Description
米国特許出願公開第2003165875A号に説明されているように、ヒトIgG1定常領域と、マウスTREM-1の細胞外ドメイン又はヒトTREM-1の細胞外ドメインとの間の融合蛋白質は、マウスの内毒素血症に対する作用を示す。
先に本発明者らは、TREM-1の可溶型(sTREM-1)を同定し、これを敗血症ショック患者の血清試料中においては有意なレベルで認めたが、対照においては認めなかった。本願明細書においても説明されたように、本発明者らは、敗血症時の炎症の変化におけるその推定上の役割を調べた(Gibotらの論文、Ann. Intern. Med.、141(1):9-15(2004)、及びGibotらの論文、N. Engl. J. Med.、350(5):451-8(2004))。
本発明者らは、sTREM-1は、LPSによりin vitroにおいて、更には敗血症ショックの実験モデルに関連した動物の血清中に、活性化された単球により分泌されることを認めている。In vitro及びin vivoの両方において、sTREM-1の短い高度に保存されたドメインを模倣している合成ペプチドは、ヒト単球によるサイトカイン生成を減弱し、敗血症動物を過剰反応及び死亡から保護する。これらのペプチドは、前炎症性サイトカインの有害な作用を単に妨害することのみではなく、ダウンレギュレーションすることにおいても効率的である。これらのデータは、TREM-1ペプチドによるTREM-1のin vivo変化は、感染症、例えば敗血症もしくは敗血症ショックの治療、又は敗血症-様状態の治療のための価値のある治療道具であることを明らかにしている。
従って本発明は、TREM-1蛋白質のCDR2又はCDR3由来の1種以上の配列を含むポリペプチドを提供する。好ましくは、該ポリペプチドは、該TREM-1蛋白質の30個未満の連続的なアミノ酸を含む。
表2は、ヒトTREM-1アミノ酸配列NP_061113を示している。下線を付けたアミノ酸は、Radaevらの論文(Structure(Camb.)、11(12)1527-1535(2003))に記された、ヒトTREM-1相補性決定領域(CDR)2(RPSKNS;[配列番号:20])及び3(QPPKE [配列番号:21])に広がっている。
表3は、マウスTREM-1アミノ酸配列NP_067381を示している。下線を付けたアミノ酸は、マウスTREM-1相補性決定領域(CDR)2(RPFTRP;[配列番号:22])及び3(HPPND;[配列番号:23])に広がっている。
ある実施態様において、本発明のポリペプチドは、配列番号7でないか又はこれを含まない。

本願明細書において使用される用語「骨髄細胞」は、顆粒球(好中球、好酸球、及び好塩基球)、単球、マクロファージ、及びマスト細胞を含む一連の骨髄-由来の細胞系統を意味する。更に骨髄起源の末梢血樹状細胞、並びに適当な培養条件下in vitroにおいて単球由来の樹状細胞及びマクロファージも含まれる。
典型的には本発明のポリペプチドの適応は、敗血症又は敗血症ショックである。
典型的には、「実質的配列同一性」を有するペプチド/アミノ酸配列は、少なくとも所望の活性に関与していることがわかっている領域にわたり、少なくとも50%、より好ましくは少なくとも80%配列が同一である配列である。最も好ましくは、末端でない、5個を超えない残基が異なる。好ましくは少なくとも前述の領域における配列の多様性は、「保存的修飾」の形である。
本願明細書において使用される用語「ホモログ」は、抗原性/免疫原性及び炎症調節活性を含む、共通の生物学的活性、及び/又は構造ドメインを有する、並びに本願明細書において定義された十分なアミノ酸を有する、多くの一連のペプチド又はポリペプチドを意味する。このようなホモログは、同じ又は異なる動物種のいずれかに由来することができる。
本願明細書において使用される用語「誘導体」は、別の方法で修飾された、すなわち非-天然のアミノ酸を含むペプチド又は蛋白質への、好ましくは生体活性を有する、いずれかの種類の分子の共有結合による、所定のペプチド又は蛋白質の変異を意味する。
好ましくは、このようなホモログ、変種、及び誘導体は、例えばTREM-1受容体の活性のアンタゴニストとして作用することにより、敗血症、敗血症ショック又は敗血症-様状態を治療するか、もしくは、敗血症、敗血症ショック又は敗血症-様状態の実験モデルにおいて活性があることが可能である。
語句「細胞物質を実質的に含まない」は、ポリペプチド/蛋白質が、そこから単離された又は組換えにより作出された細胞の細胞成分から分離されているポリペプチド/蛋白質の調製物を含む。従って細胞物質を実質的に含まないポリペプチド/蛋白質は、夾雑蛋白質を約30%、20%、10%、5%、2.5%、又は1%(乾量について)未満有するポリペプチド/蛋白質の調製物を含む。ポリペプチド/蛋白質が組換えにより作出される場合は、同じく培養培地を実質的に含まないことが好ましく、すなわち培養培地は、蛋白質調製物容量の約20%、10%、又は5%未満である。ポリペプチド/蛋白質が化学合成により生成される場合は、化学前駆体又は他の化学物質を実質的に含まないことが好ましく、すなわち、その蛋白質の合成に関連する化学前駆体又は他の化学物質から分離されている。従って、ポリペプチド/蛋白質のそのような調製物は、関心のあるポリペプチド/蛋白質断片以外の化学前駆体又は化学物質を約30%、20%、10%、5%(乾量について)未満有する。好ましい本発明の実施態様において、ポリペプチド/蛋白質は、単離又は精製されている。
本発明は更に、融合蛋白質を作成するために、本発明のポリペプチド又はそれらの断片が、異種ポリペプチド(すなわち、無関係のポリペプチド又はそれらの一部、好ましくはポリペプチドの少なくとも10、少なくとも20、少なくとも30、少なくとも40、少なくとも50、少なくとも60、少なくとも70、少なくとも80、少なくとも90又は少なくとも100個のアミノ酸)へ、組換えにより融合された又は化学的に複合された(共有的及び非-共有的の両方を含む)、融合蛋白質を包含している。この融合は、必ずしも直接である必要はなく、リンカー配列を介して生じることができる。
このような融合蛋白質又はスカフォールドベースの蛋白質は、本発明のポリペプチド又はそれらの断片を認識する特異的抗体の作成のための免疫原として使用することができる。別の好ましい実施態様において、リガンドとその受容体の間のin vivoにおける相互作用を阻害するために、このような融合蛋白質又はスカフォールドベースの蛋白質を対象へ投与することができる。このような相互作用の阻害は、敗血症及び敗血症ショックに関連したある種の細胞反応をブロック又は抑制するであろう。
本願明細書において使用される用語「医薬として許容できる希釈剤、担体又は賦形剤」は、医薬投与に適合性のある、溶媒、分散媒、コーティング、抗菌剤及び抗真菌剤、等張化剤及び吸収遅延剤などのいずれか及び全てを含むことが意図されている。医薬として活性のある物質のためのこのような媒体及び物質の使用は、当該技術分野において周知である。従来の媒体又は物質がこの活性化合物と不適合である場合を除いて、組成物中のそれらの使用が企図されている。補助的活性化合物も、この組成物に混入することができる。

全身投与は、経粘膜又は経皮手段によっても行うことができる。経粘膜又は経皮投与に関して、浸透されるべき障壁に適した浸透剤が、製剤において使用される。このような浸透剤は、一般に当該技術分野において公知であり、例えば経粘膜投与のためには、デタージェント、胆汁酸塩、及びフシジン酸誘導体を含む。経粘膜投与は、鼻腔内スプレー又は坐剤により実現することができる。経皮投与に関して、活性化合物は、当該技術分野において一般に公知である軟膏剤、塗り薬(salve)、ゲル剤又はクリーム剤に製剤される。これらの化合物は、直腸送達のために、坐剤(例えば、通常の坐薬用基剤、例えばココアバター及び他のグリセリドを伴う)又は持続性浣腸剤の形で調製することもできる。
抗体に関して、好ましい用量は、0.1 mg/kg〜100 mg/kg体重(一般に、10 mg/kg〜20 mg/kg)である。抗体が脳に作用する場合は、用量50 mg/kg〜100 mg/kgが通常適している。一般に、部分的ヒト抗体及び完全なヒト抗体は、ヒトの体内で他の抗体よりも長い半減期を有する。従って、より低い用量及びより少ない投与回数が可能になることが多い。脂質化のような修飾を用い、抗体を安定化させ、取込み及び組織浸透(例えば脳への)を促進することができる。抗体の脂質化の方法は、Cruikshankらの論文(J. Acquired Immune Deficiency Syndromes and Human Retrovirology、14:193(1997))に記載されている。
本発明は更に、本発明の発明的ペプチドもしくはポリペプチド、又は本発明のポリペプチドを模倣するそれらの抗体もしくは断片を、好ましくは例えば敗血症、敗血症ショック又は敗血症-様状態の治療における使用説明書と共に備えたキットを提供する。
本発明は更に、前述のスクリーニングアッセイにより同定された新規物質及び本願明細書に説明されたような治療のためのそれらの使用に関する。
本発明の各態様の好ましい特徴は、必要な変更を加え、互いの態様に適応される。
本発明は、ここで下記の限定的でない実施例を参照し、図面を基に説明される。
下記基準に合致するTREM-1ペプチドを合成した:i)ヒト及びマウスTREM-1の間の最高の相同性、並びにTREM-2との最低の相同性;ii)TREM-1の相補性決定領域(CDR)に広がるペプチド。公表されたTREM-1の結晶構造、及び抗体との類似点に従い、これらの残基は、おそらくコグネイトリガンド認識に関与しているであろう(Radaevらの論文、Structure(Camb)、Dec;11(12):1527-35(2003)、及びKelkerらの論文、J. Mol. Biol.、Sep 24;342(4):1237-48(2004))(図1参照)。1種のペプチド(P1)はCDR2領域で、3種のペプチド(P2、P4及びP5)はCDR3領域でデザインした。第四のペプチド(P3)は、V-型免疫グロブリン(Ig)-様ドメイン(Ig-V)を膜貫通ドメインに接続している首領域でデザインした。TREM-1とTREM-2の間の高い配列相同性のために、CDR1領域ではペプチドをデザインしなかった。

CLPにおいて試験したTREM-1由来のペプチドの中で、ペプチドP1、P2、及びP5は、保護活性を示している。可能性のある作用機序は、TREM-1由来のペプチドの、TREM-1/TREM-1リガンド相互作用を妨害する能力であろう。この疑問に対処するために、本発明者らは、競合実験をTREM-1リガンド陽性細胞:CLP処置したマウスから得たPEC(腹膜滲出細胞)について行った。
(末梢血からの単球の調製)
末梢血試料10mLを、実験室スタッフ5名の健常志願者ドナーからEDTA-Kで採取した。RPMI(Life Technologies社、グランドアイランド、NY)中に希釈(v/v)後、Ficoll勾配(Amersham Pharmacia社、ウプスラ、スウェーデン)上、室温で30分間遠心し、PBMCを分離した。勾配の上部に回収された細胞を、洗浄し、計測した。リンパ球浮遊液を涸渇するために、次に細胞を、24-ウェル平底組織培養プレート(Corning社、コーニング、NY)に、濃度5x106/mLで播種し、2時間かけて37℃で接着させた。得られるリンパ球浮遊液を廃棄し、接着している単球細胞を、10%FCS(Invitrogen社、セルギー、フランス)を補充した完全培地(RPMI 1640、0.1mMピルビン酸ナトリウム、2mMペニシリン、50μg/mLストレプトマイシン;Life Technologies社)において、5%CO2恒温培養器中で37℃で維持した。
Gen-BankのヒトTREM-1配列(寄託番号AF287008)及びマウスTREM-1配列(AF241219)を使用し、ペプチド「P5」(LQVTDSGLYRCVIYHPP;[配列番号:7])を、C-末端がアミド化されたペプチドとして化学的に合成した(Pepscan Systems社、レーリスタッド、オランダ)。正確なペプチドを、収率99%以上で得、これは計算した質量は1962Daであるのに対し測定した質量は1961Daであり、分取精製後均質であることが、質量分析及び分析用逆相高速液体クロマトグラフィーにより確認された。P5と同じアミノ酸を含むが配列の順番が異なるペプチド「P5sc」(TDSRCVIGLYHPPLQVY;[配列番号:9])を、同様に合成し、「コントロールペプチド」として利用した。
活性化のために、単球を、大腸菌LPS(O111:B4、1μg/mL、Sigma-Aldrich社、ラベルピレール、フランス)の存在下で培養した。細胞生存力を、トリパンブルー排除及び乳酸デヒドロゲナーゼ放出の測定により評価した。一部の実験において、この刺激は、TNF-α(5〜100 ng/mL、R&D Systems社、リル、フランス)、lL-1β(5〜100 ng/mL、R&D Systems社)、rIFN-γ(最大100 U/mL、R&D Systems社)、rlL-10(500 U/ml、R&D Systems社)又はP5もしくはコントロールペプチド最大100 ng/mLを組合せて行った。単球をTREM-1により活性化するために、抗-TREM-1アゴニストモノクローナル抗体(R&D Systems社)を下記のように添加した:平底プレートを、1ウェルにつき抗-TREM-1 10μg/mLで予めコートした。リン酸緩衝生理食塩水(PBS)で洗浄した後、単球浮遊液を、先と同じ濃度で添加した。一部の実験は、プロテアーゼインヒビター(PMSF及びプロテアーゼカクテルインヒビター;Invitrogen社)の存在下で行った。細胞-非含有上清を、TNF-α及びlL-1βの生成について、製造業者(BD Biosciences社、サンディエゴ、USA)の推奨に従いELISAによりアッセイした。単球のNF-kB活性に対するP5の作用を説明するために、ELISA-ベースのアッセイを行った(BD Mercury(商標)Transfactor Kit、BD Biosciences社)。単球を、大腸菌LPS(O111:B4、1μg/mL)、及び/又はアゴニスト抗-TREM-1モノクローナル抗体(10μg/mL)、及び/又はP5(100 ng/mL)の存在下で、24時間培養した。次に全細胞抽出物を調製し、NF-kB p50及びp65のレベルを、製造業者の推奨に従い決定した。全ての実験は、3つ組で行い、データは平均(SEM)で表した。
初代単球浮遊液を、前述のように培養した。これらの細胞を、大腸菌LPS(O111:B4、1μg/mL)により37℃で24時間処理した。抗-TREM-1により認識された27kDa物質の存在を確認するために、細胞-馴化培地に、抗-TREM-1モノクローナル抗体(R&D Systems社)を使用するウェスタンブロットを行った。可溶性TREM-1レベルを、別所記載(18)のように、反射型スキャナー及びQuantity One Quantitation Software(Bio-Rad社、セルギー、フランス)によりイムノドット上のバンドの光学強度を評価することにより測定した。各試料からの可溶性TREM-1濃度は、試料の光学濃度を、精製したTREM-1で作成された標準曲線参照と比較することにより決定した。全ての測定は、3つ組で行った。この技術の感度は、5 pg/mLと低いsTREM-1レベルの検出を可能にした。
総mRNAを、TRIzol試薬(Invitrogen社)を用いLPSの存在下で培養された初代単球から抽出し、Superscript RT II(Invitrogen社)を用い逆転写し、cDNAを作成した。全ての反応に使用したRT-PCR条件は、94℃、30秒/65℃、30秒/68℃、1分間を30サイクルであった。増幅は、2.5 mM MgCl2、0.2 mM dNTP、2.0 U Taqポリメラーゼ、並びに20 pM 5'及び3'オリゴヌクレオチドプライマー(Proligos社、パリ、フランス)で行った。

地域の倫理委員会が承認した後、雄のBalb/Cマウス(20〜23 g)を、無作為に群別し、LPSチャレンジ前後のP5(通常の生理食塩水500μl中)又はコントロールベクターと組合せ、大腸菌LPSで腹腔内(i.p.)処置した。一部の実験において、抗TREM-1モノクローナル抗体5μgを、LPS注射後1時間でi.p.投与した。マウスの生存力を、毎時間試験するか、又は動物を一定期間で屠殺した。血清試料を、心臓穿刺により採取し、TNF-α及びlL-1βについて、ELISA(BD Biosciences社)で、及びsTREM-1レベルについてイムノドットでアッセイした。
雄のBalb/Cマウス(7〜9週齢、20〜23 g)を、0.2 mL滅菌パイロジェン-非含有生理食塩水中のケタミン及びキシラジンのi.p.投与により麻酔をかけた。1.0 cmの腹部正中切開により盲腸を露出し、先端部の半分で(distal half)結紮し、その後G21針で2箇所に穿孔した。少量の糞便を、穿孔から排出し、開通性を確認した。盲腸を腹腔に戻し、腹部切開を二層縫合した。手術後、全てのマウスに、輸液蘇生のために、0.5 ml生理食塩水を注射し、イミペネム1.25 mg(すなわち50μg/g)を12時間毎にs.c.注射した。これらの動物を無作為に群別し、通常の生理食塩水(n=14)、コントロールペプチド(n=14、100μg)又はP5(100μg)を、H0(n=18)、H+4(n=18)又はH+24(n=18)に単回注射することにより処置した。最後の群のマウス(n=18)は、P5(100μg)を、H+4、H+8、及びH+24に反復注射することにより処置した。全ての処置は、通常の生理食塩水500μlに希釈し、i.p.投与した。本発明者らは次に、様々な用量のP5の作用を決定しようとした。この目的のために、マウス(n=15/群)を、生理食塩水又はP5の10μg、20μg、50μg、100μgもしくは200μgのCLP 後H0での単回注射により処置し、生存をモニタリングした。1群につき追加動物5匹を、CLPの24時間後に麻酔下で屠殺し、細菌数及びサイトカインレベルを決定した。2 mL RPMI 1640(Life Technologies社)を用い腹膜洗浄液を得、心臓穿刺により血液を収集した。血清中のTNF-α及びlL-1βの濃度を、ELISA(BD Biosciences社)により決定した。細菌数の評価のために、血液及び腹膜洗浄液を、連続対数希釈し、5%ヒツジ血液を補充したトリプシン処理したダイズ寒天プレートに播種した。播種後、トリプシン処理したダイズ寒天プレートを、37℃で好気性に24時間、嫌気性に48時間インキュベーションした。結果は、血液についてのCFU/mL及び腹膜洗浄液についてのCFU/マウスとして表した。
血清sTREM-1及びサイトカインレベルを、平均(±SD)として表した。P5によるLPS死亡率に対する保護は、Log-Rank検定を用い、生存曲線の比較により評価した。全ての統計解析は、Statviewソフトウェア(Abacus Concepts社、バークレー、CA)により完了し、及び両側P<0.05を有意とみなした。
(可溶型TREM-1は、大腸菌LPSによる刺激後、培養したヒト単球から放出される)
sTREM-1のin vitroにおける放出の可能性を確定するために、本発明者らは、ヒト単球をLPSで刺激し、SDS-PAGEにより馴化培養培地を分析した。LPS刺激は、時間依存型で27-kDa蛋白質の出現を誘導した(図7A)。ウェスタンブロット分析は、この蛋白質は、TREM-1の細胞外ドメインに対するモノクローナル抗体により特異的に認識されたことを明らかにした(図7A)。細胞生存力は、馴化培地中のsTREM-1の存在を誘導したLPS濃度では影響を受けず、このことは、TREM-1放出は、細胞死に起因するものではないことを示している。同様に、単球のプロテアーゼインヒビターによる処置は、TREM-1放出に影響を及ぼさなかった(図7A)。TREM-1 mRNAレベルは、LPS処置時に増加した(図7B)のに対し、TREM-1sv mRNAレベルは依然検出不能であった。このことは、TREM-1放出はおそらく、その遺伝子の転写増大に関連し、TREM-1sv発現とは無関係であることを示している。TNF-α(5〜100 ng/mL)又はlL-1β(5〜100 ng/mL)による単球の16時間の刺激は、サイトカインの用量に依存した方式で非常に少量のTREM-1放出を誘導した。IFN-γは、最大濃度100 U/mLであっても、TREM-1放出を誘導しなかった。
有意なTNF-α及びlL-1β生成は、LPSと共に培養された単球の上清において観察された。TNF-α及びlL-1β生成は、TREM-1 mAb及びLPSの両方と共に培養された細胞の方が、mAb又はLPS単独と共に培養されたものと比べて、更により高かった(図8A)。前炎症性サイトカインの誘導性放出は、培地にP5又はlL-10が補充された場合、LPS刺激後有意に低下した。P5は、濃度依存した方式で、LPS又はLPS及びmAbと共に培養された細胞からのTNF-α及びlL-1β生成を低下し、同時にLPSと共に培養した細胞からのsTREM-1放出を増加した。コントロールペプチドは、サイトカイン又はsTREM-1放出に対する作用を示さなかった(データは示さず)。際だって対照的なことに、lL-10はTREM-1及び炎症性サイトカインの両方の放出を全般的に阻害した(図8A)。LPS及びTREM-1 mAbの両方は、単球NF-kB p50及びp65の強力な活性化を誘導し、並びにLPS及びTREM-1 mAbの併用投与は、相乗作用につながっている。P5は、TREM-1の結合により誘導されたNF-kB活性化を阻害したが、LPSの作用は変更しなかった(図8B)。
sTREM-1は、マウスにおいて内毒素血症時に全身放出されるかどうかを決定するために、本発明者らは、LPS投与後の血清sTREM-1レベルを測定した。血清sTREM-1は、LPSのLD50用量投与後1時間で容易に検出可能であり、LPS処置後4〜6時間はピークプラトーレベルに維持された(図9)。
LPS致死量(LD100)投与の60分前にP5の単回用量により処置されたマウスは、用量依存方式で死亡から保護された(図10A)。P5処置を、LPS投与後まで遅延できるかどうかを調べるために、本発明者らは、LPS注射後4又は6時間でP5注射を開始した。この最大4時間遅延された処置は、LD100用量のLPSに対する有意な保護をもたらした(図10B)。遅れた死亡例は1週間にわたり生じず、このことは、P5は、LPS死亡の発生が単に遅延されるのみではなく、持続性の保護を提供することを示している。コントロールマウスは全て、死亡前に嗜眠、起毛、及び下痢を生じた。対照的にP5-処置マウスは、よく毛繕いし活動的であり、下痢をせず、活発であった。P5がマウスをLPS死亡から保護する機序を明確にするために、本発明者らは内毒素血症マウスの血清レベルのTNF-α、lL-1β、及びsTREM-1を、2及び4時間で決定した。対照と比較して、P5 100μgによる前処置は、表4に示されるように、サイトカインレベルを30%低下し、sTREM-1レベルを2倍に増加した:
更にLPS-媒介した死亡におけるTREM-1結合の役割に注目するために、マウスを、LD50用量のLPS投与と組合せて、アゴニスト抗-TREM-1 mAbで処置した。これは、死亡率の50%から100%への有意な増加を誘導した(図10C)。
敗血症ショックのより関連性のあるモデルにおけるP5の役割を調べるために、本発明者らは、CLP実験を行った(図11A)。対照群は、通常の生理食塩水又はコントロールペプチドが注射されたマウスで構成された。この複数菌による敗血症モデルにおいて、敗血症発症後24時間と遅く投与された場合にも、P5は依然、死亡に対する有意な保護をもたらした。興味深いことに、P5の反復注射は、生存により好ましい作用を有した(P<0.01)。P5の生存(図11B)及びサイトカイン生成(表5)に対する用量反応作用があった。P5は、細菌クリアランスには作用を有さなかった(図12)。
TREM-1は、単球活性化及び炎症反応に関連した最近同定された分子である(12, 14)。これは、下流のシグナル伝達事象を活性化するNK細胞受容体に関連したファミリーに属する。TREM-1のPNN及び単球/マクロファージ上での発現は、LPSにより誘導されることが示されている(16, 17)。
炎症性サイトカイン、及び特にTNF-αは、有害であると考えられるが、損なわれたTNF-α反応を伴う動物における腹膜炎の致命的問題により示されるように(9-11)、これらは敗血症において有益な作用を有するとも考えられる(5)。更に臨床試験において、TNF-αの阻害は死亡率を上昇した(8)。最後に、感染症のクリアランスにおけるTNF-αの役割が、敗血症はTNF-αアンタゴニストで治療された関節リウマチ患者における頻出する合併症であるという知見(26)により注目されている。
対照的に、TREM-1のアゴニスト抗-TREM-1モノクローナル抗体による結合は、LPS-チャレンジしたマウスにおける死亡率の劇的な増加を媒介し:これは更に、敗血症ショック時のTREM-1結合の有害な作用を強調している。
敗血症ショックの更なるモデルにおけるTREM-1ペプチドの役割を、ラットにおいてLPS及びCLP(盲腸結紮穿刺)実験を行うことにより調べた。
(材料及び方法)
(LPS-誘導した内毒素血症)
動物は、無作為に群別し(n=10〜20)、大腸菌LPS(O111:B4, Sigma-Aldrich社、リヨン、フランス)の、TREM-1ペプチド又はスクランブルペプチドとの組合せのi.p.投与により処置した。
この手法は、別所に詳細に説明されている(Mansart, A.らの論文、Shock、19:38-44(2003)参照)。簡単に述べると、ラット(n=6〜10/群)を、ケタミン(150 mg/kg)のi.p.投与により麻酔をかけた。3.0 cmの腹部正中切開により盲腸を露出し、先端部の半分で結紮し、その後G21針で2箇所に穿孔した。少量の糞便を、穿孔から排出し、開通性を確認した。盲腸を腹腔に戻し、腹部切開を二層縫合した。手術後、全てのマウスに、輸液蘇生のために、50 ml/kgの通常の生理食塩水をs.c.注射した。TREM-1ペプチド又はスクランブルペプチドを前述のように投与した。
LPS投与直後に加え、CLPの16時間後に、動脈血圧(BP)(収縮期、拡張期及び平均)、心拍数、腹部動脈血流量、及び腸間膜血流量を、別所記載の方法を用い記録した(Mansart, A.らの論文、Shock、19:38-44(2003)参照)。簡単に述べると、左頚動脈及び左頚静脈を、PE-50チューブでカニューレ挿管した。動脈BPを、圧変換器及び増幅器-記録システム(IOX EMKA Technologies社、パリ、フランス)により連続モニタリングした。血管周囲探子(Transonic Systems社、イタカ、NY)を、上腹部大動脈及び腸間膜動脈で包み、それらの各流れを流量計(Transonic Systems社)により、モニタリングした。最後の測定後(LPS実験時の4時間目、及びCLP時の24時間目)、動物を、過量のチオペンタールナトリウムのi.v.投与により屠殺した。
血液を、左頚動脈から逐次採取した。動脈血中乳酸濃度及び血液ガス分析を、自動血液ガス分析装置(ABL 735、Radiometer社、コペンハーゲン、デンマーク)において行った。血漿中のTNF-α及びlL-1β濃度を、ELISA試験(Biosource社、ニベル、ベルギー)により製造業者の推奨に従い決定した。硝酸塩/亜硝酸塩の血漿濃度を、Griess反応(R&D Systems社、アビンドン、英国)を用い測定した。
結果は、平均±SDとして表した。群間比較は、スチューデントt検定を用い行った。全ての統計解析は、Statviewソフトウェア(Abacus Concepts, CA)を用いて完了し、及び両側P<0.05を有意とみなした。
(内毒素血症モデル)
LPS投与後、動脈圧、大動脈及び腸管膜血流量は、コントロール動物(スクランブルペプチド処置したラット)において急激に低下したが、心拍数は変化しなかった(表6)。動脈圧及び動脈血流量の低下は、TREM-1ペプチド処置した動物においては2時間目まで遅延し、その時点までコントロール動物よりもより有意に高い値であった。P1とP5の処置群間に差異はなかった。対照的に、これら2種のペプチドは、腸間膜血流量の減少に対する作用を有さなかった(表6)。
予想されたように、TNF-α血漿濃度のピークは、注射後30分から1時間の間に、LPSにより誘導され、その後漸減した(図13A)。P-1ペプチド注射は、この生成に影響を示さなかったが、P5はTNF-α生成を〜30%減少した。
P1は、lL-1βピークをLPS注射後3時間目まで遅らせたが、減弱しなかった、対照的に、P5は、lL-1β放出を強力に低下した(図13B)。
硝酸塩/亜硝酸塩濃度は、対照及びP1処置動物において、LPS投与後迅速に増加したが、P5処置においては安定して続けた(図14)。
本発明者らのモデルの重症度は、CLPの完了後16〜20時間で最高であったので、本発明者らは、16時間目まで(by)動物を調べることを選択した。重要なことは、この時点以前には死亡例はなかったことである。全ての動物は輸液蘇生されたが、これらのペプチドの役割を厳密に考慮するために抗生物質は受け取らなかった。
コントロール動物において経時的な動脈圧の劇的な低下があり、並びにH24までに、収縮期、拡張期及び平均動脈圧は、各々、58±7mmHg、25±4mmHg、及び38±2mmHgであった。この減少は、P1又はP5処置ではほぼ完全に失われ、H16及びH24の間で有意差はなかった(図15)。P1及びP5処置ラットの間に差異はなかった。
コントロールラットにおいて発症した進行性代謝アシドーシスは、P1ペプチドにより減弱され、P5によりほぼ取り消された。同じ保護的傾向が、動脈血乳酸上昇において認められ、P5の作用はより顕著であった(表7)。
亜硝酸塩/硝酸塩濃度は、コントロール動物において増加したが、両方のTREM-1ペプチド処置群においては低レベルであり続けた(図17)。
まとめると、これらのデータは、TREM-1ペプチドは、1)対象動物を敗血症-関連した血行力学の悪化から効率的に保護すること;2)乳酸アシドーシスの発生を減弱すること;3)TNF-α及びlL-1βのような前炎症性サイトカインの生成を変化すること、及び4)一酸化窒素の生成を減少することを示している。従ってTREM-1ペプチドは、敗血症、敗血症ショック又は敗血症-様状態の患者の血行動態パラメータの回復に有用な可能性がある。

Claims (8)
- 配列番号1により定義されるTREM-1蛋白質のアンタゴニストとして作用し得るポリペプチド又はその誘導体であって、該ポリペプチドが、
(i)配列番号16、17、18又は19のアミノ酸配列;又は、
(ii)配列番号16、17、18又は19のアミノ酸配列であって、該ポリペプチドの1個のアミノ酸が、極性残基を別の極性残基で、親水性残基を別の親水性残基で、疎水性残基を別の疎水性残基で、正帯電した残基を別の正帯電した残基で、又は負帯電した残基を別の負帯電した残基で置換することにより保存的に置換されている、前記アミノ酸配列;からなり、
該ポリペプチドの誘導体が、グリコシル化、アセチル化、PEG化、リン酸化、アミド化、又は公知の保護基/ブロック基による誘導体化によって修飾されたものである、前記ポリペプチド又はその誘導体。 - 配列番号2により定義されるTREM-1蛋白質のアンタゴニストとして作用し得るポリペプチド又はその誘導体であって、該ポリペプチドが、
(i)配列番号3、4、6又は7のアミノ酸配列;又は、
(ii)配列番号3、4、6又は7のアミノ酸配列であって、該ポリペプチドの1個のアミノ酸が、極性残基を別の極性残基で、親水性残基を別の親水性残基で、疎水性残基を別の疎水性残基で、正帯電した残基を別の正帯電した残基で、又は負帯電した残基を別の負帯電した残基で置換することにより保存的に置換されている、前記アミノ酸配列;からなり、
該ポリペプチドの誘導体が、グリコシル化、アセチル化、PEG化、リン酸化、アミド化、又は公知の保護基/ブロック基による誘導体化によって修飾されたものである、前記ポリペプチド又はその誘導体。 - 請求項1又は2記載のポリペプチド又はその誘導体、並びに免疫グロブリン、非-古典的代替蛋白質スカフォールド、該ポリペプチド若しくはその誘導体のN末端に融合された異種シグナル配列、及びタグ配列に由来する配列からなる群から選択される異種ポリペプチドからなる融合タンパク質である、ポリペプチド。
- 敗血症、又は敗血症ショックを治療する能力を特徴とする、請求項1〜3のいずれか1項記載のポリペプチド。
- 請求項1〜4のいずれか1項記載のポリペプチドをコードし得る、単離されたポリヌクレオチド。
- 請求項5記載のポリヌクレオチドを含む、ベクター。
- 請求項1〜4のいずれか1項記載のポリペプチドを含む、医薬組成物。
- 敗血症、又は敗血症ショックの治療のための医薬品の製造における、請求項1〜4のいずれか1項記載のポリペプチドの使用。
