Description

養子T細胞療法を始めとするがん免疫療法は、対象自身の免疫系を活用してがん細胞を攻撃するため、有望ながん処置戦略である。それにもかかわらず、この手法の主要な限界は、移植されたTリンパ球の生存度および機能の低下が急速であることである。腫瘍において多数の生存可能な腫瘍特異的細胞傷害性Tリンパ球を維持するために、移入細胞とともに免疫刺激薬を共投与することが必要である。こうした薬剤は、高用量で全身投与されると、移入(すなわち、ドナー)細胞のin vivo生存度を高め、移入細胞の治療機能を向上させ、したがって、がんに対する有効性の全体としての向上につながり得るが、しかし、高用量のこのような薬剤は、生命を脅かす副作用ももたらしかねない。例えば、インターロイキン2(IL-2)のアジュバントとしての使用によって、黒色腫の養子T細胞療法は大いに支持され、IL-2は、移入T細胞に対する重要なアジュバントシグナルとなるが、重度の炎症性の用量制限毒性も惹起し、調節性T細胞(Treg)も増大させる。移入細胞に対するアジュバント活性に注目した一手法は、移入細胞を、自らの支持因子を分泌するように遺伝子操作することである。技術的な問題および課題に加えて、遺伝子操作されたTリンパ球を大量生産するには高い費用がかかるために、これまで、臨床応用におけるこの方法の可能性には大きな限界があった。
便宜上、明細書、実施例、および添付の特許請求の範囲において用いる一定の用語をここにまとめる。別段定義しない限り、本明細書で使用するすべての技術用語および科学用語は、本開示が属する分野の業者に一般に理解されているのと同じ意味を有する。

図1Aに示すとおり、本明細書で開示する1種または複数の架橋剤を使用して様々な治療用タンパク質単量体を架橋することにより、バックパックまたはナノ粒子を調製することができる。この図は、ジスルフィドを含有したリンカーを示しているが、本明細書で開示する他の生分解可能リンカーを使用してもよい。

(実施例2)
リンカー2の合成

免疫細胞のバックパック負荷
目的:ヒト細胞(例えば、T細胞、CAR-T、NK細胞、他の免疫細胞)は、細胞濃度を50M/mLとして、5つの濃度のHBSS中IL15バックパックで標識することができる。標識した後、次の項目について細胞を試験することができる。
a. FACSによって測定される7-AAD染色による、生存度
b. FACSによって測定されるビーズを計数することによる、培養物の増大
c. IL15およびヒト抗IgGに対する抗体による、バックパック表面標識
IL15バックパックの解凍:
1. BP溶液が解凍された後、細胞標識実験の15分前に、それを室温まで温める。
2. BP保存溶液をHBSSで調整して、3mg/mLの最終希釈標準溶液(working solution)とする。
a. BP-用量1:3mg/mL
b. BP-用量2:1.5mg/mL

1.丸底96ウェルプレートにおいてバックパックの段階希釈を行う。
注:バックパック毒性が限定されるため、V底より丸底プレートが好ましい。
細胞洗浄およびバックパック負荷:
1.培養物から30×106個の細胞を集め、500gで5分間ペレット化する。
2.細胞上清を吸引によって除去する。
3.ペレットを10mLの予熱した(37℃)HBSS緩衝液に再懸濁することにより細胞を洗浄し、Cellometer(AOPI色素を用いる)またはトリパンブルーによって計数する。
4.500gで5分間遠心分離する。
5.上清を吸引し、細胞ペレットを、予熱した(37℃)HBSSに懸濁させて、細胞の濃度を100×106/mLとする(およそ300ulの緩衝液)。
6.10ulの細胞をバックパックまたはHBSSと共にピペットで各ウェルに移し、それをピペット操作によって穏やかに混合する。
8.細胞を37℃で1時間インキュベートする。
9.180uLの予熱した完全細胞培地(血清を含有する)を各ウェルに加える。
10.細胞を500gで5分間ペレット化し、マルチウェルマニホールドを用いて培地を吸引する。
11.ステップ9および10のように、細胞を200uLの完全培地(full media)でもう2回洗浄し、細胞をペレット化し、上清を吸引する。
12.3回目の洗浄後、各試料からの細胞を200uLの完全培地に再懸濁する。細胞は、約5×106細胞/mLの密度にすべきであり、プレーティングの際にさらに1:10希釈する必要がある。
13.20ulの細胞懸濁液を96ウェルU底から96ウェル平底組織培養プレートに移し、次いで、(サイトカインが加えられていない)180ulの細胞培地を加えることにより細胞を1:10希釈して、5×105細胞/mLの最終プレーティング密度を実現する。
14.ステップ13を別の3枚の96ウェル平底プレートにおいてさらに3回繰り返す(合計4プレート:0日目、1日目、3日目、さらなる時点のために分割されるX日目)。

フローサイトメーターにおいて7-AADおよびCountBright計数用ビーズを使用して、細胞を計数する。
1.各時点において、96ウェル平底プレートをインキュベーターから取り出し、上下のピペット操作によって細胞を培地に再懸濁させる。
2.20uLの細胞溶液を96ウェルV底プレートに移す。
3.各ウェルに、20uLの「CountBrightビード溶液」を加える。
CountBrightビード溶液は、次のものを含有する(1つのウェルを標識するための体積を以下に示す)。
a. 19.6ulのCountBrightビード保存液
b. 0.4ulの100倍7AAD(7-AAD、LifeTech、A1310、10ug/mLが100倍である)
4.これらのステップを、培養後1、3、およびX日目に繰り返して、生存度および増大を評価する。
0日目および1日目に、IL-15バックパックの表面レベルを、抗IL15および抗ヒトIgG抗体を使用しながら、フローサイトメトリーによって分析する。
1.96ウェル平底プレートをインキュベーターから取り出し、上下のピペット操作によって細胞を培地に再懸濁させる。
2.100uLの細胞溶液を新たなV底96ウェルプレートに移す(これが、約50,000個の細胞を含有すべきである)。

4.細胞を40uLの「抗体細胞表面染色溶液」に再懸濁させる。

a. 0.4uLのマウス抗ヒトIgG BV421-Biolegendカタログ番号409318、1:100希釈
b. 0.4uLの抗IL15 PE:R&DSystemsカタログ番号IC2471IP、1:100希釈
c. 0.4uLの100倍7AAD(7-AAD、LifeTech、A1310、10ug/mLが100倍である)
d. 38.8uLのMACS緩衝液
5.細胞を室温で10分間インキュベートする。
6.160uLの冷MACS緩衝液を各ウェルに加え、細胞を500gで5分間ペレット化し、吸引する。
7.細胞を200uLの冷MACS緩衝液でさらに1回洗浄する。
8.ウェルあたり30uLのMACS緩衝液に再懸濁し、フローサイトメーター(HTSモード)で分析する。

IL-15バックパックは、制御され、集中したIL-15放出によって推進される養子移入後に、T細胞の自己分泌刺激および増大をもたらす
CD8およびNK細胞増大の強力な刺激因子であるインターロイキン15は、養子移入されたT細胞の抗腫瘍活性を推進しうる。しかし、全身送達では、T細胞の増大、生着、および抗腫瘍活性を推進するのに十分な用量が安全に提供されない。
Deep IL-15プライミングPMEL T細胞の薬理活性
Deep IL-15とは、切断可能な架橋剤(リンカー2)によって接続され、ポリエチレングリコール(PEG)-ポリリシン30ブロックコポリマー(PK30)で非共有結合性にコーティングされた、会合した2つのIL-15分子を有するヒトIL-15受容体α-スシドメイン-Fc融合ホモ二量体(IL15-Fc)の多量体を指す。詳細には、Deep IL-15は、加水分解可能架橋剤(CL17)によって接続され、ポリエチレングリコール(PEG)-ポリリシン30ブロックコポリマー(PK30)で非共有結合性にコーティングされた、ヒトIL15-Fc単量体の多量体である。IL15-Fc単量体は、それぞれが、IL-15の分子に非共有結合性に結合したIL-15受容体α-スシドメインと融合させた、エフェクターによって減弱されたIgG2 Fcバリアントからなる、2つのサブユニットからなる。Deep IL-15プライミングT細胞は、標的細胞を高濃度のDeep IL-15とともにインキュベートする負荷過程を経て生成される。この過程で、Deep IL-15は、静電的相互作用によって細胞と会合するようになり、内部移行して、Deep IL-15の細胞内レザバーを作出する。Deep IL-15は、こうしたレザバーから、架橋剤の加水分解によって、生理活性IL15-Fcをゆっくりと放出する。IL15-Fcのこの長期間にわたる放出によって、Deep IL-15プライミングT細胞の増殖および生存が促進されて、標的化され、制御可能であり、時間依存的な免疫刺激が実現される。
B16-F10腫瘍樹立および腫瘍測定
研究-12日目に、雌C57BL/6マウス(Jackson Labs)の剪毛した右脇腹に、B16-F10黒色腫腫瘍細胞(0.2×106)を皮内注射した。1週間に2~3回、体重を記録し、腫瘍寸法(長さ[L]および幅[W]、略語一覧において定義したもの)をカリパスで測定した。腫瘍体積を、式:W2×L×π/6を使用して算出した。
14匹の雌トランスジェニックPMELマウス(Jackson Laboratories、Bar Harbor、ME)の脾臓およびリンパ節(鼠径、腋窩、および頚部)から、PMEL細胞を単離した。脾臓およびリンパ節は、GentleMACS Octo Dissociator(Miltenyi Biotech、Auburn、CA)で加工処理し、40μmのストレーナに通した。細胞を遠心分離によって洗浄し、IMACSナイーブCD8a+単離キット(Miltenyi Biotech)ならびにMultiMACS cell24ブロック(Miltenyi Biotech)およびセパレータ(Miltenyi Biotech)を、製造者のプロトコールに従い、18本のカラムで使用して、CD8a+細胞を精製した。非CD8a+細胞をアフィニティーカラムによって除去し、CD8a+T細胞をカラム溶出液に集めた。CD8a+細胞の純度をフローサイトメトリーによって確認した。
5mLのPMEL細胞(100×106細胞/mL)を5.5mLのDeep IL-15(1.36mg/ml)と混合し、回旋させながら37℃で1時間インキュベートして、Deep IL-15プライミングPMEL細胞を作出した。Deep IL-15プライミングPMEL細胞を遠心分離(500g)によって洗浄し(3回、最初に培地で、次いでHBSSで2回)、計数した。Deep IL-15プライミングPMEL細胞を、50×106細胞/mLの濃度で再懸濁した。群5Aおよび5Bのマウスに、この調製物200μLを注射して、マウス1匹当たり合計で10×106個のDeep IL-15プライミングPMEL細胞とした。PMEL細胞(100×106細胞/mLを15mL)を15mLのHBSSと混合し、回旋させながら37℃で1時間インキュベートし、遠心分離(500g)によって洗浄し(3回、最初に培地で、次いでHBSSで2回)、計数した。PMEL細胞を、50×106細胞/mLの濃度で再懸濁した。群2Aおよび2Bのマウスに、この調製物200μLを静脈内注射して、マウス1匹当たり合計で10×106個のPMEL細胞とした。群3Aおよび3Bのマウスには、この調製物200μLを静脈内注射して、マウス1匹当たり合計で10×106個のPMEL細胞とし、IL15-Fc(50μlのHBSS中10μg/マウス、ロット#TS0)の眼窩後方注射を施した。39%という平均負荷効率に基づき、10×106個のPMEL細胞と会合するIL15-Fcの合計量は、58.5μgになるが、これは、群3Aおよび3BにおいてIL15-Fc(10μg)の注射によって全身に送達される量の5.85倍の高さである。
Fc-IL15酵素結合免疫吸着アッセイ(ELISA)を使用して、投与後2時間、D1、2、4、および10の時点で集められた試料中のIL15 Fc濃度を求めた。ELISAプレートを、4℃で終夜、ヤギ抗ヒトIgG Fc捕捉抗体でコーティングした。プレートを洗浄し、30℃で少なくとも2時間、試薬希釈剤でブロックした。プレートを洗浄し、(試薬希釈剤中に希釈された)試料およびIL15-Fc標準物質(二通り、試薬希釈剤中に31~2000pg/mL)をウェルに加え、プレートを37℃で1時間インキュベートした。プレートを洗浄した後、ビオチン-抗IL15検出抗体を加え、37℃で1時間インキュベートした。プレートを洗浄し、ストレプトアビジン-HRPとともに37℃で20分間インキュベートした。プレートを洗浄した後、3,3’,5,5’-テトラメチルベンジジン(TMB)基質溶液を加え、反応を停止するまで、暗所にて室温で20分間インキュベートした。プレートをマイクロプレートリーダー(450nm)に読み取らせた。
ThermoFisher ProcartaPlexマウス高感度パネル5plex カタログ#EPXS0S0-22199-901キットを製造者のプロトコールに従って使用し、試料をBio-Plex200システムで分析した。血清を氷上で解凍し、20μLの血清を、IFN-γ、TNF-α、IL-2、IL-4、およびIL-6レベルについて試験した。少数の試料において、20μLの血清が利用可能でなかったため、より少ない体積を利用した。希釈倍率を調整して、検量線に従って濃度を算出した。GraphPad Prismにおいて統計分析を行った。
臨床化学
血清試料において臨床化学パラメーターを測定した。図9は、投与後D1およびD4の時点で、ナイーブマウスについて統計的に有意な変化が認められた、臨床化学パラメーターを示す。投与後D1の時点で、PMEL+IL15-Fc群における、Deep IL-15プライミングPMEL群を基準としたアルブミンレベルの有意な低下(p<0.05)、ならびに、ビヒクル対照およびDeep IL-15プライミングPMELの両方との比較による、血中尿素窒素(BUN)レベルの有意な低下(両方についてp<0.05)が認められた。投与後D4の時点で、PMEL+IL15-Fc群は、アルブミン(他のすべての処置群に比べてp<0.05)、総タンパク質(ビヒクル対照に比べてp<0.05)、グルコース(Deep IL-15プライミングPMELに比べてp<0.05)、アルブミン/グロブリン(ALB/GLOB)比(ビヒクル対照に比べてp<0.05、PMELおよびDeep IL-15プライミングPMELに比べてp<0.01)の有意な低下を示した。加えて、PMEL+IL15-Fc群は、コレステロールレベルの有意な増大(ビヒクル対照およびDeep IL-15プライミングPMELに比べてp<0.05)を示した。すべての処置群が、ビヒクル対照に比べて、カルシウムレベルの低下に向かう傾向を示し、この傾向は、PMEL群で統計的に有意であった(p<0.05)。Deep IL-15プライミングPMEL群は、総ビリルビン(ビヒクル対照およびPMELに比べてp<0.05)およびリン(PMELに比べてp<0.05)の統計的に有意な変化を示した。
Luminex 5-plexキットを使用して、投与後2時間、24時間、および96時間の時点で、血清サイトカイン(IFN-γ、IL-2、IL-4、IL-6、およびTNFα)を測定した。ナイーブ非腫瘍保持マウスでは、PMEL+IL15-Fc群におけるIFN-γのレベルが12.8±3.7pg/mLであったが、Deep IL-15プライミングPMEL群においては、IFN-γが定量下限(LLOQ=0.06pg/mL)を下回った(図11)。腫瘍保持マウスでは、平均で、PMEL+IL15-Fc群におけるIFN-γ濃度(20.5±0.5pg/mL)が、Deep IL-15プライミングPMEL群(0.5±0.1pg/mL)に比べて、41倍の高さであった。他の群に比べて高いレベルのIL-2、IL-6、およびTNFαも、PMEL+IL15-Fc群で見られた。
サンドイッチELISA(抗Fc捕捉抗体に続いて抗IL15検出抗体)を使用して、PMEL+IL15-Fc(10μg)およびDeep IL-15プライミングPMEL(58.5ugのIL15-Fcを収容する)を注射したマウスの血中IL15-Fcを測定した。
D0(投薬当日)に、腫瘍は、およそ140mm3の平均体積に達していた。投与後D4の時点で、すべての処置群において、ビヒクル対照に比べて、腫瘍成長の統計的に有意な阻害が認められ(p<0.0001)、この差は、時間とともにより顕著になった(図13、左のパネル)。研究D16には、ビヒクル対照群では2/5匹の動物しか残っていなかったが(他の動物は、甚大な腫瘍負荷のために犠牲になった)、処置群のそれぞれでは、4/5匹の動物が残っていた。ビヒクル対照群における腫瘍体積は、他のすべての群と有意差があった(p<0.0001)。PMEL群における腫瘍体積は、Deep IL-15プライミングPMELおよびPMEL+IL15-Fc群における腫瘍体積より有意に(p<0.05)大きかった。D16において、PMEL+IL15-Fc群とDeep IL-15プライミングPMEL群における腫瘍成長の阻害には、互いに差がなかった(図13、左および右のパネル)。投与後D1、4、10、および16において、屠殺後に腫瘍を秤量した(n=2~5、各群、各時点)。腫瘍重量を図14に示す。
研究の主要な所見を以下に要約する。
1.Deep IL-15プライミングPMEL細胞は、10×106細胞という投与用量で十分に許容された。
2.PMEL、PMEL+IL15-Fc、およびDeep IL-15プライミングPMEL細胞のいずれも、ビヒクル対照に比べて腫瘍成長が阻害される結果となった。阻害は、PMELに比べて、PMEL+IL15-FcおよびDeep IL-15プライミングPMEL細胞で高かった。
3.毒物学的関連のある臨床化学パラメーター変化は、PMELまたはDeep IL-15プライミングPMEL細胞のいずれでも認められなかった。多少の変化が、PMEL+IL-15 Fcで認められた。
4.血清IFN-γ、TNF-α、またはIL-6の変化は、PMELまたはDeep IL-15プライミングPMEL細胞では、いずれの時点でも検出されなかった。PMEL+IL15-Fcでは、24時間の時点で、血清IFN-γおよびTNF-αの有意な変化が認められた。PMEL+IL15-Fcでは、IL-6が、2時間(非腫瘍保持(ナイーブ)マウスのみ)および24時間の時点で増大した。
5.Deep IL-15プライミングPMEL群におけるIL15-Fcの血清レベルは、PMEL+IL15-Fc群において検出されたレベルに比べて3000分の1を下回る低さであり、PMEL+IL15-Fc群に比べて、体重減少がなく、CBCおよび内因性免疫細胞(CD8+、NK1.1+、およびCD4+細胞)の有意な変化がなく、IFN-γ血清レベルおよび関連する薬理学的変化が低減していることと一致した。

Claims (14)
- 互いに可逆的に架橋された複数の治療用タンパク質単量体を含むタンパク質クラスター、および
薬学的に許容される担体または賦形剤
を含む治療用組成物であって、
前記タンパク質クラスターは、動的光散乱によって測定されるサイズが直径30nm~1000nmの間であり、
前記タンパク質クラスターは、複数の生分解性架橋剤と前記複数の治療用タンパク質単量体を反応させることによって入手されうるものであり、
前記架橋剤は、
であり、そうすることで前記治療用タンパク質単量体を前記タンパク質クラスターに架橋させ、それを必要とする対象に投与された後、前記タンパク質クラスターの前記架橋剤の部分は、生理的条件下で分解されて、前記タンパク質クラスターから前記治療用タンパク質単量体を放出する、
