Description
Oppfinnelsen vedrører en fremgangsmåte for å isolere den terapeutisk anvendbare stamme Lactobacillus acidophilus ATCC
53103 fra en prøve som inneholder en multiplisitet av bakteriestammer.
L. acidophilus er en komponent av den normale tarmflora i
de fleste friske mennesker. Slekten Lactobacillus er vidt utbredt i naturen. Mange arter, slik som L. bulgaricus og L. casei, finnes i meieriprodukter, og også i frukt og grønnsaker. Andre Lactobacillus-arter påtreffes i tarmkanalen til pattedyr
og insekter. I mange århundrer er lactobacillus-bakterier blitt brukt i melk og meieriprodukter som fermenteringsorganismer,
ved produksjon av yoghurt og forskjellige typer syrnet melk og tykkmelk. Artene av Lactobacillus som brukes i meieriindustrien er hovedakelig L. bulgaricus, selv om L. acidophilus-stammer også er blitt brukt i nokså utstrakt grad.
I de senere år er det blitt utført en rekke studier hvor
man har brukt L.acidophilus-stammer isolert ved North Carolina State University ("NCSU"). Disse studier har gjort det klart at NCSU-stammene, når de fordøyes av mennesker eller dyr, har fordelaktig innvirkning på forskjellige funksjoner i mage/tarmkanalen, inkludert undertrykkelse av aktivering av kjemiske karsinogener i tykktarmen, en reduksjon av fekal utskillelse av kolesterol (relatert til en redusert risiko for tykktarmkreft)
og en reduksjon av gastrointestinale bivirkninger av antibiotika-terapi.
Fremgangsmåten i henhold til oppfinnelsen er karakterisert ved at den omfatter følgende isoleringstrinn av prøven i en hvilken som helst rekkefølge: (a) det utvelges bakterier som er i stand til sterkt å feste seg til gastrointestinale celler, idet minst et gjennomsnitt av minst 50 av nevnte bakterier kan feste seg til en human tynntarm-mucosa-celle etter en 5-minutters inkubering av nevnte bakterier med nevnte celle, (b) det utvelges bakterier som har evne til å formere seg i næringsmedium ved pH under 5,5, og (c) det utvelges bakterier som har evne til å formere seg i næringsmedium som inneholder minst 0,1%
galle.
L. acidophilus-stammer viser en rekke karakteristika som gjør dem fordelaktige for human helse, og spesielt gjør dem anvendbare i behandling av bivirkninger av antibiotikaterapi, ulcerøs kolikk, og forstoppelse; ved at de gir resistens for gastrointestinal kolonidannelse av patogene mikroorgansimer og gjærarter; ved at de reduserer fekal kolesterolutskillelse; og ved at de reduserer fekal østrogenutskillelse. L. acidophilus-stammer som her beskrevet er karakterisert ved at et gjennomsnitt på minst 50, mer foretrukket minst 100, av bakteriene kan feste seg til en human tynntarm-mucosacelle etter en 5 minutters inkubering av bakterien med cellene. Fortrinnsvis er bakteriene videre karakterisert ved at de produserer minst 3,5 milliekvivalenter ("mekv") melkesyre pr. 10<10>CFU (kolonidannende enheter) i næringsmedium ved 37^C pir.
dag; de er i stand til å vokse ved 37\ Q i næringsmedium ved en pH-verdi på 3,0; de er i stand til å vokse i næringsmedium som inneholder 0,1% oksegalle; de har en generasjonstid på mindre enn 1 time i næringsmedium ved 37^C. ("Næringsmedium" refererer til ethvert kulturmedium som inneholder de næringsstoffer som kreves for veksten av L. acidophilus-bakterier).
Ved fremgangsmåten i henhold til oppfinnelsen inkuberes bakteriene av prøven i næringsmedium ved en pH-verdi under 3,5 og selekteres bakterier som vokser i nevnte medium. Bakterier av prøven inkuberes i næringsmedium som inneholder minst 0,1% galle, og bakterier som vokser i nevnte medium, selekteres. Videre inkuberes bakterier av prøven med næringsmedium, og bakterier som har en generasjonstid på mindre enn 1 time ved ca. 37^C, selekteres.
Andre viktige momenter og fordeler ved oppfinnelsen vil vise seg fra den følgende beskrivelse av foretrukne utførelses-former.
Stamme- isolerinq
Friske mennesker har bakterier av arten L. acidophilus som del av sin normale flora, generelt i konsentrasjoner på omtrent IO<9> lactobaciller pr. gram tarminnhold; således har et friskt
individ i overskudd av IO<11> lactobacciller i sin tarmkanal. Det første trinn i metoden er å screene fekale prøver fra mennesker og selektere de prøver hvor høye lactobacillus-konsentrasjoner er til stede.
Deretter blir disse prøver screenet ved å bruke en fremgangsmåte utarbeidet for å isolere stammer som viser karakteristika diskutert ovenfor, og som kort kan summeres som:
(1) Stabilitet overfor syre.
(2) Stabilitet overfor galle (en egenskap tidligere identifisert som ønskelig av Department of Food Science, North Carolina State University). (3) Evne til å feste seg sterkt til mucosa-celler i den humane tarmkanal.
(4) Melkesyreproduksjon.
(5) Hardfør in vitro-vekst.
Stabilitet overfor syre og galle
Screening for stabilitet overfor galle, og for stabilitet overfor et i noen grad surt miljø blir utført som følger. LBS-agar (beskrevet i J. Bacteriol. [1951] 6_2, 132) blir fremstilt, idet det har følgende sammensetning, pr. liter:
Til 84 g pulver med sammensetningen ovenfor, dehydratisert, blir det tilsatt 300 ml tomatsaft og 800 ml destillert vann og, etter sammenblanding, 1,32 ml eddiksyre. 1,5 g oksegalle blir
så tilsatt, og pH blir justert til 5,5 med IN HC1. Skåler
blir lagret i et anaerobt kammer 1-3 dager før bruk og så
brukt uten autoklavering. Fekale prøver fra friske individer ble homogenisert i vann under anaerobe betingelser og seriefor-tynninger inkubert på skålene i 48 timer i et anaerobt kammer. Bakteriene som vokser på skålene, blir selektert og videre screenet. Dette trinnet eliminerer mange av mikroorganismene i prøven.
Screening for stabilitet overfor surere betingelser blir utført ved å bruke en MRS-buljong (Difco; beskrevet i J.
Applied Bacteriol. [1960] 23, 130) med sammensetning:
55 g pulver med sammensetningen ovenfor, dehydratisert, blir blandet med 1000 ml destillert vann og sterilisert ved autoklavering i 15 min. ved 15 lb. Buljongen har en slutt-pH-verdi på 6,5 ved 25°C. pH blir så justert ved å bruke 1,0 N HCl og, hvis nødvendig, 1,0 N NaOH, og buljongfilteret sterilisert. Bakteriene blir inkubert i buljongen i 48 timer i et anaerobt kammer. Selekterte bakterier kan fortrinnsvis vokse ved pH under 3,5, fortrinnsvis ved pH 3,0.
L. acidophilus- seleksion
Det neste screening-trinnet er isoleringen fra bakteriene som er i stand til å vokse i syre og galle, av bakterier av arten L. acidophilus. Dette trinnet blir utført ved å bruke standard mikrobiologiske teknikker.
Adherens
Det neste trinnet er å teste syre og galle-stabile L.acidophilus-bakterier for evnen til å feste seg til gastrointestinale celler. Humane gastrointestinale celler blir først isolert ved en hvilken som helst egnet metode. En teknikk involverer oppsam-ling av avføring fra en pasient med en velfunksjonerende ileosto-rtii ved vasking med saltvann.
Saltvanns-avføringen blir samlet i kald 1/10 volum av
buffer I (0,1% gelatin, 1% glukose, 500 mM natriumfosfat, pH 7,4). Denne løsningen blir så filtrert gjennom silke nr. 25 for å fjerne klumper og rester og tynntarms(ileum)mucosaceller samlet ved sentrifugering.ved 100 G i 10 min. Cellene blir så vasket i buffer II (0,45 % NaCl, 0,1 % glukose, 0,01 % gelatin, 50 mM natriumfosfat, pH 7,4) ved å bruke de samme sentrifugebetingel-ser. Til slutt blir cellene resuspendert i buffer II, 1/3 volum vevskultur medium NCTC 135 blir tilsatt, og cellene blir lagret ved 4°C inntil bruk. Ileum-celletall og levedyktighet blir bestemt ved Trypan-blåfarvning. De humane ileumceller kan fryses
i nitrogen i væskeform etter tilsetning av 10 vol.% glycerol.
Bakteriene blir dyrket i 18 timer ved 37°C i MRS-buljong
med 6 uCi av 3 H-alanin og <3>H-leucin tilsatt. Bakteriene blir så vasket i 5 volumer fosfat-bufret saltvann, resuspendert i buffer II og filtrert gjennom 2,0 pm polykarbonatfiltre for å fjerne klumper. Bakteriene blir så blandet med de humane tynn-tarmsceller, inkubert ved 37°C og prøvene tatt 5 minutter senere. Prøvene blir plassert på 2,0 jjm polykarbonatf iltre og overskudds-væske fjernet ved forsiktig suging. Filtrene blir vasket, først med 2 ml, så med 5 ml fosfatbufret saltvann (pH 7,4). Ved å bruke disse filtre blir ileumceller og tilsatte bakterier holdt tilbake, men de frie bakterier passerer gjennom. Kontroller inkluderer reaksjonsblåndingen uten de humane tynntarmceller.
Den tilbakeholdte radioaktivitet blir bestemt ved væskescintilla-sjon. Den spesifikke bakterielle aktivitet blir bestemt fra de kolonidannende enheter beregnet fra den optiske tetthet ved 550 nm og radioaktiviteten tilbakeholdt ved 0,2 u filtre. Den spesifikke bakterielle aktivitet blir brukt til å regne ut antall bundete bakterier.
Bakterier som binder seg, i henhold til fremgangsmåten ovenfor, i et forhold på minst 50, og mer foretrukket 100, bakterier pr. gastrointestinal celle (som representerer omtrent 10 % av bakteriene som har vært i kontakt med cellene) er de ønskete bakterier.
Melkesyreproduksjon
Melkesyreproduksjon blir målt ved å inkubere bakterier i MRS-buljong eller Scott Peptone Yeast Broth under anaerobe betingelser og ved å måle melkesyre ved 8 eller 24 timer ved å bruke gass/væske-kromatografi. Selekterte bakterier produserer minst 3,5 mekv., og mer foretrukket over 4 mekv,, melkesyre pr. IO<10 >CFU ved 37°C over en 24-timers periode.
Hardfør vekst
Slutt-screening-trinnet er seleksjonen av bakterier som viser hardfør vekst in vitro, som målt ved generasjonstid. 5 x IO<4> CFU bakterier blir brukt til å inokulere næringsmedium, dvs. MRS-buljong, og CFU målt ved 1, 2, 3, 4, 5, 6, 7 og 8 timer ved
å ha 0,1 ml av fortynnete alikvote mengder på skåler på LBS-
agar og inkubere skålene i 48 timer ved 37°C under anaerobe betingelser. Bakterier som har en generasjonstid ved 37°C på mindre enn 1 time, mer foretrukket omtrent 45 minutter, blir selektert.
Screening- prosedyre
Hvert trinn i screening-prosedyren ovenfor er utarbeidet
for å isolere L.acidophilus-stammer som har karakteristika som gjør dem i stand til å forbedre human helse.
Syre- og gallestabilitet er viktig fordi bakteriene blir inntatt og må passere gjennom det sure miljø i maven og også den galleinneholdende tynntarm, og de må være i stand til å overleve og vokse under disse betingelser hvis bakteriene skal kunne gi helsefordeler for personen som tar dem inn.
Adherens er viktig, fordi denne egenskap tillater L.acidophilus-bakteriene å kolonidannes og bli etablert i mave/tarmkanalen.
Melkesyreproduksjon er viktig fordi man tror at melkesyren produsert av bakteriene delvis er ansvarlig for de fordelaktige virkningene av kolonidannelse i mave/tarm-kanalen av stammer som her beskrevet. Melkesyre er et antimikrobielt stoff, og produksjonen av dette av de her beskrevne stammer kan hjelpe dem til å danne en beskyttende barriere overfor patogene bakterier i mave/tarm-kanalen. Melkesyre kan også hjelpe i inaktiveringen av karsinogener i mave/tarm-kanalen.
Hardfør in vitro-vekst er en viktig egenskap fordi de her beskrevne stammer må produseres i stor skala; harfør vekst er også en indikator på at bakteriene sannsynligvis også vil vokse godt in vivo.
Karakteristika av stamme GG
En stamme som her beskrevet, betegnet GG, har følgende karakteristika.
Syrestabilitet
GG-stammen kan vokse ved pH 3,0 opp til en konsentrasjon på 1 x 10 7 CFU. Ved høyere pH-verdier er veksten enda høyere, og når IO<9> CFU ved pH 5,0.
I normal mavesaft kan GG-stammen, som starter ved et inokulum på 10 g CFU, overleve, med mindre enn 2 log senkning i inoku-lumstørrelse, i minst 30 minutter ved pH 2,5.
Bakterier av!GG-stammen (10 qCFU) ble inokulert ned gjennom et Levine-rør direkte inn i maven til en frivillig, frisk person. Med gastrisk innhold ved pH 1,0-2,0, overlevde minst 1000 organismer av GG-stammen i et minimum på 2 timer.
Galle- stabilitet
GG-stammen kunne vokse i oksegalle ved en konsentrasjon på 0,15 % i LBS-agar.
Adherens til intestinale mucosaceller
Tabellen nedenfor viser graden av adherens av bakterier av GG-stammen til ileum-mucosaceller.
Melkesyreproduksj on
Produksjonen av melkesyre ble målt ved gass/væske-kromatografi, etterfulgt av inkubering av GG-stammen i næringsbuljong i væskeform. De følgende melkesyrekonsentrasjoner ble målt, ved de gitte inkubasjonstider.
Generasjonstid
GG-stammen viste hardfør vekst in vitro. Ved å bruke et start-inokulum på 5 x IO<4> CFU i MRS-buljong viste stammen en 40 gangers økning i vekst i løpet av en 4-timers periode, og nådde en tetthet på 2 x IO<6> CFU. Således var det i løpet av denne 4-timers perioden 5,2 generasjoner, som gir en enkelt generasjonstid på 45 minutter.
Anvendelse
L.-acidophilus-stammer som her beskrevet kan dyrkes i stor skala og brukes terapeutisk, i lyofilisert form eller i tilknytning til et farmasøytisk akseptabelt bærerstoff, og kan tilsettes til matprodukter, spesielt meieriprodukter slik som melk og yoghurt, hvilket er blitt gjort med andre L. acidophilus-stammer i mange år. Bakteriestammene, alene i mat eller i andre bærere, kan tas oralt av en human pasient i en effektiv mengde (dvs. 10<6->10<10> bakterier pr. person pr. dag) for å behandle eller forhindre en rekke medisinske forstyrrelser, nevnt ovenfor, og nå diskutert mer i detalj.
Kulturbetingelser er som følger.
En stokk-løsning består av steril skummet melk som inneholder IO"''<0> CFU levende organismer. 1 ml av denne stokk-løsning blir tilsatt til 2 liter MRS-buljong. Karet blir plassert i et anaerobt inkuberingsrom og dyrket over natten i 14 timer med — generell røring ved 37 °C. Buljongen blir sentrifugert ved 10 000 G i 20 minutter, og pelleten blir resuspendert i 50-100 ml steril skummet melk og lagret ved -80°C.
I en spesiell administrasjonsmåte blir 10 9 til 10 10 organismer tilsatt til melk, enten i volumer på 10-20 ml, eller i stør-re volumer, dvs. 250-500 ml. Melken blir administrert én gang om dagen, slik at IO<9> til IO<10> CFU blir gitt i en enkelt administrering. Til dyr, blir 0,2-1,0 ml av stokk-melkeløsningen
Q IQ
blandet i maten, slik at de mottar 10 til 10 CFU av bakteriene daglig.
Bivirkninger av antibiotika- terapi
Diaré er hovedkomplikasjonen av antibiotika-behandling, og det forekommer i omtrent 10 % av pasientene som mottar antibiotika, spesielt ampicillin, amoxacillin, cephalosporiner, clindamycin, erytromycin og tetracykliner. De her beskrevne stammer kan brukes til å behandle antibiotika-indusert diaré på samme måte som andre L. acidophilus-preparater er blitt brukt i den senere tid. I tillegg kan stammene brukes før eller samtidig med antibiotika-terapi, som forebyggende behandling. Slik forebyggende administrering bør forekomme innen 24 timer før antibiotika-terapien.
Ulcerøs kolitt
Idiopatisk ulcerøs kolitt, også kjent som inflammatorisk tarmsykdom, representerer et vanskelig terapeutisk problem. Syk-dommen er nokså vanlig, og pasientene lider av tilbakevendende angrep av diaré, som kan forekomme ved mange anledninger over år med aktiv sykdom. På det nåværende tidspunkt er de eneste tera-piene som har vist seg vellykket, salicylazosulfapyridin (SAS)
og kortikosteroider. Mange pasienter får tilbakefall til tross for disse behandlinger. I tillegg er behandlingene i seg selv assosiert med en stor hyppighet av bivirkninger.
Flere bevislinjer har antydet at tarm-mikrofloraen er involvert i denne sykdomsprosessen, enten som en initiator eller som en determinant for symptomer.
Bakteriene av L. acidophilus-stammer som her beskrevet kan administreres som beskrevet ovenfor for antibiotika-bivirkninger, for å behandle eller forhindre ulcerøs kolitt.
Forstoppelse
Treg tarmvirkning og kronisk forstoppelse er hovedproblemer for eldre mennesker. Forstoppelse fører til utvidelse av maven, kramper og generell illebefinnende. Den generelle offentlige interesse for å fremme god tarmaktivitet gjenspeiles i rikhol-digheten av produkter som er tilgjengelige i matvarehandelen for å lette denne tilstanden. Disse produkter varierer fra sterke lakserende midler til kortkjedete fettsyrer, til varierende na-tur fiberprodukter slik som pektiner, ligniner, gummier og slim-dannende stoffer.
Kulturerte meieriprodukter, yoghurter og syrnete melkepro-dukter er blitt anvendt gjennom historien for å avhjelpe kronisk forstoppelse.
Bakterier av L. acidophilus-stammer som her beskrevet kan anvendes alene, som beskrevet ovenfor, eller kan blandes med en underterapeutisk dose av laktulose, et syntetisk disakkarid som er blitt brukt til å behandle forstoppelse. Laktulosen vil bli metabolisert av bakteriene og fremme deres fordelaktiske virkning uten å produsere bivirkninger som noen ganger er assosiert med terapeutiske doser av laktulose.
Resistens mot kolonidannelse
Det har vist seg i de senere år at kolonidannelse i mave/- tarm-kanalen i mennesker gir et reservoar for formering av patogene mikroorganismer, spesielt i immunosvekkete verter. Den intestinale mikroflora i friske individer kan være resistent mot implantering av uønskede mikroorganismer gjennom en rekke indre kontrollmekanismer. Mens.disse kontrollmekanismer er dårlig forstått, er netto-resultatet relativ stabilitet av den humane tarmflora, men opprettholdelse av de samme typer og samme konsentrasjoner av bakterier over lange tidsperioder.
Gjennom påvirkningen av antibiotika-behandling er uønskede mikroorganismer i stand til å kolonidanne i mave/tarm-kanalen. Blant bakteriene inkluderer disse uønskede organismer Pseudomonas, Enterobacter, Serratia, Klebsiella, Citrobacter og Proteus-Providencia. Sopper kan også kolonidanne i tarmen, spesielt gjærarter slik som Candida.
Mens friske individer kan stå imot kolonidannelse i tarmen, selv under antibiotikabehandling, kan -immunsvekkete verter stå overfor en alvorlig trussel under denne omstendighet. Disse uønskede mikroorganismer kan invadere mucosa, produsere blodfor-giftning og livstruende sykdom.
De kliniske situasjoner hvor kolonidannelse i tarmen av uønskede mikroorganismer blir viktig, inkluderer cancer, spesielt i de pasienter som er behandlet med sterk chemoterapi, med arve-lig eller ervervet immunodefisiens-syndromer; pasienter behandlet med kortikosteroider for en rekke tilstander; og mottagere av transplanterte organer slik som nyre, benmarg, hjerte og lever. Det er også eldre, svekkete pasienter som blir kolonisert og eventuelt infisert av disse uønskete mikroorganismer. Blant barn er innleggelse på intensiv-avdelinger ofte forbundet med antibiotikabehandling, dårlig næringsstatus og immunosvekkende tilstander. Denne gruppen av høyrisikobarn er også utfordret av proble-mene med kolonidannelse med potensielt patogene bakterier og gjærarter.
Opprettholdelse av den naturlige resistens for overvekst~av uønskete mikroorganismer er kjent i den senere vitenskapelige litteratur ved betegnelsen "colonization resistance" (CR)• Forskjellige metoder er blitt tatt i bruk for enten å fremme resistens mot kolonidannelse eller alternativt å forhindre overvekst av tarm-mikroorganismer. En forbedring av CR oppnås ved å bruke antibiotika som produserer bare minimale virkninger i sammensetningen av den normale tarmflora, spesielt ved å opprettholde de normale anaerobe komponenter som synes å være ansvarlig for å stå imot overvekst av uønskede mikroorganismer. Den alter-native metoden er å forhindre kolonidannelse ved administrering av orale antibiotika som hovedsakelig dreper alle nye inntrengere i mikrofloraen. Denne metoden produserer en "steril" mikroflora, i det minste for en kort tidsperiode.
Mens hver av disse metoder for å forhindre kolonidannelse har spesielle tiltrekkende egenskaper, er de ikke nødvendigvis effektive. Relativt få terapeutiske antibiotika er i stand til å opprettholde CR, så denne metoden er ofte uvirksom. Den andre metoden som involverer administrering av orale antibiotika, produserer mange bivirkninger i seg selv, og den er ofte uvirksom siden høyt resistente mikroorganismer kan kolonidanne selv i nærvær av antimikrobielle stoffer. Således forblir pro-blemene ved kolonisering av uønskete mikroorganismer i immunosvekkete verter, enten voksne eller barn, et hovedproblem i mo-derne medisin.
Bakterier av L. acidophilus-stammer som her beskrevet kan administreres til pasienter som beskrevet ovenfor, for å gi CR på en god måte, siden bakteriene ikke har noen indre patogeni-tet. Kolonisering av tarmen ved denne naturlige organisme kunne forhindre andre patogene former fra å komme inn i floraen, mens de samtidig opprettholder et relativt friskt tarmmiljø.
Kolesterol- utskillelse
Betraktelig interesse er blitt reist i nylige år i fekal utskillelse av steroler, spesielt kolesterol, siden det kan være en sammenheng med colon cancer. Studier i forsøksdyr utført for over 10 år siden, viste at kjemisk-indusert colon cancer kunne påvirkes av økt utskillelse av kolesterol i feces.
Det er blitt vist for noen tid siden at kolesterol undergår en enterohepatisk sirkulasjon, som resulterer i intestinal reabsorpsjon av noe kolesterol og fekal utskillelse av resten.
I tykktarmen blir kolesterol utsatt for mikrobiell reduksjon
til koprostanol og koprostanon. De fleste individer utskiller det meste av kolesterolet som disse reduserte endeprodukter,
men omtrent 20 % av mennesker utskiller mest ren kolesterol,
med bare små mengder av reduksjonsproduktene.
Mens disse dyrestudier var interessante, ble deres relevans ikke regnet å være signifikant inntil kliniske prøver på kolesterol-senkende medisiner ble publisert. Disse studier viste at de kolesterolsenkende medisiner, slik som atromid, var suksess-fulle i å redusere serum-kolesterolnivået og i å redusere hjerte-attakker. Den logiske konsekvens var imidlertid en økning i tykktarmskreft, for senkningen av serumkolesterol kom på bekost-ning av økning av fekal kolesterol. Disse kliniske prøver verifiserte resultatene som er sett i dyr, hvorved høyere nivåer av fekal kolesterol vas assosiert med økt risiko for colon cancer.
Bakterier av L. acidophilus-stammer som her beskrevet kan administreres til pasienter, som beskrevet ovenfor, for å redusere mengden av kolesterol utskilt i pasientenes feces.
Østrogen- utskillelse
Forbedringer i laboratorieteknikker for å måle østrogener har utvidet vår forståelse for disse viktige kjønnshormoner'. Østrogener produseres stort sett i ovariene og i en mindre grad i fett og andre vev. Hovedutskillelsen av østrogener er via urinen.
En viktig utvikling i senere år er forståelsen av den enterohepatiske sirkulering av østrogener. Det er blitt vist at så mye som halvdelen av sirkulerende østrogener blir resirkulert gjennom gallen til tarmkanalen, hvor de blir reabsorbert til kroppen. Bare 5 % av østrogenene blir utskilt i feces.
For å bli utskilt i gallen er østrogenene konjugerte, generelt som et glucuruonid eller sulfonat. Disse konjugerte østrogener passerer gjennom gallen til tarmkanalen, hvor de blir de-konjugert ved tarmbakterier og videre reabsorbert.
Viktigheten av den enterohepatiske sirkulering ble vist i studier av Adlercreutz et al., hvor fekale og urinære østrogener ble testet i kvinner som mottok ampicillin for en liten infeksjon. Dette orale antibiotikum forårsaket 30% reduksjon i utskillelse av urinære østrogener, med en samtidig økning i fekal østrogenutskillelse. Forklaringen er at antibiotika undertrykte bakterier i tarmen som er ansvarlige for dekonjuge-ring av østrogenene. Dette førte til minket reabsorpsjon i tarmen og øket fekal utskillelse av østrogener.
Mange studier har påvist at høyere østrogennivåer i urin og plasma er korrelert med økt risiko for brystkreft. Dette viser seg å passe med observasjonen at vegetariske kvinner som har la-vere nivåer av urin og plasma-østrogener, har reduserte grader av brystkreft.
Bakterier av L. acidophilus-stammer som beskrevet her kan administreres som beskrevet ovenfor, til menstruerende kvinner for å senke mengden av utskilt østrogen; de samme dekonj.ugerende enzymer involvert i østrogen-reabsorpsjon er influert av tilfør-sel av L. acidophilus.
Deponering
GG-stammen av L. acidophilus er deponert i American Type Culture Collection, Rockwille, MA, og er blitt gitt ATCC
nr. 53103. Claims (1)
- Fremgangsmåte for å isolere den terapeutisk anvendbare stamme Lactobacillus acidophilus ATCC 53103 fra en prøve som inneholder en multiplisitet av bakteriestammer,karakterisert ved at den omfatter følgende isoleringstrinn av prøven i en hvilken som helst rekkefølge: (a) det utvelges bakterier som er i stand til sterkt å feste seg til gastrointestinale celler, idet minst et gjennomsnitt av minst 50 av nevnte bakterier kan feste seg til en human tynntarm-mucosa-celle etter en 5-minutters inkubering av nevnte bakterier med nevnte celle, (b) det utvelges bakterier som har evne til å formere seg i næringsmedium ved pH under 5,5, og (c) det utvelges bakterier som har evne til å formere seg i næringsmedium som inneholder minst 0,1% galle.
