinger eller som rene cis- eller trans-isomerer. Oppfinnelsen angår både antipoden, racemformen, diastereomerblandinger så vel som rene isomerer. De racemiske formene lar seg likeledes som diastereomerene adskille på kjent måte i stereoisomerisk enhetlige bestanddeler. Adskillelse i de stereoisomerisk enhetlige forbindelsene foregår f.eks. over en kromatografisk racematisk spalting av diastereomere estere og amider eller i optisk aktive faser. Dessuten er det mulig med en krystallisasjon av diastereomere salter.
Utgangspunktet for foreliggende oppfinnelse var oppklaring av følgende mekanisme: De her beskrevne cyklopentan-if-aminosyrene blir akkumulert av forskjellige gjærarter gjennom aminosyretransport. Transport av B-aminosyrer kan hemmes av alifatiske aminosyrer, særlig L-isoleucin, L-leucin, L-alanin, L-metionin og L-valin. B-aminosyren hemmer proteinbiosyntese. Denne hemmingen kan bli antagonisert av en av de alifatiske aminosyrene, særlig ved L-alanin. Den samtidig tilføringen av fi-aminosyre og den antagoniserende naturlige amionsyre som blanding fører til reduksjon av bivikningene som opptrer hos varmblodige organismer ved samtidig opprettholdelse av antimykotisk virkning in vivo.
Blandingene ifølge oppfinnelsen viser således et ikke forutsigbart, verdifullt farmakologisk virkespektrum.
Blandingene ifølge oppfinnelsen og deres syreaddisjonssalter oppviser in vivo antimikrobiell, særlig sterk antimykotisk virkning. Samtidig viser de lavere toksisitet og en bedre godtagbarhet. De besitter et bredt antimykotisk virkningsspektrum mot dermatofytter, som trikofyton mentagrofytes og mikrosporum cards, mot sporesopper som candida albicans, candida glabrata, epidermofyton floccusum og mot muggsopper som aspergillus niger og aspergillus fumigatus. Oppnevning av disse mikroorganismene innebærer på ingen måte en begrensning av bekjempbare kimer, men er bare av forklarende karakter. De er dessuten egnet for behandling av dermaf omykoser og systemmykoser.
Prøving av m- vivo- virkning
Som testmodell for antimykotisk in-vivo-virkning ble det benyttet systemisk mus-candidose: CFWi-hannmus med 20 g vekt ble infisert ved injeksjon av 3 x 10^ KBE C. albicans pr. dyr i nålevenen.
Ubehandlede kontrolldyr dør fullstendig i løpet av en uke etter infeksjon (p.i.) av en generalisert candidose med granulomdannelse i nyrene. For virkningsprøving blir preparatene oppløst i en 0,2% vandig glukose-agar-løsning, applisert i de infiserte dyrene 2 ganger daglig oralt pr. svelgsonde.
Dagsdosen utgjorde 2 x 25 mg/kg og 2 x 50 mg/kg kroppsvekt (KG), og terapivarigheten utgjorde 5 dager.
Overlevelsesraten av behandlede dyr ble notert daglig inntil dag 10. Ved dette tidspunktet overlevde ingen av de ubehandlede kontrolldyrene.
For disse preparatene ble det anvendt 10 dyr pr. dose- og kontrollgruppe.
Resultatene er angitt i tabell A:
Alternativt kan in-vivo-virkningen også bli testet på wistar-rotter. Disse trengte mindre dagsdose, med hensyn på mg/kg KG, for å oppnå en sammenlignbar terapieffekt. Testen blir i dette tilfellet gjennomført som følger: 8 uker gamle, spesifikt patogenfrie, Wistar-hannrotter på 200 g infiseres med 5x10^ KBE Candida albicans i 0,5 ml PBS i den laterale nålevenen. Dette fører til en 100% mortalitet i løpet av åtte dager. Dyrene viser allerede en dag etter infeksjon blødninger i medial øyevinkel; ved siden av nyrene angripes andre organsystemer som hjerne, hjerte, lever, milt, retina og lunger. Substansapplikasjonen foregår to ganger daglig i løpet av 5 dager peroralt i hver gang 1 ml glukose (5%)-agar (0,2%)-oppløsning begynnende ved dagen av infeksjon.
Denne bedrede godtagbarheten av blandingene ifølge oppfinnelsen ble testet på følgende måte: Wistar-rotter ble foret daglig med tilsvarende substanser og vektforløpet protokollert. Det ble enten administrert 6-aminosyre alene eller en ekvimolar mengde av tilsvarende blanding med en a-aminosyre. Etter 5 dagers behandlingsvarighet var kroppsvekten hos rottene ved tilførsel av blandingene ifølge oppfinnelsen lik eller litt øket, mens ved behandling med fi-aminosyre var den avtatt med 5-10 %.
Til foreliggende oppfinnelse hører også legemidler som inneholder blandingene ifølge oppfinnelsen. Legemidlene kan også inneholde ikke-toksiske, inerte farmasøytiske bære- og hjelpestoffer og anvendes til bekjempelse av mykoser.
Virkestoffet eller virkestoffene kan likeledes foreligge i en eller flere av de ovenfor angitte bærestoffene også i rnikroforkapslet form.
Som foretrukne farmasøytiske tilberedninger skal det nevnes tabletter, drageer, kapsler, piller, granulater, suppositorer, oppløsninger, suspensjoner og emulsjoner, pasta, salter, geler, kremer, lotioner, pudder og sprayer.
De terapeutisk virksomme blandingene bør foreligge i de ovenfor angitte farmasøytiske tilberedningene, fortrinnsvis i en konsentrasjon på ca. 0,1 til 99,5, fortrinnsvis 0,5 til 95 vekt-% av den samlede blandingen.
De ovenfor angitte farmasøytiske tilberedningene kan foruten forbindelsen ifølge oppfinnelsen også inneholde andre farmasøytiske virkestoffer.
Virkestoffene eller legemene kan tilføres oralt og parenteralt.
Generelt har det vist seg fordelaktig innenfor human- og også innenfor veterinærmedisin at virkestoffene ifølge oppfinnelsen administreres samlet mengde ca. 0,5 til ca. 500, fortrinnsvis 5 til 100 mg/kg kroppsvekt hver 24 time, eventuelt i form av flere enkeltdoser for oppnåelse av de ønskede resultatene. En enkeltdose inneholder det eller de virksomme stoffene ifølge oppfinnelsen, fortrinnsvis i mengder fra ca. 1 til ca. 80, særlig 3 til 30 mg/kg kroppsvekt.
Med legemidlene ifølge oppfinnelsen dreier det seg vanligvis om kombinasjonspre-paratet for samtidig, adskilt, eller trinnvis anvendelse av bekjempelse mot sykdommer.
Ved kombinasjonspreparater for samtidig anvendelse dreier det seg om resultater hvori enkeltkomponentene i blandingene ifølge oppfinnelsen foreligger som en fysikalsk blanding. Til disse tilhører særlig tabletter, drageer, kapsler, piller, suppositorer og ampuller. Likeledes kan man tenke seg anvendelse av slike blandinger som oppløsning, suspensjon eller emulsjon. Til enkeltkomponentene av blandingene ifølge oppfinnelsen hører a-aminosyrene, i det følgende betegnet a-aminosyre-komponenter og cyklopentan-6-aminosyre, i det følgende betegnet B-aminosyrekomponenter.
Ved kombinasjonspreparatene for adskilt anvendelse dreier det seg om de der enkeltkomponentene av blandingene ifølge oppfinnelsen foreligger adskilt rommelig i forhold til hverandre. Her dreier det seg særlig om tabletter, drageer, kapsler, piller og suppositorer der man oppnår disse kravene.
Likeledes kan man tenke seg kombinasjonspreprater for tidsmessig oppdelt anvendelse av disse. Dette muliggjør en administrering av enkeltkomponentene av blandingene ifølge oppfinnelsen i en tidsmessig forskjøvet rekkefølge. Ved den tidsmessig trinnvise anvendelse av slike kombinasjonspreparater kan man tenke seg å administrere a-aminosyrekomponentene på bestemt måte i forhold til administrering av 6-aminosyrekomponentene. a-aminosyrekom-ponentene kan dermed bli administrert samtidig eller i en annen vanlig applikasjonsform enn Q-aminosyrekomponentene, f.eks kan fi-aminosyrekomponentene bli administrert intravenøst, a-aminosyrekomponentene kan derimot bli administrert peroralt eller intravenøst.
Samtidig trinnvis anvendelse kan også foregå ved at bare en deldose av a-aminosyre-komponentene i forannevnte tidsmessige relasjon til actaiinistrering av fi-aminosyre-komponentene og restmengden av den samlede dosen av a-aminosyre-komponentene i en eller flere deldoser blir administrert i løpet av et bestemt tidsronretter administrering av 6-aminosyrekomponentene.

(-)-1,2-cis-2-((N-(9-fluorenylmetyloksy cyklopentan-1-karboksylsyre
Til en oppløsning av (-)-l,2-cis-2-amino-4-metylen-cyklopentan-l-karboksylsyre
(35,1 g, 0,198 mol) og natriumhydrogenkarbonat (33,36 g, 0,397 mol) i 480 ml vann blir ved romtemperatur en oppløsning av N-(9-flurenylmetyloksykarbonyl)-(S)-isoleucin-hydroksysuksinimidester (89,2 g, 0,198 mol) i 600 ml dimetoksyetan dråpevis tilsatt. Dette blir omrørt over natten ved romtemperatur. Reaksjonsblandingen blir deretter surgjort med fortynnet saltsyre til pH 2 og ekstrahert flere ganger med dietyleter. De forenede organiske fasene blir tørket over natriumsulfat og inndampet i vakuum. Produktet blir krystallisert fra dietyleter/petroleter.

(lR,2S)-2-amino-4-metylen-cyklopentan-l-karboksylsyre x (S)-isoleucin
(-)-(lR,2S)-2-amino-4-metylen-cyklopentan-l -karboksylsyre (25,0 g, 177 mmol) og (S)-isoleucin (23,2 g, 177 mmol) blir oppløst i vann (250 ml) og etanol under oppvarming. Man lar oppløsningen avkjøles til dråpetemperatur og destillerer oppløsningen i vakuum ved 60°C.
Utbytte: 48,2 g (100% av teoretisk)
Smeltepunkt: 230°C (spaltning)

En blanding av (-)-(lR,2S)-2-amino-4-metylen-cyklopentan-l-karboksylsyre (14,1 g, 100 mmol) og (S)-isoleucin (65,5 g, 500 mmol) blir fremstilt analog til beskrivelsen i eksempel 3.

Antimikrobielt aktive blandinger,karakterisert vedat de inneholder a-aminosyrer og cyklopentan-B-aminosyrer fra gruppen bestående av "(S)-isoleucin med 2-amino-4-metylencyklopentan-l -karboksylsyre, (S)-alanin med 2-amino-4-metylencyklopenten-l-karboksylsyre, (S)-prolin med 1,2-cis-aminocyklo-pentan-1-karboksylsyre" eller isomere former derav og eventuelt en eller flere farmasøytisk akseptable bærestoffer.
2.
Blandinger ifølge krav 1 for anvendelse ved behandling av sykdommer, spesielt av mykoser.
3.
Legemiddel,karakterisert vedat det inneholder en blanding ifølge krav 1.
4.
Anvendelse av blandinger ifølge krav 1 for fremstilling av legemidler for behandling av mykoser.
