Abstract
Description

本明細書に記載の全ての刊行物および特許出願は、各個々の刊行物または特許出願が、具体的かつ個々に、参照することによって組み込まれることが示される場合と同様に参照することによって本明細書に組み込まれる。
本開示は、概して、医療方法および装置に関する。より具体的には、本開示は、隣接する組織層に穿刺し、結果として生じる穿孔を拡大するための方法および装置に関する。
いくつかの腔間および腔内内視鏡下手技は、アンカまたはステントの精密な留置を要求する。例えば、いくつかの手技は、第1の器官または構造、例えば、食道、胃、十二指腸、小腸、または大腸を通して、胃腸(GI)管に進入し、アンカまたはステントを隣接する器官および管腔または組織構造、例えば、GI管、胆管、膵管、胆嚢、膵臓、嚢胞、偽性嚢胞、膿瘍、および同等物の隣接する部分に送達することによって行われてもよい。主に、GI管内での使用が意図されるが、そのような方法および装置はまた、尿路、例えば、膀胱および尿管、肺気道、例えば、気管および気管支、ならびに胆道、例えば、胆管および胆嚢の一部へおよびそこからのアクセスのためにも同様に使用されることができる。
第1および第2の身体管腔の対向する第1および第2の管腔壁を通して、カテーテルを遠位に前進させるための方法が、本明細書に開示される。本方法は、第1の管腔壁に近位の遠位先端を伴う、カテーテルを位置付けることであって、カテーテルは、ステントを含む、ことと、電流を遠位先端の伝導性部分に提供することと、カテーテルの遠位先端を第1および第2の管腔壁を通して遠位に前進させ、それを通して通路を作成することであって、遠位先端の伝導性部分は、第1の切除特徴と、第1の切除特徴から遠位先端の外径に向かって延在する1つ以上の突起とを含む、ことと、ステントの遠位フランジを第2の身体管腔内で展開することと、遠位フランジを近位に抜去し、第1および第2の管腔壁を相互に向かって引張することとを含む。
改良されたカテーテル先端設計が、本明細書に開示される。本明細書に開示される設計は、先行技術設計に優るいくつかの利点をもたらす。本明細書に開示されるデバイスは、以前の設計より迅速かつ効率的に、周囲組織への漏出、外傷、および熱損傷が少なく、組織を通して切除することができる。従来の鈍的先端設計は、過剰加熱を生じさせ得る、より高い電力を要求し、また、標的面積に隣接する組織を損傷し得る。先行技術先端設計はまた、組織を切除するために、より長い時間量を要求し得る。暴露時間が長いほど、組織への熱損傷をさらに悪化させ得る。先行技術先端設計に関する問題の多くを克服する、改良されたカテーテル先端設計が望ましい。
Claims (50)
- 第1および第2の身体管腔の対向する第1および第2の管腔壁を通して、カテーテルを遠位に前進させるための方法であって、前記方法は、
前記第1の管腔壁に近位の遠位先端を伴うカテーテルを位置付けることであって、前記カテーテルは、ステントを含む、ことと、
電流を前記遠位先端の伝導性部分に提供することと、
前記カテーテルの遠位先端を前記第1および第2の管腔壁を通して遠位に前進させ、前記第1および第2の管腔壁を通して通路を作成することであって、前記遠位先端の伝導性部分は、第1の切除特徴と、前記第1の切除特徴から前記遠位先端の外径に向かって延在する1つ以上の突起とを含む、ことと、
前記ステントの遠位フランジを前記第2の身体管腔内で展開することと、
前記遠位フランジを近位に抜去し、前記第1および第2の管腔壁を相互に向かって引張することと
を含む、方法。 - 前記カテーテルを位置付けることに先立って、針を使用して、前記第1および第2の身体管腔内に孔を形成することをさらに含む、請求項1に記載の方法。
- 前記第1の切除特徴および1つ以上の突起は、弧状構成を備える、請求項1に記載の方法。
- ガイドワイヤは、前記カテーテルを位置付けるために使用されず、前記遠位先端は、ガイドワイヤ管腔を有していない、請求項1に記載の方法。
- 前記第1の切除特徴は、前記カテーテルの遠位先端内の管腔と同心である同心リングを備える、請求項1に記載の方法。
- 前記カテーテルは、ガイドワイヤを中心として配置され、位置付けることは、前記ガイドワイヤに沿って、前記カテーテルを前進させることを含み、前記同心リングは、前記ガイドワイヤを中心として配置されるように構成される、請求項5に記載の方法。
- 近位に抜去することに先立って、前記遠位フランジの展開を検証することをさらに含む、請求項1に記載の方法。
- 前記遠位フランジの展開の検証は、超音波を使用して行われる、請求項7に記載の方法。
- 前記ステントの近位フランジを前記第1の身体管腔内に解放することをさらに含む、請求項1に記載の方法。
- 前記カテーテルは、前記ステントを半径方向に拘束するシースを含み、前記ステントは、自己拡張式ステントであり、前記遠位フランジを展開することは、前記シースを後退させ、前記ステントの遠位フランジが拡張することを可能にすることを含む、請求項1に記載の方法。
- 前記カテーテルは、前記ステントを半径方向に拘束するシースを含み、前記ステントは、自己拡張式ステントであり、前記近位フランジを展開することは、前記シースを後退させ、前記ステントの近位フランジが拡張することを可能にすることを含む、請求項9に記載の方法。
- 前記カテーテルを位置付けることに先立って、前記カテーテルを内視鏡に取り付けることをさらに含む、請求項1に記載の方法。
- 前記遠位先端を前記第1および第2の管腔壁を通して前進させ、前記第1および第2の管腔壁を通して通路を作成することは、前記第1の管腔壁内に第1のパターン化された孔および前記第2の管腔壁内に第2のパターン化された孔を形成する、請求項1に記載の方法。
- 前記第1および第2のパターン化された孔は、前記遠位先端の最大直径未満の直径を有する、請求項13に記載の方法。
- 前記第1および第2のパターン化された孔はそれぞれ、中心孔を含み、1つ以上の突起が、前記中心孔から放射状に広がる、請求項13に記載の方法。
- 前記第1の身体管腔は、消化管内にある、請求項1に記載の方法。
- 前記第1の身体管腔は、食道、胃、十二指腸、空腸、小腸、大腸、結腸、および直腸から成る群から選択される、請求項16に記載の方法。
- 前記第2の身体管腔は、消化管または胆樹の一部である、請求項17に記載の方法。
- 前記第2の身体管腔は、食道、胃、十二指腸、空腸、小腸、大腸、結腸、胆管、膵管、胆嚢、および膵臓から成る群から選択される、請求項1に記載の方法。
- 前記第1および第2の身体管腔は、結腸の区画である、請求項1に記載の方法。
- 前記第1の身体管腔は、胃または十二指腸であり、前記第2の身体管腔は、前記胆嚢である、請求項1に記載の方法。
- 前記第1および第2の身体管腔は、胃穹窿部、および、十二指腸または空腸の区画である、請求項1に記載の方法。
- 前記第1および第2の身体管腔間に吻合を形成することをさらに含む、請求項9に記載の方法。
- 前記吻合を形成した後、前記ステントを除去することをさらに含む、請求項23に記載の方法。

を備える、カテーテルアセンブリ。 - 前記第1の切除特徴および1つ以上の突起は、弧状構成を備える、請求項25に記載の方法。
- 前記先端は、ガイドワイヤ管腔を有していない、請求項25に記載の方法。
- 前記第1の切除特徴は、同心リングを備える、請求項25に記載のカテーテルアセンブリ。
- 前記カテーテルは、ガイドワイヤ管腔をさらに備え、前記同心リングは、前記ガイドワイヤ管腔を中心として配置され、前記ガイドワイヤ管腔から絶縁されるように構成される、請求項28に記載のカテーテルアセンブリ。
- 前記1つ以上の外向きに延在する突起は、前記第1の切除特徴から最大半径方向位置まで延在し、前記最大半径方向位置は、半径方向位置と前記遠位先端の中心点との間の長さを画定し、前記長さは、前記先端の外径の50%未満である、請求項29に記載のカテーテルアセンブリ。
- 前記伝導性部分は、ステンレス鋼から作製された外部部分と、前記外部部分に電気的に連結され、電気外科手術用発電機に電気的に連結されるように構成される、銅を含む、内部部分とを含む、請求項25に記載のカテーテルアセンブリ。
- 前記第1の切除特徴から前記先端の外径に向かって外向きに延在する前記1つ以上の突起は、前記先端の最大直径に隣接する被覆された部分を含む、請求項25に記載のカテーテルアセンブリ。
- 電気外科手術用発電機は、約50ワット〜約100ワットの電力を前記先端の伝導性部分に供給するように構成される、請求項25に記載のカテーテルアセンブリ。
- 前記伝導性部分は、2つの突起を含む、請求項25に記載のカテーテルアセンブリ。
- 前記伝導性部分は、4つの突起を含む、請求項25に記載のカテーテルアセンブリ。
- ステントを保持するように構成された前記カテーテル本体内のコンパートメントをさらに備える、請求項25に記載のカテーテルアセンブリ。

- 前記カテーテルアセンブリは、前記遠位先端と係合され、前記カテーテル本体の中心通路内の前記カテーテル本体に対して移動するように構成される、内側カテーテルシャフトをさらに備え、前記内側カテーテルシャフトおよび遠位先端は、前記カテーテル本体から可撤性であるように構成される、請求項25に記載のカテーテル。
- カテーテルを第1の身体管腔の管腔壁を通して遠位に前進させるための方法であって、前記方法は、
前記第1の身体管腔の管腔壁に近位の遠位先端を伴う、外側管状本体および内側本体を備えるカテーテルを位置付けることと、
電流を前記遠位先端の伝導性部分に提供することと、
前記外側管状本体の一部とともに、前記カテーテルの遠位先端を前記第1の管腔壁を通して遠位に前進させ、前記第1の管腔壁を通した通路を作成することであって、前記遠位先端の伝導性部分は、第1の切除特徴と、前記第1の切除特徴から前記遠位先端の外径に向かって延在する1つ以上の突起とを含む、ことと、
前記内側本体および遠位先端を前記外側管状本体を通して抜去し、前記外側管状本体の一部を前記第1の身体管腔の管腔壁内の前記通路内に残すことと
を含む、方法。 - 前記第1の管腔壁は、経胃壁または経十二指腸壁である、請求項43に記載の方法。
- 遠位に前進させる間に、偽性嚢胞内に孔を形成することをさらに含む、請求項44に記載の方法。
- カテーテル、ステント、またはドレナージデバイスを前記外側管状本体を通して、前記偽性嚢胞内の孔内に通過させることをさらに含む、請求項45に記載の方法。
- 位置付けることは、ガイドワイヤを前記第1の身体管腔の管腔壁の標的場所に追従させることを含む、請求項43に記載の方法。
