Description

항생제는 그 항균 및 항염증 특성 때문에 최초의 성공적인 여드름 치료였다. 국소적 및 전신적 항생제 모두 매우 성공적이었으나 오랜 치료 기간이 요구되어 피.아크네스 및 다른 비표적 (그리고 잠재적으로 병원성인) 공생 유기체들에서의 내성 발달을 야기하였다. 항생제와 국소 레티노이드의 조합은 여드름과 관련된 네 개의 주요 병원성 인자 중 세 개 (피지 생성을 제외한 모두)를 표적으로 한다. 경구 레티노이드 이소트레티노인(isotretinoin) (예를 들어, 아큐탄(Accutane)®)이 여드름과 관련된 네 개의 병원성 인자 모두에 영향을 미치는 것으로 알려진 유일한 약물이다. 그러나 (최기형성으로 알려져 있고 우울증, 정신병 및 자살에 연관된) 그것의 잠재적 부작용의 심각성이 그것의 사용을 제한하였고 막대한 소송을 야기하였다.

NO-방출 화합물은 본 발명의 실시태양에 따라 임의의 적당한 농도로 제약학상 허용되는 조성물에 존재할 수 있으나, 일부 실시태양에서 NO-방출 화합물은 여드름을 감소, 제거 또는 예방하고(거나) 피지 생성을 감소하기에 충분한 농도로 조성물에 존재한다. 일부 실시태양에서, NO-방출 화합물의 농도는 조성물에서 0.1 % 내지 20 % w/w 범위이다.
상기 기재된 바와 같이, 일부 실시태양에서, 제약학상 허용되는 조성물은 항균, 항염증, 통증완화, 면역억제 또는 혈관확장 성질을 가지는 것과 같은 하나 이상의 부가적인 치료제를 포함한다. 레티노이드와 같은 다른 항여드름 치료제가 또한 본 발명의 실시태양에 따른 조성물에 포함될 수 있다.

다른 예시적인 부형제는 수용성 기공 형성제(porogen)을 포함한다. 수용성 기공 형성제는 제약학상 허용되는 조성물로의 물 흡수 및 확산을 촉진할 수 있는 첨가제이다. 임의의 적절한 기공 형성제가 사용될 수 있지만, 일부 실시태양에서, 기공 형성제는 염화나트륨, 과당, 포도당, 젖당, 소르비톨, 자일리톨, 폴리에틸렌 글리콜, 폴리비닐피롤리돈, 폴리비닐 알코올 또는 이들의 혼합물을 포함할 수 있다. NaCl과 같은 전해질 역시 부형제로서 가할 수 있다.
중합체는 또한 부형제로서 작용할 수 있다. 예시적인 중합체는 친수성 폴리우레탄, 친수성 폴리아크릴레이트, 카르복시메틸셀룰로스와 아크릴산의 공중합체, N-비닐피롤리돈, 폴리(히드록시산), 폴리무수물, 폴리오르토에스테르, 폴리아미드, 폴리카르보네이트, 폴리알킬렌 (예를 들어, 폴리에틸렌 및 폴리프로필렌), 폴리알킬렌 글리콜 (예를 들어, 폴리(에틸렌 글리콜)), 폴리알킬렌 옥시드 (예를 들어, 폴리에틸렌 옥시드), 폴리알킬렌 테레프탈레이트 (예를 들어, 폴리에틸렌 테레프탈레이트), 폴리비닐 알코올, 폴리비닐 에테르, 폴리비닐 에스테르, 폴리비닐 할라이드 (예를 들어, 폴리(비닐 클로라이드)), 폴리비닐피롤리돈, 폴리실록산, 폴리(비닐 아세테이트), 폴리스티렌, 폴리우레탄 공중합체, 셀룰로스, 유도체화된 셀룰로스, 알기네이트, 폴리(아크릴산), 폴리(아크릴산) 유도체, 아크릴산 공중합체, 메타크릴산, 메타크릴산 유도체, 메타크릴산 공중합체, 폴리(부티르산), 폴리(발레르산), 폴리(락티드-코-카프로락톤), 이들의 공중합체 및 이들의 배합물을 포함한다.
본 발명의 일부 실시태양에서, 중합체는 초흡수성 중합체 (SAP)일 수 있다. 중합체는 IUPAC에 따라 정의된 대로 그 자신의 질량에 비해 매우 많은 양의 물을 흡수하고 보유할 수 있는 중합체로서 초흡수제로 여겨진다. SAP는 그 자신의 중량의 500 배까지 물을 흡수할 수 있고 그 본래 부피의 1000 배까지 팽윤할 수 있다. 관심대상인 특정 SAP는 폴리아크릴산 나트륨, 폴리우레탄 테코필릭(Tecophilic) TG-2000, 및 폴리아크릴아미드 공중합체, 에틸렌 무수말레산 공중합체, 가교 카르복시-메틸-셀룰로스, 폴리비닐 알코올 공중합체, 폴리비닐피롤리돈과 가교 폴리에틸렌 옥시드를 이용하여 만들어진 중합체를 포함한다. 일부 실시태양에서, SAP는 피부로부터 물을 흡수할 수 있고 이로써 NO가 NO-방출 화합물로부터 방출되도록 한다.
본 발명의 일부 실시태양에서, 비교적 소수성인 중합체가 사용될 수 있다. 임의의 적절한 소수성 중합체가 사용될 수 있다. 그러나, 비교적 소수성인 예시적인 중합체는 방향족 폴리우레탄, 실리콘 고무, 폴리실록산, 폴리카프롤락톤, 폴리카르보네이트, 폴리비닐클로라이드, 폴리에틸렌, 폴리-L-락티드, 폴리-DL-글리콜리드, 폴리에테르에테르케톤(PEEK), 폴리아미드, 폴리이미드 및 폴리비닐 아세테이트를 포함한다. 또한, 소수성 겔 기재 및/또는 레올로지 변형제가 사용될 수 있다.
본 발명의 일부 실시태양에서, 특히 겔에서, 중합체가 약제에서의 증점제로서 작용할 수 있다. 특히, 겔의 중합체 부분은 그 안에 분산되어 있는 NO-방출 화합물과 함께 점탄성 물질로서 작용할 수 있으며 적용 부위에 겔을 함유할 수 있다.
일부 다른 실시태양에서, 특히 겔과 연고에서, 중합체를 포함하는 약제는 퍼짐성을 가질 수 있어서 피부 표면에 적용하였을 때 얇은 필름을 형성한다. 이러한 필름은 함유된 NO-방출 화합물의 적용을 넓은 영역에 걸쳐 가능하게 할 수 있고 피부의 환부상에 NO-방출 화합물을 유지하도록 도울 수 있다.

이온성 화합물의 예는 염화나트륨, 염화칼륨과 같은 염; 나트륨 도데실 술페이트 (SDS), 퍼플루오로옥타노에이트 (PFOA), 퍼플루오로옥탄술포네이트 (PFOS), 암모늄 라우릴 술페이트 (ALS), 나트륨 라우릴 에테르 술페이트 (SLES), 알킬 벤젠 술포네이트, 세틸 트리메틸암모늄 브로마이드 (CTAB), 세틸피리디늄 클로라이드 (CPC), 폴리에톡실화 탈로우 아민 (POEA), 벤잘코늄 클로라이드 (BAC), 벤제토늄 클로라이드, 도데실 베타인, 코카미도프로필 베타인 및 코코암포글리시네이트와 같은 양이온성, 음이온성 또는 양쪽이온성 계면활성제를 포함할 수 있다.
부형제로서 작용할 수 있는 비이온성 화합물의 예는 플루로닉(Pluronic), 트윈(Tween), AMP, 및 계면활성제의 브리지 계통 (Brij family); 및 생물학적 공급원으로부터 얻어진 계면활성제, 예를 들어, 올레산, 소르비탄 트리올레에이트, 소르비탄 모노올레에이트, 레시틴, 코카미드 MEA, 코카미드 DEA 및 코카미도프로필 베타인과 같은 천연 또는 반합성 계면활성제를 포함한다. 계면활성제 (이온성 및 비이온성 모두)는 계면 에너지를 감소시킬 수 있으며 더 넓은 영역에 걸쳐 연고 또는 액체의 퍼짐을 촉진할 수 있다.
본 발명의 일부 실시태양에서, 용매 부형제는 NO-방출 화합물 및 다른 부형제의 수송체 비히클로서 사용될 수 있다. 중합체 사슬이 용매와 상호작용할 수 있으며 약제에 점탄성을 부여할 수 있는 망상구조를 형성하는 팽윤을 거칠 수 있다. 본 약제의 일부 실시태양에서, 용매는 적용시 봉입된 NO-방출 화합물과 함께 중합체의 잔여 필름을 남기고 증발할 수 있다.
용매 부형제의 예는 디메틸 이소소르비드, 프로필렌 글리콜, 글리세롤, 이소프로판올, 에탄올, 에틸렌 글리콜, 폴리에틸렌 글리콜, 에톡시디글리콜 또는 이들의 혼합물을 포함한다. 소수성 제형에서 유용할 수 있는 예시적인 용매 부형제는 이소도데칸, 이소데실 네오펜타노에이트, 부틸렌 글리콜, 펜틸렌 글리콜, 헥실렌 글리콜, 메톡시폴리에틸렌글리콜, 시클로펜타실록산, 시클로테트라실록산, 디메티콘, 카프릴릴 메티콘 또는 이들의 혼합물을 포함할 수 있다.

치료 방법
위에 논의된 바와 같이, 본 발명의 일부 실시태양에 따라 피부에 기체 상태의 산화질소 및/또는 하나 이상의 산화질소 공급원을 적용함으로써 대상자의 피부에서 피지 생성을 감소시키는 방법이 제공된다. 여드름 감소는 여드름의 양 또는 중증도의 시각적 감소에 의해 및/또는 대상자에 의해 확인되듯이 여드름과 관련된 통증 또는 불편의 감소에 의해 확인될 수 있다. 기체 상태의 산화질소 및/또는 하나 이상의 산화질소 공급원을 피부에 적용함으로써 대상자의 피부에서 피지 생성을 감소시키는 방법은 피부에서 피지 과생성을 겪고 있는 그러한 대상자들에게 사용될 수 있다. 일부 경우에서, 특히 피지 과생성을 겪는 그러한 대상자들에게서 피부에서의 피지 생성을 줄이는 것이 여드름을 감소, 제거 또는 예방할 수 있다.

본 발명의 방법 실시태양으로 치료하기 적합한 대상자는 조류 및 포유동물 대상자를 포함하나 이에 국한되지 않는다. 본 발명의 포유동물은 개, 고양이, 소, 염소, 말, 양, 돼지, 설치류 (예를 들어, 래트와 마우스), 토끼, 영장류, 인간 등과 자궁 내 포유동물을 포함하나 이에 국한되지 않는다. 본 발명에 따라 치료 받을 필요가 있는 임의의 포유동물 대상자가 적합하다. 양 성별 및 임의의 발달 단계 (즉, 신생아, 유아, 유년, 청소년, 성인)에 있는 인간 대상자가 본 발명에 따라 치료될 수 있다.
본 발명에 따른 예시적인 조류는 닭, 오리, 칠면조, 거위, 메추라기, 꿩, 주조류 (예를 들어, 타조) 및 길들인 새 (예를 들어, 앵무새와 카나리아) 및 알 속의 새를 포함한다.
본 발명은 또한 동물 대상자, 특히, 수의학 목적과 약물 선별 및 약물 개발 목적의 마우스, 래트, 개, 고양이, 가축 및 말과 같은 포유동물 대상자에서 수행될 수 있다.

본 발명의 일부 실시태양에서, 제약학상 허용되는 조성물은 분무식 전달을 통해 피부에 투여될 수 있다. 비수성 전달 추진제가 디아제늄디올레이트 변형 화합물과 같은 물에 민감한 NO-방출 화합물에 사용될 수 있다. 또한, 일부 실시태양에서, 약제의 특정 요소가 약제의 적용에 앞서 어느 시점에서 분리될 수 있다. 예를 들어, 물에 반응하는 NO-방출 화합물은 적용시 (예를 들어, 분무를 통해 또는 겔 적용)까지 수성 요소 또는 추진제로부터 분리되어 저장될 수 있다. 일부 실시태양에서, NO-방출 화합물은 적용에 앞서 수성 구성요소와 결합할 수 있고 또는 NO-방출 화합물과 수성 구성요소는 순차적으로 피부에 적용될 수 있다.
일부 실시태양에서 니트로소티올 변형 화합물을 함유하는 연고는 열 분해 및 NO 방출을 최소화하기 위해 낮은 온도 (예를 들어, < 0 ℃)로 유지될 수 있다. 이 때, 차가운 연고가 피부에 적용될 수 있고, 상승된 피부의 온도가 NO를 방출할 수 있다. 일부 실시태양에서, 니트로소티올은 바구니 효과로 실온에서 안정하고 이후 피부에 적용시 NO를 방출하도록 약제 (예를 들어, NO 확산을 제한할 수 있는 친수성 제형) 내에 존재할 수 있다. 니트로소티올 변형 화합물을 포함하는 약제에 빛 역시 적용될 수 있다. 융제에서의 빛의 적용은 NO의 흐름을 만들기 위해 적용될 수 있다.
여기 개진된 본 발명의 많은 변형과 다른 실시태양이 앞의 기술 및 관련된 도면에서 나타난 내용의 이점을 지니면서 본 발명과 관련된 분야의 통상의 지식을 가진 자에게 떠오를 것이다. 따라서 본 발명은 개시된 특정 실시태양에 국한된 것이 아님을 이해해야 하며 그러한 변형 및 다른 실시태양은 첨부된 청구항의 범위 내에 포함되도록 의도된 것이다. 특정 용어가 여기서 사용되었다고 할지라도 그것들은 오직 포괄적이고 기술적인 의미로 쓰였으며 제한할 목적이 아니다.

실시예
피부 지방생성의 증가가 피지 생성 증가 및 여드름의 중증도 증가와 관련되어 있다는 것이 알려져 있다. 따라서 산화질소 공급원의 적용이 SEB-1 세보사이트에서 1 ㎍/mL 의 인슐린에 의해 촉진된 지방생성을 감소시키는지를 측정하기 위해 실험을 고안하였다.
실시예 1
노스 캐롤라이나 더럼의 노반, 인크.(Novan, Inc., Durham, North Carolina)로부터 입수가능한 니트리실™(Nitricil™)-NJ0109의 세포독성을 측정하였다. 니트리실™-NJ0109는 NO-방출 공축합 실리카이다. 세포독성은 셀 타이터-블루®(Cell Titer-Blue) 세포 생존능 분석 (프로메가(Promega))을 이용하여 정상 인간 각질세포 (NHEK, 론자(Lonza))에서 측정하였다. 세포는 96-웰 플레이트에서 거의 융합성이 되도록 배양하였다. 치료에 앞서, 지방생성 분석에서 이용되는 조건을 모방하기 위하여 배양 배지를 제거하였고 인슐린을 함유하는 무혈청배지로 대체하였다. 니트리실™-NJ0109 입자 (1, 0.5, 0.1 mg/mL)를 웰에 가하였고 세포 생존능을 24, 48 그리고 72 시간 후에 평가하였다 (각 시점 당 처리 당 n=3). 결과를 표 1에 나타내었다.

니트리실™-NJ0109 입자는 모든 농도 및 시점에서 각질세포보다 세보사이트에 더욱 독성이었다.
다음, 니트리실™-NJ0109 입자의 비독성 농축물의 인슐린 활성화된 SEB-1 세포에서의 지방생성에 영향을 미치는 능력을 연구하였다. 표준 배양 조건 하의 SEB-1에서 매우 낮은 지방생성 레벨이 치료로 유도된 감소를 확인할 수 있을 정도로 충분히 오르는 것을 확실히 하기 위해 세포를 인슐린 1 ㎍/mL로 유도하였다. 총 지방 생성은 중성 지방으로의 14C-아세테이트 혼입의 양을 측정함으로써 분석하였다. 표 2에 보여진 것과 같이, SEB-1 세포에 세포독성이 없는 농도에서 니트리실™-NJ0109는 24 시간 내에 17 %까지 지방생성을 감소시켰다.

NO-방출 조성물이 S-니트로글루티온(GSNO)이었고 니트리실™ 입자를 대체한 것을 제외하고는 실시예 1을 반복하였다. GSNO는 미시건 앤아버의 케이멘 화학(Cayman Chemical, Ann Arbor, Michigan)으로부터 입수가능하다. 그 결과가 표 3과 4에 나타나 있다.

- NO 공여체로서 디아제늄디올레이트 관능기를 포함하는 NO-방출 화합물을 포함하고, 여기서 NO-방출 화합물은 N-디아제늄디올레이트 공축합된 폴리실록산 거대분자를 포함하는 것인, 대상자의 피부에서 피지 생성을 감소시키기 위한 조성물.
