Description
Foreliggende oppfinnelse vedrører et legemiddelpreparat
inneholdende fosfolipid.
Transdermal administrering av medikamenter, f.eks. anti-inflammatoriske eller antitrombotiske medikamenter, er hensikts-messig i tilfeller hvor det er nødvendig med en høy lokal konsentrasjon av medikamentene i vevet under den intakte hud, mens oral eller parenteral systemisk administrering fører til uønskede systemiske bivirkninger eller det er ikke mulig ved denne type administrering å tilby den nødvendige lokale konsentrasjon. Medikamenter så som heparin-natrium anvendes vanligvis lokalt i høye konsentrasjoner på grunn av lav permeabilitet gjennom huden.
Absorpsjonen i tarmene av dette medikament er neglisjerbar.
Systemiske effekter er uønsket når det gjelder perifere skader. Mange lokal-medikament-formuleringer kan sensibilisere, tørke ut eller endog skade huden hvor de påføres. Vannholdige kremer eller salver, som ofte har til hensikt å unngå noen av disse uønskede bivirkninger, må inneholde mikrobiologiske konserveringsmidler som for sin egen del ofte.fører til sensibilisering av huden. Det er derfor et behov for sikrere, ikke-irriterende absorpsjonsakseleratorer som i tillegg beskytter huden, og som for medikamenter med lav hud-permeasjonsevne, er fordelaktige for å øke medikamentkonsentrasjonen i målvevet. Diklofenak-natrium er et eksempel på et ikke-steroidalt anti-inflammatorisk medikament (NSAID) som anvendes ekstremt ofte, særlig med sikte på dets problemfrie og etablerte kliniske virkning. For å nedsette de kjente lokale slimhinneirritasjoner etter oral administrering er det ønskelig med lokal-administreringsformer for kronisk anvendelse når det gjelder regionale reumatiske symptomer. Metabolismen i den første passasje etter oral administrering,
som foregår i leveren og fører til delvis inaktivering, kan også . unngås ved lokal administrering. I den følgende fremstilling ble diklofenak valgt som et eksempel på medikamenter med lav hud-gj ennomtrengningsevne.
Virkningen av lokale diklofenak-natriumpreparater avhenger
i betydelig grad av preparatets evne til å tillate medikamentet
å trenge gjennom den intakte hud. Spesielt representerer horn-sjiktet en barriere for eventuell medikament-permeasjon av huden og inntrengning i det underliggende betente vev. Siden permea-biliteten av intakt hud for diklofenak-natrium er lav, er det gjennomført forskjellige eksperimenter for å øke permeasjonen gjennom huden ved anvendelse av forskjellige salter. IL-A-62.160 beskriver aminsalter av diklofenak for øket hudpenetrering. DE-A-3.336.047 (= GB-A-2.128.087) beskriver et lokalpreparat basert på en olje-i-vann-emulsjon, som inneholder dietylammoniumsaltet av diklofenak. Dette salt er mer lipofilt enn natrium-saltet, og kan trenge gjennom huden med høyere hastighet. Anvendelse av dette salt er imidlertid ikke uten risiko. Det er kjent at sekundære aminer danner svært karsinogene nitrosaminer. Preparatet inneholder dessuten dietylamin som nøytraliseringsmiddel for akrylsyre, som virker som geldannende middel for gel-preparatet. Siden sekundære aminer har visse potensielle bivirkninger, har de tyske ansvarlige registreringsmyndigheter (BGA) anbefalt utbytting av disse aminer i kosmetiske og farmasøytiske preparater. EP-A-0.372.527 beskriver anvendelse av et hydroksyetylpyrrolidinsalt av diklofenak med høyere vannløselighet enn i tilfelle natriumsaltet. Dette salt ble kombinert med forskjellige absorpsjonspromoterer, f.eks. etoksylerte glycerider, lanolinestere eller lecitin. Bare i kombinasjon med disse nye salter var absorpsjonspromoterne i stand til å akselerere absorpsjonen i samme grad som dietylaminsaltet. Sammenlign for dette Clin. Tri. J., 26 (1989) 304-309 (Galzigna et al.: Percutaneous absorption of diclofenac after topical application, two different gel formulations).
JP-A-02 . 049 . 722 beskriver anvendelse av ammoniakk nøytralise-ringsmiddel for polyakrylsyre-geler inneholdende diklofenak-natrium. Dersom det ble tatt som nøytraliseringsmiddel, ikke ammoniakk, men et alkylamin, f.eks. monoetylamin, fait hud-gjennomtrengningen in vitro fra 44,8 til 1,7%. EP-A-0.498 . 011 beskriver rubidium- eller cesiumsalter av diklofenak som har en høyere hud-gjennomtrengning in vitro enn i tilfelle natriumsaltet.
Anvendelse av disse salter er imidlertid kritisk, siden begge ioner har sine egne farmakologiske effekter. Antidepressiva virkning forekommer således, og særlig cesium øker de cerebrale konsentrasjoner av serotonin og tryptofan. Dets svært lange halveringstid (50 til 100 dager) kan føre til akkumulering i kroppen etter kronisk adminstrering. Rubidium har likeså en lang halveringstid (40 dager) og fører til langtids-akkumulering. I noen tilfeller kan det øke sentralnerveaktiviteten og aggres-siviteten. EP-A-0.245.126 beskriver kalsiumsaltet av diklofenak i kombinasjon med toksikologisk farlige absorpsjonspromoterer, så som DMSO og azoner. I tillegg til anvendelsen av nye salter av diklofenak er det utviklet forskjellige preparater inneholdende syntetiske absorpsjonspromoterer. JP-A-01.013.020 beskriver således diklofenak-natrium-emulsjoner, som inneholder dialkylkar-boksylater og fettsyrer, for lokal anvendelse. Det blir dessuten tilsatt et beskyttelsesmiddel for å redusere mikrobiologisk forurensning. Ifølge WO-A-88/04.938 anvendes heterocykliske penetrasjonsakseleratorer, .så som imidazoliner eller oksozoliner for lokal farmasøytika omfattende diklofenak. Ulempene med slike akseleratorer er imidlertid deres mulige hudirritasjon og -sensibilisering. US-A-4.670.254 beskriver diklofenak-natrium-geler som inneholder en høy mengde etanol og propylenglykol. Etanol er en kjent absorpsjonspromoter for forskjellige medikamenter. Etanol og propylenglykol er løsningsmidler for lipider og har den ulempe at de tørker ut huden. JP-A-60.146.823 anvender nikotinater som absorpsjonsakseleratorer. Som medikamenter fører de til øket sirkulasjon og øket blodstrøm og akselererer således transporten fra administreringsstedet. De fører til hetebølger og forstyrrende bivirkninger. EP-A-0.147.476 anvender glykoler og salisylater eller peppermynteolje eller mentol som absorpsjonsakseleratorer. Disse substanser kan imidlertid forårsake hud-irritas j oner , og peppermynteolje eller mentol kan føre til bren-ning eller til sensibilisering av huden.
Det er også kjent at fosfolipider kan øke hudens fuktighets-innhold og permeasjonen av medikamenter gjennom huden. Litteratur-rapportene for diklofenak-natrium er imidlertid motstridende. Ifølge Chem. Pharm. Bull., 35 (1987) 3807-3812 (Nishihata et al.) vil permeasjonen in vitro gjennom rottehud øke dersom diklofenak anvendes i en vandig gel inneholdende hydrogenert soyabønne-fosfolipider (30% fosfatidylkolin og 70% fosfatidyletanolamin; jod-tall ca. 6%). Den mengde som trengte gjennom etter 7 timer var 5 ganger høyere når gelen inneholdt 0,5% hydrogenerte fosfolipider enn i løsning uten fosfolipider. Medikamentkonsentrasjonen var 1,25% i hvert eksperiment. På den annen side, ifølge 2nd Liposome Research Days, NL Leiden, juni 1992, side 5 (Enghausen & Muller-Goymann), burde fosfolipider ikke vise noen akselererende virkning in vitro selv når en isolert stratum corneum var for-behandlet med fosfolipider. Dette sistnevnte resultat stemmer med oppfinnernes observasjoner (sammenligningseksempel 1).
Denne oppfinnelse er basert på det formål å forbedre permea-sjonsevnen for transdermale medikament-preparater inneholdende fosfolipider.
Oppfinnelsen er særpreget ved det som fremgår av krav 1's karakteriserende del. Foretrukne utførelsesformer derav fremgår av underkravene 2-6.
Ifølge denne oppfinnelse ble det overraskende funnet at i tilfelle slike transdermale medikamentpreparater kan permeasjons-evnen fordelaktig forbedres ved tilsetning av vitamin E (a-tokoferol) eller ved tilsetning av vitamin E-derivater.
Kombinasjonen av fosfolipider med tokoferol eller tokoferolderivater og kortkjedede alifatiske alkoholer fører til en uventet høy hudpermeasjon i tilfelle medikamenter som har lav permeasjonsevne. Eksempler på slike medikamenter er ikke-steroide anti-inflammatoriske, antitrombotiske, analgetiske, anestetiske, anti-biotiske, hormonale og antiallergiske medikamenter. Medikament-preparatene kan være topiske løsninger, skum, sprayer, geler, pastaer, salver, emulsjoner og liposomale dispersjoner. Siden det i tilfelle lave alkoholkonsentrasjoner produseres en antimikrobiell aktivitet, vil anvendelse av ytterligere konserveringsmidler ikke være nødvendig. Siden det ifølge denne oppfinnelse anvendes tokoferol eller et tokoferolderivat, vil ytterligere antioksydanter ikke være nødvendig.
Medikamentpreparater med sammensetning ifølge denne oppfinnelse er sikre, ikke-toksiske, vel tålbare og har en hudbeskyttende virkning som kan tilskrives tokoferol eller tokoferolderivater. Referanse kan gjøres til de kosmetiske preparater som anvendes i bred utstrekning og som er ment å forebygge eksogen hudskade, f.eks. p.g.a. UV-bestråling. Preparatene ifølge denne oppfinnelse er overlegne sammenlignet med kjente lokal-medikamentpreparater, siden absorpsjonspromotorer eller under visse omstendigheter ikke ufarlige salter blir unngått, som kan føre til hudirritasjon eller endog til skade på huden.
Det fremgår av de følgende eksperimentelle data at tilsetning av vitamin E (a-tokoferol) eller ett av dets derivater overraskende øker permeasjonen drastisk over den verdi som gjelder for en dietylammonium-diklofenak-gel. Verdier på opp til ca. 4000 ug av medikament/2,5 cm<2> kunne således bestemmes (tabell 1).
Oppfinnelsen illustreres i større detalj nedenfor ved hjelp av eksemplene og figurene: Fig. 1 viser en hudpermeasjonsprofil in vitro av en diklofenak-
natrium-gel ifølge tidligere teknologi; Fig. 2 viser en hudpermeasjonsprofil in vitro av en diklofenak-
natrium-gel ifølge denne oppfinnelse: Fig. 3 viser plasmanivåer som funksjon av tiden for en diklofenak-Na-gel henholdsvis ifølge denne oppfinnelse og ifølge tidligere teknologi.
Sammenligningsekseirtpel 1 (dietylammonium-diklof enak)
Hudpermeasjonsstudier ble utført idet det ble anvendt disse-kert hud fra hårløse mus i modifiserte Franz celler. Permeasjons-arealet var 2,5 cm . Akseptormediet var fysiologisk saltløsning ved 34°C. En typisk hudpermeasjonsprofil er vist i fig. 1.
En kommersielt tilgjengelig emulgel (Voltaren) ble anvendt som referanse. Den inneholder dietylammoniumsaltet av diklofenak. Alle mengder som trenger igjennom ble uttrykt på basis av diklofenak-natrium. Ut ifra dette eksperiment var det mulig å beregne en tilnærmet strømningshastighet på J = 36 ug medikament/cm<2>/time. Dietylammonium-diklofenak-gelen hadde en permeasjon på ca. 2000 ug medikament pr. 2,5 cm<2>/24 timer.
Sammenligningseksempel 2 (diklofenak-natrium)
Hudpermeasjonen av mettede vandige løsninger og 1% løsninger ble målt for en blanding av isopropanol og propylenglykol. Resultatene kan sees i tabell 1 nedenfor. Den medikamentmengde som trengte gjennom huden in vitro i løpet av 24 timer i tilfellet en mettet og vandig løsning var således 984 ug medikament/2,5 cm<2>.
Denne løsning hadde en maksimum termodynamisk aktivitet. En 1% vandig løsning inneholdende isopropylalkohol og propylenglykol hadde en mye lavere permeasjon på 135 ug medikament/2,5 cm<2>. Tilsetning av 2% renset naturlig soyabønne-lecitin (ca. 80% fosfatidylkolin, 2% lysofosfatidylkolin, 4% fosfatidinsyre og 1% monofosfatidylinositol) øket permeasjonshastigheten med en faktor på ca. 5 til 750 ug medikament/2,5 cm<2>, som tilsvarer omtrent verdien for en mettet løsning uten absorpsjonspromoter. Fosfolipider uten tilsetning av alkoholer førte til lavere verdi på 230 ug medikament/2,5 cm<2>.
Siden løseligheten i vann er ca. 1,9% stemmer verdiene for hudpermeasjon for løsninger i vann med konsentrasjonsforskjellene. 1:1-blandingen av isopropanol (IPA) og propylenglykol (PG) med 6% vann synes å nedsette medikamentpermeasjonen. Anvendelse av fosfolipidet alene uten tilsetning av alkoholer er også utilstrek-kelig til å oppnå permeasjon av en mettet løsning i vann.
Eksempel 1
Tilsetning av a-tokoferol eller a-tokoferolacetat til
løsningen av diklofenak-natrium som medikamentet, PL og alkoholer førte til en uventet høy hudpermeasjon (tabell 2). Det ble laget geler av medikamentløsningene med polyakrylsyre og trietanolamin for å danne et lett administrerbart preparat. Hudpermeasjonen blir bare svakt påvirket av denne modifikasjon.
Begge preparater inneholdt identiske mengder av PL og IPA. Løsningen inneholdt 5% d,1-a-tokoferolacetat og gelen 1% d-a-tokoferol. Gelen inneholdt dessuten 1,5% polyakrylsyre og en tilstrekkelig mengde trietanolamin for å danne en gel av polymeren. For å øke utspredningen av gelen ble det inkorporert en blanding av syntetiske oljekomponenter i gelen. Hud-permeas j ons-data in vitro av et fullstendig diklofenak-Na-gel-preparat kan sees av fig. 2.
Strømningshastigheten beregnet ut ifra disse data er
J = 56 ug medikament/cm /time. Det er 1,55 ganger høyere enn den for et standardpreparat.

Følgende gel kan svært...lett spres ut på huden. Den kan påføres på betente områder av kroppen.
Anvendelseseksempel 4 og
sammenlignings-anvendelseseksempel 1 (diklof enak-Na-geler)
Anvendelseseksempel 3 ble gjentatt (anvendelseseksempel 4). 7,5 g av den Hige diklofenak-Na-gelen ble påført i hvert tilfelle til ryggene av 12 individer og vasket av etter en påføringsperiode på 6 timer.
For sammenligning ble 7,5 g av en kommersielt tilgjengelig l%ig diklofenak-Na-gel (Voltaren) i hvert tilfelle påført på ryggene av 12 individer, og gelene ble igjen vasket av etter en påføringsperiode på 6 timer.
De gjennomsnittlige verdier for plasmanivåer er vist grafisk i fig. 3. Fig. 3 viser at i anvendelseseksempel 4 var plasmanivået fremdeles økende når påføringene ble avsluttet. I sammenlignings-anvendelseseksempel 1 falt plasmanivået derimot endog før avslutning av påføringene, ved en påføringsperiode på ca. 5 timer.
Dette fall må tilskrives det faktum at sammenligningsgelen ikke inneholder noe fosfolipid og/eller a-tokoferol.
Claims (6)
1. Legemiddelpreparat inneholdende fosfolipid, karakterisert ved at det er et transdermalt medikament inneholdende
vann,
en alifatisk Ci-6-alkohol i området fra 1 til 50% basert på
den samlede vekt av preparatet, idet 7,2% og mindre er ute-lukket ,
a-tokoferol eller a-tokoferol-derivat og
et hormon, diklofenak-Na eller heparin-Na som legemiddel.
2. Legemiddelpreparat ifølge krav 1,
karakterisert ved at det inneholder a-tokoferol eller a-tokoferol-derivat i området fra 0,1 til 10% og spesielt 0,5 til 7%, basert på den samlede vekt av preparatet.
3. Legemiddelpreparat ifølge et av de foregående krav, karakterisert ved en alifatisk C2-4-alkohol som den alifatiske Ci-6-alkohol, eventuelt i tillegg til en ytterligere vanlig alkohol.
4. Legemiddelpreparat ifølge et av de foregående krav, karakterisert ved at det inneholder fosfolipid i området fra 0,5 til 20%, basert på den samlede vekt av preparatet.
5. Legemiddelpreparat ifølge et av de foregående krav, karakterisert ved liposomer basert på fosfo
lipidet.
6. Legemiddelpreparat ifølge et av de foregående krav, karakterisert ved at det foreligger som en salve, gel eller spray.
